Dentinal Tubule Penetrability and Bond Strength of Two Novel Calcium Silicate-Based Root Canal Sealers.

Once the chemo-mechanical preparation of root canals is finished, achieving a complete seal of the root canal system becomes crucial in determining the long-term success of endodontic treatment. The important goals of root canal obturation are to minimize leakage and achieve an adequate seal. Thus, a material that possesses satisfactory mechanical characteristics, is biocompatible, and has the ability to penetrate the dentine tubules adequately is needed. This study aimed to compare the penetrability and bond strength between two calcium silicate-based sealers and an epoxy resin-based sealer, as well as examine the relationship between penetrability and bond strength for the different sealers. Thirty-nine recently extracted single-rooted human premolar teeth were instrumented and divided evenly into three groups (n = 13), according to the sealer used for obturation: AH Plus Jet, EndoSequence, and AH Plus Bioceramic Sealer. Three teeth (30 slices) were randomly selected out of each for analysis using confocal laser scanning microscopy to assess penetrability. The remaining ten teeth (90 slices) in each group were subject to push-out tests using a universal testing machine. All teeth were sectioned into nine transverse slices of 0.9 mm thickness for their respective tests (apical, middle, coronal). AH Plus Jet exhibited significantly lower penetrability and significantly higher bond strength compared to EndoSequence BC sealer (p = 0.002) and AH Plus Bioceramic Sealer (p = 0.006). There was no significant difference between EndoSequence BC sealer and AH Plus Bioceramic Sealer in terms of either penetrability or bond strength. No correlation was found between penetrability and bond strength. Within the limitation of this study and regardless of the location in the canal, the bioceramic based root canal sealers appeared to perform better than the epoxy resin-based sealer in terms of dentinal penetration rate. Further studies are required to compare other biomechanical properties of bioceramic sealers including setting characteristics and bacterial leakage.

Efficacy of Shock Wave-Enhanced Emission Photoacoustic Streaming (SWEEPS) in the Removal of Different Combinations of Sealers Used with Two Obturation Techniques: A Micro-CT Study.

This study sought to evaluate the efficacy of SWEEPS in the removal of epoxy-resin-based and calcium-silicate-containing endodontic sealer combined with single-cone and carrier-based obturation techniques through a micro-CT analysis. Seventy-six single-rooted extracted human teeth with single root canal were instrumented with Reciproc instruments. Specimens were randomly divided into four groups (n = 19) according to the root canal filling material and obturation technique: (1) AH Plus sealer + Reciproc gutta-percha, (2) TotalFill BC sealer + TotalFill BC Points, (3) AH Plus sealer + Guttafusion obturator, and (4) MTA Fillapex + Guttafusion obturator. All specimens were re-treated one week later using Reciproc instruments. Following re-treatment, root canals were additionally irrigated using the Auto SWEEPS modality. The differences in the root canal filling remnants were analyzed by micro-CT scanning of each tooth after root canal obturation, after re-treatment, and after additional SWEEPS treatment. Statistical analysis was performed using an analysis of variance (p < 0.05). The additional treatment with SWEEPS significantly reduced the volume of the root canal filling materials in all experimental groups compared to the removal of root canal filling using only reciprocating instruments (p < 0.05). However, the root canal filling was not removed completely from any of the samples. SWEEPS can be used to enhance the removal of both epoxy-resin-based and calcium-silicate-containing sealers, in combination with single-cone and carrier-based obturation techniques.

The Effect of Two Different Root Canal Sealers on Post Obturation Pain

Objective: The purpose of this research was to evaluate the effectiveness of various root canal sealers in reducing postoperative pain and the need for analgesics. Methods: The asymptomatic necrosis and apical periodontitis of 60 single-rooted teeth were studied by randomly assigning them to 2experimental groups (n=30) according to the root canal sealer: AH Plus and MTA Fillapex. Two appointments were needed to complete the endodontic treatment, and calcium hydroxide was employed as the intracanal dressing. The patients were asked to assess the severity of their discomfort as no pain, slight and moderate pain. After 24 hours, 48 hours, and 7 days, patients rated their discomfort on a scale from 1 to 4. The use of pain relievers was also documented. Postoperative pain and the requirement for analgesia were compared using the t-test. Results: The Mean±S.D of the participants age in AH plus and MTA Fillapex group was 39.1±9.4 and 43±9.2 years. Overall, 62% were female and 38% were males were included in the present research. Overall, 37% of the cases (22 patients) had postoperative discomfort after 24 hours, with the number decreasing to 18% (11 patients) after 48 hours. After 7 days, no one in either of the groups had any discomfort. There were no statistically significant differences between groups in the occurrence or severity of postoperative pain or in the use of analgesics at any of the assessment points. Conclusion: Similar rates of postoperative discomfort and the requirement for analgesics were seen with the use of either AH Plus or MTA Fillapex for root canal filling. Evaluation of chronic apical periodontitis treatment results with Tricalcium silicate

Background: Chronic apical periodontitis is potential infection with poor symptoms that are easily overlooked. Disease progession can lead to cellulitis, osteomyelitis, sinusitis, endocarditis, nephritis, arthritis, persistent fever and sepsis. In recent times, thanks to advances in endodontic in improving instruments, root canal antiseptics, techniques for shaping, obturation canals, endodontic treatment has achieved remarkable success rate. BioRootTM RCS is born from Active Biosilicate technology, is the lasted generation biomaterial, with the main ingredient being Tricalcium silicate and recommended for root canal obturation. Materials and Methods: 33 tooth with diagnosis of chronic apical periodontitis of 33 patients with inclusion criterias were selected in study sample, carried out endodontis treatment and obturation canals with BioRootTM RCS and cone Gutta Percha. Evaluating treatment results after 3 months, 6 months and factors associated with treatment results. Results: About treatment result after 3 months, 6 months: very good accounted for 87.9% and 90.9%, good were 12.1% and 9.1%, no poor cases; there was a relationship between the periapical index, lesion size, lesion boudaries with treatment results. A Comparative Analysis of Voids and Sealing Ability of Different Obturating Techniques Using CBCT.

To evaluate voids and sealing ability using a disposable syringe, endodontic pressure syringe, and Skinni syringe with NaviTip in primary molars with cone-beam computed tomography (CBCT). The 15 extracted primary mandibular molars with at least one root ≥8 mm length and an equal number of mesiobuccal canals were divided into three groups, i.e., obturation using a disposable syringe, an endodontic pressure syringe, and a Skinni syringe with NaviTip, respectively. The evaluation of the apical seal was determined as the measurement between the apical end of the filling material and the radiographic apex. The quality of the filling was determined by the size, number, type, and location of voids present. Statistical analysis was done using the Chi-square test and post-hoc test. The endodontic pressure syringe score was the highest and statistically significant in obtaining apical seal (p = 0.013). Disposable syringe shows highest size of voids (p = 0.01) in which type I-voids (p = 0.04) and type S-voids (p = 0.07) were statistically significant. The location of voids was maximum at the middle third of the root (p = 0.016). The endodontic pressure syringe provided the best root canal obturation of primary molars, whereas the disposable syringe was least effective with the maximum number and size of voids. Comparing the voids and sealing abilities of different obturating techniques with CBCT would help the pediatric practitioners for better outcome of obturation in primary teeth.

Fatigue performance of endodontically treated molars reinforced with different fiber systems.

The aim was to investigate the fatigue performance of root canal-treated (RCT) molars restored with different direct restorations utilizing discontinuous and continuous fiber-reinforced composite (FRC) systems. The impact of direct cuspal coverage was also evaluated. One hundred and twenty intact third molars extracted for periodontal or orthodontic reasons were randomly divided into six groups (n=20). Standardized MOD, regular cavities for direct restorations were prepared in all specimens, and subsequently, root canal treatment and root canal obturation was carried out. After the endodontic treatment, the cavities were restored with different fiber-reinforced direct restorations as follows: SFC group (control), discontinuous short fiber-reinforced composite (SFC) without cuspal coverage (CC); SFC+CC group, SFC with cuspal coverage; PFRC group, transcoronal fixation with continuous polyethylene fibers without CC; PFRC+CC group, transcoronal fixation with continuous polyethylene fibers with CC; GFRC group, continuous glass FRC post without CC; and GFRC+CC, continuous glass FRC post with CC. All specimens underwent a fatigue survival test in a cyclic loading machine until fracture occurred or 40,000 cycles were completed. The Kaplan-Meier survival analysis was conducted, followed by pairwise log-rank post hoc comparisons between the individual groups (Mantel-Cox). The PFRC+CC group was characterized by significantly higher survival compared to all the groups (p < 0.05), except for the control group (p = 0.317). In contrast, the GFRC group showed significantly lower survival compared to all the groups (p < 0.05), except for the SFC+CC group (p = 0.118). The control group (SFC) showed statistically higher survival than the SFRC+CC group (p < 0.05) and GFRC group (p < 0.05), but it did not differ significantly from the rest of the groups in terms of survival. Direct restorations utilizing continuous FRC systems (in the form of polyethylene fibers or FRC post) to restore RCT molar MOD cavities performed better in terms of fatigue resistance when CC was performed compared to the same FRC restorations without CC. On the contrary, teeth restored with SFC restorations performed better without CC compared to the ones where SFC was covered. In the case of fiber-reinforced direct restorations for MOD cavities in RCT molars, direct CC is recommended when utilizing long continuous fibers for reinforcement, however, should be avoided when only SFC is used for their reinforcement.

Comparison sealing ability in three bioceramic sealers applied in hydraulic condensation technique by using micro-computed tomography

Background/purposeSealing ability in root canal obturation has always been a key concern for endodontic success. The purpose of this study was to analyze the percentage of voids in root canal space obturated by using single cone hydraulic condensation with different root canal sealers and to compare those with AH Plus sealer. Materials and methodsExperiments were conducted using twenty 3D-printed upper first premolars. After the buccal root canals were prepared using Ni–Ti rotary instruments, the teeth were divided into four groups: the AH Plus, BC sealer, BC sealer HiFlow, and Endoseal MTA groups. All buccal canals were obturated by single-cone hydraulic condensation. All specimens were scanned using micro-computed tomography and the percentage volume of the voids inside and outside the filled materials (Vin and Vout) at three different canal depth intervals were calculated by a Bruker micro-CT software. Differences according to root canal sealers were evaluated statistically using the Kruskal–Wallis test and the Wilcoxon Rank Sum test at a significance level of 0.05. ResultsThe results indicated that most of the voids were presented near the interface (Vout), the Vin is very small and not significant different between groups. The Vout decreased in the following order: AH Plus(1.837% ± 1.226%)≅BC sealer (1.225% ± 0.836%)>BC sealer Hiflow(0.349% ± 0.071%)>Endoseal MTA(0.203% ± 0.049%) ConclusionFor the percentage volume of voids between the root canal filling material and root canal surface, though the BC sealer Hiflow is slightly larger than Endoseal MTA, which is still much less than BC sealer and AH Plus.

An In-Vitro Analysis of Root Fracture Strength Using Single File Systems.

Aim Root canal obturation during endodontic therapy supports the root canal space and helps the extra tooth structure resist fracture.Some believe endodontic-treated teeth are more likely to break than natural teeth. The most common causes of tooth decay are endodontic treatment's extensive tooth structure loss and coronal and radicular dentin drying. Materials and methods Two hundred removed human permanent mandibular first molars were allowed to be stored in isotonic saline solution for a maximum of 72 hours. The collection, storing, sterilizing, and handling of the samples were done per the Occupational Safety and Health Administration (OSHA) and Centers for Disease Control and Prevention(CDC)guidelines. Out of a total of 200 newly removed mandibular first molars, 120 teethwere finally gathered, sterilized, and kept in 1% thymol in normal saline at 30 degrees Celsius. The access cavity was prepared, and the pulp chamber was cleaned and debridedusing an ultrasonic scaler tip while being irrigated with regular saline. A digital radiograph was taken after a 6# K file was placed to the working length in the mesiobuccal canal.Based on their weights, the samples were dispersed equally across the sixgroups (n=20). They looked inside them to ensure that the root morphology was normal and that the canal was open and free of any abnormalities, damage, or fillings. They looked at the curvature of the mesial root and chose samples with a curvature of 20-35 degrees. The mesial roots were dissected, labeled, and put in a different location. Results Overall, the experimental group'sincidence of buccolingual fractures was 55%, making it the most prevalent fracture type. The mesiodistal type of fracture had a 35% incidence rate, which was the second most prevalent. We found that comminuted and transverse fractures occurred in only 15% and 5% of patients, respectively, of all fractures.Both the test and the control groups had a disproportionately high number of buccolingual fractures. When comparing the root fracture loads of the two experimental groups, there was no significant difference (p>0.05) Conclusion Within this study's restrictions and standardization techniques, it can be concluded that the single file system-prepared roots' resistance to fracture was comparable to that of the control group. It is recommended to conduct additional research on these single file systems using different metrics and to assess them in a clinical setting.

Assessment of Bacterial Sealing Ability of Two Different Bio-Ceramic Sealers in Single-Rooted Teeth Using Single Cone Obturation Technique: An In Vitro Study

The long-term success of root canal treatment requires the prevention of bacterial leakage between canal walls and root canal obturation. In this study, we compared the leakage of Enterococcus faecalis in thirty single-rooted mandibular premolars divided into three groups. The roots in two experimental groups were obturated using a single-cone obturation technique with two different bio-ceramic sealers (MTA Fillapex and CeraSeal) while the roots in the conventional group were obturated using the lateral condensation technique with an epoxy resin sealer (AH Plus). A bacterial leakage test was carried out using a modified microbial leakage model. In the MTA Fillapex group, 30% of the specimens showed a positive sign for bacterial leakage, while 80% of samples in the CeraSeal group leaked. In the AH Plus group, 50% of specimens had bacterial leakage in the canals. The variation of the average number of days between the groups was not found to be significantly different. Taken together, bio-ceramic sealers with single-cone obturation demonstrated comparable results to the resin-based sealer with lateral condensation technique in terms of sealing efficiency against Enterococcus faecalis. Additionally, MTA Fillapex could provide a better sealing ability than CeraSeal bio-ceramic sealer.

Efficacy of Smear Layer Removal at the Apical One-Third of the Root Using Different Protocols of Erbium-Doped Yttrium Aluminium Garnet (Er:YAG) Laser.

Background and Objectives: Smear layer forms during cleaning and shaping can obstruct the entry of both irrigant and sealant into the dentinal tubules, resulting in the accumulation of the bacteria and their byproducts. To ensure effective adhesion and better periapical healing, it is strongly advised to remove the smear layer before proceeding with root canal obturation. This study was designed to compare the efficiency of laser-activated irrigation (LAI) in removing the smear layer and debriding the most apical third of the root canal. Materials and Methods: Sixty-five extracted human teeth with straight single roots were randomly and equally divided into four laser and one control groups. Root canals in all samples were shaped using prime size TruNatomy rotary files. During preparation, each canal was irrigated with 3 mL of 3% NaOCl and 3 mL of 17% EDTA alternately, followed by the irrigation with 10 mL of distilled water to avoid the prolonged effect of EDTA and NaOCl solutions. Final irrigation of 5 mL of 17% EDTA of the root canal was done to eliminate the smear layer and was subsequently activated by an endodontic ultrasonic tip for 20 s three times (control group), a flat-end laser tip (test groups 1 and 3) or a taper-end laser tip (test groups 2 and 4) for two cycles. The time of each cycle activation was 10 s (groups 1 and 2) or 20 s (groups 3 and 4) in which the Er:YAG laser of 2940 nm was used. The laser operating parameters were 15 Hz and 50 μs pulse duration. The samples were then split longitudinally and subjected to scanning electron microscopy (SEM). Results: The remaining smear layer at the apical part of the root canals was statistically significant between the control group and the laser groups 1 (p = 0.040) and 2 (p = 0.000). Within the laser groups, the exposed tubules count was greater in the laser with the flat tip as compared with the tapered tip (Laser 1 > Laser 2 and Laser 3 > Laser 4). Finally, no significant differences in the count of debris between the laser groups and control group were observed, except for laser 4 (p < 0.05), which had the highest count of debris. Conclusion: LAI to remove debris and smear layer at the apical third of the root canal is inferior to the current ultrasonic technique. However, when using the Er:YAG LAI, it is recommended to use a flat tip design for 10 s for two cycles to ensure maximum debridement of the apical dentin surface.

Retreatment of XP-endo Shaper and R-Endo files in curved root canals

ObjectivesTo compare the retreatment efficiency of XP-endo Shaper and R-Endo files in curved root canals using ImageJ software.MethodsForty extracted mandibular first molars with curved mesial canals (25°–35°) were chosen. Access cavities, preparation and obturation of root canals were performed. Roots were randomly distributed into two groups corresponding to the retreatment files used. Group 1 retreated with XP-endo Shaper file, and group 2 retreated with R-Endo files. Each root was sectioned and photographed. ImageJ software was used to estimate the percentage of residual filling material that existed on the three root canal segments. Mann–Whitney U and the Kruskal–Wallis tests were used to compare the percentages of residual filling material between the teeth segments (P < .05).ResultsThe R-Endo group showed a significantly higher median percentage of residual filling material than the XP-endo Shaper group. In both groups, the coronal segments had the highest median of the residual filling material, followed by the middle and apical segments.ConclusionXP-endo Shaper is significantly more effective than R-Endo in removing root canal filling materials in the coronal, middle, and apical segments.

Management of Non-Vital Teeth with open Apices using MTA as an Apical Plug: A Case Report

Dental professionals face a challenge in managing nonvital teeth with an open apex. In this situation, keeping the obturation material inside the boundaries of the root canal without extending into the periapical area is challenging. In order to allow root filling to be compacted, the apexification procedure attempts to close the open root apex with a hard tissue barrier. Therefore, the root canal obturation described in the current case report involved the use of a thermoplasticized gutta-percha after an MTA-made apical plug in an open apex tooth. Assessment of Modality and Accuracy of Single Root Canal Treatment Performed by Undergraduate Students in Saudi Arabia: A Retrospective Study.

Aim To assess the radiographic quality of root canal treatment (RCT) performed on single-rooted anterior teeth by undergraduate dental students of Imam Abdulrahman Bin Faisal University (IAU). Moreover, the study also aimed to assess the types of procedural errors encountered during root canal treatment and to compare the results between male and female students. Methodology The record of patients who visited the endodontic clinics at IAU between the years 2018 and 2021 was obtained from the medical records department. The inclusion criteria for the study involved: i) RCT performed on anterior teeth with a single root; ii) RCT performed by fourth, fifth, and sixth-year undergraduate dental students; and iii) availability of pre-operative and post-operative peri-apical radiographs. After the inclusion criteria, a total of 278 records were selected. The radiographs were accessed by two calibrated examiners for the length of the obturation, homogeneity, and taper. In addition, procedural errors such as the presence of a ledge, perforation, or fractured instruments were also observed. Analysis was done using the Chi-square test. Results A total of 139 teeth (50%) were found to have an adequate quality root canal treatment. Regarding the length of the obturation, 85.6% were considered acceptable, while 65.1% of the obturations had acceptable radiographic homogeneity. The acceptable taper was found in 71.9% of the obturations. Dental students who participated in this study demonstrated a low rate of procedural errors, with 4.7% ledge formation and 1% perforation. A statistically significant difference was found in the quality of root canal obturation (P=<0.001) performed by fourth, fifth, and sixth-year students. Moreover, a significant difference was also observed between maxillary and mandibular teeth (P= 0.032). Conclusion The quality of RCT performed by undergraduate dental students demonstrated that improvements are required. The teaching methods used in the endodontic courses need to be developed and improved to ensure the best possible learning and treatment outcomes.

Root perforating internal replacement resorption: a case report clinical with 10-year follow-up

This study aims to report a root perforating internal replacement resorption treated and followed up for ten years. A 19-years female showed in periapical tomography enlargement of the entire length of the root canal of tooth 11. The cone-beam computed tomography (CBCT) showed a hypodense area with jagged edges involving the pulp cavity space and communication with the periodontium. After being accessed and prepared, the canal and its resorption defect were obturated with mineral trioxide aggregate. The cervical third was sealed with glass ionomer, and the tooth was restored with composite resin. Ten years later, the tomography showed intact root canal filling and periapical and periodontal injury absence. The use of repair cement for root canal obturation with perforating internal replacement resorption is an alternative for these cases, being fundamental to have a clinical, radiographic and tomographic follow-up to evaluate the treatment success.

To achieve high efficiency of endodontic treatment, it is necessary to ensure high-quality cleaning of the root canal. Using only mechanical processing, it is impossible to completely clean the entire root system. For drug treatment of root canals, a wide range of antiseptic agents is used, the most popular of which is a remedy based on sodium hypochlorite solution. For maximum therapeutic effect, the irrigation solution must be in direct contact with the entire surface of the root canal dentin, penetrating into all its branches. Therefore, the creation of an adequate taper, the choice of the optimal method of supplying the agent based on sodium hypochlorite solution affects the quality of drug treatment of the root canal and subsequent obturation. Target. To evaluate the penetration efficiency of the agent based on 3% sodium hypochlorite solution into the macrocanal of the tooth and its branches, depending on the choice of the method of mechanical and medical treatment of root canals in vitro. Materials and research methods. The data obtained as a result of a survey of practicing dentists were statistically processed in Microsoft Excel 6. In the course of the work, root canals were treated with manual and machine tools, followed by washing with a drug based on sodium hypochlorite solution using various methods Results. The highest degree of diffusion of the agent based on sodium hypochlorite solution was established in root canals treated with machine tools using the aspiration method of sanitation. Manual preparation of root canals showed the lowest degree of penetration of the irrigant into the root canal compared to machine processing. Regardless of the method of root canal preparation, it was found that the best degree of penetration of the agent based on sodium hypochlorite solution was observed with the aspiration method of debridement than with the traditional method of antiseptic treatment using an endodontic needle.

Rotary systems in root canal treatment of primary teeth

Introduction: Root therapy in primary teeth aims to remove the necrotic pulp, to prevent premature tooth loss, and to maintain the primary teeth until exfoliation. Rotary systems allow for more effective root canal treatments. Objective: To analyze the literature for the characteristics of the rotary systems such as root canal preparation, obturation time, post-operative pain and clinical success.Methodology: A exhaustive research was performed in PUBMED, to find relevant studies about the use of rotary systems in pediatric dentistry. Results: Rotary files provide a greater root canal preparation, show more conservation of tooth structure with less mistakes in comparison with other techniques. This technique simplifies the procedure, shows less post operative pain, and is reflected in the cooperation of the pediatric patient. Rotary files exhibit a significant cleaning efficacy and a greater bacterial removal that led to a significant clinical success. Conclusion: due to the complexity of the primary roots and how precise the root canal treatment must be. The use of rotary techniques can improve root canal treatment of primary teeth.

Quality of Root Canal Filling in Manually and Rotary-Filed Single-Rooted Teeth

Objective: This study aimed to determine if there was a significant difference in root canal obturation quality between single-rooted teeth produced using a manual approach and those prepared using a rotational method, as determined by intraoral periapical radiographs. Study Design: Randomized control trial Place and Duration: This randomized study was conducted at Bakhtawaramin Medical and Dental College during in the period from April, 2022 to September, 2022. Methods: Total 80 patients of both genders were presented. All the included cases had single rooted teeth. Age, gender, and residence were some of the detailed demographic information that was documented after getting informed written consent. Patients were equally divided in two groups. Group I underwent manual root canal treatment in 40 cases while in group II rotary endodontic preparation. Outcomes among both groups were recorded in terms of quality of root canal filling. SPSS 23.0 was used to analyze all data. Results: There were 45 (56.3%) females and 35 (43.7%) males. Mean age of the patients in group I was 33.3±8.35 years and in group II mean age was 34.9±7.23 years. We did not find any significant difference in root canal filling and homogeneity among both groups. Group II showed a significantly good results of T score as compared to group I with p value &lt;0.005. Conclusion: According to the results of this research, the rotary approach yielded higher quality obturations in terms of taper and overall quality than did the traditional method. Despite this, there was no difference between rotary and manual instrumentation in terms of the radiographic technical quality of the root canal obturation with respect to length and density.
