Childhood ossification of the anterior longitudinal ligament of the cervical spine

Two paediatric cases of acquired ossification of the cervical anterior longitudinal ligament are described. This has not been previously reported in childhood. The first was associated with corrosive ingestion requiring oesophagectomy and colonic interposition and the second with acute lymphoblastic leukaemia initially presenting as idiopathic hypereosinophilic syndrome. Possible mechanisms are discussed.

神経症状,特に脊髄症を呈した頸椎後縦靭帯骨化症の外科的治療には,後方到達による脊髄除圧術と前方到達による脊髄除圧術がある.われわれは病変が脊柱管の前方,硬膜嚢の前方に位置するので,病変を除去するために前方到達法を術式として選択してきた.前方到達法に必要な外科解剖,特にいわゆる硬膜外静脈叢の位置およびその取扱い,脊髄除圧のための削除する椎体の幅,また実際の手術に際しての麻酔,体位における注意点,種々の手術器具の使用法などについて詳細に述べた.

Ａｎｋｙｌｏｓｉｎｇ Ｈｙｐｅｒｏｓｔｏｓｉｓ ｏｆ ｔｈｅ Ｓｐｉｎｅにおける頚椎後縦靭帯骨化例の経時的Ｘ線学的検討

The purpose of this paper is to describe the relation between ossification of the posterior longitudinal ligament in the cervical spine (OPLL) and ankylosing hyperostosis of the spine (AH).The radiograms of 10 patients who had AH with OPLL comprised the material for this study. The mean age of this group of patients was 64 years with a range from 49 to 81 years. There were 9 males and one female. Duration of follow-up study varied from 51 months to 128 months, the average being 87 months.The main results found in this study were as follows;1) Of the 10 patients studied, two advance in the stage of AH and all 10 pateients had growing of ossification of anterior longitudinal ligament in the spine.2) Common type of OPLL with AH was cotinuous type in Seki's classification.3) Five patients (50%) had growing of OPLL and advance in the stage of AH in cervical spine. Generally, these patients had the early stage. Four patients advanced from the first stage to the second stage and one patient advanced from the second stage to the third stage.In cosideration of these results, it is important to pay attention to radiographic changes of the first stage of AH-predical nucleus and the changes along the anterior surface of vertebral bodies-to anticipate growing of OPLL.

EVALUATION OF CERVICAL SPINAL CORD INJURIES WITH METRIZAMIDE MYELOGRAPHY-CT SCANNING

✓ In the past, patients with injuries of the cervical spine and spinal cord have been diagnosed by means of myelography and polytomography. In an attempt to improve the radiographic evaluation of patients with cervical spinal cord injuries the authors performed computerized tomography (CT) scanning of the cervical spine following injection of metrizamide into the spinal subarachnoid space. In 23 patients with cervical spinal cord injuries, metrizamide myelography was performed via a C1–2 puncture. Myelography was used only for localization of the lesion and to determine the site of CT scanning. After myelography, CT scanning of the cervical spine in the transaxial plane was effective in determining the exact nature of compressive lesions and distinguishing the etiology among hematoma, disc, bone fragments, osteophytes, or ossification of the posterior longitudinal ligament. Clinical Studies on 23 Patients with the Ossification of the Posterior Longitudinal Ligament in the Dorsal and Lumbar Spine

Since 1974, twenty three patients with the ossification of the posterior longitudinal ligament (OPLL) in the dorsal and lumbar spine have been seen in our clinic. Sixteen cases of them were associated with OPLL in the cervical region, and fourteen cases had the ossification of the yellow ligament, in the lower dorsal region. Myelopathy had developed in nine cases, most of them were obese, and showed difficulties in walking, sensory disturbances and hyperactive tendon jerks in the lower extremities. Decompressive laminectomy was operated on seven out of nine myelopathic patients with good results.
