Risk of venous thromboembolism in relation to high physical activity level in men over 27 year follow up

Venous thromboembolism (VTE) is the third most common type of cardiovascular disease. An association between high level of physical activity (PA) and the onset of VTE has been found in some, but not all previous studies. We aim to study the association between PA-level and VTE in a cohort of men with updated data on PA levels at four occasions. We used data from the Uppsala Longitudinal Study of Adult Men (ULSAM) study initiated in 1970, a study of men at age 50 years (n = 2,294 at baseline) examined on leisure time PA by questionnaire and traditional cardiovascular risk factors. Examinations were repeated at ages 60, 70, and 77, and follow-up was completed after a median time of 33 years. Cox regression analysis with hazard ratios (HRs) using updated covariates for PA and risk factors was performed on the association of PA levels with incident VTE, with adjustments for established cardiovascular risk factors (systolic blood pressure, LDL- and HDL-cholesterol, BMI, diabetes, and smoking). Totally 186 men experienced a VTE during follow-up of 68,263 person-years at risk. Individuals with the highest PA level had an increased relative risk of VTE, adjusted HR, 2.22 (95% CI 1.05–4.67), when compared to individuals with the lowest level of PA. In this cohort of men with a follow-up of 27 years, the risk of VTE was increased at the highest PA level. Is slowness a better discriminator of disability than frailty in older adults?

BackgroundThe trajectory of incident disability that occurs simultaneously with changes in frailty status, as well as how much each frailty component contributes to this process in the different sexes, are unknown. The objective of this study is to analyse the trajectory of the incidence of disability on basic and instrumental activities of daily living (BADL and IADL) as a function of the frailty changes and their components by sex over time.MethodsLongitudinal analyses of 1522 and 1548 of the English Longitudinal Study of Ageing study participants without BADL and IADL disability, respectively, and without frailty at baseline. BADL and IADL were assessed using the Katz and Lawton Scales and frailty by phenotype at 4, 8, and 12 years of follow‐up. Generalized mixed linear models were calculated for the incidence of BADL and IADL disability, as an outcome, using changes in the state of frailty and its components, as the exposure, by sex in models fully adjusted for sociodemographic, behavioural, biochemical, and clinical characteristics.ResultsThe mean age, at baseline, of the 1522 eligible individuals free of BADL and free of frailty was 68.1 ± 6.2 years (52.1% women) and of the 1548 individuals free IADL and free frailty was 68.1 ± 6.1 years (50.6% women). Women who became pre‐frail had a higher risk of incidence of disability for BADL and IADL when compared with those who remained non‐frail (P < 0.05). Men and women who became frail had a higher risk of incidence of disability regarding BADL and IADL when compared with those who remained non‐frail (P < 0.05). Slowness was the only component capable of discriminating the incidence of disability regarding BADL and IADL when compared with those who remained without slowness (P < 0.05). Weakness and low physical activity level in men and exhaustion in women also discriminated the incidence of disability (P < 0.05).ConclusionsSlowness is the main warning sign of functional decline in older adults. As its evaluation is easy, fast, and accessible, screening for this frailty component should be prioritized in different clinical contexts so that rehabilitation strategies can be developed to avoid the onset of disability.

Brief Theory-based Intervention to Improve Physical Activity in Men with Psychosis and Obesity: A Feasibility Study

Introduction: Physical activity (PA) is recommended to improve physical health in people with psychosis, however, most of them are inactive. Objectives: The objectives of the study were to assess the feasibility of a four-week brief theory-based intervention to improve PA in men with psychosis and obesity. Methods: A Quasi- experimental pre- post design evaluating the feasibility, the participation, and the intervention drop-out rate was conducted. PA, sedentary time, motivational processes of change, and anthropometric measures were evaluated. Results: A total of 38 men with psychotic disorders were approached and 32% participated (n = 12). Among these 12 participants, 75% had schizophrenia, their mean age was 33.2 ± 10.2, and mean Body Mass Index: 35.8 ± 7.7 kg/m2. Besides, all 12 participants (100%) completed the intervention. Participants reported a high satisfaction rate in the intervention. Following the intervention, PA level was increased with a moderate effect size. Similarly, an increase in behavioral processes of change was observed with a moderate effect size. Discussion and conclusion: Hence, the brief theory-based intervention was feasible and could probably improve the PA level in men with psychosis and obesity.

Physical activity levels in men with Haemophilia—A single centre UK survey

Historically persons with haemophilia (PWH) were not encouraged to participate in exercise due to the risk of bleeding and the lack of factor products available. This has now changed, and the availability of safe products allows PWH to be active and participate in sports. Studies have found that exercise has a positive effect on pain, joint health and movement with PWH. To record the amount and types of physical activity undertaken by a haemophilia population at a single treatment centre. An observational cross-sectional study to assess physical activity and quality of life of all registered patients over the age of 18 at the Oxford Haemophilia and Thrombosis Centre. Participants were posted questionnaires including the International Physical activity Questionnaire (IPAQ), Haemophilia Activity List (HAL), EQ-5D-5L and asked to list their physical activities. A total of 256 questionnaires were sent, with a 40% response rate for severe and 28% for mild patients. 85% met the UK physical activity guidelines. Sedentary behaviour for an average weekday was 6.4hours, and weekends were 5.3hours. Joint disease and severity type influenced the amount of activity undertaken, together with values for HAL and EQ-5D-5L. Twenty two types of activities were listed. It is encouraging to see the amount of physical activity PWH participate in, however, time spent in a sedentary state needs monitoring. PWH want to be active and the challenge for caregivers is to find activities they can do and strategies to maintain participation.

한국 중년의 신체활동 수준에 따른 남성과 여성의 신체활동습관과 영양섭취상태 비교

이 연구는 국민건강영양조사 제 6기(2013-2015)자료를 토대로 중년기 성인을 추출하여 신체활동 수준에 따른 신체활동 습관 및 영양섭취상태를 분석하고자 실시하였다. 이 연구의 목적을 위해 만 40-64세의 중년을 추출한 결과 중년남성은 1,235명, 중년여성은 1,591명으로써 총 2,826명을 대상으로 국제신체활동설문지를 활용하여 비활동 신체활동군, 중등도 신체활동군, 건강증진형 신체활동군으로 구분하였다. 구분된 성별과 신체활동 수준을 토대로 신체활동 및 영양섭취상태를 비교분석한 결과 나이는 모든 신체활동 수준에서 중년남성이 중년여성보다 높았고 신체활동 수준은 중년남성이 월등히 높았으나 건강증진형 신체활동을 실시하는 비율은 중년여성이 중년남성에 비해 유의하게 높았다. 또한 좌식시간은 중년남성이 중년여성에 비해 현저히 높은 것으로 나타났고 이에 따라 체질량지수와 허리둘레가 중년여성에 비해 높은 것을 알 수 있었다. 또한 영양섭취상태 중 총에너지, 수분 및 단백질섭취량은 중년남성이 중년여성에 비해 높은 것을 알 수 있었으나 지방섭취량은 비활동 신체활동군에서 중년남성과 중년여성의 차이가 나타나지 않았다. 이러한 결과를 종합해 볼 때 중년남성은 좌식시간을 줄이고 건강증진형 신체활동을 늘리는 행동습관의 개선이 필요하겠으나 중년여성의 경우 지방섭취량을 줄이는 등 식생활습관의 개선이 필요하다.

Cumplimiento de las recomendaciones de práctica de actividad física en función de los cutoffs points y el género en estudiantes universitarios españoles

&lt;p&gt;Los principales objetivos de este estudio fueron: a) estudiar los niveles de actividad física (AF) según los diferentes puntos de corte utilizados y el género; b) analizar el cumplimiento de las diferentes recomendaciones internacionales de AF según el género y puntos de corte. 95 universitarios, 33 hombres y 62 mujeres (21, 96 años &lt;span style="text-decoration: underline;"&gt;+&lt;/span&gt; 2,33) llevaron acelerómetro para medir sus niveles de AF utilizando puntos de corte uniaxiales y triaxiales. Los resultados señalan mayores niveles de AF en los hombres para la AF vigorosa, pero no en la moderada. La mayoría de la muestra cumple con las recomendaciones de AF moderada, sin embargo, en la AF vigorosa el cumplimiento es bajo. El análisis de la AF con puntos de corte triaxiales son mayores a los uniaxiales, así como un mayor porcentaje de cumplimiento cuando se utilizan las recomendaciones establecidas por la Physical Activity Guidelines Advisory y la World Health Organization.&lt;/p&gt;

Effects of an Aquatic-Based Exercise Program to Improve Cardiometabolic Profile, Quality of Life, and Physical Activity Levels in Men With Type 2 Diabetes Mellitus

BackgroundThe role of structured exercise in improving cardiometabolic profile and quality of life in patients with type 2 diabetes mellitus (2DM) has been widely demonstrated. Little is known about the effects of an aquatic-based exercise program in patients with 2DM. ObjectiveTo evaluate the effects of a supervised aquatic-based exercise program on cardiometabolic profile, quality of life, and physical activity levels in patients with 2DM. Design and SettingObservational study, community pre-post aquatic-based exercise program, primary care intervention. PatientsEighteen men diagnosed with 2DM (52.2 ± 9.3 years). Methodsand Main Outcome Measurements: Cardiometabolic profile, quality of life, and physical activity levels were assessed before and after 12 weeks of an aquatic-based exercise program. ResultsThe results show a significant improvement of cardiometabolic assessments (maximum oxygen consumption: 24.1 versus 21.1 mL/kg/min, P < .05; blood pressure: 125.4/77 versus 130.7/82.5 mm Hg, P < .05; fasting blood glucose: 119.6 versus 132.5 mg/dL, P < .05; body mass index: 29.9 versus 31.1 kg/m², P < .005; low-density lipoprotein cholesterol: 95.2 versus 104.9 mg/dL, P < .05; and diastolic function: E/E′ 9.1 versus 10.1, P < .005) and an increase in quality of life and physical activity levels (Medical Outcome Study 36-item Short Form Health Survey mental component summary: 72.3 versus 67, P < .05; Problems Area About Diabetes: 20.1 versus 33.2, P < .005) and energy expenditure in general physical activity (physical activity: 3888.7 versus 1239.5 kcal/wk, P < .05). ConclusionsThese findings demonstrate that an aquatic-based exercise program produces benefits for the cardiovascular system and metabolic profile and appears to be safe and effective in improving quality of life and increasing physical activity levels in patients with 2DM.

Education and the prevention of postural defects

PurposeThe aim of this study was to determine: whether and at what stage of education is proper body posture learned, the intention of young adults to participate in activities teaching proper posture, and the effects of factors related with the said intention.MethodsThe study involved 430 university students aged 18-24 years. Anthropometric data was collected. Participants completed questionnaires assessing physical activity level (IPAQ) and their intention to participate in extracurricular activities teaching proper posture while sitting or walking, proper running technique, corrective gymnastics, or weight loss exercises. A self-assessment of posture, physical fitness, attractiveness, and body satisfaction was also completed.ResultsLower back pain was experienced by 41% of the respondents. Most were taught proper posture-related habits in primary school, followed by secondary school, and then at university. Many students expressed their intention to participate in the extracurricular activities. None of the questionnaire variables were associated with the intention to learn proper walking posture or proper running technique. The intention to participate in classes teaching proper sitting posture was associated with lower back pain in women and low physical activity level in men. In women, a relationship was found between the intention to participate in weight loss exercises and body dissatisfaction, high BMI, and poor self-evaluations of posture and attractiveness. In men, this activity was associated with body dissatisfaction.ConclusionsThere is a need for further education on the development of proper postural habits at the university level.

A pilot survey of physical activity in men with an intellectual disability

People with intellectual disability (ID) are reported as a sedentary population with increased risks of poor health due to an inactive and sedentary lifestyle. As the benefits of physical activity are acknowledged, measuring physical activity accurately is important to help identify reasons for low and high physical activity in order to assist and maintain recommended levels for optimal health. This article reports a pilot study undertaken to validate the use of a physical activity monitor (Sensewear Armband) and the International Physical Activity Questionnaire (IPAQ) as instruments for measuring and exploring physical activity of men with ID. The design was a one-group descriptive study and the data were collected over a 7-day period from 17 men. The Sensewear Armband enabled continuous and long-term measurement of 14 objective physical activity metrics. The IPAQ examined details of physical activity reported over 7days. Equivalent results were obtained from both the instruments, indicating a positive correlation between the Sensewear Armband and the IPAQ. The results show 50% have low activity levels, and the national recommended physical activity levels have been achieved at a very low active intensity. No sustainable high physical activity intensity levels were recorded. The results confirmed the Sensewear Armband and the IPAQ as a practical means of measuring and understanding physical activity levels in men with ID.

Leisure time physical activity and risk of non-vertebral fracture in men and women aged 55 years and older: the Tromsø Study

Our aim was to examine associations between leisure time physical activity and risk of non-vertebral fractures in men and women aged ≥55 years, with focus on the anatomical fracture location. Self-reported physical activity was registered in 3,450 men and 4,072 women aged 55-97 years at baseline in the Tromsø Study, Norway, in 1994-1995. Non-vertebral fractures were registered through December 31, 2009. During 75,546 person-years at risk, 1,693 non-vertebral fractures were identified. Risk of any non-vertebral fracture decreased with increasing physical activity level in men (P (trend) = 0.006) and non-significantly in women (P (trend) = 0.15), after adjustment for age, body mass index, height, smoking, and previous fracture. The reduced fracture risk was due to a reduced risk in the weight-bearing skeleton, particular at the hip, whereas risk of fracture in the non-weight-bearing skeleton was not related to physical activity. At weight-bearing sites, an inverse relationship between physical activity and fracture risk was present in both sexes (P (trend) ≤ 0.013). Compared with sedentary subjects, the most active men and women had respectively 37% (HR = 0.63, 95% CI: 0.45, 0.88) and 23% (HR = 0.77, 95% CI: 0.62, 0.95) reduced fracture risk in the weight-bearing skeleton. Physical activity is associated with reduced fracture risk at weight-bearing sites, with no associations at non-weight-bearing sites, in both sexes. Habitual physical activity is an important amendable approach to prevent hip fracture.

Correlates of physical activity in Morocco

PurposeTo estimate the prevalence of physical activity (PA) and associated variables in the Moroccan adult population. MethodsPopulation-based, May 2008 survey of a representative sample of Moroccan adults. PA was assessed using the International Physical Activity Questionnaire (IPAQ). ResultsMean age was 41.4years (26.2-56.6). Of the 2613 subjects, 48.1% were women and 58% lived in urban areas. The prevalence of the lowest physically active category was 16.5% overall, 24% in women and 9% in men (p<0.001). Unemployed (18.6%) and retired individuals (17.9%), housewives (28.2%) and married persons (19.7%) reported lower levels of physical activity. Older age, unemployment or retirement, having high income, and being overweight or obese were the main determinants of low PA levels in men. In women, the main determinants of low PA levels were living in an urban area and being a housewife. ConclusionThis survey has identified that urbanization and having high income are main determinants of low physical activity in Moroccan adults in a country undergoing economic transition.

Reductions in incident coronary heart disease risk above guideline physical activity levels in men

Background One-half of Americans currently meet guideline physical activity levels. For these individuals, exceeding guideline levels may provide additional health benefits. Methods Incident physician-diagnosed myocardial infarction and angina, revascularization procedures (CABG, PTCA), and ischemic heart disease deaths during 7.7-year follow-up were compared to baseline usual distance run in 35,402 male runners. Results Men reported 467 incident CHD and the National Death Index identified an additional 54 ischemic heart disease deaths. Per km/day run, the men's risks declined 5% for fatal and nonfatal CHD ( P = 0.001), nonfatal CHD ( P = 0.0008), and revascularization procedures ( P = 0.002). Their risks for nonfatal myocardial infarctions and angina declined 7% ( P = 0.02) and 10% ( P = 0.003), respectively. Compared to <3 km/day run (upper limit guideline level), >9 km/day run produced risks 65% lower for angina ( P = 0.008), 29% lower for nonfatal CHD ( P = 0.04), and 26% lower for fatal and nonfatal CHD ( P = 0.06). Conclusions Exceeding guideline physical activity levels produce important CHD-risk reductions.

Total energy expenditure, resting metabolic rate and physical activity level in free-living rural elderly men and women from Cuba, Chile and México

The objective of this study was to assess total energy expenditure (TEE), resting metabolic rate (RMR) and physical activity level (PAL), and to estimate energy requirements (ERs) in free-living elderly subjects from Cuba, Chile and Mexico. Cross-sectional study designed to estimate ERs. Rural regions of Cuba, Chile and Mexico. Forty subjects >60 years old were selected to participate in this study. A dose of doubly labeled water (DLW) was administered and urine samples were collected in the following 12-14 days. From the isotopic analysis, TEE was derived. RMR was measured by indirect calorimetry. TEE in Chilean (8.8+/-1.6 MJ/day) and Cuban (8.3+/-1.3 MJ/day) elderly was not different, and was higher for the Mexican group (9.5+/-1.5 MJ/day) (P < 0.0001). RMR was not different between countries. PAL and activity energy expenditure (AEE) were only different between Chile and Mexico (P < 0.002). For the three regions, overall PALs were 1.70+/-0.16 for male and 1.62+/-0.13 for female subjects (P < 0.02), and AEE was 3.05+/-0.66 and 2.27+/-0.66 MJ/day for male and female subjects, respectively (P < 0.001). Predictive equations (MJ/day) were RMR = 1.6447 + 0.05714 x weight (kg) + 0.449 sex (male = 1; female = 0) (R2 = 0.75; SEE = 0.479) and TEE = 3.414 + 0.0795 x weight (kg) + 1.227 x sex (male = 1; female = 0) (R2 = 0.75; SEE = 0.668). There were differences in TEE and PAL owing to sex and region. The average PAL in men was higher than the PAL reported either with factorial approach or with the DLW method in elderly. Predictive ER equations based on RMR and TEE gave very similar results to calculations from the 2004 FAO/WHO/UNU report.

Physical Activity and its Relationship to Physical Performance in Patients With End Stage Knee Osteoarthritis

Cross-sectional observational design. To compare physical activity levels in men and women with end-stage knee osteoarthritis to those of a comparison group and to examine the relationship between physical activity level and physical performance. Osteoarthritis of the knee is associated with significant losses in functional performance and high social costs. Although reductions in physical activity are reported, they have not been quantified or explored. Fifty-nine candidates awaiting total knee arthroplasty (TKAC group) and 79 individuals without osteoarthritis (comparison group) participated. Physical activity was assessed using the Voorrips Questionnaire. Performance measures included fast self-paced walk test, timed up-and-go test, and a timed stair performance measure. A subset of subjects completed the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) and underwent muscular strength and endurance testing. The effects of gender and group were tested using GLM ANOVA. Pearson product moment correlations were used to examine relationships between the variables. All aspects of physical activity were lower (P<.001) in the TKAC group, with a moderate difference in household score (18%) and a large difference in leisure activities (63%). Unlike the comparison group, modest but significant correlations (r = 0.31-0.33, P<.03) were observed between overall physical activity and performance test scores for the TKAC group. Physical activity was not significantly related to pain reported on the WOMAC or during the performance tasks. The belief that pain limits the physical activity of patients with severe osteoarthritis requires further investigation. The profound differences between a comparison group and patients with end-stage osteoarthritis in physical activity have critical implications for the well-being and effective treatment of this population.

Association Between Physical Activity and Forearm Bone Mineral Density in 20-72-year-olds

Physical activity may influence bone mineral density (BMD) in different ways. In the present study the amount of physical activity and the associations between forearm BMD, body mass index, physical activity and several other lifestyle factors were explored. A random sample of the population in a Swedish municipality was invited to the study and examined by a questionnaire and by forearm bone mineral measurements using single photon absorptiometry. Forearm BMD was measured in 880 subjects. Forearm BMD was stable from 20 to 50 years of age in women and from 20 to 60 years of age in men. Reported moderate leisure-time physical activity levels in men were significantly associated with higher forearm BMD than low leisure-time physical activity levels ( p = 0.042). The findings that moderate levels of leisure-time physical activity in men were associated with higher forearm BMD, give some support to the hypothesis that increased amount of physical activity in a population might be of benefit in the prevention of osteoporosis. Further longitudinal studies of the effect of physical activity in the prevention of osteoporosis, falls and fractures are warranted.

Lumbar lordosis and pelvic inclination of asymptomatic adults.

We examined the association between pelvic inclination and lumbar lordosis during relaxed standing and eight variables thought to contribute to lordosis. Ninety subjects (45 men, 45 women) without back pain or a history of surgery were examined. The mean age was 54.8 years (SD = 8.5) for male subjects and 58.9 years (SD = 8.8) for female subjects. Multiple linear regression modeling was used to assess the association of pelvic inclination and size of lumbar lordosis in a standing position with age, gender, body mass index, physical activity level, back and one-joint hip flexor muscle length, and performance and length of abdominal muscles. Abdominal muscle performance was associated with angle of pelvic inclination for women (R2 = .23), but not for men. Standing lumbar lordosis was associated with abdominal muscle length in women (R2 = .40), but it was multivariately associated with length of abdominal and one-joint hip flexor muscles and physical activity level in men (R2 = .38). No correlation was found between angle of pelvic inclination and depth of lumbar lordosis in a standing position. Neither univariate nor multivariate regression models account for variability in the angle of pelvic inclination or size of lumbar lordosis in adults during upright stance; no correlation was found in standing between these two variables. The use of abdominal muscle strengthening exercises or stretching exercises of the back and one-joint hip flexor muscles to correct faulty standing posture should be questioned.
