Small brown circles: An important diagnostic clue for pigmented squamous cell carcinoma

A 70-year-old woman presented for a periodic skin examination. A pink-brown plaque was identified on the left thigh; the clinical differential diagnoses included solar lentigo and lichen planus–like keratosis because of the sharp borders, homogeneous brown color, and absence of scales (Fig 1).

Pigmented Invasive Squamous Cell Carcinoma of the Conjunctiva in a Young Nigerian Female – A case report

Squamous cell carcinoma of the conjunctiva (SCCC) rarely presents as a pigmented lesion. This report is on a 32-year old healthy Nigerian female who presented on account of a 6-month history of left ocular irritation with associated increase in the size of a supposed ‘birth mark’ which had been present in the left eye for 6 years. Six years prior to this she had noticed a dark patch in the left eye which had shown no changes until 6 months to the time of presentation when it started to increase in size with associated telangectasia. Examination did not reveal obvious leukoplakia. Histology showed invasive squamous cell carcinoma of the conjunctiva. There have not been many reported cases of pigmented squamous cell carcinoma of the conjunctiva among Nigerians. Such pigmentation might have resulted from secondary acquired melanosis. Of interest in this case is the presence of a pre-existing dark patch at the site of the lesion which could have been due to racial melanosis or conjunctiva nevus as there was no prior histological evaluation. A histological examination of some of the supposedly innocuous pigmented conjunctiva lesions was recommended. Key words : squamous cell carcinoma, conjunctiva, pigmented, Nigerian

Pigmented Squamous Cell Carcinoma of Oral Mucosa: Clinicopathologic Study of 3 Cases

Squamous cell carcinoma (SCC) of the skin and mucosa occasionally show pleomorphic histologic forms. One of these rare pleomorphic forms is pigmented SCC (PSCC). Although melanin pigments are widely distributed in the skin and in certain types of mucosa, PSCC is rarely seen.1-22 Only 12 cases of PSCC in the oral mucosa have been reported.1,2,15,17,19,20,22 Clinically, the differential diagnosis of PSCC with dark pigmentation often includes melanoma and other melanocyte lesions. However, PSCC is not always accompanied by dark pigmented lesions on the overlying epithelium,19,20 although many melanin pigments and melanocytes can be found intermingled with the tumor cells.

Diagnostic accuracy of dermatoscopy for melanocytic and nonmelanocytic pigmented lesions

It is unknown whether dermatoscopy improves the diagnostic accuracy for all types of pigmented skin lesions or only for those that are melanocytic. We sought to assess if the addition of dermatoscopy to clinical examination with the unaided eye improves diagnostic accuracy for all types of pigmented lesions. We analyzed 463 consecutively excised pigmented skin lesions collected during a period of 30 months in a primary care skin cancer practice in Queensland, Australia. Of 463 lesions, 217 (46.9%) were nonmelanocytic. Overall 30% (n = 138) were malignant including 29 melanomas, 72 basal cell carcinomas, and 37 squamous cell carcinomas. The diagnostic accuracy for malignant neoplasms measured as area under receiver operating characteristic curves was 0.89 with dermatoscopy and 0.83 without it (P < .001). Given a fixed specificity of 80%, the corresponding sensitivity was 82.6% with dermatoscopy and 70.5% without it. The improvement achieved by dermatoscopy was higher for nonmelanocytic lesions than for melanocytic lesions. A short algorithm based on pattern analysis reached a sensitivity of 98.6% for basal cell carcinoma, 86.5% for pigmented squamous cell carcinoma, and 79.3% for melanoma. Among benign conditions, the highest false-positive rate (90.5%) was observed for lichen planus-like keratosis. Estimates of diagnostic accuracy are influenced by verification bias. Dermatoscopy improves the diagnostic accuracy for nonmelanocytic lesions. A simple algorithm based on pattern analysis is suitable for the detection of melanoma and nonmelanoma skin cancer.

The diagnosis of squamous cell carcinoma (SCC) is, generally, a clinical diagnosis, but in some cases, when the lesion is pigmented, as in our case, the differential diagnosis between pigmented SCC and other pigmented skin lesions, in particular melanocytic lesions, is difficult. Dermoscopy may improve the early diagnosis of SCC and thus play a role in its preoperative classification. However, its potential role has been hampered so far by the fact that little is known about the dermoscopic features of pigmented SCC. We report the case of a rare pigmented SCC dermoscopically mimicking a melanocytic lesion whose dermoscopic features have been investigated. On the basis of the literature and our experience, pigmented SCC can present dermoscopic features typical of melanocytic lesions, such as radial streaks, globules, and homogeneous blue pigmentation, and can lead dermatologists to diagnostic errors.

Pigmented squamous cell carcinoma is a rare variant of squamous cell carcinoma. Only 13 cases arising in the skin have been reported in the English literature, five cases in Latin America and none in Brazil. The authors report a case of pigmented squamous cell carcinoma, with emphasis on clinical, pathological and immunohistochemical aspects.

Ulcerative pigmented squamous cell carcinoma in a 101‐year‐old Japanese woman

Pigmented squamous cell carcinoma is a rare variant of squamous cell carcinoma. We report a case of pigmented squamous cell carcinoma with dermoscopic examination probably arisen from actinic keratosis in a 101-year-old woman who was surgically treated under general anesthesia. In addition, we discuss indications of general anesthesia in elderly patients with skin cancer, differential diagnosis and dermoscopic features of pigmented squamous cell carcinoma.

Pigmented Oral Squamous Cell Carcinoma: A Case Report and Brief Review of the Literature

Melanin impregnation in squamous cell carcinoma (SCC) is an uncommon histological finding. Rare nonmelanocytic entities were previously described as having melanocyte colonization. A 57-year-old Brazilian woman was referred with a pigmented lesion in the lower lip and alveolar ridge with a prior clinical diagnosis of melanoma. The incisional biopsy of the tumor revealed an SCC with strong colonization of melanocytes in the stroma. The authors report a case of an unusual SCC variant and a brief review of the literature.

Although dendritic, melanin-containing melanocytes can be seen in a variety of epithelial neoplasms, only 0.01% to 7% of all squamous cell carcinomas (SCC) are pigmented. Furthermore, most reported cases have occurred in the oral and ocular mucosa, with relatively few cases reported to originate in the skin. Herein we report a case of a 61-year-old Caucasian male who presented with a large blue-black nodule on his left cheek, clinically suspicious for a melanoma; however, histological evaluation revealed an acantholytic pigmented squamous cell carcinoma. Previous cases are reviewed and the clinical and histological differential diagnoses are discussed.

Possible involvement of stem cell factor and endothelin‐1 in the emergence of pigmented squamous cell carcinoma in oral mucosa

We present here the clinical, morphological and immunohistochemical features of a pigmented squamous cell carcinoma (SCC) in the oral mucosa of the hard palate of a 76-year-old Japanese man. He underwent a partial resection of the maxilla subsequent to radiotherapy. The tumor was typical, moderately well-differentiated SCC but had many melanocytes (melanocytosis) within it. Immunohistochemical analysis for stem cell factor (SCF) and endothelin-1, both of which are known to stimulate proliferation and differentiation of melanocytes, revealed prominent expression of both factors in the neoplastic squamous cells of the pigmented SCC, while the non-pigmented oral SCC showed little sign of either factor. These findings strongly suggest that SCF and endothelin-1 secreted by neoplasmic squamous cells are involved in the emergence of a rare variant of oral SCC.

Spreading pigmented actinic keratoses.

The clinical and histological features of ten pigmented keratoses arising on the face are described. They were all greater than 1.5 cm in diameter and had been spreading centrifugally. They were variably pigmented and possessed either a smooth, verrucous or slightly scaly surface. Two lesions had progressed to become pigmented squamous cell carcinomas.

Abstract A heavily pigmented tumor was removed from the lateral, perilimbal, bulbar conjunctiva of a bay Thoroughbred horse. Excision was incomplete and the base of the tumor was lasered on the assumption that the tumor was a melanoma. No other ophthalmic lesions were seen. Histology showed the mass to be a pigmented squamous cell carcinoma (SCC). The prognosis and recommended treatment protocols are different for SCC and melanoma. This is, to the best of our knowledge, the first report of an ocular, pigmented SCC in the veterinary literature.

Cutaneous pigmented invasive squamous cell carcinoma: a case report with dermatoscopy and histology

Cutaneous invasive pigmented squamous cell carcinoma (SCC) is rare. There have previously been only a few case reports or small case series in the English literature [1–13]. In the largest published study there were five cases of pigmented invasive SCC in 46,791 archived cases of invasive SCC (approximately 0.01%) [2]. This compares to pigmented SCC in-situ (pigmented Bowen’s disease) in which the largest study found 52 of 951 (5.5%) consecutive cases of SCC in-situ to be pigmented [14]. The differential diagnosis of pigmented invasive SCC includes benign and malignant melanocytic neoplasms (melanoma and nevus) as well as pigmented types of benign and malignant epithelial neoplasms such as basal cell carcinoma (BCC), actinic keratosis (AK), SCC in-situ, seborrheic keratosis and melanoacanthoma [1, 12]. Our search has revealed five cases of pigmented invasive SCC with a description of dermatoscopy features [1, 9, 10, 12, 13] and in none of those cases was the diagnosis predicted.
