Nursing care to COVID-19 patients on hemodialysis and in prone position: an experience report.

To describe the experience report of a nurse providing care to COVID-19 patients on hemodialysis and in prone position. Experience report. This study was conducted in a philanthropic hospital in a Brazilian capital from May to August 2020 during the Coronavirus pandemic. The nurse has faced several challenges concerning structure, material, human resources, and care when treating COVID-19 patients on hemodialysis and in prone position. Management based on dialogue and shared evidence-based information and implementation of a new care protocol were a foundation for care reorganization of the Nursing team for providing care to COVID-19 patients. This experience emphasizes the importance of nursing care to patients' responses, with actions based on care protocols, strengthening human relations. The conduction of different study designs is necessary to contribute to an improved nursing care and survival of patients experiencing COVID-19 complications.

The cost of metastatic prostate cancer using time-driven activity-based costing.

To estimate the cost of metastatic prostate cancer (mPC) treatment using the time-driven activity-based costing (TDABC) method from the perspective of a philanthropic hospital in the Brazilian public health system (PHS) and to identify determinants of costs. We used data from patients who received docetaxel chemotherapy in the Brazilian PHS from September 2012 to May 2017. Direct medical costs were estimated with the TDABC microcosting method, taking into account the multiple departments and services the patients interacted with during their oncological treatment. The median overall survival of the forty-three patient sample was 1.8 years (95% CI 1.45-2.30), and the total cost of the sample was BRL 917.005 (USD 250,878). The median monthly cost per patient was BRL 20.201 (USD 5,526). The end-of-life cost per patient using the TDABC method was BRL 5.151 (USD 1,409). Patients who had received previous treatment at the center registered the lowest cost for hospitalizations and exams, suggesting an opportunity to better manage healthcare resources. This is the first study on the economic burden of mPC in the Brazilian PHS using the TDABC costing evaluation method. Accurate cost information obtained with the TDABC can be helpful in guiding disease management to guarantee better use of ever-scarcer resources.

Awake craniotomy in brain tumors - Technique systematization and the state of the art.

The anesthesia for awake craniotomy (AC) is a consecrated anesthetic technique that has been perfected over the years. Initially used to map epileptic foci, it later became the standard technique for the removal of glial neoplasms in eloquent brain areas. We present an AC anesthesia technique consisting of three primordial times, called awake-asleep-awake, and their respective particularities, as well as delve into the anesthetic medications used. Its use in patients with low and high-grade gliomas was favorable for the resection of tumors within the functional boundaries of patients, with shorter hospital stay and lower direct costs. The present study aims to systematize the technique based on the experience of the largest philanthropic hospital in Latin America and discusses the most relevant aspects that have consolidated this technique as the most appropriate in the surgery of gliomas in eloquent areas.

O trabalho do enfermeiro cirúrgico e o potencial para minimizar complicações pós-operatórias

Objetivo: Relatar o potencial do trabalho do enfermeiro de centro cirúrgico para minimizar complicações pós-operatórias. Relato de experiência: Construído por discentes e docentes de enfermagem a partir de atividades práticas em um Centro Cirúrgico de um hospital filantrópico. Os resultados foram categorizados em: o potencial para a segurança do paciente está relacionada a utilização de instrumentos como Lista de Verificação de Cirurgia Segura, enquanto que o potencial de autonomia está na implementação de instrumentos que permitam a comunicação verbal e escrita para a continuidade da assistência, bem como gerenciamento do serviço. O potencial para o trabalho colaborativo está na ação conjunta entre os envolvidos nos períodos pré-intra e pós-operatório do paciente, os quais permitem minimizar complicações pós-operatórias. Considerações finais: Fica evidente, a partir da vivência em campo prático, a importância do papel do enfermeiro de centro cirúrgico para minimizar complicações pós-operatórias. As não conformidades identificadas na prática pelos discentes demonstram fragilidades que devem ser reorganizadas para garantir a segurança do paciente, desenvolvimento da autonomia profissional e trabalho colaborativo. Todavia, observa-se que o enfermeiro ao deter-se de conhecimento e instrumentos de avaliação do cuidado, reforçam sua autonomia profissional e trabalho colaborativo.

PCN25 First Real-World Evidence of Immunotherapy in the Brazilian Public Health System: A Retrospective Study in Melanoma Patients Treated with Pembrolizumab

Pembrolizumab is a PD-1 antibody-mediated inhibitor approved for metastatic melanoma (MM) treatment with proven evidence of increasing patient survival. Immunotherapy use is not yet a reality in the Brazilian public health system and there is no data outside clinical trials regarding the efficacy of pembrolizumab in MM patients. The objective of the present study was to assess the effectiveness of pembrolizumab in MM patients in a Brazilian, oncologic, philanthropic hospital. Among 155 patients diagnosed with MM between 2017 and 2020, 42 received at least one course of treatment with pembrolizumab. Demographic and clinical characteristics of patients and treatment outcomes were extracted from the hospital-based registry and analyzed by SPSS 20.0. Progression-free survival (PFS) and overall survival (OS) were analyzed. Cox regression was performed for PFS and OS using gender, age, presence of brain metastases, number of metastatic sites, lines of therapy, body mass index and BRAF status as covariates. The baseline profile of patients was: 59.5% were under 60 years of age at diagnosis; 54.8% were males and 54.8 were BRAF V600E–mutant; 64.3% of patients received pembrolizumab as first line therapy and 42.9% had more than 4 different metastasis sites. The median OS and PFS of pembrolizumab-treated patients were 14.9 and 4.5 months, respectively. Among patients with BRAF V600E–mutant, median OS was 14.9 months vs 18.1 months in BRAF wild-type patients (p=0.047). The median OS was 11.3 months among male and was not reached in female (p=0.028). No statistical difference was detected in the other covariates. This study provides the first real-world evidence on the effectiveness of pembrolizumab in MM patients in Brazil, confirming the clinical benefits of immunotherapy in the public health system, in which dacarbazine is currently the available standard of care treatment for MM despite not being effective in prolonging patient survival.

Levantamento dos custos do diagnóstico e tratamento oncológico no paciente idoso

Objetivo: investigar o custeio do diagnóstico e tratamento de câncer em idosos de um Centro Oncológico de Minas Gerais. Método: Os dados foram coletados através de fonte secundária do Registro Hospitalar do Câncer, de um Hospital Filantrópico do Interior de Minas Gerais, entre o ano de 2009 a 2016, com uma amostra de 3.666 idosos em tratamento oncológico. A análise dos resultados foi feita por meio de estatí­stica descritiva simples. Todos os aspectos éticos foram resguardados. Resultados: Entre as principais fontes de diagnóstico das neoplasias destaca-se a histologia do tumor, correspondendo a 85,50%. Quanto ao custeio do diagnóstico e tratamento, em 25,09% e 86,05% dos casos, respectivamente, foram custeados pelo Sistema Único de Saúde. Conclusão: é importante ressaltar que o diagnóstico e tratamento precoce aumentam as chances de cura e que o Sistema Único de Saúde cobre a maioria dos custos, tornando possí­veis as intervenções apropriadas para estes pacientes.

The response of clinical medical librarians to the COVID-19 pandemic: a case study

The COVID-19 pandemic has changed the world with a large part of the population in lockdown for months. Given the need medical teams have for information, clinical medical librarians have played an important role in searching for the best scientific evidence. The objective of this paper is to describe the role of librarians in Brazil during the pandemic in public and private hospital libraries and in the Health Technology Assessment Service. This is a case study report of the work of clinical medical librarians during the COVID-19 pandemic. The paper is drawing on experience from three clinical medical librarians acting in the private hospital, philanthropic hospital and Health Technology Assessment Service in the city of Belo Horizonte, Minas Gerais, Brazil. The report describes the activities of clinical librarians in providing clinicians with information on how to care for patients with COVID-19. In Brazil clinical medical librarians play an important role in supporting medical teams by identifying reliable sources of information and assisting with research.

Diminuindo barreiras organizacionais a comunicação interna como estratégia de gestão hospitalar

A comunicação interna impacta positivamente na satisfação com o trabalho e no comprometimento dos funcionários com a organização. Porém, pouco se discute sobre os benefícios da comunicação interna em organizações hospitalares. Esta pesquisa teve como objetivo analisar como os funcionários de um hospital avaliam as estratégias de comunicação interna e como a comunicação interna impacta no sentimento de orgulho dos funcionários em fazerem parte da organização. Foram realizadas pesquisa descritiva, com 89 colaboradores, observações in loco e análise de documentos internos da organização. Observou-se que o hospital possui poucos canais formais de comunicação interna, um organograma verticalizado e gestão não democrática dos processos decisórios. A avaliação das estratégias de comunicação interna depende do gênero e do nível de escolaridade do funcionário. Demonstrou-se, que os funcionários sentem mais orgulho em pertencer a organização quando ela facilita a comunicação interna e quando as suas manifestações espontâneas são valorizadas pela direção do hospital.

Perfil de força de preensão manual em pacientes idosos com câncer de próstata

Objetivo: analisar o perfil de Força de Preensão Manual em pacientes idosos com câncer de próstata, atendidos em instituição hospitalar filantrópica do Recife.Métodos: estudo transversal com análise de dados secundários, coletados no Ambulatório de Oncogeriatria do Instituto de Medicina Integral Professor Fernando Figueira, utilizando uma amostra por conveniência com uma amostragem de 72 idosos. A análise estatística foi realizada utilizando o programa estatístico STATA 12. O teste de Kolmogorov-Smirnov foi aplicado a variáveis numéricas para se determinar a normalidade da amostra. Em caso de não normalidade, foi utilizado o teste de Mann-Whitney. Adotou-se o nível de significância de 0,05%.Resultados: observou-se uma Força de Preensão Manual reduzida nos pacientes com estadiamento de uma doença mais avançada quando comparado a outros estágios mais localizados ou clinicamente melhores (estadiamento I: 27,78 vs. estadiamento IV: 27,20 no braço dominante (p=0, 340)/ estadiamento I: 27,33 vs. estadiamento IV: 24,13 no braço não dominante) sendo esse resultado melhor reproduzido na mão não dominante (p=0,090). Por meio da análise das comorbidades (hipertensão, diabetes, cardiomiopatia e osteoartrite) em relação à mão dominante e não dominante, não foram verificadas diferenças significativas na Força de Preensão Manual na presença ou ausência das comorbidades analisadas tanto na mão dominante quanto da não dominante (p=0,189 vs.p=0,437).Conclusões: o estudo trouxe subsídios relevantes sobre a importância da Força de Preensão Manual como um importante parâmetro geral para força e um indicativo de saúde, principalmente quando analisados em idosos com câncer, onde afeta significativamente sua função física e neuromuscular, incluindo atrofia muscular, diminuição da força muscular e diminuição do desempenho funcional em atividades diárias, comprometendo significativamente a qualidade de vida e seu prognóstico.

Interações Medicamentosas Potenciais em pacientes internados em um hospital filantrópico

Objetivo: identificar interações medicamentosas potenciais em prontuários de pacientes de um hospital e maternidade de um município brasileiro. Métodos: estudo documental, retrospectivo, com abordagem quantitativa, realizado entre janeiro e junho de 2017, em um hospital filantrópico do Estado do Ceará, Brasil. A fonte documental foram os prontuários de 167 pacientes. Os dados foram analisados por estatística descritiva com frequência absoluta e relativa. As interações medicamentosas potenciais foram verificadas por meio do Sistema Micromedex. Resultados: dos 167 prontuários, 80 (48%) eram de pacientes do sexo masculino e 87 (52%) do sexo feminino. Identificou-se que 24 das interações medicamentosas foram “potenciais maiores”, presentes, sobretudo, na associação dos seguintes fármacos: dipirona/ibuprofeno, dipirona/prednisona, dipirona/diclofenaco. Conclusão: o estudo mostra a necessidade de maior cautela na prescrição de dipirona com outros medicamentos, haja vista um maior envolvimento dele em interações medicamentosas potenciais.

Sofrimento moral em trabalhadores de Enfermagem de um hospital filantrópico

Objetivou-se investigar a frequência e intensidade de sofrimento moral nos trabalhadores de enfermagem de um hospital filantrópico. Estudo transversal, realizado com 177 trabalhadores de enfermagem de um hospital filantrópico. Utilizou-se instrumento com dados sociodemográficos, laborais e a escala Moral Distress Scale-Revised. Para análise dos dados empregou-se estatística descritiva. A média de intensidade de sofrimento moral vivido pela equipe de enfermagem variou de 1,32 a 2,12, enquanto a de frequência variou de 0,52 até 2,08. Evidenciaram-se maiores médias em situações que envolviam seguir os desejos da família para manter a vida e de obstinação terapêutica. O sofrimento moral global apresentou frequência e intensidade média de 67,83. Conclui-se que os trabalhadores de enfermagem apresentaram-se em baixo a moderado sofrimento moral.

Characterization of congenital syphilis cases with emphasis on the therapeutic scheme in a philanthropic maternity hospital in Espírito Santo

Introduction: Syphilis is still a worldwide problem; with approximately 12 million people infected every year. Over the last decade, Brazil had an increment in the number of cases. The year 2016 reported 37,436 cases of syphilis in pregnant women and 20,474 cases of congenital syphilis, with 185 deaths. The Southeast region reached the highest numbers, especially in the state of Espírito Santo with high rates in pregnant women, being the third highest incidence rate in the country with 10.4 cases/1,000 live births, above the national average of 6.8 cases/1,000 live births.
 Objective: Describe the therapeutic treatment of children affected with congenital syphilis as well as the clinical, radiological and laboratory changes associated to this disease.
 Methods: Retrospective, descriptive, exploratory, quantitative study, based on 204 notification forms of congenital syphilis from January 2016 to December 2017.
 Results: The findings showed that 88.7% of the puerperal women performed prenatal care. Regarding newborns, 85.3% were asymptomatic. When analyzing the therapeutic regimen instituted, 22.5% used procaine Penicillin G, 22.5% crystalline Penicillin G and 20.6% benzathine Penicillin G. 
 Conclusion: The treatment instituted by this philanthropic maternity for newborns with congenital syphilis is in line with the guidelines proposed by the Ministry of Health. The clinical symptoms do not represent a frequent finding during the neonatal period, however, they can occur later.

The Client Capital in Nursing Management in Hospitals

Abstract Objective: To analyze how the components of the client's capital are used in the management of nursing in hospitals. Method: A qualitative research carried out in five public hospitals, four private hospitals and three philanthropic hospitals in the period from October 2014 to May 2015. Data were collected through a semi-structured interview with twelve nursing managers and analyzed according to content analysis. Results: The components of the Client's Capital were used by the nurses when performing the management of the client service to promote changes and improve the service after evaluating the satisfaction of the nursing care through active systematic search, with its own evaluation tools and the ombudsman's information and in the interaction with suppliers of materials, equipment and services. Conclusion and implications for practice: The managers use managerial actions for the development of the client's capital of organizations. In order to improve them, they must evaluate the nursing care in a continuous and strategic process to satisfy the clients, throughout their trajectory in the organization, since clients are permanent sources of innovation that affect professional and organizational development and productivity.

Implementation of an Artificial Intelligence Algorithm for sepsis detection.

to present the nurses' experience with technological tools to support the early identification of sepsis. experience report before and after the implementation of artificial intelligence algorithms in the clinical practice of a philanthropic hospital, in the first half of 2018. describe the motivation for the creation and use of the algorithm; the role of the nurse in the development and implementation of this technology and its effects on the nursing work process. technological innovations need to contribute to the improvement of professional practices in health. Thus, nurses must recognize their role in all stages of this process, in order to guarantee safe, effective and patient-centered care. In the case presented, the participation of the nurses in the technology incorporation process enables a rapid decision-making in the early identification of sepsis.

NURSE'S PROTAGONISM IN STRUCTURING AND MANAGING A SPECIFIC UNIT FOR COVID-19

ABSTRACT Objective: to report the experience in the structuring and managing process of a specific unit for COVID-19, highlighting the role of nurses in decision-making. Method: an experience report on the creation and management of the Coronavirus Disease 2019 (COVID-19) unit, in March 2020, in 2020, in a philanthropic hospital of the state of Santa Catarina. Results: the unit was structured with 10 intensive care beds and 20 infirmary beds. Meetings were held to make decisions, as well as to create protocols and flows with active participation of the nurse. In questions related to direct assistance, adaptations were developed in the nursing process performed at the hospital and the organization of new flows and routines. The physical space was structured, considering the high risk of transmissibility for the disease. Professionals were hired with staffing readjustment according to the complexity of the service, making up a team of professionals with experience in critical care. There were trainings for developing knowledge and skills prior to the first cases, which were systematically maintained. In addition, it was observed that the nurses were concerned about the mental health of the professionals working in this unit and, therefore, support actions were programmed. Conclusion: the foundation in the scientific evidence and recommendations of the competent bodies at the world and national levels for the structuring of the COVID-19 unit is emphasized. The role of the nurse in all the interfaces stands out, assuming a fundamental role from the composition of the commissions, going through the planning and functioning of the physical structure, management of human resources, and construction of care protocols and flows, in addition to acting directly in the care provided.

CUSTOS HOSPITALARES: APLICAÇÃO DOS MÉTODOS DE CUSTEIO ABC E TDABC NO PROCESSO DE CONSULTA MÉDICA

O estudo objetiva aferir os custos inerentes ao processo de consulta médica utilizando os métodos de custeio ABC e TDABC em uma unidade hospitalar filantrópica, relacionando-os e evidenciando possíveis ociosidades. A pesquisa caracteriza-se como como um estudo de caso, descritiva e com abordagem qualitativa. Como resultado do estudo encontrou-se um custo para a consulta médica de R$ 85,10 utilizando o método de custeio ABC e R$ 19,61 utilizando o método de custeio TDABC. No cálculo do ABC está incluso a capacidade não utilizada, enquanto no TDABC, a capacidade não utilizada não compõe o custo, bem como, fica expressa para que os gestores possam gerir reduzindo ociosidades.

Ocorrência de sintomas depressivos em paciente idosos com doença arterial periférica

Objetivo: investigar a ocorrência de sintomas depressivos em idosos com doença arterial periférica (DAP) internados num hospital filantrópico em Aracaju. Métodos: estudo descritivo, observacional, transversal realizado no Serviço de Cirurgia Vascular de um hospital terciário durante o período de novembro de 2018 a abril de 2019. Foram incluídos pacientes com idade igual ou superior a 60 anos, com diagnóstico de DAP e que responderam aos questionários sociodemográfico e escala de depressão geriátrica de 15 itens (GDS-15). Resultados: a prevalência de sintomas depressivos encontrada na população estudada foi de 61,8%, sendo 14,6% classificados como sintomas graves e 47,2% como leves. Em relação às características sociodemográficas, os pacientes tinham em média 69,1 anos de idade, a maioria era do sexo masculino, casado, proveniente do interior do estado, aposentado, católico e com renda menor ou igual a um salário mínimo. Ao comparar os pacientes idosos com e sem depressão, as variáveis estado ocupacional, etilismo, isquemia crítica, hipertensão arterial, diabetes mellitus, amputação prévia e idade foram estatisticamente significantes (p<0,05). Conclusão: A depressão em idosos esteve associada a doenças crônicas, fatores sociodemográficos e amputação. Consideramos importante uma abordagem de atendimento multidisciplinar desses pacientes com o intuito de diminuir o risco de complicações.

O Trabalho do Clown Voluntário

Esta pesquisa, descritivo-exploratória, com abordagem qualitativa dos dados, objetivou descrever e refletir sobre a organização e o desenvolvimento das intervenções realizadas por cinco clowns voluntários, com pacientes idosos em um hospital filantrópico as percepções destes voluntários sobre suas intervenções. Foram realizadas seis observações com auxílio de uma matriz de observação sistemática e aplicadas entrevistas semiestruturadas com dois clowns. Os dados foram tratados por meio de elementos da técnica de análise de conteúdo. Evidenciou-se que a conversa, a música e o teatro constituem-se instrumentos potencializadores das manifestações lúdicas e artísticas utilizados pelo grupo; a relação dos voluntários com conteúdos artísticos e lúdicos durante a infância foi fundamental no processo da construção do ser clown e na atuação no contexto hospitalar; a autonomia no grupo e no hospital é fundamental para trabalhar a especificidade de cada clown.

Reações adversas ao medicamento L-asparaginase em pacientes oncopediátricos

Introduction: Adverse drug reaction (ADR) is defined as “any harmful or undesirable and unintended response occurring with medicaments at doses usually employed in humans”. Objective: To describe the adverse reactions to L-asparaginase, developed by oncopediatric patients, accompanied by the Pharmacy Clinic service, in a philanthropic hospital, in Salvador, Bahia, Brazil. Method: This is a cross-sectional, retrospective, descriptive study, in which an analysis of nine electronic medical records of oncopediatic patients was performed using L-asparaginase from June 2017 to February 2018. Results: Among the patients, 66.7% were male and 55.6% were aged ≤ 5 years. Acute lymphoid leukemia was the diagnosis of all patients. 66.7% of the study group presented some type of adverse reaction, pruritus being the most frequent (36%) and in 66.7% of the situations, the drug was suspended. All reactions were reported. Conclusions: The results suggest that special attention is needed in oncopediatric patients, since the possibility of adverse drug reactions is higher. With the performance of the pharmacist in centers of high complexity in Oncology, it is possible to develop strategies and promote actions to prevent or minimize the occurrence of these adverse events during antineoplastic therapy.

A CONSTRUÇÃO DA ESTRATÉGIA EM UM HOSPITAL FILANTRÓPICO: UMA ANÁLISE COM BASE NA ESTRATÉGIA COMO PRÁTICA

O artigo tem como tema a gestão estratégica em estabelecimentos hospitalares e seu desenvolvimento foi alicerçado sob a abordagem da Estratégia como Prática. Assim, teve como objetivo analisar o processo de formação da estratégia na prática de um hospital filantrópico de grande porte, para tanto, identificaram-se os atores, o modo como as práticas organizacionais interferem na formação das estratégias e as práxis relacionadas. Também foi estudada qual a postura da instituição frente a seu meio. Configura-se como uma pesquisa aplicada, caracterizada como um estudo de caso; a análise de dados foi qualitativa e pautada na análise crítica do discurso. Os resultados obtidos evidenciam que as estratégias da instituição não são dissociadas das práticas organizacionais, mas, ao contrário, exercem influências recíprocas. Isso propicia uma visão sistêmica, foco para ações e resulta no alcance dos objetivos estipulados. O presente trabalho contribui para um melhor entendimento do processo estratégico em ambientes dinâmicos e altamente regulados, mostrando que, embora haja determinismo ambiental, a organização adota uma postura dialética, com espaço para escolhas estratégicas. Além disso, contribui também com uma reflexão acerca dos atores envolvidos no processo estratégico, uma vez que mostrou que alguns atores não têm interesse em participar na construção de um plano formal.
