Preliminary results of iliosacral screws and plate combination in the management of tile B3 pelvic fractures: A retrospective case series.

The aim of this study was to present preliminary results and experience with iliosacral screws and plate combination (horizontal triangular osteosynthesis, HTO) in the management of Tile B3 pelvic fractures. In this retrospective study, 26 patients (18 male, 8 female; mean age = 46, age range = 31-58 years) who were treated with horizontal triangular osteosynthesis using the combination of bilateral percutaneous iliosacral screws and transiliac plates due to Tile B3 pelvic fractures in our institution were included. The accompanying symptoms, blood loss, operation time , fracture healing time, and postoperative complications were observed and recorded. Patients were followed up retrospectively with routine post-operation visits for clinical and radiographic examination. The reduction quality was evaluated according to Matta criterion. Clinical and nerve function outcomes were evaluated by Majeed and Gibbons criterion. The mean follow-up time was 15 months (range = 12-21 months). The mean blood loss and operation time were 24.6 ± 6.7mL and 30.5 ± 3.47 mins, respectively. Patients experienced early weight-bearing ability and no fracture reduction loss. Due to Matta criterion for fracture reduction, the results were excellent in 16 cases, good in 9 cases, and fair in 1 case. Due to Majeed functional scoring at the last follow-up, the results were excellent in 17 cases, good in 9 cases. Of 3 patients in whom neurologic impairment was detected preoperatively, 2 achieved complete recovery, and 1 achieved partial improvement postoperatively. As a new surgical concept in the management of Tile B3 pelvic fractures, HTO seems to provide the following advantages: horizontal triangular fixation, minimally invasive incisions, less blood loss, and permitting early weight-bearing ability. However, there still exist several problems regarding the technique and the option of implants. Level IV, Therapeutic Study.

Screw misplacement in percutaneous posterior pelvic iliosacral screwing with and without navigation: A prospective clinical study of 174 screws in 127 patients

BackgroundRecent studies of iliosacral screw fixation performed using intraoperative navigation systems have shown promising results. The Surgivisio Platform is a new-generation three-dimensional intraoperative navigation tool that has been used at our institution for 2years. The aim of this prospective study was to assess the contribution of navigation in terms of iliosacral screw positioning accuracy and of radiation exposure, by comparing outcomes with vs. without navigation. HypothesisNavigation allows more accurate percutaneous iliosacral screw positioning regardless of the type of screw fixation (with sacral dysmorphism and/or cemented screw fixation and/or multiple screw fixation). Materials and methodsBetween January 2018 and December 2019, consecutive patients who underwent percutaneous iliosacral screw fixation of pelvic ring fractures without vertical instability were included in this single-centre prospective study. Screw position accuracy was evaluated by postoperative high-resolution computed tomography (HRCT). Operative time, radiation dose, and complications were recorded. ResultsWe included 127 patients with 174 iliosacral screws, of which 129 were positioned under fluoroscopic guidance and 45 using navigation. According to the modified Gras classification, 7% (12/174) of the screws were incorrectly positioned and 2% (4/174) required repositioning. The frequency of screw malposition was not significantly different between the fluoroscopy and navigation groups (8.5%, 11/129 vs. 2.2%, 1/45, respectively; p=0.19). However, screw position in dysmorphic sacra was significantly better with navigation (p=0.04), whereas no significant difference in final screw position was found for cemented or multiple screw implantations. In the navigation group, the operative time was significantly longer (28.2min vs. 21.6min, p=0.003), and the mean dose-area product significantly greater (6.6Gy·cm2 vs. 4.9Gy·cm2, p=0.02). The complication rates were not different between the two groups. ConclusionIn patients who have pelvic ring fractures without vertical instability, navigation of percutaneous iliosacral screw placement using the Surgivisio Platform improves screw positioning in dysmorphic sacra, at the cost of a longer operative time and greater radiation exposure of the patient. Iliosacral screw fixation of pelvic ring disruption with tridimensional patient-specific template guidance

IntroductionPosterior pelvic ring disruption includes sacral fractures, sacroiliac joint fracture dislocations and ilium fractures. Percutaneous iliosacral screw fixation of sacral fractures and sacraoiliac joint fracture dislocations have been prevailing, it has the advantages of minimal invasiveness, less blood loss and low wound infection rate. HypothesisThis study was to evaluate the application of three-dimensional (3D) printed patient-specific guide template in closed reduction and iliosacral screw fixation of posterior pelvic ring disruption. Material and methodsThe data of patients, who were treated with closed reduction and iliosacral screw fixation of posterior pelvic ring disruption with the assistance of 3D printed guide template from December 2014 to September 2018, were collected. The screw placement time, fluoroscopy time, intraoperative blood loss, fracture reduction, screw position, and functional assessment were recorded. ResultsThere were 17 cases of unstable pelvic fractures,and 20 screws were inserted for fixation of sacral fractures or sacroiliac joint dislocations, with bilateral screw placement in 3 cases. The average time for each screw placement was 45.9±8.6min (30–60min). The average fluoroscopy time for each screw insertion was 50.3±19.7s (24–96 s). The mean blood loss for each screw placement was 32.0±11.1ml (20–50ml). According to Matta scale, the fracture reduction was graded as excellent in all the 17 cases. According to the modified Gras classification, the 3D CT reconstruction of the pelvis demonstrated Grade 1 for 18 screws and Grade 2 for 2 screw. Functional outcome 1 year postoperatively was rated as 15 excellent and 2 good, according to the Majeed functional scale. DiscussionIt is feasible and safe to stabilize the posterior pelvic ring disruption using iliosacral screw fixation under assistance of the 3D printed guide template. It could reduce fluoroscopy time, screw placement time and intraoperative blood loss and achieve good postoperative recovery. Level of proofIV; Retrospective study.

Percutaneous iliosacral screw and trans-iliac trans-sacral screw with single C-arm fluoroscope intensifier is a safe treatment for pelvic ring injuries

To elucidate the accuracy, efficacy, and safety of percutaneous iliosacral screw (ISS) and trans-iliac trans-sacral screw (TITS) insertion using a single C-arm fluoroscopy intensifier. Additionally, the potential risk factors that might cause mal-positioned screws were identified. Patients with pelvic ring injuries who underwent percutaneous screw fixation in a single medical institute were divided into an ISS group (n = 59) and a TITS group (n = 62) and assessed. The angles deviated from ideal orientation (ADIO) of the implanted screw were measured, and potential risk factors for mal-positioned screws were analyzed. Overall, the reduction quality of the pelvic ring was good or excellent in 70 patients (82.4%) by Matta’s criteria and in 48 patients (56.5%) by Lefaivre’s criteria. ADIO measurements of the ISS and TITS groups via multi-planar computed tomography were 9.16° ± 6.97° and 3.09° ± 2.8° in the axial view, respectively, and 5.92° ± 3.65° and 2.10° ± 2.01° in the coronal view, respectively. Univariate statistical analysis revealed body mass index as the single potential risk factor of mal-positioned screws. With careful preoperative planning and intraoperative preparations, placing ISS and TITS under the guidance of single C-arm fluoroscopy intensifier is a reliable and safe technique. Caution should be exercised when performing this procedure in patients with a high body mass index.

An anatomical study defining the safe range of angles in percutaneous iliosacral and transsacral screw fixation.

Percutaneous iliosacral screw fixation and transsacral fixation are challenging procedures requiring extensive knowledge of sacral anatomy to avoid damaging nearby neurovascular structures. Greater knowledge of anatomical screw trajectory and size allowances would be helpful to guide surgical placement. An anatomical study of 40 cadaveric sacra in specimens ages 18-65 was performed. Three-dimensional surface scans were obtained, and computer modeling software was used to simulate a 7.3 mm diameter screw with 1 mm buffer inserted orthogonal to the sacroiliac joint in the pelvic inlet and outlet views. Transsacral screws were also inserted into S1 and S2 vertebrae. For screws orthogonal to the sacroiliac joint, the overall mean screw insertion angle was 4.1° ± 7.5° (range, -18.3° to 22.0°) in the inlet view in the posterior to anterior direction, and 21.7° ± 5.1° (range, 8.2°-36.3°) in the outlet view in the caudal to cranial direction. Before breaching the sacrum, the range of sacral tunnel lengths was between 31.1 and 70.1 mm with a range of diameters between 9.3 and 13.3 mm. Transsacral screws inserted into either the S1 or S2 vertebrae did not breach the sacrum in 40% (16/40) at each level. 30% (12/40) of sacra could not safely accommodate both S1 and S2 transsacral screws. There is an initial screw insertion angle range of -4° to 12° in the inlet view and 16°-27° in the outlet view. There was always adequate size to accept a 7.3 mm or larger screw.

Percutaneous two unilateral iliosacral S1 screw fixation for pelvic ring injuries: a retrospective review of 38 patients

Purpose: Percutaneous iliosacral (IS) screw fixation for pelvic ring injuries is a minimally invasive technique that reduces the amount of blood loss and shortens the procedure time. Moreover, two unilateral IS S1 screws exhibit superior stability to a single IS screw and are also safer for neurological injuries than an S2 screw. Therefore, this study aimed to evaluate fixation using percutaneous two unilateral IS S1 screws for pelvic ring injuries and its subsequent clinical outcomes. Methods: We retrospectively reviewed 38 patients who underwent percutaneous two unilateral IS S1 screw fixation for pelvic ring injuries. The procedure time, blood loss, achievement of bone union, radiological outcomes (Matta and Tornetta grade), and postoperative complications were evaluated. Results: The mean procedure time, hemoglobin loss, bone union rate, and time to union were 40.1 minutes (range, 18–102), 0.6 g/dL (range, 0.3–1.0), 100%, and 153.2 days (range, 61–327), respectively. The Matta and Tornetta grades were excellent, good, and fair in 24 (63.1%), 11 (28.9%), and 3 patients (7.9%), respectively, and the postoperative complications were S1 screw loosening, widening of the symphysis pubis (2.3 and 2.5 mm), lumbosacral plexopathy, and S1 radiculopathy in one (2.6%), two (5.3%), one (2.6%), and one patient (2.6%), respectively. However, all neurological complications recovered spontaneously. Conclusions: Percutaneous two unilateral IS S1 screw fixation was useful for treating pelvic ring injuries. In particular, it involved a short procedure time with little blood loss and also led to 100% bone union and good radiological outcomes.

Current Minimally Invasive Surgical Concepts for Sacral Insufficiency Fractures.

An increasing incidence of sacral insufficiency fractures in geriatric patients has been documented, representing a major challenge to our healthcare system. Determining the accurate diagnosis requires the use of sectional imaging, including computed tomography and magnetic resonance imaging. Initially, non-surgical treatment is indicated for the majority of patients. If non-surgical treatment fails, several minimally invasive therapeutic strategies can be used, which have shown promising results in small case series. These approaches are sacroplasty, percutaneous iliosacral screw fixation (S1 with or without S2), trans-sacral screw fixation or implantation of a trans-sacral bar, transiliac internal fixator stabilisation, and spinopelvic stabilisation. These surgical strategies and their indications are reported in detail. Generally, treatment-related decision making depends on the clinical presentation, fracture morphology, and attending surgeon's experience.

Pelvic antropometric measurement in 3D CT for placement of two unilateral iliosacral S1 - 7.3 mm screws

PurposeStability of the dorsal pelvic ring is important for patient mobilisation and can be restored using several surgical procedures after fracture. Placement of percutaneous iliosacral screws is a reliable and minimal-invasive technique to achieve stabilisation of the dorsal pelvic ring by placement of two screws in the first sacral vertebra. Aim of this study was to evaluate 3D CT scans regarding the anatomical possibility to place two 7.3 mm iliosacral screws for fixation of the dorsal pelvic ring.Methods3D CT datasets of 500 consecutive trauma patients with 1000 hemipelves of a mid-european level I trauma centre with or without pelvic injury were evaluated and measured bilaterally in this retrospective study.ResultsOne thousand hemipelvic datasets of 500 patients (157 females, 343 males) with a mean age of 49.7 years (18 to 95) were included in this study. Only 16 hemipelves (1.6%, 11 in females, 5 in males) in 14 patients (2.8%, 9 females = 5.73%, 5 males = 1.5%) showed too narrow corridors so that 7.3 mm screw placement would not be possible (p = 0.001). In women, too narrow corridors occurred 3.9 times as often as in men. Only two females showed this bilaterally.ConclusionThe evaluation of 3D CT scans of the pelvis showed the importance of planning iliosacral screw placement, especially if two 7.3 mm screws are intended to be placed in the first sacral vertebra.

Evaluation of the early radiological and functional results of percutaneous fixation of unstable pelvic ring injuries

Recent years have seen improvements in all phases of pelvic fracture management. The surgical treatment of pelvic ring injuries is technically demanding The aim of treatment is to achieve anatomical or near anatomical restoration of the articular surface, accurate reduction of the pelvic ring in mechanically unstable injuries, reduce pain, allows early mobilization of the patient and optimizes long term functional results This study aimed to evaluate the early radiological and functional results of percutaneous fixation of unstable pelvic ring injuries. From October 2019 till December 2020 twenty patients (11male and 9 female) with unstable pelvic ring injuries were included in the study. The mean age 42 years (range 18–66 years). The most common mode of injury was road traffic accident (RTA) followed by fall from height. The most common injury pattern according to Young and Burgess lateral compression injury, anteroposterior compression injury and vertically unstable. Four patients had medical comorbidities diabetes mellitus and hypertension at presentation. All patients treated by DCO protocol. 3 patients with posterior pelvic ring injuries treated by percutaneous iliosacral screw. 3 patients with anterior pelvic ring injuries treated by external fixator. 2 patient with anterior pelvic ring injuries treated by INFIX .8 patients with anterior and posterior pelvic ring injuries treated by external fixator and percutaneous iliosacral screw.1 patients with anterior and posterior pelvic ring injuries treated by INFIX and percutaneous iliosacral screw.2patients with anterior pelvic ring and posterior pelvic ring injuries treated by INFIX and spinopelvic fixation.1 patients with anterior pelvic ring and posterior pelvic ring injuries treated by external fixator and spinopelvic fixation. The mean injury-to-surgery interval was 6.1 days (range 0-12 days). The mean intraoperative blood loss 160ml range 100ml -240 ml. Percutaneous fixation of pelvic ring injuries offers safe, short operative time technique, less complication, with better results, short hospital stay, early healing and should be the standard way for pelvic ring fractures fixation. High quality pre-operative and intra-operative imaging, image-guidance, are essential for an equally high quality of surgical results.

Iliosacral screwing under navigation control: Technical note

Percutaneous iliosacral screw fixation is an essential osteosynthesis technique for pelvic fractures and requires precise imaging tools to assist correct screw placement. 3D imaging coupled to navigation has significantly improved this technique. Several possibilities exist, depending on the instrument set used, with varying degrees of difficulty and very variable risk of error. The techniques traditionally described use a guide to navigate a drill bit, where a one-degree difference in trajectory can alter screw tip placement by several millimeters. The present article proposes a standardization of the procedure by using navigated pedicular screw instruments that have the advantage of navigating the instrument itself and not a projection. In a series of 90 screws implanted in 62 patients using this technique, only one path had to be repeated in the light of intraoperative control. No improperly positioned screws were found on postoperative control.

Surgical Technique of Percutaneous Iliosacral Screw Fixation in S3 Level in Unstable Pelvic Fracture with Closed Degloving Injury and Morrell Lavallee Lesion: Two Case Reports

Unstable pelvic ring fractures raise treatment challenges in severely injured patients. Beside patient survival, demanding surgical technique also required. Classic technique of internal fixation requires extensive surgical exposure that leads to most complications of the pelvic fractures fixation. Therefore less invasive technique is a reasonable alternative especially in unstable pelvic fracture with soft tissue injury or potential infection. Unfortunately in sacral dysmorphism pelvic injury or in complete vertical sacral fracture, we need S3 level insertion to improve stability of iliosacral (IS) screw in S1 or S2. Purpose of this study was to show feasibility technically inserting IS screw in S3 level.Methods:We reported 2 cases of unstable pelvic injury. First case was an 11 years old boy with Torode and Zieg IV or Marvin Tile C1 pelvic fracture with right sacroiliac joint disruption and soft tissue injuries of skin avulsion on the left hip and Morel-Lavallée lesion on the right hip. He was managed with early anatomic reduction and fixation with percutaneous screws on both pubic rami and IS screw (sacroiliac lag screw type) on S1 and S3. Second case was a 30 years old male with open pelvic fracture Faringer zone III type, Marvin Tile B2 or YoungBurgess LC I and also with vertical sacral fracture Denis zone 1. This polytrauma case had associated injuries includes Morel-Lavallée lesion, intraperitoneal bladder rupture, infected laparotomy wound dehiscence, and immunocompromised. The same minimal invasive management of pelvic fracture was performed in this case by inserting percutaneous screws on pubic rami and IS (sacral screw type) on S1 and S3. Functional outcome was evaluated using Majeed and Hannover pelvic scoring system.Results:All patients survived and considered to have good reduction, with no residual displacement on the sacroiliac joint. The former case, at 21-month follow up, present with excellent outcome (80 out of 80) by Majeed score and very good outcome (4 out of 4) by Hannover score; while the latter case, at 18-month follow up, present with poor outcome (50 out of 100) by Majeed score and fair outcome (2 out of 4) by Hannover score.Conclusion:Percutaneous screw fixation for pelvic ring injury is a less invasive alternative that can be used for early stabilization of unstable anterior and posterior pelvic fractures and provide stable internal fixation. IS screw is feasible to be inserted in S3 level either sacroiliac joint type or sacral screw type.

Multidimensional Pelvic Fluoroscopy: A New and Novel Technique for Assessing Safety and Accuracy of Percutaneous Iliosacral Screw Fixation

Multidimensional fluoroscopy is new imaging technology that generates intraoperative cross sectional imaging. Can this technology be used to assess accuracy and safety of percutaneously placed iliosacral screws intraoperatively? Retrospective study. Level 1 academic trauma center. Fifty-two consecutive patients during a 7-month period with unstable posterior pelvic ring disruptions. All patients were treated with percutaneous iliosacral and/or transsacral screw fixation by a single experienced surgeon. Traditional triplanar fluoroscopy was performed during guidepin insertion. Intraoperative multidimensional fluoroscopy was used for all patients after iliosacral screw fixation. Intraoperative multidimensional fluoroscopy and postoperative computed tomography (CT) scans for each patient were retrospectively reviewed by the treating surgeon and another trauma surgeon. Screw position in relation to the sacral neuroforamen was assessed using multidimensional fluoroscopy and compared to postoperative CT scan. Screws were classified as extraforaminal, juxtaforaminal, or intraforaminal. No screws were intraforaminally seen on intraoperative multidimensional fluoroscopy or postoperative CT scan. All iliosacral and transsacral screws were considered safe. Intraobserver and interobserver variability existed between reviewers when grading screws as extraforaminal versus juxtaforaminal. This was not clinically significant because both agreed that extraforaminal and juxtaforaminal screw positions are safe. Multidimensional fluoroscopy was used in 3 patients to assess guidepin placement before definitive screw fixation. Two patients underwent a change of fixation after reviewing multidimensional fluoroscopy. No postoperative neurological examination changes occurred. Multidimensional fluoroscopy is a novel imaging technology that can safely be used intraoperatively to accurately determine iliosacral and transsacral screw placement. Diagnostic Level III. Anterior subcutaneous internal fixation combined with posterior percutaneous iliosacral screw for treatment of unstable pelvic fractures

To assess the effectiveness of anterior subcutaneous internal fixation (INFIX) combined with posterior percutaneous iliosacral screw for the treatment of unstable pelvic fractures. Between August 2016 and November 2017, 19 cases of unstable pelvic fractures were treated with anterior subcutaneous INFIX combined with posterior percutaneous iliosacral screw. There were 14 males and 5 females, with an average age of 40.6 years (range, 17-69 years). Causes of injury included traffic accident injury in 11 cases, falling from height in 5 cases, bruise injury by heavy object in 3 cases. According to Tile classification, there were 2 cases of type B1, 6 cases of type B2, and 11 cases of type C. Anterior ring injuries included bilateral pubic ischial ramus fractures in 12 cases, unilateral pubic ischial ramus fractures in 5 cases, and symphysis pubis separation in 2 cases. Posterior ring injuries included sacroiliac ligament injuries in 2 cases, unilateral iliac bone fractures in 3 cases, unilateral sacral fractures in 11 cases, unilateral sacroiliac joint dislocation in 2 cases, and bilateral sacral fracture in 1 case. The intraoperative blood loss and operation time were recorded, and the fracture healing and postoperative complications were observed. Matta score was used to evaluate the reduction of fracture, and Majeed score was used to evaluate the postoperative function of patients. The operation time was 47-123 minutes (mean, 61.4 minutes) and the intraoperative blood loss was 50-115 mL (mean, 61.1 mL). One case had superficial infection at the site of screw implantation, and 1 case had unilateral cutaneous nerve stimulation, which were cured after corresponding treatment. There was no damage of urinary system, reproductive system, and intestine. All cases were followed up 12-25 months (mean, 18.1 months). All the fractures healed after operation, the average healing time was 9.5 weeks (range, 8-13 weeks); no nonunion, delayed healing, internal fixation loosening, fracture, and other situations occurred. Of the 2 patients with lumbosacral plexus injury before operation, 1 recovered completely and 1 had residual mild claudication. At last follow-up, the reduction of fracture was evaluated by Matta scoring standard, the results were excellent in 13 cases and good in 6 cases, with an excellent and good rate of 100%; the function was evaluated by Majeed scoring standard, the results were excellent in 15 cases and good in 4 cases, with an excellent and good rate of 100%. Minimally invasive pelvic stabilization by using anterior subcutaneous INFIX and posterior percutaneous iliosacral screw for treatment of unstable pelvic fractures, can achieve good fracture reduction and definitive stabilization with minimum complications and obtain excellent functional outcomes.

Efficacy and Safety of Minimally Invasive Percutaneous Ilio-Sacral Fixation Technique for Sacral Fractures in The Initial Twenty Cases at Department of Neurosurgery in Alexandria University

Background Data: Sacral fractures constitute a major entity of pelvic fractures. 50% of these sacral fractures are not recognized on initial physical examination of the traumatized patients. The most important prognostic factor in management of sacral fractures is the presence or absence of neurological deficit. Some studies adopt the concern regarding fixation of sacral fractures in poly-traumatized patients to avoid systemic effects and complications of recumbency. Purpose: Evaluation of the efficacy and safety of the minimally invasive percutaneous ilio-sacral fixation technique in management of initial twenty cases of sacral fractures at department of Neurosurgery in Alexandria University. Study Design: Retrospective clinical case cohort study. Patients and Methods: Between March 2017 and January 2019; 20 patients were presented at Neurosurgery Department in Alexandria University hospitals with traumatic sacral fractures. They were 13 males and 7 females with the mean of 34 years (ranged from 18-55 years). Plain X ray of pelvis including antero-posterior, inlet and outlet views of pelvis, CT scan with 3D reconstructions were done. We used Dennis classification and Roy-Camille classification in our study. Stabilization of sacral fracture was done percutaneously using 7 mm cannulated partially threaded ilio-sacral screws. Clinical, neurologic and radiographic examinations were performed in the follow up period (6 months) to assess healing, evaluate clinical improvement and to detect any implant changes. Results: Total 39 percutaneous ilio-sacral screws were placed in 20 patients. Partially threaded cancellous 7.0-millimeter cannulated screws were used. Fifteen patients had unilateral double screws; one patient had unilateral triple screws; two patients had bilateral single screws for bilateral sacral fracture and two patients had unilateral single screw. Two Misplaced screws out of thirty-nine screws were found in the post-operative CT. One patient developed post-operative foot drop. All fractures healed clinically and radio-graphically except one case fixed by single screw and did not show radiological healing. Conclusion: Percutaneous ilio-sacral fixation for sacral fractures is a safe minimally invasive method for management of sacral fractures. It is effective regarding pain relief with high fusion rate. It needs a long learning curve. Single screw fixation is not a rigid fixation and may lead to hardware failure. (2019ESJ193)

Outcome of Percutaneous Iliosacral Screw Fixation of Sacroiliac Joint Disruptions

Background: Closed reduction and percutaneous fixation of sacroiliac joint disruptions has the advantage of minimal invasion of compromised soft tissue, limited blood loss, decreased operation time, and decreased infection rates. The objectives of this study was to evaluate the functional and radiological outcome after percutaneous fixation of sacroiliac joint (SIJ) disruptions by cannulated screw. Objectives: Management of sacroiliac joint disruptions by closed reduction and percutaneous fixation by iliosacral screw. Patients and Methods: This study was carried out in Orthopedic Surgery Department, Zagazig University Hospitals, during the period from May 2018 to September 2019. The study included 12 patients with age ranged from 19-59 years, 7 males and 5 females. Mode of injury was road traffic accident in 58.3% of cases and fall from height in 41.7%, right side was affected in 6 patients, left side was affected in 5 patient and one patient was affected bilaterally. Results: All patients were evaluated clinically according to Majeed scoring system. The overall Majeed score was satisfactory in 75%, while unsatisfactory results were in 25% of cases. Postoperative radiological evaluation was done according to Matta and tornetta. 25% patients had excellent results, 50% had good results and 25% had fair results. Postoperative complication were superficial infection in 6 patients and one patient had chronic posterior pelvic pain. Conclusion: Percutaneous method of fixation of the SIJ disruptions is effective and favourable method ensuring low operative time, high chance of obtaining stable pelvic ring with minimal blood loss, low incidence of complications and good rehabilitation results especially early mobilization.

Pinhole effect and manual bowel gas displacement: Simple two tricks for better fluoroscopy imaging in iliosacral screw fixation

Introduction: Presently written abstract. Material and Methods: Total 50 out of 75 patients with pelvic fractures were managed with ilio-sacral screws. Results: At mean followup of 18 months non had iatrogenic injuries and ilio sacral screws were well within bony corridors. Conclusion: The use of pin hole effect and manual displacement of bowel gas shadows can reduce the margin of error while passing percutaneous iliosacral screws.

Feasibility of iliosacral screw placement in patients with upper sacral dysplasia

BackgroundExact knowledge of the sacral anatomy is crucial for the percutaneous insertion of iliosacral screws. However, dysplastic anatomical patterns are common. In addition to a preoperative computed tomography (CT) analysis, conventional radiographic measures may help to identify upper sacral dysplasia and to avoid damage to surrounding structures. Aiming to further increase safety in percutaneous iliosacral screw placement in the presence of sacral dysmorphism, this study examined the prevalence of previously established radiographic signs and, in addition, defined the “critical SI angle” as a new radiographic criterion.MethodsPelvic CT scans of 98 consecutive trauma patients were analysed. Next to assessment of established signs indicating upper sacral dysplasia, the critical sacroiliac (SI) angle was defined in standardized pelvic outlet views.ResultsThe critical SI angle significantly correlates with the presence of mammillary bodies and an intraarticular vacuum phenomenon. With a cut-off value of − 14.2°, the critical SI angle detects the feasibility of a safe iliosacral screw insertion in pelvic outlet views with a sensitivity of 85.9% and a specificity of 85.7%.ConclusionsThe critical SI angle can support the decision-making when planning iliosacral screw fixation. The clinical value of the established signs of upper sacral dysplasia remains uncertain.

Effectiveness of closed reduction and percutaneous fixation of isolated sacral fractures. Functional outcomes and sagittal alignment after 3.6 years in 20 patients

BackgroundDisplaced U- or H-shaped sacral fractures (Roy-Camille Grade II or III) are treated at our institution by early transcondylar traction and manual countertraction, hyperlordosis induced by a pad positioned under the lumbo-sacral junction, and percutaneous ilio-sacral screw fixation. The objective of this study was to evaluate the outcome of this technique used in a level 1 trauma centre.Hypothesis Our early reduction technique provides anatomical reduction of U- or H-shaped sacral fractures by correcting the sagittal malalignment due to the intra-sacral kyphosis, thereby obviating the need for decompression laminectomy and improving neurological outcomes. Material and methodsWe retrospectively evaluated 20 patients treated for U- or H-shaped sacral fractures using our original reduction technique followed by percutaneous fixation only. Mean follow-up was 42.4 months. Mean displacement of the S1 posterior wall was measured on computed tomography scans obtained before and after surgery. Pelvic incidence (PI) and measured lumbar lordosis (LLm) were evaluated on standard radiographs before surgery and on stereoradiographs after surgery. Expected lumbar lordosis (LLe) was computed as LLe=PI+9°. A 25% or greater difference between LLe and LLm defined lumbo-pelvic mismatch. At last follow-up, functional outcomes were assessed based on the Majeed score and the Iowa Pelvic Score (IPS), and a neurological examination was performed. ResultsMean S1 posterior wall displacement in the sagittal and axial planes was 64% and 64.8%, respectively, before surgery versus 5.6% and 15.2%, respectively, after surgery. At last follow-up, LLm was 63.5° and the LLe-LLm difference was 11.2%; only 3 (15%) patients had lumbo-pelvic mismatch at last follow-up. The mean Majeed score and IPS values were 86.6 and 79, respectively, and lumbo-pelvic mismatch correlated significantly with a worse functional outcome defined as a Majeed score below 75 (p=0.0087). At last follow-up, the neurological dysfunctions were improved in 90% of patients, and 70% of patients had achieved a full neurological recovery. Discussion/ConclusionGiven these encouraging findings, we advocate early reduction and percutaneous fixation of U- or H-shaped sacral fractures. Although technically demanding, this method restores the normal pelvic parameters and improves neurological function. Level of evidenceIV, retrospective observational study.

Short term functional outcome of single percutaneous Ilio-sacral screw fixation in posterior pelvic ring injuries

Objective: To evaluate functional outcome of single percutaneous iliosacral screw fixation in posterior pelvic ring injuries. Materials and Methods: 20 adult patients of either sex with partially or completely unstable posterior pelvic ring injuries with or without anterior ring injuries admitted between October 2016 to November 2018 were treated with single percutaneous iliosacral screw fixation. The patients were followed clinically and radiologically at four, ten, sixteen and twenty four weeks and assessed with Majeed Pelvic Score.Results: There were 15 male and 5 female with mean average age of 38.1 with 60% being Tiles Type C and 40% being Tiles type B injuries. Out of 20 patients 17 patients (85%) were found to have excellent functional outcome 2 patients (10%) had Good and 1 patient (5%) had fair functional outcome based on clinical grade of Majeed Pelvic score. 1 patient had screw breakage. No patient had Neurological deficit. No case of infection was reported. Conclusion: The results obtained in our study compare well with other studies undertaken in the past and shows that single percutaneous iliosacral screw fixation is safe and efficacious procedure for fixation of sacral fracture and sacroiliac disruption.

A novel patient-specific three-dimensional-printed external template to guide iliosacral screw insertion: a retrospective study

BackgroundIliosacral screw fixation is a popular method for the management of posterior pelvic ring fractures or dislocations, providing adequate biomechanical stability. Our aim in this study was to describe the use of a new patient-specific external template to guide the insertion of iliosacral screws and to evaluate the efficacy and safety of this technique compared with the conventional fluoroscopy-guided technique.MethodsThis was a retrospective study of patients with incomplete or complete posterior pelvic ring disruptions who required iliosacral screw fixation. For analysis, patients were divided into two groups: the external template group (37 screws in 22 patients) and the conventional group (28 screws in 18 patients). The operative time per screw, radiation exposure time and the rate of screw perforation (accuracy) were compared between groups. In the external template group, the difference between the actual and planned iliosacral screw position was also compared.ResultsIn the conventional group, the average operative time per screw was 39.7 ± 10.6 min, with an average radiation exposure dose of 1904.0 ± 844.5 cGy/cm2, with 4 cases of screw perforation. In the external template group, the average operative time per screw was 17.9 ± 4.7 min, with an average radiation exposure dose of 742.8 ± 230.6 cGy/cm2 and 1 case of screw perforation. In the template group, the mean deviation distance between the actual and planned screw position was 2.75 ± 1.0 mm at the tip, 1.83 ± 0.67 mm in the nerve root tunnel zone and 1.52 ± 0.48 mm at the entry point, with a mean deviation angle of 1.73 ± 0.80°.ConclusionsThe external template provides an accurate and safe navigation tool for percutaneous iliosacral screw insertion that could decrease the operative time and radiation exposure.
