Spectral computed tomography in the assessment of metastatic lymph nodes in cervical cancer patients treated with definitive radiotherapy: a single-center, prospective study.

Identifying metastatic lymph nodes (LNs) in patients with cervical cancer treated with definitive radiotherapy may inform treatment strategy and determine prognosis, but available methods have limitations, especially in developing regions. Herein, we aimed to evaluate the performance of quantitative parameters in spectral computed tomography (CT) scanning in this context, focusing on its complementary role alongside conventional diagnostic approaches like 18-fluorine-fuorodeoxyglucose positron emission tomography computed tomography (18F FDG-PET/CT). Patients with cervical cancer, who underwent pretreatment spectral CT simulation scanning and planned radiotherapy, were enrolled in this prospective study. The LNs were categorized as "metastatic" and "non-metastatic", based on a procedure that included 18F FDG-PET/CT as well as CT, magnetic resonance imaging, Node Reporting and Data System and follow-up results. Iodine concentrations (IC), normalized IC (NIC), effective atom number (effZ), and spectral curve slope (λHU) in the arterial (AP) and venous (VP) phases, were compared between metastatic and non-metastatic LNs. IC were derived from iodine-based material decomposition through manual delineation and normalized to the iodine concentration in the adjacent artery (NIC). effZ and λHU were calculated based on the effective atom number image and virtual monochromatic images. Univariate and multivariate logistic regression analyses were used to determine spectral CT factors independently associated with LNs metastasis, and their diagnostic efficacies were assessed using the area under the curve (AUC) analysis. The diagnostic efficiency of 18F FDG-PET/CT and spectral CT was compared. A total of 115 metastatic and 97 non-metastatic LNs were detected, and spectral CT parameters (IC, NIC, effZ, λHU) significantly differed between the two groups. In univariate and multivariable logistic regression analysis, λHU in the AP and NIC in the VP were independent predictors for metastatic LNs and their combination improved AUC to 0.923, with a sensitivity of 84.4%, and a specificity of 85.6%. Spectral CT could achieve similar sensitivity as 18FFDG-PET/CT in total LNs, and, more importantly, a higher sensitivity (95.5% vs. 59.1%) and diagnostic accuracy (92.9% vs. 67.9%) for para-aortic LNs. Quantitative spectral CT parameters can help distinguish metastatic from non-metastatic LNs in patients with cervical cancer treated with definitive radiotherapy. Combination of λHU in AP and NIC in VP further improves diagnostic performance. Spectral CT, while promising, complements rather than replaces PET/CT, especially for diagnosing para-aortic LNs, where PET/CT may have limitations. It could be a valuable adjunct to conventional imaging, particularly in settings with limited access to advanced tools.

Impact of Lumbar Degenerative Changes on Vertebral Bone Strength: A Finite Element Analysis.

Assessing the bone condition in patients with spinal disease is clinically valuable. However, evaluating bone strength in the presence of spine degenerative changes is challenging. Quantitative computed tomography (QCT) and finite element analysis (FEA) have been proposed as methods for more accurate bone quality assessment. This study investigates the relationship between bone strength predicted by FEA and other relevant biological parameters. This retrospective cross-sectional study included 127 patients with spinal disease who underwent preoperative CT scans between 2014 and 2020. Baseline patient characteristics, volumetric bone mineral density (vBMD) measured by QCT, and vertebral bone strength predicted by FEA were collected. The degree of degeneration was evaluated by classifying osteophyte formation, disc height narrowing, vertebral sclerosis, and spondylolisthesis into a grading scale ranging from 0 to 2. Multiple linear regression analysis was conducted to assess the effect of each factor on bone strength predicted by FEA. Of 127 patients, 120 patients (median age was 62 years) were included. The median vBMD and vertebral strength were 114.3 mg/cm3 and 7892.9 N, respectively. After adjusting for age, sex, body mass index, smoking status, diabetes mellitus, vBMD, and degenerative changes, multiple linear regression analysis revealed that sex, vBMD, and degree of degeneration independently increased the vertebral strength measured by FEA. This study suggests that in patients with spinal disease, vertebral bone strength is affected not only by sex and bone mineral density but also by degenerative changes. Thus, bone strength could be predicted more accurately in patients with spinal disease using FEA.

Expiration CT Gas Trapping Measures with Texture-Based Radiomics Improves Association with Lung Function and Lung Function Decline in COPD.

Several methods quantify gas-trapping on expiration computed tomography (CT) images, but they do not consider the spatial relationship of voxels. The objective of this study was to determine if the addition of expiration CT texture-based radiomics features to existing gas-trapping measurements improves model performance for lung function, lung function decline, COPD classification and visual gas-trapping. CanCOLD participants performed spirometry, plethysmography and CT chest imaging at full-inspiration/expiration with radiologist-assessed gas-trapping. Quantitative CT measurements were performed: low attenuation areas≤-856HU (LAA856), ratio of expiratory-to-inspiratory mean lung attenuation (E/I MLA), and difference between expiratory-inspiratory lung volumes between -856 and -950 HU (RVC856-950). Texture-based radiomics analysis generated 95 features; LASSO regression coefficients were summed to create a representative variable (RadScore). Multivariable linear regression models determined associations for baseline RV/TLC, FEV1/FVC, FEV1, FEF25-75, and 6-year ΔFEV1, with established CT gas-trapping and RadScore. Binary logistic regression determined associations for COPD classification and visual gas-trapping. 1111 participants were investigated (n=234 never-smokers, n=325 at-risk, n=314 mild COPD, n=238 moderate-severe COPD). In separate models for baseline RV/TLC, FEV1/FVC, FEV1, and FEF25-75, ΔFEV1, COPD classification and visual gas-trapping, all CT gas-trapping and CT RadScore measurements were independently significant (p<0.05). When CT gas-trapping and CT RadScore were included in the same model, all model performance metrics improved significantly (p<0.05). CT measures extracted from full-expiratory images that quantify the distribution, not just extent, of gas-trapping provide important information related to lung function and lung function decline in COPD. Full-expiratory CT texture-based radiomics improves model performance when used in combination with conventional gas-trapping measurements for lung function and lung function decline, COPD classification and presence of visual gas-trapping.

Centering prayer in the treatment of Post-Traumatic Stress Disorder (PTSD)

Post-traumatic stress disorder (PTSD) is a mental and behavioral disorder that develops from experiencing a traumatic event. A significant proportion of sexual assault survivors develop PTSD. Research indicates that 75% of sexual assault survivors meet the criteria for PTSD one month after the assault. PTSD treatment combines psychotherapy, medication, hypnosis, meditation, and emerging therapies to help individuals manage trauma-related symptoms and regain control over their lives. Nonetheless, despite advances in PTSD treatments, there remains a pressing need for new approaches to address gaps in current care. We recently demonstrated that centering praying (CP) is useful in treating Parkinson’s disease. Objective: To treat female patients complaining of PTSD due to sexual abuse or rape with CP. Methods: We will develop a randomized controlled trial studying five females from 18 to 35 years old who complain of PTSD due to sexual abuse or rape. Five women paired in an age without any neurological, psychiatric, or systemic disorder will be the control group. Patients will be randomly selected blindly according to the DSM-5 criteria by two neurologists. Comorbid major psychiatric disorders, substance abuse, or severe medical conditions will be excluded. Each patient and the control subjects will be studied by computer tomography to exclude any neurological condition. Outcome Measures will be the PTSD Checklist for DSM-5 (PCL-5) and the quantitative computer tomography (QEEGt). Secondary outcomes will be the PHQ-9 (Patient Health Questionnaire-9) to assess depression specifically, Results: Demographic data showed a significant improvement in patients’ clinical condition assessed by PLC-5 and PHQ-9 after CP treatment. QEEGt records demonstrated a stable pattern in all five patients: an increase of theta activity (4–8 Hz) in the left parietal-temporal-occipital areas of PTSD patients. Figure 3 shows a grand average of the five patients, demonstrating an important augmentation of theta activity in this region. Z-values in the QEEGt map show a progressive decrement of the statistically significant Theta increment above our normative data as long as the CP treatment progresses. Conclusion: We conclude that CP appears to offer a distinct and accessible pathway for managing PTSD, promoting emotional regulation, resilience, and a sense of inner peace rooted in spirituality.

Bone mineral density surrounding the screw thread predicts the risk of pedicle screw loosening.

Screw loosening remains a serious complication for patients undergoing pedicle screw fixation surgeries. An accurate risk prediction is significant for prevention of screw loosening through preoperative planning. In this study, we proposed a novel index, namely the bone mineral density surrounding the screw thread (thread BMD), and tested its predictability in screw loosening. 86 screws (18 loosening and 68 non-loosening) from L3-L5 of 20 patients who experienced pedicle screw loosening were analyzed. The preoperative and postoperative quantitative CT scans of the same vertebra were spatially registered and a helix-based approach was developed to extract the thread BMD. BMDs of the vertebral body, the pedicle and the screw trajectory were also measured from the preoperative CT scans. Finite element analysis was conducted to determine pullout strength and tissue failure around the screw. Receiver operating characteristic (ROC) curve analysis was used to assess the performances of all BMD indices and pullout strength in predicting screw loosening. Linear regression was used to examine correlations between different BMD indices and screw pullout strength. The thread BMD had the greatest value of area under the curve (AUC=0.73, p=0.004) compared to vertebral BMD (AUC=0.51, p=0.923), pedicle BMD (AUC=0.56, p=0.474) and trajectory BMD (AUC=0.67, p=0.020). Also, the thread BMD showed a stronger correlation with the pullout strength (r=0.83, p<0.001) than vertebral BMD (r=0.59, p<0.001), pedicle BMD (r=0.65, p<0.001) and trajectory BMD (r=0.60, p<0.001). We developed a novel approach to measure a newly-defined thread BMD, which indicates superior capacities over other BMD indices in predicting pedicle screw loosening.

Prediction of pulmonary function decline in fibrous interstitial lung abnormalities based on quantitative chest CT parameters

BackgroundInterstitial lung abnormalities (ILA) are a proposed imaging concept. Fibrous ILA have a higher risk of progression and death. Clinically, computed tomography (CT) examination is a frequently used and convenient method compared with pulmonary function tests. This study aimed to correlate quantitative CT airway parameters with pulmonary function parameters in patients with fibrous ILA, with the goal of establishing a prediction model for abnormal pulmonary function parameters in patients with fibrous ILA.MethodsNinety-five cases of fibrous ILA including CT images and 64 normal control cases were collected. All patients completed pulmonary function tests within one week. The airway parameters of the CT images of the two groups of cases were measured using a commercial software (Aview). Differences in airway parameters and lung function parameters between the two groups were analyzed by logistic multifactorial regression. The correlation between airway parameters and lung function parameters among 95 patients with fibrous ILA and a prediction model was determined for the decreased percentage forced vital capacity to predicted normal value (FVC%pred) in patients with fibrous ILA.ResultsLogistic multifactorial regression correlated FVC%pred and bronchial wall thickness (WT) were correlated with fibrous ILA. The 95 patients with fibrous ILA were divided into normal FVC%pred (n = 69) and decreased FVC%pred (n = 26) groups at the 80% cut-off. Logistic multifactorial regression revealed that FVC%pred decline in patients with fibrous ILA was effectively predicted by age (odds ratio [OR]: 1.11, 95% confidence interval [CI]: 1.02–1.21, p = 0.014), gender (OR: 4.16,95% CI: 1.27–13.71, p = 0.019), luminal area of the sixth generation brochi (LA6; OR: 0.87, 95%CI: 0.78–0.970,p = 0.015), and airway wall area (WA; OR: 1.12, 95%CI: 1.02–1.24, p = 0.020) were effective predictors of. The area under the curve of the prediction model based on the four parameters was 0.8428.ConclusionWT is a quantitative CT biomarker and FVC%pred is a valid lung function parameter in fibrous ILA patients. Age, gender, LA6, and WA are effective predictors of FVC%pred decline in fibrous ILA patients. The combined model has good predictive value.Clinical trial number2024K249.

Assessing the Effects of Immune Checkpoint Inhibitors on Bone Utilizing Machine Learning-assisted Opportunistic Quantitative Computed Tomography.

Immune checkpoint inhibitors (ICI) are widely used in cancer treatment, yet their impact on bone health remains unclear. This study aimed to perform a retrospective cohort study utilizing routine computed tomography (CT) scans from patients with melanoma to perform opportunistic quantitative CT (QCT) analysis to investigate the effects of ICI treatment on skeletal health, including volumetric bone mineral density (vBMD) measurements and osteoarthritis (OA) parameters. A previously established machine-learning assisted opportunistic QCT pipeline was used to estimate lumbar spine vBMD from baseline and 12-month follow-up CT scans in patients with melanoma treated with ICI therapy and those not treated with ICI therapy. Facet joint OA, osteophyte formation and endplate sclerosis was also graded. Independent and paired t tests were used to determine any differences in vBMD and OA parameters between ICI users and non-ICI users. Multivariable linear regression models were used to control for confounding variables. Non-ICI users had a significant decrease in vBMD of -6.96mg/cm3 from baseline to follow-up, whereas the ICI users had no significant change. There was a significant difference in change in vBMD from baseline to follow-up between the two groups, with the non-ICI users experiencing a 11.22mg/cm3 larger decrease in vBMD. After adjusting for baseline age, sex, baseline vBMD and change in OA parameters, this difference remained significant at -13.04mg/cm3. Among the ICI users, those who had a decline in vBMD had a lower baseline vBMD compared to those who increased vBMD. Neither group showed a significant change in OA parameters over the follow-up period, nor a difference in change between ICI and non-ICI users, even after adjusting for sex, age and baseline OA parameters. While the effects of ICI treatment on vBMD may vary based on baseline bone health, ICIs do not significantly impact OA parameters in the short term.

Evaluation of Bone Mineral Metabolism in Pre-Dialysis Chronic Kidney Disease: Quantitative Computed Tomography vs. Dual-Energy Absorptiometry and Correlation with Bone Turnover Markers.

Background and Objectives: Bone and mineral disease (BMD) is a prevalent complication of advanced chronic kidney disease (CKD). The risk of fractures can be assessed via dual-energy X-ray absorptiometry (DXA) and quantitative computed tomography (QCT). This study aims to evaluate the effectiveness of two imaging modalities in identifying bone mineral status in individuals with pre-dialysis chronic renal disease and to assess their correlation with bone turnover markers. Materials and Methods: This controlled cross-sectional study, conducted at a single center from 2019 to 2022, assessed two groups of individuals aged 18 to 50. The patient cohort consisted of individuals with stage 4-5 chronic kidney disease, whereas the control cohort consisted of healthy participants. The participants' bone and mineral status was evaluated using both QCT and DXA methods. Diagnostic measurements of the lumbar spine and femoral neck, obtained using DXA and QCT, were compared. Z-scores were utilized to evaluate low bone mineral density, with low Z-scores identified in either lumbar spine or femoral neck measures being seen as indicative of low bone mineral density. Results: Data from 38 participants (patient group: 18; control group: 20) who underwent QCT and/or DXA were evaluated. Thirty-three subjects were assessed using both QCT and DXA (patient group: 14; control group: 19). The median age of the patient cohort was 44 (range: 22-50), whereas the median age of the control cohort was 42 (range: 27-48) (p = 0.72). Women constituted 33% of the patient cohort and 50% of the control cohort (p = 0.23). In the patient cohort, low bone mineral density was detected in four individuals (28%) through QCT, and in just two patients (14%) through DXA. Compared to DXA, QCT identified a higher number of cases of low bone mineral density in the CKD cohort; however, no statistically significant difference was observed (p = 0.06). In addition, our study found that TRACP5b had a strong negative correlation with the DXA L1-L4 Z-score. Conclusions: This study revealed that QCT may be more sensitive than DXA for detecting low bone density in pre-dialysis CKD patients. Additionally, DXA may overestimate lumbar spine BMD in this population, and the strong negative correlation between TRACP5b levels and the DXA L1-L4 Z-score highlights the potential role of biochemical markers in assessing bone status in CKD.

Ischemic Stroke Lesion Segmentation on Multiparametric CT Perfusion Maps Using Deep Neural Network

Background: Accurate delineation of lesions in acute ischemic stroke is important for determining the extent of tissue damage and the identification of potentially salvageable brain tissues. Automatic segmentation on CT images is challenging due to the poor contrast-to-noise ratio. Quantitative CT perfusion images improve the estimation of the perfusion deficit regions; however, they are limited by a poor signal-to-noise ratio. The study aims to investigate the potential of deep learning (DL) algorithms for the improved segmentation of ischemic lesions. Methods: This study proposes a novel DL architecture, DenseResU-NetCTPSS, for stroke segmentation using multiparametric CT perfusion images. The proposed network is benchmarked against state-of-the-art DL models. Its performance is assessed using the ISLES-2018 challenge dataset, a widely recognized dataset for stroke segmentation in CT images. The proposed network was evaluated on both training and test datasets. Results: The final optimized network takes three image sequences, namely CT, cerebral blood volume (CBV), and time to max (Tmax), as input to perform segmentation. The network achieved a dice score of 0.65 ± 0.19 and 0.45 ± 0.32 on the training and testing datasets. The model demonstrated a notable improvement over existing state-of-the-art DL models. Conclusions: The optimized model combines CT, CBV, and Tmax images, enabling automatic lesion identification with reasonable accuracy and aiding radiologists in faster, more objective assessments.

Decreased attenuation difference between non-contrast and portal-venous phases of CT predicts the ultrasonography-unspecified adnexal torsion

ObjectivesTo evaluate the value of contrast-enhanced CT in diagnosing ultrasonography-unspecified adnexal torsion (AT).MethodsSurgically confirmed patients with painful pelvic masses (n = 165) were retrospectively collected from two institutes. Two senior radiologists independently reviewed the CT images and determined the Hounsfield unit difference between non-contrast vs portal venous phases (ΔHUPV-NC) in both derivation and validation samples. The cutoff value, sensitivity, specificity, predictivity, and reproducibility of the ΔHUPV-NC and other visually assessed CT signs were analyzed and compared using the receiver-operating characteristic curve, multivariable regression, and inter-rater agreement assays, respectively.ResultsWomen with twisted (n = 73 [47 ± 19 years]) or untwisted (n = 92 [40 ± 15 years]) adnexal lesions were reviewed. The ΔHUPV-NC ≤ 17.5 HU (AUC: 0.91 [95% CI: 0.86, 0.96]; sensitivity: 95% [95% CI: 87, 98]; and specificity: 88% [95% CI: 80, 94]) was the independent predictor of AT (OR: 137 [95% CI: 39, 481], p < 0.001). After training in ΔHUPV-NC measurement, the agreement between two junior residents and the consensus increased from fair (resident-1: 0.29 [95% CI: 0.17, 0.41]; resident-2: 0.24 [95% CI: 0.1, 0.39]) to substantial (resident-1: 0.75 [95% CI: 0.65, 0.85]; resident-2: 0.72 [95% CI: 0.62, 0.83]). GOLD grade-specific characterization of COPD in the COSYCONET multi-center trial: comparison of semiquantitative MRI and quantitative CT.

We hypothesized that semiquantitative visual scoring of lung MRI is suitable for GOLD-grade specific characterization of parenchymal and airway disease in COPD and that MRI scores correlate with quantitative CT (QCT) and pulmonary function test (PFT) parameters. Five hundred ninety-eight subjects from the COSYCONET study (median age = 67 (60-72)) at risk for COPD or with GOLD1-4 underwent PFT, same-day paired inspiratory/expiratory CT, and structural and contrast-enhanced MRI. QCT assessed total lung volume (TLV), emphysema, and air trapping by parametric response mapping (PRMEmph, PRMfSAD) and airway disease by wall percentage (WP). MRI was analyzed using a semiquantitative visual scoring system for parenchymal defects, perfusion defects, and airway abnormalities. Descriptive statistics, Spearman correlations, and ANOVA analyses were performed. TLV, PRMEmph, and MRI scores for parenchymal and perfusion defects were all higher with each GOLD grade, reflecting the extension of emphysema (all p < 0.001). Airway analysis showed the same trends with higher WP and higher MRI large airway disease scores in GOLD3 and lower WP and MRI scores in GOLD4 (p = 0.236 and p < 0.001). Regional heterogeneity was less evident on MRI, while PRMEmph and MRI perfusion defect scores were higher in the upper lobes, and WP and MRI large airway disease scores were higher in the lower lobes. MRI parenchymal and perfusion scores correlated moderately with PRMEmph (r = 0.61 and r = 0.60) and moderately with FEV1/FVC (r = -0.56). Multi-center semiquantitative MRI assessments of parenchymal and airway disease in COPD matched GOLD grade-specific imaging features on QCT and detected regional disease heterogeneity. MRI parenchymal disease scores were correlated with QCT and lung function parameters. Question Do MRI-based scores correlate with QCT and PFT parameters for GOLD-grade specific disease characterization of COPD? Findings MRI can visualize the parenchymal and airway disease features of COPD. Clinical relevance Lung MRI is suitable for GOLD-grade specific disease characterization of COPD and may serve as a radiation-free imaging modality in scientific and clinical settings, given careful consideration of its potential and limitations.

Comparing quantitative imaging biomarker alliance volumetric CT classifications with RECIST response categories.

To assess agreement between CT volumetry change classifications derived from Quantitative Imaging Biomarker Alliance Profile cut-points (ie, QIBA CTvol classifications) and the Response Evaluation Criteria in Solid Tumors (RECIST) categories. Target lesions in lung, liver, and lymph nodes were randomly chosen from patients in 10 historical clinical trials for various cancers, ensuring a balanced representation of lesion types, diameter ranges described in the QIBA Profile, and variations in change magnitudes. Three radiologists independently segmented these lesions at baseline and follow-up scans using 2 software tools. Two types of predefined disagreements were assessed: Type I: substantive disagreement, where the disagreement between QIBA CTvol classifications and RECIST categories could not be attributed to the improved sensitivity of volumetry in detecting changes; and Type II: disagreement potentially arising from the improved sensitivity of volumetry in detecting changes. The proportion of lesions with disagreements between QIBA CTvol and RECIST, as well as the type of disagreements, was reported along with 95% CIs, both overall and within subgroups representing various factors. A total of 2390 measurements from 478 lesions (158 lungs, 170 livers, 150 lymph nodes) in 281 patients were included. QIBA CTvol agreed with RECIST in 66.6% of interpretations. Of the 33.4% of interpretations with discrepancies, substantive disagreement (Type I) occurred in only 1.5% (95% CI: [0.8%, 2.1%]). Factors such as scanner vendor (P = .584), segmentation tool (P = .331), and lesion type (P = .492) were not significant predictors of disagreement. Significantly more disagreements were observed for larger lesions (≥50 mm, as defined in the QIBA Profile). We conclude that QIBA CTvol classifications agree with RECIST categories.

Anti-Osteoporosis Is Imperative in Prevention of Progress of Ankylosing Spondylitis.

Ankylosing spondylitis (AS) is a chronic autoimmune disease that affects the spine and peripheral joints, often leading to kyphosis, joint stiffness, and even ankylosis. Sagittal parameters, such as total thoracic kyphosis (TTK), thoracic kyphosis (TK), major thoracic kyphosis (MTK), and thoracolumbar kyphosis (TLK), are crucial indices for evaluating spinal alignment in AS patients and can reflect disease progression. This study aims to explore the relationship between bone mineral density (BMD), sagittal parameters, and joint ankylosis in AS patients. A retrospective study was conducted on 147 AS patients. Participants were divided into three groups based on cervical and hip joint mobility. BMD was measured using quantitative computed tomography (QCT). Sagittal parameters (TTK, TK, MTK, TLK) were assessed using X-rays. Ordinal multinomial logistic regression and Spearman correlation analyses were performed to identify factors influencing joint stiffness. Significant differences in age, BMD, and sagittal parameters (TTK, TK, MTK, TLK) were observed among the groups. Ordinal logistic revealed that BMD (Estimate = 0.012) was negatively correlated with joint stiffness, while TTK (Estimate = 0.020) and TLK (Estimate = 0.030) were positively correlated. Age, TK, and MTK do not have a significant impact on joint stiffness. Spearman analysis showed no correlation between BMD and sagittal parameters (TTK and TLK). Besides, TTK and TLK were correlated. In AS patients, BMD is an independent protective factor against joint stiffness, whereas sagittal parameters (TTK and TLK) contribute to increased joint stiffness. These findings highlight the importance of monitoring both bone mineral density and key sagittal parameters in clinical practice. Early anti-osteoporosis treatment, alongside interventions targeting abnormal spinal alignment, may help preserve joint mobility and potentially prevent progression to joint ankylosis.

Quantitative measurement of regional brain imaging using computed tomography in canine with suspected stroke

Background and Aim: Brain stroke is increasingly being recognized in veterinary medicine and difficult to diagnosis. Computed tomography (CT) scan is a tool that can help the diagnosis more easily and rapidly. The objective of this study was to determine the clinical characteristics and quantitated CT images in terms of volume and histogram analysis of the ROIs (rostral cerebral artery, middle cerebral artery, caudal cerebral artery, and caudal cerebellar artery) in dogs with suspected stroke. Materials and Methods: Six dogs with suspected stroke and 3 healthy dogs were evaluated. General clinical neurological examinations and CT characteristic of the brain were performed on 9 dogs. CT imaging detected regions of brain change in suspected stroke dogs, observed in several regions including frontal, thalamus, and pituitary. We performed quantitative CT including Hounsfield unit (HU), histogram and volume of interest (VOI) of arteries of interested in the brain. Results: The HU value of the left and right caudal cerebellar artery of suspected stroke dogs were significantly higher than that of control dogs. The histogram of middle cerebral artery of suspected stroke dogs were also significantly higher than that of control dogs. Additionally, the VOI of caudal and cerebellar artery also trend to increase in suspected stroke dogs. Conclusion: our results demonstrated that CT finding abnormalities in several regions of the brain and in suspected stroke dogs, which helps in formulation of a treatment plan and determining prognosis.

Noninvasive Quantitative CT for Diffuse Liver Diseases: Steatosis, Iron Overload, and Fibrosis.

Chronic diffuse liver disease continues to increase in prevalence and represents a global health concern. Noninvasive detection and quantification of hepatic steatosis, iron overload, and fibrosis are critical, especially given the many relative disadvantages and potential risks of invasive liver biopsy. Although MRI techniques have emerged as the preferred reference standard for quantification of liver fat, iron, and fibrosis, CT can play an important role in opportunistic detection of unsuspected disease and is performed at much higher volumes. For hepatic steatosis, noncontrast CT provides a close approximation to MRI-based proton-density fat fraction (PDFF) quantification, with liver attenuation values less than or equal to 40 HU signifying at least moderate steatosis. Liver fat quantification with postcontrast CT is less precise but can generally provide categorical assessment (eg, mild vs moderate steatosis). Noncontrast CT can also trigger appropriate assessment for iron overload when increased parenchymal attenuation values are observed (eg, >75 HU). A variety of morphologic and functional CT features indicate the presence of underlying hepatic fibrosis and cirrhosis. Beyond subjective assessment, quantitative CT methods for staging fibrosis can provide comparable performance to that of elastography. Furthermore, quantitative CT assessment can be performed retrospectively, since prospective techniques are not required. Many of these CT quantitative measures are now fully automated via artificial intelligence (AI) deep learning algorithms. These retrospective and automated advantages have important implications for longitudinal clinical care and research. Ultimately, regardless of the indication for CT, opportunistic detection of steatosis, iron overload, and fibrosis can result in appropriate clinical awareness and management. ©RSNA, 2024 See the invited commentary by Yeh in this issue.

Type 2 diabetes patients exhibit delayed but coupled bone remodelling, maintaining cortical porosity comparable to healthy controls: A histomorphometric analysis of trans-iliac bone biopsies.

Fracture risk is increased in longstanding type 2 diabetes (T2D). High-resolution peripheral quantitative CT scans have demonstrated higher cortical porosity in T2D complicated by microvascular disease (MVD). We investigated if cortical bone resorption is followed by inadequate bone formation in individuals with T2D complicated by MVD. Thirty-five adult men and women with T2D were recruited from outpatient clinics and through public advertisement. All participants had at least one previous measure of c-peptide >700, a negative GAD antibody test, and 13 had known microvascular disease status. Trans iliac crest bone biopsies were collected for histomorphometric analysis. Glucose control was assessed using HbA1c. Additionally, trans iliac bone specimens from 10 individuals without T2D were included as controls. Following quality assessment, samples from 30 T2D and 10 controls were used for histomorphometric analyses of cortical bone remodelling. The final study population included 23 men and 7 postmenopausal women with a mean age of 65.8years for the T2D-MVD group (CI95% 61.2-70.3) and 65.2years in the T2D+MVD group (CI95% 59.6-70.9), and a mean T2D disease duration of 16.9years. Seventeen had MVD (57%). The controls included 5 men and 5 women with a mean age of 64.7years (CI95% 58.5-70.9). The area, diameter, and density of cortical pores were the same in cases with and without MVD, but the pore diameter was lower than controls. While T2D had significantly more eroded-formative pores compared to controls, there were no significant differences in the proportion of eroded and formative pores between the groups. In quiescent pores/osteons, the osteon diameter and wall thickness were larger in T2D groups than controls. Cortical bone porosity was not increased in individuals with T2D complicated by MVD. However, an enhanced prevalence of eroded-formative pores and increased osteon diameter concur with a slightly prolonged reversal-resorption phase in T2D irrespective of the presence of MVD.

Radiomics models based on thoracic and upper lumbar spine in chest LDCT to predict low bone mineral density

This study aims to develop and validate different radiomics models based on thoracic and upper lumbar spine in chest low-dose computed tomography (LDCT) to predict low bone mineral density (BMD) using quantitative computed tomography (QCT) as standard of reference. A total of 905 participants underwent chest LDCT and paired QCT BMD examination were retrospectively included from August 2018 and June 2019. The patients with low BMD (n = 388) and the normal (n = 517) were randomly divided into a training set (n = 622) and a validation set (n = 283). Radiomics features (RFs) were extracted from the single and consecutive vertebrae in chest LDCT images to construct the single vertebra RFs models, mixed RFs models and Radscore models, respectively. The performance of these models was evaluated by the area under the curve (AUC) of receiver operator characteristic curve, using QCT as standard of reference. The Radscore models, mixed RFs models, and single vertebra RFs models yielded the AUC values ranging from 0.809 to 0.906, 0.792 to 0.883, and 0.731 to 0.884 for predicting low BMD in the validation set, respectively. For predicting low BMD, the Radscore model of L1-L2 vertebrae yielded the highest AUC of 0.906, and of T1-T3 yielded the lowest AUC of 0.809 (P < 0.05), respectively. However, there was no significant difference among the AUC values of three Radscore models constructed on the vertebrae of T4-T6 (AUC = 0.855), T7-T9 (AUC = 0.845), and T10-T12 (AUC = 0.871) for predicting low BMD in the validation set (P > 0.1). The Radscore model of L1-L2 have potential to serve as an important tool for predicting and screening low BMD from normal in chest LDCT images.

Individuals with Heterogenous Trabecular Bone Texture by Clinical MRI have Lower Bone Strength and Stiffness by QCT Based Finite Element Analysis.

Opportunistic screening is essential to improve the identification of individuals with osteoporosis. Our group has utilized image texture features to assess bone quality using clinical MRIs. We have previously demonstrated that greater heterogeneity of MRI texture related to history of fragility fractures, lower bone density, and worse microarchitecture. The present study investigated relationships between MRI-based texture features and biomechanical properties of bone using CT-based finite element analyses (FEA). We hypothesized that individuals with greater texture heterogeneity would have lower stiffness and failure load. Thirty individuals included in this prospective study had CT and MRI of L1 and L2 vertebrae. Using T1-weighted MR images, a gray-level co-occurrence matrix was generated to characterize the distribution and spatial organization of voxelar signal intensities to derive the following texture features: contrast (variability), entropy (disorder), angular second moment (ASM; uniformity), and inverse difference moment (IDM; homogeneity). Features were calculated in five directions relative to the image plane. Whole-bone stiffness and failure load were calculated from phantom-calibrated lumbar QCT. Mean age of subjects was 59 ± 11years (57% female). Individuals with lower vertebral stiffness had greater texture heterogeneity; specifically, higher contrast (r = -0.54, P<.01), higher entropy (r = -0.52, P<.01), lower IDM (r = 0.54, P<.01) and lower ASM (r = 0.51, P<.01). Lower vertebral failure load and lower vBMD were similarly associated with greater texture heterogeneity. Relationships were unchanged when using the average of texture in all directions or the vertical direction in isolation. In summary, individuals with more heterogeneous MRI-based trabecular texture had lower stiffness and failure load by FEA, and lower vBMD by central quantitative CT. These results-the first relating MRI-based texture features and biomechanical properties of bone-provide further support that MRI-based texture measurements can be used to opportunistically detect skeletal fragility.
