Overview of Medication Administration to Hypertension Patients at Harapan Raya Community Health Center

Hypertension or high blood pressure is an increase in systolic blood pressure of more than 140 mmHg and diastolic blood pressure of more than 90 mmHg in two measurements with an interval of five minutes in a state of sufficient rest/calm. This research is a descriptive observational study. The research was conducted at Harapan Raya Community Health Center in April 2024. The sample in this study was hypertension patients who received treatment at Harapan Raya Community Health Center in March 2024. The instrument used in this research was secondary data from the monthly report of Harapan Raya Community Health Center in March 2024. The data used obtained and then analyzed by calculating the percentage of drug use. Based on the results at the Harapan Raya Community Health Center, it can be concluded that: Patients who consume a lot of hypertension medication are female with 75 respondents and 39 male respondents. The highest number of patients who took hypertension medication were in the elderly age range, with 68 respondents, 44 respondents for middle adults, and 2 respondents for early adults. The drug that is often used in community health centers is amlodipine 10 mg.

The Usage Of Government Formulary For BPJS Inpatients With Ischemic Stroke In Indonesia

Introduction: Ischemic stroke patients tend to use some medicine. In National Health Security (JKN), the government has created a government formulary as quality control and cost control. The study aims to determine the percentage of government formulary in Hospitals and the affected factors. Method: It was cross sectional quantitative research. It used retrospective data from medical record and prescription of ischemic stroke inpatients from January to June 2019. The data collected include hospital characteristics, patient characteristics, supporting characteristics and treatment costs. Result: The data cames from 17 hospitals. Based on the inclusion and exclusion criteria, 257 medical records were analyzed. The average percentage of drug use was 73.22%. Its was influenced by the hospital class, the number of supporting examinations, the number of drug items. Type B and C hospitals used non government formulary. There were citicolin, mecobalamin, piracetam, neurotropic multivitamin, flunarizine. Conclusion: The average percentage of drug use was still less than 100%. There were still non government formulary drugs used for the therapy of inpatient ischemic stroke. The use of non government formulary drugs was influenced by the hospital class, the number of supporting examinations, the number of drug items.

Evaluasi Peresepan Pada Pasien Rawat Jalan Geriatri Hipertensi Berdasarkan Beers Criteria Poli Penyakit Dalam Di RSUD dr. Soekardjo Kota Tasikmalaya

Hypertension is a condition that experiences an abnormal increase in blood pressure, the elderly are said to be hypertensive if their blodd pressure reaches ≥130/80 mmHg and is a non-communicable disease. A study has been carried out regarding the Evaluation of Prescribing in Outpatient Geriatric Hypertension Based on the Beers Criteria for Internal Medicine at RSUD dr. Soekardjo City of Tasikmalaya using a descriptive method of retrospective medical record data of hypertensive geriatric patients. This study aims to determine the treatment profile and the percentage of drug use included in the 2019 beers criteria list. The results showed that the most commonly prescribed treatment combination in this study was the combination of the ACE Inhibitor drug class (Ramipril) and the CCB drug group (Amlodipine), and Inappropriate drug prescribing in geriatric patients with hypertension in the outpatient installation of RSUD dr. Soekardjo City of Tasikmalaya in the 2020 period is still high with the number of inappropriate prescriptions as many as 47 (67,1%) prescriptions classified as PIMs (Potentially Inappropriate Medication). So it can be concluded that the high mismatch of therapy can be caused becaused because patients receive more than one drug that is not suitable for geriatric when getting drug prescriptions and to reduce these incidents it can be done by increasing the selection os safe drugs in geriatric using the 2019 beers criteria

Effects of the COVID-19 Pandemic on Drug Abuse Among Inmates in Kosovo Prisons

Background: The COVID-19 pandemic has led to an increase in drug use among inmates in prisons worldwide, including Kosovo, due to restrictions on visitation and addiction recovery programs. Objectives: This study analyzed drug use among 33,144 inmates in Kosovo prisons over nine years and aimed to identify potential solutions, foster collaboration between relevant institutions, and develop better programs and treatments. Methods and Results: The study is based on data from the medical files of inmates examined over nine years (2014-2022) in 10 Kosovo prison health units. From the total number of inmates, 8.22% were identified as drug users, and the percentage of drug use among inmates steadily increased from 5.8% in 2014 to 15.48% in 2019. In 2020, the first year of the pandemic, there was a decrease of 1.16% (from 15.48% to 14.28%), which was the first decrease in six years. The study also found a disproportional decrease in the number of inmates admitted during the same period. The Medico Psycho Therapeutic Center “Labyrinth” showed a similar trend. Suicide attempts, death under custody, and tobacco use increased linearly, with alcohol use remaining stable. Conclusion: The study highlights the need for effective prevention, treatment, and management strategies for drug use among inmates and the general population in Kosovo. Continuous collaboration between relevant institutions and stakeholders is essential to improve the health and well-being of those affected by drug use and create a brighter future for Kosovo.

Evaluation of morphine, methadone, digoxin, and dronabinol poisoning during the years 2017 to 2019 in Ilam, Iran.

Every year, drug poisoning is the most prevalent reason for referring patients to medical centers. This study aimed to evaluation of morphine, methadone, digoxin, and dronabinol poisoning in Shahid Mostafa Khomeini Hospital in Ilam. In this In this Cross-sectional study, patient samples suspected of morphine, methadone, digoxin, and dronabinol poisoning referred to the toxicology laboratory of Ilam University of Medical Sciences were analyzed using the HPLC method, and the results were analysed using SPSS software. Results showed that the percentage of drug use is greater in men than in women. The highest percentage of morphine and methadone poisonings were detected in those under the age of 40, whereas the highest percentage of digoxin poisonings were recorded in those over the age of 80. As a result, the average age of digoxin users was substantially greater in men than in women. Methadone consumers showed significantly greater blood levels than others. In addition, there was a significant difference (P<0.01) in blood levels between men and women who used morphine. In general, it is important to understand the status of drug poisoning with drugs such as morphine, methadone, digoxin, and dronabinol, as well as the prognosis associated with the treatment process of such poisoning.

PROFIL PERESEPAN OBAT HIPERTENSI PADA PASIEN RAWAT JALAN DI PUSKESMAS BETOAMBARI PERIODE JANUARI – JUNI TAHUN 2020 DI KOTA BAUBAU

Hypertension or high blood pressure is a condition in which blood vessels have persistently elevated pressure. This research method is done by collecting data in the form of secondary data. Secondary data was obtained from the Pharmacy section to find out the data on the number of patient visits. The data taken in the form of age group, gender, type of drug, drug class and drug combination. The results showed that the number of prescriptions obtained from January to June was 174 prescriptions and the highest percentage of drug use in hypertension patients based on gender was 115 prescriptions (66.91%) women, namely in the age group 50 years, 96 prescriptions were used. (83.47%), the use of drugs based on Hypertension drugs that was most widely prescribed was Amlodipine with 144 prescriptions (78.68%), the most prescribed group of Hypertension drugs was Calcium Antagonists with 144 (83.75%) prescriptions The most combined drugs were Amlodipine + Captopril with 18 prescriptions (58.06%)

Profile of Drug Prescribing in Outpatient Heart Disease Patients at Mitra Medika Bondowoso Hospital

The heart is the most important organ in the body, because there are no cells and tissues that can function properly without adequate oxygen and blood supply.This study aims to determine the percentage of drug use and the number of drugs used as a reference in the process of planning and drug procurement. Sampling was determined by total sampling technique. The sample used was 279 prescriptions for outpatient heart disease at Mitra Medika Bondowoso Hospital for the period March 2021.The results of research conducted on a sample of 279 prescriptions for outpatient heart disease patients at Mitra Medika Bondowoso Hospital for the period March 2021, based on the name of the most prescribed drug, 153 prescriptions for candesartan with a percentage (14.35%), based on the number of drugs used. The most widely used was clopidogrel with a total usage of 4,231 tablets. Then based on the class of drugs the most prescribed was the diuretic group with 261 prescriptions with a percentage (24.68%), based on the strength or dose of the drug the most widely used was clopidogrel with 144 prescriptions with a percentage (13.51%).

EVALUASI PENGGUNAAN OBAT ANTIDIABETIK ORAL PADA PASIEN DIABETES MELITUS TIPE 2 MENGGUNAKAN METODE ATC/DDD DAN DU 90%

Diabetes mellitus is a chronic metabolic disease caused by the pancreas not producing enough insulin or not being able to use the required insulin effectively. Evaluate the use of oral antidiabetic drugs using ATC Diabetes mellitus is a chronic metabolic disease caused by the pancreas not producing enough insulin or not being able to use the required insulin effectively. Oral antidiabetic used in Makassar Hospita Hospital there are 3 types of metformin as many as 1020 units with a dose per unit that is 500 mg with a total use of 601000 mg 99.56%, more than 48 units with a perunit dose of 60 mg with a total dose of 2520 mg 0 , 41%, the last is 41 units of glimepiride with a perunit dose of 2 mg with a total use of 82 0.01%. Of the 3 types of oral antidiabetic, 3 usage patterns were obtained, namely 84% single metformin, 9% metformin + gliclazide combination pattern and 7% metformin + glimepirid combination. Included in the 90% DU segment or included in the list of incoming drugs is collected 90% of drug use after being sorted from the highest and highest percentage of drug use for compilation and low use is the use of metformin, using the correct use of drugs according to the choice, right dose, right delivery time, exact route and right information.

Gambaran Pengobatan Diare Akut Anak di Puskesmas Jiwan Madiun

Diarrhea is a digestive tract disease with a consistency of loose stools and an increased frequency. The morbidity and mortality rates for children diarrhea in Indonesia still tend to be high. This study aims to determine the description of pharmacological therapy in the treatment of outpatients with acute diarrhea in Jiwan Public Health Center, Madiun by referring to the management of diarrhea therapy from the Ministry of Health. This research was conducted in a descriptive observational manner with retrospective data collection on outpatients with acute diarrhea at the Jiwan Public Health Center, Madiun in the Januri-June 2019 period. Sampling used a purposive sampling technique that met the inclusion criteria. The type of data used in this study is secondary data taken from patient medical records. Data analysis of the results of the study was carried out descriptively to determine the demographic distribution and treatment of patients with acute diarrhea in children at the Jiwan Public Health Center, Madiun. Most of the treatments had met the management of diarrhea. The percentage of drug use included using ORS as much as 29%, antidiarrheal attalpulgite and kaolin pectin by 26%, zinc 25%, anti-vomiting used by domperidone 7% and metochlopramide 4%, cotrimoxazole antibiotics 49% of the total cases 100% (75 cases). Evaluation of the dose of co-trimoxazole and zinc, there were 10% and 75% incorrect doses.

Therefore, this study aims to determine the percentage of drug use of antidote category indicated as an antidote therapy in Undata hospitals Central Sulawesi Province period 2016-2018. This is a descriptive study with retrospective approach and using purposive sampling techniques.The results showed that the sample size was 75 patients, with the percentage of drug use as antidotes category in Undata Hospital Central Sulawesi province period 2016-2018, namely pyridoxine (vitamin B6) of 49.33%, N-acetylcysteine drug of 42.66% and the atropine drug of 8%. The use of antidotes drug category are indicated as an antidote to the poisoning therapy drugs obtained 0%. It can be concluded that there is no antidote drug category that used as an antidote therapy on patients with drug poisoning in inpatient instalation Undata hospital Central Sulawesi Province Period 2016-2018.

INFLUENCE OF DOSAGE ON RETENTION IN PATIENTS METHADONE MAINTENANCE THERAPY PROGRAM FOR MAINTENANCE PHASE OF H. ADAM MALIK MEDAN CENTRAL GENERAL HOSPITAL

Objective: This study aims to see the effect of methadone dose on patient retention in following the program.Materials and Methods: This study is a clinical study with cross-sectional study design. Data obtained from the patient’s medical record and patient’s primary data. The study was conducted at Methadone Clinic of H.Adam Malik Hospital Medan. Results: The number of research samples is injecting drug users who follow PTRM as many as 39 people. From a study of 39 patients, the percentage of male patients (100%) was obtained; age between 25 and 35 years (64.1%); junior high school education equal to 84.6%; already married 46.2%; already working 69.2%; and the distance of the average residence with the clinic is 16.93 km. The results showed that the smallest maintenance dose of maintenance phase was 2 mg; the largest maintenance dose in the maintenance phase was 165 mg; the mean maintenance stage dose of 62.35 mg with a retention value &gt;2 years or more (&gt;730 days) was 79.5%.Conclusion: There was a significant effect on retention which is the biggest maintenance dose with p=0.04 value and take home dose with p=0.027. Percentage of drug use and other substances that is benzodiazepines (BZO) (64,1%); amphetamines (AMP) (38.5%); methamphetamine (MET) (20.5%); and THC (20.5%). There was no significant effect between the use of other substance to retention with p value of BZO (0.389); p AMP (0.360); p MET (0.195); and p THC (0.470). It can be concluded that the greater the dose of methadone has an effect on its retention.

Analysis of characteristics of traditional Chinese medicine and western medicine clinical use in patients with viral hepatitis based on real world hospital information system data

Viral hepatitis is clinical multiple strong infectious disease, to know characteristics of traditional Chinese medicine and western medicine clinical use in patients with viral hepatitis, the research object of this study is 41 180 cases of hospitalized patients with viral hepatitis in hospital information system from 17 grade A hospitals, using frequency statistics and association rules method to analyze the traditional Chinese medicine and western medicine clinical use information, the drug kinds analysis results: western medicine of reduced glutathione tablets use frequency is highest, 14 079 cases (34.61%), traditional Chinese medicine of diammonium glycyrrhizinateuse frequency is highest, 14 058 cases (34.56%); traditional Chinese medicine and western medicine drug combination in diammonium glycyrrhizinate combined with reduced glutathione tabletsuse frequency is highest, 8 607 cases (25.09%). The mechanism of drug classification results :both traditional Chinese medicine and western medicine are the sort of educed enzyme medicine that has the highest percentage of drug use, traditional Chinese medicine 10 983 cases (27.01%), western medicine, 9 595 cases (23.59%); traditional Chinese medicine and western medicine combination in a kind of medicine to clear heat and promote diuresis combined with educed enzyme drug use frequency is highest, 5 621 cases (13.82%). Through the analysis above, combine traditional Chinese and western medicine therapy for the treatment of viral hepatitis should be given priority. Descripción clínica y tratamiento de los pacientes con síndrome coronario agudo

Antecedentes: el síndrome coronario agudo es una emergencia clínica que se manifiesta principalmente por dolor torácico. Su diagnóstico representa un desafío clínico y su reconocimiento temprano es fundamental para el tratamiento. Objetivo: describir las características clínicas, identificar abordajes terapéuticos y conocer los desenlaces de los pacientes con diagnóstico definitivo de síndrome coronario agudo hospitalizados por el servicio de medicina interna y cardiología entre el 1° de enero de 2009 hasta 31 de diciembre de 2010 en el Hospital San José. Métodos: se realizó una descripción de una cohorte de pacientes con diagnóstico de síndrome coronario agudo (infarto miocárdico con y sin elevación del segmento ST y angina inestable). Se analizaron variables demográficas, características clínicas, tratamiento y desenlaces a los cuales se calcularon promedio, desviación estándar y proporciones según la naturaleza de las mismas. Resultados: se reportan 133 pacientes. El 63% correspondió a hombres, el promedio de edad fue de 64.8 años. La frecuencia de eventos fue 45.9% IMEST, 39% IMSEST y 15.1% anginas inestables. El 96.2% de los pacientes consultó por dolor torácico, 50.7% presentaron dolor en las primeras seis horas de evolución. El 93% recibió betabloqueador, 88% IECA, 87% clopidogrel y 97% ASA. Se trombolizó al 82% de los pacientes en ventana. La mortalidad intrahospitalaria fue del 8.7%. Conclusiones: en la población estudiada predominó el síndrome coronario agudo sin elevación del segmento ST. La mayoría de pacientes consultaron por dolor torácico retroesternal, en las primeras horas de inicio. El porcentaje de uso de los medicamentos fue adecuado. La reperfusión inmediata en pacientes con IMEST fue alta, sin embargo aún tenemos pacientes que no se benefician de esta terapia y el uso de angioplastias primarias fue muy bajo.

The control of asthma in Italy. A multicentre descriptive study on young adults with doctor diagnosed current asthma.

Few data are available on the management of asthma in the general population. The aim of this study was to evaluate the level of asthma control in Italian patients, a decade after the publication of the international guidelines. Within the framework of a multicentre, population-based study on people aged 20-44 years, 18 873 subjects replied to a postal screening questionnaire (response rate = 72.7%) on the presence of asthma symptoms and exacerbations, and their impact on daily life. All subjects reporting having ever had a doctor diagnosis of asthma and either taking medicine for asthma when interviewed or having had an asthma attack in the last 12 months, were considered current asthmatics. Of the screened subjects, 649 (3.4%) were current asthmatics. Of these, only 14% did not report exacerbations or had been symptom-free in the last 12 months. More than 20% of current asthmatics had their daily life activities seriously impaired and 54 patients (8%) had at least one hospital/emergency department admission as a result of asthma exacerbation in the last 12 months. The life impairment and the rates of hospitalization significantly increased as the control of the symptoms worsened. The use of asthma drugs was quite common in current asthmatics: 586 (90.2%) reported having been under pharmacological treatment in the last 12 months. Only 63 patients (10%) had the disease under control (neither symptoms nor life impairment): they had a significantly higher percentage of drug use (100%vs 89%) and of daily use prescriptions (50%vs 36%) than non/moderately controlled asthmatics. Poorly controlled asthmatics had a significantly higher percentage of women (63%vs 44%), of patients with the coexistence of chronic cough and phlegm (47%vs 30%) than moderately/well-controlled asthmatics. The comparison between our data and similar data collected in 1991 in Italy showed that the use of asthma drugs increased by about 12%, while the control of symptoms did not improve in the last decade. Despite the high percentage of drug users, the control of symptoms and exacerbations was overall poor in Italy and resulted in a heavy individual and social burden, pointing out that the guideline goals have far from been reached in Italy.

Problems and Solutions for Estimating the Prevalence of Drug Abuse among Arrestees

The Drug Use Forecasting program is an important national data set that estimates the prevalence of psychoactive drug use among recent arrestees. However, because provision of urine samples is voluntary and many arrestees refuse to provide specimens, use of the urinalysis results in estimating prevalence of use may lead to inaccurate estimates. This paper provides a selection model to estimate drug use among those who refuse to take urine tests. The results indicate that those arrestees who refuse urine testing generally have a higher percentage of drug use than those who agree to take the test. The applications of this model allow an overall estimation of the extent of drug usage for all arrestees.
