Management of antiretroviral therapy (ART) in Albania

of results There are 45 male and 28 female HIV patients under therapy. Resistance testing, HLA B 5701 screening testing and Pap smear screening testing are not available as part of the initial assessment of the patient. Criteria for initiation of therapy include symptomatic patient and CD4 <200. Most used first-line combinations are: ZDV/3TC + EFV in 40 adult patients and d4T + 3TC + EFV in 14 adult patients. The preferred back-bone nucleotide combination is AZT + 3TC. NNRTI-based initial regimen is preferred, with EFV as the most preferred drug. The only PI drug available in Albania is LPV/r which is used in six patients for the second-line therapy in combination with TNF and ddI.

Unit cost variation in a social security company in Querétaro, México

Comparing unit cost variation between departments and reasons for consultation in outpatient health services provided by a social security company from Querétaro, Mexico. A study of costs (in US dollars) was carried out in outpatient health service units during 2004. Fixed unit costs were estimated per department and adjusted for one year's productivity. Material, physical and consumer resources were included. Weighting was assigned to resources invested in each department. Unit cost was estimated by using the micro cost technique; medicaments, materials used during treatment and reagents were considered to be consumer items. Unit cost resulted from adding fixed unit cost to the variable unit cost corresponding to the reason for consulting. Units costs were then compared between the medical units. Unit cost per month for diabetic treatment varied from 34.8 US dollars, 32,2 US dollars to US 34 US dollars, pap smear screening test costs were 7,2 US dollars, 8,7 US dollars and 7,3 US dollars and dental treatment 27 US dollars, 33 US dollars, 6 and 28,7 US dollars. Unit cost variation was more important in the emergency room and the dental service.

Prevalence of human papillomavirus types in women with pre-neoplastic and neoplastic cervical lesions in the Federal District of Brazil.

As a contribution to the public health authorities in planning prophylactic and therapeutic vaccine strategies, we describe the prevalence of human papillomavirus (HPV) types in women presenting abnormal cytological results in Pap smear screening tests in the Federal District, Central Brazil. We studied 129 cervical scraping samples from women whose cytological tests showed either pre-neoplastic or neoplastic lesions. Amplification of HPV DNA was performed by polymerase chain reaction using consensus primers MY09 and MY11 followed by identification of isolates by restriction fragment length polymorphism. We detected HPV DNA in 62% of the samples, including HPV-16 in 43.8%, HPV-58 in 12.5%, HPV-31 in 10%, HPV-53 in 6.3%, each of HPV-18 and HPV-33 in 3.8% of the isolates. Other types (HPV-35, -52, -66, -CP8304, -6, -11, and -CP8061) were less frequent (= or < 2.5% each). The prevalence of HPV-58 was relatively higher in this population than in data in South America, but similar to results obtained in other studies in Latin America, Europe, and Eastern Asia. Case-control studies need to be carried out to establish the association between the prevalence of HPV types specially the less frequent high-risk types and cervical cancer.

Properties of the summary receiver operating characteristic (SROC) curve for diagnostic test data.

The summary receiver operating characteristic (SROC) curve has been recommended to represent the performance of a diagnostic test, based on data from a meta-analysis. However, little is known about the basic properties of the SROC curve or its estimate. In this paper, the position of the SROC curve is characterized in terms of the overall diagnostic odds ratio and the magnitude of inter-study heterogeneity in the odds ratio. The area under the curve (AUC) and an index Q(*) are discussed as potentially useful summaries of the curve. It is shown that AUC is maximized when the study odds ratios are homogeneous, and that it is quite robust to heterogeneity. An upper bound is derived for AUC based on an exact analytic expression for the homogeneous situation, and a lower bound based on the limit case Q(*), defined by the point where sensitivity equals specificity: Q(*) is invariant to heterogeneity. The standard error of AUC is derived for homogeneous studies, and shown to be a reasonable approximation with heterogeneous studies. The expressions for AUC and its standard error are easily computed in the homogeneous case, and avoid the need for numerical integration in the more general case. SE(AUC) and SE(Q(*)) are found to be numerically close, with SE(Q(*)) being larger if the odds ratio is very large. The methods are illustrated using data for the Pap smear screening test for cervical cancer, and for three tests for the diagnosis of metastases in cervical cancer patients.

Effectiveness of Interventions to Improve Follow-up after Abnormal Cervical Cancer Screening

Objective. The purpose of this study was to determine the effectiveness of interventions designed to improve follow-up after an abnormal Pap smear.Methods. We performed a qualitative meta-analysis of interventions designed to improve follow-up after an abnormal Pap smear and included studies that met the following criteria: randomized or concurrently controlled study design, defined outcomes, and data available for abstraction. Interventions were classified as behavioral, cognitive, sociologic, or combined strategies (e.g., behavioral and cognitive). Abnormal Pap smears were defined as any test result requiring additional follow-up. Effectiveness was measured by the rate of compliance with recommended follow-up.Results. Twenty-two interventions in 10 studies met the inclusion criteria. Cognitive interventions utilizing interactive telephone counseling were the most effective, improving compliance by 24 to 31%. Behavioral interventions, such as patient reminders, were also effective, increasing follow-up by up to 18%. Not all of these results achieved statistical significance. The single sociologic intervention we identified used video-taped peer discussions to provide a message about abnormal Pap smears and appropriate follow-up. This intervention was not associated with increased follow-up after an abnormal test. The effectiveness of interventions using multiple types of strategies to improve follow-up was inconsistent.Conclusions. Cognitive strategies led to the greatest improvement in compliance with follow-up of abnormal Pap smear screening tests. Extension of similar interventions to follow-up of abnormal breast and colon cancer screening, development of physician- and system-targeted interventions, and evaluation of the cost-effectiveness of these strategies are important priorities for future research.

Loss to follow-up of patients with abnormal pap smear: Magnitude and reasons

We studied the magnitude and reasons for loss to follow-up in cases of abnormal Pap smears. We studied 162 women whose Pap smear results were abnormal. The lost follow-up rate was 41.1 per cent (95% CI: 33.1 to 49.3). Reasons for not coming to receive treatment was mostly related to communication such as not receiving informed letters from the hospital (35.6%), not understanding the messages in the informed letters (10.2%), were informed from the hospital's letters that their results were normal (5.1%), and perceiving that the test result was not serious (13.6%). One third of them had traveling-related reasons. There were 25.4 per cent of patients who sought treatment at other hospitals and 8.5 per cent went to see a traditional healer for treatment. The results provide information to prevent such unnecessary loss after having a Pap smear screening test for cervical carcinoma.
