Obesity-diabetes and anorexigenics.

Previous observations on 62 obese diabetic patients showed that phenmetrazine hydrochloride significantly reduced appetite and weight. In the present study 30 obese diabetic patients were divided into two groups. Each patient received 25 mg. per day of either phenmetrazine hydrochloride or diethylpropion under double-blind conditions. Group A (14 patients) lost, on the average, 6.71 lb. (3.05 kg.) during an initial six-week phenmetrazine period and 1.37 lb. (0.62 kg.) during the subsequent six-week diethylpropion period. Group B (16 patients) lost 2.77 lb. (1.26 kg.) during their initial six weeks of taking diethylpropion and 3.44 lb. (1.56 kg.) during the subsequent six-week phenmetrazine period. Both drugs were significantly anorexigenic; but in the dosages used, phenmetrazine caused both greater weight-loss and a higher incidence of side-effects.

The use of these agents in the treatment of maturity-onset, mild diabetes, at least, is now firmly entrenched and widespread. Although they have proved their worth, in selected cases, by providing control of the disease while freeing significant numbers of this type of diabetic patient from the pain and inconvenience of daily insulin injections, there is still foremost among the dangers inherent in this long sought-for modality an enormous potentiality for abuse.Practical...Bibliography1. Dangers of "Preludin"

Weight control; a practical office approach.

Phenmetrazine hydrochloride was used, alternating with a placebo, in a weight reduction program that included diet therapy, close doctor-patient cooperation, follow-up visits, exercise, and instruction in posture. The usual dosage was one 25-mg. tablet by mouth before the noon and evening meal; the maximum dosage was three tablets thrice daily to a total of 225 mg. Effects of suggestion were eliminated by a double-blind method of administering drug and placebo; odd-numbered patients were started on placebo, even-numbered on drug therapy. Each drug (or placebo) period lasted four weeks. At the end of each period, each patient was automatically shifted to the drug or placebo group, depending upon what he had received during the previous period. The patients were studied from a minimum of 8 weeks to a miximum of 35 weeks. Fifty-seven (61.2%) of the 93 patients treated lost an average of 0.86 kg. (1.9 lb.) per week while receiving phenmetrazine as compared with 0.25 kg. (0.54 lb.) average weekly weight loss while receiving the placebo. The importance of the double-blind technique was illustrated by the occurrence of anorexia, nausea, xerostomia, insomnia, and nervousness in 15 patients during placebo periods. Side-effects during periods of active medication were easily controlled.

Treatment of obesity with phenmetrazine hydrochloride, a new anorexiant.

By means of a double-blind procedure, 30 obese patients were tested to measure the effectiveness of a new anorexiant. The total weight lost and the average weight lost weekly by each patient were five times as great for those on therapy with the active drug as compared to those on therapy with a placebo. No special stress was placed on diet, although the elements of a well-balanced, 1,100-calorie regimen were explained. There were no unusual side-reactions, subjective complaints, or changes in physical findings to indicate toxicity of the drug.
