Real-time computing in patient monitoring

The Research Department of Anaesthetics at the Royal College of Surgeons has been involved in computer-oriented patient monitoring for the last three years. Monitoring, in this context, means that not only should the clinician be able to detect that a dangerous situation has arisen, but that the system should be able to predict that such a situation will arise unless averted by the clinician. For this purpose, signals obtained from the patient must be reliable over long periods and should be relevant to the patient's condition. In our case, such signals are transmitted from the hospital over the public telephone network to the computer, an Elliott 903, where they are digitised and processed and the relevant information transmitted back to a visual display unit in the operating theatre. A number of programs are available, including those to process information concerning cardiac arrhythmias, cardiac output, blood pressure and the frequency distribution of the EEG. The recent installation of a PDP 12 computer has enabled work to be started on the detection of trends in physiological variables and on the response of particular systems of the patient to small stresses. By this means it is hoped to be able to predict when a dangerous situation is about to arise. This, in turn, will enable the correct treatment to be instituted, thus increasing the safety of the patient. Over the past few years the research work carried out jointly in the Department of Anaesthetics in the Royal College of Surgeons and the St Peter's Hospitals has been largely concerned with developing rapid, easily-managed, analytical methods based on micro-sampling, with increasing the effectiveness of sample-handling techniques, especially in relation to computer processing, and with improving methods of patient monitoring with particular reference to cardiovascular and respiratory dynamics.

THE USE OF ANALOGUE TELEPHONE DATA LINKS FOR THE TRANSMISSION OF PHYSIOLOGICAL SIGNALS

SUMMARY A data link, operating on the frequency modulation principle, has been developed to transmit physiological signals in analogue form over the telephone network. The transmitted signals have included e.c.g. and e.e.g. complexes, arterial blood pressure and central venous pressure contours, pneumotachogram and plethysmogram waveforms and dye dilution curves, but virtually every physiological signal is suitable for this type of handling. Over the past two years the data link has been used regularly between St Peter's Hospital, Covent Garden, and the Research Department of Anaesthetics in the Royal College of Surgeons to facilitate computer processing of the physiological signals and up to three signals have been sent simultaneously. The successful use of this three-channel analogue data link combined with digital computer facilities provides a new dimension in clinical research and patient care. In particular the ability to process physiological signals on-line in real time offers the opportunity to improve the accuracy of measurements, to increase the number and range of physiological variables suitable for patient assessment and to develop more dependable methods for clinical evaluation.

Patient Education Leads to Better Care for Heart Patients

AS the nation's economists join leaders in the health field in looking for ways to halt spiraling hospital cost3, one avenue of approach that holds great promise for further exploration is well-organized treatment and education programs for patients. Recognizing that congestive heart failure is an important public health problem, particularly in persons over 60, the staff of the heart and circulatory disease program, New Jersey State Department of Health, initiated a special heart project early in 1964 (1). The project was conducted at St. Peter's General Hospital, New Brunswick, N.J., from 1964 to 1966. In essence, the New Jersey study showed that an education program for 50 patients with congestive heart failure increased the patient's knowledge of his disease, medication, and diet as well as his adherence to a prescribed regimen. When studied against the previous experiences of these patients as well as against the experiences of a control group, readmissions and total readmission days were significantly reduced. It is known that a myocardial infarction may precede the onset of decompensation and congestive heart failure. Other causes may be acute rheumatic carditis, infections, and pulmonary embolus. In many if not most recurrences, none of these rather obvious causes have existed. Other factors that might precipitate an acute bout of congestive heart failure are stress, dietary indiscretions, failure to take medication as prescribed, and failure to follow limitations in activity. Each study patient had a history of congestive heart failure, characterized by recurrent attacks requiring immediate hospitalization, which might be prolonged into a period of weeks. Such recurrences disrupted the patient's life, were costly to him and to his family or to the community, and caused him acute anxiety because of the fear of recurrences, which might result in death.

Diverticulum of the bladder--presentation and evaluation of treatment of 115 cases.

SUMMARY One hundred and fifteen patients with bladder diverticula requiring operative treatment are presented, clinical and pathological findings are described, and the various methods of operative management evaluated. Immediate and later results of treatment are reported and related to the operative methods. We are indebted to the Consultant Surgeons of St Peter's Hospital for the opportunity to investigate their patients and for their kind advice and criticism. Our thanks are also due to Mr R. Hunner's ulcer.

SUMMARYHunner's ulcer is a rare condition; only twenty‐seven definite cases have been seen at St Peter's and St Paul's hospitals in the last ten years. During this period forty‐two patients were classified as having this condition. The average age was 53, and all but two were women. The symptoms common to the whole group were frequency and pain when the bladder was full. and these were temporarily relieved by hydrostatic dilatation of the bladder. The urine was always sterile. The cystoscopic appearances, although insignificant in comparison with the symptoms, were specific and were found only in the free part of the bladder.Treatment is discussed. Conservative therapy, apart from dilatation, was not effective. Ureterosigmoidostomy was performed ten times with two deaths; the surviving patients obtained complete relief. Ileocystoplasty was carried out three times, but its value is difficult to assess. It appears that either the stoma has to be very wide or extensive partial cystectomy has to be performed for relief of symptoms.The differential diagnosis is from tuberculous and other forms of chronic cystitis. and carcinoma of the bladder. Hunner's ulcer is only one form of chronic interstitial cystitis.The letiology is still unknown and the possible causes are discussed. It is not likely to be infective, but may be due to hormonal influences and may be partly psychosomatic. The pathology is quite non‐specific.

Osteitis condensans.

A review of the literature is given. Radiological evidence of osteitis condensans is not infrequent and was found in 2·2 per cent. in a recent review of 760 lumbosacral spines, at St. Peter's Hospital, Botleys Park. The radiological appearances and differential diagnosis are discussed. The macroscopic and microscopic appearances of a piece of bone removed from an osteitis condensans lesion are described, and the aetiology and pathology are discussed. The writers favour, as a cause, the vascular deficiency hypothesis. Reference is made to the clinical significance of the condition. The ultimate conclusion is drawn that osteitis condensans at the pelvic articulations is evidence of an old trauma, frequently associated with disc lesions. If the radiological findings are associated with severe backache and demonstrable physical signs, the cause of the pain should be sought elsewhere.
