Papuloerythroderma of Ofuji in A Young Adult

Papuloerythroderma of Ofuji (PEO) is an inflammatory skin condition with typical clinical presentation including generalized itchy papular eruptions, sparing the major skin folds in the body. They usually have peripheral eosinophilia and elevated serum IgE levels. This condition may be associated with an internal malignancy, infection or drug therapy. It usually affects the elderly and very few cases have been reported in young adults. We report a 31 year old male with PEO in the absence of an underlying inciting factor.

Clinico-etiological study of 30 erythroderma cases from tertiary center in South India

Background:Erythroderma is a morphological reaction pattern of skin having many underlying causes and finding the etiology helps in the proper management of erythroderma cases.Aim:To evaluate the clinical profile, etiology of erythroderma and to correlate clinical diagnosis with histopathology.Materials and Methods:This study was performed at the department of dermatology, Father Muller Medical College and Hospital, Mangalore, South India. We studied 30 consecutive cases of erythroderma with respect to the epidemiological, clinical and histological data. Clinico-histological correlation was analyzed for etiology of erythroderma.Results:The mean age of onset was 52.3 years with a male to female ratio of 14:1. In addition to erythroderma, other co-existent features included pruritus, fever, lymphadenopathy, and edema. Of the pre-existing dermatoses, psoriasis was the most common (33.3%) disease followed by eczema (20%), atopic dermatitis (6.6%), pityriasis rubra pilaris (3.3%) and drug-induced erythroderma (16.6%). In 16.6% of cases, etiology could not be ascertained. Clinico-histopathological correlation could be established in 73.3% of cases.Conclusion:Clinical features were identical irrespective of etiology. Detailed clinico-histopathological examination helps to establish the etiology of erythroderma.

Papuloerythroderma of Ofuji in a 41-year-old woman

Papuloerythroderma, described by Ofuji in 1984, is characterized by the occurrence of polygonal, erythematous-brown papules, covering the entire skin surface, except skin folds; these changes are accompanied by pruritus and eosinophilia. We report a case of a 41-year-old female patient with melanoerythroderma that has been intensifying since August 2010. The patient's personal and family history for atopy were negative. No triggering factors were identified in the course of hospitalizations. Preceding infections and neoplasms were excluded. The diagnosis of papuloerythroderma of Ofuji was established on the basis of major and minor criteria proposed by Torchia et al. The patient met all the five major criteria: 1) erythroderma-like eruption formed by coalescence of flat-topped, red-to-brown papules with a cobblestone-like appearance, 2) itch, 3) sparing of skin folds and creases, 4) histopathological exclusion of cutaneous lymphoma and other skin diseases, 5) absence of the causative factors such as tumors, infections, drugs and atopy. Regarding the 5 minor criteria, the patient met the following three: 1) peripheral (33.8%) and tissue eosinophilia, 2) elevated level of the total serum IgE (10935 IU/ml), 3) lymphopenia (6.6%). The patient went into remission after 9-month treatment with cyclosporine at a dose of 3 mg/kg.

Papuloerythroderma of Ofuji

Papuloerythroderma of Ofuji is a rare type of erythroderma of the elderly and consists of flat-topped erythematous papules that coalesce, forming diffuse sheets that characteristically spare skin folds (so called ‘deck-chair’ sign). The etiopathogenesis of the disease remains unknown, but a number of cases have demonstrated the connection of this disorder with malignancy, mainly cutaneous T cell lymphoma. We report a case of papuloerythroderma of Ofuji unassociated with underlying malignancy, with the biopsy specimen showing nonspecific eczematous findings. There are cases of papuloerythroderma of Ofuji associated with malignancy, and one needs to be mindful of such in examinations.

Drug-induced Papuloerythroderma: Analysis of T-cell Populations and a Literature Review

Papuloerythroderma of Ofuji is characterized by coalescent solid papules that spare the skin folds. Although cutaneous lymphomas and internal malignancies are known associated conditions, the causative agents are unclear in most cases. A number of recent reports have documente d that drugs can induce papuloerythroderma. We review ed the reported cases and our own cases of drug-induc ed papulo erythroderma, together with our data from lympho cyte transformation tests and T-cell subsets of peri pheral blood. All of the 9 patients were male, and the causative drugs were various. Provocation tests were positive in all 6 patients examined. Whereas drug patch tests were negative in all 5 cases tested, the patients' peripheral blood lymphocytes responded well to the culprit drug in 4 of 5 patients tested. The patients had higher percentages of circulating CCR4+CD4+ T helper (Th) 2 cells than CXCR3+CD4+ Th1 cells. Drug-induced papuloerythroderma seems to be mediated by Th2 cells reacting with the causative drug.

To the Editor: Papuloerythroderma was first described in 1984 by Ofuji et al.1 It may occur as a result of allergic reaction to unknown substances with T helper (Th) 2 deviation.2,3 Recently, we have found that one of the causative agents of papuloerythroderma is a drug, as documented in a patient administered with aspirin.4 Here, we describe a case of papuloerythroderma caused by furosemide.

Mycosis fungoides: a dermatological masquerader

Mycosis fungoides (MF), a low-grade lymphoproliferative disorder, is the most common type of cutaneous T-cell lymphoma. Typically, neoplastic T cells localize to the skin and produce patches, plaques, tumours or erythroderma. Diagnosis of MF can be difficult due to highly variable presentations and the sometimes nonspecific nature of histological findings. Molecular biology has improved the diagnostic accuracy. Nevertheless, clinical experience is of substantial importance as MF can resemble a wide variety of skin diseases. We performed a literature review and found that MF can mimic >50 different clinical entities. We present a structured framework of clinical variations of classical, unusual and distinct forms of MF. Distinct subforms such as ichthyotic MF, adnexotropic (including syringotropic and folliculotropic) MF, MF with follicular mucinosis, granulomatous MF with granulomatous slack skin and papuloerythroderma of Ofuji are delineated in more detail.

Ofuji's papuloerythroderma: a study of 17 cases.

In this retrospective study the clinical, laboratory and histological features, treatment methods and disease course in 17 patients with papuloerythroderma (PE) are reviewed. The median age at diagnosis was 72 years and the female/male ratio was 1:4:7. The most common abnormal laboratory findings were eosinophilia and an elevated serum IgE level. Psoralen photochemotherapy (PUVA) and oral prednisolone 10-20 mg daily given in combination or alone were very efficient treatments, while UVB phototherapy in combination with topical steroids was also successful. However, potent topical corticosteroids alone or in combination with oral antihistamines were not effective. The patients were observed for a median of 19 months from diagnosis, three being followed for more than 5 years. Five of the patients relapsed, but only one had multiple relapses, two developed cutaneous T-cell lymphoma (CTCL) and two others had histological features suggestive of that disorder. Six of the patients died, cardiovascular disease being the most common cause. PE is a distinct clinical entity with a polymorphous aetiology which frequently includes an association with CTCL or visceral malignancy. PUVA, oral corticosteroids and UVB in combination with topical corticosteroids appear to be effective therapeutic modalities.

Papuloerythroderma of Ofuji: a report of three cases and review of the literature

Papuloerythroderma of Ofuji is a rare, distinctive entity featuring widespread erythematous, flat-topped papules, with a striking sparing of body folds (the so-called 'deck-chair' sign), and peripheral eosinophilia. We report three elderly men who showed the typical features of this disease, and who responded to corticosteroid therapy. Immunohistochemical studies with anti-S-100 and UCHL-1 (CD45 RO) antibodies indicated the presence of abundant dendritic cells and mature T cells in the dermis. In addition, one patient had concomitant tinea corporis. Treatment with griseofulvin alone resulted in clearing of the fungal infection and improvement of his papuloerythroderma, and, worsening of his papuloerythroderma occurred when the tinea relapsed.
