Aim. People with attractive facial appearance are more easily integrated in society and are considered to be more capable, more intelligent and more responsible. The aesthetic aspect is influenced by expression, by facial symmetry, facial contours and teeth colour. In dental restorations, the correct determination of teeth colour is a critical step with extreme importance for successful treatment. From this perspective the practitioner must have experience, must have the ability to distinguish colours and must train through repetitive visual practical exercises that can have an impact on the perception of dental aesthetics. Materials and method. The study followed comparatively the evolution of teeth colour determination and its impact on dental aesthetics perception. It was conducted on a group of fifth year students from the Faculty of Dental Medicine, Bucharest using the Tooth Training Box colour determination exercise that is based on the 3-D Master colour key principle. Results. Using training exercises to determine colour participants developed a better understanding of the physical principles behind the procedure but also a significant increase in accuracy of their measurements and confidence in their colour determinations. Conclusions. Using an aesthetic evaluation protocol that translates teeth colour in a universal language for both practitioners as well as dental technicians has a significant impact in aesthetic dentistry.

Perception of Personal Dental Appearance and Dental Aesthetic Index Score Among 18- to 20-year-old College Students in Rural South India.

To assess the correlation between self-perceived dental appearance and dental malocclusion among 18- to 20-year-old college students in Virajpet, Karnataka. A stratified random sample consisting of 280 18- to 20-year-old college students in Virajpet were recruited for the study. A pretested Oral Aesthetic Subjective Impact Scale (OASIS) and Visual Analogue Scale (VAS) were used to assess the subjects' perception of dental aesthetics. Malocclusion was clinically assessed using the Dental Aesthetic Index (DAI). The data were tabulated and analysed using SPSS version 16. The Spearman rankorder correlation coefficient was employed to determine the correlation between self-perceived dental appearance and dental malocclusion. Of the 280 students, 41.8% were females and 58.2% were males. The mean OASIS score was 23.93 (±3.72), the VAS score was 69.61 (±15.78) and DAI 24.80 (±6.29). There was a negative correlation between perception of personal dental appearance and the DAI scores in this group.

Differences in facial profile and dental esthetic perceptions between young adults and orthodontists

The principal aim of this study was to investigate differences in perception of soft-tissue facial profiles and dental esthetics between young Chinese adults and orthodontists. Eight hundred ninety-two subjects (444 male, 448 female), ages 16 to 24 years, chose 1 image from among 5 profile silhouettes and from among 10 ranked color photographs of the aesthetic component (AC) of the index of orthodontic treatment need that most closely resembled their own profile and dental esthetic appearance, respectively. A panel of 20 orthodontists then independently repeated the same image selection process. Each subject also completed the Eysenck personality questionnaire for psychoticism. We used the Mann-Whitney U test and the Spearman rank correlation test, with statistical significance set at α= 0.05. Only 37.0% of subjects had straight profiles by objective orthodontic assessment, but 85.0% chose straight profiles by subjective self-perception. About 17.5%, mainly females, chose the mild convex as the ideal profile. Only 2.5% of the subjects were ranked 1 on the AC by orthodontists, but 43.6% chose 1, or ideal, by self-perception. Male subjects scored significantly higher than did female subjects for self-perceived facial profiles (more protruded chins) and for the AC (more attractive dental appearance). Subjects with high psychoticism Tscores (>50) scored significantly lower for self-perceived facial profiles (more retruded chins) and on the AC (less attractive dental appearance). Young Chinese adults perceived their facial profiles and dental appearances to be significantly more straight and attractive, respectively, than did the orthodontists. A significant proportion of the young adults, mainly women, preferred a mild convex facial profile. High psychoticism scores might significantly affect the self-perception of orthodontic treatment needs.

Comparison of dental esthetic perceptions of young adolescents and their parents

To compare dental esthetic perceptions of adolescents at age 13 with those of parents and to assess associations with dental fluorosis. Adolescents aged 13 underwent dental examinations for fluorosis on maxillary anterior teeth using the Fluorosis Risk Index. Adolescents and parents completed questionnaires concerning satisfaction with adolescents' dental appearance. McNemar and Bowker tests of symmetry were used for comparisons of esthetics ratings between parents and adolescents. Comparison of satisfaction between subjects with fluorosis and subjects with no fluorosis was made using Cochran-Armitage Trend and Fisher's Exact tests. From 376 adolescents included in the study, 26 percent had definitive fluorosis, mostly at a mild level. Fifteen percent of adolescents were dissatisfied with dental appearance, and concerns were mainly about color (45 percent) and alignment (35 percent). Adolescents were less satisfied with overall dental appearance (P < 0.001) and color (P = 0.048) and more concerned about tooth shape (P = 0.002) than were their parents. Fluorosis status (yes/no) was not significantly associated with adolescents' perceptions (P > 0.05). Parents of subjects with fluorosis were more dissatisfied with dental appearance (P = 0.014) and color (P < 0.001) than other parents. The number of maxillary anterior zones exhibiting fluorosis was negatively associated with adolescent (P = 0.03) and parent (P = 0.002) satisfaction. Adolescents generally had less satisfaction with dental appearance and color, and were more concerned with tooth shape than were parents. For both parents and adolescents, decreased satisfaction was associated with the number of zones with definitive fluorosis.

The perception of dental aesthetics and orthodontic treatment need by 10- to 11-year-old children

The aim of the study was to assess the perception of dental aesthetics and treatment need in 10- to 11-year-old children using the Aesthetic Component (AC) of the Index of Orthodontic Treatment Need. Subjects were asked to rank the 10 AC photographs in order from the one which looked the best set of teeth to the worst. They were also asked to say whether or not the teeth in each picture required orthodontic treatment. Three hundred and seventy-nine children completed the first task but only 369 were able to decide on treatment need for every picture. Girls ranked the pictures in the order 1, 3, 2, 4, 5, 6, 7, 9, 8, and 10; the boys' sequence was 1, 2, 3, 4, 5, 6, 9, 7, 8, and 10. Significant differences were found between girls and boys for the median rankings of photographs 2 (P < 0.02), 3 (P < 0.004), 5 (P < 0.03), 6 (P < 0.05), and 8 (P < 0.01). The sequence selected by the total sample was similar to that chosen by boys. The cut-off point for which photograph indicated a need for treatment was grade 4 (54.5 per cent), which was 34.7 per cent above the grade 3 score of 19.8 per cent. Three pairs of photographs were allocated similar median ranks, two and three received a rank of 3; five and six a rank of 6; and seven and nine a rank of 7. It is therefore possible that the number of AC grades could be reduced to the five photographs: 1, 4, 6, 8, and 10 in order to simplify the index without reducing its reliability. This premise could be tested by presenting firstly the five photographs and then the 10 on a separate occasion to see how the same participants rated the two sets of pictures.

The effect of axial midline angulation on dental esthetics.

The purpose of this study was to analyze the effect of various degrees of axial midline angulation on the attractiveness of a smile. We explored the influence of age, race, sex, direction of midline deviation, education, occupation, and dominant hand on each evaluator's perception of dental esthetics. Photographs of smiling subjects--one man and one woman--were altered to produce both left and right axial midline angulations in 5 degree increments. Fifty orthodontists and 50 laypeople evaluated these altered photographs by assigning both a numerical attractiveness rating and an acceptable or unacceptable rating to each. The results showed that attractiveness scores and acceptability ratings declined consistently as axial midline angulation increased. Statistical analysis showed that both sex of the subject and occupation of the judge were significant variables (P < .05) in the evaluation of the subjects. Age, race, sex of the judge, education level, direction of midline deviation, and dominant hand were not statistically significant. The mean acceptable midline angulation for the male subject was 6.6 +/- 4.5 degrees for orthodontists and 10.7 +/- 6.2 degrees for laypeople. For the female subject, the mean acceptable threshold was 6.4 +/- 4.0 degrees for orthodontists and 10.0 +/- 6.1 degrees for laypeople (P < .001). Discrepancies of 10 degrees were unacceptable by 68% of orthodontists and 41% of laypeople.

Patients' perceptions, treatment need, and complexity of orthodontic re-treatment

The aim of the present study was to investigate the subjective perception and objective treatment need and complexity of patients seeking orthodontic re-treatment. One hundred subjects (66 females, 34 males, age 26.7+/-8.2 years) seeking re-treatment were asked to complete a questionnaire which was constructed based on pilot interviews with 15 patients. The questions focussed on treatment experiences, retention procedures following the first course of treatment, and expectations of and motivations for re-treatment. A visual analogue scale (VAS 0-10) was used. The 're-treatment' group was matched with an untreated control group by age, gender, and the first consultation date. The study models of both groups were scored with the Index of Complexity, Outcome, and Need (ICON). Analysis of variance was used for across-time comparisons of VAS scores of patient's perception of their dental appearance, paired t-test for comparisons of the motivation VAS scores between the first treatment and re-treatment, and Mann-Whitney test for comparisons between the re-treatment and control groups. Eighty-eight patients (26.3+/-8.4 years) completed the questionnaire. After the initial treatment, 36 per cent of the patients did not have any retention measures. The mean VAS scores for dental aesthetics at the start and end of the initial treatment were 2.3+/-2.1 and 6.6+/-2.7, respectively. The scores for the present situation and expected results of re-treatment were 4.1+/-2.7 and 8.8+/-1, respectively. These scores differed significantly from each other. Seventy-nine pairs of models were matched for evaluation of treatment need and complexity. The mean ICON scores of the re-treatment group were significantly lower than the controls (45+/-21 versus 57+/-24), the aesthetic component being the main contributing factor to this difference (25+/-16 versus 36+/-18). Both groups showed a treatment need (ICON>43), with the untreated controls having a relatively higher complexity. These results indicate that patients seeking re-treatment had a good perception of dental aesthetics, strong motivation, and an objective treatment need.

Perceptions of dental esthetics of Asian orthodontists and laypersons

The aim of the study was to determine the impact of anterior dental crowding, overjet, overbite, dental spacing, and type of malocclusion on dental esthetic perceptions of Asian orthodontists. Twenty-one Asian orthodontists (10 men, 11 women) and 158 adult laypersons (38 men, 120 women) rated the attractiveness of 50 pairs of black-and-white intraoral photographic prints of various types of malocclusions. Each pair included a frontal and a right buccal view of the dentition at maximum intercuspation. Raters used a visual analogue scale (VAS), with "least attractive" (0 mm) and "most attractive" (100 mm) as the 2 extremes, to score the dental esthetics of the photographs. A principal investigator objectively measured the dental traits from study models. Linear regression analysis was performed with VAS scores as the dependent variable and anterior dental crowding, overjet, and overbite measurements as the independent variables. Independent sample t tests were used to determine the effect of dental spacing on the VAS scores. The Kruskal-Wallis test was used to determine the effect of type of malocclusion (Class I, Class II Division 1, Class II Division 2, Class III) on VAS scores. Increased overjet (>6 mm) and severe maxillary dental crowding (>8 mm) were found to be significantly correlated with laypersons' VAS scores (P <.05). Reverse overjet was found to be significantly correlated with orthodontists' VAS scores (P <.05). No significant correlation was found between overbite, dental spacing, and VAS scores. Class III malocclusions were significantly perceived to be the most unesthetic by orthodontists. Overjet was the major occlusal trait that influenced the perceptions of dental esthetics of both laypersons and orthodontists.

The value of the aesthetic component of the Index of Orthodontic Treatment Need in the assessment of subjective orthodontic treatment need.

Previous studies carried out using the Index of Orthodontic Treatment Need (IOTN) have reported that the Aesthetic Component (AC) has limited use in schoolchildren. The purpose of this study was to estimate whether dental concern expressed by the grade of the AC chosen by subjects is reliable and whether it may be predictive for potential co-operation. Such a correlation would indicate if the AC of the IOTN may help to identify individuals interested in orthodontic treatment who would co-operate well, and consequently who might derive the greatest benefits. The investigation was carried out in north-west Poland among 84 schoolchildren (42 girls and 42 boys) aged 12 years and was based on a questionnaire and clinical examination. The questionnaire contained items relating to the subjective assessment of dental appearance, demand for orthodontic treatment, the influence of the dentition on the general appearance, and any functional disorders (speech, mastication, muscular pain, etc.). Clinical examination was carried out at the schools each time by the same dentist. For statistical analysis chi-square (Yates corrected) and McNemar tests were used. A probability at the 5 per cent level or less (P < 0.05) was considered statistically significant. The outcome shows that the AC of the IOTN moderately reflects the subjective perception of dental aesthetics and demand for orthodontic treatment. The results indicate that using professional rating the AC scale does not seem to be more precise or reliable than self-evaluation. The correlation between dental concern and the AC would be higher if the 'no treatment need' category was split into two parts (e.g. 1-2 'no need', 3-4 'slight need') or the 'borderline need' category was moved two grades lower. The AC would then help to identify patients interested in treatment who would potentially be co-operative.
