In previous studies, one relevant previous article was used to review each independent variable. Management of genetically determined kidney stone disease: consensus from a panel of urologists and nephrologists.

Available evidence suggests that monogenic causes of kidney stones are likely under-diagnosed, particularly in young adults, needing expert multidisciplinary recommendations to improve diagnosis, management and therapeutic outcomes. To increase the awareness among the medical community on the recognition of the signs and symptoms of genetically determined kidney stone disease in adult patients, with a special focus on primary hyperoxaluria (PH), a group of nephrologists and urologists started a consensus process through the Delphi method. A list of 40 statements (23 regarding genetically determined stone disease and 17 regarding primary hyperoxaluria) was defined by the authors and included in an online Delphi survey, which was sent to 16 urologists and 22 nephrologists with expertise in managing patients with kidney stone disease. An agreement threshold of 75% was established for consensus. After two rounds of Delphi voting, consensus was reached for 33 statements, 18 regarding genetically determined stone disease and 15 regarding PH. The Delphi process highlighted several areas of agreement with regard to the characteristic or anamnestic data suggesting diagnostic investigation, optimal diagnostic patterns, treatment strategies and management of patients with genetically determined nephrolithiasis. The process also highlighted some grey areas, which deserve further investigation and highlight the need for educational initiatives focused on rare diseases in the field of kidney stones.

Anesthetic Management of Parturients With Achondroplasia During Labor and Delivery: A Narrative Review.

Achondroplasia accounts for approximately 70% of all forms of dwarfism. Cesarean delivery is often required in parturients with achondroplasia due to cephalopelvic disproportion. There is no consensus on the optimal management for cesarean delivery considering the difficulties in both general and regional anesthesia in patients with achondroplasia. The aim of this study was to explore the literature for prior case reports and series to determine the optimum anesthetic management for cesarean delivery in achondroplastic patients. We conducted a review of the literature using Embase, Medline, Scopus, and Web of Science database searches for case series and case reports on achondroplasia and pregnancy through January 2024. Conference abstracts >3 years old were excluded, as well as data on forms of dwarfism other than achondroplasia, patients taller than 147 cm, and non-English language papers. Extracted data included demographic information, anesthetic management, and reported complications. The literature review resulted in 57 manuscripts with a total of 80 anesthetics. Anesthetic management consisted of planned general anesthesia (n = 16), single injection spinal (n = 28), epidural (n = 17), combined spinal-epidural (n = 12), and intrathecal catheter (n = 1). Six patients required conversion from neuraxial anesthesia to general anesthesia due to failed neuraxial placement (n = 3), inadequate blockade (n = 2), and high neuraxial block (n = 1). Reduced dose of intrathecal bupivacaine was common in this population. Complications such as hypotension (4 in 64), inadvertent dural puncture (1 in 64), and transient paresthesia (3 in 64) during neuraxial technique were reported but were infrequent. Neuraxial anesthesia is more common and a viable option in carefully selected parturients with achondroplasia. We recommend reduction of intrathecal local anesthetic as part of a titratable neuraxial technique (ie, combined spinal-epidural) that minimizes the risk of hypotension, high spinal, and emergent intubation.

Navigating the Uncommon: Inguinal Metastasis in Dermatofibrosarcoma Protuberans

Abstract Dermatofibrosarcoma protuberans (DFSP) is a rare, locally aggressive soft tissue sarcoma with a low rate of metastasis, affecting around four per million annually. Representing about 1% of sarcomas, often manifests on the trunk or proximal extremities. Most cases are low-grade, but some have a high-grade fibrosarcomatous component, which is linked to a higher risk of recurrence and metastasis. Here, we present a 55-year-old male with DFSP, initially presenting as a non-healing ulcer on his foot. Despite surgical excision, radiotherapy, and chemotherapy with imatinib, the patient developed a recurrent ulcer and calcaneum involvement, leading to below-knee amputation. One year later, inguinal lymph node metastasis was detected. After inguinal lymph node dissection and additional radiotherapy, the patient showed no further active lesions. This case highlights the rarity of lymph node metastasis in DFSP and the ongoing need for research into optimal management strategies.

The management of bacterial conjunctivitis

Introduction and Purpose: Bacterial conjunctivitis, a common eye disease characterized by inflammation of the conjunctiva, poses a major challenge in clinical practice due to its prevalence, diverse etiology and the need for effective pharmacological treatment. The conjunctiva is constantly exposed to the adverse effects of various external and internal factors. This review is devoted to the pharmacological treatment of bacterial conjunctivitis, with a focus on the most common causative agents. Materials and Methods: A comprehensive survey of articles published in scientific journals was conducted via PubMed and Google Scholar online research platforms. Articles were searched by entering keywords in the appropriate configuration: “bacterial conjunctivitis,” ‘pharmacotherapy,’ ‘bacterial,’ and ”antibiotics.” Description of current knowledge: Emerging research has shown that the choice of antibiotic for the treatment of bacterial conjunctivitis depends primarily on the etiology and severity of the infection. Summary: Antibiotic therapy is an effective tool in the treatment of bacterial conjunctivitis, but its use requires consideration of etiology, severity of infection and potential risk of resistance. Collaboration between ophthalmologists and microbiologists is necessary for the optimal management of this condition. Acute bacterial conjunctivitis is a prevalent eye condition that requires prompt and appropriate pharmacological management.

Optimal Management of the Inpatient With Decompensated Cirrhosis.

Over the past several years, there has been a wealth of new data pertaining to the management of complications of cirrhosis, resulting in several important updates to best practices and consensus guidelines. Despite these advancements and numerous recent targeted quality initiatives, hospitalizations resulting from complications of cirrhosis remain frequent, costly and associated with poor patient outcomes. An emphasis on evidence-based management of hospitalized patients with decompensated cirrhosis has the potential to decrease readmission rates and length of stay while improving overall patient outcomes. Herein, we provide an updated, evidence-based overview of the optimal inpatient management of the most frequently encountered complications associated with cirrhosis.

Optimization Protection Coordination by Optimizing Time Dial Settings at PT. Pupuk Sriwidjaja Palembang Using Grey Wolf Method

Overcurrent Rele (OCR) is an important component in the electric power protection system. One of the parameters that must be set on the OCR is the Pickup Current (Ip) and Time Dial Setting (TDS). To achieve optimum relay coordination by setting the time dial, several optimisation methods have been used such as Genetic Algorithm (GA), Particle Swarm Optimisation (PSO), Fire Fly Algorithm (FA) and Grasshopper Optimisation Algorithm (GOA).In this study, the TDS setting was determined through Using the Grey Wolf Optimisation (GWO) method. The GWO algorithm was able to determine the optimum value of the TDS setting for each OCR process. This is validated by the results of tests conducted using the ETAP application. The primary and secondary relays are able to coordinate effectively to secure three-phase faults that occur on each bus.Furthermore, GWO demonstrated a superior convergence rate in compared to GOA, therefore enhancing its capacity to identify a more optimal TDS setting value. Furthermore, the objective function of GWO is more effective than that of GOA, thereby enhancing its ability to function in a more optimal manner. The results of this test demonstrate that the GWO algorithm is a reliable method for directly identifying the optimal TDS setting value for determining relay coordination settings.Keywords—Protection Coordination Overcurrent Rele ( OCR ), Short Circuit Maximum ( IscMax ), Time Dial Setting ( TDS ), Grey Wolf Optimization ( GWO ).

Early evaluation and treatment of thoracolumbar kyphosis in children with achondroplasia.

Thoracolumbar kyphosis (TLK) is frequently reported in children with achondroplasia. The combination of TLK and the narrow spinal canal in achondroplasia increases the risk of developing symptomatic spinal stenosis. However, there is no consensus on the optimal management of TLK. This retrospective cohort study evaluated children under four years old with achondroplasia, monitoring TLK every six months. Pathologic TLK was defined as a Cobb angle of 20 degrees or more between T10 and L2. Management involved either a wait-and-see policy, which prohibited unsupported sitting, or bracing. Surgery was reserved for severe progressive TLK or spinal stenosis cases. TLK was evaluated over time. A receiver operating characteristic curve determined the baseline threshold where wait-and-see management failed to resolve TLK below 20 degrees. Multiple linear regression compared bracing versus wait-and-see management for cases exceeding 40 degrees. Sixty-two patients were included, with a median age of 10 months and a median follow-up of 31 months. TLK prevalence decreased from 85% at baseline to 42% at final follow-up. The mean Cobb angle decreased from 31 ± 11 degrees to 22 ± 16 degrees (p < 0.001). The threshold for ineffective wait-and-see management was identified as 33 degrees. Bracing resulted in significantly more TLK reduction than wait-and-see management for cases exceeding 40 degrees (15 degree difference, 95% CI 2-28, p = 0.023). Three patients required surgery. TLK is highly prevalent in achondroplasia, necessitating careful monitoring. A wait-and-see policy with restrictions on unsupported sitting is recommended initially, but early bracing should be considered for more severe cases.

ANALYSIS OF THE IMPACT OF TASK PRIORITIZATION LISTS ON THE POTENTIAL FOR AVOIDING ANOMALIES IN TASK SCHEDULING

This paper addresses relevant issues related to the anomalous deterioration in objective function values when attempting to improve the initial parameters in one of the discrete optimization problems. The primary focus is on investigating the conditions under which it is possible to prevent the occurrence of such anomalies. Contemporary scientific works devoted to schedule optimization and task prioritization management, particularly for location-allocation problems arising in the fields of computer science, engineering, and operations research, are reviewed. A priority dynamic redistribution algorithm is proposed, which al-lows minimizing delays and ensuring efficient resource utilization during parallel task execu-tion. An example of applying the algorithm is provided, and its effectiveness in preventing anomalies.

Climate-adaptive optimal water resources management: A multi-sectoral approach for the Munneru river basin, India.

Climate change profoundly affects water resource allocation by disrupting the availability, distribution, and quality of water across various regions. Optimal allocation of water resources represents a comprehensive strategy for water resource management by addressing the intricate connections between water allocation systems and their repercussions on the environment, society, and economy. In this study, an Optimal Water Resources Management (OWRM) framework was developed, focusing on the optimal allocation of water resources and crop planting structures across various sectors. The Munneru river basin, located in the lower Krishna River region of India, was selected as the study area to validate the proposed framework. Five distinct water-demanding sectors-irrigation, domestic, livestock, industrial, and irrigation water requirements for major agricultural seasons-were identified in the study area, and their sectoral water demands were calculated at the basin level. The crop water and irrigation water requirements for various crops were estimated using the CROPWAT tool, while the framework also optimized crop planting structures to maximize returns and resource efficiency. The Non-dominated Sorting Genetic Algorithm-II (NSGA-II) was applied, with two objectives focused on equity and economic value. Superior solutions were then identified using the Technique for Order of Preference by Similarity to Ideal Solution (TOPSIS). The OWRM framework was applied after identifying critical cases of water availability in future periods under climate change scenarios. Through this integrated approach, an average annual increase of 52.6% in agricultural sector returns was achieved for the simulation period (2016-17 to 2022-23). For a condition of providing at least 90% water supply to each sector, the optimal crop patterns led to revenue increases of 136.4%, 59.2%, and 74.7% compared to actual revenues for the water years 2020-21, 2021-22 and 2022-23 respectively. The developed OWRM methodology can be applied to other basins across the world that are impacted by climate change.

Remote organ cancer induces kidney injury, inflammation, and fibrosis and adversely alters renal function.

Approximately 30% of the patients with cancer experience kidney complications, which hinder optimal cancer management, imposing a burden on patients' quality of life and the healthcare system. The etiology of kidney complications in patients with cancer is often attributed to oncological therapies. However, the direct impact of cancer on kidney health is underestimated. Our previous study demonstrated that metastatic lung cancer adversely alters the kidney and exacerbates chemotherapy-induced nephrotoxicity, indicating lung cancer-kidney crosstalk. The current study examines whether this phenomenon is specific to the employed cancer model. Female and male mice of various strains were injected with different cell lines of remote organ cancer, and their kidney tissues were analyzed for toxicity and fibrosis. The impact of cancer on the kidney varied by cancer type. Breast cancer and specific subtypes of lung cancer, including KRAS- and epidermal growth factor receptor (EGFR)-mutant cancer, pathologically altered kidney physiology and function in a manner dependent on the metastatic potential of the cell line. This was independent of mouse strain, sex, and cancer cell line origin. Moreover, tumor DNA was not detected in the renal tissue, excluding metastases to the kidney as a causative factor for the observed pathological alterations. Lewis lung carcinoma and B16 melanoma did not cause nephrotoxicity, regardless of the tumor size. Our results confirm cancer-kidney crosstalk in specific cancer types. In the era of precision medicine, further research is essential to identify at-risk oncology populations, enabling early detection and management of renal complications.NEW & NOTEWORTHY Patients with cancer frequently experience kidney complications, often attributed to antineoplastic therapies. This emphasis on therapy-induced nephrotoxicity has led to the underestimation of the impact of cancer on the kidney. Our study demonstrates that distant organ cancer is sufficient to induce nephrotoxicity, highlighting the existence of cancer-kidney crosstalk. Our findings underscore a gap in our understanding of renal complications in patients with cancer and provide a rationale for identifying the underlying mechanisms for the development of nephroprotective agents.

Moderate grazing reduces while mowing increases greenhouse gas emissions from a steppe grassland: Key modulating function played by plant standing biomass.

Grassland represents one of the most expansive terrestrial ecosystems, exerting a profound influence on atmospheric greenhouse gas (GHG) levels within the broader context of global change. Both climate and land use changes play important roles in modulating grassland GHG emissions by directly or indirectly altering soil physical and chemical properties, especially soil temperature and inorganic nitrogen content. The optimal grassland management practices need to simultaneously meet the requirements of reducing GHG emissions, maintaining biological biodiversity, and ensuring productivity. However, the information on the management effects on GHG emissions from natural grasslands is still insufficient. Here we conducted a six-year grazing and mowing experiment in a semi-arid steppe grassland in central Inner Mongolia, and employed the static chamber method to investigate the effects of three major management measures, fencing, grazing and mowing, on ecosystem respiration (CO2 emission), methane uptake (CH4), and nitrous oxide emission (N2O) patterns in the experimental grassland. The results demonstrated that: (i) moderate grazing reduced plant aboveground standing biomass and CO2 emissions, but promoted belowground nutrient cycling and CH4 uptake; (ii) mowing enhanced plant biomass production, increased soil carbon and nitrogen content, and also increased CO2 emission; (iii) reducing grazing frequency reduced plant biomass loss and N2O emissions. We conclude that grazing at a moderate intensity and frequency is the best for mitigating GHG emissions while maintaining grassland production, and that mowing enhancement of plant production and GHG emissions should be considered in optimizing grassland management.

Individual patient data (IPD) pooled analysis on the optimal therapeutic management of patients with microsatellite instability-high (MSI) resectable gastroesophageal adenocarcinoma (GEA).

455 Background: Patients with resectable MSI/dMMR GEA showed improved survival and modest if any benefit from chemotherapy. Preoperative treatment with immune checkpoint inhibition (ICI) showed high rate of major-complete pathologic response in single arm trials possibly allowing the design of chemotherapy/surgery-free approaches. Methods: This was a multinational IPD analysis including patients with resectable GEA with MSI/dMMR status enrolled in INFINITY and NEONIPIGA phase II trials, with dual CTLA-4/PD-(L)1 ICI followed by surgery +/- adjuvant ICI; PROSECCO retrospective study, with perioperative FLOT chemotherapy and surgery, and the dataset of our previous IPD analysis on MAGIC, CLASSIC, ARTIST and ITACA-S randomized trials of patients treated with surgery alone or plus older perioperative/adjuvant chemo(radio)therapy regimen. Primary endpoint was the evaluation of rates of pathologic complete response (pCR) defined as TRG1a Becker and major-complete pathologic response (pCR/MPR) defined as TRG1a/b Becker according to preoperative treatment schedule in patients who underwent surgery. Univariable and multivariable analyses were conducted using a random effects logistic model adjusted with propensity score. Secondary endpoints were event-free survival (EFS) and overall survival (OS) according to the therapeutic strategy in the overall study population. Multivariable mixed-effects Cox models weighted with propensity score were performed. Results: The IPD included 197 patients. Of these, 49 received ICI +/- surgery, 27 FLOT chemotherapy plus surgery, 33 surgery alone and 88 older chemo(radio)therapy regimens plus surgery. In the 69 patients resected after neoadjuvant ICI or FLOT standard of care treatment, ICI demonstrated a higher rate of pathologic response compared to chemotherapy (pCR 61.9% vs 3.7%, OR 54.8 p=0.002; pCR/MPR 78.6% vs 10%, OR 39.3 p&lt;0.001). In ITT population, no significant difference in OS and EFS was shown in patients treated with ICI, FLOT plus surgery, old chemo(radio)therapy plus surgery or surgery alone. Conclusions: In resectable MSI/dMMR GEA, upfront ICI showed comparable survival outcomes to surgery alone, with limitations of study design and sample size. The impact on survival of ICI versus surgery alone should be investigated prospectively to avoid overtreatment or identify specific risk categories with benefit. The high rate of major-complete pathologic response may allow to study or perform organ sparing surgery procedures or non-operative management to reduce surgical morbidity/mortality and improve quality of life. Urinary retention during and after labor with programmed intermittent epidural bolus (PIEB) analgesia: a prospective observational study.

This study investigates the incidence and risk factors for urinary retention during and after labor in women receiving programmed intermittent epidural bolus (PIEB) analgesia and evaluates the optimal bladder management strategy. This prospective observational study assessed urinary retention during voiding attempts every two to three hours during labor and postpartum, among women with labor epidural analgesia using PIEB. Urinary retention was defined as a post-void residual volume>150mL, determined by catheterization after spontaneous voiding. Among 137 women included, with 277 voiding attempts during labor, the urinary retention rate was 20.6%, occurring in 48 women (35%). When the spontaneously voided volume was>50mL, urinary retention was observed in less than 10% of attempts. Postpartum urinary retention occurred in nine women (6.7%) with a mean post-void residual volume of 1133±447mL; all were nulliparous, seven had induced labor, and five had an episiotomy. Women who are able to successfully void during labor have a low risk of intrapartum urinary retention. We would recommend considering catheterization every 3 to 4hours for women who are unable to urinate spontaneously, void less than 50ml, or experience complete motor blockade during labor.
