Patient-specific quality assurance in SBRT treatments using 3D polymer gel dosimetry

In stereotactic body radiation therapy (SBRT), patient-specific quality assurance (PSQA) is required due to the complex dose distributions delivered. In this context, polymer gels are considered reliable as a three-dimensional dosimeter. This work used polymer MAGIC-gel sealed in 2 mm thin-wall glass spheres to assess the three-dimensional (3D) dose distribution imparted to patients and compared the results with that provided by the treatment planning system. Four SBRT treatments targeting vertebral, ilium, pelvic lymph node and lung regions were selected for this study. A computed tomography (CT) scan of each glass sphere was obtained, and the patient treatment plan was superimposed on the glass sphere CT image. Thereafter, the spheres were irradiated under similar conditions to the patient using a Varian TrueBeam linear accelerator. A high-spatial resolution optical CT scanner was used to read the glass sphere containing the gel. Planned and delivered 3D dose distributions were compared quantitatively through 3D gamma passing rates at several selected acceptance criteria and qualitatively through the evaluation of dose-profiles, isodose-curves, and dose-volume histograms. We observed that the overall 3D gamma passing rate exceeded 90% at acceptance criteria of 2% dose difference and 2 mm distance-to-agreement for a dose threshold of 20%. The results of this study suggest that the polymer MAGIC-gel sealed in thin-wall glass spheres in conjunction with the optical CT readout method is a suitable PSQA system for SBRT treatments.

Stereotactic body radiation therapy (SBRT) for prostate cancer: Improving treatment delivery efficiency and accuracy

PurposeIn stereotactic body radiation therapy (SBRT) for prostate cancer, intrafraction motion is an important source of treatment uncertainty as it could not be completely smoothed through fractionation. Herein, we compared different arrangements and beam qualities for extreme hypofractionated treatments to minimize beam delivery time and so intrafractional errors. MethodsA retrospective dataset of 11 patients was used. Three volumetric modulated arc therapy (VMAT) beam arrangements were compared for a prescription dose of 40 Gy/5 fractions: two full arcs, 6 MV flattening filter free (FFF); one full arc, 6 MV FFF; one full arc, 10 MV FFF. A plan quality index was defined to compare achievement of the planning goals. Plan complexity was evaluated with the modulation factor. Dose delivery accuracy and efficiency were measured with patient-specific quality assurance plans. ResultsAll treatment plans fulfilled all dose objectives. No statistical differences were found both in plan quality and complexity. Very accurate dose delivery was achieved with the three arrangements, with mean γ passing rates >96.5 % (2 %/2 mm criteria). Slightly but significantly higher γ passing rates were observed with single-arc 6 MV FFF. Contrariwise, statistically significant reductions of the delivery time were obtained with single-arc geometries: the average delivery times were 1.6 min (−46.1 %) and 1.3 min (−56.2 %) for 6 and 10 MV FFF respectively. ConclusionsThe high-quality, very fast and accurate dose delivery of single-arc plans confirmed the suitability of this arrangement for prostate SBRT. In particular, the significant reduction of delivery time would improve treatment robustness against intrafraction prostate motion.

Alanine dosimeters for LET measurement in proton radiotherapy

Proton radiotherapy enables highly conformal dose delivery to the tumor while minimizing exposure to healthy tissues surrounding the target. However, to achieve the most optimal treatment plan, it is necessary to consider various parameters, including variable relative biological effectiveness (RBE) and linear energy transfer (LET) within the target and in the normal tissues. The proton treatment planning systems (TPS) currently under development incorporate both of these parameters. However, before introducing biologically guided treatment planning into the clinic, commissioning and end-to-end tests must be conducted. These procedures require specific tools to verify the planned LET distribution. The same tools will also be necessary for subsequent patient-specific quality assurance (QA).The objective of this work was to evaluate the alanine/EPR dosimetry system developed at the Cyclotron Center Bronowice at the Institute of Nuclear Physics (CCB IFJ PAN) for verifying LET values in therapeutic proton fields. The change in the shape of the EPR spectra of alanine irradiated in proton fields of different LETs, described by the ratio of the intensities of the satellite and central lines (x/y ratio), was used to discriminate LET values. The method does not provide absolute LET values. However, as demonstrated, it can verify the LET values calculated in the TPS after calibration using Monte Carlo (MC) simulation.

Feasibility of patient-specific quality assurance (PSQA) for real-time robotic stereotactic body radiotherapy (SBRT) based on tumor motion traces.

To design a patient specific quality assurance (PSQA) process for the CyberKnife Synchrony system and quantify its dosimetric accuracy using a motion platform driven by patient tumor traces with rotation. The CyberKnife Synchrony system was evaluated using a motion platform (MODUSQA) and a SRS MapCHECK phantom. The platform was programed to move in the superior-inferior (SI) direction based on tumor traces. The detector array housed by the StereoPhan was placed on the platform. Extra rotational angles in pitch (head down, 4.0°±0.15° or 1.2°±0.1°) were added to the moving phantom to examine robot capability of angle correction during delivery. A total of 15 Synchrony patients were performed SBRT PSQA on the moving phantom. All the results were benchmarked by the PSQA results based on static phantom. For smaller pitch angles, the mean gamma passing rates were 99.75%±0.87%, 98.63%±2.05%, and 93.11%±5.52%, for 3%/1mm, 2%/1mm, and 1%/1mm, respectively. Large discrepancy in the passing rates was observed for different pitch angles due to limited angle correction by the robot. For larger pitch angles, the corresponding mean passing rates were dropped to 93.00%±10.91%, 88.05%±14.93%, and 80.38%±17.40%. When comparing with the static phantom, no significant statistic difference was observed for smaller pitch angles (p=0.1 for 3%/1mm), whereas a larger statistic difference was observed for larger pitch angles (p<0.02 for all criteria). All the gamma passing rates were improved, if applying shift and rotation correction. The significance of this work is that it is the first study to benchmark PSQA for the CyberKnife Synchrony system using realistically moving phantoms with rotation. With reasonable delivery time, we found it may be feasible to perform PSQA for Synchrony patients with a realistic breathing pattern.

Lateral response artifact correction method using image stitching technique in radiochromic film dosimetry.

Lateral response artifact (LRA) is caused by the interaction between film and flatbed scanner in the direction perpendicular to the scanning direction. This can significantly affect the accuracy of patient-specific quality assurance (QA) in cases involving large irradiation fields. We hypothesized that by utilizing the central area of the flatbed scanner, where the magnitude of LRA is relatively small, the LRA could be mitigated effectively. This study proposes a practical solution using the image-stitching technique to correct LRA for patient-specific QA involving large irradiation fields. Gafchromic™ EBT4 film and Epson Expression ES-G11000 flatbed scanner were used in this study. The image-stitching algorithm requires a spot between adjacent images to combine them. The film was scanned at three locations on a flatbed scanner, and these images were combined using the image-stitching technique. The combined film dose was then calculated and compared with the treatment planning system (TPS)-calculated dose using gamma analysis (3%/2mm). Our proposed LRA correction was applied to several films exposed to 18 × 18 cm2 open fields at doses of 200, 400, and 600 cGy, as well as to four clinical Volumetric Modulated Arc Therapy (VMAT) treatment plans involving large fields. For doses of 200, 400, and 600 cGy, the gamma analysis values with and without LRA corrections were 95.7%versus 67.8%, 95.5%versus 66.2%, and 91.8%versus 35.9%, respectively. For the clinical VMAT treatment plan, the average pass rate±standard deviation in gamma analysis was 94.1%±0.4% with LRA corrections and 72.5%±1.5% without LRA corrections. The effectiveness of our proposed LRA correction using the image-stitching technique was demonstrated to significantly improve the accuracy of patient-specific QA for VMAT treatment plans involving large irradiation fields.

Assessment of the deep learning-based gamma passing rate prediction system for 1.5 T magnetic resonance-guided linear accelerator.

Measurement-based verification is impossible for the patient-specific quality assurance (QA) of online adaptive magnetic resonance imaging-guided radiotherapy (oMRgRT) because the patient remains on the couch throughout the session. We assessed a deep learning (DL) system for oMRgRT to predict the gamma passing rate (GPR). This study collected 125 verification plans [reference plan (RP), 100; adapted plan (AP), 25] from patients with prostate cancer treated using Elekta Unity. Based on our previous study, we employed a convolutional neural network that predicted the GPRs of nine pairs of gamma criteria from 1%/1mm to 3%/3mm. First, we trained and tested the DL model using RPs (n=75 and n=25 for training and testing, respectively) for its optimization. Second, we tested the GPR prediction accuracy using APs to determine whether the DL model could be applied to APs. The mean absolute error (MAE) and correlation coefficient (r) of the RPs were 1.22 ± 0.27% and 0.29 ± 0.10 in 3%/2mm, 1.35 ± 0.16% and 0.37 ± 0.15 in 2%/2mm, and 3.62 ± 0.55% and 0.32 ± 0.14 in 1%/1mm, respectively. The MAE and r of the APs were 1.13 ± 0.33% and 0.35 ± 0.22 in 3%/2mm, 1.68 ± 0.47% and 0.30 ± 0.11 in 2%/2mm, and 5.08 ± 0.29% and 0.15 ± 0.10 in 1%/1mm, respectively. The time cost was within 3s for the prediction. The results suggest the DL-based model has the potential for rapid GPR prediction in Elekta Unity.

Patterns of quality assurance and treatment planning for stereotactic body radiation therapy in India - A survey.

The use of stereotactic body radiation therapy (SBRT) is an increasing trend in the country. The aim of this study is to gain knowledge on patterns of quality assurance (QA) and treatment planning (TP) aspects with respect to SBRT. A questionnaire with multiple choice was designed to determine practices of SBRT covering areas such as years of experience, type of linear accelerator, tumor-motion strategies, calculation algorithm used in the TP system (TPS), the protocol used for small field dosimetry, the detector used for small field dosimetry and QA, respiratory management during delivery. The survey was sent to all radiotherapy institutes in the country having a minimum of one linear accelerator, and responses were analyzed. From June 2022 to December 2022, 265 responses to the SBRT survey were received with response rate as 60.4%. The most common reason for not adopting SBRT was reported as a lack of capability of treatment machines to deliver SBRT (61.6%). Lung (81.1%) was the most practiced site. The most common delivery unit was a conventional linear accelerator (83%); 6 MV FFF (85.7%) was mostly used energy; volumetric-modulated arc radiotherapy (VMAT) (91.5%) was mostly used delivery technique; most of the equipment (more than 91.5%) used multileaf collimator (MLC) leaf width ≤5 mm. The most popular methods used for motion strategies during computed tomography (CT) were motion-encompassing and breath-hold techniques used by 65 (62.5%) and 62 (59.6%) respondents, respectively. The most popular method used for respiratory management during delivery was breath-hold by 55 (52.4%) respondents. Most TPS are equipped with either Type-C or Type-B algorithms. Heterogeneity was observed in the QA protocol and acceptance criteria for analysis of patient-specific QA. The survey resulted in heterogeneity in QA and TP aspects among users of SBRT and demands for harmonizing the dosimetric aspects of SBRT in the country.

Characterization of an enhanced formulation N-(3-methoxypropyl) acrylamide polymer gel dosimeter by the addition of an organic sensitizer for clinical practice

Polymer gel dosimeters are being developed and improved to perform measurements of radiation dose distributions in 3-dimensions (3-D) for the purpose of patient specific quality assurance for cancer patients and the configuration of treatment planning systems. The effect of organic glucose sensitizer (GL) material on N-(3-methoxypropyl) acrylamide polymer gel dosimeter (NMPAGAT) was presented in this study and evaluated after irradiation using nuclear magnetic resonance (NMR) read-out technique in terms of spin–spin relaxation rate (R2) of hydrogen protons within the water molecule. The improved tissue equivalent polymers of GL-NMPAGAT were irradiated using a medical linear accelerator with photon beams with doses up to 20 Gy, energies range 6–15 MV, dose rates range 50–500 cGy/min, and irradiation temperatures range 15–25 °C. The R2 dose sensitivity of GL-NMPAGT polymer with a concentration of 25 % wt GL was improved significantly and found to be two times higher than the same gel without GL in the linear dose response of 0.07–8 Gy. The 0.07 Gy represents the minimum detectable dose in the improved dosimeters. The dosimeter was found stable within a period of up to two weeks after irradiation, which is significant. No significant dose rate, energy dependence, and irradiation temperature for the dosimeter was observed over the range studied. There is decrease in R2 values with increasing scanning temperature. The gel is water equivalent and has theoretical energy-independent response from 0.1 to 20 MeV with density of 1.023 ± 0.003 g/cm3 and Zeff of 7.53. The results of the improved dosimeter in this study (i.e., 100 %, 0.32 s−1 Gy−1, and 8 Gy linearity) are toward the upper limit compared to other polymer gel dosimeters even with sensitizer additives.

Commissioning and dosimetric verification of volumetric modulated arc therapy for multiple modalities using electronic portal imaging device-based 3D dosimetry system: a novel approach.

The purpose of this study was to validate an electronic portal imaging device (EPID) based 3-dimensional (3D) dosimetry system for the commissioning of volumetric modulated arc therapy (VMAT) delivery for flattening filter (FF) and flattening filter free (FFF) modalities based on test suites developed according to American Association of Physicists in Medicine Task Group 119 (AAPM TG 119) and pre-treatment patient specific quality assurance (PSQA).With ionisation chamber, multiple-point measurement in various planes becomes extremely difficult and time-consuming, necessitating repeated exposure of the plan. The average agreement between measured and planned doses for TG plans is recommended to be within 3%, and both the ionisation chamber and PerFRACTION™ measurement were well within this prescribed limit. Both point dose differences with the planned dose and gamma passing rates are comparable with TG reported multi-institution results. From our study, we found that no significant differences were found between FF and FFF beams for measurements using PerFRACTION™ and ion chamber. Overall, PerFRACTION™ produces acceptable results to be used for commissioning and validating VMAT and for performing PSQA. The findings support the feasibility of integrating PerFRACTION™ into routine quality assurance procedures for VMAT delivery. Further multi-institutional studies are recommended to establish global baseline values and enhance the understanding of PerFRACTION™'s capabilities in diverse clinical settings.

Error detection using a multi-channel hybrid network with a low-resolution detector in patient-specific quality assurance.

This study aimed to develop a hybrid multi-channel network to detect multileaf collimator (MLC) positional errors using dose difference (DD) maps and gamma maps generated from low-resolution detectors in patient-specific quality assurance (QA) for Intensity Modulated Radiation Therapy (IMRT). A total of 68 plans with 358 beams of IMRT were included in this study. The MLC leaf positions of all control points in the original IMRT plans were modified to simulate four types of errors: shift error, opening error, closing error, and random error. These modified plans were imported into the treatment planning system (TPS) to calculate the predicted dose, while the PTW seven29 phantom was utilized to obtain the measured dose distributions. Based on the measured and predicted dose, DD maps and gamma maps, both with and without errors, were generated, resulting in a dataset with 3222 samples. The network's performance was evaluated using various metrics, including accuracy, sensitivity, specificity, precision, F1-score, ROC curves, and normalized confusion matrix. Besides, other baseline methods, such as single-channel hybrid network, ResNet-18, and Swin-Transformer, were also evaluated as a comparison. The experimental results showed that the multi-channel hybrid network outperformed other methods, demonstrating higher average precision, accuracy, sensitivity, specificity, and F1-scores, with values of 0.87, 0.89, 0.85, 0.97, and 0.85, respectively. The multi-channel hybrid network also achieved higher AUC values in the random errors (0.964) and the error-free (0.946) categories. Although the average accuracy of the multi-channel hybrid network was only marginally better than that of ResNet-18 and Swin Transformer, it significantly outperformed them regarding precision in the error-free category. The proposed multi-channel hybrid network exhibits a high level of accuracy in identifying MLC errors using low-resolution detectors. The method offers an effective and reliable solution for promoting quality and safety of IMRT QA.

Technical note: A simple method for patient-specific quality assurance for lateral targets on a 1.5 T MR-Linac.

The Elekta Unity magnetic resonance (MR) linac is limited to longitudinal couch motion and a sagittal-only laser, which restricts the ability to perform patient-specific quality assurance (PSQA) intensity-modulated radiotherapy (IMRT) measurements for very lateral targets. This work introduces a simple method to perform PSQA using the Sun Nuclear ArcCheck-MR phantom at left and right lateral positions without additional equipment or in-house construction. The proposed setup places the center of the phantom 1.3cm vertical and 12.9cm lateral to isocenter in either the left or right direction. Computed tomography (CT) scans are used to simulate the setup and create a QA plan template in the Monaco treatment planning system (TPS). The workflow is demonstrated for four patients, with an average axial distance from the center of the bore to the planning target volume (PTV) of 12.4cm. Gamma pass rates were above 94% for all plans using global 3%/2mm gamma criterion with a 10% threshold. Setup uncertainties are slightly larger for the proposed lateral setup compared to the centered setup on the Elekta platform (∼1mm compared to ∼0.5mm), but acceptable pass rates are achievable without optimizing shifts in the gamma analysis software. In general, adding the left and right lateral positions increases the axial area in the bore encompassed by the cylindrical measurement array by 147%, substantially increasing the flexibility of measurements for offset targets. Based on this work, we propose using the lateral QA setup if the closest distance to the PTV edge from isocenter is larger than the array radius (10.5cm) or the percent of the PTV encompassed by the diode array would be increased with the lateral setup compared to the centered setup.

Efficacious patient-specific QA for Vertebra SBRT using a high-resolution detector array SRS MapCHECK: AAPM TG-218 analysis.

Patient-specific quality assurance (PSQA) for vertebra stereotactic body radiation therapy (SBRT) presents challenges due to highly modulated small fields with high-dose gradients between the target and spinal cord. This study aims to explore the use of the SRS MapCHECK® (SRSMC) for vertebra SBRT PSQA. Twenty vertebra SBRT treatment plans including prescriptions 20Gy/1 fraction and 24Gy/2 fractions were selected for each of Millennium (M)-Multileaf Collimator (MLC), and high-definition (HD)-MLC. All 40 plans were measured using Gafchromic EBT3 film (film) and SRSMC, using the StereoPHAN phantom. Plan complexity was assessed using modulation complexity score (MCS), edge metric (EM) (mm-1 ), modulation factor (MU/cGy), and average leaf pair opening (ALPO) (mm) and its correlation with gamma-pass rate was investigated. The high dose gradient between the target and the spinal cord was analyzed for film and SRSMC and compared against the treatment planning system (TPS). Applying the methodology proposed by AAPM TG-218, action and tolerance values specific to the SRSMC for vertebra SBRT were determined for β values ranging from 5 to 8. Film and SRSMC gamma-pass rates showed no correlation (p>0.05). A moderate negative correlation (R=-0.57, p=0.01) is present between EM and SRSMC 3%/1mm gamma-pass rate for HD-MLC plans. Both film and SRSMC accurately measured high dose gradients between the target and the spinal cord (R2 >0.86, p ≤ 0.05). Notably, dose-gradient of HD-MLC plans is 22% steeper and has a smaller standard deviation to M-MLC plans (p ≤ 0.05). Applying TG-218, the film tolerance limit was 96% with action limit 95% for 5%/1mm (β=6) and for the SRSMC tolerance limit was 97% with an action limit of 96% for 4%/1mm (β=6). Our findings suggest that universal TG-218 limits may not be suitable for vertebra SBRT PSQA. This study demonstrates that SRSMC is a viable tool for vertebra SBRT PSQA, supported by TG-218 implementation of process-based tolerance and action limits.

Rapid calibration curve generation using single-sheet radiochromic film for proton-based stereotactic radiosurgery verification

Proton-based stereotactic radiosurgery (PSRS) holds promise for treating benign tumours, with patient-specific quality assurance (PSQA) playing a crucial role in ensuring high-quality clinical services. EBT3 films are a perfect tool for PSQA, particularly owing to their compatibility with small-field dosimetry. However, a successful absolute EBT3 dosimetry relies on detailed dose calibration, consuming much time and films. A novel approach has been developed in this study and tested to establish a calibration curve using a single sheet of EBT3 radiochromic films exposed to six multi-leaf collimator (MLC) fields. Notably, this method allows for the completion of a calibration curve in just 15 min, enabling the practical generation of new EBT3 calibrations daily. This single-sheet approach can generate a dose calibration curve with an error of less than 1.5% across the entire dose range and is well agreed to by traditional multi-film calibrations involving large fields, with a discrepancy of less than 0.8%.

Influence of dose calculation algorithms on the helical diode array using volumetric-modulated arc therapy for small targets.

For patient-specific quality assurance (PSQA) for small targets, the dose resolution can change depending on the characteristics of the dose calculation algorithms. This study aimed to evaluate the influence of the dose calculation algorithms Acuros XB (AXB), anisotropic analytical algorithm (AAA), photon Monte Carlo (pMC), and collapsed cone (CC) on a helical diode array using volumetric-modulated arc therapy (VMAT) for small targets. ArcCHECK detectors were inserted with a physical depth of 2.9cm from the surface. To evaluate the influence of the dose calculation algorithms for small targets, rectangular fields of 2×100, 5×100, 10×100, 20×100, 50×100, and 100×100 mm2 were irradiated and measured using ArcCHECK with TrueBeam STx. A total of 20 VMAT plans for small targets, including the clinical sites of 19 brain metastases and one spine, were also evaluated. The gamma passing rates (GPRs) were evaluated for the rectangular fields and the 20 VMAT plans using AXB, AAA, pMC, and CC. For rectangular fields of 2×100 and 5×100 mm2 , the GPR at 3%/2mm of AXB was<50% because AXB resulted in a coarser dose resolution with narrow beams. For field sizes>10×100 mm2, the GPR at 3%/2mm was>88.1% and comparable for all dose calculation algorithms. For the 20 VMAT plans, the GPRs at 3%/2mm were 79.1±15.7%, 93.2±5.8%, 94.9±4.1%, and 94.5±4.1% for AXB, AAA, pMC, and CC, respectively. The behavior of the dose distribution on the helical diode array differed depending on the dose calculation algorithm for small targets. Measurements using ArcCHECK for VMAT with small targets can have lower GPRs owing to the coarse dose resolution of AXB around the detector area.

Evaluation of monte carlo to support commissioning of the treatment planning system of new pencil beam scanning proton therapy facilities

ABSTRACT Objective. To demonstrate the potential of Monte Carlo (MC) to support the resource-intensive measurements that comprise the commissioning of the treatment planning system (TPS) of new proton therapy facilities. Approach. Beam models of a pencil beam scanning system (Varian ProBeam) were developed in GATE (v8.2), Eclipse proton convolution superposition algorithm (v16.1, Varian Medical Systems) and RayStation MC (v12.0.100.0, RaySearch Laboratories), using the beam commissioning data. All models were first benchmarked against the same commissioning data and validated on seven spread-out Bragg peak (SOBP) plans. Then, we explored the use of MC to optimise dose calculation parameters, fully understand the performance and limitations of TPS in homogeneous fields and support the development of patient-specific quality assurance (PSQA) processes. We compared the dose calculations of the TPSs against measurements (DDTPSvs.Meas.) or GATE (DDTPSvs.GATE) for an extensive set of plans of varying complexity. This included homogeneous plans with varying field-size, range, width, and range-shifters (RSs) (n = 46) and PSQA plans for different anatomical sites (n = 11). Main results. The three beam models showed good agreement against the commissioning data, and dose differences of 3.5% and 5% were found for SOBP plans without and with RSs, respectively. DDTPSvs.Meas. and DDTPSvs.GATE were correlated in most scenarios. In homogeneous fields the Pearson’s correlation coefficient was 0.92 and 0.68 for Eclipse and RayStation, respectively. The standard deviation of the differences between GATE and measurements (±0.5% for homogeneous and ±0.8% for PSQA plans) was applied as tolerance when comparing TPSs with GATE. 72% and 60% of the plans were within the GATE predicted dose difference for both TPSs, for homogeneous and PSQA cases, respectively. Significance. Developing and validating a MC beam model early on into the commissioning of new proton therapy facilities can support the validation of the TPS and facilitate comprehensive investigation of its capabilities and limitations.

ACPSEM position paper: pre-treatment patient specific plan checks and quality assurance in radiation oncology.

The Australasian College of Physical Scientists and Engineers in Medicine (ACPSEM) has not previously made recommendations outlining the requirements for physics plan checks in Australia and New Zealand. A recent workforce modelling exercise, undertaken by the ACPSEM, revealed that the workload of a clinical radiation oncology medical physicist can comprise of up to 50% patient specific quality assurance activities. Therefore, in 2022 the ACPSEM Radiation Oncology Specialty Group (ROSG) set up a working group to address this issue. This position paper authored by ROSG endorses the recommendations of the American Association of Physicists in Medicine (AAPM) Task Group 218, 219 and 275 reports with some contextualisation for the Australia and New Zealand settings. Plan complexity metrics for head and neck VMAT competition plans

Previous plan competitions have largely focused on dose metric assessments. However, whether the submitted plans were realistic and reasonable from a quality assurance (QA) perspective remains unclear. This study aimed to investigate the relationship between aperture-based plan complexity metrics (PCM) in volumetric modulated arc therapy (VMAT) competition plans and clinical treatment plans verified through patient-specific QA (PSQA). In addition, the association of PCMs with plan quality was examined. A head and neck (HN) plan competition was held for Japanese institutions from June 2019 to July 2019, in which 210 competition plans were submitted. Dose distribution quality was quantified based on dose-volume histogram (DVH) metrics by calculating the dose distribution plan score (DDPS). Differences in PCMs between the two VMAT treatment plan groups (HN plan competitions held in Japan and clinically accepted HN VMAT plans through PSQA) were investigated. The mean (± standard deviation) DDPS for the 98 HN competition plans was 158.5 ± 20.6 (maximum DDPS: 200). DDPS showed a weak correlation with PCMs with a maximum r of 0.45 for monitor unit (MU); its correlation with some PCMs was "very weak." Significant differences were found in some PCMs between plans with the highest 20% DDPSs and the remaining plans. The clinical VMAT and competition plans revealed similar distributions for some PCMs. Deviations in PCMs for the two groups were comparable, indicating considerable variability among planners regarding planning skills. The plan complexity for HN VMAT competition plans increased for high-quality plans, as shown by the dose distribution. Direct comparison of PCMs between competition plans and clinically accepted plans showed that the submitted HN VMAT competition plans were realistic and reasonable from the QA perspective. This evaluation may provide a set of criteria for evaluating plan quality in plan competitions.

Single plan techniques for multiple brain targets (MBT) stereotactic radiosurgery (SRS) are now routine. Patient specific quality assurance (QA) for MBT poses challenges due to the limited capabilities of existing QA tools which necessitates several plan redeliveries. This study sought to develop an SRS QA phantom that enables flexible MBT patient specific QA in a single delivery, along with complex SRS commissioning. PLA marble and PLA StoneFil materials were selected based on the literature and previous research conducted in our department. The HU numbers were investigated to determine the appropriate percentage infill for skull and soft-tissue equivalence. A Prusa MK3S printer in conjunction with the above-mentioned filaments were used to print the SRS QA phantom. Quality control (QC) was performed on the printed skull, film inserts and plugs for point dose measurements. EBT3 film and point dose measurements were performed using a CC04 ionisation chamber. QC demonstrated that the SRS QA phantom transverse, coronal and sagittal film planes were orthogonal within 0.5°. HU numbers for the skull, film inserts and plugs were 858 ± 20 and 35 ± 12 respectively. Point and EBT3 film dose measurements were within 2.5% and 3%/2mm 95% gamma pass rate, respectively except one Gross Tumour Volume (GTV) that had a slightly lower gamma pass rate. Dose distributions to five GTVs were measured with EBT3 film in a single plan delivery on CyberKnife. In conclusion, an SRS QA phantom was designed, and 3D printed and its use for performing complex MBT patient specific QA in a single delivery was demonstrated.
