Comparison of patellofemoral contact pressure after semi-cylindrical recession trochleoplasty and trochlear block recession in feline cadavers.

The purpose of this study was to compare the changes in the patellofemoral joint (PFJ) contact mechanisms of the normal state, trochlear hypoplasia model and after performing trochleoplasty on the hypoplasia model in feline cadavers. Twenty normal pelvic limbs were acquired from the 10 feline cadavers. First, the PFJ contact mechanisms were measured in normal state, then trochlear hypoplasia models were created using customized trochlear ridge cutting guides. After measuring PFJ contact mechanisms in the trochlear hypoplasia models, they were divided into two groups and performed semi-cylindrical recession trochleoplasty (SCRT) and trochlear block recession (TBR) were performed, respectively. After that, the PFJ contact mechanisms were measured and the values of the 4 groups (normal state, trochlear hypoplasia, SCRT, TBR) were compared. The trochlear hypoplasia group showed increased contact pressure and decreased contact areas compared to the normal state group. In the groups that underwent tracheoplasty (SCRT and TBR), PFJ contact mechanisms were recovered similarly to that of the normal state group. The PFJ of the SCRP group was measured similar to that of the normal group than that of the TBR group. Tracheoplasty can be useful in restoring PFJ contact mechanisms and SCRT can be considered as a good alternative to the conventional methods of trochleoplasty.

Adding Tibial Tuberosity Medialization to Medial Patellofemoral Ligament Reconstruction Reduces Lateral Patellar Maltracking During Multidirectional Motion in a Computational Simulation Model

To determine whether adding tibial tuberosity medialization to medial patellofemoral ligament (MPFL) reconstruction reduces lateral patellar maltracking during a dynamic multidirectional activity and to investigate when medial patellofemoral contact pressures are elevated during daily activities, such as squatting. Seven computational models representing knees with patellar instability, including lateral patellar maltracking, were evaluated following simulated MPFL reconstruction (bisect offset index > .75). Tibial tuberosity medialization was added to MPFL reconstruction for each model. Patellar tracking during multidirectional motion was evaluated by simulating pivot landing. Analysis of pivoting focused on early flexion (5° to 40°). Patellofemoral contact pressures during daily function were evaluated by simulating knee squatting. Data were analyzed with paired comparisons between MPFL reconstruction with and without tuberosity medialization. The patella dislocated during pivoting for 2 models with an isolated MPFL reconstruction and for 1 model including tibial tuberosity medialization. Adding tibial tuberosity medialization to MPFL reconstruction significantly decreased bisect offset index by ∼0.1 from 5° to 40° (P < .03). For knee squatting, medializing the tibial tuberosity significantly increased maximum medial contract pressure by ∼0.5 MPa from 30° to 85° (P < .05) but did not significantly influence maximum lateral pressure. In this study of simulated multidirectional motion, MPFL reconstruction did not sufficiently constrain the patella for some knees. Adding tibial tuberosity medialization to MPFL reconstruction in these models reduced lateral patellar maltracking during multidirectional motion but increased pressure applied to medial cartilage during squatting. After establishing the influence of tibial tuberosity medialization on patellar maltracking for an idealized population, as was done in the current study, future simulation studies can be performed to better determine the anatomical characteristics of patients for whom tibial tuberosity medialization is needed to reduce the risk of postoperative patellar maltracking.

Poster 129: Biomechanical Evaluation of the Isometry of Medial Patellofemoral Ligament Reconstruction Grafts Fixed at Non-Anatomic Femoral Insertion Points

Objectives: In MPFL reconstruction surgery, achieving perfect placement of the femoral tunnel intraoperatively is difficult. An understanding of the effects femoral tunnel malpositioning has on graft isometry is crucial to minimizing instability or overconstraint which leads to anterior knee pain, increased patellofemoral contact pressures, or graft failure. The purpose of this study was to identify differences in MPFL reconstruction graft isometry with femoral tunnel malpositioning, specifically evaluating isometry differences as the femoral position is moved anterior, posterior, proximal, and distal relative to the femoral radiographic landmark of the MPFL, Schottle’s point. Methods: A biomechanical study evaluating 11 fresh frozen cadaveric knees was conducted. Non-elastic suture, used as an MPFL graft analogue, was anchored at Schottle’s point with the knee at 30° of flexion, and at 5 mm and 10 mm anterior, posterior, superior, and distal to Schottle’s point. A draw wire displacement sensor was used to evaluate length changes of the MPFL graft analogue through 0-120° of knee flexion. Knee flexion position was continuously measured using a motion tracking system. Pairwise t-tests with Bonferroni corrections were used to compare isometry between Schottle’s point and non- anatomic femoral insertion points. Results: Grafts placed at Schottle’s point demonstrated mild anisometry with tightening in extension and loosening in flexion. Similarly, grafts placed distally and posteriorly also demonstrated tightening in extension and loosening in flexion. Grafts placed anteriorly and proximally demonstrated tightening in flexion. Mean and range of total draw wire excursion over full ROM across specimens is shown in Figure 1. The mean draw wire length was found to be greater with further displacement from Schottle’s point in all directions but posterior. Pairwise comparisons relative to Schottle’s point found grafts placed in the proximal or distal direction demonstrated significant differences in MPFL excursion, while grafts placed in the anterior or posterior direction did not (10 mm proximal: 0.36 (p=0.028), 5 mm distal: 0.14 (p=0.012), 10 mm distal: 0.22 (p&lt;0.001)) (Table 1). Conclusions: In order to minimize the risk of graft tightening during knee flexion, if deviating from Schottle’s point, it is safest to err posterior or distal in femoral tunnel position. Travel away from Schottle’s point in the proximal or distal direction has the greatest effects on graft isometry. The greatest risk of overconstraint and graft tightening in flexion is seen with proximal placement of the femoral insertion point.

Poster 132: Femoral length on lateral knee radiographs influences accuracy of radiographic landmarks for MPFC reconstruction

Objectives: Accuracy of femoral tunnel positioning is critical during medial patellofemoral complex (MPFC) reconstruction, as even 5mm of malpositioning has been associated with altered patellofemoral contact pressures. To help identify the appropriate position for femoral tunnel placement, Schottle and others have described radiographic landmarks to identify the MPFC footprint on lateral knee radiographs. These measurements are based on an extension of the posterior cortical line, which can vary based on the length of the femur visible on the radiograph. Because the scope of view can vary between intraoperative imaging modalities, the aim of this study was to assess the effect of femoral length on the accuracy of radiographic landmarks of the MPFC. Methods: In 9 unpaired cadaveric knees, the MPFC footprint was exposed on the medial femur, and the proximal and distal boundaries of the footprint were marked. Lateral fluoroscopic images of the knee were obtained and assessed in 1 cm length increments, beginning 1cm proximal to the posterior condyle and continuing proximally to a femoral length of 8 cms. The MPFC midpoint was described on each image relative to the posterior cortical line of the femur and a line perpendicular to this line, relative to the proximal margin of the posterior condyle. Linear regression analysis was used to assess the effect of femoral length on the radiographic position of the MPFC. ROC curve analysis and Delong test were used, and the minimum amount of femoral length required on radiographs to accurately identify an anatomic femoral tunnel was determined using Youden’s J statistic. Results: Using the posterior cortical line as a reference, the radiographic description of the MPFC footprint moved anteriorly with decreasing femoral length on the radiographs, particularly at 4cm and less. However, no proximal-distal change was seen in relation to the line through the proximal margin of the posterior condyle with changing femoral lengths. Linear regression analysis showed a significant relationship between the femoral length and anterior radiographic position of the MPFC on radiographs (R = 0.461, R2 = 0.212, B= -0.636, p &lt; 0.001). The slope coefficient was -0.636 mm, indicating that for every cm decrease in femoral length, the actual anatomic footprint of the MPFC moves anteriorly by 0.636 mm in relation to the posterior cortical line. Furthermore, ROC curve analysis revealed that a minimum of 4 cm of femoral length on lateral radiographs is required to accurately localize the footprint of the MPFC (AUC 0.80; sensitivity 76.7 %; specificity 69 %; p &lt; 0.001). Conclusions: The radiographic landmarks for the MPFC femoral footprint can change depending on the length of the distal femur visible on radiographs. We found that at least 4cm of the femoral shaft should be visible for the radiographic landmarks to be accurate. As fluoroscopy is frequently used intraoperatively for MPFC reconstruction, our findings may serve as a guide for accurate femoral tunnel placement.

The effects of lateral meniscus posterior root tears or repair with anterior cruciate ligament reconstruction on the pressure of the patellofemoral joint: A biomechanical evaluation.

The aim of our study was to evaluate the effect of tear and repair of the lateral meniscal posterior root (LMPR) on the patellofemoral contact pressure of the knee after anterior cruciate ligament (ACL) reconstructed. This was a descriptive study. Six fresh-frozen cadaveric knees collected by The Third Hospital of Hebei Medical University from January 2019 and January 2022 were placed on a customized testing rig. Patellofemoral contact pressures were measured at 30°, 60°, and 90° of flexion using pressure-sensitive film inserted between the patella and trochlea. The following knee states were tested: ACL reconstruction and intact lateral meniscus, ACL reconstruction, and LMPR tear, and ACL reconstruction and LMPR repair. Pressure measurements were recorded for each state. In the ACL-reconstructed knee, a tear of the LMPR increased patellofemoral contact pressure at 30° of knee flexion. The repair of the posterior root by transosseous pull-out suture reduced the patellofemoral contact pressure as the status of intact lateral meniscal posterior root at 30° of knee flexion. There was no statistical difference between ACL reconstruction with the intact meniscal root and with the meniscal root tear and with the meniscal root repair at 60° and 90° of knee flexion. The posterior root tear and repair of the lateral meniscus could have an influence on patellofemoral contact stress of the knee after ACL reconstruction at 30° of knee flexion.

Influence of medial patellofemoral ligament reconstruction on patellar tracking and patellofemoral contact pressures in patella alta

Knee squatting was simulated to characterize the influence of medial patellofemoral ligament (MPFL) reconstruction on patellar tracking and contact pressures for knees with mild patella alta (Caton-Deschamps index = 1.3-1.4). Eight computational models represented knees in the pre-operative condition and following MPFL reconstruction. MPFL reconstruction significantly reduced patellar lateral tracking at low flexion angles based on bisect offset index, significantly decreased the maximum lateral pressure in mid-flexion, and significantly increased the maximum medial pressure in mid-flexion. MPFL reconstruction improves patellar stability for knees with mild patella alta and can reduce the pressure applied to lateral cartilage on the patella.

Lateral retinacular release in concordance with medial patellofemoral ligament reconstruction in patients with recurrent patellar instability: A computational model

BackgroundThe aim of this study was to develop and validate a finite element (FE) model of the patellofemoral joint to analyze the biomechanics of lateral retinacular release after medial patellofemoral ligament (MPFL) reconstruction in patellar malalignment (increased tibial tubercle–trochlear groove distance (TT-TG)). We hypothesized that lateral retinacular release is not appropriate in patellar instability addressed by MPFL reconstruction due to decreased lateral stability and inappropriate adjustment in patellofemoral contact pressures. MethodsA FE in-silico model of the patellofemoral joint was developed and validated. The model was used analyze the effect of lateral retinacular release in association with MPFL reconstruction on patellofemoral contact pressures, contact area, and lateral patellar displacement during knee flexion. ResultsMPFL reconstruction alone results in restoration of patellofemoral contact pressures throughout the entire range of motion (0–90°), mimicking the results from healthy condition. The addition of the lateral retinacular release to the MPFL reconstruction resulted in significant reductions in both patellofemoral contact pressure and contact area. Lateral retinacular release resulted in more lateral patellar displacement during the mid-flexion knee range of motion. ConclusionsCombination of lateral retinacular release with MPFL reconstruction in patients with increased TT-TG is not recommended as MPFL reconstruction alone for first-line management of recurrent patellar instability offers a greater biomechanical advantage and restoration of contact forces to resemble that of the healthy knee. The presented biomechanical data outlines the effect of concomitant MPFL reconstruction and lateral retinacular release to help guide surgical planning for patients with recurrent patellar instability due to malalignment.

Biomechanical Comparison of 3 Medial Patellofemoral Complex Reconstruction Techniques Shows Medial Overconstraint but No Significant Difference in Patella Lateralization and Contact Pressure

The purpose of this study was to investigate biomechanical differences of medial patellofemoral ligament (MPFL) reconstruction, medial quadriceps tendon femoral ligament (MQTFL) reconstruction, and a combination of these techniques to restore lateral patellar constraint and contact pressures. Eight fresh frozen cadaver knees were mounted to a custom jig with physiological quadriceps tendon loading. Flexion angles and contact pressure (CP) were dynamically measured using Tekscan® pressure sensors and Polhemus® Liberty 6 degree of freedom (6DOF) positioning sensors in the following conditions: 1) intact 2) MPFL and MQTFL deficient, 3) MPFL reconstructed, 4) Combined MPFL+ MQTFL reconstructed, and 5) MQTFL reconstructed. Lateral patellar translation was tested using horizontally directed 30 N force applied at 30° of knee flexion. The knees were flexed in dynamic fashion, and CP values were recorded for 10°, 20°, 30°, 50°, 70°, and 90° degrees of flexion. Group differences were assessed with ANOVA's followed by pairwise comparisons with Bonferroni correction. MPFL (P= .002) and combined MPFL/MQTFL (P= .034) reconstruction significantly reduced patellar lateralization from+19.28% (9.78%, 28.78%) in the deficient condition to-17.57% (-27.84%,-7.29%) and-15.56% (-33.61%, 2.30%), respectively. MPFL reconstruction was most restrictive and MQTFL reconstruction the least-7.29% (-22.01%, 7.45%). No significant differences were found between the three reconstruction techniques. Differences in CP between the three reconstruction techniques were not significant (<.02 MPa) at all flexion angles. The present study found no significant difference for patellar lateralization and patellofemoral CP between MPFL, combined MPFL/MQTFL, and MQTFL reconstruction. All 3 techniques resulted in stronger lateral patellar constraint compared to the native state, while the MQTFL reconstruction emulated the intact state the closest. Various surgical techniques for medial patellofemoral complex reconstruction can restore patellar stability with similar patellofemoral articular pressures.

Effect of Lateral Retinacular Release on Medial Patellofemoral Ligament Reconstruction.

Background:When performing a medial patellofemoral ligament (MPFL) reconstruction, surgeons may place the MPFL graft under higher than anatomic tension to minimize the chance of recurrent instability.Purpose:To investigate whether a lateral retinacular release (LRR) significantly decreases patellofemoral contact pressures after an overtensioned (OT) MPFL reconstruction.Study Design:Controlled laboratory study.Methods:Mean and peak pressure across the patellofemoral joint at 30°, 45°, and 60° of flexion was assessed in 14 cadaveric knee specimens with intact MPFL, transected MPFL, reconstructed MPFL with graft OT, and OT MPFL with LRR. The Wilcoxon signed rank test was used to determine differences across states, with W and C values calculated when possible.Results:Mean pressure decreased significantly after MPFL transection compared with intact at 30° (456.9 ± 116.8 vs 410.9 ± 109.4 N, P = .006, W < 7) and 45° (404.9 ± 91.7 vs 369.4 ± 85.3 N, P = .005, W < 5) and increased significantly from intact to OT graft at 30° (456.9 ± 116.8 vs 563.0 ± 11.2 N, P = .003, W < 7), 45° (404.9 ± 91.7 vs 481.4 ± 14.8 N, P = .005, W < 5), and 60° (272.9 ± 139.0 vs 367.0 ± 53.7 N, P = .007, W < 3). Peak pressure increased significantly between intact and OT graft at 30° (1364.0 ± 478.2 vs 2094.4 ± 619.8 N, P = .002, W < 9), 45° (1224.7 ± 491.5 vs 1676.7 ± 779.1 N, P = .005, W < 5), and 60° (1117.7 ± 566.8 vs 1604.2 ± 772.9 N, W < 3). In knees with significantly increased mean pressure after overtensioning, mean pressure increased by 23.3% (11/14 knees) at 30°, 18.3% (10/14 knees) at 45°, and 35.0% (10/14 knees) at 60°. Peak pressure increased significantly by 35.3% (30°), 25.2% (45°), and 29.3% (60°). A significant decrease in mean pressure, toward but not to baseline, was observed between the OT and LRR states at 30° (563.0 ± 11.2 vs 501.5 ± 9.3 N, W < 7) and 60° (367.0 ± 53.7 vs 302.0 ± 13.8 N, W < 5) and a decrease in peak pressure at 30° (2094.4 ± 619.8 vs 1886.5 ± 655.3 N; W < 9).Conclusion:LRR led to a statistically significant decrease in pressure across the patellofemoral joint in knees that demonstrated increased contact pressures after an OT MPFL graft.Clinical Relevance:LRR after an MPFL reconstruction in which the MPFL graft has been OT may help reduce patellofemoral contact pressures at the time of surgery.

Groove-deepening trochleoplasty reduces lateral patellar maltracking and increases patellofemoral contact pressures: Dynamic simulation.

Groove-deepening trochleoplasty is performed to restore patellar stability by increasing the lateral constraint applied to the patella by the trochlear groove. Multibody dynamic simulation of knee function was used to characterize the influence of groove-deepening trochleoplasty on patellar tracking and patellofemoral contact pressures. Computational models were created to represent seven knees with trochlear dysplasia, indicated by a flat trochlear groove and supratrochlear spur. The models were manipulated to remove the spur and deepen the trochlear groove to represent the average shape following a trochleoplasty. Knee squatting was simulated for the preoperative and postoperative conditions. Statistically significant (p < 0.05) differences in output parameters were identified with repeated measures comparisons at every 5° of knee flexion. Trochleoplasty significantly decreased lateral patellar tracking, particularly at low knee flexion angles. Trochleoplasty decreased the peak lateral shift of the patella (bisect offset index) with the knee extended from 0.87 ± 0.14 to 0.75 ± 0.12. Trochleoplasty also significantly decreased the contact area and increased the maximum contact pressure at multiple flexion angles. Trochleoplasty decreased the average contact area by approximately 10% in mid-flexion, with a corresponding increase in the average maximum contact pressure of 13%-23%. Decreased contact area and increased contact pressures are related to altered patellofemoral congruity due to reshaping the femur without a corresponding change to the patella. Clinical significance: The results indicate groove-deepening trochleoplasty decreases lateral patellar maltracking, reducing the risk of patellar dislocations, but can elevate patellofemoral contact pressures, which could contribute to long-term degradation of cartilage.

Lateral Release With Tibial Tuberosity Transfer Alters Patellofemoral Biomechanics Promoting Multidirectional Patellar Instability

The purpose of this study was to develop and validate a finite element (FE) model of the patellofemoral (PF) joint to characterize patellofemoral instability, and to highlight the effect of lateral retinacular release in combination with tibial tuberosity transfer with respect to contact pressures (CP), contact area (CA), and kinematics during knee flexion. A comprehensive, dynamic FE model of the knee joint was developed and validated through parametric comparison of PF kinematics, CP, and CA between FE simulations and invitro, cadaveric experiments. Using this FE model, we characterized the effect of patellar instability, lateral retinacular release (LR), and tibial tuberosity transfer (TTT) in the setting of medial patellofemoral ligament injury during knee flexion. There was a high level of agreement in CP, CA, lateral patellar displacement, anterior patellar displacement, and superior patellar displacement between the FE model and the invitro data (Pvalues 0.19, 0.16, 0.81, 0.10, and 0.36, respectively). Instability conditions demonstrated the greatest CP compared to all of the other conditions. During all degrees of flexion, TTT and concomitant lateral release (TTT+ LR) decreased CP significantly. TTT alone shows a consistently lower CA compared to nonrelease conditions with subsequent lateral release further decreasing CA. The results of this study demonstrate that the FE model described reliably simulates PF kinematics and CP within 1 SD in uncomplicated cadaveric specimens. The FE model is able to show that tibial tubercle transfer in combination with lateral retinacular release markedly decreases patellofemoral CP and CA and increases lateral patellar displacement that may decrease bony stabilization of the patella within the trochlear groove and promote lateral patellar instability. The goal of surgical correction for patellar instability focuses on reestablishing normal PF kinematics. By developing an FE model that can demonstrate patient PF kinematics and the results of different surgical approaches, surgeons may tailor their treatment to the best possible outcome. Of the surgical approaches that have been described, the biomechanical effects of the combination of TTT with lateral retinacular release have not been studied. Thus, the FE analysis will help shed light on the effect of the combination of TTT with lateral retinacular release on PF kinematics.

REVIEW: PATELLOFEMORAL PAIN SYNDROME

Patellofemoral pain syndrome is a common musculoskeletal condition and a major cause for anterior knee pain. It has been associated with excessive compression between the patella and the lateral femoral condyle. It is one of those overuse disorder that can limit activity of daily living along with participation in sports. The prevalence is more on female than those of male with the ratio of 2:1 and it affect athletic female more solely due to greater internal rotation of femur during running which leads to greater hip adduction. The reason for patellofemoral pain syndrome have been multifactorial increased femoral internal rotation, decreased hip abduction and external rotation strength, decreased VMO function and lateral retinaculum tightness. These factor leads to increase in dynamic Q angle that directly increases patellofemoral contact pressure which map a way to patellofemoral pain syndrome. The literature will have an explanation about altered hip kinetics, kinematics and its association with patellofemoral pain syndrome along with rehab protocol for patients with the same condition.

Tibia Tubercle Distalization Reduces Contact Pressures in Patella Alta and Instability

Patella alta is associated with patellar instability and arthritis. Tibial tuberosity distalization is commonly performed to treat recurrent patellar instability related to patella alta. The current study was performed to characterize the influence of tuberosity distalization on patellar tracking and the pressure applied to patellofemoral cartilage. High resolution 3T MRI scans of eight patients (1 male, 7 female, mean age 17, range 13-21) being treated for patellar instability were used to create individual multibody dynamic computer simulation models with patella alta (Caton-Deschamps index ≥ 1.3). Dynamic knee squatting was simulated for 3 conditions: the native knee with patella alta; following distalization (Caton-Deschamps index = 1.0); and following distalization combined with patellar tendon tenodesis. Lateral patella tracking was measured by the bisect offset index (percentage of the patella lateral to the deepest part of the trochlear groove) and the patellofemoral contact pressures and distribution were based on discrete element analysis). Repeated measures comparisons were used to identify significant (p < 0.05) differences between the conditions. : Tibial tuberosity distalization significantly reduced lateral patellar maltracking at 10°, 15° and 25° of knee flexion. At 10°, the average bisect offset index decreased from 0.85 to 0.78. Tuberosity distalization significantly decreased the maximum pressure applied to cartilage at 15°, 20°, 30°, and 35° of flexion, with the decrease more than 20% of the pre-operative pressure. Distalization also significantly increased contact area from 15° to 40° of flexion. Tenodesis had a relatively small influence on the kinematics and pressure, compared to distalization. Tibial tuberosity distalization improves patella tracking by decreasing lateral patellar maltracking at lower flexion angles. Additionally, this improved tracking is also associated with a reduction in peak patella contact pressure at low flexion angles.

Comparison of Ligament Isometry and Patellofemoral Contact Pressures for Medial Patellofemoral Ligament Reconstruction Techniques in Skeletally Immature Patients

Background: Adult medial patellofemoral ligament (MPFL) reconstruction techniques are not appropriate for the skeletally immature patient given the proximity of the distal femoral physis. Biomechanical consequences of reconstructions aimed at avoiding the physis have not been adequately studied. Purpose: To quantify the biomechanical effects of MPFL reconstruction techniques intended for skeletally immature patients. Study Design: Controlled laboratory study Methods: Four MPFL reconstruction techniques were evaluated using a computationally augmented cadaveric model: (1) Schoettle point: adult-type reconstruction; (2) epiphyseal: socket distal to the femoral physis; (3) adductor sling: graft wrapped around the adductor tendon; (4) adductor transfer: adductor tendon transferred to patella. A custom testing frame was used to cycle 8 knees for each technique from 10° to 110° of flexion. Patellofemoral kinematics were recorded using a motion camera system, contact stresses were recorded using Tekscan pressure sensors, and MPFL length was computed using an inverse kinematics computational model. Change in MPFL length, patellar facet forces, and patellar kinematics were compared using generalized estimating equation modeling. Results: Schoettle point reconstruction was the most isometric, demonstrating isometry from 10° to 100°. The epiphyseal technique was isometric until 60°, after which the graft loosened with increasing flexion. The adductor sling and adductor transfer techniques were significantly more anisometric from 40° to 110°. Both grafts tightened with knee flexion and resulted in significantly more lateral patellar tilt versus the intact state in early flexion and significantly higher contact forces on the medial facet versus the epiphyseal technique in late flexion. Conclusion: In this cadaveric simulation, the epiphyseal technique allowed for a more isometric ligament until midflexion, when the patella engaged within the trochlear groove. The adductor sling and adductor transfer grafts became tighter in flexion, resulting in potential loss of motion, pain, graft stretching, and failure. Marginal between-condition differences in patellofemoral contact mechanics and patellar kinematics were observed in late flexion. Clinical Relevance: In the skeletally immature patient, using an epiphyseal type MPFL reconstruction with the femoral attachment site distal to the physis results in a more isometric graft compared with techniques with attachment sites proximal to the physis.

Effect of Vastus Medialis Loss on Rabbit Patellofemoral Joint Contact Pressure Distribution.

Vastus medialis (VM) weakness is thought to alter patellar tracking, thereby changing the loading of the patellofemoral joint (PFJ), resulting in patellofemoral pain. However, it is challenging to measure VM force and weakness in human studies, nor is it possible to measure the associated mechanical changes in the PFJ. To obtain fundamental insight into VM weakness and its effects on PFJ mechanics, the authors determined PFJ loading in the presence of experimentally simulated VM weakness. Skeletally mature New Zealand White rabbits were used (n = 6), and the vastus lateralis, VM, and rectus femoris were stimulated individually through 3 custom-built nerve cuff electrodes. Muscle torque and PFJ pressure distribution were measured while activating all muscles simultaneously, or when the vastus lateralis and rectus femoris were activated, while VM was not, to simulate a quadriceps muscle strength imbalance. For a given muscular joint torque, peak pressures were greater and joint contact areas were smaller when simulating VM weakness compared with the condition where all muscles were activated simultaneously. The results in the rabbit model support that VM weakness results in altered PFJ loading, which may cause patellofemoral pain, often associated with a strength imbalance of the knee extensor muscle group.

Kinematic evaluation of isolated and combined medial patellofemoral and patellotibial ligament reconstruction to address lateral patellar instability

Objectives:The roles of the medial patellofemoral and patellotibial ligaments in patellofemoral kinematics have been previously reported. However, the effect of isolated and combined surgical reconstruction of these ligaments to restore native patellofemoral kinematics is not well understood. The purpose of this study was to determine the isolated and combined effects of medial patellofemoral and patellotibial ligament deficiency and reconstruction on patellofemoral kinematics.Methods:Sixteen matched-paired female, fresh-frozen, cadaveric knee specimens with a mean age of 53.5 years (range, 26-65) were used. Specimens were tested in five conditions; 1) intact, 2) MPFL or MPTL cut, 3) MPFL and MPTL combined cut, 4) MPFL or MPTL reconstruction, and 5) MPFL and MPTL combined reconstruction. Dynamic testing was performed by taking the knees through range of motion from 0° to 90° of knee flexion at 60 mm/min using a dynamic tensile testing machine by applying a force through the quadricep tendon. Following dynamic testing, knees were tested statically using a lateral load of 45 N applied to the patella at 0°, 30°, 60°, and 90° of flexion. In both dynamic and static loading tests, a motion capture system detected patellar position for each testing state to distinguish changes in patellar kinematics. Linear mixed effects modeling was performed to compare testing states.Results:Isolated MPFL deficiency created the greatest lateral translation of the patella at all knee flexion angles (p<0.001). MPTL deficiency alone did not create significant patella instability (p=0.68 or greater), but further increased instability when the MPFL was deficient (p<0.001). Isolated MPFL and combined MPFL and MPTL reconstruction decreased lateral patella translation (p<0.001). Native patella tracking was best recreated with combined ligament reconstruction while MPFL reconstruction alone over constrained the patella in extension and under constrained in flexion.Conclusions:The MPFL plays the greatest role in medial patellar stability, but the MPTL appears to have a large influence on patella tracking. The MPTL may be most important in preventing patellofemoral osteoarthritis following patella dislocation. Further investigation is needed to evaluate patellofemoral contact pressure following medial patellar ligament reconstruction.

Comparison of Patellofemoral Kinematics and Stability After Medial Patellofemoral Ligament and Medial Quadriceps Tendon–Femoral Ligament Reconstruction

Background: There is a lack of evidence regarding the optimum extensor-sided fixation method for medial patellofemoral ligament (MPFL) reconstruction. There is increased interest in avoiding patellar drilling via soft tissue–only fixation to the distal quadriceps, thus reconstructing the medial quadriceps tendon–femoral ligament (MQTFL). The biomechanical implications of differing extensor-sided fixation constructs remain unknown. Hypothesis: The null hypothesis was there would be no differences between traditional MPFL reconstruction and MQTFL reconstruction with respect to resistance to lateral translation, patellar position, or patellofemoral contact pressures. Study Design: Controlled laboratory study. Methods: Nine adult knee specimens were mounted on a jig that applied static, physiologic loads to the quadriceps tendons. Patellar position and orientation, knee flexion angle, and patellofemoral pressure were recorded at 8 different flexion angles between 0° and 110°. Additionally, a lateral patellar excursion test was conducted wherein a load was applied directly to the patella in the lateral direction with the knee at 30° of flexion and subjected to 2-N quadriceps loads. Testing was conducted under 4 conditions: intact, transected MPFL, MQTFL reconstruction, and MPFL reconstruction. For MQTFL reconstruction, the surgical technique established by Fulkerson was employed. For MPFL reconstruction, a traditional technique was utilized. Results: The patellar excursion test showed no significant difference between the MQTFL and intact states with respect to lateral translation. MPFL reconstruction led to significantly less lateral translation (P < .05) than all other states. There were no significant differences between MPFL and MQTFL reconstructions with respect to peak patellofemoral contact pressure. MPFL and MQTFL reconstructions both resulted in increased internal rotation of the patella with the knee in full extension. Conclusion: Soft tissue-only extensor-sided fixation to the distal quadriceps (MQTFL) during patella stabilization appears to re-create native stability in this time 0 cadaver model. Fixation to the patella (MPFL) was associated with increased resistance to lateral translation. Clinical Relevance: Evolving anatomic knowledge and concern for patellar fracture has led to increased interest in MQTFL reconstruction. Both MQTFL and MPFL reconstructions restored patellofemoral stability to lateral translation without increasing contact pressures under appropriate graft tensioning, with MQTFL more closely restoring native resistance to lateral translation at the time of surgery.

Isolated MPTL reconstruction fails to restore lateral patellar stability when compared to MPFL reconstruction.

To biomechanically evaluate MPTL reconstruction and compare it with two techniques for MPFL reconstruction in regard to changes in patellofemoral contact pressures and restoration of patellar stability. This is an experimental laboratory study in eight human cadaveric knees. None had patellofemoral cartilage lesions or trochlear dysplasia as evaluated by conventional radiographs and MRI examinations. The specimens were secured in a testing apparatus, and the quadriceps was tensioned in line with the femoral shaft. Contact pressures were measured using the TekScan sensor at 30°, 60° and 90°. The sensor was placed in the patellofemoral joint through a proximal approach between femoral shaft and quadriceps tendon to not violate the medial and lateral patellofemoral complex. TekScan data were analysed to determine mean contact pressures on the medial and lateral patellar facets. Patellar lateral displacement was evaluated with the knee positioned at 30° of flexion and 9N of quadriceps load, then a lateral force of 22N was applied. The same protocol was used for each condition: native, medial patellofemoral complex lesion, medial patellofemoral ligament reconstruction (MPFL-R) using gracilis tendon, MPFL-R using quadriceps tendon transfer, and medial patellotibial ligament reconstruction (MPTL-R) using patellar tendon transfer. No statistical differences were found for mean and peak contact pressures, medial or lateral, among all three techniques. However, while both techniques of MPFL-R were able to restore the medial restraint, MPTL-R failed to restore resistance to lateral patellar translation to the native state (mean lateralization of the patella [mm]: native: 9.4; lesion: 22; gracilis MPFL-R: 8.1; quadriceps MPFL-R: 11.3; MPTL-R: 23.4 (p < 0.001). MPTL-R and both techniques for MPFL-R did not increase patellofemoral contact pressures; however, MPTL-R failed to provide a sufficient restraint against lateral patellar translation lateral translation in 30° of flexion. It, therefore, cannot be recommended as an isolated procedure for the treatment of patellar instability.

Biomechanical evaluation of PCL reconstruction with suture augmentation

PurposeThe purpose of this study was to compare kinematics and patellofemoral contact pressures of all inside and transtibial single bundle PCL reconstructions and determine if suture augmentation further improves the biomechanics of either technique. MethodsCadaveric knees were tested with a posterior drawer force, and varus, valgus, internal and external moments at 30, 60, 90, and 120° of flexion. Displacement, rotation, and patellofemoral contact pressures were compared between: Intact, PCL-deficient, All-Inside PCL reconstruction with (AI-SA) and without (AI) suture augmentation, and transtibial PCL reconstruction with (TT-SA) and without (TT) suture augmentation. ResultsSectioning the PCL increased posterior tibial translation (PTT) from intact at 60° to 120° of flexion, p < 0.001. AI PCL reconstruction improved stability from the deficient-state but had greater PTT than intact at 90° of flexion, p < 0.05. Adding suture augmentation to the AI reconstruction further reduced PTT to levels that were not statistically different from intact at all flexion angles. TT reconstructed knees had greater PTT than intact knees at 60, 90, and 120° of flexion, p < 0.01. Adding suture augmentation (TT-SA) improved posterior stability to PTT levels that were not statistically different from intact knees at 30, 60, and 120° of flexion. Patellofemoral pressures were highest in PCL-deficient knees at increased angles of flexion and were reduced after reconstruction, but this was not significant. ConclusionIn this time-zero study, both the all-inside and transtibial single bundle PCL reconstructions effectively reduce posterior translation from the deficient-PCL state. In addition, suture augmentation of both techniques provided further anterior–posterior stability.
