Associations of maternal postpartum depressive and anxiety symptoms with 4-month infant and mother self- and interactive contingency of gaze, affect, and touch.

Maternal depression and anxiety are associated with infant and mother self- and interactive difficulties. Although maternal depression and anxiety usually co-occur, studies taking this comorbidity into account are few. Despite some literature, we lack a detailed understanding of how maternal depressive and anxiety symptoms may be associated with patterns of mother-infant interaction. We examined associations of maternal postpartum depressive and anxiety symptoms with infant and mother self- and interactive patterns by conducting multi-level time-series models in a sample of 56 Turkish mothers and their 4-month infants. Time-series models assessed the temporal dynamics of interaction via infant and mother self- and interactive contingency. Videotaped face-to-face interaction was coded on a 1s time base for infant and mother gaze and facial affect, infant vocal affect, and mother touch. Results indicated that mothers with high depressive symptoms were vulnerable to infants looking away, reacting with negative touch; their infants remained affectively midrange, metaphorically distancing themselves from mothers' affect. Mothers with high anxiety symptoms were vulnerable to infants becoming facially dampened and mothers reacted with negative facial affect. Altered infant and mother self-contingency patterns were largely opposite for maternal depressive and anxiety symptoms. These patterns describe foundational processes by which maternal postpartum mood is transmitted to the infant and which may affect infant development.

Mothers with and without bipolar disorder and their infants: group differences in mother-infant interaction patterns at three months postpartum

BackgroundWomen with bipolar disorder (BD) have a high risk of illness relapse postpartum. The risk coincides with the period when mother-infant interactions are evolving. We compared mother-infant interactions in dyads where the mothers have BD with dyads where the mothers have no mental disorder. The association between concurrent affective symptoms of BD mothers and interaction quality was investigated.MethodsTwenty-six women with BD and 30 comparison women with infants were included. The Parent-Child Early Relational Assessment (PCERA) was used to assess maternal behaviour, infant behaviour and dyadic coordination in interactions at 3 months postpartum. The Inventory of Depressive Symptomatology and Young Mania Rating Scale were used to assess affective symptoms of BD mothers at the time of interaction.ResultsThere were significant group differences with medium to large effect sizes (0.73–1.32) on five of six subscales within the three interactional domains. Most interactional concerns were identified in dyadic coordination. No significant associations were found between maternal symptom load and interaction quality within the BD sample. Forty-six percent of the BD mothers experienced a mood episode within 0–3 months postpartum.ConclusionsThe present study identified challenges for mothers with BD and their infants in “finding” each other in interaction at 3 months postpartum. If sustained, this interaction pattern may have a long-term impact on children’s development. We suggest interventions specifically focusing on sensitising and supporting mothers to read infants’ cues on a micro-level. This may help them to respond contingently and improve dyadic coordination and synchronicity.

The Baby Out of the Bathwater: Microseconds, Psychic Structure, and Psychotherapy

Infant development research has proposed links between early development and adult personality and psychopathology. Although clinicians have long taken these for granted, these research approaches raise a number of methodological and clinical issues worth exploring. Analysts have relied on varied formulations of “psychic structure,” but these are sometimes elusive and have not lent themselves to the precision, specificity, and validation that some analysts and many developmentalists prefer. New models have emerged over the last decades, including conceptions of internal working models of attachment, implicit relational knowing, and the like. Attachment research has shown strong correlations between infant attachment classification and adult personality. Recent research such as that of Beebe, Jaffe, and their colleagues has extended the correlations between early mother–infant interaction patterns and adulthood. All of this supports clinicians' interest in attention to the details of analyst–patient interaction, emotion regulation, inersubjective meaning-making, and other markers that can be observed in both infancy and adult analysis.

Iron deficiency and iron-deficiency anemia in the first two years of life: strategies to prevent loss of developmental potential

This article examines the association of iron deficiency (ID) and iron deficiency anemia (IDA) with children's development and behavior, with the goal of providing recommendations to prevent the developmental loss associated with these conditions. Children's risk for ID and IDA is particularly high during the second 6 months of life when prenatal stores are depleted. Longitudinal studies from infancy through adolescence and early adulthood suggest that socioemotional development is uniquely vulnerable to ID and IDA, perhaps being associated with shared neural pathways, and the effects of early iron deficiencies may be irreversible. In addition to direct effects on brain function, ID and IDA may also affect child development indirectly through non-responsive mother-child interactions. Maternal ID is a global problem that may contribute to high rates of maternal depression and non-responsive caregiving. Intervention trials illustrate that children benefit from both nutritional intervention and early learning interventions that promote responsive mother-child interactions. Recommendations to reduce the developmental loss associated with ID and IDA are to reduce the incidence of these conditions by efforts to prevent premature birth, delay cord clamping, ensure adequate maternal iron status, provide iron-rich complementary foods, and ensure access to postnatal interventions that promote responsive mother-infant interaction patterns and early learning opportunities for infants.

Interactive coordination of currently depressed inpatient mothers and their infants during the postpartum period.

In healthy mother-infant dyads, interactions are characterized by a pattern of matching and mismatching interactive states with quick reparation of mismatches into matches. In contrast, dyads in which mothers have postpartum depression show impaired mother-infant interaction patterns over the first few months of the infant's life. The majority of studies that have examined such interaction patterns have drawn on community samples rather than on depressed inpatient samples of mothers who were in a state of current depression at the time of assessment. To date, no study has investigated specific microanalytic patterns of interactive coordination between depressed German mothers and their infants using the Face-to-Face Still-Face paradigm (FFSF). The primary goal of this study was to evaluate specific patterns of dyadic coordination and the capacity for repairing states of miscoordination in an inpatient sample of postpartum currently depressed mothers and their infants as compared with a healthy control group. A sample of 28 depressed inpatient German mothers and their infants (age range = 1-8 months, M age = 4.06 months) and 34 healthy dyads (range = 1-8 months, M age = 3.89 months) were videotaped while engaging in the FFSF. A focus was placed on the play and reunion episodes. Compared with healthy dyads, dyads with depressed mothers showed less coordination of positive matched states and longer latencies when repairing interactive mismatching states into positive matched states. Clinical implications are discussed.

Borderline Personality Disorder, Mother–Infant Interaction and Parenting Perceptions: Preliminary Findings

Parents diagnosed with borderline personality disorder (BPD) are likely to find the emotional aspects of parenting challenging. Research into the difficulties that these parents experience, however, is lacking. The aims of the present study were to (i) gain an understanding of the interactional patterns of mothers with BPD and their infants and (ii) to explore the parenting perceptions of mothers with BPD. Two groups of mother-infant dyads were recruited: mothers with BPD and their infants; and community mothers and their infants. Groups were compared on mother-infant interaction patterns and on maternal self-perceptions of parenting. Mothers with BPD were found to be less sensitive and demonstrated less structuring in their interaction with their infants, and their infants were found to be less attentive, less interested and less eager to interact with their mother. Furthermore, mothers with BPD reported being less satisfied, less competent and more distressed. Early intervention needs to be provided to mothers with BPD to promote maternal sensitivity and maternal perceptions of competence.

Body ego and the preoedipal roots of feminine gender identity.

Traditionally, visual cues (that is, seeing the difference between one's own genitals and those of the opposite sex) and the fantasies that they prompt were thought to be the central determinants in the organization of gender identity. More recently attention has been focused on mother-infant interaction patterns, on the construction of the body itself, on its kinesthetic sensations, and on the fantasies that these evoke. These matters are now thought to be equal in importance to the traditional considerations. The resulting concept of a core gender identity has suggested the possibility of various forms of preoedipal genital anxiety. This paper discusses how two experiences common to childhood as well as adulthood--the tumescence of erectile tissue and the involuntary vaginal secretions of sexual arousal--may contribute to the organization of feminine gender identity. Case material, including a close examination of an analytic hour, is presented to show how this process may manifest itself in adult neurotic conflict.

Severe maternal psychopathology and infant-mother attachment.

Eighty-two mother-infant dyads, comprising women with psychiatric disorder and individually matched controls, were followed up over the children's 1st year of life. The mothers with mental illness consisted of two subgroups: first, 25 severely mentally ill mothers who had been admitted to a psychiatric unit with their infants; and second, 16 mothers from a community sample meeting research diagnostic criteria for unipolar, nonpsychotic depression. With the exception of six dyads in the in-patient group, observations were made of the mother-infant interaction and the quality of the infant-mother attachment relationship at 12 months. The nature and course of the mothers' illness was also documented. Although few residual symptoms of maternal mental illness were detected at 1 year postpartum, interactional disturbances were evident among the case group dyads. A strong association was revealed between infant-mother attachment quality and maternal diagnosis; a manic episode of illness in the postpartum period was related to security in the attachment relationship, and psychotic or nonpsychotic depression was related to insecurity. Concurrent patterns of mother-infant interaction provided support for this finding.

Manifestações iniciais de trocas interativas mãe-bebê e suas transformações

Neste trabalho, adota-se a perspectiva sócio-cultural e pressupõe-se que as interações adulto-bebê são matrizes nas quais se constrói o desenvolvimento. Esta pesquisa teve como objetivo descrever e analisar as transformações nas atividades da mãe e do bebê, nas trocas interativas e nos contextos específicos em que tais atividades e interações ocorrem, em etapas iniciais de desenvolvimento do bebê. Foi realizado o registro em vídeo, em ambiente natural, da observação de uma díade mãe-bebê em quatro momentos de desenvolvimento do bebê: 2, 10, 15 e 21 semanas. Foram analisadas as modificações na natureza das interações, mudanças nas atividades dos parceiros (e.g., vocalização, fala, sorriso), nos tipos de estimulação por parte das mães (e.g., estimulação voltada para a mãe ou voltada para os objeto) e os diferentes contextos de interação. Os resultados obtidos mostraram-se convergentes com achados de pesquisas na área, ampliando-os. Foram identificadas, desde fases iniciais, interações como processos recíprocos de engajamento que tornaram-se mais freqüentes e complexos. Foi possível identificar interações mãe-bebê, caracterizá-las e ilustrar a natureza diferenciada das atividades e interações dos parceiros em momentos distintos do desenvolvimento do bebê. Os resultados apresentados puderam ser interpretados segundo a abordagem sócio-cultural, que considera as interações sociais como constitutivas do desenvolvimento.

Early mother-infant interaction during breast- and bottle-feeding

Thirty-two primiparous mothers with their neonates, 20 breast-fed and 12 bottle-fed, were videotaped during feeding longitudinally (at 3, 10 days, 1 and 3 months) in order to investigate whether patterns of mother-infant interaction observed during breast- and bottle-feeding differ also for behavior not directly related to sucking. Infant state, sucking behavior, mother behavior and mother-infant interaction were analyzed by both sequential and non sequential analysis. Results suggest that the method of feedings affects mother-infant interaction observed during and just after feeding also for some behavior not directly related to sucking. The percentage of mutual touch, tactile stimulation and mother's gaze to infant was significantly more elevated during breast-feeding. Independently of the feeding mode, significant sequential dependencies between infant state of alertness and auditory stimulation, and auditory stimulation and mutual gaze were found.

Patterns of mother-infant interaction and attachment: A cluster-analytic approach

This study examined stability and change in patterns of mother-infant interaction between 5 and 10 months and their relation to attachment security at 18 months. Cluster analysis was used to identify patterns of dyadic interaction. There was significant, but not substantial, stability in cluster membership from 5 to 10 months. For females, being in the most optimal cluster at 5 months related to subsequent attachment security, regardless of the 10-month pattern of interaction. For males, who were significantly more likely to be insecure than females, insecure attachment was predicted by negative change from 5 to 10 months (moving from a more optimal to a less optimal cluster).

Mother and Infant Activity and Interaction in France and in the United States: A Comparative Study

Infants experiences are often thought to influence social and intellectual development in the individual, and on a societal level they are sometimes credited for some of the distinctiveness that typifies cultural style. To compare and contrast the experiences of French and U.S. American infants, mother-infant dyads in Paris and in New York City were observed interacting in the natural setting of their homes. This report focuses on infants' visual attention, tactual exploration, and vocalisation and on mothers' mediated and unmediated stimulation and speech to infants. The study had two main goals: One was to identify and describe activities and interaction patterns that may be similar and different in these two Western cultures, and the other was to test the cross-cultural validity of a hypothesis that states that specific mother and infant activities relate to one another in dyadic interaction. Mothers and infants in the two cultures showed some similarities and some different emphases in their activities, and patterns of mother-infant interaction in the two cultures tended to correspond.

Towards Breaking the Cycle of Intergenerational Abuse

A quantitative single case study of an adolescent mother with a history of being abused as a child is presented. It illustrates the diagnostic and treatment issues during an intervention designed to break the cycle of intergenerational abuse. Drawing upon an ecological, transactional model of development, the case study utilized a multimethod, longitudinal approach to assess the mother's history and current psychosocial functioning, the infant's developmental competence and attachment status, patterns of mother-infant interaction and components of the family's social ecology. Measures were administered during a baseline period and systematically repeated throughout the one-year period of intervention. The treatment involved two weekly therapy sessions; one, an individual session for the mother and the other, a session in which mother and infant were seen together. The findings at the one-year evaluation showed improved maternal psychosocial functioning, the infant's shift from an insecure to a secure attachment classification and improved patterns of mother-infant interactions. The implications of this therapeutic approach and the use of single-case methodology are discussed.

The use of multitreatment modalities in early intervention: A quantitative case study

A quantitative single case study is presented to illustrate how an early intervention program used two therapeutic modalities to treat a depressed mother and her 2-month-old son. Drawing upon an ecological, transactional model of development, the case study utilized a multimethod, longitudinal approach to assess the infant's developmental competence and attachment status, the mother's history and current psychosocial functioning, patterns of mother-infant interaction, and components of the family's social ecology. Measures were administered during a baseline period to assess pre-intervention functioning and were systematically repeated throughout the 2-year period of intervention. The treatment modalities included psychodynamically oriented individual therapy and Parent-Infant Relationship Treatment (PIRT) in which the dyad was also seen by a second therapist to foster more adaptive mother-infant transactional patterns. The findings indicated increased infant developmental competence and a shift from an insecure to a secure attachment classification, improved maternal psychosocial functioning, and a decline in the dyadic interactional pattern of maternal intrusiveness and infant withdrawal. The advantages of using two treatment modalities and a single case approach to evaluation are discussed.

Interaction Pattern and Developmental Outcome of Infants with Severe Asphyxia

Seven full-term infants with severe encephalopathy following perinatal asphyxia were followed longitudinally to 2 years of age to determine health and developmental outcome and to investigate mother-infant interaction patterns over time. Infants received either ongoing or follow-up intervention that differed in terms of intensity and focus of intervention procedures. Developmental assessment scores at 12 and 24 months of age indicated that all infants except one demonstrated varying degrees of delayed development. Five infants were diagnosed with cerebral palsy at or before their 12-month follow-up clinic visit. One was diagnosed with and one remained without evidence of cerebral palsy at the 24-month visit. Mother-infant interactions differentiated among degree of infant disability at outcome where medical signs alone were unreliable predictors of later developmental outcome. Interactions between mothers and their infants upon discharge from the hospital differentiated babies with severe disability from those with moderate, minimal, or no disability. At 6 months, interaction patterns were different for all levels of disability, and these differences remained at 12 months.

Hospital-based primary prevention strategy in child abuse: a multi-level needs addressment

The purpose of this study was to evaluate a primary prevention effort in child maltreatment, the focus of which was a community-wide endeavor (i.e., subjects were not prescreened for risk potential). Specific components of the planned intervention addressed four causal levels of maltreatment: individual, family, community, and cultural factors. That addressment was intended to build strengths in those parenting areas which are predictive of abusive or neglectful outcomes. Target areas included knowledge of child development, child-rearing attitudes, mother-infant interaction patterns, and parenting skills. A posttest-only control group design was employed. The control postpartum mothers received traditional or routine hospital services; experimental mothers received special in-hospital and aftercare services by trained student nurse volunteers. These volunteers functioned in a dual educative/supportive role. Experimental mothers reported more realistic expectations of behavior, embraced more democratic child-rearing principles, provided more verbal stimulation to their infants (reflecting an increased sensitivity), and displayed increased problem-solving abilities.

A theoretical overview and empirical study of mediated learning experience: Prediction of preschool performance from mother-infant interaction patterns

This research is based on a 4-year longitudinal study of 51 normal, low SES infants and their mothers. It was conducted at the Clinical Infant Development Program sponsored by the Clinical Infant-Child Development Research Center (NIMH-HRSA). Infants' free-play sessions with their mothers were analyzed at 4, 8, 12, 24, and 36 months of age by use of an observation method based on Feuerstein's theory of Mediated Learning Experience (MLE). The participating children were tested with the Bayley Scales (to age 2 years) and the McCarthy Scales (at 36 and 48 months). Ten-minute observations of motherchild interactions, which used molar criteria of observation based on MLE theory, predicted cognitive performance at 2, 3, and 4 years of age. The observation made at 12 months was most predictive of long-term cognitive performance. Mother-infant interaction patterns were less predictive of cognitive performance in infancy than they were of performance at 2, 3, and 4 years of age. Each of the observed MLE criteria increased with age, although the frequency of some of the criteria remained low throughout the study. Stability of all MLE criteria was significant across most comparisons over time.

Effects of short-term interaction coaching with mothers of preterm infants

This study examined the effects of eight 40-minute interaction coaching sessions on the mother-infant interaction patterns of mother-preterm infant dyads. Thirty-five preterm infants and their mothers were matched for sex and then were assigned randomly to a treatment and a no-treatment control group. Behavior counts and ratings of maternal and infant interactive behavior (Interaction Rating Scale; Field, 1980) were obtained from naturalistic home observations, precoaching, postcoaching, and at a 2-month follow-up. It was hypothesized that coaching, which was intended to facilitate more sensitive responding by mothers, would positively influence mother-infant interaction and mothers' knowledge of infant development. The only marginally significant difference between the groups that emerged over time was in knowledge of infant development, in which the treatment group surpassed the control group.

Navajo Infancy: An Ethological Study of Child Development

Navajo Infancy describes the major sources of change and continuity in Navajo infant development. It does so by combining concepts and methods of classical ethology with those of social-cultural anthropology. The goal is to establish the relationships between human nature and culture. Buy considering the nature of adaptation, and the evolution of human developmental patterns, and through analyses of the determinants of change and continuity in Navajo infant development, Navajo Infancy outlines how the process of development itself may bridge nature and culture. With its special focus on the effect of the cradleboard on Navajo mother-infant interaction, Navajo Infancy raises important developmental issues in its analyses of why the effects of the cradleboard do not last. Incorporating the Brazelton Neonatal Behavioral Assessment Scale into its ethological-anthropological methods, Navajo Infancy demonstrates significant Navajo-Anglo-American differences in newborn temperament. It finds a strong correlation between newborn behavior and prenatal environmental factors, arguing that racial and ethnic differences in behavior at birth go well beyond simple gene pool differences. Navajo Infancy also describes the individual and group differences in the development of Navajo and Anglo- American children's fear of strangers and patterns of mother-infant interaction. Aspects of attachment theory, transactional theories of development, and anthropological theories of socialization are related to this broad new evolutionary approach to the process of development and nature-culture interaction.

Cross-cultural Differences in Maternal Perceptions of Cries of Low- and High-Risk Infants

The tape-recorded cries of low- and high-risk newborn infants were rated by 150 inner-city Anglo-American, Black-American, and Cuban-American mothers during the hospital lying-in period following childbirth. Half of each cultural group was primiparous and half was multiparous. The mothers rated the cries along 4 perceptual and 6 caregiving response scale items. Reliable differences were found between low- and high-risk infant cries on all perceptual responses with the effects of culture and parental experience affecting the degree of differences. Generally, Anglo-American mothers found the cries more distressing, urgent, arousing, and sick sounding than Black-American mothers, while Cuban-American mothers showed similarities to both Black- and Anglo-American mothers depending on the scale items. The ratings on the caregiving response scale items paralleled cultural differences found on the perception scale items and previous reports of the mother-infant interaction patterns of other Anglo-, Black-, and Cuban-American samples. The results are discussed as being important in developing nonethnocentric views of the functional significance of the behaviors of the infant at risk, yet as providing evidence of the cross-cultural significance of the cry sound of the infant at risk.
