Tapeworm Infection Diagnosed after Campylobacter jejuni-induced Enteritis.

A 35-year-old man with fever and diarrhea visited our hospital because of white string-like fecal excretion. Based on a morphological examination of the excreted object, a Diphyllobothrium infection was suspected. Additionally, Gram staining of a fecal sample revealed Campylobacter infection. After the intraduodenal administration of meglumine/diatrizoate sodium, the tapeworm was excreted. A polymerase chain reaction-based DNA sequence analysis demonstrated that the tapeworm excreted in this case was Diphyllobothrium nihonkaiensis. This report presents a rare case of coinfection with Diphyllobothrium nihonkaiensis and Campylobacter jejuni. Therefore, it is important to consider the coexistence of other intestinal infections when diagnosing parasitic infections in patients with fever.

Domestic Parasitic Infections in Patients with Asthma and Eosinophilia in Germany - Three Cases with Learnings in the Era of Anti- IL5 Treatments.

Eosinophilic inflammation is a hallmark of asthma, and blood eosinophilia has been established as a biomarker for phenotyping asthma and predicting the response to anti-IL5 treatments. Although parasitic infections are rare in European adults, they remain an important differential diagnosis for blood eosinophilia. We present three patients with both domestic parasitic infections and asthma to raise awareness of the potential challenge of eosinophilia and to provide experience in the management of parasitic infections in the setting of planned or ongoing anti-IL5 treatment. One, a patient from Croatia with moderate asthma but severe blood eosinophilia had an underlying Strongyloides stercoralis infection, with positive stool cultures. Second, a patient with severe allergic asthma and gastrointestinal symptoms had a positive S. stercoralis titer in serology with a clinical response to treatment with ivermectin. Third, a patient with severe nonallergic eosinophilic asthma and eosinophilic granulomatosis with polyangiitis (EGPA) showed an increasing hepatic tumour under anti-IL5-receptor therapy. Positive serology confirmed the diagnosis of Echinococcus multilocularis, and albendazole therapy was initiated. Anti-IL5 therapies were safely started (Patient 2) or resumed (Patient 3) after the initiation of antiparasitic treatment. Screening for parasitic infections is useful in cases of hypereosinophilia, extrapulmonary symptoms or stay in endemic regions.

Prevalence of Intestinal Parasitic Infections among children in Kirkuk City, Iraq.

Intestinal parasites are endemic in many developing areas, and they are regarded as one of the most frequent tropical diseases, where they are responsible for adult and child morbidity and mortality. The current study is a survey to determine the prevalence of intestinal parasite infections in patients attending the General Hospital for Pediatrics in Kirkuk City from January 2021 to September 2021. Stool samples were examined by direct smear method with normal saline 0.9 % NaCl and 1% iodine-Lugol solution staining method to determine the outcomes of the current investigation. The results were assessed using the Q-square statistic test. The current study's findings revealed that intestinal parasites were reported in 69(31.7), the most affected group of patients with intestinal parasites, those 1-3 years old (11.4%), Intestinal protozoa infection were (24.3%) significantly more than helminths with (7.4%). The most common protozoa were Entamoeba histolytica (17.9%), the next common protozoan found was Giardia lamblia (6.4%). Intestinal helminth infections were reported in this study (7.4%). Males were more infected with intestinal parasites (18.4%) than females (13.3%). The highest infection rate with intestinal parasites was recorded between August (6.4%) and September (4.6%). Double infection was detected in three cases with Entamoeba histolytica and Giardia lamblia. At the same time, no triple infection was reported during this study. An observational study of the prevalence of intestinal parasite infections in patients with chronic kidney disease

Abstract Objectives: The objective of this study was to ascertain the prevalence and relationship between intestinal parasite infections (IPIs) and the severity of chronic kidney disease (CKD). Methods: One hundred and forty-two adult patients (18 years of age) with established CKD were compared to 30 controls in a case–control descriptive observational study. The primary outcome measures were the prevalence of IPIs in CKD patients and healthy controls. The secondary outcome measures were the association of IPIs with the severity of kidney disease. Results: Twenty one (14.8%) participants in the study group and 7 (23.3%) in the control group had IPIs (P = 0.278). Among the organisms causing IPI, hookworm and Entamoeba histolytica were the most common as seen in 23.8% of cases and 19.1% of cases, respectively. In different stages of CKD, the prevalence of IPI showed an increase with the increase in the grading (r = 0.2547, P = 0.036). However, the type of IPIs were similar in all three grades. Conclusion: IPIs are common among CKD patients and healthy populations in developing countries. The increasing severity of CKD increases the occurrence of IPI owing to increased uraemia and decreased immunity. Monitoring of such cases may help in improving the outcomes of such patients.

Prevalence of intestinal parasitic infections in patients with diabetes: a systematic review and meta-analysis.

Patients with diabetes are at an increased risk of intestinal parasitic infections (IPIs). We evaluated the pooled prevalence and OR of IPIs in patients with diabetes through a systematic review and meta-analysis. A systematic search was performed using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) protocol for studies reporting IPIs in patients with diabetes through 1 August 2022. The collected data were analyzed using comprehensive meta-analysis software version 2. Thirteen case-control studies and nine cross-sectional studies were included in this study. The overall prevalence of IPIs in patients with diabetes was calculated to be 24.4% (95% CI 18.8 to 31%). Considering the case-control design, the prevalence of IPIs in case (25.7%; 95% CI 18.4 to 34.5%) was higher than controls (15.5%; 95% CI 8.4 to 26.9%) and a significant correlation was observed (OR, 1.80; 95% CI 1.08 to 2.97%). Moreover, a significant correlation was seen in the prevalence of Cryptosporidium spp. (OR, 3.30%; 95% CI 1.86 to 5.86%), Blastocystis sp. (OR, 1.57%; 95% CI 1.11 to 2.22%) and hookworm (OR, 6.09%; 95% CI 1.11 to 33.41%) in the cases group. The present results revealed a higher prevalence of IPIs in patients with diabetes than in controls. Therefore, the results of this study suggest a proper health education program to preventing measures for the acquisition of IPIs in patients with diabetes.

Clinical characteristics and pathogen spectra of parasitic infections in a tertiary hospital of Shanghai: A 13-year retrospective study.

BackgroundThis study performed a follow-up investigation of parasitic infections and the evolution of the infection spectra in Shanghai and its surrounding areas in Eastern China. The current study was conducted in the Shanghai Ruijin Hospital, a tertiary hospital affiliated with Shanghai Jiao Tong University School of Medicine.MethodsThis retrospective investigation reviewed a total of 412 parasitic infections in patients admitted to the Department of Infectious Diseases, Ruijin Hospital from January 1, 2010 to July 31, 2022. Detailed information for these patients was retrieved from the Electronic Medical Record System. Analysis was performed using GraphPad Prism 5.0 and SPSS Statistics 26.ResultsOverall, 17 species of parasites were detected from the 412 admissions. Over the 13 years, the number of patients peaked in 2021 and food-born parasites (FBPs) were the primary species. During the most recent 5 years, Clonorchis sinensis, replacing Paragonimus westermani, has become the primary parasite detected among the patients, consistent with the observation that eating uncooked fish has turned into the most common route of transmission. Paragonimus westermani infections declined with age, but Cysticercus increased with age. The periods from the onset of symptoms to definite diagnosis for some patients infected with Sparganum mansoni, Paragonimus westermani, and Cysticercus were more than 6 months. Interestingly, eosinophilia was only detected in 51.83% of parasite-infected patients. In addition, superinfections of parasites were common in our study.ConclusionOur study demonstrates the transitional change in the prevalence of parasitic infection over the latest 13 years in a single center in Eastern China. The incidence of parasitic infections peaked in 2021, and the dominant parasitic species switched from a soil origin to foodborne. The direction for the diagnosis and prevention of parasitic infection among different age groups should alter according to age. It is difficult to diagnose parasitic infections and superinfections that occur in some patients. Thus, more sensitive and efficient detection methods should be developed. In addition, although eosinophilia and elevated IgE are still reliable indicators for initiating screening of parasitic infection, the development of novel parasitic diagnostic kits is still in urgent need for occult infection.

Prevalence of intestinal parasitic infection in patients at Wasit Province, Iraq

Intestinal parasites are the causative agents of number of important human infections in developing countries. The present study was aimed to determine the prevalence of protozoan and helminthes infections in diarrheal patients at Wasit province during the period from July, 2022 to September, 2022. A total of 120 stool samples from both gender and different age suffering from diarrheal, abdominal pain and fever. All stool samples were examined microscopically by the direct method for identification the trophozoite and cyst of protozoa and ova of helminthes. Only 55 samples were positive for intestinal parasite. The result showed that the percent of parasite infection among patients and control were 55 (45.83) % and 0(0%) respectively. The higher infection was recorded with Entameba histolytica was 30 (54.5%). The males infection were predominance 17(30.9%) than the females 13(23.63%). According the residence urban areas were reported12 (21.81%) and rural areas were 18(32.72%) infected with Entameba histolytica. The number and percent of co-infection with Entamoeba histolytica and Blastocystis hominis was 7(12.72%) and the male was 3(5.45%) while the female was 4(7. 27%). The infection of Giardia lamblia was 6(10.9%) and the males was 4(7.27%) while the females was 2(3.63%).

Protozoan Intestinal Parasitic Infection in Patients with Hematological Malignancies.

The aim of this study was to evaluate the frequency of gastrointestinal protozoan infection in patients with hematological malignancies (HMs) undergoing intensive hemato-oncological treatment and to determine the influence of certain biological factors on the incidence of intestinal parasite infection. Stool samples were collected from hematological malignancy patients (n = 50) hospitalized at the Department of Hematology and Transplantology of the Pomeranian Medical University in Szczecin. The control group consisted of 50 healthy participants. We used a direct smear examination and a commercial immunoenzymatic test. Intestinal protozoans were detected in 16% of patients with hematological malignancies and in 6% of individuals in the control group. In stool samples from patients with HM, cysts of Giardia intestinalis (2%), oocysts of Cryptosporidium spp. (10%), vacuolar forms of potentially pathogenic Blastocystis spp. (2%), and cysts of nonpathogenic Entamoeba coli (2%) were found. Cryptosporidium spp. and Giardia intestinalis coproantigens were detected in 5 (10%) and 1 (2%) patients with HM, respectively. In three participants from the control group, vacuolar forms of Blastocystis spp. were found. In the patients with HM, a significantly higher prevalence of intestinal parasite infection was found in individuals working in the garden without protective gloves and those in contact with animals. In patients with hematological malignancies, intestinal parasites should be excluded, even during intensive chemotherapy treatment.

The implementation of the Kinyoun staining technique in a resource-limited setting is feasible and reveals a high prevalence of intestinal cryptosporidiosis in patients with HIV

ObjectivesIn resource-limited settings, intestinal Cryptosporidial or coccidian infections are common causes of chronic diarrhea but usually remain undiagnosed by routine stool investigation. Here, the addition of the Kinyoun staining technique after stool concentration was evaluated as an easy and inexpensive method for diagnosis of intestinal parasitic infection in patients with HIV. MethodsThis cross-sectional study investigated patients with HIV with diarrhea and randomly selected patients with HIV without diarrhea as controls. Stool samples were examined by wet mount microscopy and Kinyoun staining after stool concentration. Clinical, sociodemographic, and behavioral data were collected. Statistical analysis was performed using chi-squared test and multivariate regression analysis. ResultsIn total, 163 participants were included (62.0% female, mean age 38.2 [SD ± 10.7] years). Diarrhea was present in 52.1% (85/163). The prevalence of intestinal parasites was 18.4% (30/163). Cryptosporidial infections were more frequent among patients with diarrhea (12.9% [11/85] vs 1.3% [1/78], P = 0.005) and in patients with CD4+ cell count <200 cells/µl (25.9% [7/27] vs 3.7% [5/136], P = 0.001). Risk factors for intestinal parasitic infections were diarrhea and the habit of regularly eating uncooked food. Kinyoun staining was necessary for the detection of cryptosporidiosis. ConclusionIn our cohort, the prevalence of intestinal parasitic infection was high, especially after additional use of Kinyoun staining for detection of Cryptosporidia or intestinal coccidia. Considering its clinical relevance, particularly in individuals at risk, the implementation of this technique should be considered in resource-limited settings.

Visible Worms in a Crohn’s Patient Treated with Infliximab

Question: A 23-year-old man presented with a complaint of visible worms seen after defecation. He has had small bowel and perianal Crohn’s disease diagnosed 6 months earlier, which required three abscess drainage procedures and hospitalizations. Initial treatment with a several antibiotic courses and 6-mercaptopurine lead to a slight improvement. Treatment with infliximab therapy (5 mg/kg) added to 6-mercaptopurine was instituted and resulted in marked improvement and decrease of his C-reactive protein to a near normal level. Before the fourth infliximab infusion, the patient reported visible worms, several centimeters long, seen in the toilet after defecation (Figure A). The patient had no fever, abdominal pain, rectal bleeding, anal pruritus, or skin rash. There was also no history of travel and no exacerbation of the Crohn’s disease. The infliximab infusion was deferred, and the patient was referred to our tropical clinic for consultation. Laboratory results were within normal limits. Blood eosinophils were not elevated. A repeat thorough history taking at the tropical clinic revealed that his sister also saw worms, both in the toilet after bowel movements, but also while taking a bath. Taken together with the fact that the patient lives in a private ground floor house, it was postulated that the worms arose from the sewage water pipe system. To confirm this diagnosis, the worms were analyzed by the Ministry of Health central parasitology laboratory and identified as Earthworms (disambiguation). Proper history taking may obviate an unnecessary excessive workup and prevent unjustified delays in biologics treatment or empiric anthelminthics therapy.

Prevalence of Parasitic Infections in Patients Attending Tertiary Hospital ASCOMS Jammu

Background: Intestinal parasitic infections are endemic worldwide and varies considerably from place to place. According to World health organization two billion people are infected with parasitic infections globally. This prevalence is due to low levels of sanitation, open defecation, lack of safe water supply, poor hygiene, low socio- economic status, age group and impoverished health services.Subjects and Methods:In order to know the infection rate of these intestinal parasites 2500 stool specimens were studied microscopically for parasitic infections by direct wet mount and iodine mount.Results:It was observed that only 5.28% were infected with intestinal parasites. The ova and cysts of various intestinal parasites were identified microscopically and pattern of different isolated parasites were studied and it was observed that most commonly isolated was Giardia intestinalis 69.69%, followed by Entamoebahistolytica 19.69%, Ascarislumbricoides 7.57%, Ankylostomaduodenale 1.51% and Trichuristrichuira 1.51%.Conclusion: The decrease in parasitic infections evidence the success of health education, improved sanitation and healthy lifestyle.

Opportunistic parasitic infections in patients with human immunodeficiency virus/acquired immunodeficiency syndrome: A review.

The number of human immunodeficiency virus (HIV) cases increases annually, and Indonesia has become the country with the fastest HIV/acquired immunodeficiency syndrome (AIDS) epidemic spread among the five Southeast Asian countries. Indonesia entered the critical phase of HIV/AIDS infections after 5 out of the 33 provinces, namely, Papua, Jakarta, Bali, West Java, and East Java, reported HIV/AIDS epidemic since 2004. In AIDS pathophysiology and immune-suppression are severe, thus, opportunistic intestinal parasitic infections that cause diarrhea in HIV infection may be fatal. Several studies have suggested that Cryptosporidium parvum, Isospora belli, and Blastocystis hominis are the most common intestinal protozoan parasites categorized as AIDS associated illness. Diarrhea caused by parasites is considerably suspected in the cases of chronic and persistent diarrhea in adults, in an era of increasing HIV/AIDS cases nowadays. The present review highlights the current advances in etiologic agents of HIV/AIDS opportunistic infections among countries, epidemiology and prevalence, lifecycle, risk factors, examination methods, and treatment.

EOSINOPHILIC CYSTITIS AND PARASITIC INFECTION

Introduction Eosinophilic cystitis is a rare condition characterized by eosinophilic inflammation of the bladder with few documented pediatric cases. The mechanism of disease remains unclear, although it has been associated with various disorders such as allergic diseases, malignancy, trauma, and infection. Treatment regimens are non-standardized and have included antihistamines, leukotriene antagonists, antimicrobials, steroids, and anti-inflammatory agents. Case Description A 12-year-old African American male with history of food allergy and allergic rhinitis presented to urology due to persistent hematuria and dysuria. Cystouretheroscopy with biopsy demonstrated evidence of eosinophilic cystitis. He denied use of any medications or history of international travel. CBC revealed a normal AEC of 388 cells/µL; however, he was noted to have eosinophilia to 1670 cells/µL on CBC performed four years prior. He was initially treated with phenazopyridine, oxybutynin, and cetirizine. Montelukast and ibuprofen were added for refractory symptoms. Due to consideration for immunosuppressive steroid therapy, stool O&P, Strongyloides, Trichinella, Toxocara serologies, and GI PCR panel were sent to exclude parasitic infection. Strongyloides IgG and Clostridium difficile toxin were positive. He was treated with ivermectin and metronidazole for latent Strongyloides infection and C. difficile colitis, respectively, and demonstrated interval clinical improvement. Discussion To the authors’ knowledge, this is the first case of eosinophilic cystitis associated with Strongyloides stercoralis infection. A high index of suspicion for parasitic infection in patients with eosinophilic cystitis must be maintained regardless of exposure history or peripheral eosinophil counts as chronic infection may be asymptomatic. Initiation of steroid therapy without confirming negative testing may result in life-threatening, disseminated strongyloidiasis.

Prevalence of intestinal parasitic infection in patients attending National Referral Hospital, Thimphu, from 2013 to 2015: A retrospective study

Introduction: Infection with intestinal parasites continues to be a significant public health problem worldwide. There is no information on the prevalence of these intestinal parasites in patients visiting the National Referral Hospital, Thimphu, Bhutan.&#x0D; Methods: The study utilized information of stool data from 2013 to 2015 and was analyzed using SPSS 16.0. Wet and Iodine mount technique were used for identification.&#x0D; Results: The overall intestinal prevalence rate was 2.1%. Out of 5919 stool samples, Giardia lamblia has the highest rate of 0.93%, followed by Entamoeba coli 0.15%, Trichomonahomonis 0.19% and Taeniaspp. 0.17%.Giardia lamblia infection was 1.03% in males and 0.85% in females. The overall prevalence rate of parasitic infections was highest in &gt;56 years age group with 2.83% in females and 2.0% in males.&#x0D; Conclusion: The overall prevalence of intestinal parasites is a concern in the country. Furthermore, this study provides insights to develop effective public health intervention for reducing parasitic infections in the country.

Parasitic Infections Associated with Unfavourable Outcomes in Transplant Recipients.

Introduction. The immunosuppression used after transplantation (Tx) is associated with an increased risk of opportunistic infections. In Europe, parasitic infections after Tx are much less common than viral, bacterial and fungal ones. However, diseases caused by parasites are very common in tropical countries. In the last years the number of travellers with immunosuppression visiting tropical countries has increased. Methods. We performed a literature review to evaluate a risk of parasitic infections after Tx in Europe. Results. There is a real risk of parasitic infection in patients after Tx travelling to tropical countries. Malaria, leishmaniasis, strongyloidiasis and schistosomiasis are the most dangerous and relatively common. Although the incidence of these tropical infections after Tx has not increased, the course of disease could be fatal. There are also some cosmopolitan parasitic infections dangerous for patients after Tx. The greatest threat in Europe is toxoplasmosis, especially in heart and bone marrow recipients. The most severe manifestations of toxoplasmosis are myocarditis, encephalitis and disseminated disease. Diarrhoea is one of the most common symptoms of parasitic infection. In Europe the most prevalent pathogens causing diarrhoea are Giardia duodenalis and Cryptosporidium. Conclusions. Solid organ and bone marrow transplantations, blood transfusions and immunosuppressive treatment are associated with a small but real risk of parasitic infections in European citizens. In patients with severe parasitic infection, i.e., those with lung or brain involvement or a disseminated disease, the progression is very rapid and the prognosis is bad. Establishing a diagnosis before the patient’s death is challenging.

A case of atopic dermatitis caused by Ascaris lumbricoides infection

BackgroundParasite infections stimulate total and specific IgE production that, in the case of Toxocara canis infection, corresponds to chronic allergic symptoms. There may also be other infections which have similar symptoms, such as Ascaris lumbricoides infection. Ascaris lumbricoides is a large nematode that causes abdominal pain, nausea, vomiting, bloating, anorexia and intermittent diarrhoea. Patients with ascaridiasis and high IgE levels may also have allergy-like symptoms such as asthma, urticaria and atopic dermatitis.Case presentationWe report a case of atopic dermatitis caused by Ascaris lumbricoides which shows the important role of parasitic infection in patients with long-lasting dermatitis. The patient was a 12-year old female suffering since early infancy from atopic dermatitis and asthma. She was treated for dermatitis with oral bethametasone and topical pimecrolimus with little benefit. After two cycles of mebendazole therapy, the patient showed progressive improvement of symptoms.ConclusionsIn patients with dermatitis, Ascaris lumbricoides infection should be not excluded: adequate anthelmintic treatment may result in complete regression from the disease.

Introduction: Intestinal parasite infections still are very common in Vietnam and have special cause serious consequences for human health. Mastery on the prevalence of them should be done. Materials and methods: A cross session with monitoring study were carried out to use the wet mount direct examination, Kato technique and the complete blood count for evaluation the rate of intestinal parasite infections in 590 patients attending to the Parasitology Department in Hue University Hospital. We also interviewed them to reveal their life’s hygienic condition, knowledge and risk behaviour of intestinal parasite infections. Result: The rate of soil transmitted intestinal helminth was 14.1%. Primarily, patients were infected: hookworm (8.5%), Trichiuris trichiura (4.3%) and then Clonorchis sinensis 0.9%. Being 100% of patients infected with Clonorchis sinensis had eosinophile increased and decreased significantly after 1 month of treatment response. Being 53% of patients infected with hookworm and/or Trichuris trichiura had eosinophils increased, and decrease to the normal after 1 month of treatment response. Only patients, who were infected with hookworm and/or Trichiuris trichiura, had a mild anemia, the rate of cases had mild anemia: hookworm 18.8%, Trichuris trichiura 13.3%. The rate of risk behavior of intesstinal parasite infection were: no hand washing before eating and after defecation, unregular anthelmintic treatment, eating raw beef, pork, and fish. Conclusion: Intestinal parasite infections in patients attending to the Hue University Hospital were mainly hookworm, Trichuris trichiura and Clonorchis sinensis. Increasing the eosinophils or decreasing the hemoglobin values or mild anemia is the signs of intestinal parasitic diseases. To prevention of intestinal helminth infection, it should be eating well cooking dishes, washing hands totally and regular anthelmintic treatment.
