Analysis of the Completeness of Medical Records in RSJ. Prof. Dr. Saanin Padang

Complete medical records are very useful for knowing in detail the patient's history, examinations that have been carried out, and planning further actions. To achieve this, the medical records department is obliged to carry out quality monitoring by carrying out quantitative, qualitative, and statistical analysis. To know whether a medical record file is complete or incomplete, it is necessary to carry out a quantitative analysis, namely four reviews consisting of an identity review, recording review, reporting review, and authentication review. Based on the results of a preliminary study with existing medical record officers, it was stated that every patient's medical record file received from inpatient care was always assessed for completeness but was still not optimal, and incomplete values ​​were always obtained. One of the problems, among others, is in terms of input, namely the high workload of medical record officers, facilities, and infrastructure, especially storage space and technology, which are not yet optimal. It is important to discuss this problem so that researchers research to find out the completeness of medical record files in RSJ. Prof. HB. Saanin Padang City. From the study, it was found that the highest percentage of completeness in filling out identification components was the name item at 97.98%, the highest percentage of completeness in important report components was found in the discharge summary form, CPPT and General consent each at 100%, the percentage of completeness in the recording method component was highest in You can use item type x at 100%. In the medical record file, no type-x was used. There is a need for policies regarding filling rules and periodic audits regarding the completeness of medical record files. Pengembangan Media Pembelajaran Go Puzzle Pada Mata Pelajaran Pendidikan Pancasila dan Kewarganegaraan untuk Siswa Sekolah Menengah Pertama

This study aims to develop and test the validity, attractiveness, effectiveness, and applicability of Go Puzzle learning media in the subjects of Pancasila and Civic Education. The procedure used is research and development based on the steps proposed by Borg and Gall. The products produced in this development study are Go Puzzle learning media material for the Formulation and Determination of Pancasila as a State Basis that is valid, interesting, effective, and can be applied in learning. The product validity test is based on the results of material expert validation of 92.78 percent, media expert validation of 91.42 percent, expert validation of learning implementation plans of 95.63 percent, and field learning expert validation of 98.21 percent. The product attractiveness test is based on the results of small group trials of 90.36 percent and the results of large group trials of 93.46 percent. The product effectiveness test is based on the results of large group trials which resulted in an average completeness of students’ scores when pretest was 20.89 percent and post test was 86.89. This shows that there is an increase in the percentage of completeness of scores obtained by students, which is 66 percent. The product applicability test is based on the educator response questionnaire, which is 96.88% which shows that the product developed can be applied in learning. Development of Animation Video Learning Media to Improve Class XI Science Students’ Understanding

This research and development (R&D) aim to produce valid, practical and effective animated video learning media. The research subjects are divided into two parts, namely the first part of the product design validation subject or expert test conducted by 2 material experts and 1 media expert and the second part of the product trial subject conducted by testing the learning media in class XI IPA 3 and 2 class XI teachers of SMA Negeri 1 Gowa as observers. This learning media has been validated, has been tested limitedly and revised. The results of this study are: (1) validation by media experts of 3.4 with very valid criteria and (2) validation by material experts of 3.79 with very valid criteria. Animated video media based on the implementation test, teacher and student responses with an average percentage score of 88%, 86.11% and 86% respectively. For animated video media, the effectiveness criteria are in the very high category, with student learning outcomes meeting the effective criteria (≥80%), where the class completion percentage is 88.8%. Animated video learning media developed using the ADDIE model is suitable (valid, practical and effective) for use in learning.

Analysis of Revenue Recognition Based on PSAK Number 23: Case Study In Trading Company PT. Solusindo Ganda Kharisma

This study aims to analyze the implementation of revenue recognition based on the Statement of Financial Accounting Standards (PSAK) No. 23 in the trading company PT. Solusindo Ganda Kharisma. In an increasingly competitive business environment, companies are required to implement an effective accounting information system to produce accurate and timely financial information. Revenue is one of the key elements in financial statements, where its recognition and measurement affect the quality of the information presented. This research uses a descriptive qualitative approach. Primary data were obtained through interviews with relevant parties in the company, while secondary data were sourced from the company’s financial statements, internal accounting policies, and other documents. The results of the study indicate that PT. Solusindo Ganda Kharisma has effectively implemented PSAK Number 23 in the recognition of revenue from the sale of goods and services. Revenue from the sale of goods is recognized when the goods have been received by the customer, while service revenue is recognized based on the percentage of work completion. However, several challenges remain, such as delayed revenue recognition due to late delivery of goods and discrepancies in estimating the percentage of work completion for service sales.

Improving data quality by implementing an electronic medical record seems to depend on the context

Abstract Background The European healthcare system is reliant on its digital transformation to deal with challenges like rising expenditures or workforce shortage. The digital transformation is inevitably accompanied by the implementation of electronic medical records (EMR) and the ongoing adaptation of existing ones. Systematic reviews indicate that this process can have an impact on medical records data quality (DQ) [1]. At micro level, DQ is essential to ensure high quality of care. At macro level, sufficient DQ is a prerequisite for big data analyzability. In this field, completeness is a commonly analyzed dimension of DQ and empirical results indicate that completeness can improve but also deteriorate as a result of the described implementation or adoption of EMRs [2]. The aim of this work was to investigate the implementation of EMRs in comparable settings and to observe and discuss possible differences in the change in DQ. Methods Data was collected on three surgical clinics of a German academic teaching hospital before and after the implementation of an EMR. Paper-based and electronic medical records were compared. Analysis focused on ten items that were commonly documented in both record types (e.g. pain). T-tests and χ²-tests were used to compare average completeness per record type and percentage of completeness per item. Results A total of N = 659 records was analyzed. Overall, results show a significant improvement in completeness from an average of 6.0/10 items in the paper-based record type to 7.2/10 in the EMR (p<.05). At clinic level, improvement rates vary from 0.9 to 1.4. At the level of the specific items, significant deteriorations are visible in certain clinics. Conclusions Results suggest that DQs variability is context-dependent (e.g. on the clinic’s turnover rate or its patient’s length of stay). Due to the unavoidable digital transformation, a detailed context and needs analysis involving all stakeholders should be carried out before any changes are made. Key messages • The application of advanced analytics such as big data or AI training is reliant on the availability of high-quality datasets. • Electronic medical records have been demonstrated to enhance data quality, but it remains uncertain how and why improvements appear to be context-dependent.

Which videos are better for the students? Analyzing the student behavior and video metadata

Due to its inherent flexibility and adaptability, e-learning has become a versatile and effective educational option, with video-based learning (VBL) enabling asynchronous engagement. Despite the wealth of video resources, evaluating their effectiveness remains challenging. This study employs a comprehensive approach, analyzing student behavior and video metadata to assess educational videos’ efficacy. Metrics such as average playback speed, video and course completion rates, and video metadata were examined using the XuetangX dataset, MOOC- CubeX. Results indicate that viewing patterns are more influenced by students’ choices than by video content. Well-conceptualized videos increase viewing time, with 17 % of students engaging with videos over 30 min if they cover focused topics. While shorter videos generally have higher completion rates, many remain untouched. Playback speed analysis shows a preference for standard speed, with positive correlations between speed and both completion percentage and viewing time. Courses under 5 h are likely to be watched for less than an hour, reflecting brief viewing durations. Metadata analysis reveals that generic titles may deter engagement, underscoring the importance of precise tagging and content relevance for enhancing student interaction.

Penerapan Strategi Pendekatan Culturally Responsive Teaching (CRT) dalam Pembelajaran Berdiferensiasi untuk Meningkatkan Hasil Belajar Matematika Peserta Didik Kelas XII-5 SMAN 6 Surabaya

This research aims to improve the mathematics learning outcomes of students in grades XII-5 of SMAN 6 Surabaya by applying the Culturally Responsive Teaching (CRT) approach and the Problem Based Learning (PBL) model in differentiated learning on functional transformation materials. The CRT approach used emphasizes cultural integration in mathematics learning to facilitate student engagement according to their cultural context and background. This study uses the Classroom Action Research (PTK) method which is carried out in two cycles. Each cycle consists of stages of planning, action, observation, and reflection. The subjects of this study are 36 students in grades XII-5 of SMAN 6 Surabaya. Data was collected through learning outcome tests and observations that were analyzed using quantitative and qualitative analysis methods. The results of the study showed a significant improvement in student learning outcomes. In the pre-cycle, the percentage of student completion was 22% with an average score of 62.6. In the first cycle, the completeness increased to 91.6% with an average score of 90.7, and in the second cycle the completeness reached 97.22% with an average score of 96.25. In addition, student activity in group discussions also increased during the implementation of cycle II. The conclusion of this study is that the application of the Culturally Responsive Teaching approach in differentiated learning is effective in improving the mathematics learning outcomes of students in grades XII-5 of SMAN 6 Surabaya, especially in the material of functional transformation. The integration of cultural context in learning is proven to be able to increase student motivation, engagement, and understanding of learning materials.

Peningkatan Kemampuan Kolaborasi Pembelajaran PAK BP Metode PBL Berbantuan Video Kelas VII SMP Negeri 3 Juhar Tahun 2024/2025.

Collaboration Ability is one of the skills of the 21st Century which is believed to have an influence on learning success, but often this skill is simply missed in learning This research aims to increase student collaboration in Catholic Religious Education subjects in grade VII-1 SMP NEGERI 3 Juhar by using the Problem Based Learning learning model. This research is a Classroom Action Research that aims to overcome problems in the classroom. The subject of the study is students in grades VII-1 of SMP NEGERI 3 Juhar who are Catholic for the 2023/2024 school year, totaling 8 students. The research is carried out in two cycles and at the end of each cycle is reflected on the actions given. The data collection technique in the study uses observation sheets, and documentation. The results of the study show that the implementation of learning using the video-assisted Problem Based Learning learning model in Catholic Religious Education subjects can improve students' collaboration skills. This is shown from the results of the percentage of completeness which was initially only 41.7% with the proficient category in the first cycle to 58.3% with the advanced category in the second cycle.

Meningkatkan Hasil Belajar PAK Dengan Metode Problem Based Learning Fase C Kelas 6 SDN 13 Nyayum

This study discusses the improvement of learning outcomes of Catholic Religious Education (PAK) and students by using the Problem Based Learning method at SD Negeri 13 Nyayum, Landak Regency. The research subjects were grade VI students with a total of 16 students. The subject matter used was “I am a citizen of the world”. For data collection methods researchers use observation, assessment, and tests. The aspects of learning outcomes in this study are affective, cognitive, and psychomotor. Before the implementation of the study, the percentage of completeness or achievement of PAK learning outcomes was so low, namely 32% with KKTP 75. After research in cycle I, the completeness of learning outcomes increased to 81.25% with proficient criteria for 8 students, proficient for 5 students, and 3 students who were not complete with proficient criteria. Then at the end of cycle II the completeness increased to 100% with 88% proficient criteria (14 learners) and 12% proficient criteria (2 learners). From the data on the results of research in activities, it can be concluded that the Problem Based Learning Method succeeded in improving the learning outcomes of Catholic Religious Education and Grade VI students of SD Negeri 13 Nyayum.

Peningkatkan Kemandirian Belajar Siswa Dalam Mata Pelajaran Pendidikan Agama Katoik Dan Budi Pekerti Dengan Model Problem Based Learning Materi Manusia Sebagai Citra Allah Fase D Kelas VII Smp Setya Budi Nanga Pinoh

This research aims to improve the learning outcomes of grade VII students of SMP Setya Budi Nanga Pinoh for the 2024/2025 school year in the subject of Catholic Religious Education through the application of the Problem Based Learning (PBL) learning model. This type of research is Classroom Action Research (PTK) which is carried out in two cycles with 2 meetings each cycle. Each cycle consists of several stages, namely planning, implementation of actions, observation and reflection. The subject matter discussed was "I am the Unique Image of Allah". The subjects in this study are grade VII students of SMP Setya Budi Nanga Pinoh for the 2024/2025 school year consisting of 28 students. The data collection technique is in the form of test results carried out at the end of cycle I and cycle II, observation results during learning in cycle I and cycle II. The results of the study show that the application of the Problem Based Learning (PBL) learning model can improve the learning outcomes of grade VII students of SMP Setya Budi Nanga Pinoh starting from cycle I and cycle II. In the first cycle, the percentage of student learning completeness was 80%, and in the second cycle, the percentage of completeness reached 100%. The conclusion of this study is that the application of the Problem Based Learning (PBL) learning model can improve the learning outcomes of Catholic Religious Education students in grade VII of SMP Setya Budi Nanga Pinoh.

Meningkatkan Hasil Belajar PAKBP pada Materi Manusia Berkembang Berkat Peran Sesama Menggunakan Model Problem Based Learning di Kelas VII SMP Negeri 1 Batang Kuis

This study aims to improve the learning outcomes of seventh-grade students on the material "Human Development Through the Roles of Others" using the Problem-Based Learning (PBL) model at SMP Negeri 1 Batang Kuis. This research employed a Classroom Action Research design conducted in two cycles. Each cycle consisted of four stages: planning, action, observation, and reflection. The research subjects were 7 students from the seventh-grade class at SMP Negeri 1 Batang Kuis. Data collection techniques were carried out through tests (formative tests) and non-tests (observations). The results of the study in cycle I showed an average score of 68.14 and a classical learning completeness percentage of 72.42%. In cycle II, the average score increased to 84.85. The average student attendance in cycle I was 100% and remained 100% in cycle II. Student involvement in learning in cycle I was 66.28% (high) and increased to 76.50% (very high) in cycle II. It can be concluded that the PBL model can improve the learning outcomes and activities of students on the material "Human Development Through the Roles of Others" in the seventh-grade class at SMP Negeri 1 Batang Kuis.

Peningkatan Hasil Belajar Siswa pada Pembelajaran Pendidikan Agama Katolik (PAK) Materi Aku Pribadi yang Unik melalui Metode Problem Based Learning (PBL) di Kelas III SDN 071163 Siwalubanua Tahun Pelajaran 2024/2025

This research aims to improve the learning outcomes of 3rd grade elementary school students in Catholic Religious Education subjects, especially the material "I am a Unique Personality", through the application of the Problem Based Learning (PBL) method. This Classroom Action Research (PTK) was carried out in two cycles with 5 students as research subjects. The research results showed an increase in the percentage of learning completion from 40% in the pre-cycle to 80% in the second cycle, as well as an increase in student learning activities from 60% to 100%. The application of PBL has proven to be effective in improving students' conceptual understanding, critical thinking abilities and problem solving skills in a small class context.

Penerapan Model Pembelajaran SETS (Science, Environment, Tecnology and Society) untuk Meningkatkan Hasil Belajar PLH

This research aims to improve student learning outcomes in Environmental Education (PLH) subjects through the application of the SETS (Science, Environment, Technology and Society) learning model. The research subjects were grade 2 students at SDN 221 Babakan Sentral. This study uses a classroom action research method which is divided into two stages. Each stage includes four steps: plan, implement, observe, and evaluate. Information is collected by observing and testing learning outcomes. The research results show that the application of the SETS model can improve student PLH learning outcomes. This increase can be seen from the increase in the average class score and the percentage of student learning completion from cycle I to the next cycle. Apart from that, the SETS model also increases students' active participation in learning and their ability to relate PLH concepts to science, technology and society. In conclusion, the SETS learning model is effective in improving learning outcomes and students' understanding of PLH material.

Peningkatan Gerak Dasar Lokomotor dengan Sirkuit Game pada Siswa Kelas II SD Negeri 8 Pakis Surabaya

Games as an alternative that helps to improve locomotor movements in children because there are fun movement tasks, one of which is by applying circuit games in improving the basic locomotor movement skills of students in class II SDN 8 Pakis 8 Surabaya students experience an increase in locomotor movement skills (walking, running jumping), this research is a class action research where in this study using 2 cycles conducted for 2 meetings. The subjects in this study were class II C SDN 8 Pakis students totaling 20 students. The techniques used in data collection are tests, observation and documentation. Based on the results of the study, it shows that through circuit games it can improve the learning outcomes of locomotor basic movements (walking, running jumping) in class II SDN 8 Pakis Surabaya This can be seen from the percentage of completeness of learning outcomes in cycle 1 and cycle 2 increased, with a percentage of completeness of 50% in cycle 1 and a percentage of completeness of 97% in cycle 2.

Using Problem-Based Learning to Boost Social Studies Performance: Classroom Action Research in Grade 5 at SD INPRES 3 Talise

The role of education in developing effective problem solvers, critical thinkers, and responsible members of society is of great importance. This research deals with the low performance in social studies subjects caused by the continued application of conventional teaching models. The aim of this study is to improve the learning outcomes of grade V students in social studies subjects at SD INPRES 3 Talise by applying the Problem-Based Learning (PBL) model. This classroom action research was conducted in two cycles, following the Kemmis and McTaggart model, involving 22 students from grade V. Data were collected through tests, observations, and interviews, then analysed quantitatively and qualitatively. Initial observations revealed that only 36.36% of students achieved the learning objectives with an average score of 50.90. The implementation of PBL involved the collaborative planning, action, observation, and reflection phases. The results demonstrated significant improvements in student engagement and learning outcomes. The average score increased, and the percentage of students who achieved the learning objectives rose significantly, respectively from 63.63 in cycle I to 81.59 in cycle II. However, the average score did not yet reach the KKM. Furthermore, the percentage of completeness has also increased, namely in cycle I by 59.09% and cycle II by 86.36%. This research demonstrates that PBL can effectively enhance the quality of social studies education by fostering learning.

IMPROVING SOCIAL STUDIES LEARNING ACHIEVEMENT THROUGH THE APPLICATION OF E-COMIC LEARNING MEDIA

The aim of this research is to improve student learning achievement in social studies subjects through the application of e-comic learning media. The data collection method is a learning achievement test. The data analysis method is descriptive for both qualitative data and quantitative data. The results obtained from this research are that the application of e-comic learning media can improve student learning achievement. This is evident from the results obtained initially reaching an average value of 60.76, in cycle I reaching an average value of 70.51 and in cycle II reaching an average value of 79.74. The percentage of student learning completeness has also increased. At the beginning of learning, students' learning completion only reached 30.76%. In cycle I there was an increase to 82.05%, and in cycle II student learning completion increased to 100%. The conclusion obtained from this research is that the application of e-comic learning media can improve social studies learning achievement.

Feasibility and acceptability of a telehealth, individually-tailored healthy lifestyle intervention for adolescent and young adult pediatric cancer survivors.

Poor nutrition and physical activity pose negative health risks for adolescent and young adult pediatric cancer survivors (AYACS). Our pilot randomized controlled trial (RCT) evaluated feasibility and acceptability of a telehealth intervention (ENHANCE) supporting AYACS' nutrition and physical activity. We randomized 58 AYACS and their adult care partner, when applicable, to the ENHANCE or control condition. Inclusion required an age of 15-25 years, previous chemotherapy or radiation therapy, and no eating disorder history. We utilized a 2:1 (ENHANCE [n = 35]: control [n = 23]) double-blind block randomization protocol in blocks of 6. ENHANCE comprised 12 (60-min) sessions that included education, skill-building, and values-based healthy lifestyle decision-making. The first five sessions included motivational interviewing and the final seven sessions included problem-solving training/goal setting. We evaluated feasibility based on eligibility, enrollment, attrition, and study completion percentages. We assessed fidelity using the Session Content-Fidelity Rating Tool and the Motivational Interviewing Treatment Integrity Coding form. We assessed acceptability using the Abbreviated Acceptability Rating Profile, the Revised Helping Alliance Questionnaire, and participants' exit interviews. ENHANCE was feasible and highly acceptable; all but one participant who started the intervention completed all 12 sessions and all participants reported enjoying the intervention via objective assessment and during exit interviews. Interventionists delivered ENHANCE with high fidelity. ENHANCE was highly acceptable among AYACS and care partners. The telehealth study design was feasible, suggesting benefits and feasibility of a future, larger, multisite RCT. Promising findings support our methodological decisions as important considerations to inform a larger RCT.

Development of Discovery Learning-Based E-LKPD to Enhance Students' Learning Motivation and Achieve Critical Thinking Skills in Salt Hydrolysis Material

This study aims to evaluate the viability, practicality, and effectiveness of based on discovery learning (E-LKPD) in raising students' motivation to learn and cultivating their critical thinking abilities (CTS). This study adopts a quantitative methodology by utilizing the research and development (R&D) technique and the Borg and Gall model. Sixty-two students from SMA Negeri 1 Baitussalam's class XI make up the study sample. The average results of the viability test for the E-LKPD, reviewed from the aspects of material completeness, accuracy, currency, and alignment with objectives, obtained a percentage of 91.25% with the criterion of "very feasible." Meanwhile, the practicality of the E-LKPD based on the responses from students and teachers achieved average percentages of 87.38% and 82.5%, respectively, with the criterion of "very practical." The effectiveness assessment of the E-LKPD, based on non-test results of learning motivation with an N-Gain percentage of 0.56 and the students' CTS test results with a completeness percentage of 79.48, falls under the criterion of "quite effective." Therefore, it can be concluded that this discovery learning-based E-LKPD is viability, practicality, and effectiveness in enhancing learning motivation and achieving students' critical thinking skills (CTS).

Upaya Meningkatkan Hasil Belajar Siswa Kelas III Pada Materi Pancaindra Melalui Model Pembalajaran PBL Berbantu Media Flashcard

By using a problem-based learning (PBL) model supported by flashcard media, classroom action research (CAR) was conducted in two cycles, with observation, reflection, implementation, and planning. The aim was to improve student learning outcomes in the five senses material in grade III. The results showed that, in the first cycle, 44% of students achieved completion, with an average score of 68%; in the second cycle, the percentage of completion increased to 86%, with an average score of 82%. These results indicate that the application of the PBL model with flashcard media can help students understand the five senses material better. As a result, this learning approach can be used as an alternative to improve the quality of learning in elementary schools.
