A systematic review and meta-analysis of the correlation between Helicobacter pylori infection and chronic urticaria.

A meta-analysis was conducted to examine the correlation between Helicobacter pylori infection and chronic urticaria. We searched Chinese and English databases, including CNKI, Wanfang, and Weipu, using search terms such as Helicobacter pylori infection, and chronic urticaria for articles published from the establishment of the databases to February 2021 examining the correlation between Helicobacter pylori infection and chronic urticaria. The retrieved articles contained data on Helicobacter pylori infection rates in chronic urticaria cases in different regions of the north and south in China. The retrieved articles underwent strict screenings according to inclusion and exclusion criteria. Revman5.3 software was used to perform a meta-analysis on the data of the included articles. A total of 39 documents were retrieved following the searches. According to the inclusion and exclusion criteria, a total of 6 articles on 6 studies, comprising a total of 1,320 patients, were finally included in the meta-analysis. The results showed that the heterogeneity was high (I2=58%). A random-effects model was performed. An analysis of the correlation between Helicobacter pylori infection and chronic urticaria revealed significant differences between the study group and the control group [odds ratio (OR) =3.00; 95% confidence interval (CI): 1.98-4.55; P<0.00001]. The infection rate of Helicobacter pylori among chronic urticaria cases in the northern population was 16.1% (95% CI: 15.6-16.6%); of these patients 12.2% were male and 21.4% were female. The infection rate of Helicobacter pylori among chronic urticaria cases in the southern population was 18.0% (95% CI: 17.5-18.5%); of these patients, 12.3% were male and 23.1% were female. There was no significant difference in the prevalence between the male population, the female population, and the general population in the north and the south (P>0.05). Helicobacter pylori infection is correlated with the occurrence of chronic urticaria. There is no significant difference in the infection rate of Helicobacter pylori in chronic urticaria cases in different regions of the north and south. This study had some limitations. First, the number of patients included in each study was low, which may affect the accuracy of the results. Second, the detection methods were not uniform; thus, further research is required to support the conclusions drawn.

Detection of serum levels of 25-hydroxy vitamin D in patients with chronic urticaria and its clinical significance

Objective To measure serum levels of 25-hydroxyvitamin D (25HVD) in patients with chronic urticaria (CU) , and to explore its role in the occurrence of CU. Methods Peripheral blood samples were obtained from 50 patients with CU and 40 healthy controls. The urticaria activity score (UAS) was used to assess the severity of CU and to group the patients with CU. Enzyme-linked immunosorbent assay (ELISA) was performed to measure the serum levels of 25HVD, interferon-γ (IFN-γ) , interleukin-4 (IL-4) and immunoglobulin E (IgE) . Statistical analysis was carried out by t test, rank sum test and linear correlation analysis. Results The serum level of 25HVD was significantly lower in the patients with CU than in the healthy controls (15.20 ± 7.72 vs. 21.54 ± 8.31 μg/L, t= 3.75, P 0.05) . Compared with the control group, the patient group showed significantly decreased serum levels of IFN-γ (t= 15.34, P 0.05) . Conclusion The serum level of 25HVD evidently decreased in patients with CU, and it may participate in the occurrence of CU by mediating the Th1/Th2 imbalance. Correlation between dehydroepiandrosterone and occurrence of chronic urticaria

Chronic urticaria (CU) is a common skin disease with complex etiology and pathogenesis, and its cause is unclear in about 80% of patients. In clinic, there are many problems with CU, such as complicated condition, frequent recurrence, poor therapeutic effect of antihistamines, and so on, which seriously affect life quality of patients. In recent years, neuroendocrine-immune system dysfunction has got more and more attention, and has been suggested to play an important role in the pathogenesis of CU. Dehydroepiandrosterone (DHEA) and its sulfate derivative (DHEA-S) , which are secreted and regulated by the nervous system, participate in the process of neuroimmune regulation, and are regarded as potential factors regulating immune homeostasis. It has been reported that serum levels of DHEA and DHEA-S markedly decrease in patients with CU. It is unclear whether the decrease of DHEA and DHEA-S circulation concentrations is a primary cause or just a concomitant symptom of CU, and what role it plays in the development of CU. An individualized diagnostic approach based on guidelines for chronic urticaria (CU).

Chronic urticaria (CU), defined as the spontaneous or inducible appearance of hives, angioedema or both for 6 weeks or more, presents with a number of subtypes which all substantially impair patients' quality of life (QoL). International urticaria guidelines give clear recommendations on workup and treatment but the occurrence of CU with multiple causes and triggers (sometimes with more than one subtype occurring in a single patient) presents challenges for an individualized assessment by physicians. This review summarizes recent guidance on the classification, diagnosis and assessment of CU subtypes and discusses how currently available patient assessment tools and laboratory tests can be used in clinical practice as part of an individualized patient management plan.

Expressions of interleukin-9 and the transcription factor PU.1 in peripheral blood of patients with chronic urticaria and their significance

Objective To investigate the relationship of interleukin-9（IL-9）and the transcription factor PU.1 with chronic urticaria. Methods Thirty patients with chronic urticaria（patient group）and 30 healthy volunteers（control group）were included in this study. Venous blood samples were collected from these subjects. Enzyme-linked immunosorbent assay（ELISA）was performed to detect serum levels of IgE, flow cytometry（FCM）to determine the percentage of CD4+IL-9+ T cells, real-time fluorescence-based quantitative PCR（RTFQ-PCR）to measure the mRNA expressions of IL-9 and PU.1 in peripheral blood mononuclear cells（PBMCs）. Measurement data were compared between the two groups by independent-samples t test, and relationship among these parameters was assessed by Pearson correlation analysis. Results The expressions of IL-9 and PU.1 mRNAs in patients with chronic urticaria were significantly higher than those in healthy controls（4.44 ± 1.90 vs. 3.20 ± 1.78, t = 2.60, P < 0.05; 3.26 ± 1.59 vs. 2.34 ± 1.47, t = 2.34, P < 0.05）. Compared with the control group, the patient group showed increased percentage of CD4+IL-9+ T cells in peripheral blood（0.63% ± 0.44% vs. 0.22% ± 0.12%, t = 5.04, P < 0.01）and serum IgE levels（（82.04 ± 31.72）vs.（60.74 ± 28.26）IU/ml, t = 2.75, P < 0.01）. The percentage of CD4+IL-9+ T cells and levels of IL-9 and PU.1 mRNAs in peripheral blood were all positively correlated with serum levels of IgE in these patients（r = 0.596, 0.767, 0.746, respectively, all P < 0.01）. Conclusion T helper type 9（Th9）cells and IL-9 may take part in the occurrence of chronic urticaria. Key words: Urticaria; Interleukin-9; Th9 cells

Levels of plasma D-dimer, activated coagulation factor VII and XII in patients with chronic urticaria

Objective To measure the levels of plasma D-dimer, activated coagulation factor Ⅶ (FⅦa) and activated clotting factor Ⅻ (FⅫa) in patients with chronic urticaria (CU) , and to investigate their relationship with the occurrence of CU. Methods Venous blood samples were collected from 50 patients with CU and 50 healthy human controls. Dry-column immune scattering chromatography was performed to detect the plasma level of D-dimer, and enzyme-linked immunosorbent assay (ELISA) to measure the levels of F Ⅶa and FⅫa. In addition, autologous plasma skin test (APST) was conducted in 43 patients with CU, and autologous serum skin test (ASST) in 41 patients with CU. A correlation analysis was carried out between the above three parameters and disease severity as well as between the results of APST and ASST and plasma level of D-dimer. Results The levels of plasma D-dimer and FⅦa were significantly higher in patients with CU than in healthy human controls (both P 0.05) . Moreover, the degree of increase in D-dimer plasma level was positively correlated with disease severity in the patients with CU. The plasma level of D-dimer was significantly higher in APST-positive patients than in APST-negative patients (P 0.05) . Conclusions Coagulation mechanism, especially the extrinsic coagulation pathway, is related to the occurrence of CU. Studies on coagulation mechanism are beneficial to the evaluation of severity, and clinical treatment, of CU. Key words: Urticaria; Factor Ⅶa; Factor Ⅻa

Negative acute phase proteins in patients with chronic urticaria

Objective To investigate the relationship between negative acute phase proteins and chronic urticaria (CU) . Methods Fifty patients with CU were enrolled into this study, and divided into three grades, i.e., mild (grade 1) , moderate (grade 2) and severe (grade 3) according to symptoms. Twenty-eight health checkup examinees served as the control group. Immunoturbidimetry was performed to determine serum levels of prealbumin (PA) and transferrin (TRF) . Enzyme-linked immunosorbent assay was conducted to measure serum levels of insulin-like growth factor-1 (IGF-1) and tumor necrosis factor-α (TNF-α) . Statistical analysis was carried out to assess differences in these indices between these two groups, the relationship among these indices and between these indices and disease severity. Results Compared with the control group, the patients with CU showed reduced serum levels of PA ( (229.99 ± 54.16) vs. (272.06 ± 36.42) mg/L, t =3.667, P 0.05) . The serum level of PA showed a significant negative correlation with that of TNF-α (r = - 0.312, P 0.05) in the patients with CU. Conclusions Serum levels of some negative acute phase proteins decrease and negatively correlate with disease severity in patients with CU. Platelet and other hemostatic characteristics in patients with chronic urticaria.

Several publications have pointed out the importance of coagulation and fibrinolysis in the occurrence of chronic urticaria (CU), but only a few indicated the direct role of platelets. We assessed platelet aggregation and evaluated parameters of coagulation and fibrinolysis in patients with CU. Patients (n = 34) diagnosed as having CU and 36 healthy controls were enrolled. Platelet aggregation was assayed using an impedance aggregometer and adenosine diphosphate, arachidonic acid, thrombin receptor-activating peptide (TRAP), and ristocetin as agonists. In patients with CU, significantly decreased platelet aggregation to some agonists (ristocetin and TRAP) was observed. The D-dimer levels were elevated, mean platelet volume was decreased, but no alteration was observed in other coagulation assays. Elevated D-dimer levels indicated that coagulation and fibrinolysis are activated in the patients with CU. Evaluation of platelet function may contribute to identify the role of these cells in the pathogenesis of CU.

Urticaire chronique et thyroïdite de HashimotoHashimoto :à propos de six cas

Purpose. – Chronic urticaria is a common skin disorder. The cause is rarely determined. Autoimmune diseases, particularly autoimmune thyroiditis, have been implicated in the occurrence of chronic urticaria. Methods. – We reviewed clinical records of patients with Hashimoto’s disease and chronic urticaria. Results. – In our department, six patients had presented chronic urticaria associated with Hashimoto’s thyroiditis: four patients, of which three treated with L-thyroxine were euthyroid, the other two were hypothyroid. Hashimoto’s thyroiditis had been diagnosed for three patients during the investigation of chronic urticaria. Three patients developed chronic urticaria though they were treated with thyroid suppression for Hashimoto’s disease. Two of them had a dramatic improvement with opotherapy. One patient who was euthyroid without treatment improved with hormonal therapy. The fourth patient had a partial remission with thyroid hormones and was cured with corticotherapy. Conclusion. – The mecanism by which thyroid autoimmunity is associated with urticaria is poorly understood. A cross-linking of IgE receptors of mastocytes induced by antithyroid antibodies may be a cause of histamine release. Hormonal therapy may be a potent event for the clinical improvement by the suppression of chronic thyroid stimulation. Assay of thyroid hormone and antithyroid antibodies should be performed in patients with chronic urticaria. Discovery of Hashimoto’s thyroiditis with chronic urticaria requires thyroid hormone replacement not only in hypothyroid but also euthyroid patients.
