The Effectiveness of Supportive Psychotherapy on the Anxiety and Depression Experienced by Patients Receiving Fiberoptic Bronchoscope.

ObjectivesAs the largest cohort of healthcare workers and nurses can practice as psychotherapists to integrate the psychotherapeutic interventions as part of routine care. The present study aims to evaluate the effectiveness of supportive psychotherapy (SPT) on patients who had been scheduled to undergo a fiberoptic bronchoscopy (FOB) procedure.MethodsThis study retrospectively analyzed 92 patients who underwent FOB, which was divided into the SPT group and usual-care group based on whether patients were given SPT interventions or not. The Patient Health Questionnaire-9 (PHQ-9) and Hospital Anxiety and Depression Scale (HADS) were used to determine the severity of depression and anxiety, as well as the 36-Item Short-Form Health Survey questionnaire (SF-36) to evaluate the health-related quality of life (HRQoL). Moreover, the patients' satisfaction was assessed based on the Likert 5-Point Scale.ResultsThe baseline status of anxiety, depression, and HRQoL in patients was similar in the SPT group and the usual-care group with no significant difference. Both PHQ-9 score and HADS-D score in the SPT group after intervention were lower than that in the usual-care group, accompanied by the deceased HADS-A subscale. Moreover, the improved HRQoL was found in the patients undergoing FOB after SPT interventions as compared to those receiving conventional nursing care using the SF-36 score. Additionally, the patient satisfaction in the SPT group was higher than in the usual-care group.ConclusionsThe study demonstrated that anxiety and depression, as negative emotions, can be reduced by supportive psychotherapy in patients receiving FOB with improved mental health and satisfaction with nursing care.

A Chinese adaptation of six items, self-report Hamilton Depression Scale: Factor structure and psychometric properties.

The objective of this research was to verify the psychometric characteristics of the Chinese Adaptation of self-report HAMD-6. Outpatients and inpatients who met the DSM-5 criterion for major depressive disorder (MDD) were evaluated by the Chinese self-report HAMD-6, seventeen items of Hamilton Depression Rating Scale (HAMD-17), Patient Health Questionnaire Depression Scale (PHQ-9) and Improved Clinical Global Impression Scale (iCGI-S). The internal consistency reliability, retest reliability, criterion validity and construct validity of the Chinese self-report HAMD-6 were tested. Pearson correlation coefficient was used to assess the correlativity between the total score and the item scores. By drawing the Receiver Operating Characteristics (ROC) curve, the best cut-off value, sensitivity and specificity of Chinese Adaptation self-report HAMD-6 were obtained. Cronbach's alpha coefficient of the Chinese self-report HAMD-6 was 0.91, and the intra-group correlation coefficient (ICC) of retest reliability was 0.81(P<0.01). The Spearman correlation coefficients of the Chinese self-report HAMD-6, Chinese clinician version of HAMD-6, PHQ-9 and HAMD-17 were 0.86, 0.81 and 0.86, respectively (all P<0.01). Results of the confirmatory factor analysis (CFA) supported a unidimensional construct. In addition, HAMD-17≤7 and iCGI-S=1 were taken as the remission criteria for depression disorder, and the ROC curves of the Chinese self-report HAMD-6 were plotted with a cut-off value of 3/4, the specificity and sensitivity were 0.85/0.92 and 0.96/0.93 respectively. These results suggested that the abbreviated Chinese self-report HAMD-6 has good reliability and validity among the Chinese population. This study suggested that the remission cut-off value of the scale is 3/4.

Behavioral Health Screening in Military Cystic Fibrosis Centers: A Survey.

Cystic fibrosis (CF) is the most common life-threatening genetic illness in the United States. People with CF as well as their caregivers are up to three times more likely to report experiencing symptoms of depression and anxiety than those without CF. In 2016, the Cystic Fibrosis Foundation and the European Cystic Fibrosis Society came together to form the International Committee on Mental Health in CF and released guidelines outlining behavioral health (BH) screening recommendations for patients with CF and at least one primary caregiver. This study sought to characterize the role of BH care in routine CF treatment within the DoD health care system and identify potential opportunities for improvement. The resultant brief report is intended to elucidate and present identified areas of improvement as well as to inform further research projects in this field. A representative sample of program leaders (8 of 12; five program directors and three nurse coordinators) from all six affiliate CF centers in the DoD completed a 23-item web-based survey. This study sought to identify the following: (1) What tools are DoD affiliate CF centers using to screen patients with CF and their caregiver(s) for psychological distress and how often does screening take place? (2) What is the composition of the DoD's CF BH teams by specialty and to what degree are BH personnel available to support the needs of CF patients? (3) How comfortable are program directors and nurse coordinators in screening patients with CF and their caregiver(s) for indicators of psychological distress? (4) How familiar are CF BH teams with the use of the U.S. Military's Behavioral Health Data Portal (BHDP)? This descriptive study was approved by the Human Use Committee at the Tripler Army Medical Center. The results of this study indicated that 80% of the DoD affiliate CF centers are screening patients with CF who are 12 years and older and at least one caregiver at least annually for depression and anxiety with the Patient Health Questionnaire depression module and generalized anxiety disorder screening tool, respectively. Reported screening tools for suicidality were not standardized across centers. All respondents indicated that there is a designated social worker in their CF clinic team. Three-quarters of respondents reported that their social worker is physically present in CF clinics 75%-100% of the time. Other types of BH team members varied by clinic. Program directors and nurse coordinators on average indicated feeling "somewhat comfortable" in screening patients with CF for depression, anxiety, and suicidality. Eighty percent of program directors reported being "not so comfortable" in screening caregivers for depression, anxiety, and suicidality, with nurse coordinators on average reporting feeling "somewhat comfortable." Eighty percent of affiliate CF centers indicated that they are unaware of, are not utilizing, or do not have access to the BHDP to screen and record BH data for patients with CF or their caregiver(s). This study characterized routine CF BH care at DoD affiliate CF centers. Areas for improvement include the standardized use of screening tools for suicidality, increased provider comfort with screening, and streamlined recording and tracking of this data using the BHDP. Limitations of this study include inherent self-report bias, specifically social desirability bias. Steps toward suggested improvements and further utilization of the BHDP may improve BH care for patients with CF and their caregiver(s) in addition to facilitating future research.

Comparison of Negative Pressure Wound Therapy with or without a Split-Thickness Skin Graft in the Surgical Management of Axillary Hidradenitis Suppurativa: A Retrospective Cohort Study

Introduction: Negative pressure wound therapy (NPWT) alone or with the addition of a split-thickness skin graft (STSG) are 2 reconstructive options available after surgical excision of axillary hidradenitis suppurativa (HS). The aim of this study was to retrospectively examine patients undergoing these treatments and to assess clinical and patient-related outcome measures. Methods: A single-centre, retrospective analysis was conducted, evaluating surgical excision of axillary HS, with STSG and NPWT, or NPWT alone. Data collected included No. of post-operative clinic visits, time to heal, size of wound, disease recurrence, follow-up time, Dermatology Life Quality Index (DLQI), the Generalised Anxiety Disorder Assessment (GAD-7), the Patient Health Questionnaire Depression Scale (PHQ-9), Pain Visual Analogue Scale (PAINVAS2), the Brief Illness Perception Questionnaire (BIPQ), and Dermatology Visual Analogue Scale (DERMVAS). Two-tailed t-test and Mann–Whitney Wilcoxon U-tests were used to assess for significant relationships. Results: One hundred five patients were included in the study, 44 who received NPWT alone, and 61 who received NPWT + STSG. There was no significant difference in follow-up time ( P = .934) or No. of follow-up appointments between groups ( P = .287). There was a significant difference in time to heal between groups, with STSG + NPWT observing a mean time of 2.77 months and NPWT alone observing a mean time of 4.40 months ( P = .0006). There was no difference in patient-reported outcomes between the 2 groups. Conclusion: There is no difference in patient-reported outcomes with the addition of an STSG to NPWT after surgical excision of HS. Wide excision and use of NPWT alone is an effective procedure for the treatment of axillary HS.

Change in Asthma Is Associated with Change in PTSD in World Trade Center Health Registrants, 2011 to 2016.

The WTC Health Registry (WTCHR) is a closed, longitudinal cohort of rescue/recovery workers and survivors exposed to the 11 September 2001 disaster. WTCHR enrollees diagnosed with asthma after 11 September 2001 continued to experience poor control despite treatment. Asthma is associated with mental problems, although their bidirectional movement has not been studied. This study tested whether a clinical change in mental problems was associated with a difference in asthma control, and whether a change in asthma control varied with a change in quality of life (QoL). Difference in the Asthma Control Test (ACT) on the WTCHR from 2011–12 to 2015–16 was compared with the change in the Post-traumatic Stress Disorder Checklist (PCL-17), the Patient Health Questionnaire depression scale, self-reported heartburn, and change of physical and mental QoL over this period. In adjusted multinomial multivariable logistic regression, improved PCL-17 was associated with a better ACT score, odds ratio (OR) = 1.42 (95% C.I. 1.01, 1.99), and a worsened PCL-17 score was associated with a worsened ACT score, OR = 1.77 (95% C.I. 1.26, 2.50). Decreased ACT was associated with poor physical QoL, OR = 1.97 (95% C.I. 1.48, 2.62). Change in mental health measures tracked with change in asthma control, which correlated with a change in QoL. Careful follow-up and treatment of all three are indicated to improve these inter-related issues.

Predicting Depression in Adolescents Using Mobile and Wearable Sensors: Multimodal Machine Learning-Based Exploratory Study.

BackgroundDepression levels in adolescents have trended upward over the past several years. According to a 2020 survey by the National Survey on Drug Use and Health, 4.1 million US adolescents have experienced at least one major depressive episode. This number constitutes approximately 16% of adolescents aged 12 to 17 years. However, only 32.3% of adolescents received some form of specialized or nonspecialized treatment. Identifying worsening symptoms earlier using mobile and wearable sensors may lead to earlier intervention. Most studies on predicting depression using sensor-based data are geared toward the adult population. Very few studies look into predicting depression in adolescents.ObjectiveThe aim of our work was to study passively sensed data from adolescents with depression and investigate the predictive capabilities of 2 machine learning approaches to predict depression scores and change in depression levels in adolescents. This work also provided an in-depth analysis of sensor features that serve as key indicators of change in depressive symptoms and the effect of variation of data samples on model accuracy levels.MethodsThis study included 55 adolescents with symptoms of depression aged 12 to 17 years. Each participant was passively monitored through smartphone sensors and Fitbit wearable devices for 24 weeks. Passive sensors collected call, conversation, location, and heart rate information daily. Following data preprocessing, 67% (37/55) of the participants in the aggregated data set were analyzed. Weekly Patient Health Questionnaire-9 surveys answered by participants served as the ground truth. We applied regression-based approaches to predict the Patient Health Questionnaire-9 depression score and change in depression severity. These approaches were consolidated using universal and personalized modeling strategies. The universal strategies consisted of Leave One Participant Out and Leave Week X Out. The personalized strategy models were based on Accumulated Weeks and Leave One Week One User Instance Out. Linear and nonlinear machine learning algorithms were trained to model the data.ResultsWe observed that personalized approaches performed better on adolescent depression prediction compared with universal approaches. The best models were able to predict depression score and weekly change in depression level with root mean squared errors of 2.83 and 3.21, respectively, following the Accumulated Weeks personalized modeling strategy. Our feature importance investigation showed that the contribution of screen-, call-, and location-based features influenced optimal models and were predictive of adolescent depression.ConclusionsThis study provides insight into the feasibility of using passively sensed data for predicting adolescent depression. We demonstrated prediction capabilities in terms of depression score and change in depression level. The prediction results revealed that personalized models performed better on adolescents than universal approaches. Feature importance provided a better understanding of depression and sensor data. Our findings can help in the development of advanced adolescent depression predictions.

PB2354: THE IMPACT OF ACUTE HEPATIC PORPHYRIA ON MENTAL HEALTH: RESULTS FROM THE PORPHYRIA WORLDWIDE PATIENT EXPERIENCE RESEARCH (POWER) STUDY

Background: Acute hepatic porphyria (AHP) is a group of rare genetic diseases of heme biosynthesis characterised by potentially life-threatening attacks, most frequently occurring in the abdomen. AHP is also associated with psychiatric symptoms, including depression and anxiety. Prior studies on AHP have also documented its psychosocial burden, which can limit patients’ day-to-day activities, including social interactions and ability to work. Aims: To investigate the burden of AHP on mental health, the study evaluated the effects of the disease on social life, personal life/goals, depression, and anxiety. Methods: Patients age ≥18 years who experienced >1 AHP attack within the past 2 years or who were receiving intravenous hemin and/or glucose for attack prevention were recruited from the United States, Italy, Spain, Australia, Mexico, and Brazil from January 19 to April 26, 2021. Patients taking givosiran were excluded. Participants were administered an online survey that used standardised questionnaires and validated screening instruments to evaluate the impact of AHP symptoms on social life, personal life/goals, and mental health. The 8-item Patient Health Questionnaire depression scale (PHQ-8; range, 0–24) and the 7-item Generalized Anxiety Disorder scale (GAD-7; range, 0–21) were used to screen patients for depression and anxiety. Current moderate to severe depression was identified with a PHQ-8 cutoff score of ≥10, and threshold scores of 5, 10, and 15 on the GAD-7 identified mild, moderate, and severe anxiety, respectively. Depression and anxiety were evaluated among the overall patient population as well as several subgroups, including those with sporadic attacks (0–5 attacks over 2 years) vs recurrent attacks (≥6 attacks over 2 years), those receiving vs not receiving prophylactic treatment for AHP, and those with an active disease duration of 0–5 years vs those with a disease duration of ≥6 years. Results: Ninety-two patients with AHP completed the survey. Mean age was 41.1 years; 90% were female. Patients reported substantial impact on social life—76.1% of patients reported that most of their symptoms were hidden and that people in their social circle did not know they had AHP. An equal percentage of patients reported feeling frustrated that people do not understand the challenges they face, because their symptoms are not visible. Similarly, 72.8% of patients reported feeling guilty/upset that their symptoms and disabilities affect others (Figure 1A). Patients also reported AHP has impacted their personal life/goals. For example, >80% of patients reported having had to modify or give up goals important to them, more than half reported that the decline in their mental and physical health feels never-ending, and over a third reported they had lost their sense of purpose (Figure 1B). PHQ-8 scores indicating moderate to severe depression were reported in more than half of patients with AHP (58.7%) regardless of attack rate or prophylactic treatment status. GAD-7 scores indicating moderate to severe anxiety were reported in 48.9% of patients, and were highest in patients experiencing recurrent attacks (56.8%) (Figure 1C). Image:Summary/Conclusion: Patients with AHP, regardless of attack rate, treatment received, or duration of active disease, experience a high mental health burden on their personal and social life. This study suggests that approximately half of patients with AHP experience moderate to severe anxiety or depression. These results highlight the importance of mental health monitoring as part of disease management for AHP.

Improving Employee Mental Health: A Health Facility-Based Study in the United States

BackgroundIn the US, over 52.9 million (21%) adults lived with a mental health illness in 2020, with depression, being one of the commonest of these conditions. The World Health Organization ranks depression as the most important contributor to global disability. As frontline workers who are responsible for taking care of a myriad of patients daily, health workers are usually exposed to depressive situations which eventually result in the development of the condition among them. This study, therefore, developed an intervention to reduce depression among workers at the Outpatient Mental Health Clinic in Washington District of Columbia, United States.MethodsA pre-intervention survey was conducted among 43 employees. The survey used the already validated Patient Health Questionnaire depression scale (PHQ-9) to determine the prevalence of depression. The WHO Healthy Workplace Model was adopted in designing an instrument for the workplace determinants of depression. An mHealth intervention was then developed and implemented among the workers. After this, a post-intervention survey was conducted among the cohort. Descriptive and inferential statistics were adopted in analyzing the data with STATA.ResultsThe pre-intervention survey showed a depression prevalence of 30.2% among the employees. The post-intervention survey, however, showed that the prevalence of depression among the employees reduced to 12.6%. The surveys also showed that the majority of employees who felt exposed to workplace hazards including harmful chemicals, expressed feelings of depression (pre-intervention = 53.6%; post-intervention = 80%).ConclusionThe intervention designed for this study was effective in reducing self-reported depression among employees. Improving employee mental health in health care facilities will require awareness raising among employees, mental health friendly policies, and regular follow up of employee mental health needs. Though this intervention was on a small scale, it shows promise for using cheap mhealth solutions in improving mental health at the work place.

Postpartum Care in the Time of COVID-19: The Use of Telemedicine for Postpartum Care.

Background: Telemedicine was implemented at our institution in response to the COVID-19 pandemic. Data do not currently exist about the use of telemedicine in providing comprehensive postpartum care. Objective: This project aimed to evaluate the impact of telemedicine on postpartum care at an urban Federally Qualified Health Center (FQHC). Study Design: This was a retrospective cohort study of patients who delivered at an urban hospital in New York between September and November 2019 (pre-COVID), February through April 2020 (peak-COVID) and June through August 2020 (ongoing-COVID). The primary outcome was postpartum visit attendance. Secondary outcomes included contraception use, breastfeeding, depression screening, hospital readmission, and emergency department visit rates. Log-binomial regression models were used to estimate relative risk. Results: Telemedicine accounted for 1% of postpartum visits in the pre-COVID cohort, 60% in the peak-COVID cohort, and 48% in the ongoing-COVID cohort. Postpartum visit attendance rates were 52% in the pre-COVID cohort, 43% in the peak-COVID cohort, and 56% in the ongoing-COVID cohort (p > 0.05). There was a nonsignificant increase in postpartum visit show rate for telemedicine visits compared to in-person visits in the peak-COVID cohort (76% vs. 65%; relative risk [RR] 1.17 [0.87-1.57]) and ongoing-COVID cohort (85% vs. 74%; RR 1.16 [0.90-1.50]). Patients were significantly less likely to have a Patient Health Questionnaire-2 Depression screen in the peak-COVID and ongoing-COVID cohorts (22% and 33%) than in the pre-COVID cohort (74%) (p < 0.01). There were no significant differences in hospital readmissions, contraceptive use or breastfeeding rates across cohorts (p > 0.05). Conclusions: At our urban FQHC, telemedicine was comparable to in-person postpartum care in terms of attendance rates during the COVID-19 pandemic, without an increase in rates of hospital visits or readmissions. However, postpartum depression screening needs to be better integrated into the telemedicine visit type.

Depression and health-related quality of life in critical COVID-19 survivors

IntroductionLong-term neuropsychiatric consequences of critical illness are well known. Therefore, it is expected that critical COVID-19 patients might also present several psychiatric symptoms such as depression, with inevitable negative effect on health-related quality of life (HRQoL), commonly used as an indicator of illness and treatment impact.ObjectivesTo identify depressive symptoms in critical COVID-19 survivors and to examine its association with HRQoL domains.MethodsThis preliminary study involved critical COVID-19 patients admitted into the Intensive Care Medicine Department (ICMD) of a University Hospital, between October and December of 2020. Patients with an ICMD length of stay (LoS)≤24h, terminal illness, major auditory loss, or inability to communicate at the follow-up time were excluded. From 1-2 months after discharge, all participants were evaluated by telephone at follow-up appointment, with Patient Health Questionnaire (PHQ-9) (depression) and EuroQol 5-dimension 5-level EQ-5D-5L (HRQoL). This study is part of the longitudinal MAPA project.ResultsEighty-three patients were included with a median age of 63 years (range: 31-86) and the majority were male (63%). The most reported problems on EQ-5D-5L domains were usual activities (82%) and mobility (76%). About 27% presented depressive symptoms, and with more problems of self-care (68%vs41%; p=0.029), pain/discomfort (86%vs49%; p=0.002), and anxiety/depression (96%vs54%; p&lt;0.001).ConclusionsThese preliminary results are in line in previous studies in critical COVID-19 survivors, with depression being associated with worse HRQoL. Bearing this in mind, follow-up approaches with an early screening and treatment of these psychiatric symptoms will be fundamental to optimize the recovery of these patients.DisclosureNo significant relationships.

Development the societal preference-based utility value set for the Patient Health Questionnaire depression scale in Hungary

IntroductionDepression is associated with high impact on health-related quality of life (HRQoL). Health state valuations are used for cost-effectiveness analysis to provide results for health-policy interventions.ObjectivesThe study aims to estimate a population-based value set of depression described by Patient Health Questionnaire (PHQ-9). We intend to assess vignettes describing PHQ-9 health states to estimate utility valuesMethodsCurrent research elicited direct utility scores using time trade-off (TTO) method obtained from the Hungarian general population (N=2,000). TTO vignettes were created to describe hypothetical health states of depression based on the nine items of PHQ-9. The hypothetical health states were sorted orthogonally in 11 blocks, each containing 4 vignettes (combinations of no; mild; moderate; severe depression). All respondents valued the four health states of one randomly given block. Conventional TTO method was applied, using a 10-year timeframe, while the first iteration step was randomized to 1,3 and 5 year. Preference weights were estimated using regression model fitted to TTO utility results.ResultsAltogether 1,999 respondents valued overall 34 different health states. The mean age was 47.3 (16.9) years, the majority was female (57.2%). Nearly half of the respondents were secondary educated 45.4%, 27.3% higher educated and 27.2% completed primary school. The mean TTO utility of selected mild, moderate and severe depression was: 0.83; 0.82 and 0.77 respectively.ConclusionsOur results constitute the first population-based value set for PHQ-9. Utility scores give useful information for cost-effectiveness assessments. Analysis of Factors Influencing Depressive Mood: A Focus on Medical Students in Targeted Employment

ObjectiveTo study depression and its influencing factors in medical students with directed employment in medical schools. MethodsIn this study, 586 students enrolled in Wenzhou Medical University were studied, and the Patient Health Questionnaire Depression Scale (PHQ-9) was used to distinguish depressed mood from depressive symptoms and to establish a multivariate logistic regression model. ResultsUp to 60.9% of the survey respondents experienced depressive mood within two weeks, and 21.6% experienced mild or higher depressive symptoms. The majority of the students showed only depressive mood (64.5%), and depressive symptoms accounted for a small percentage (35.5%). The detection rate of depressive mood within 2 weeks was higher in female students (70.4%) than in male students. Medical specialty, whether or not students were directed medical students, whether or not students performed directed employment, being an only child, monthly living expenses, financial status, health status, personality, and self-perceived academic stress were significant factors (P<0.05) associated with depressed mood among university students. In this study, non-medical students were more likely to experience depression (P<0.001). Depression was higher among non-directed medical students (P<0.001). Among medical students with directed employment, those with non-compliance intention had a greater risk of depression (P<0.01). Medical students with depressed mood tended to self-regulate (82.1%) and had a less significant tendency to choose medication (25.3%). ConclusionsWith a high detection rate of depressive mood and symptoms and most students presenting only with depressive mood, early detection and interventions for depressive mood are essential to prevent deterioration of the target students’ condition.

Self-Efficacy for Symptom Management in Long-Term Adult Hematopoietic Stem Cell Survivors

Hematopoietic cell transplantation (HCT) survivors have a complex and multiphase recovery period. Health care delivery and psychosocial interventions for HCT survivors are challenging because many HCT recipients live great distances from the facility where they had their HCT. Therefore identifying factors associated with a patient's capability to self-manage symptoms is a significant focus of survivorship research. A patient's self-efficacy may be important for the successful management of major stressors associated with treatments and recovery. Here, we aimed to evaluate the impact of perceived self-efficacy on distress, quality of life (QoL), depression, and fatigue and identify the factors associated with lower self-efficacy. This cross-sectional study analyzed baseline data from a randomized controlled trial INSPIRE (NCT01602211) in adult (age 18 and older) survivors 2 to 10 years after HCT. Patients with recurrence or subsequent malignancy requiring cancer treatment during the 2 years before enrollment, inability to read and understand English, and lack of access to email and the Internet were excluded. Data included medical records and patient-reported outcomes including Cancer and Treatment Distress (CTXD) with 6 subscales, Patient Health Questionnaire depression scale (PHQ-8), Short Form 12 Health Survey (SF-12) physical function and mental function scores, Brief Fatigue Inventory (BFI) and Self-Efficacy. Pearson correlations were used to test bivariate associations for self-efficacy of CTXD, SF-12, BFI, and PHQ-8. General linear models were used to test the independent associations for CTXD and SF-12 outcomes with self-efficacy, controlling for selected sociodemographic and treatment covariates. Tenability of statistical model assumptions were examined, and no remediation was necessary. A total of 1078 HCT survivors were included in the analysis. Participants were 19 to 85 years (mean age 58), 53% male, and over 90% White and non-Hispanic. Only 16% reported living in a rural area. A majority received an autologous HCT (55%) and were less than 5 years from their first HCT (54%). Among the allogeneic HCT recipients, more than half (55%) had active chronic Graft-versus-Host (cGVHD) and nearly 40% were on active systemic treatment. The mean self-efficacy score was 3.01 (SD = 0.49). Female sex (P = .014), younger age at HCT, younger age at cGVHD presentation (P = .031), moderate to severe currently active cGVHD (P = .003) and household income less than $40,000 (P< .001) were associated with lower self-efficacy. In bivariate analyses, self-efficacy was negatively correlated with mean total distress (CTXD, r = −.5, P< .001) and each of the CTXD subscales. HCT survivors with higher self-efficacy also reported better physical (r 0.48, P< .001) and mental function on the SF-12 (r = 0.57, P< .001). Moreover, self-efficacy was negatively correlated with symptoms such as fatigue (r = −.44, P< .001) and depression (r = −.48, P< .001). In a regression model investigating the impact of self-efficacy on CTXD controlled for demographics and disease characteristics, lower self-efficacy was independently associated with higher distress (CTXD, β = −.232; 95% CI [−.294, −.169], P< .001). Moreover, there was a significant positive relationship between self-efficacy and both mental (β = 4.68; 95% CI [3.82, 5.54]; P< .001) and physical (β = 2.69; 95% CI [1.74, 3.64]; P< .001) components of QoL. Our study demonstrates that lower levels of self-efficacy reported by HCT survivors were independently associated with higher levels of symptoms such as fatigue and depression, lower QoL, and more cancer-related distress. Furthermore, self-efficacy was more likely to be impaired in females, younger adults, those with lower incomes, and survivors with active cGVHD. These findings support the value of self-management interventions focused on improving self-efficacy as having the potential to improve multiple symptoms and QoL in HCT survivors.

The effects of survey mode on self-reported psychological functioning: Measurement invariance and latent mean comparison across face-to-face and web modes.

Despite the increasing popularity of mixed-mode approaches to data collection, studies examining measurement equivalence across different survey modes in representative samples of the general population, particularly focusing on measures of socially sensitive psychological constructs, are sparse. In this study, we used data from a large representative sample of the Slovenian population (N = 9,900) collected as part of the third wave of the European Health Interview Survey (EHIS) to examine mode-inherent effects (i.e., measurement effects that cannot be neutralized by clever survey design) of the traditional interviewer-mediated face-to-face mode and the increasingly popular self-administered web mode on three measures of psychological functioning, namely the Patient Health Questionnaire Depression Scale (PHQ-8), the Satisfaction with Life Scale (SWLS), and the Mental Health Continuum - Short Form (MHC-SF). After controlling for self-selection propensities, our results showed strict measurement invariance for all three scales across the two survey modes, but statistically significant and notable differences in latent means, suggesting that individuals who respond face-to-face systematically report better psychological functioning than individuals who respond over the web. These results suggest significant mode inherent effects that cannot be attributed to measurement non-invariance of the scales between face-to-face and web survey modes, but most likely to social desirability bias in responses achieved in the presence of an interviewer. Administration mode effects must be considered when interpreting and comparing results obtained through different survey modes, particularly interviewer-mediated versus self-administered modes, especially when using measures of culturally desirable traits and behaviors, such as mental health and well-being.

Prevalence and risk factors for depression among training physicians in China and the United States

During their first year of medical residency (internship), 35% of training physicians in the United States suffer at least one depression episode. We assessed whether there is a similar increase of depression among first year residents in China, and identified predictors of depression in the two systems. 1006 residents across three cohorts (2016–2017, 2017–2018 and 2018–2019) at Shanghai Jiao Tong University and Peking Union Medical College were assessed in parallel with three cohorts of 7028 residents at 100 + US institutions. The Patient Health Questionnaire-9 (PHQ-9) depressive symptoms were measured at baseline and quarterly. Demographic, personal and residency factors were assessed as potential predictors of PHQ-9 depression scores. Similar to training interns in the US, the proportion of participants in China who met depression criteria at least once during the first year of residency increased substantially, from 9.1 to 35.1%. History of depression and symptoms at baseline were common factors significantly associated with depression during residency. By contrast, neuroticism, early family environment, female gender and not being coupled were associated with depression risk only in the US, while young age was a predictor of depression only in China. Fear of workplace violence also was a predictor in China. Long duty hours and reduced sleep duration emerged as training predictors of depression in both countries. The magnitude of depression increase and work-related drivers of depression were similar between China and the US, suggesting a need for effective system reforms in both systems.

A Machine Learning Approach for Detecting Digital Behavioral Patterns of Depression Using Nonintrusive Smartphone Data (Complementary Path to Patient Health Questionnaire-9 Assessment): Prospective Observational Study.

BackgroundDepression is a major global cause of morbidity, an economic burden, and the greatest health challenge leading to chronic disability. Mobile monitoring of mental conditions has long been a sought-after metric to overcome the problems associated with the screening, diagnosis, and monitoring of depression and its heterogeneous presentation. The widespread availability of smartphones has made it possible to use their data to generate digital behavioral models that can be used for both clinical and remote screening and monitoring purposes. This study is novel as it adds to the field by conducting a trial using private and nonintrusive sensors that can help detect and monitor depression in a continuous, passive manner.ObjectiveThis study demonstrates a novel mental behavioral profiling metric (the Mental Health Similarity Score), derived from analyzing passively monitored, private, and nonintrusive smartphone use data, to identify and track depressive behavior and its progression.MethodsSmartphone data sets and self-reported Patient Health Questionnaire-9 (PHQ-9) depression assessments were collected from 558 smartphone users on the Android operating system in an observational study over an average of 10.7 (SD 23.7) days. We quantified 37 digital behavioral markers from the passive smartphone data set and explored the relationship between the digital behavioral markers and depression using correlation coefficients and random forest models. We leveraged 4 supervised machine learning classification algorithms to predict depression and its severity using PHQ-9 scores as the ground truth. We also quantified an additional 3 digital markers from gyroscope sensors and explored their feasibility in improving the model’s accuracy in detecting depression.ResultsThe PHQ-9 2-class model (none vs severe) achieved the following metrics: precision of 85% to 89%, recall of 85% to 89%, F1 of 87%, and accuracy of 87%. The PHQ-9 3-class model (none vs mild vs severe) achieved the following metrics: precision of 74% to 86%, recall of 76% to 83%, F1 of 75% to 84%, and accuracy of 78%. A significant positive Pearson correlation was found between PHQ-9 questions 2, 6, and 9 within the severely depressed users and the mental behavioral profiling metric (r=0.73). The PHQ-9 question-specific model achieved the following metrics: precision of 76% to 80%, recall of 75% to 81%, F1 of 78% to 89%, and accuracy of 78%. When a gyroscope sensor was added as a feature, the Pearson correlation among questions 2, 6, and 9 decreased from 0.73 to 0.46. The PHQ-9 2-class model+gyro features achieved the following metrics: precision of 74% to 78%, recall of 67% to 83%, F1 of 72% to 78%, and accuracy of 76%.ConclusionsOur results demonstrate that the Mental Health Similarity Score can be used to identify and track depressive behavior and its progression with high accuracy.

COVID-19 Information Overload, Negative Emotions and Posttraumatic Stress Disorder: A Cross-Sectional Study

BackgroundUnder the context of the COVID-19 pandemic, a large amount of COVID-19-related information can cause an individual's perceived information overload, further halting the individual's psychological health. As a minor psychological discomfort could develop severe mental disorders such as posttraumatic stress disorder, it is necessary to understand the chain linkage of COVID-19 information overload turn to posttraumatic stress disorder to ensure timely intervention can be offered at each point of mental state transformation. Hence, we examined the negative outcomes of COVID-19 information overload and investigated the direct and indirect effects of COVID-19 on posttraumatic stress disorder.MethodsA convenient sample of Chinese adults (n = 1150) was investigated by an online survey from July 2020 to March 2021. The extent of COVID-19 information overload was measured by the information overload severity scale on the text of the COVID-19 pandemic. Psychological distress symptoms were measured using a 7-item anxiety scale (GAD-7), the 9-item Patient Health Questionnaire depression module (PHQ-9), and the psychometric properties of the PTSD Checklist (PCL-C). Structural equation modeling and bootstrap methods were utilized to analyze the relationships between variables.ResultsCOVID-19 information overload is positively related to an individual's anxiety, depression, and posttraumatic stress disorder. Furthermore, COVID-19 information overload can indirectly affect an individual's PTSD symptoms by increasing the feeling of depression. R2 values of anxiety, depression, and PTSD were 0.471, 0.324, and 0.795, respectively.ConclusionCOVID-19 information overload, anxiety, depression, and PTSD are negative psychological states, and each variable is closely linked with the others, suggesting the need for potential psychological interventions at specific times. Practical public training, such as crisis coping and information filtering, is essential. Regulation of technology companies is also essential.

Psychosocial Outcomes Among Veteran and Non-Veteran Survivors of Sexual Assault.

Although it is well-established that sexual assault results in variable and long-lasting negative impacts on emotional well-being, perceptions of physical health, and relationship functioning, these "psychosocial" outcomes may vary based on the type(s) of sexual trauma experienced. To identify the differential impact of sexual trauma type(s) on psychosocial outcomes among veterans and non-veterans, we conducted a secondary analysis of data from the Comparative Health Assessment Interview Research Study, a large, national survey study sponsored by the Department of Veterans Affairs. Participants included veterans (n = 3588) and non-veterans (n = 935) who endorsed experiencing childhood sexual assault (CSA), adult sexual assault (ASA, outside of military service for veteran participants), and/or military sexual assault (MSA). Eight measures were used to assess psychosocial outcomes: Well-Being Inventory (WBI) health satisfaction and physical health functioning items, Posttraumatic Stress Disorder Checklist, Patient Health Questionnaire (depression symptoms), Generalized Anxiety Disorder Questionnaire, WBI social satisfaction items, WBI social functioning items, and the Multidimensional Scale of Perceived Social Support (social). A profile analysis was used to determine how sexual trauma type(s) influenced the pattern of responding to the eight psychosocial outcome measures. Veteran sexual assault survivors reported poorer psychological outcomes compared to non-veteran sexual assault survivors. Non-veteran sexual assault survivors reported poorer outcomes on the majority of social variables compared to veteran sexual assault survivors. Survivors of MSA-only reported poorer psychosocial outcomes compared to veteran and non-veteran survivors of CSA-only and ASA-only on most of the variables assessed. Survivors of ASA-only reported similar or modestly worse psychosocial outcomes when compared to survivors of CSA-only on the majority of variables assessed. Survivors of different types of sexual trauma reported distinct psychosocial outcomes, suggesting that assessment and treatment needs may differ by trauma type.

Evaluation of Individual Items of the Patient Health Questionnaire (PHQ-9) and Montgomery-Asberg Depression Rating Scale (MADRS) in Adults with Treatment-Resistant Depression Treated with Esketamine Nasal Spray Combined with a New Oral Antidepressant.

ObjectiveThe objective of this study was to determine which symptoms measured by the Patient Health Questionnaire (PHQ-9) and Montgomery-Asberg Depression Rating Scale (MADRS) improve in those treated with esketamine nasal spray in combination with oral antidepressant (AD) compared with those treated with placebo plus AD for adult patients with treatment-resistant depression (TRD). These results complement the interpretation of PHQ-9 and MADRS total scores.MethodsThe TRANSFORM 2 study evaluated the efficacy and safety of esketamine nasal spray in combination with AD. This post-hoc analysis used PHQ-9 and MADRS data to evaluate symptom changes. The total scores change and proportions of individual item change scores on the PHQ-9 and MADRS were evaluated at days 15 and 28; analysis of variance was used to test differences on total scores. Generalized estimation equations of logistic regression models were used to estimate the likelihood of improvement on instrument items.ResultsThe mean total score reduction of the PHQ-9, indicating improvement, was greater in the esketamine plus AD arm compared with placebo plus AD at day 15 (− 1.8; p = 0.045) and day 28 (− 2.8; p = 0.006). Proportions of those who improved (≥ 1 point on a 4-point scale and ≥ 2 points on a 7-point scale for the PHQ-9 and MADRS, respectively) was greater in the esketamine plus AD group compared with the placebo plus AD group across all items. The odds of improving for those in the esketamine plus AD group compared with the placebo plus AD group were over two times greater on the PHQ-9 items: “Little interest/pleasure in things” (OR 2.252, 95% CI 1.165–4.355); “Feeling down, depressed, or hopeless” (OR 2.767, 95% CI 1.400–5.470); and “Feeling tired or having little energy” (OR 2.171, 95% CI 1.153–4.087). The mean reduction in total scores on the MADRS, indicating improvement, was numerically greater at day 15 (− 2.0; p = 0.189) and statistically significantly greater at day 28 (− 4.4; p = 0.017) in the esketamine plus AD arm compared with placebo plus AD. The odds of improving for those in the esketamine plus AD group compared with the placebo plus AD group were over two times greater on the MADRS items measuring “Apparent sadness” (OR 2.007, 95% CI 1.096–3.674); and “Inability to feel” (OR 2.099, 95% CI 1.180–3.735).ConclusionImprovement in mean total scores in those treated with esketamine plus AD compared with placebo plus AD are important results to confirm efficacy. The odds of improving in those treated with esketamine plus AD was at least two times greater than with placebo plus AD on three patient- and two clinician-reported individual symptoms of TRD. These findings provide patient-relevant quantification of the esketamine plus AD treatment benefit, adding understanding as to which symptoms are most improved with treatment.Trial RegistrationClinicalTrials.gov identifier: NCT02418585; first posted 16 April 2015.

Low cholesterol is not associated with depression: data from the 2005-2018 National Health and Nutrition Examination Survey

BackgroundAlthough high serum cholesterol is widely recognized as a major risk factor for heart disease, the health effects of low cholesterol are less clear. Several studies have found a correlation between low cholesterol and depression, but the results are inconsistent.MethodsData from the National Health and Nutrition Examination Survey (NHANES) 2005-2018 were utilized in this cross-sectional study. The analysis of the relationship between cholesterol and depression was performed at three levels: low total cholesterol, low high-density lipoprotein (HDL) cholesterol and low-density lipoprotein (LDL) cholesterol. The inclusion criteria were as follows: (1) people with low (<4.14 mmol/L) or normal (4.14-5.16 mmol/L) total cholesterol for Sample 1; people with low (<1 mmol/L) or normal (≥1 mmol/L) HDL cholesterol levels for Sample 2; and people with low (<1.8 mmol/L) or normal (1.8-3.4 mmol/L) LDL cholesterol levels for Sample 3; and (2) people who completed the Patient Health Questionnaire-9 depression scale. Age, sex, educational level, race, marital status, self-rated health, alcohol status, smoking status, body mass index (BMI), poverty income ratio, physical function, comorbidities, and prescription use were considered potential confounders. The missing data were handled by multiple imputations of chained equations. Logistic regression was used to assess the relationship between low cholesterol and depression.ResultsAfter controlling for potential confounding factors in the multivariate logistic regression, no association was observed between depression and low total cholesterol (OR=1.0, 95% CI: 0.9-1.2), low LDL cholesterol (OR=1.0, 95% CI: 0.8-1.4), or low HDL cholesterol (OR=0.9, 95% CI: 0.8-1.1). The results stratified by sex also showed no association between low total cholesterol, low LDL cholesterol, low HDL cholesterol and depression in either men or women.ConclusionThis population-based study did not support the assumption that low cholesterol was related to a higher risk of depression. This information may contribute to the debate on how to manage people with low cholesterol in clinical practice.
