Cognitive behavioural therapy on improving the depression symptoms in patients with diabetes: a meta-analysis of randomized control trials.

This meta-analysis was performed to evaluate the effect of cognitive behavioural therapy (CBT) in improving the depression symptoms of patients with diabetes. Literature search was conducted in PubMed and Embase up to October 2016 without the initial date. The pooled SMD (standard mean difference) and its 95% confidence interval (CI) were calculated by Revman 5.3. Subgroup analyses were performed by type of diabetes and evaluation criteria of depression. A total of five randomized control trials involving 834 patients with diabetes mellitus (including 417 patients in CBT group and 417 patients in control group) were included in this meta-analysis. The pooled estimates indicated significant improvement of depression by CBT compared with routine approaches in overall outcomes (SMD =–0.33, 95% CI =–0.46 to –0.21, P<0.00001), post-intervention outcomes (SMD =–0.43, 95% CI =–0.73 to –0.12, P=0.006) and outcomes after 12 months intervention (SMD =–0.38, 95% CI = –0.54 to –0.23, P<0.0001). Subgroup analyses showed that the results were not influenced by the type of diabetes. However, the effect of CBT on improving the depression symptoms disappeared when only using CES-D (Centre for Epidemiological Studies scale for Depression) to evaluate depression.

Effect of cognitive-behavioral therapy on the quality of life of dderly depression patients with chronic pain

Objective To explore the influence of cognitive-behavioral therapy on the quality of life of elderly depression patients with chronic pain.Methods 60 patients were divided into the cognitive-behavioral therapy(CBT) group and non-cognitive-behavioral therapy(N-CBT) group,30 cases in each group.The patients in CBT group were treated with cognitive-behavioral therapy combined with mirtazapine,and the patients in N-CBT group were treated with mirtazapine only.Before treating and after the 4th week and 12th week,depression,anxiety and chronic pain were assessed respectively with Hamilton Depression Scale ( HAMD),Hamilton Anxiety Scale (HAMA),and Visual Analogue Scales (VAS),the quality of life was assessed with the WHO Quality of Life ( WHO.QOL-100).No significant different scores of HAMD,HAMA,VAS and WHOQOL-100 existed between two groups before treatment.Results Depression,anxiety and chronic pain were significantly related to the life quality of elderly depression patients with chronic pain ( r =-0.506 ～ -0.676,P ＜ 0.01 ).At the end of the 12th treating week,the scores of HAMD(36.82 ±7.93),HAMA(8.26 ±3.13),VAS(4.35 ±0.52) and WHOQOL100 ( respectively ( 65.71 ± 17.29 ),( 92.36 ± 13.55 ),( 77.91 ± 12.26 ),( 78.33 ± 12.81 ),( 81.79 ± 12.43 ),( 18.95 ± 11.91 ),( 19.98 ± 11.56 ) ) were significantly different from those before treatment in CBT group,and the change of each indicator score was significantly greater in CBT group than those in N-CBT group ( t =3.020 ～9.761,P＜ 0.01 ).Conclusion Cognitive-behavioral therapy can effectively improve the life quality of the elderly depression patients with chronic pain. Comparative study on cognitive-behavioral therapy in patients with chronic insomnia

目的 探讨慢性失眠症患者实施认知行为治疗的临床效果.方法 对74例慢性失眠症患者随机分为认知行为治疗组(36例)和对照组(38例).比较两组患者持续治疗4周后及2个月后随访时的疗效.结果 (1)两组治疗后睡眠时间(289.9±38.0,239.2±36.1,P <0.01)和SRSS评分(32.1±4.7,38.7±5.4,P <0.01)比较差异均有显著性.(2)随访时治疗有效率认知行为组(75%)明显优于对照组(37%).结论 认知行为疗法可促进患者失眠症状的改善,提高临床治疗效果.
