Modification of the palatal lift speech aid

A modification in the construction of a speech aid restoration, and the incorporation of the palatal lift design into the cleft palate prosthesis were described. The palatal lift restoration reduces hypernasal speech by elevating the soft palate in an upward and backward direction, approximately into a position which the velum would attain during normal speech. In patients whose soft palates are insufficient in length, the lift portion of the restoration is extended, and the pharyngeal part is modified in order to effect velopharyngeal closure. The various steps in the construction of a speech aid restoration, and the use of cephalometric roentgenography to demonstrate elevation of the soft palate and velopharyngeal closure were described.

Some of the surgical results for congenital cleft palate patients fail to secure adequate velopharyngeal closure with living tissue. In these instances, the dentist must provide the mechanism by which the patient may produce acceptable speech. A technique for the construction of a restoration with a hinged palatovelar section has been described. This technique does not require redivision of the surgically closed palate for many patients. The restorations are stable and exert minimal stress upon abutment teeth. They have been comfortable and have been well accepted by patients.

The authors have presented the prosthetic aspects in the rehabilitation of the patient with cleft palate. The book provides inclusive material on prosthetic procedures for dentists who are primarily interested in constructing a prosthesis for such a patient, but the authors ignored the new advances in orthodontia and surgery. It is pointed out that the prosthodontist is called upon not only to salvage surgical failures but to better the condition of the patient who has an acquired cleft palate due to trauma, infection, or tumor as well. The book is profusely illustrated. Step by step it covers techniques for determining a recommendation for treatment, making a primary impression, and constructing of a study model, constructing and applying a prosthesis for the patient with a complete or incomplete cleft not surgically treated and postoperative problems. Speech considerations in cleft palate prosthesis

One of the major functions of an oral prosthesis for cleft palate patients is to subserve speech. To aid the prosthetist in understanding the speech process, a brief description of the physiology and phonetics of speech production is presented. The speech considerations to be observed in constructing a prosthesis may be summarized as follows: I. Construction of the prosthesis A. Palatal section: 1. Should establish occlusion between the anterior maxillary and mandibular teeth to aid in the production of the labiodental, linguadental, and lingua-alveolar sounds. 2. Should be free from obstructions (such as protruding malpositioned teeth) which would interfere with the free movement of the tip of the tongue. 3. Should be vaulted to produce an oral cavity of as near normal size as possible. 4. Should provide adequate retention so that movements of the appliance will not interfere with speaking. B. Velar section: 1. Should be constructed to compensate for lateral movement of velar tissue in unoperated or redivided cases in order to prevent leakage of air or sound into the nasal cavities during speaking. 2. Should be placed as high as is anatomically and prosthetically possible to avoid interference with tongue movements and to restore an oral cavity of normal size. 3. Should provide a sturdy support for the pharyngeal section. C. Pharyngeal section: 1. Should be located at the point of maximum constriction of the pharynx as determined by muscle trimming during deglutition. 2. Should be large enough for the pharyngeal musculature to grasp during deglutition and to approximate during speech production. 3. Should not be over-infraextended, i.e., it should not extend inferiorly into the oropharynx and, thus, reduce the size of this important resonating cavity, nor should it be over-supraextended. 4. Should be constructed of some readily modifiable material which will permit adjustment as muscular development results from speech training. 5. Should be constructed of some transparent material to enable the speech pathologist to observe the muscular adjustments taking place during vocalization. II. Age factor in prosthesis The prosthesis can be employed to prevent the development of serious speech defects. The speech pathologist recommends that temporary prostheses be supplied for children as soon as the deciduous teeth have erupted sufficiently to provide retention and the child will cooperate. III. Management of postoperative cases Before a prosthesis is constructed for a patient whose palate is insufficient or immobile, a thorough study of the lingual and mandibular movements employed in speech production should be made by a speech pathologist. On his recommendation experimental speech training should be provided for certain patients before a prosthesis is supplied.
