Study on liver tissue derived-extracellular vesicles regulating the osteogenic differentiation ability of mesenchymal stem cells and promoting the healing of jaw bone defects

Objective: To explore the biological process of liver tissue-derived extracellular vesicle (LT-EV) in promoting osteogenic differentiation of mesenchymal stem cells and healing of jaw defects to provide a feasible treatment method for the clinical treatment of jaw bone defects. Methods: Enzymatic hydrolysis and differential centrifugation were used to extract LT-EV, scanning electron microscopy, Western blotting, and nanoparticle tracking analyzers were used to identify and characterize LT-EV, and further to explore the biological functions of LT-EV through proteomics and Kyoto Encyclopedia of Genes and Genomes. Flow cytometry was used to detect LT-EV plasma concentration and to calculate the plasma half-life of LT-EV. Small animal in vivo imaging system was used to detect the biological distribution of LT-EV 24 hours after injection. Six C57BL/6 mice were divided into control group and LT-EV group (3 mice in each group) by simple random sampling method. All mice underwent jaw bone defect surgery and tail vein injection every 7 days (the control group was injected with phosphoric buffer saline, LT-EV group was injected with LT-EV), micro-CT was used to evaluate the degree of mouse jaw bone healing 28 days after surgery, HE staining was used to analyze the multi-organ biosafety of LT-EV, and immunofluorescence staining was used to detect the jaw bone expression of osteogenic marker proteins in the defect area. Human jaw bone mesenchymal stem cells (hJBMSC) induced by osteogenic differentiation were treated with LT-EV (obtained from orthognathic surgery patients provided by the Department of Traumatology and Orthognathic Surgery of School of Stomatology of The Fourth Military Medical University resected normal jaw bone fragments), and the difference in osteogenic differentiation ability between the hJBMSC group and the control group (phosphate buffer saline treatment) was compared, and the in vitro bone differentiation promoting effect of LT-EV was verified through alkaline phosphatase (ALP) staining and real-time fluorescence quantitative PCR. Results: The yield of LT-EV was high, and proteomics and Kyoto Encyclopedia of Genes and Genomes showed that LT-EV contained a series of proteins that regulated cell biological functions. LT-EV injected into the tail vein could reach the mouse jaw bone defect area and promote the regeneration and repair of the jaw bone defect [the bone volume fractions of the LT-EV group and the control group were (36.06±4.20)% and (18.58±5.61)%, respectively; t=4.32, P=0.013], and had good biosafety. LT-EV could promote osteogenic differentiation of hJBMSC in vitro. Compared to the control group, ALP staining and osteogenic gene expression levels were significantly enhanced after osteogenic differentiation of hJBMSC (P<0.05). Conclusions: LT-EV exhibits a high yield, ease of acquisition, high biological safety, and excellent bone-promoting effects. It holds promise as a novel cell-free therapy strategy for regenerating craniofacial bone defects.

Comprehensive 3-dimensional Positional and Morphological Analyses of Condyle and Glenoid Fossa in Patients with Skeletal Class II Malocclusion following Bimaxillary Orthognathic Surgery.

This study aimed to comprehensively and quantitatively characterize 3-dimensional (3D) positional and morphological changes of the condyle and glenoid fossa in patients with skeletal Class II malocclusion treated with bimaxillary orthognathic surgery. Twenty eligible patients treated at our institution from January 2016 to December 2021 with more than 12 months of postoperative follow-up were retrospectively enrolled. Radiographic data of cone-beam computed tomography (CBCT) for each patient were collected at 3 stages: 1 week preoperatively (T0), immediately after surgery (T1), and at least 12 months postoperatively (T2). Positional changes, surface and volumetric alterations of condyle, and bone remodeling in glenoid fossa were measured and compared based on voxel- and surface registrations in visual 3D methods. Most patients exhibited a tendency for condyles to shift posteriorly, laterally, superiorly, and rotated outward, downward, and forward immediately after surgery. Posterior, medial, superior movement and outward, upward, and backward rotation of condyles were observed during follow-up (T1-T2). Bone resorption frequently occurred in the posterior area of condylar surfaces, while bone remodeling was more common in the anterior region of the glenoid fossa. Reduced volume of the condyle was found in most cases, which was not associated with the amount of mandibular advancement. Overall, the condyle and its corresponding glenoid fossa remained relatively stable during the follow-up. Our results reveal positional and morphological alterations in the condyle and the glenoid fossa after bimaxillary orthognathic surgery in patients with skeletal class II malocclusion. These changes predominantly fall within the spectrum of physical adaption.

Assessment of Masseter Volume and Postoperative Stability After Orthognathic Surgery in Patients With Skeletal Class III Malocclusion With Facial Asymmetry.

This study aimed to evaluate the effect of orthognathic surgery on masseter volume in patients with skeletal Class III malocclusion with facial asymmetry and the effect of masseter volume on stability in orthognathic surgery. This research studied 16 patients with Class III malocclusion with facial asymmetry who received combined orthodontic-orthognathic treatment and underwent craniofacial computed tomography (CT) before (T0), 2 weeks after (T1), and 6 months after (T2) surgery. Three-dimensional (3D) CT images were retrospectively analyzed, using 3D volume reconstruction to obtain the masseter volume and examine the impact of the masseter volume on stability in orthognathic surgery. A statistically significant difference (P < 0.05) in the volume of the masseter was found up to 6 months after orthognathic surgery compared with the preoperative period, and the reduction in the masticatory muscle volume on the lengthened side is greater than on the shortened side (P < 0.05). The volume of both masseters differed according to facial asymmetry, and the difference was significantly reduced after orthognathic surgery (P < 0.05). During the period time (T1-T2), cephalometric maxillary marker points were not significantly different (P > 0.05), and mandibular marker points were significantly anteriorly shifted (P < 0.05). There was an association between the masseter volume and anterior shift of point B (R > 0.5, P < 0.05), the upward and anterior shifts of the gonion point differed between the lengthened and shortened sides (P < 0.05). The size of the masseter becomes smaller 6 months after orthognathic surgery, and orthognathic surgery improves both bone and soft tissue symmetry. A larger sagittal relapse of mandibular setback occurred in patients with greater masseter volume. Considering these alterations may be helpful in planning orthognathic surgery.

Change in taste sensation after orthognathic surgery

ObjectiveThe objective of this study was to evaluate the effect of orthognathic surgery on taste sensation.Materials and methodsThirty-five patients scheduled to undergo Le Fort I osteotomy (LFIO), sagittal split ramus osteotomy (SSRO), and bimaxillary surgery (BMS) were evaluated by administering localized and whole-mouth taste tests preoperatively and postoperatively at months 1, 3, and 6. The patients were asked to identify the quality of four basic tastes applied to six locations on the palate and tongue and to rate the taste intensities they perceived. Taste recognition thresholds and taste intesity scores were evaluted according to operation groups and follow-ups.ResultsThere were significant decreases in the quinine HCl recognition thresholds at the postoperative follow-ups compared to the preoperative in LFIO patients (p = 0.043). There were significant decreases in sucrose taste intensity scores in the right posterolateral part of the tongue at months 3 and 6 compared to preoperative in SSRO patients (p = 0.046), and significant increases in quinine HCL taste intensity scores in the right and left anterior parts of the tongue at month 6 compared to preoperative in LFIO patients (p < 0.05).ConclusionTaste perception is affected due to potential damage to the chemosensory nerves during orthognathic surgical procedures. Generally, non-significant alterations have been observed in taste perception after orthognathic surgery, except for significant alterations in bitter and sweet taste perceptions.Clinical relevanceMaxillofacial surgeons should be aware of taste perception change after orthognathic surgery procedures and patients should be informed accordingly.The trial registration number (TRN)NCT06103422/Date of registration: 10.17.2023 (retrospectively registered).

Optimizing Patient Outcomes in Orthognathic Surgery: A Proposed Nutritional Protocol for Weight Loss Control and Bone Formation

Objective: The primary objective of this research was to conduct a comprehensive literature review, offering an in-depth examination of the nutritional considerations associated with orthognathic surgery and suggest a perioperative nutritional protocol for enhanced recovery. Methods: To achieve this objective, the authors rigorously examined existing literature while adhering to the guidelines outlined in PRISMA-ScR. The scope of eligible studies encompassed various types, with the exclusion of case reports and reviews. The research applied specific inclusion criteria, concentrating on the nutritional aspects relevant to perioperative and/or postoperative phases of major maxillofacial surgery. Results: A total of 39 articles were identified that met the screening criteria. These articles were then used to generate a discussion and propose a protocol aimed at reducing body weight loss following major maxillofacial surgery and improving bone formation post-osteotomies. Conclusions: While the realm of maxillofacial surgery sees significant advancements in other fields, there exists a noticeable gap in addressing the nutritional status of surgical patients. This oversight can potentially lead to suboptimal postoperative outcomes. Recognizing the pivotal role of nutrition in enhancing surgical results, this paper emphasizes the importance of maintaining optimal nutritional status among orthognathic surgery patients.

Repair of Inferior Alveolar Nerve in Orthognathic Surgery Simulator (RIANOS): A Novel, Open-Source, Combined 3D Printed, and Ex-Vivo Chicken Sciatic Nerve Training Model

Study Design Face and content validation of a surgical simulation model. Objective Accidental transection of the inferior alveolar nerve (IAN) during bilateral sagittal split osteotomies (BSSO) has a reported incidence of up to 7%, determining important sensory disturbances in patients. Proper repair demands the need of microsurgical anastomosis skills. No previous training models have been described to simulate this. Therefore, we present a validated simulation model for intraoral repair of transected IAN. Methods A CT scan of an orthognathic surgery patient was modified and a 3D model of a mandible with BSSO was printed. Chicken thigh anatomy was reviewed, and 2.5 mm sciatic nerves were dissected and mounted in the model. In order to simulate intraoral work depth, it was put inside a dental phantom or medical glove box. The model was tested by a group of experts (n = 12), simulating a transected IAN repair inside the mouth with both loupes and a double visor surgical training microscope. A survey was conducted to assess Face and Content validity. Results The model was named RIANOS after Repair of Inferior Alveolar Nerve in Orthognathic Surgery Simulator. The printing cost of each model was approximately US$3 and the design file is open-source and available for download. All experts “Strongly Agreed” that the model was useful for training inferior alveolar nerve microsurgical repair and would consider implementing it with their residents. Conclusions We developed a low cost, reproducible, open-source simulator for IAN injury repair training during BSSO. Face and Content validity was achieved through evaluation by a group of experts.

Evaluation of pre- and postoperative nutrition and oral health–related quality of life in orthognathic surgery patients

ObjectiveOrthognathic surgery is a complex orofacial surgery that can significantly impact occlusal function and effect nutritional and quality of life parameters. This study aimed to evaluate alterations in dietary intake, chewing function, physical activity, and oral health–related quality of life of patients undergoing orthognathic surgery. Research Methods and ProceduresIn this prospective longitudinal study, the assessments were conducted at: preoperatively (T0) and postoperative first week (T1), second week (T2), first month (T3), and third month (T4) between May 2021 and March 2023. Sociodemographic questionnaire, 24-h dietary recall record, chewing ability form, International Physical Activity Questionnaire, and Oral Health Impact Profile-14 (OHIP-14) was applied at face-to-face interviews. ResultsSeventy eligible orthognathic surgery patients were evaluated, and 37 patients (52.8%) completed this study. Energy and fat intake significantly decreased from T0 to T1 (P < 0.001) and returned to basis by T4 (P = 0.015). Fiber intake was found to be lowest at T1 and T2 compared with other time points (P < 0.001). Chewing ability showed a deterioration and then improvement; however, patients still had difficulties chewing hard foods at T4. The OHIP-14 increased at T2 and T3 from T0 (P < 0.001 and P = 0.021, respectively) and showed a significant improvement at T4 (P < 0.05). ConclusionThe findings indicate a temporary decline in nutritional intake and chewing ability with subsequent recovery by the third month postsurgery. These changes, along with the trends in oral health-related quality of life, underscore the need for tailored nutritional and functional rehabilitation programs following orthognathic surgery.

Radiographic Imaging for the Diagnosis and Treatment of Patients with Skeletal Class III Malocclusion.

Skeletal Class III malocclusion is one type of dentofacial deformity that significantly affects patients' facial aesthetics and oral health. The orthodontic treatment of skeletal Class III malocclusion presents challenges due to uncertainties surrounding mandibular growth patterns and treatment outcomes. In recent years, disease-specific radiographic features have garnered interest from researchers in various fields including orthodontics, for their exceptional performance in enhancing diagnostic precision and treatment effect predictability. The aim of this narrative review is to provide an overview of the valuable radiographic features in the diagnosis and management of skeletal Class III malocclusion. Based on the existing literature, a series of analyses on lateral cephalograms have been concluded to identify the significant variables related to facial type classification, growth prediction, and decision-making for tooth extractions and orthognathic surgery in patients with skeletal Class III malocclusion. Furthermore, we summarize the parameters regarding the inter-maxillary relationship, as well as different anatomical structures including the maxilla, mandible, craniofacial base, and soft tissues from conventional and machine learning statistical models. Several distinct radiographic features for Class III malocclusion have also been preliminarily observed using cone beam computed tomography (CBCT) and magnetic resonance imaging (MRI).

Fully digital occlusion planning in orthognathic surgery – A crossover study

Orthognathic surgery enables patients with severe jaw malocclusions to normalise their chewing function and, as such, to improve their quality of life. Over the last few years, digitalisation has been set in motion by intraoral scanners and the improvement of planning software in the field of oral and maxillofacial surgery. Previous studies based on plaster cast models showed that the virtual occlusion based on digitally scanned models can be comparable to conventional methods. This retrospective crossover study aimed to prove that the virtual occlusion finding with the IPS CaseDesigner® (version 2.3.5.2, KLS Martin, Tuttlingen, Germany) is accurate enough to use intraoral scans exclusively. Materials and methodsA total of 23 orthognathic surgery patients receiving an intraoral scan for their treatment were included in this study. Two experienced maxillofacial surgeons haptically performed the occlusion finding on three-dimensional (3D) stereolithographic models using the fully digital pathway. One surgeon repeated the procedure a second time to evaluate intra-observer variability. The study aimed to show the difference between these two planning methods by upholding the surgical accuracy of less than 2 mm in translation and 2° in rotation. The conventional haptic occlusion was set as a reference throughout the whole study. The data were tested with a one-sample Wilcoxon test for the fit into the surgical accuracy. ResultsThe difference between the virtual and conventional groups was significantly smaller than the surgical accuracy (all p < 0.001). Both translational movements (anterior/posterior (median 0.51 mm [0.28, 0.88]), left/right (median 0.46 mm [0.20, 0.87]), cranial/caudal (median 0.37 mm [0.11, 0.69])) and rotations (Roll (median 0.71° [0.29, 1.35]), Pitch (median 0.72° [0.29, 1.44]), Yaw (median 1.09° [0.33, 1.60])) were in the range of surgical accuracy (2 mm/2°). The most significant differences were found in the anterior/posterior translation (median 0.51 mm [0.28, 0.88]) and the Yaw rotation (median 1.09° [0.33, 1.60]). ConclusionThese results demonstrate that the entirely virtual workflow in orthognathic surgery, including intraoral scanning and the virtual semi-automatic occlusion finding, represents a reliable and state-of-the-art alternative to the conventional haptic method.

Complications of orthognathic surgery in patients with cleft lip and palate: A systematic review

ObjectiveDetermine the main complications of orthognathic surgery in patients with cleft lip and palate. MethodsPubMed, LILACS, Cochrane, Embase, Scopus, and Google Scholar were systematically reviewed. Studies addressing the complications of orthognathic surgery in patients with cleft lip and palate were included. For the search, the strategy was used with the descriptors extracted from MeSH “Cleft Palate”, “Orthognathic Surgery” and “Complications”. The Newcastle-Ottawa Scale (NOS) was used to assess the risk of bias in the included studies. Patients of any sex, age, and ethnicity with cleft lip and palate submitted to orthognathic surgery were included in this systematic review. The study followed the PRISMA 2020 standards and was registered in PROSPERO with protocol CRD42020195927. ResultsIn the initial search, 1090 articles were found and after applying the inclusion and exclusion criteria, eleven studies were selected. The sample consisted of 629 patients who underwent Orthognathic Surgery, with an average age of 21.52 years. The majority of patients (390) presented unilateral transforamen proposals. In total, 150 complications were identified in the included studies, the most frequent being relapse of movement with 77 cases (51.3 %). Other reported, but less frequent, complications were gingival recession with root exposure, premaxillary mobility, intraoperative hemorrhage, fistulas and infection and velopharyngeal impairment. Most included studies did not have a control group, making meta-analysis unfeasible. Seven of the included studies presented a low risk of bias according to the NOS. ConclusionsOrthognathic surgery in cleft patients is a safe procedure, however it presents particularities and more complications when compared to a non-cleft patient. In this study, the most common complication found was the relapse, and the surgeon must be aware of this complication and others, and try to minimize its negative effects on the patient. We strongly recommend further investigations with detailed methodologies, control groups, well-described criteria for reported complications, and comprehensive sample characteristics to provide higher-quality evidence.

ORTOGNATİK CERRAHİ İLE İLGİLİ INSTAGRAM GÖNDERİLERİNİN DEĞERLENDİRİLMESİ: METODOLOJİK ÇALIŞMA

Objective: The use of social media has increased with the widespread use of the internet. Patients use social media for many reasons, such as to get information about their illness, get social support from other patients, and communicate with doctors. The purpose of this study is to analyze Instagram posts about orthognathic surgery.&#x0D; Materials and Methods: A new Instagram account has been opened so that the outcomes of the searches are not affected by bias. Three hashtags with the most posts about orthognathic surgery were determined. #orthognathicsurgery, #orthodonticsurgery, and #jawsurgery hashtags were classified on Instagram. The first hundred posts for each hashtag were analyzed. The sources of the posts were classified as healthcare professionals, individual users, and healthcare industries. The contents of the posts were categorized as patient experience and educational. &#x0D; Results: In the posts about #orthognaticsurgery, a statistically significant difference was found between the number of likes. According to the upload source, there was no statistically significant difference between the number of comments and number of likes on posts about #orthodonticsurgery and #jawsurgery.&#x0D; Conclusions: Healthcare professionals should point out to patients that Instagram needs to be an adequate resource for patients in its current state.&#x0D; The study results revealed that there needed to be more information about orthognathic surgery for patients on Instagram.

Steiner Analysis of Cephalometric in Orthognathic Surgery Patients of Department Oral and Maxillofacial Surgery Hasan Sadikin General Hospital Bandung of Period April 2017 – April 2022

Objective: Orthognathic surgery is surgery on abnormalities of dentofacial that occur in the maxilla, mandible, or both. Determine measurement of the type of dentoskeletal abnormality in malocclusion using cephalometric analysis. Steiner analysis is used because it is considered that the Sela tursica-nasion-point A (SNA) angle, Sela tursica-nasion-point B (SNB) used to describe the relationship between the maxilla and the mandible Material and Methods: The study involved 30 patients who underwent orthognathic surgery between April 2017 and April 2022 at Hasan Sadikin Hospital Bandung. Attention was carried out using cephalometric radiography and analyzed using the stainer method by observing angles also observing patient characteristics such as gender and age, average angle of SNA, SNB, ANB, diagnosis, number of corrected jaws and type of orthognathic surgery. Results: The results of this study showed that orthognathic surgery patients were more male (63.3%), most aged between 19 - 20 years (40%), diagnosis of grade III malocclusion (96.7%), double jaw correction action (90%), bilateral sagittal split osteotomy (93%), osteotomy le fort I (96.7%), chinplasty (16.7%), mean angle SNA (83.8), angle SNB (90.6), angle ANB (-6.8). What Is Health Literacy Among Orthognathic Surgery Patients?

BackgroundHealth literacy of orthognathic surgery patients has not been thoroughly evaluated. PurposeThe purpose of this study was to estimate health literacy and identify risk factors associated with inadequate health literacy in orthognathic surgery patients. Study Design, Setting, SampleA cross-sectional study was implemented utilizing patients ages 14-80 years who presented for orthognathic surgery evaluation between September 2021 and December 2022. Subjects were excluded from the study if they did not complete the orthognathic surgery evaluation, were not between the ages of 14-80 years old, or did not complete the Brief Health Literacy Screening Tool (BRIEF) questionnaire during intake. Subjects who have not undergone orthognathic surgery but completed the initial evaluation for orthognathic surgery were included in the study. Predictor variablesThe predictor variables were a set of risk factors for inadequate health literacy: age, sex, primary language, race, estimated household income, and diagnosis. Main Outcome VariableThe main outcome variable was health literacy assessed using the Brief Health Literacy Screening Tool (BRIEF). During intake, subjects completed the BRIEF questionnaire consisting of four questions scored on an ordinal scale of 1-5. Inadequate health literacy was defined as a BRIEF score ≤16. CovariatesNot applicable. AnalysesBivariate and multivariate analyses were performed. P < 0.05 was considered statistically significant. ResultsOf 150 patients presenting for orthognathic surgery, fifteen percent of patients had inadequate health literacy via the BRIEF test. The mean age of those with adequate health literacy was 27.9 years (SD, +12.5) compared to 18.5 years (SD, +5.7) for those with inadequate health literacy (p=<0.001). After adjusting for sex, language, race, estimated household income, and diagnosis via multivariate analysis, increasing age was associated with decreased odds of inadequate health literacy (adjusted odds ratio= 0.81 (CI, 0.72-0.92), p=<0.001). Conclusion and RelevanceIn the complex process of orthognathic surgery, it is essential to identify patients with inadequate health literacy that may require additional health literacy interventions. Ultimately, 15% of orthognathic surgery subjects had inadequate health literacy, and younger patients were the most susceptible as the odds of inadequate health literacy decreased with increasing age.

Influence of differences in mandibular incisor inclination on skeletal stability after orthognathic surgery in patients with skeletal class III malocclusion

Objective: The preoperative inclination angle of mandibular incisors was crucial for surgical and postoperative stability while the effect of proclined mandibular incisors on skeletal stability has not been investigated. This study aimed to evaluate the effects of differences in presurgical mandibular incisor inclination on skeletal stability after orthognathic surgery in patients with skeletal Class III malocclusion.Methods: A retrospective cohort study of 80 consecutive patients with skeletal Class III malocclusion who underwent bimaxillary orthognathic surgery was conducted. According to incisor mandibular plane angle (IMPA), patients were divided into 3 groups: retroclined inclination (IMPA < 87°), normal inclination (87° ≤ IMPA < 93°) and proclined inclination (IMPA ≥ 93°). Preoperative characteristics, surgical changes and postoperative stability were compared based on lateral cephalograms obtained 1 week before surgery (T0), 1 week after surgery (T1), and at 6 to 12 months postoperatively (T2).Results: The mandible demonstrated a forward and upward relapse in all three groups. No significant differences in skeletal relapse were observed in the 3 groups of patients. However, the proclined inclination group showed a negative overbite tendency postoperatively compared with the other two groups and a clinically significant mandibular relapse pattern. Proclined IMPA both pre- and postoperatively was correlated with mandibular relapse.Conclusion: Sufficient presurgical mandibular incisor decompensation was of crucial importance for the maintenance of skeletal stability in patients with skeletal Class III malocclusion who subsequently underwent orthognathic surgery.

Evaluation of Surface Changes of Stainless Steel Miniplates and Screws Following Retrieval from Maxillofacial Trauma and Orthognathic Surgery Patients: A Comparative Study.

This study examines the surface changes of stainless steel miniplates and screws after their retrieval from patients who underwent maxillofacial trauma and orthognathic surgery. The assessment focuses on comparing the alterations in these materials, aiming to contribute to our understanding of their durability and performance in clinical settings. A total of 60 stainless steel miniplates and screws were collected from 30 patients who had previously undergone either maxillofacial trauma or orthognathic surgery. The retrieved miniplates and screws were carefully removed from the patients and cleaned to remove any organic debris. Each specimen was then examined for surface changes. Surface changes were evaluated using visual inspection, scanning electron microscopy (SEM), and roughness measurements. Visual inspection provided a qualitative assessment, while SEM allowed for a more detailed examination of the surfaces. Roughness measurements were conducted using a profilometer. Visual inspection revealed varying degrees of surface changes in the retrieved miniplates and screws. These changes included scratches, corrosion, and discoloration. SEM analysis confirmed the presence of surface alterations, with some specimens showing more significant damage, such as pitting and cracks. Roughness measurements indicated an increase in surface roughness for both miniplates and screws, suggesting that the surfaces had become less smooth. This comparative study of stainless steel miniplates and screws retrieved from maxillofacial trauma and orthognathic surgery patients demonstrated that these materials undergo surface changes over time.

Optimizing Orthognathic Surgery: Leveraging the Average Skull as a Dynamic Template for Surgical Simulation and Planning in 30 Patient Cases.

Virtual planning has revolutionized orthognathic surgery (OGS), marking a significant advancement in the field. This study aims to showcase the practical application of our established 3D average skull template as a guiding framework for surgical planning, and to share valuable insights from our clinical experience. We enrolled 30 consecutive Taiwanese patients (18 females and 12 males) who underwent two-jaw orthognathic surgery with surgical simulation, utilizing the average skull template for planning. Results indicate the method's applicability and precision. By adhering to the surgical plan, post-operative outcomes closely aligned with the average skull template, showing negligible deviations of less than 2 mm. Moreover, patients expressed high satisfaction with post-surgery facial changes, with the chin appearance receiving the highest satisfaction scores, while the lowest scores were attributed to nose appearance. Notably, the substantial change in lower jaw position post-mandibular setback surgery contributed to increased satisfaction with the chin position. In conclusion, this study does not seek to replace established surgical planning methods, but underscores that utilizing an average skull as a surgical design template provides a viable, accurate, and efficient option for OGS patients.

Evaluation of occlusal bite force distribution by T-Scan in orthodontic patients with different occlusal characteristics: a cross sectional-observational study

BackgroundThe aim of orthodontic treatment, apart from esthetic and functional corrections, is uniform force distribution. Hence Occlusal analysis using a T scan gives scope for a precisely targeted treatment plan. The T-scan evaluation of occlusal force, time, and location of contacts from initial occlusal contact to maximum intercuspation enables the orthodontist to sequentially balance the occlusal forces on the right and left sides through specific treatment plan options.ObjectiveThe current study aimed to determine the force distribution in the different individuals by using a T-Scan as well as the net discrepancies of forces generated at a maximum intercuspation position in the first molar region between the left and right sides of the mouth.MethodsThis is a descriptive-correlational study that was carried out in Ras Al Khaimah College of Dental Sciences clinics and Ajman University clinics from January 2020 to September 2022 by using the convenience sampling technique. The T-scan III Novus was employed in this investigation to record multi-bite scans for several patients. T-scan was utilised to examine various malocclusions.ResultsThe present study consisted of 158 participants. Analysis of Variance (ANOVA) showed that there is a statistically significant difference in the percentage of force between the three types of malocclusions (I, II, and III) on the right molar side (B-16 and B-46) (p < 0.05). Moreover, the overall discrepancy showed a statistically significant difference in the three types of malocclusion classifications (p < 0.05). On the other hand, there was no statistically significant difference in the percentage of force between B-26 and B-36 (p > 0.05). Post hoc analysis showed a statistically significant difference in the percentage of force between malocclusion classes I and III on the right molar, with a mean difference of 4.11190 (p < 0.05). Similarly, there was a statistically significant difference in B-46 between Malocclusion Classes I and II, 4.01806 (p < 0.05). Additionally, post hoc analysis showed a statistically significant difference between malocclusion classes I and III, with a mean difference of -4.79841 (p < 0.05) on the right molar.ConclusionThe T-Scan is a useful tool for assessing occlusal discrepancies and can be helpful during treatment planning and follow-up, especially for orthognathic surgery patients. A T-scan could be used in orthodontic therapy in a simple and efficient way. Also, it turned out to be a useful tool for diagnosing problems and gave us new information about how therapies work. In this study, T-Scan showed that it can measure occlusal forces in timing in an objective, accurate, and repeated manner. The current study found that T-Scan was better able to report the difference in the percentage of force on the right molar side than on the left side.

Male and Female Discrepancies in Anxiety, Depressive Symptoms, and Coping Strategies among Orthognathic Patients: A Cross-Sectional Analysis.

With an increasing understanding of the differences between men and women's psychological experiences, this study aimed to probe the sex-based differences in anxiety, depressive symptoms, and coping strategies among orthognathic patients. The study hypothesis was that female patients would show higher levels of anxiety and depressive symptoms than males, and that coping mechanisms would differ between male and female sexes. A cross-sectional design was adopted, examining orthognathic patients from the Department of Oral and Maxillo-Facial Surgery at the Emergency Clinical Municipal Hospital in Timisoara, Romania, from 2020 to 2023. Eligible participants (18+ years with no prior orthognathic treatment) completed a comprehensive online questionnaire 6 weeks before scheduled surgery. This was composed of validated self-report instruments comprising the SF-36, GAD-7, and the PHQ-9, and the COPE-60, along with additional sociodemographic data. Of the 127 orthognathic patients analyzed (68 men and 59 women, aged 18 to 65 years, mean age 32), men rated their physical health status slightly better on the SF-36 scale. However, the most notable difference was in mental health, with females scoring higher on both the PHQ-9 (indicative of depression) and the GAD-7 (indicative of anxiety) scales. Specifically, female participants exhibited average PHQ-9 scores 1.8 points higher and GAD-7 scores 1.5 points higher than their male counterparts. Coping mechanisms also varied: 42% of male patients primarily employed "Disengagement" strategies, while 58% of females predominantly used "Engagement" and "Emotion Focused" strategies. Emotion-focused coping was associated with a 1.6-fold increased risk of depressive symptoms. Sex differences play a crucial role in the psychological experiences of orthognathic patients, evident in anxiety and depression levels, perceived health status, and coping strategies. This underlines the importance of sex-tailored psychological support in the preoperative phase for orthognathic surgery patients.

Can smartphones be used for routine dental clinical application? A validation study for using smartphone-generated 3D facial images

ObjectivesTo compare the accuracy of smartphone-generated three-dimensional (3D) facial images to that of direct anthropometry (DA) and 3dMD with the aim of assessing the validity and reliability of smartphone-generated 3D facial images for routine clinical applications. Materials and methodsTwenty-five anthropometric soft-tissue facial landmarks were labelled manually on 22 orthognathic surgery patients (11 males and 11 females; mean age 26.2 ± 5.3 years). For each labelled face, two imaging operations were performed using two different surface imaging systems: 3dMDface and Bellus3D FaceApp. Next, 42 inter-landmark facial measurements amongst the identified facial landmarks were measured directly on each labelled face and also digitally on 3D facial images. The measurements obtained from smartphone-generated 3D facial images (SGI) were statistically compared with those from DA and 3dMD. ResultsSGI had slightly higher measurement values than DA and 3dMD, but there was no statistically significant difference between the mean values of inter-landmark measures across the three methods. Clinically acceptable differences (≤3 mm or ≤5°) were observed for 67 % and 74 % of measurements with good agreement between DA and SGI, and 3dMD and SGI, respectively. An overall small systematic bias of ± 0.2 mm was observed between the three methods. Furthermore, the mean absolute difference between DA and SGI methods was highest for linear (1.41 ± 0.33 mm) as well as angular measurements (3.07 ± 0.73°). ConclusionsSGI demonstrated fair trueness compared to DA and 3dMD. The central region and flat areas of the face in SGI are more accurate. Despite this, SGI have limited clinical application, and the panfacial accuracy of the SGI would be more desirable from a clinical application standpoint. Clinical significanceThe usage of SGI in clinical practice for region-specific macro-proportional facial assessment involving central and flat regions of the face or for patient education purposes, which does not require accuracy within 3 mm and 5° can be considered.
