Calcaneal Ultrasound and Its Relation to Dietary and Lifestyle Factors, Anthropometry, and Vitamin D Deficiency in Young Medical Students.

BackgroundBone quality and peak bone mass are greatly affected by lifestyle factors. The objective of the study was to investigate the relationships between anthropometry, dietary and caloric intake, body composition measurements, physical activity, and vitamin D status with quantitative ultrasound-based bone parameters among medical students.MethodsBoth male and female medical college students were included in this study. A detailed questionnaire was administered, collecting clinical, dietary, physical activity information, physical examination details, including body mass index (BMI). Body composition (total body fat, total body water, muscle mass, mean visceral fat mass, basal metabolic rate, bone mass using a bioelectrical impedance analyzer) and calcaneal heel ultrasound parameters were measured using an Osteosys Sonost-3000, Ultrasound Bone Densitometer were measured, respectively.ResultsIn this cross-sectional study, 211 healthy students with a mean age of 20.1 ± 1.1 years, 51.7% (n = 109) were males. Majority (79.4%) of the young adults had vitamin D deficiency. The mean BMI, calcium intake, and vitamin D levels were 22.35 ± 3.43 kg/m2, 788.7 ± 364.8 mg/day, and 15.02 ± 8.63 ng/ml, respectively. Female subjects compared to males had statistically significantly lower daily energy intake, muscle mass, visceral fat mass, calcium intake, and vitamin D levels. In addition the median Z-scores in females [−1.40 (−0.57 to −1.82)] was significantly poorer than the male [−0.50 (0.20 to −1.3)] counterparts, p-value <0.001. Multiple regression analysis showed that overall body fat percent (p-value 0.016) and visceral fat percent (p-value 0.029) were the only significant negative predictors to the calcaneal bone quality index (BQI) values.ConclusionAdolescent lifestyle patterns can influence young adult bone strength. The young Pakistani females exhibited significantly lower dietary intakes and more inadequate bone parameters compared to males. Our data suggest that total body and visceral fat percent are the predominant negatively associated determinant of bone strength for this cohort. Calcaneal ultrasound can be utilized for mass screening of young adults for identification of low BMD.

Physical Activity and Psychological Stress Detection and Assessment of Their Effects on Glucose Concentration Predictions in Diabetes Management.

Continuous glucose monitoring (CGM) enables prediction of the future glucose concentration (GC) trajectory for making informed diabetes management decisions. The glucose concentration values are affected by various physiological and metabolic variations, such as physical activity (PA) and acute psychological stress (APS), in addition to meals and insulin. In this work, we extend our adaptive glucose modeling framework to incorporate the effects of PA and APS on the GC predictions. A wristband conducive of use by free-living ambulatory people is used. The measured physiological variables are analyzed to generate new quantifiable input features for PA and APS. Machine learning techniques estimate the type and intensity of the PA and APS when they occur individually and concurrently. Variables quantifying the characteristics of both PA and APS are integrated as exogenous inputs in an adaptive system identification technique for enhancing the accuracy of GC predictions. Data from clinical experiments illustrate the improvement in GC prediction accuracy. The average mean absolute error (MAE) of one-hour-ahead GC predictions with testing data decreases from 35.1 to 31.9mg/dL (p-value = 0.01) with the inclusion of PA information, and it decreases from 16.9 to 14.2mg/dL (p-value = 0.006) with the inclusion of PA and APS information. The first-ever glucose prediction model is developed that incorporates measures of physical activity and acute psychological stress to improve GC prediction accuracy. Modeling the effects of physical activity and acute psychological stress on glucose concentration values will improve diabetes management and enable informed meal, activity and insulin dosing decisions.

The effect of educational program on self-care behaviors of candidates for coronary artery angiography

Context: Cardiovascular disease is one of the leading causes of mortality and disability in the world and its prevalence is increasing.Aim: The aim of the study was to determine the effect of educational program on self-care behaviors of candidates for coronary artery angiography.Setting and Design: This study was a quasi-experimental interventional study conducted at Imam Hossein Hospital in Mehran/Ilam in 2018.Materials and Methods: One hundred and twenty-six candidates for coronary artery angiography were randomly divided into two groups of intervention and control and completed the Cardiac Patients Learning Needs Inventory. The intervention group was trained with cardiovascular care educational program after angiography. After 2 weeks, the questionnaire was completed again by both groups.Statistical Analysis Used: Data were analyzed using Kolmogorov–Smirnov test, t-test, and paired t-test using SPSS software version 18.Results: The educational needs of the subjects were measured in eight areas of self-care (introduction to the cardiac intensive care unit, anatomy and physiology, psychological factors, risk factors, pharmaceutical information, nutritional information, physical activity, and other information). Most of the samples had moderate-to-high educational need (intervention group = 3.46) and (control group = 3.86), and the educational need in intervention group reached 1.62, which was significant (P < 0.001), but there was no significant change (P = 0.871) in the control group (3.31).Conclusion: Promoting self-care behaviors in patients undergoing coronary angiography using educational interventions is important and necessary. The curriculum that is designed and used in this study is recommended as a sample.

Self-compassion training within a workplace physical activity program: A pilot study.

Desire2Move (D2M) is an 8-week team-based, workplace program that incorporates competition and self-monitoring to encourage physical activity (PA). The purpose of this pilot study was to evaluate the effectiveness of a self-compassion (SC) intervention within the existing workplace PA program. University employees (n = 20) were assigned to a SC treatment group or an attention control group. Participants logged their PA minutes using MapMyRun. Treatment group. participants received a 7-week, SC podcast intervention. Participants self-reported PA, SC, and demographic information at pre- and post-intervention. Separate ANOVAs were used to determine group differences. There was not a statistically significant group x time interaction for SC, F(1, 18) = 0.02, p = 0.90, or main effects for time, F(1, 18) = 0.28, p = 0.61, or group, F(1, 18) = 1.70, p = 0.21, g = -0.42 [95%CI = -3.13, 2.29]. There were not significant differences between groups for total PA minutes during D2M, F(1, 18) = 2.15, p = 0.16, g = -0.63 [-1.53,0.27]. Overall, participants engaged in more than the recommended levels of PA each week. This pilot study provided feedback and guidance for future online SC training within a workplace PA intervention.

Perspectives of older adults with chronic disease on the use of wearable technology and video games for physical activity.

BackgroundThere is increasing interest in technology to deliver physical rehabilitation and allow clinicians to monitor progress. Examples include wearable activity trackers and active video games (AVGs), where physical activity is required to play the game. However, few studies have explored what may influence the effectiveness of these as technology-based physical activity interventions in older adults with chronic diseases.ObjectiveThis study aimed to explore: 1) perceptions about wearable physical activity trackers; 2) perceptions about using technology to share physical activity information with clinicians; 3) barriers and motivators to playing games, including AVGs for rehabilitation.MethodsQualitative study based on semi-structured interviews with older adults (n = 19) with chronic obstructive pulmonary disease (COPD).ResultsWearable activity trackers were perceived as useful to quantify activity, facilitate goal-setting, visualize long-term improvements and provide reminders. Participants generally wished to share data with their clinicians to gain greater accountability, receive useful feedback and improve the quality of clinical care. Participants were motivated to play games (including AVGs) by seeking fun, social interaction and health benefits. Some felt that AVGs were of no benefit or were too difficult. Competition was both a motivator and a barrier.ConclusionsThe findings of the present study seek to inform the design of technology to encourage physical activity in older adults with chronic diseases.

Promoting physical activity through text messages: the impact of attitude and goal priority messages

ABSTRACT Introduction Many young adults demonstrate insufficient rates of physical activity (PA) to yield health benefits. The study tested the effectiveness of a text messaging intervention targeting key psychological determinants and PA. Methods Participants received either attitude messages, goal priority messages, a combination of these, or generic PA information (control). After confirming that groups were matched at baseline, a 2 (attitude: yes vs. no) by 2 (goal priority: yes vs. no) by 2 (time: immediately post-intervention, four weeks post-intervention) randomized control trial tested main and interactive effects. Results Results showed participants that received attitude messages had significantly more positive attitudes, intentions and rates of PA. Mediational analyses showed the influence of attitude messages on PA to be fully mediated through the serial path via attitude and intention. There were no other main or interactive effects. Conclusion The study provides support for using attitudinal messages delivered via text messaging to influence key psychological determinants and PA.

Daily motivational text messages impact on college students’ nutritional awareness and physical activity levels

Objective: To examine the effect of receiving daily motivational text messages on the change in college students’ nutritional choices and levels of physical activity. Participants: Two hundred and one university students participated in an eight-week experimental/control group study. Methods: All participants received a brochure that contained nutritional and physical-activity information. The survey consisted of three parts: demographic information, health and diet, and physical activity. All participants completed a self-administered survey at the beginning and the end of the eight-week study. The experimental group participants received a daily motivational-text message. Results: There were significant interactions between group and time on most of the health and diet items, but there were no significant interactions on the physical activity items. Conclusions: It is crucial to monitor and promote college students’ nutrition and physical activity behaviors. The study findings will be vital in designing interventions that enhance college students’ general health.

Discrimination of simultaneous psychological and physical stressors using wristband biosignals

Background and objective: In this work, we address the problem of detecting and discriminating acute psychological stress (APS) in the presence of concurrent physical activity (PA) using wristband biosignals. We focused on signals available from wearable devices that can be worn in daily life because the ultimate objective of this work is to provide APS and PA information in real-time management of chronic conditions such as diabetes by automated personalized insulin delivery. Monitoring APS noninvasively throughout free-living conditions remains challenging because the responses to APS and PA of many physiological variables measured by wearable devices are similar. Methods: Various classification algorithms are compared to simultaneously detect and discriminate the PA (sedentary state, treadmill running, and stationary bike) and the type of APS (non-stress state, mental stress, and emotional anxiety). The impact of APS inducements is verified with commonly used self-reported questionnaires (The State-Trait Anxiety Inventory (STAI)). To aid the classification algorithms, novel features are generated from the physiological variables reported by a wristband device during 117 hours of experiments involving simultaneous APS inducement and PA. We also translate the APS assessment into a quantitative metric for use in predicting the adverse outcomes. Results: An accurate classification of the concurrent PA and APS states is achieved with an overall classification accuracy of 99% for PA and 92% for APS. The average accuracy of APS detection during sedentary state, treadmill running, and stationary bike is 97.3, 94.1, and 84.5%, respectively. Conclusions: The simultaneous assessment of APS and PA throughout free-living conditions from a convenient wristband device is useful for monitoring the factors contributing to an elevated risk of acute events in people with chronic diseases like cardiovascular complications and diabetes.

Features of patient-education websites for patients with chronic kidney disease: an analysis of recommended websites

BackgroundChronic kidney disease (CKD) requires lifelong self-management. With the rise in access to the Internet, many CKD patients and their caregivers increasingly use the internet for information on CKD self-management. A recent environmental scan by Smekal et al. identified 11 CKD-related websites that covered the greatest number of content areas. This paper aims to evaluate these 11 selected websites in order to identify those that most effectively address content areas relevant to patients with CKD.MethodsEach website was assessed for information to 6 content areas: diet, physical activity, financial information, emotional support, general CKD information, and medication adherence. A three-tiered scoring metric was used in which a 0 was given if a content area was completely unaddressed, a (+) was given for a category that was generally addressed, and a (++) was given for a category that was addressed with actionable guidance.ResultsWhile CKD information and diet were very comprehensively covered with scores of 11 (++) and 8 (++), respectively; physical activity, emotional support and medication adherence received the fewest (++) scores (3 for physical activity and five for both emotional support and medication adherence). For each content area, recommendations are made for websites that are particularly useful. Common themes for these highlighted websites include specific instructions, multiple modalities of information, downloadable and printable resources, and contact references for personal inquiries.ConclusionThe recommended websites can help CKD patients and caregivers utilize the most applicable information for their specific self-management needs. Website improvements related to physical activity, emotional support, and financial information for persons with CKD are warranted.

Promoting Active Aging: Lessons Learned in an Implementation Pilot in Primary Care.

Physical activity (PA) preserves mobility, but few practices screen older adults for mobility impairment or counsel on PA. "Promoting Active Aging" (PAA) was a mixed-methods randomized-controlled pilot, to test the feasibility and acceptability of a video-based PA counseling tool and implementation into practice of two mobility assessment tools. Three primary care practices affiliated with Wake Forest Baptist Health. Adults aged 65 years and older who presented for primary care follow-up and were willing and able to answer self-report questions and walk 4 meters (n = 59). Video-based PA counseling intervention versus control video, "Healthy Eating." Potential participants completed mobility assessments: self-report (Mobility Assessment Tool-short form (MAT-sf)) and performance based (4-meter walk test). We assessed PAA's implementation-feasibility, acceptability, and value-via interviews and surveys. Effectiveness was measured via participant attendance at a PA information session. Of 92 patients approached, 89 (96.7%) agreed to mobility assessment. Eighty-nine completed MAT-sf, and 97.8% (87/89) completed 4-meter walk test. Sixty-seven (75%) met eligibility criteria, and 59 (88%) consented to be randomized either to the PA counseling intervention (Video-PA) or to active control (Video-C). Most participants viewed the walk test positively (51/59; 86.4%). Staff reported that completion of patient surveys, MAT-sf, and videos required significant staff time and support (median = 26 minutes for all), resulting in low acceptability of MAT-sf and the videos. Attendance at a PA information session did not differ by randomization group (Video-PA = 11/29 (37.9%); Video-C = 12/30 (40%); 95% confidence interval for difference in proportion = -0.29 to 0.25). Mobility assessment, particularly a 4-meter walk test, was feasible in primary care. Tablet-based assessment (MAT-sf) and video counseling tools, selected to reduce staff effort, instead required significant time to implement. Future work to promote PA should identify effective ways to facilitate adoption of PA in sedentary older adults that do not burden staff.

Infertility and physical activity: A cross-sectional study of women living in Yazd aged 20-49 yr, 2014-2015.

BackgroundChanges in the state of energy balance owing to changes in physical activity (PA) may affect the reproductive system.Objectivethe aim of this study was to assess the association between PA and infertility of women living in Yazd 2014 - 2015.Materials and Methods The study method was analytical cross-sectional on the Yazd Health Study (YaHS) data which was conducted on 10,000 people. We studied 2,611 women (20 and 49 yrs old), living in Greater Yazd area. PA information was collected using a physical activity questionnaire (IPAQ) short form. The standardized instruments were used for anthropometrics measurement.ResultsTotally 135 cases of infertility were documented. The overall prevalence of infertility was 4.73% (95% CI: 3.94-5.59%). The median of PA scores (METs) in women was 746.66 and 25-75% interquartile range was 361.25-1277.25. The relationship according to the Chi-square test between infertility and PA, as categorized variables, was significant (p = 0.015). It showed over 90% of those who suffered from infertility had a low or moderate PA.ConclusionThe results of our study showed that there is a significant relationship between infertility and PA level in women living in Greater Yazd area. Also, women with infertility had lower activity levels, consistent with most previous studies.

Physical Activity During Pregnancy and Preterm Birth: Findings From the 2015 Pelotas (Brazil) Birth Cohort Study.

Physical activity (PA) during pregnancy is associated with several benefits in maternal and child outcomes, and its relationship with preterm birth is still conflicting. This study aims to examine the associations between PA during pregnancy and occurrence of preterm birth. PA was assessed by questionnaire (for each trimester) and accelerometry (second trimester) in women enrolled in a birth cohort study that started during pregnancy and included births that occurred between January 1 and December 31, 2015. Gestational age was based on the last menstrual period and ultrasonography. All deliveries before 37weeks of gestation were considered preterm births. A Poisson regression model was used to measure associations controlling for potential confounders. PA information was available for 4163 women and 13.8% of births were preterm. A total of 15.8% of women were engaged in PA during pregnancy. Multivariate analysis showed that only PA performed in the third trimester of pregnancy (prevalence ratio = 0.58; 95% confidence interval, 0.36-0.96) was associated with the outcome. PA performed in the third trimester of pregnancy was associated with a protection to preterm birth. Pregnant women should be counseled to engage in PA to lower the risk of premature delivery.

Identification of human activity considering social interactions and group dynamics non-intrusively has been one of the fundamental problems and a challenging area of research. In real life, it is required for designing human-centric applications like assisted living, health care, and creating a smart home environment. As human beings spend 90% of time indoors, such a system will be helpful to monitor the behavioral anomalies of the inhabitants. Existing approaches have used intrusive or invasive methods like camera or wearable devices. In this work, we present a device-free, non-invasive, and non-intrusive sensing framework called HumanSense using an array of heterogeneous sensor grid for human activity monitoring. The sensor grids, comprising the ultrasonic and sound sensors, have been deployed for collective sensing combining a person’s physical activity and verbal interaction information. The proposed system senses a stream of events when the occupant(s) perform different physical activities categorized as atomic and group activities like sitting, standing, and walking. Simultaneously, it also tracks person-person verbal interactions such as monologue and discussion. Both information are then integrated into a single framework to understand the overall behavioral scenario of the indoor environment. The experimental results have shown that HumanSense can detect different activities with accuracy more than 90% and also improves overall identification accuracy compared to existing works. Our developed system can be further evolved into ready-to-deploy smart sensing panels which can be effective for human activity monitoring in an indoor environment.

A gender-sensitised weight-loss and healthy living program for men with overweight and obesity in Australian Football League settings (Aussie-FIT): A pilot randomised controlled trial.

Recent evidence shows that sport settings can act as a powerful draw to engage men in weight loss. The primary objective of this pilot study was to test the feasibility of delivering and to evaluate preliminary efficacy of Aussie-FIT, a weight-loss program for men with overweight/obesity delivered in Australian Football League (AFL) settings, in preparation for a future definitive trial. This 6-month pilot trial took place in Perth, Australia. Participants were overweight/obese (Body Mass Index [BMI] ≥ 28 kg/m2), middle-aged (35-65 years old) men. Participants were recruited in May 2018, and the intervention took place between June and December 2018. The intervention involved 12 weekly 90-min face-to-face sessions, incorporating physical activity, nutrition, and behaviour change information and practical activities delivered by coaches at 2 clubs. Data were collected at baseline and immediately postintervention. For trial feasibility purposes, 6-month follow-ups were completed. Outcomes were differences in weight loss (primary outcome) and recruitment and retention rates, self-reported measures (for example, psychological well-being), device-measured physical activity, waist size, and blood pressure at 3 months. Within 3 days of advertising at each club, 426 men registered interest; 306 (72%) were eligible. Men were selected on a first-come first-served basis (n = 130; M age = 45.8, SD = 8; M BMI = 34.48 kg/m2, SD = 4.87) and randomised by a blinded researcher. Trial retention was 86% and 63% at 3- and 6-month follow-ups (respectively). No adverse events were reported. At 3 months, mean difference in weight between groups, adjusted for baseline weight and group, was 3.3 kg (95% CI 1.9, 4.8) in favour of the intervention group (p < 0.001). The intervention group's moderate-to-vigorous physical activity (MVPA) was higher than the control group by 8.54 min/day (95% CI 1.37, 15.71, p = 0.02). MVPA among men attracted to Aussie-FIT was high at baseline (intervention arm 35.61 min/day, control arm 38.38 min/day), which may have limited the scope for improvement. Aussie-FIT was feasible to deliver; participants increased physical activity, decreased weight, and reported improvements in other outcomes. Issues with retention were a limitation of this trial. In a future, fully powered randomised controlled trial (RCT), retention could be improved by conducting assessments outside of holiday seasons. Testing the effectiveness of physical activity advice delivered via text messaging vs. human phone advisors in a Latino population: The On The Move randomized controlled trial design and methods

Physical inactivity is a key risk factor for a range of chronic diseases and conditions, yet, approximately 50% of U.S. adults fall below recommended levels of regular aerobic physical activity (PA). This is particularly true for ethnic minority populations such as Latino adults for whom few culturally adapted programs have been developed and tested. Text messaging (SMS) represents a convenient and accessible communication channel for delivering targeted PA information and support, but has not been rigorously evaluated against standard telehealth advising programs. The objective of the On The Move randomized controlled trial is to test the effectiveness of a linguistically and culturally targeted SMS PA intervention (SMS PA Advisor) versus two comparison conditions: a) a standard, staff-delivered phone PA intervention (Telephone PA Advisor) and b) an attention-control arm consisting of a culturally targeted SMS intervention to promote a healthy diet (SMS Nutrition Advisor). The study sample (N = 350) consists of generally healthy, insufficiently active Latino adults ages 35 years and older living in five northern California counties. Study assessments occur at baseline, 6, and 12 months, with a subset of participants completing 18-month assessments. The primary outcome is 12-month change in walking, and secondary outcomes include other forms of PA, assessed via validated self-report measures and supported by accelerometry, and physical function and well-being variables. Potential mediators and moderators of intervention success will be explored to better determine which subgroups do best with which type of intervention. Using a think-aloud methodology to understand online physical activity information search experiences and preferences of parents of children and youth with disabilities.

The Internet is a preferred source of physical activity (PA) information. However, limited research exists regarding the experiences of parents of children and youth with disabilities (CYWD) in searching for PA programme information online. This research examined the experiences and preferences of parents of CYWD in searching for PA programme information online. Parents of CYWD (n=10) participated in a think-aloud exercise while searching for PA programme information online. Following the think-aloud exercise, semi-structured interviews were used to further understand parents' experiences and preferences in searching for PA programme information online. Parents identified key features that contributed to a positive online search experience. Additionally, parents noted challenges and resulting negative affect that was experienced. This research can inform the development and dissemination of online PA programme information that is accessible and relevant to the preferences of parents of CYWD and can facilitate positive search experiences.

Abstract C049: The association between breast cancer and physical activity levels by race in a prospective cohort study

Abstract Background: Although the relationship between physical activity and breast cancer is well established, research demonstrating levels of physical activity and breast cancer association among different racial groups is less definitive. It is perceived by the public that beneficial effects of physical activity depend on the rigorousness of activity performed, and within these levels of physical activity, racial differences are observed. Objectives: The aim of this study is to evaluate levels of physical activity among different racial groups, while stratifying for menopause status, and their relationship to breast cancer. Specifically examining incidence and prevalent breast cancer cases, this study examined potential variation of physical activity in relation to time of cancer diagnosis among different racial groups. Methods: A cohort of 26,387 women in the Arkansas Rural Community Health Study (ARCH) was used to evaluate physical activity and its association with breast cancer among different racial groups. Multiple logistic regression (OR, 95%CI) was used to examine the association between breast cancer and intensity, length, and frequency of physical activity while adjusting for confounding variables, such as age and parity. Demographic and weekly physical activity information was reported at baseline. A Metabolic Equivalent of Task score was derived from the self-reported questionnaire data. Results: A total of 1,455 participants were excluded due to insufficient information regarding breast cancer status, race, and menopausal status, resulting in a sample of 23,980 participants. The analysis consisted of 155 newly diagnosed breast cancer cases after the baseline (incident), 2,330 existing breast cancer cases when entering the study (prevalent), and 21,495 noncancer subjects, of whom 78% identified as European American (EA) and 22% identified as African American (AA). Approximately 49% of eligible participants reported being postmenopausal and 51% being premenopausal. A significant downward trend was observed in vigorous physical activity among EA women for incident and prevalent breast cancer (p= 0.002 and 0.04), whereas no clear trend was seen among AA women due to small sample size. Similar trends can be seen in vigorous physical activity postmenopausal EA women (p= 0.005 and 0.002). The beneficial association was not observed among AA women. On the other hand, overall physical activity appears to be positively associated with breast cancer among AA women (p for trend = 0.04). Conclusions: There appears to be a beneficial effect of vigorous physical activity and breast cancer among EA, regardless of pre- or postmenopausal status. 690-P: Use of Physiological and Psychological States to Enhance Glucose Concentration Estimation

The accuracy of blood glucose concentration (BGC) estimation affects insulin dosing decisions. Wristband biosignals (Empatica E4) are used to estimate physical activity type and intensity, and psychological stress to enhance BGC prediction accuracy. Three cases are compared: (1) BGC prediction by using only continuous glucose monitor (CGM) data, which considers only meal effects and plasma insulin concentration (PIC); (2) Incorporating physical activity information to complement CGM data; and (3) Incorporating both physical activity and psychological stress information to complement CGM data. Twenty-eight experiments with seven subjects with T1D are conducted including various physical activities and acute psychological stress inducements. The subjects wore a CGM, insulin pump and E4 wristband during the experiments. Physical activities included treadmill running, stationary bicycling, resistance exercises and activities of daily living. Acute psychological stress inducements included the Trier Social Stress test, playing competitive videogames, solving arithmetic problems, and daily stressful events. The overall mean absolute errors (%) were 10.42, 8.69. and 8.42 for Cases 1, 2, and 3, respectively. The results illustrate that incorporating estimates of psychological and psychological effects improve the BGC prediction accuracy by 17.18% (Freidman’s ANOVA test, p-value &amp;lt; 0.01).
