Clinical efficacy of motion-insensitive imaging technique with deep learning reconstruction to improve image quality in cervical spine MR imaging.

To demonstrate that a T2 periodically rotated overlapping parallel lines with enhanced reconstruction (PROPELLER) technique using deep learning reconstruction (DLR) will provide better image quality and decrease image noise. From December 2020 to March 2021, 35 patients examined cervical spine MRI were included in this study. Four sets of images including fast spin echo (FSE), original PROPELLER, PROPELLER DLR50%, and DLR75% were quantitatively and qualitatively reviewed. We calculated the signal-to-noise ratio (SNR) of the spinal cord and sternocleidomastoid (SCM) muscle and the contrast-to-noise ratio (CNR) of the spinal cord by applying region-of-interest at the spinal cord, SCM muscle, and background air. We evaluated image noise with regard to the spinal cord, SCM, and back muscles at each level from C2-3 to C6-7 in the 4 sets. At all disc levels, the mean SNR values for the spinal cord and SCM muscles were significantly higher in PROPELLER DLR50% and DLR75% compared to FSE and original PROPELLER images (P < .0083). The mean CNR values of the spinal cord were significantly higher in PROPELLER DLR50% and DLR75% compared to FSE at the C3-4 and 4-5 levels and PROPELLER DLR75% compared to FSE at the C6-7 level (P < .0083). Qualitative analysis of image noise on the spinal cord, SCM, and back muscles showed that PROPELLER DLR50% and PROPELLER DLR75% images showed a significant denoising effect compared to the FSE and original PROPELLER images. The combination of PROPELLER and DLR improved image quality with a high SNR and CNR and reduced noise. Motion-insensitive imaging technique (PROPELLER) increased the image quality compared to conventional FSE images. PROPELLER technique with a DLR reduced image noise and improved image quality.

Ultra-High-Resolution T2-Weighted PROPELLER MRI of the Rectum With Deep Learning Reconstruction: Assessment of Image Quality and Diagnostic Performance.

The aim of this study was to evaluate the impact of ultra-high-resolution acquisition and deep learning reconstruction (DLR) on the image quality and diagnostic performance of T2-weighted periodically rotated overlapping parallel lines with enhanced reconstruction (PROPELLER) imaging of the rectum. This prospective study included 34 patients who underwent magnetic resonance imaging (MRI) for initial staging or restaging of rectal tumors. The following 4 types of oblique axial PROPELLER images perpendicular to the tumor were obtained: a standard 3-mm slice thickness with conventional reconstruction (3-CR) and DLR (3-DLR), and 1.2-mm slice thickness with CR (1.2-CR) and DLR (1.2-DLR). Three radiologists independently evaluated the image quality and tumor extent by using a 5-point scoring system. Diagnostic accuracy was evaluated in 22 patients with rectal cancer who underwent surgery after MRI without additional neoadjuvant therapy (median interval between MRI and surgery, 22 days). The signal-to-noise ratio and tissue contrast were measured on the 4 types of PROPELLER imaging. 1.2-DLR imaging showed the best sharpness, overall image quality, and rectal and lesion conspicuity for all readers (P < 0.01). Of the assigned scores for tumor extent, extramural venous invasion (EMVI) scores showed moderate agreement across the 4 types of PROPELLER sequences in all readers (intraclass correlation coefficient, 0.60-0.71). Compared with 3-CR imaging, the number of cases with MRI-detected extramural tumor spread was significantly higher with 1.2-DLR imaging (19.0 ± 2.9 vs 23.3 ± 0.9, P = 0.03), and the number of cases with MRI-detected EMVI was significantly increased with 1.2-CR, 3-DLR, and 1.2-DLR imaging (8.0 ± 0.0 vs 9.7 ± 0.5, 11.0 ± 2.2, and 12.3 ± 1.7, respectively; P = 0.02). For the diagnosis of histopathologic extramural tumor spread, 3-CR and 1.2-CR had significantly higher specificity than 3-DLR and 1.2-DLR imaging (0.75 and 0.78 vs 0.64 and 0.58, respectively; P = 0.02), and only 1.2-CR had significantly higher accuracy than 3-CR imaging (0.83 vs 0.79, P = 0.01). The accuracy of MRI-detected EMVI with reference to pathological EMVI was significantly lower for 3-CR and 3-DLR compared with 1.2-CR (0.77 and 0.74 vs 0.85, respectively; P < 0.01), and was not significantly different between 1.2-CR and 1.2-DLR (0.85 vs 0.80). Using any pathological venous invasion as the reference standard, the accuracy of MRI-detected EMVI was significantly the highest with 1.2-DLR, followed by 1.2-CR, 3-CR, and 3-DLR (0.71 vs 0.67 vs 0.59 vs 0.56, respectively; P < 0.01). The signal-to-noise ratio was significantly highest with 3-DLR imaging (P < 0.05). There were no significant differences in tumor-to-muscle contrast between the 4 types of PROPELLER imaging. Ultra-high-resolution PROPELLER T2-weighted imaging of the rectum combined with DLR improved image quality, increased the number of cases with MRI-detected extramural tumor spread and EMVI, but did not improve diagnostic accuracy with respect to pathology in rectal cancer, possibly because of false-positive MRI findings or false-negative pathologic findings.

Suppressing image blurring of PROPELLER MRI via untrained method

Objective. Periodically rotated overlapping parallel lines with enhanced reconstruction (PROPELLER) used in magnetic resonance imaging (MRI) is inherently insensitive to motion artifacts but with an expense of around 60% increase in minimum scan time. An untrained deep learning method is proposed to accelerate PROPELLER MRI while suppressing image blurring. Approach. Several reconstruction methods have been developed to accelerate PROPELLER with reduced sampling on blades. However, image quality is degraded due to blurring. Deep learning has been applied to enhance MRI reconstruction quality, and external training data are therefore needed. In addition, the distribution shift problem in deep learning also exists between the external training data and to-be-reconstructed target blade data. This paper introduces an untrained neural network (UNN) to suppress image blurring, which is applied to improve PROPELLER MRI. This network structure was then incorporated into blade k-space. Results. The untrained method improved the blade image quality from brain MRI data. Furthermore, it enhanced the sharpness of the reconstructed image compared to PROPELLER reconstructions using parallel imaging methods and supervised learning methods using external training data. PROPELLER blade acquisition was accelerated by undersampling data with reduction factors 2, 3 and 4. Significance. The reported UNN enhanced PROPELLER method can improve image quality by suppressing blurring. External training data are not needed to mitigate the challenge of collecting high-quality clinical data for training without affecting clinical workflow and the standard care for patients.

Feasibility of online radial magnetic resonance imaging for adaptive radiotherapy of pancreatic tumors.

Online adaptive magnetic resonance (MR)-guided treatment planning for pancreatic tumors on 1.5T systems typically employs Cartesian 3D T 2w magnetic resonance imaging (MRI). The main disadvantage of this sequence is that respiratory motion results in substantial blurring in the abdomen, which can hamper delineation accuracy. This study investigated the use of two motion-robust radial MRI sequences as main delineation scan for pancreatic MR-guided radiotherapy. Twelve patients with pancreatic tumors were imaged with a 3D T 2w scan, a Periodically Rotated Overlapping ParallEL Lines with Enhanced Reconstruction (PROPELLER) scan (partially overlapping strips), and a 3D Vane scan (stack-of-stars), on a 1.5T MR-Linac under abdominal compression. The scans were assessed by three radiation oncologists for their suitability for online adaptive delineation. A quantitative comparison was made for gradient entropy and the effect of motion on apparent target position. The PROPELLER scans were selected as first preference in 56% of the cases, the 3D T 2w in 42% and the 3D Vane in 3%. PROPELLER scans sometimes contained a large interslice variation which would have compromised delineation. Gradient entropy was significantly higher in 3D T 2w patient scans. The apparent target position was more sensitive to motion amplitude in the PROPELLER scans, but substantial offsets did not occur under 10mm peak-to-peak. PROPELLER MRI may be a superior imaging sequence for pancreatic MRgRT compared to standard Cartesian sequences. The large interslice variation should be mitigated through further sequence optimization before PROPELLER can be adopted for online treatment adaptation.

Comparison of deep learning-based reconstruction of PROPELLER Shoulder MRI with conventional reconstruction.

To compare the image quality and agreement among conventional and accelerated periodically rotated overlapping parallel lines with enhanced reconstruction (PROPELLER) MRI with both conventional reconstruction (CR) and deep learning-based reconstruction (DLR) methods for evaluation of shoulder. We included patients who underwent conventional (acquisition time, 8min) and accelerated (acquisition time, 4min and 24s; 45% reduction) PROPELLER shoulder MRI using both CR and DLR methods between February 2021 and February 2022 on a 3T MRI system. Quantitative evaluation was performed by calculating the signal-to-noise ratio (SNR). Two musculoskeletal radiologists compared the image quality using conventional sequence with CR as the reference standard. Interobserver agreement between image sets for evaluating shoulder was analyzed using weighted/unweighted kappa statistics. Ninety-two patients with 100 shoulder MRI scans were included. Conventional sequence with DLR had the highest SNR (P < .001), followed by accelerated sequence with DLR, conventional sequence with CR, and accelerated sequence with CR. Comparison of image quality by both readers revealed that conventional sequence with DLR (P = .003 and P < .001) and accelerated sequence with DLR (P = .016 and P < .001) had better image quality than the conventional sequence with CR. Interobserver agreement was substantial to almost perfect for detecting shoulder abnormalities (κ = 0.600-0.884). Agreement between the image sets was substantial to almost perfect (κ = 0.691-1). Accelerated PROPELLER with DLR showed even better image quality than conventional PROPELLER with CR and interobserver agreement for shoulder pathologies comparable to that of conventional PROPELLER with CR, despite the shorter scan time.

High Resolution MR Imaging of the Testis Using a Small Radiofrequency Coil.

We have developed a new device, consisting of a 3-cm RF coil and an immobilizer, to acquire high-resolution MR images of the testis. With the approval of our institutional review board, we conducted an MRI study on a cohort of healthy volunteers to test this device. With the participants in the supine position, we placed the dedicated immobilizer and RF coil on the scrotum for typically no more than 3 min. Subsequently, T2-weighted images were acquired with an in-plane resolution of 117 µm using a 3-T MR scanner and the periodically rotated overlapping parallel lines with enhanced reconstruction (PROPELLER) sequence. The total scan time ranged from 12 to 30 min (average 20 min). High-resolution MR images of the testis were acquired without deterioration by motion artifacts. Our results showed that the combined use of a small RF coil and an immobilizer is a feasible option for acquiring high-resolution MR images of the testis.

State-of-the-art magnetic resonance imaging sequences for pediatric body imaging.

Longer examination time, need for anesthesia in smaller children and the inability of most children to hold their breath are major limitations of MRI in pediatric body imaging. Fortunately, with technical advances, many new and upcoming MRI sequences are overcoming these limitations. Advances in data acquisition and k-space sampling methods have enabled sequences with improved temporal and spatial resolution, and minimal artifacts. Sequences to minimize movement artifacts mainly utilize radial k-space filling, and examples include the stack-of-stars method for T1-weighted imaging and the periodically rotated overlapping parallel lines with enhanced reconstruction (PROPELLER)/BLADE method for T2-weighted imaging. Similarly, the sequences with improved temporal resolution and the ability to obtain multiple phases in a single breath-hold in dynamic imaging mainly use some form of partial k-space filling method. New sequences use a variable combination of data sampling methods like compressed sensing, golden-angle radial k-space filling, parallel imaging and partial k-space filling to achieve free-breathing, faster sequences that could be useful for pediatric abdominal and thoracic imaging. Simultaneous multi-slice method has improved diffusion-weighted imaging (DWI) with reduction in scan time and artifacts. In this review, we provide an overview of data sampling methods like parallel imaging, compressed sensing, radial k-space sampling, partial k-space sampling and simultaneous multi-slice. This is followed by newer available and upcoming sequences for T1-, T2- and DWI based on these other advances. We also discuss the Dixon method and newer approaches to reducing metal artifacts.

Performance of PROPELLER FSE T2WI in reducing metal artifacts of material porcelain fused to metal crown: a clinical preliminary study

This study aimed to compare MRI quality between conventional fast spin echo T2 weighted imaging (FSE T2WI) with periodically rotated overlapping parallel lines with enhanced reconstruction (PROPELLER) FSE T2WI for patients with various porcelain fused to metal (PFM) crown and analyze the value of PROPELLER technique in reducing metal artifacts. Conventional FSE T2WI and PROPELLER FSE T2WI sequences for axial imaging of head were applied in participants with different PFM crowns: cobalt-chromium (Co–Cr) alloy, pure titanium (Ti), gold–palladium (Au–Pd) alloy. Two radiologists evaluated overall image quality of section in PFM using a 5-point scale qualitatively and measured the maximum artifact area and artifact signal-to-noise ratio (SNR) quantitatively. Fifty-nine participants were evaluated. The metal crown with the least artifacts and the optimum image quality shown in conventional FSE T2WI and PROPELLER FSE T2WI were in Au–Pd alloy, Ti, and Co–Cr alloy order. PROPELLER FSE T2WI was superior to conventional FSE T2WI in improving image quality and reducing artifact area for Co-Cr alloy (17.0 ± 0.2% smaller artifact area, p < 0.001) and Ti (11.6 ± 0.7% smaller artifact area, p = 0.005), but had similar performance compared to FSE T2WI for Au–Pd alloy. The SNRs of the tongue and masseter muscle were significantly higher on PROPELLER FSE T2WI compared with conventional FSE T2WI (tongue: 29.76 ± 8.45 vs. 21.54 ± 9.31, p = 0.007; masseter muscle: 19.11 ± 8.24 vs. 15.26 ± 6.08, p = 0.016). Therefore, the different PFM crown generate varying degrees of metal artifacts in MRI, and the PROPELLER can effectively reduce metal artifacts especially in the PFM crown of Co-Cr alloy.

Impact of Deep Learning Reconstruction Combined With a Sharpening Filter on Single-Shot Fast Spin-Echo T2-Weighted Magnetic Resonance Imaging of the Uterus.

This study aimed to evaluate the effects of deep learning (DL) reconstruction and a postprocessing sharpening filter on the image quality of single-shot fast spin-echo (SSFSE) T2-weighted imaging (T2WI) of the uterus. Fifty consecutive patients who underwent pelvic magnetic resonance imaging were included. Parasagittal T2WI with a slice thickness of 4 mm was obtained with the periodically rotated overlapping parallel lines with enhanced reconstruction (PROPELLER) and SSFSE sequences (mean scan time, 204 and 22 seconds, respectively). The following 3 types of SSFSE images were reconstructed, and the signal-to-noise ratio (SNR) and tissue contrast were assessed: conventional reconstruction (SSFSE-C), DL reconstruction (SSFSE-DL), and DL with a sharpening filter (SSFSE-DLF). Three radiologists independently assessed image quality, and area under the visual grading characteristics curve (AUCVGC) analysis was performed to compare the SSFSE and PROPELLER images. Compared with that of the PROPELLER images, the SNR of the SSFSE-C, SSFSE-DL, and SSFSE-DLF images was significantly lower (P < 0.05), significantly higher (P < 0.05), and equivalent, respectively. The SSFSE-DL images exhibited significantly lower contrast between the junctional zone and myometrium than those obtained with the other sequences (P < 0.05). In qualitative comparisons with the PROPELLER images, all 3 SSFSE sequences, SSFSE-DL, and SSFSE-DLF demonstrated significantly higher scores for artifacts, noise, and sharpness, respectively (P < 0.01). The overall image quality of SSFSE-C (mean AUCVGC, 0.03; P < 0.01) and SSFSE-DL (mean AUCVGC, 0.23; P < 0.01) was rated as significantly inferior, whereas that of SSFSE-DLF (mean AUCVGC, 0.69) was equivalent or significantly higher (P < 0.01). Using a combination of DL and a sharpening filter markedly increases the image quality of SSFSE of the uterus to the level of the PROPELLER sequence.

Control of a wireless sensor using the pulse sequence for prospective motion correction in brain MRI.

To synchronize and pass information between a wireless motion-tracking device and a pulse sequence and show how this can be used to implement customizable navigator interleaving schemes that are part of the pulse sequence design. The device tracks motion by sampling the voltages induced in 3 orthogonal pickup coils by the changing gradient fields. These coils were modified to also detect RF-transmit events using a 3D RF-detection circuit. The device could then detect and decode a set RF signatures while ignoring excitations in the parent pulse sequence. A set of unique RF signatures were then paired with a collection of navigators and used to trigger readouts on the wireless device synchronous to the pulse sequence execution. Navigator interleaving schemes were then demonstrated in 3D RF-spoiled gradient echo, T1 -FLAIR (fluid-attenuated inversion recovery) PROPELLER (periodically rotated overlapping parallel lines with enhanced reconstruction), and T2 -FLAIR PROPELLER pulse sequences. Excitations in the parent pulse sequences were successfully rejected and the RF signatures successfully decoded. For the 3D gradient echo sequence, distortions were removed by interleaving flipped polarity navigators and taking the difference between consecutive readouts. The impact on scan duration was reduced by 54% by breaking up the navigators into smaller parts. Successful motion correction was performed using the PROPELLER pulse sequences in 3 Tesla and 1.5 Tesla MRI scanners without modifications to the device hardware or software. The proposed RF signature-based triggering scheme enables complex interactions between the pulse sequence and a wireless device. Thus, enabling prospective motion correction that is repeatable, versatile, and minimally invasive with respect to hardware setup.

T1 -FLAIR imaging during continuous head motion: Combining PROPELLER with an intelligent marker.

The purpose of this work is to describe a T1 -weighted fluid-attenuated inversion recovery (FLAIR) sequence that is able to produce sharp magnetic resonance images even if the subject is moving their head throughout the acquisition. The robustness to motion artifacts and retrospective motion correction capabilities of the PROPELLER (periodically rotated overlapping parallel lines with enhanced reconstruction) trajectory were combined with prospective motion correction. The prospective correction was done using an intelligent marker attached to the subject. This marker wirelessly synchronizes to the pulse sequence to measure the directionality and magnitude of the magnetic fields present in the MRI machine during a short navigator, thus enabling it to determine its position and orientation in the scanner coordinate frame. Three approaches to incorporating the marker-navigator into the PROPELLER sequence were evaluated. The specific absorption rate, and subsequent scan time, of the T1 -weighted FLAIR PROPELLER sequence, was reduced using a variable refocusing flip-angle scheme. Evaluations of motion correction performance were done with 4 volunteers and 3 types of head motion. During minimal out-of-plane movement, retrospective PROPELLER correction performed similarly to the prospective correction. However, the prospective clearly outperformed the retrospective correction when there was out-of-plane motion. Finally, the combination of retrospective and prospective correction produced the sharpest images even during large continuous motion. Prospective motion correction of a PROPELLER sequence makes it possible to handle continuous, large, and high-speed head motions with only minor reductions in image quality.

Periodically rotated overlapping parallel lines with enhanced reconstruction acquisition to improve motion-induced artifacts in bladder cancer imaging: Initial findings.

Motion-induced artifacts have been a major drawback in bladder cancer imaging. This study is to evaluate the clinical utility of periodically rotated overlapping parallel lines with enhanced reconstruction (PROPELLER) acquisition in improving motion-induced artifacts in T2-weighted (T2W) magnetic resonance imaging (MRI) of bladder cancer at 3T.Sixteen patient MRI exams were included. Using a Likert scale, 2 radiologists independently scored T2W data without and with PROPELLER in terms of artifact severity and tumor visualization. Statistical analysis was done to assess the image quality improvement by PROPELLER and inter-observer variability.Without PROPELLER, the median scores of artifact severity and tumor visualization were 1.5 and 1.5 for reviewer 1, and 2.0 and 2.0 for reviewer 2. With PROPELLER, the scores increased to 3 and 3.5 for reviewer 1, and 3.5 and 3.5 for reviewer 2. Despite the inter-observer variability (κ scores < 0.2), both reviewers found significant improvement in artifacts and visualization (all P < .001).PROPELLER acquisition significantly improved the image quality of T2W-MRI. These initial findings indicate that this technique should be utilized in clinical MRI of the bladder.

Influence of blade width and magnetic field strength on the ADC on PROPELLER DWI in head and neck.

The purpose of this study was to investigate the influence of blade width and magnetic field strength on apparent diffusion coefficient values of periodically rotated overlapping parallel lines with enhanced reconstruction (PROPELLER) diffusion-weighted imaging in the head and neck at 1.5 and 3.0 T magnetic resonance imaging. Fifteen healthy volunteers (eight men and seven women; mean age 36.4 ± 10.1 years) underwent PROPELLER diffusion-weighted imaging using four blade widths at 3.0 T and three blade widths at 1.5 T. For the multigroup comparison of apparent diffusion coefficient values, we performed analysis of variance and Tukey-Kramer tests. The apparent diffusion coefficient values in each organ were compared among the different blade widths and magnetic field strengths by a t test. Inter and intra-observer agreements regarding apparent diffusion coefficient value measurements were evaluated using the intraclass correlation coefficient. No significant differences were observed in apparent diffusion coefficient values of the cerebellum in each blade width at both field strengths. The apparent diffusion coefficient values of the right parotid glands were significantly lower for blade width 32 than for blade width 16 at 3.0 T (1.16 × 10-3 mm2/s vs. 1.01 × 10-3 mm2/s; P < 0.05). The apparent diffusion coefficient values of bilateral parotid glands were significantly higher at 1.5 T than at 3.0 T. Intraclass correlation coefficients were almost perfect to substantial in the cerebellum, whereas they showed moderate agreement in the parotid glands. The largest blade width 32 at 3.0 T and high magnetic field strength lowered the apparent diffusion coefficient values of parotid glands of PROPELLER diffusion-weighted imaging. The apparent diffusion coefficient values in the parotid glands of PROPELLER diffusion-weighted imaging may be affected for blade width and magnetic field strength.

Efficacy of PROPELLER in reducing ocular motion artefacts and improving image quality of orbital MRI at 3 T using an eye surface coil

To evaluate the efficacy of periodically rotated overlapping parallel lines with enhanced reconstruction (PROPELLER) in reducing ocular motion artefacts and improving image quality of orbital magnetic resonance imaging (MRI) at 3 T using an eye surface coil. Forty-six patients underwent orbital 3 T MRI using an eye surface coil. The pre-contrast axial T2-wighted (W) PROPELLER and T1 fluid-attenuated inversion recovery (FLAIR) PROPELLER imaging were performed on 21 patients, and conventional T2W and T1W imaging were performed on 25 patients. The ocular motion artefacts, delineation of anatomical structures, depiction of lesions, and image quality were evaluated independently by two radiologists using a five-point scale. The signal-to-noise ratios (SNRs) of anatomical structures and lesions were measured. The interobserver agreement was good to excellent (kappa values from 0.79 to 0.91). PROPELLER sequences had higher scoring than the conventional sequences in all cases (p<0.05). PROPELLER images showed fewer motion artefacts, higher image quality, and more clear delineation of anatomical structures and lesions than the non-PROPELLER images. T2W PROPELLER images produced higher SNRs in lens, vitreous body, lacrimal gland, and lesions than conventional T2W images (p<0.05). T1 FLAIR PROPELLER images showed lower SNRs in lens, vitreous body, medial rectus, lateral rectus, temporalis, and posterior half of optic nerve than conventional T1W images (p<0.05). PROPELLER can effectively reduce ocular motion artefacts and improve image quality, which plays an important role in clearly delineating anatomical structures and lesions on orbital 3 T MRI using an eye surface coil.

Can MRI contribute to pulmonary nodule analysis?

There is no accurate method distinguishing different types of pulmonary nodules. To investigate whether multiparametric 3T MRI biomarkers can distinguish malignant from benign pulmonary nodules, differentiate different types of neoplasms, and compare MRI-derived measurements with values from commonly used noninvasive imaging modalities. Prospective. Sixty-eight adults with pulmonary nodules undergoing resection. Respiratory triggered diffusion-weighted imaging (DWI), periodically rotated overlapping parallel lines with enhanced reconstruction (PROPELLER) fat saturated T2 -weighted imaging, T1 -weighted 3D volumetric interpolated breath-hold examination (VIBE) using CAIPIRINHA (controlled aliasing in parallel imaging results in a higher acceleration). Apparent diffusion coefficient (ADC), T1 , T2 , T1 and T2 normalized to muscle (T1 /M and T2 /M), and dynamic contrast enhancement (DCE) values were compared with histology to determine whether they could distinguish malignant from benign nodules and discern primary from secondary malignancies using logistic regression. Predictability of primary neoplasm types was assessed using two-sample t-tests. MRI values were compared with positron emission tomography / computed tomography (PET/CT) to examine if they correlated with standardized uptake value (SUV) or CT Hounsfield unit (HU). Intra- and interreader agreements were assessed using intraclass correlations. Forty-nine of 74 nodules were malignant. There was a significant association between ADC and malignancy (odds ratio 4.47, P < 0.05). ADC ≥1.3 μm2 /ms predicted malignancy. ADC, T1 , and T2 together predicted malignancy (P = 0.003). No MRI parameter distinguished primary from metastatic neoplasms. T2 predicted PET positivity (P = 0.016). T2 and T1 /M correlated with SUV (P < 0.05). Of 18 PET-negative malignant nodules, 12 (67%) had an ADC ≥1.3 μm2 /ms. With the exception of T2 , all noncontrast MRI parameters distinguished adenocarcinomas from carcinoid tumors (P < 0.05). T1 , T2 , T1 /M, and T2 /M correlated with HU and therefore can predict nodule density. Combined with ADC, washout enhancement, arrival time (AT), peak enhancement intensity (PEI), Ktrans , Kep , Ve collectively were predictive of malignancy (P = 0.012). Combined washin, washout, time to peak (TTP), AT, and PEI values predicted malignancy (P = 0.043). There was good observer agreement for most noncontrast MRI biomarkers. MRI can contribute to pulmonary nodule analysis. Multiparametric MRI might be better than individual MRI biomarkers in pulmonary nodule risk stratification. 1 Technical Efficacy: Stage 2 J. Magn. Assessment of quiet T2 weighted PROPELLER sequence in pediatric abdominal imaging

IntroductionElevated acoustic noise during Magnetic Resonance Imaging (MRI) has been associated with patient anxiety and altered cochlear function. Acoustic Reduction Technique (ART) T2 weighted (T2w) periodically rotated overlapping parallel lines with enhanced reconstruction (PROPELLER) has been studied in brain MR but not abdominopelvic imaging. The purpose of our study was to evaluate the image quality and acoustic noise level of ART T2w PROPELLER sequence in comparison with the conventional T2w PROPELLER sequence in pediatric abdomino-pelvic imaging. MethodsEleven consecutive pediatric patients undergoing abdomino-pelvic MRI were scanned on a 3 Tesla magnet using standard and ART T2w PROPELLER sequences. After scanning completion, objective sound level measurements were performed with a sound level meter and microphone. Mann-Whitney U test was used for a non-parametric two-tailed statistical analysis of acoustics, image rating and scan time with significance level set to 0.05. Overall inter-rater agreement was calculated using Cohen's kappa coefficient. ResultsEleven pediatric patients (4 females and 7 males) between 26 days and 18 years of age (mean = 10.0, SD = 5.8) were included. ART T2w produced lower levels of acoustic noise than standard technique in a comparison of mean decibel readings from eleven trials of standard and ART T2w (p value = 0.00008). Streak artifacts were rated greater in ART T2w by both raters (p-value = 0.00278 and 0.00252). There was no significant difference in bile duct blurring, respiratory ghosting, pulsation, fat suppression or hepatic parenchymal depiction. ConclusionPresence of additional streaking artifacts should be considered along with the benefit of reduced acoustic noise from ART T2w.

Effectiveness of the periodically rotated overlapping parallel lines with enhanced reconstruction (PROPELLER) technique for reducing motion artifacts caused by mandibular movements on fat-suppressed T2-weighted magnetic resonance (MR) images

PurposeTo compare a fat-suppressed T2-weighted periodically rotated overlapping parallel lines with enhanced reconstruction (T2W-PROPELLER) sequence with a fat-suppressed T2-weighted fast spin-echo (T2W-FSE) sequence in the oral and maxillofacial regions for the evaluation of the presence of motion artifacts caused by mandibular movements. MethodsFifty-six healthy adult volunteers were examined in a closed mouth position and then with three different rhythmical mandibular movements throughout MR scanning: open-close movement (movement 1), lateral movement (movement 2) and open-close and lateral movement (movement 3). All subjects were scanned first with fat-suppressed T2W-FSE and then with fat-suppressed T2W-PROPELLER while performing the same movements. Motion artifacts, including ghosting or pulsation artifacts, streak artifacts, susceptibility artifacts and the overall image quality were independently evaluated by two oral and maxillofacial radiologists using a five-point scale. The score graded by the two observers was averaged. ResultsThe inter-observer agreement was almost perfect for all evaluated items (κ ≥ 0.81). The T2W-PROPELLER images showed significantly fewer ghosting artifacts than T2W-FSE images in subjects performing the mandibular movements throughout MR scanning (P < .001). T2W-PROPELLER images also showed significantly fewer pulsation artifacts than T2W-FSE images, regardless of the performance of a movement, throughout MR scanning (P < .001). Finally, the T2W-PROPELLER images showed a significantly better overall image quality than T2W-FSE images in subjects performing movements 2 or 3 throughout MR scanning (P < .001). ConclusionThe PROPELLER technique was found to be effective in reducing the motion artifacts caused by mandibular movements on fat-suppressed T2W MR images in the oral and maxillofacial regions.

Role of PROPELLER-DWI of the prostate in reducing distortion and artefact from total hip replacement metalwork

ObjectiveTo compare image quality, artefact, and distortion in standard echo-planar imaging (EPI) with periodically rotated overlapping parallel lines with enhanced reconstruction (PROPELLER) for prostate magnetic resonance imaging (MRI) diffusion-weighted imaging (DWI) in patients with previous total hip replacement (THR). Methods21 male subjects with a clinical suspicion for, or known prostate cancer and previous THR were scanned at 1.5 T using a phased-array body coil. DWI was obtained using single-shot EPI and PROPELLER techniques using fat saturation (PROPELLER-DWI-FS), and without (PROPELLER-DWI-NFS). Image quality (the overall impression of diagnostic quality) was compared to T2-weighted (T2WI) imaging using a 5-point Likert scale, with diffusion sequences additionally scored for artefact and distortion according to a 4-point scale, with artefact defined as the amount of prostate affected and distortion as the degree of warping of the organ. The T2W and DW image volumes were compared to produce quantitative distortion maps. A two-sample Wilcoxon test compared the qualitative scores, with inter-reader variability calculated using Cohen’s kappa. Results21 patients were included in the study, with an average age of 70.4 years and PSA 9.2 ng/ml. Hip metalwork was present bilaterally in 3 patients, left-sided in 9, and right-sided in 9.PROPELLER-DWI-FS significantly improved image quality (p < 0.01) and reduced distortion (p < 0.01) when compared to standard EP-DWI. Artefact was not shown to be significantly improved. The last 5 patients in the study were additionally imaged with PROPELLER-DWI-NFS, which resulted in a significant reduction in artefact compared to EP-DWI (p < 0.05). Quantitative distortion was significantly lower compared to EP-DWI for both PROPELLER with fat saturation (p < 0.01) and without fat saturation (p < 0.01). ConclusionPROPELLER-DWI demonstrates better image quality and decreases both artefact and distortion compared to conventional echo planar sequences in patients with hip metalwork.

Improve Image Quality of Transversal Relaxation Time PROPELLER and FLAIR on Magnetic Resonance Imaging

The Magnetic Resonance Imaging (MRI) is a medical imaging technique that uses the interaction between the magnetic field and the nuclear spins. MRI can be used to show disparity of pathology by transversal relaxation time (T2) weighted images. Some techniques for producing T2-weighted images are Periodically Rotated Overlapping Parallel Lines with Enhanced Reconstruction (PROPELLER) and Fluid Attenuated Inversion Recovery (FLAIR). A comparison of T2 PROPELLER and T2 FLAIR parameters in MRI image has been conducted. And improve Image Quality the image by using RadiAnt DICOM Viewer and ENVI software with method of image segmentation and Region of Interest (ROI). Brain images were randomly selected. The result of research showed that Time Repetition (TR) and Time Echo (TE) values in all types of images were not influenced by age. T2 FLAIR images had longer TR value (9000 ms), meanwhile T2 PROPELLER images had longer TE value (100.75 - 102.1 ms). Furthermore, areas with low and medium signal intensity appeared clearer by using T2 PROPELLER images (average coefficients of variation for low and medium signal intensity were 0.0431 and 0.0705, respectively). As for areas with high signal intensity appeared clearer by using T2 FLAIR images (average coefficient of variation was 0.0637).

Prospective motion correction in 2D multishot MRI using EPI navigators and multislice-to-volume image registration.

Prospective motion correction reduces artifacts in MRI by correcting for subject motion in real time, but techniques are limited for multishot 2-dimensional (2D) sequences. This study addresses this limitation by using 2D echo-planar imaging (EPI) slice navigator acquisitions together with a multislice-to-volume image registration. The 2D-EPI navigators were integrated into 2D imaging sequences to allow a rapid, real-time motion correction based on the registration of three navigator slices to a reference volume. A dedicated slice-iteration scheme was used to limit mutual spin-saturation effects between navigator and image data. The method was evaluated using T2 -weighted spin echo and multishot rapid acquisition with relaxation enhancement (RARE) sequences, and its motion-correction capabilities were compared with those of periodically rotated overlapping parallel lines with enhanced reconstruction (PROPELLER). Validation was performed in vivo using a well-defined motion protocol. Data acquired during subject motion showed residual motion parameters within ±0.5 mm and ±0.5°, and demonstrated a substantial improvement in image quality compared with uncorrected scans. In a comparison to PROPELLER, the proposed technique preserved a higher level of anatomical detail in the presence of subject motion. EPI-navigator-based prospective motion correction using multislice-to-volume image registration can substantially reduce image artifacts, while minimizing spin-saturation effects. The method can be adapted for use in other 2D MRI sequences and promises to improve image quality in routine clinical examinations.
