Effectiveness of Occupational Safety and Health interventions: a long way to go.

Occupational Safety and Health (OSH) has become an area of increasing concern for organizations and institutions. As it evolves, it has gradually posed ongoing challenges, becoming more complex, for organizations. Consequently, more comprehensive studies are required to advance academic and institutional research. From this perspective, this study aims to gather research contributions on the effectiveness of existing interventions for OSH improvement and identify areas for further exploration. According to the nature of scientific literature, the overall process of a literature review was investigated following an integrative approach, which involved searching for, selecting, and analyzing various literature in a creative and integrated manner, without a predefined structure. The analysis suggests that there is room for improvement in understanding the effectiveness of OSH interventions and more concrete guidance is still desirable. Based on the literature, some research areas for future developments in OSH interventions are identified. One potential area to explore further is fostering human-centered technological development and a more conscious network of stakeholders, with higher coordination, shared knowledge, and open communication. Focusing on the proposed directions will support scholars and practitioners in pursuing continuous OSH improvement through more effective and well-grounded workplace interventions and encourage organizations to be proactive in daily OSH management.

Enabling Effective Implementation of Occupational Safety and Health Interventions

BackgroundThe design, implementation, and evaluation are three important stages of occupational safety and health (OSH) interventions. Historically, there has been a tendency to prioritize implementation, often neglecting detailed design and rigorous outcome evaluation. Currently, much has changed, and contemporary approaches recognize the interdependence of these stages, considering them integral to the success of any intervention. This work presents a comprehensive procedure for implementing interventions, not only to ensure short-term effectiveness but also their long-term sustainability through continuous monitoring. The focus is on a national OSH project introducing a near-miss management system (NMS) in Italy. MethodsInitial meetings were convened among project partners, complemented by interviews with diverse stakeholders, to plan implementation steps and test the NMS. Tailored questionnaires were designed for diverse stakeholder groups – initial promoters, company managers and employers, and employees – facilitating targeted implementation, and three case studies were started in Italian regions to assess the structured implementation, involving intervention promoters and collaborating companies. ResultsThe primary outcome is the development of practical tools, specifically three questionnaires, which are considered valuable for establishing an effective human-centered implementation strategy, meticulously designed to facilitate ongoing monitoring of processes and continual enhancement of instruments intended for NMS integration within companies. ConclusionsThis work lays the foundation for successful NMS implementation in Italy and, although the outlined procedure had specific objectives, it also provides valuable insights applicable in enhancing the effectiveness and sustainability of interventions across diverse contexts. It underscores the importance of comprehensive planning, stakeholder engagement, and continuous evaluation in achieving lasting OSH interventions.

The Influence of Organizational Aspects of the U.S. Agricultural Industry and Socioeconomic and Political Conditions on Farmworkers’ COVID-19 Workplace Safety

Farmworkers in the U.S. experienced high rates of COVID-19 infection and mortality during the COVID-19 pandemic. Their workplace may have been a significant place of exposure to the novel coronavirus. Using political economy of health theory, this study sought to understand how organizational aspects of the agricultural industry and broader socioeconomic and political conditions shaped farmworkers’ COVID-19 workplace safety during the pandemic. Between July 2020 and April 2021, we conducted and analyzed fourteen in-depth, semi-structured phone interviews with Latinx farmworkers in California. Findings show that regulatory oversight reinforced COVID-19 workplace safety. In the absence of regulatory oversight, the organization of the agricultural industry produced COVID-19 workplace risks for farmworkers; it normalized unsafe working conditions and the worker—rather than employer—responsibility for workplace safety. Under these conditions, farmworkers enacted personal COVID-19 preventative practices but were limited by financial hardships that were exacerbated by the precarious nature of agricultural employment and legal status exclusions from pandemic-related aid. Unsafe workplace conditions negatively impacted workplace camaraderie. Study findings have implications for farmworkers’ individual and collective agency to achieve safe working conditions. Occupational safety interventions must address the organizational aspects that produce workplace health and safety inequities and disempower farmworkers in the workplace.

Evaluation of occupational health and safety intervention for the waste and sanitation workers in Bangladesh during COVID-19

Waste and sanitation workers in South-Asian countries are vulnerable to injuries and diseases, including COVID-19. In Bangladesh, an intervention was implemented during COVID-19 to lower these workers' occupational health risks through training and PPE distribution.We assessed how the intervention affected their occupational health behaviors using a randomized cluster trial in 10 Bangladeshi cities, including seven intervention and three control areas. We conducted 499 surveys (Control-152, Intervention-347) and 47 structured observations (Control:15, Intervention:32) at baseline and 499 surveys (Control:150, Intervention:349) and 50 structured observations (Control:15, Intervention:35) at endline. To evaluate the impact of intervention at the endline, we used the difference in difference (DID) method.Compared to control, workers from intervention areas were more likely to have increased knowledge of using/maintaining PPEs (adjusted DID: 21%, CI: 8, 33), major COVID-19 transmission causes (adjusted DID: 27%, CI: 14, 40), and preventive measures (adjusted DID: 12%, CI: -0.6, 25), and improved attitude about using PPEs (adjusted DID: 36%, CI: 22, 49), washing PPEs (adjusted DID: 20%, CI: 8, 32). The intervention was more likely to improve workers' self-reported practice of taking adequate precautions after getting back from work (adjusted DID: 37%, CI: 27, 47) and changing/cleaning masks every day (adjusted DID: 47%, CI: 0.03, 94), and observed practices of maintaining coughing etiquette (DID: 20%, CI: 0.2, 40) at workplace and handwashing before wearing PPEs (DID: 27%, CI: 2, 52), after finishing work (DID: 31%, CI: -7, 69) & during work intervals (DID: 30%, CI: -33, 93). There was not much improvement in observed practices of mask use (DID: 1%, CI: -40, 42), handwashing before meals (DID: 2%, CI: -61, 65), and after toilet (DID: 7%, CI: -41, 55).This intervention has improved the knowledge, attitude and practice of the workers about critical occupational risk mitigation, which may be replicated in similar settings. Future interventions need to address occupational health-related injuries and health complications, introduce regular health checkups/insurance for the workers, create a balance between the quality and comfort of the PPEs and ensure a mechanism to ensure a regular supply of PPEs.

A Systematic Review of Social Marketing Interventions Promoting Occupational Health and Safety

Background Unintentional and preventable workplace injuries are a common occurrence across employment contexts. Campaign and intervention initiatives are often implemented to promote health and safety in the workplace. However, to date, scarce research has reviewed the manner in which social marketing principles have been used to inform these efforts. Focus of Article The current review seeks to address this gap by completing a systematic review of occupational health and safety interventions that utilize social marketing approaches. Research Question This review sought to identify key trends and characteristics across occupational health and safety social marketing efforts such as what topics, theories, and design characteristics are most often used to inform these initiatives. The review also sought to determine how effective social marketing tactics were in promoting behavior change. Method In total, 4,354 article titles and abstracts were reviewed across four databases. Ultimately, 11 articles reviewed met the set inclusion criteria, were reviewed, and subsequently analyzed. Results Results indicated that social marketing interventions typically addressed behaviors related to construction safety, agricultural safety, and safety within healthcare environments. While little continuity existed among information dissemination approaches specified, it was clear that many interventions used some form of traditional media (e.g., print, radio, etc.) as a mainstay of their approach. Further, most were effective in promoting behavior change. Importance to the Social Marketing Field While this review identified vital information relating to the development and implementation of occupational health and safety initiatives, clear gaps exist in how key campaign elements, messaging approaches, and strategies are documented in the literature. Recommendations for Research and Practice Practitioners can and should use information identified from this review to guide the development of social marketing interventions. Further, this work also provides important guidance on best practices for documenting features and outcomes of future social marketing efforts.

A preliminary study on the barriers and facilitators to improving the health, safety, and well-being of aging heavy vehicle drivers

Introduction. Managers from road freight transportation organizations were interviewed on barriers and facilitators to implementation of occupational health, safety, and well-being interventions for aging heavy vehicle drivers. As aging drivers are more likely to be seriously injured or die in a work-related incident than younger drivers, it is important to recognize strengths and weaknesses throughout the system to identify intervention that addresses their specific needs. Method: A Systems Theoretic Accident Model and Processes (STAMP) control structure was constructed to chart the controllers, controls, and feedback channels in the system to identify gaps in health, safety, and well-being intervention in the system. The STAMP control structure also charted the barriers and facilitators within levels across the system. Eleven managers were recruited into the study representing a range of road freight transportation organizations throughout Australia. Results: Interview data revealed that barriers and facilitators existed at most levels of the system. Facilitators included advice from external agencies, support from upper management, modern technology, and regular social communication with drivers. Barriers were a lack of guidance on aging issues, operational conflicts with health and safety objectives, and the drivers’ fear of disclosing health information associated with their driving role. In regards to formalized intervention in place to support aging heavy vehicle drivers, the system is reliant on fitness to drive medical assessments based on age and jurisdiction. Conclusions: As there was generally a lack of senior direction cited from the upper levels of the system on aging issues, there was much variation across the study on how aging risks are managed in the workplace for heavy vehicle drivers. Practical Applications: This study recommends that managers across the road freight transportation industry receive formalized aging-awareness health and safety training in how to manage work-related driving hazards for aging heavy vehicle drivers.

Sustainable occupational safety and health interventions: A study on the factors for an effective design

Effective interventions are a priority in continuously changing occupational environments, particularly in companies struggling to manage health and safety in the workplace. Practitioners may consider practical solutions for Occupational Safety and Health (OSH) improvement as a panacea for all major problems. However, they may overlook a range of other factors that affect the success of such solutions. The way in which a solution is developed, designed, implemented, and evaluated determines its impact. Participatory interventions are one way of ensuring better results. Consequently, this study proposes a way of establishing sustainable, effective, and efficient interventions by defining the required processes and actively involving responsible actors (i.e., who, when, and how).A national OSH intervention for introducing a near-miss management system, funded by the Italian National Institute for Insurance against Accidents at Work (INAIL), is used as a reference because its development process includes an accurate design stage. Based on this intervention, a multistep design process is built to answer how (how the intervention will persist by defining the context, processes, and scenarios), who (who will be the responsible actors actively participating), and when (when actors will be involved) questions.The design process established for the intervention, although within a specific context, provides clues to discriminant factors that would enable effectiveness in general interventions, and the proposed system for near-miss management generates insights that may be generalizable to other OSH interventions developed in different environments.

Impact assessment of e-trainings in occupational safety and health: a literature review

BackgroundImplementing workplace preventive interventions reduces occupational accidents and injuries, as well as the negative consequences of those accidents and injuries. Online occupational safety and health training is one of the most effective preventive interventions. This study aims to present current knowledge on e-training interventions, make recommendations on the flexibility, accessibility, and cost-effectiveness of online training, and identify research gaps and obstacles.MethodAll studies that addressed occupational safety and health e-training interventions designed to address worker injuries, accidents, and diseases were chosen from PubMed and Scopus until 2021. Two independent reviewers conducted the screening process for titles, abstracts, and full texts, and disagreements on the inclusion or exclusion of an article were resolved by consensus and, if necessary, by a third reviewer. The included articles were analyzed and synthesized using the constant comparative analysis method.ResultThe search identified 7,497 articles and 7,325 unique records. Following the title, abstract, and full-text screening, 25 studies met the review criteria. Of the 25 studies, 23 were conducted in developed and two in developing countries. The interventions were carried out on either the mobile platform, the website platform, or both. The study designs and the number of outcomes of the interventions varied significantly (multi-outcomes vs. single-outcome). Obesity, hypertension, neck/shoulder pain, office ergonomics issues, sedentary behaviors, heart disease, physical inactivity, dairy farm injuries, nutrition, respiratory problems, and diabetes were all addressed in the articles.ConclusionAccording to the findings of this literature study, e-trainings can significantly improve occupational safety and health. E-training is adaptable, affordable, and can increase workers’ knowledge and abilities, resulting in fewer workplace injuries and accidents. Furthermore, e-training platforms can assist businesses in tracking employee development and ensuring that training needs are completed. Overall, this analysis reveals that e-training has enormous promise in the field of occupational safety and health for both businesses and employees.

Factors associated with blue-collar workers' participation in Worksite Health Promotion Programs: a scoping literature review.

A growing number of employers implement worksite health promotion programs (WHPPs). In particular, blue-collar workers may benefit from these WHPPs. However, they are less likely than other workers to participate and little is known about which factors affect their participation. The aim of this scoping literature review is to produce an overview of studies on factors associated with blue-collar workers' participation in WHPPs. Five databases were searched: BSU, PsycINFO, Medline, Web of Science and CINAHL. The review included peer-reviewed empirical studies on determinants associated with blue-collar workers' participation in WHPPs. Factors were extracted and categorized. Similar determinants were clustered and the direction of the associations was further examined. Nineteen papers describing 11 qualitative and 4 quantitative studies met the eligibility criteria. Seventy-seven determinants were analyzed (in quantitative studies) or reported (in qualitative studies). In most studies, only participant characteristics were investigated. Participation may be enhanced by addressing needs, tailoring from a broad range of activities, offering group activities, requiring little effort and commitment at the start, using incentives, leading by example and combining WHPPs with occupational safety interventions. WHPPs seem to be able to reach blue-collar workers, but it remains particularly challenging to reach shift workers and those who do not yet experience health complaints.

The prevalence and mental health correlates of exposure to offensive behaviours at work in Hungary: results of a national representative survey

BackgroundWithin the last decades, a substantial number of reports have established bullying behaviours as a severe risk to the health and safety of workers. However, in Hungary, the severity of this issue remains largely unknown. Therefore, the current study aimed to 1) determine the prevalence of offensive workplace behaviours in the Hungarian working population and 2) examine the relationship between exposure to these offensive behaviours and certain mental health indicators.MethodsThe cross-sectional analyses of the present study are based on a sample of 13,104 active workers being representative of the Hungarian working population according to gender, age, educational level, and 18 occupational sectors. The mid-length version of the Copenhagen Psychosocial Questionnaire II (COPSOQ II) was used to measure workplace offensive behaviours (bullying, sexual harassment, threats of violence, and physical violence) in the 12 months preceding the survey. Examined mental health correlates included depressive symptomatology (Beck Depression Inventory), functional somatic symptoms (PHQ-15), perceived stress (Perceived Stress Scale), and general well-being (WHO Well-being Index).ResultsAlmost half (48.7%) of the sample reported exposure to some form of offensive behaviour; 37.6% of participants reported occasional-, while 11.1% reported weekly or daily exposure. More women than men were exposed to offensive workplace behaviours, and those targeted the most were individuals aged 18–29 and in companies employing 20–49 employees. Top managers reported the lowest amount of bullying, while unskilled labourers reported the most frequent exposure. A moderately strong relationship was discovered between exposure to workplace offensive behaviours and all indicators of mental health.ConclusionWorkplace bullying was revealed to be a significant public health concern according to this large, representative data set from Hungary. Strategies to reduce the occurrence and impact of these behaviours on employee health should be a priority for occupational health and safety interventions.

Congruence Rules! Increased Self-efficacy after Occupational Health Interventions—if Leaders and Teams Agree on the Participative Safety Climate

To succeed with participatory occupational health and safety (OHS) interventions it is not sufficient to consider only the employees’ perspective, as perceptual distance between leaders and teams is known to have an effect on outcomes. The aim of this paper is to investigate the impact of leaders’ and teams’ perceptions of a non-threatening interpersonal atmosphere of trust and support (i.e., a participative safety climate) on employees’ changes in confidence in their ability at work to 1) interact socially (social self-efficacy), 2) manage emotions (emotional self-efficacy), and 3) solve tasks (cognitive self-efficacy) following a participatory OHS intervention. Thirty leaders and 348 employees in 28 teams from 5 organizations completed surveys before and after the intervention. Polynomial regression with response surface analyses revealed that agreement between leaders and teams regarding participative safety before the intervention related positively to all three self-efficacy dimensions after the intervention. These results exemplify how leaders’ and their teams’ different perceptions of the climate before implementing an intervention may affect changes in intervention-relevant outcomes. The findings contribute to the emergent understanding of how interventions are dependent on the organizational context where they are implemented. It also points to the need to consider non-linear relations in intervention research. The findings suggest that in practice, organizations conducting participatory OHS interventions should assess and address pre-intervention climate factors to succeed. Congruence matters.

Facilitators and barriers to implementing occupational safety interventions in the fishing industry: A scoping review

BackgroundHigh fatality rates among fishermen have characterized the fishing industry for decades. Several safety interventions have been implemented, with considerable decline in fatalities in some countries. However, statistics suggest that global rates are still high. In addition, little is known regarding factors that support or hinder the implementation of these interventions. ObjectiveTo explore, synthesize and analyze factors influencing the implementation of safety interventions in the fishing industry. MethodsIn accordance with the scoping review methodology, an extensive search of articles reporting on factors influencing implementation of safety interventions was conducted in PubMed, CINAHL, Embase, Scopus and grey literature. The Social Ecological Model was used as a guiding framework for mapping and analysis of results. ResultsThe search identified 618 articles with 12 meeting the inclusion criteria. The key facilitators included knowledge, management commitment, collaborative practices, supportive policies and enforcement of regulation. The key barriers included low risk perception, lack of time, inadequate skills, cultural and social norms, lack of support, high compliance cost, lack of harmonization of regulations, inadequate enforcement guidelines, lack of funding and weak coordination. ConclusionThe review highlighted that there is limited literature on factors affecting the implementation of safety interventions on most of the levels of the Social Ecological Model. There is need for extensive global research to fully understand what enables and impedes the implementation of safety interventions, as lack of attention to exploring these factors will result in non-reduction of fatalities. An effort to meet the UN's 17 global goals for sustainable development in fishing will be impossible. Furthermore, future knowledge translation should focus on the identified facilitators and barriers.

Physical trauma and injury: A multi-center study comparing local residents and refugees in Lebanon.

BackgroundRefugees are susceptible to various types of injury mechanisms associated with their dire living conditions and settlements. This study aims to compare and characterize the emergency department admissions due to physical trauma and injuries among local residents and refugees in greater Beirut.MethodsThis epidemiological study analyzes injury incidence and characteristics of patients presenting to Emergency Departments of 5 sentinel hospitals between 2017 and 2019. Using the WHO Injury Surveillance Guidelines and Pan-Asia Trauma Outcomes Study form, an injury data surveillance form was designed and used in hospital settings to collect data on injuries. Chi-square test analysis was performed to compare differences in injury characteristics between local residents and refugees. Regression models were constructed to assess the effect of being a refugee on the characteristics of injuries and outcomes of interest.ResultsA total of 4847 injuries (3933 local residents and 914 refugees) were reported. 87.4% of the total injuries among refugees were sustained by the younger age groups 0-45 years compared to 68.8% among local residents. The most prevalent injury mechanism was fall (39.4%) for locals and road traffic injury (31.5%) for refugees. The most injured body part was extremities for both populations (78.2% and 80.1%). Injuries mostly occurred at home or its vicinity (garden or inside the camp) for both populations (29.3% and 23.1%). Refugees sustained a higher proportion of injuries at work (6%) compared to locals (1.3%). On multivariate analysis, refugee status was associated with higher odds of having an injury due to a stab/gunshot (odds ratio (OR) = 3.392, 95% confidence interval (CI) = 2.605-4.416), having a concussion injury (OR = 1.718, 95% CI = 1.151-2.565), and being injured at work (OR = 4.147, 95% CI = 2.74-6.278). Refugee status was associated with increased odds of leaving the hospital with injury-related disability (OR = 2.271, 95% CI = 1.891-2.728)]ConclusionsInjury remains a major public health problem among resident and refugee communities in Beirut, Lebanon. Refugees face several injury-related vulnerabilities, which adversely affect their treatment outcomes and long-term disabilities. The high prevalence of occupational and violence-related injuries among refugees necessitates the introduction of targeted occupational safety and financial security interventions, aiming at reducing injuries while enhancing social justice among residents.

The Development of an Interdisciplinary and Integrated Model of Occupational Safety

Background: Occupational safety and health professionals facilitate safer workplaces through the development and implementation of interventions. Empirically validated theories can enhance the likelihood that an intervention will be successful in achieving the desired outcome; however, occupational safety interventions are often devoid of theory or utilize frameworks that fail to take a comprehensive approach to conceptualizing occupational safety processes. Focus of the Article: The current paper seeks to address these gaps by proposing an interdisciplinary and integrated model of occupational safety (IIMOS). Importance to the Social Marketing Field: IIMOS takes an interdisciplinary approach, examining the combined influence of concepts from psychology, social marketing, and occupational safety on behavioral change. This model accounts for the influence that threat appraisal, coping appraisal, and customer service factors can have on safety behaviors’ initiation/adoption and maintenance. Recommendations for Research and Practice: IIMOS may inform the design, implementation, and evaluation of occupational safety and health programs. Although future empirical work must still test the model’s propositions, the model’s in-depth application of social marketing techniques is a novel addition to the field. This model can encourage practitioners to develop innovative products, build relationships with consumers, and design upstream efforts to support program adoption.

Utilization of Machine Learning in Supporting Occupational Safety and Health Decisions in Hospital Workplace

The prediction of possible future incidents or accidents and the efficiency assessment of the Occupational Safety and Health (OSH) interventions are essential for the effective protection of healthcare workers, as the occupational risks in their workplace are multiple and diverse. Machine learning algorithms have been utilized for classifying post-incident and post-accident data into the following 5 classes of events: Needlestick/Cut, Falling, Incident, Accident, and Safety. 476 event reports from Metaxa Cancer Hospital (Greece), during 2014-2019, were used to train the machine learning models. The developed models showed high predictive performance, with area under the curve range 0.950-0.990 and average accuracy of 93% on the 10-fold cross set, compared to the safety engineer’s study reports. The proposed DSS model can contribute to the prediction of incidents or accidents and efficiency evaluation of OSH interventions.

Cost Benefit Analysis of Implementation Occupational Health and Safety: Literature Review

Assessment of management commitments is required for investment in Occupational health and safety program. This systemic literature review evaluates 15 cases of Occupational health and safety (OHS) interventions to assess whether the implementation of OHS is beneficial. Search engines used in literature review are Medline, Embase, Web of Science, Cochrane Library, Pubmed. Studies related to benefit cost analysis included in the search include also looking at company productivity. The focus of this analysis includes samples, design, theoretical framework, implementation OHS, validity and results.Results show,in 15 cases, cross-sectional research design looked at pre-after comparisons in the absence of control groups. The positive return on investment from the implementation of OHS is shown in 13 studies. Two other studies concluded that their chosen implementation of OHSwas not cost-effective. The main benefit is to reduce sick leave. In 3 cases the perceived benefit is an increase in productivity and quality. The conclusions in this study required a guideline for the evaluation of the implementation of OHS.

Reaching “hard to reach” workers: Evaluating approaches to disseminate worker safety information via the Mexican consular network

Mexican immigrants suffer a disproportionately large number of work-related injuries and deaths given their share of the workforce. Barriers of language, culture, and mistrust are often cited as factors that complicate efforts to reach these workers with occupational safety and health (OSH) interventions. By partnering with the Mexican government and its consulate network in the United States, researchers from the National Institute for Occupational Safety and Health were able to assess the impact of four different information dissemination approaches (posters, passively distributed brochures, actively distributed brochures, and video kiosks) in Spanish in a five-phase study. Exit interviews conducted with Mexicans seeking consular services indicated that while nearly all respondents considered OSH to be of importance, significant differences in impact measures, such as noticing the materials and liking of content, were found when comparing the different approaches. Despite these differences, even the least effective approaches were noticed by large numbers of individuals and significantly increased their stated behavioral intentions regarding OSH. Considering all materials together, significantly more participants reported liking the materials (p < 0.001) than did not, learning something new (p < 0.01), trusting the information (p < 0.05), intending to seek out additional OSH information (p < 0.01), and intending to speak to their bosses about OSH (p < 0.05). These findings contribute to building an evidence base for moving research knowledge into practice, which is an essential, but often overlooked, element of occupational safety and health research, particularly among workers from underserved communities.

Flying below the Radar: Psychoactive Drug Use among Young Male Construction Workers in Sydney, Australia

Background: Globally, there is growing concern regarding workers’ illicit drug use and its implications for health and workplace safety. Young workers in male-dominated industries, such as construction, may be more susceptible to illicit drug use, risky drinking and its associated harms. Purpose/objectives: To investigate drug use and perceptions of risk among male construction workers, drawing comparisons between workers under 25 years with older age groups. Methods: Workers in Sydney, Australia (N = 511) completed a survey measuring past year illicit drug and alcohol use, psychological distress and perceptions of drug-related risks to health and safety. Prevalence in the total sample was compared with national estimates, and differences between younger and older survey respondents were examined using logistic regression models. Results: Survey respondents’ cocaine, meth/amphetamine and cannabis use was significantly higher than estimates of male employees nationally (OR = 6.60, 3.58, 1.61, respectively). Young workers ≤24 were more likely to frequently use illicit drugs, drink heavily, and report psychological distress than those aged 35+. Workers ≤24 were least likely to perceive that drug use posed high risks to health or safety when compared with 25-34 and 35+ age groups. Conclusions/importance: The findings highlight the high prevalence of illicit drug use amongst young construction workers, representing threats to workplace safety even if used outside work hours. Greater emphasis on potential adverse effects of alcohol and drug use and closer examination of contributory workplace factors are required. These findings have practical implications to inform occupational health and safety programs and interventions in high-risk workplaces.

Economic burden of work injuries and diseases: a framework and application in five European Union countries

BackgroundEstimates of the economic burden of work injuries and diseases can help policymakers prioritize occupational health and safety policies and interventions in order to best allocate scarce resources. Several attempts have been made to estimate these economic burdens at the national level, but most have not included a comprehensive list of cost components, and none have attempted to implement a standard approach across several countries. The aim of our study is to develop a framework for estimating the economic burden of work injuries and diseases and implement it for selected European Union countries.MethodsWe develop an incidence cost framework using a bottom-up approach to estimate the societal burden of work injuries and diseases and implement it for five European Union countries. Three broad categories of costs are considered—direct healthcare, indirect productivity and intangible health-related quality of life costs. We begin with data on newly diagnosed work injuries and diseases from calendar year 2015. We consider lifetime costs for cases across all categories and incurred by all stakeholders. Sensitivity analysis is undertaken for key parameters.ResultsIndirect costs are the largest part of the economic burden, then direct costs and intangible costs. As a percentage of GDP, the highest overall costs are for Poland (10.4%), then Italy (6.7%), The Netherlands (3.6%), Germany (3.3%) and Finland (2.7%). The Netherlands has the highest per case costs (€75,342), then Italy (€58,411), Germany (€44,919), Finland (€43,069) and Poland (€38,918). Costs per working-age population are highest for Italy (€4956), then The Netherlands (€2930), Poland (€2793), Germany (€2527) and Finland (€2331).ConclusionsOur framework serves as a template for estimating the economic burden of work injuries and diseases across countries in the European Union and elsewhere. Results can assist policymakers with identifying health and safety priority areas based on the magnitude of components, particularly when stratified by key characteristics such as industry, injury/disease, age and sex. Case costing can serve as an input into the economic evaluation of prevention initiatives. Comparisons across countries provide insights into the relevant performance of health and safety systems.
