Do incisor and molar periodontal ligament mechanoreceptors stimulate antral motility to different degrees?

Teeth are not ankylosed to their sockets. A periodontal ligament (PDL) lies between each root and its socket. PDL movement during dental occlusion activates its mechanoreceptors (PDLMs)linked to the vagal motor n. to simulate antral motility (Lorber, Can J Physiol Pharmacol 67:29–35, 2000). 10 fasted adult female rats had three 5 min periods: basal, procedure and post‐proc. The procedure was vertical jaw movements at a rate of 1/s. In five rats, only back teeth contacted as in chewing. In the others the edge of a tongue blade between incisors simulated biting. Molar antral motility values were 3.20 ±1.10 (s.d.), 8.40 ± 3.28, 8.60 ± 1.34. Incisor values were 4.80 ± 1.92, 7.40 ± 4.98, 7.40 ± 3.05. Molar occlusion was significant by the Friedman ANOVA, P = 0.032, but incisor occlusion was not, P = 0.074.

Brain Activations in Response to Vibrotactile Tooth Stimulation: a Psychophysical and fMRI Study

The tactile sensitivity of the teeth, and associated periodontium, serves important sensory and motor functions. Microneurographic recordings from human periodontal ligament mechanoreceptor (PDLM) nerves, in response to tooth loading, reveal discharge patterns with sole slowly adapting (SA) II-type characteristics, highlighting the unique role of PDLMs in oral sensory processes. Here we investigate these receptors' properties, psychophysically and with neuroimaging (fMRI), in response to varying frequencies of dynamic (vibrotactile) stimulation. The finding of increased activity in primary (SI) and secondary (SII) somatosensory cortices (SI and SII) at low frequencies of stimulation (20 Hz) as compared with higher frequencies (50 and 100 Hz), shows an increased entrainment of the PDLMs at this lower frequency in line with expected SA II-type response properties. At the highest frequency (100 Hz), no significant activity was found in SI or SII, suggesting this frequency is outside the range of activity of PDLMs. An activation matrix is mapped that includes SI, SII, insular, inferior frontal gyrus, inferior parietal lobe and supplementary motor area as well as middle frontal gyrus and cerebellum. We compared the responses to tooth stimulation with those produced by identical vibrotactile stimulation of the finger. The results strongly suggest that the PDLMs play a significant role in the specification of the forces used to hold and manipulate food between teeth, and in these respects, the masticatory system appears analogous to fine finger-control mechanisms used during precision manipulation of small objects. Because fMRI reveals activations in posterior insular cortex, we also speculate that PDLMs, and SA II-type receptors in general, may be involved in one aspect of the feeling of body ownership.

Oral mucosal and periodontal ligament mechanoreceptors (PDLMs) affect antral motility differently

Do oral mucosal mechanoreceptors affect antral motility? PDLMs are activated by tooth movements in chewing. These also occur during simulated mastication, activating antral motility (Can J Physiol Pharmacol 78:29, 2000) and gastric emptying (J Dent Res 81:302, 2002). Because a mouthful of food contacts all oral tissues, the antral motility effects of stimulating 8 oral mucosal sites under pentobarbital were sought. 9 groups of 8 adult female rats fasted 18h. Visual observations avoided disturbing the stomach for cannula or transducer attachment. All studies had 3 five minute periods: baseline, procedure, and post‐procedure. The mucosal procedure was 5 min of gentle movement of a teflon rod against: the mandibular gingiva, anterior & posterior tongue & its undersurface, the oral floor, buccal mucosa, & hard & soft palates. PDLMs were activated by opening & closing the jaw so opposing molars contacted each other extremely gently. This simulated mastication was done 30 times in 12 sec with a 3 sec rest. This sequence was repeated 20 times in 5 min. Statistical analysis: ANOVA & Bonferroni. No mucosal site affected antral motility; their ANOVA P values were 0.173 to 0.969. By contrast, PDLM activation had ANOVA P = 0.010. Its 3 period values (mean + s.d.) were 2.88 + 2.47, 5.50 + 4.11, 8.00 + 2.00. Between the first and last values P = 0.003. PDLMs are the sole basis of periodontogastric reflexes. Personal research support.

The Effect of Disclusion Time Reduction on Maximal Clench Muscle Activity Levels

Simultaneous recording of excursive function and muscle activity on 62 MPDS patients demonstrated that reducing prolonged disclusion time (1.4 seconds per excursion) to short disclusion time (0.41 seconds per excursion) created a therapeutic effect such that within one month’s time following treatment, there was an observed increase in the maximal clenching capacity of the masseter and temporalis muscles. This clinical treatment effect appears to be the result of decreased ischemia in these same muscles resultant from minimizing the time posterior teeth compress their periodontal ligament mechanoreceptors as these teeth are engaged and disengaged during excursive function.

Mesencephalic trigeminal nucleus (MTN) neurons innervate the stretch receptors of the jaw elevator muscles and periodontal ligament mechanoreceptors, Bruxism activates the MTN. We analyzed how MTN cells are structured, their anatomy and physiology, and the effects of their activation.To induce and maintain sleep, gamma-aminobutyric acid (GABA), an inhibitor neurotransmitter, is released from the ventro-lateral preoptic area of the hypothalamus and acts on the ascending reticular activating system (ARAS) nuclei. The GABA neurotrasmitter induces the entry of chlorine into cells, hyperpolarizing and inhibiting these. MTN cells, on the contrary, are depolarized by GABA, as their receptors are activated upon GABA binding. They “let out” chlorine and activate ARAS cells. MTN cells release glutamate, an excitatory neurotransmitter onto their target cells, in this case onto ARAS cells. During wakefulness, ARAS activation causes cerebral cortex activation; instead, during sleep (sleep bruxism), ARAS activation avoids an excessive reduction in ARAS neurotransmitters, including noradrenaline, dopamine, serotonin, acetylcholine and glutamate. These neurotransmitters, in addition to activating the cerebral cortex, modulate vital functions such as cardiac and respiratory functions. Polysomnography shows that sleep bruxism is always accompanied by cardiac and respiratory activation and, most importantly, by brain function activation. Bruxism is not a parafunction, and it functions to activate ARAS nuclei.

Immunolocalization of corticosteroid hormone receptors in the mechanoreceptors in rat oral tissues

Previous studies have confirmed the presence of corticosteroid receptors including mineralocorticoid (MCR) and glucocorticoid receptors (GCR) in the glia cells of both central and peripheral nervous systems. However, no report has been offered as yet on the detailed localization of these receptors in the specialized Schwann cells associated with mechanoreceptors. Thus, the present study examined the immunolocalization of MCR and GCR in the specialized Schwann cells (terminal and lamellar Schwann cells) associated with the mechanoreceptors in rat periodontal ligament and palatal mucosa by the use of immunohistochemical techniques and confocal laser scanning microscopy. It further attempted to detect the localization of 11β-hydroxysteroid dehydrogenase type 2 (11β-HSD II), a key enzyme for signal transduction via MCR. Immunostaining with antisera against GCR and MCR demonstrated intense immunoreactions in the terminal or lamellar Schwann cells but not nerve fibers associated with the periodontal and palatal mechanoreceptors. A double staining with propidium iodide and either GCR- or MCR-antiserum revealed the intranuclear and cytoplasmic localization of these receptors in the terminal Schwann cells. 11β-HSD II-immunoreactivity was found in the cytoplasm and on the nuclear envelope of the terminal Schwann cells, indicating the co-localization of MCR and 11β-HSD II in them. These findings suggest that the terminal or lamellar Schwann cells are target tissues for corticosteroid hormones. Taken together with previous reports on the functional significance of the corticosteroids, the results indicate that glucocorticoids and mineralocorticoids might be respectively involved in the proliferation and/or differentiation and Na+/K+-homeostasis in the terminal or lamellar Schwann cells associated with the mechanoreceptors.

A new method for evaluating the threshold of periodontal ligament mechanoreceptor by slow speed mechanical stimulation.

This study aimed to establish a reliable method for detecting the threshold for perception of force applied to tooth and to report the basic properties of force threshold in the normal dentition subjects. The perception of mechanical stimulation exerted to tooth is formed by input from periodontal ligament mechanoreceptors and intradental mechanoreceptors. Periodontal ligament mechanoreceptors respond to a wide range speed of stimulus, whereas intradental mechanoreceptors are activated only by a rapid stimulation. Reliable properties of perception have not been reported because of the difficulties to regulate velocity and degree of the stimulus. Eighteen healthy subjects were observed in this study (mean age: 27.2 +/- 5.7 years). A loading device to generate slow speed loading was fabricated and measured as follows: (i) the fluctuation of the force threshold within a day, (ii) day-to-day fluctuation, (iii) changes in the force threshold by a transient mechanical loading. In the normal dentition, it was observed that the fluctuation within a day and day to day of the pressure sense showed no significant differences; however, a transient mechanical loading caused the sensitivity of the periodontal ligament to decrease significantly. The device examining the force threshold resulted from periodontal ligament mechanoreceptors was useful in evaluating the sensitivity of the periodontal ligament.

Quantitative ultrastructure of synapses on functionally identified primary afferent neurons in the cat trigeminal mesencephalic nucleus.

Though a number of studies have reported the presence of synapses on neurons in the trigeminal mesencephalic nucleus (Vmes), there have been no quantitative studies of either the density of innervation, or the ultrastructure, of the synapses on single, physiologically identified neurons in this nucleus. In this study we recorded from single neurons in the Vmes, identified them as being either muscle spindle afferents (MS) or periodontal ligament mechanoreceptor afferents (PL), and then labeled the neurons by intra-axonal injection of horseradish peroxidase (HRP). The material was first processed to reveal the HRP activity, following which ultrathin sections through the labeled somata were cut and examined under the electron microscope. Complete serial reconstructions were made through the soma of one MS neuron and one PL neuron, and the contacts on the neurons reconstructed. Boutons were found on the soma, spines, appendages and the axon hillock and the initial segment of the axon. The numbers of boutons terminating on the two neurons were 198 (PL) and 424 (MS), giving a packing density of 4.4 and 10.7 boutons respectively (i.e., number of boutons/100 micron 2 of the postsynaptic membrane). Boutons could be separated into two types on the basis of their vesicles: those containing clear, round vesicles (i.e., S-type) and those containing a mixture of round, oval and flattened vesicles (P-type). Ninety-five (PL neuron) and 99% (MS neuron) of terminals on the two neurons were P-type. All the S-type boutons and 80% of the P-type boutons formed asymmetric synaptic contacts while 10% of the P-type boutons made symmetric contacts. Quantitative measurements of the P-type boutons on the labeled neurons, in which the data of MS and PL neurons were pooled, revealed that bouton volume was highly correlated with bouton surface area, active zone number, total active zone area, vesicle number, and mitochondrial volume. However, comparing the quantitative measurements of the P-type boutons with those of previously reported vibrissa afferent terminals and their associated axon terminals revealed that all the parameters were smaller for the P-type boutons (on Vmes neurons) than those of the vibrissa afferent terminals but similar to those of axon terminals presynaptic to the vibrissa afferents. Taken together, our results emphasize the wide scope for synaptic interactions in the Vmes and suggest that it may be more fruitful to view the Vmes as an integrating center.

Controlled mechanical stimuli were applied to an upper central incisor tooth in 19 human subjects and the resulting reflexes in the ipsilateral masseter muscle were examined electromyographically. In most cases the force profile of the stimuli consisted of a ramp leading to a sustained plateau at an intensity of 1 N. In addition 1 N tap stimuli were employed in some subjects. The 1 N ramp stimuli with a rise time of ≤ 20 ms consistently evoked a single, short-latency (approximately 12 ms), inhibitory reflex which was often followed by an excitatory wave. The ramp stimuli with shorter rise times produced larger responses than those produced by ramp stimuli with longer rise times. By contrast the tap stimuli elicited a sequence of responses consisting of inhibitory-excitatory-inhibitory-excitatory components. The first inhibitory and excitatory responses evoked by tap stimuli had latencies similar to those of the responses evoked by the ramp stimuli. The latencies of the inhibitory responses evoked by 1 N ramps with rise times ranging between 2.5 and 20 ms did not vary significantly with the rise time. Consistent with this observation it was found that the median threshold force for evoking the short-latency inhibitory reflex was only 0.25 N. This was significantly less than the threshold for the excitatory response (median: 0.75 N). The responses to ramp-plateau forces were not dependent on the level of preloading of the tooth (at least within the range tested: < 0.25 to 1 N). These findings provide evidence that mechanoreceptors in the periodontal ligament contribute to the control of human jaw-closing muscles, notably to short-latency reflex responses. It may be concluded that the additional reflex responses produced by tapping stimuli result from the activation of receptors elsewhere due to vibration.

A possible explanation for the response characteristics of multi-tooth periodontal ligament mechanoreceptors in the cat.

During the course of a study on the morphology of periodontal ligament mechanoreceptors it was observed that a direct relation, without intervening bone, existed between the mandibular canine and first premolar tooth roots in the cat. An area, representing a window in the alveolar septal bone, extended 2-3 mm from the apex towards the tooth crown. Ruffini nerve terminals were observed amongst the collagen bundles in the ligament between the roots of the two teeth. Light and electron microscopy were used to identify the receptors. It is proposed that a periodontal ligament mechanoreceptor can respond to forces applied to adjacent teeth; movement of both teeth need not occur. This may explain the observation made in the past that single periodontal ligament mechanoreceptors respond to forces applied to more than one tooth.

A comparative physiological and morphological study of periodontal ligament mechanoreceptors represented in the trigeminal ganglion and the mesencephalic nucleus of the cat

A correlative morphological study was carried out on two electrophysiologically identified and located periodontal ligament mechanoreceptors in anaesthetised cats. One mechanoreceptor had its cell body in the mesencephalic nucleus and the other had its cell body in the trigeminal ganglion. Physiological recordings were made from each of their cell bodies. The two receptors were located by punctate and electrical stimuli in the labial aspect of the periodontal ligament of the left mandibular canine tooth. Both receptors had similar positions relative to the tooth apex and fulcrum and were situated in the labial part of the ligament in each tooth. The receptor loci were marked, and these regions were studied in a series of semi-thin and ultra-thin sections. Only Ruffini nerve endings were observed under each ink mark. Both Ruffini nerve endings branched, were unencapsulated and were incompletely surrounded by terminal Schwann cells with extensions projecting towards collagen bundles. The results indicate that periodontal ligament mechanoreceptors with cell bodies in the mesencephalic nucleus and those with their cell bodies in the trigeminal ganglion can both be Ruffini nerve endings. Furthermore, there was no apparent morphological difference between the two periodontal ligament mechanoreceptors.

The effect of temperature on the discharge of periodontal ligament mechanoreceptors in the cat canine tooth.

The response of periodontal ligament mechanoreceptors to mechanical stimuli was studied at different temperatures in anaesthetized cats. Recordings were made from functionally single fibres in the inferior alveolar nerve from mechanoreceptors which were located in the ligament by dissection and punctate stimulation and electrical stimulation. A thermode was used to alter and maintain small areas of periodontal ligament at temperatures ranging from thermode tip temperatures of 3 degrees C to 57 degrees C. The response to ramp-plateau mechanical stimuli applied to the tip of the crown of the canine tooth was greatest when the thermode temperature was 37.5 degrees C +/- 6.5 degrees C. The effect was similar for all receptors studied.

Static and dynamic responses of periodontal ligament mechanoreceptors and intradental mechanoreceptors.

1. The response properties of 39 periodontal ligament mechanoreceptors (PDLMs) and 12 intradental mechanoreceptors (IMs) related to the intact mandibular canine tooth were isolated by extracellular recording methods from the ipsilateral trigeminal semilunar ganglion. 2. The stimulus threshold and response magnitude of individual PDLMs depended on the direction of steady force applied to the intact canine tooth. Canine PDLMs as a population, however, did not have a preferred stimulus direction. IMs were activated only by a rapid mechanical transient applied to the intact tooth in any direction. The stimulus threshold and response magnitude of each IM were approximately equipotent in all stimulus directions. 3. Application of quantifiable ramp-and-hold stimulation showed that PDLMs can encode the intensity of steady forces as well as the rate of force ramps. Increasing the ramp rates decreased the total ramp discharge but increased the peak discharge frequency. IMs encoded only the rate of force ramps that were applied by percussion. Higher ramp rates increased both the total discharges and peak discharge frequency of IMs. 4. The dynamic response properties of PDLMs and IMs were clearly differentiated by sinusoidal vibratory stimulation. The maximum frequencies for entrainment of IM discharge at the stimulus cycle length (251 +/- 103 Hz, mean +/- SD) and at any periodicity including multiples of the stimulus cycle length (295 +/- 100 Hz) were significantly higher than the maximum frequencies for PDLM discharge entrainment at the stimulus cycle length (103 +/- 53 Hz) and at any periodicity (133 +/- 62 Hz). 5. The functional similarities of PDLMs and IMs, respectively, to slowly adapting type II mechanoreceptors and Pacinian corpuscle receptors in the skin are discussed. Our present findings, which complement earlier anatomic and behavioral evidence, strongly suggest that IMs subserve nonnociceptive and nonpain functions. Both PDLMs and IMs may provide a continuum of dynamic afferent inputs necessary for tactile sensibility of teeth.

The effect of tooth extraction on periodontal ligament mechanoreceptors represented in the mesencephalic nucleus of the cat

When a force is applied to a tooth, mechanoreceptors in the periodontal ligament are stimulated. When teeth are extracted the remnants of the periodontal ligament break down and disappear, but it is not known what happens to the mechanoreceptor neurones that innervated it. The present study seeks to determine the effect of tooth extraction on the population of periodontal ligament mechanoreceptor neurones represented in the mesencephalic nucleus of the fifth cranial nerve. The incisor and canine teeth were extracted from adult cats; terminal experiments were performed between 7.5 months and 2 yr later. Recordings were made in the mesencephalic nucleus with microelectrodes, and neurones were identified in the inferior alveolar nerve that previously innervated the periodontal ligament of one of the extracted mandibular teeth. The majority of these neurones responded only to electrical stimuli applied to the edentulous ridge of the mandible in the area where the incisor or canine teeth had previously been. It was not possible to stimulate them mechanically, despite the use of large forces. A small number had reinnervated new soft-tissue sites. They could be mechanically stimulated and were found adjacent to the area in which the mandibular incisor and canine teeth had been. Thus the population of periodontal ligament mechanoreceptor neurones represented in the mesencephalic nucleus do not all degenerate after tooth extraction. As the majority of those still present do not appear to reinnervate new tissues in which they can be mechanically stimulated, it is unlikely that they have any functional role after tooth loss.

The effect of vibration on the discharge of periodontal ligament mechanoreceptors to controlled loading of the cat canine tooth

This study observes the maximum 1:1 following frequencies of these mechanoreceptors, as well as their response characteristics before and after vibratory stimuli. Mechanical stimuli were applied to the tip of the crown of the left mandibular canine tooth in cats anaesthetized with alpha-chloralose while recordings were made from functionally single fibres teased from the inferior alveolar nerve. Vibratory stimuli were applied at various frequencies and durations at twice the threshold to that stimulus. Controlled ramp-plateau forces were applied before and after the period of vibration. A temporary rise in the receptor discharge was recorded and the threshold to ramp-plateau stimuli fell to about one-third of its original value. The increased post-vibration response was dependent on the frequency and duration of the stimulus. It is suggested that vibration has some temporary effect on the receptor environment.

The effect of rate of force application on the threshold of periodontal ligament mechanoreceptors in the cat canine tooth

Mechanical stimuli in the form of ramp-plateau forces were applied to the tip of the crown of the left mandibular canine tooth in cats anaesthetized with alpha-chloralose. Electrophysiological recordings were made from functionally single fibres teased from the inferior alveolar nerve. The force threshold was determined for 34 periodontal ligament mechanoreceptors at different controlled rates of force application. Force threshold was dependent on the rate of force application to the crown tip. Rate sensitivity was present for all receptors across the range from rapidly to slowly adapting; the degree of rate sensitivity was graded according to the adaptation rate of the receptor. The results suggest that the velocity of mechanical stimulus application to teeth needs to be considered in studies involving periodontal mechanoreceptor responses and their reflexes.

Mesencephalic trigeminal sensory neurons of cat: axon pathways and structure of mechanoreceptive endings in periodontal ligament.

We injected 3H-proline into cat brainstem in order to label the entire mesencephalic trigeminal nucleus (Mes-V) for autoradiographic analysis of the size and pathways of Mes-V sensory axons and for microscopic study of Mes-V receptor structure in dental tissue. Labeled sensory axons were found in the trigeminal motor and sensory tracts and roots; approximately equal numbers of axons were found in both roots. The sensory root and all three divisions of the trigeminal nerve contained larger Mes-V axons than the motor root. Labeled Mes-V axons were found at the ganglion in the dorsomedial (infratrochlear) branch of the ophthalmic nerve but not in the ventrolateral branch. The mean diameter of Mes-V axons in periodontal ligament was 4.0 +/- 1.9 micron compared to 7.3 +/- 2.1 micron in maxillary and mandibular nerve, suggesting axonal arborization prior to innervation of ligament. Mes-V receptors in dental tissue were confined to ipsilateral periodontal ligament close to the root apex, with greater innervation on the posterior side. Receptor incidence was moderate for most teeth; however, maxillary first and second incisors and maxillary and mandibular canines had focal areas with remarkably dense innervation. No labeled axons were found in pulp of any ipsilateral teeth, and none was found in any contralateral dental tissue. EM-autoradiography demonstrated that Mes-V axons form unencapsulated Ruffini-like mechanoreceptors in periodontal ligament. The preterminal axons were small and myelinated. Neighboring bundles of unmyelinated axons and rare encapsulated endings were not labeled. The labeled mechanoreceptors branched to varying degrees among the ligament fibers; they contained numerous mitochondria and glycogen particles, as well as some vesicles and rare multivesicular bodies. They were surrounded by special Schwann cells that formed one or several layers around the ending. The endings were exposed to the basal lamina at numerous sites and occasionally extended fingers beyond the lamellar Schwann cells to contact ligament collagen.

The distribution of mechanoreceptors in the periodontal ligament of the mandibular canine tooth of the cat.

1. Periodontal mechanoreceptor activity has been recorded from fibres in the inferior alveolar nerve. 2. A method has been developed for punctate and electrical stimulation of the periodontal ligament mechanoreceptors through a thin layer of bone overlying the labial aspect of the left mandibular canine tooth root. 3. The distribution of periodontal mechanoreceptors in the labial aspect of the left mandibular canine tooth has been described. 4. All receptors located responded maximally when that part of the ligament in which they lay was in tension. 5. It is suggested that there may be only one type of mechanoreceptor and that the rate of adaptation is dependent on the location of the receptor within the periodontal tissues.
