The parapelvic renal cyst. A report of 8 cases with particular emphasis on diagnosis and management.

The clinical, radiological and therapeutic aspects of 8 patients with parapelvic cyst of the kidney are reviewed. The principal clinical features were either renal colic or dull loin pain. The main radiological findings consisted of a rounded smooth mass distorting the pelvis and elongating or dilating the calices. Complications were infection, stone, hypertension or hydronephrosis. Operation was required in 6 patients, 2 of whom underwent nephrectomy.

Hypertension and Hematuria Secondary to Parapelvic Cyst

A parapelvic cyst is a well encapsulated collection of fluid outside of the renal parenchyma. It may give rise to urinary tract obstruction. Such a cyst at the renal pelvis may cause localized hydronephrosis, cortical thinning from intrarenal compression, and secondary hypertension remediable by surgical correction. However, to our knowledge, such a case has not been previously reported in the pediatric literature. CASE REPORT An 8-year-old black boy was evaluated for asymptomatic microscopic hematuria of six months' duration. There was no history of renal or cardiovascular diseases or of hypertension in the family. Blood pressure, which was 100/60 mm Hg initially, rose to 140/94 mm Hg at six-month follow-up visit.

The kidneys are beautiful organs for ultrasonography, because of their position, well defined characteristic appearance and excellent acoustic qualities. This is highly important for assessing all pathologies leading to macromorphological changes of the kidneys including these in blunt renal trauma. We could'nt find any paper on this topic in the recent literature available for us. Our own material in renal blunt trauma includes 17 patients. There were no pathological findings in 7, and pathological findings in 10 of them. Sonographic accuracy in severe trauma of the kidney is about 100%, excluding hydronephrotic kidneys, which cannot always be distinguished from a bigger parapelvic cyst. Urogramm resp. Uronephrotomogramm should be performed and studied thoroughly. Exceptions are Iodine allergy and pregnancy.

Cystic diseases of the kidney—Radiological and pathological considerations

Cysts of the kidney are of differing etiology, progress and clinical significance. Some may be clearly distinguished by radiological examination—polycystic disease of childhood, adult polycystic disease, simple cortical cysts, medullary sponge kidney, medullary necrosis, pyelogenic cysts, inflammatory cysts, cysts of calculous disease, neoplastic cysts, parapelvic cysts, and perinephric cysts. In others, radiological studies may be of inferential help—multicystic kidney, polycystic disease of the newborn, trisomy syndrome, tuberous sclerosis complex, medullary cystic disease. A classification of renal cysts is presented, with pathologic description.
