Preoperative respiratory assessment predicts post-operative survival in stage IA non-small cell lung cancer.

Respiratory impairment can lead to pulmonary complications after surgery; therefore, it should be considered when determining the choice of surgical procedure. Several studies have examined the relationship between preoperative respiratory function and postoperative mortality and morbidity after lung resection; however, there are no indicators for limited surgical procedure selection. The aim of this study was to examine the association between preoperative respiratory function and postoperative early and late complications, recurrence-free survival (RFS), and overall survival (OS) in patients undergoing pulmonary resection for stage I lung cancer. We performed a retrospective analysis of data from 192 patients undergoing pulmonary resection for primary pathological stage IA non-small cell lung cancer (NSCLC) at the Iwakuni Clinical Center in Japan between 2012 and 2015. We reviewed clinicopathological characteristics including preoperative pulmonary function and elucidated the relationship between them and postoperative survival. Obstructive ventilatory impairment was present in 55 patients (28.6%), and restrictive ventilatory impairment was present in 31 patients (16.1%). Seven patients (3.6%) had both ventilatory impairment. Obstructive ventilatory impairment did not affect the 5-year RFS (P=0.08) or OS (P=0.21). However, restrictive ventilatory impairment reduced the 5-year RFS (P=0.002) and OS (P=0.009). The rates of early and late complications were not significantly different based on the preoperative respiratory function. In patients with preoperative restrictive ventilatory impairment in whom lobectomy or segmentectomy cannot be performed, careful consideration is needed for surgical indications.

What constitutes an obstructive ventilatory impairment in a pediatric population? A study design.

There is no clear consensus as to what constitutes an obstructive ventilatory impairment (OVI) in pediatric populations. To determine the percentage of children/adolescents having an OVI among those addressed for spirometry after taking into account the definitions advanced by some international scholarly societies [British Columbia (BC), British thoracic-society (BTS), Canadian thoracic society (CTS), European respiratory society and American thoracic society (ERS-ATS), global initiative for asthma (GINA), Irish college of general practitioners (ICGP), national asthma council (NAC), national institute of clinical excellence (NICE), Société de pneumologie de langue française, Société pédiatrique de pneumologie et allergologie (SPLF-SP2A), and South African thoracic society (SATS)]. This bi-centric cross-sectional study involves two medical structures in Sousse/Tunisia, and will encompass children/adolescents aged 6-18 years. A medical questionnaire will be administered, clinical and anthropometric data will be collected, and the spirometric data will be measured by two spirometers. The following six definitions of OVI will be applied: i) GINA: Forced expiratory volume in 1 second (FEV1) < 80% and a FEV1/forced vital capacity (FVC) ≤ 0.90; ii) ICGP: FEV1/FVC < 0.70; iii) ERS-ATS or BTS or SATS or SPLF-SP2A or NAC: FEV1/FVC z-score < -1.645; iv) NICE: FEV1/FVC < 0.70 or FEV1/FVC z-score < -1.645; v) CTS: FEV1/FVC < 0.80 or a FEV1/FVC z-score < -1.645; and vi) ERS: "FEV1 z-score or FEV1/FVC z-score" < -1.645 or "FEV1 or FEV1/FVC" < 0.80. The percentage of children/adolescents having an OVI will significantly vary between the six definitions. The frequency of OVI in a pediatric population will depend on the definition chosen.

Chronic Lung Allograft Dysfunction, a Review in 2023

Chronic allograft dysfunction (CLAD) is one of the leading causes of death after lung transplantation [1]. CLAD is a progressive and irreversible decline in lung function after transplant, manifested as an obstructive, restrictive, or mixed ventilatory impairment without any identifiable etiology as infection or acute rejection. Multiple risk factors have been associated with this condition. Despite its significant effect on the mortality of transplanted patients, there is still a lack of powerfully effective therapies for patients with CLAD. Avoiding and correcting risk factors and close patient monitoring is critical in preventing disease progression. This article will discuss CLAD, the risk factors for developing the umbrella of syndromes under this term, and the current treatment alternatives and management available up to 2023.

Defining and Grading an Obstructive Ventilatory Impairment: American Thoracic Society/ European Respiratory Society Interpretive Strategies of 2005 Versus 2022

Background: The American Thoracic Society (ATS) and the European Respiratory Society (ERS) have issued several updates to their guidelines for lung function testing between 2005 and 2022.&#x0D; Objective: We aimed to compare ATS/ERS recommendations for 2005(R1) and 2022(R2) in defining Obstructive Ventilatory Impairment (OVI) and in classifying its severity.&#x0D; Patients and Methods: It was a retrospective comparative study including 1129 patients. All patients underwent spirometry with measurement of Forced Expiratory Volume in 1 second (FEV1) and Forced Vital Capacity (FVC). An OVI was considered according to R1 when FEV1/FVC ratio is under the Lower Limit of Normal (LLN) and when the z-score of FEV1/FVC ratio is under -1.645 according to R2. For the severity levels of airflow obstruction: ATS/ERS previously recommended the use of percent predicted FEV1 with 5 levels using cut values of 70%, 60%, 50% and 35%. Recently updated for z-scores with cut values of -2, -2.5 and -4. Mean age was 54.23±19.23 years.&#x0D; Results: For defining an OVI, both definitions were comparable (529 patients with OVI). For the severity classification, the following proportions were assessed: 151 mild, 86 moderate,84 moderately severe, 133 severe and 75 very severe vs 148 mild, 238 moderate, 76 severe and 67 cases having a normal FEV1 (z-score of FEV1 above -1.645), which were classified as mild according to R1. Mild OVI(R2) were distributed according to R1 into 74 mild, 51 moderate, 16 moderately severe and 7 severe. Moderate OVI (R2) were dispatched using R1 to 10 mild, 34 moderate, 66 moderately severe, 103 severe and 25 very severe. Severe OVI(R2) were classified as 1 moderate, 2 moderately severe, 23 severe and 50 very severe.&#x0D; Conclusion: ATS/ERS new and previous recommendations seem to be comparable in defining OVI. However, discrepancies were assessed in classifying its severity.

Intracellular Drug Delivery Process of Am80-Encapsulated Lipid Nanoparticles Aiming for Alveolar Regeneration.

Chronic obstructive pulmonary disease (COPD) results in obstructive ventilatory impairment caused by emphysema, and current treatment is limited to symptomatic therapy or lung transplantation. Therefore, the development of new treatments to repair alveolar destruction is especially urgent. Our previous study revealed that 1.0 mg/kg of synthetic retinoid Am80 had a repair effect on collapsed alveoli in a mouse model of elastase-induced emphysema. From these results, however, the clinical dose calculated in accordance with FDA guidance is estimated to be 5.0 mg/60 kg, and it is desirable to further reduce the dose to allow the formulation of a powder inhaler for clinical application. To efficiently deliver Am80 to the retinoic acid receptor in the cell nucleus, which is the site of action, we focused on SS-cleavable proton-activated lipid-like material O-Phentyl-P4C2COATSOME®SS-OP, hereinafter referred to as "SS-OP"). In this study, we investigated the cellular uptake and intracellular drug delivery process of Am80-encapsulated SS-OP nanoparticles to elucidate the mechanism of Am80 by nanoparticulation. Am80-encapsulated SS-OP nanoparticles were taken up into the cells via ApoE, and then Am80 was efficiently delivered into the nucleus via RARα. These results indicated the usefulness of SS-OP nanoparticles as drug delivery system carriers of Am80 for COPD treatment.

Impact of obstructive ventilatory impairment on intraoperative bleeding during laparoscopic hepatectomy.

An animal model of laparoscopic hepatectomy showed that bleeding from the hepatic vein is influenced by airway pressure. However, there are little research reports on how airway pressure leads to risks in clinical practice. The main objective of this study was to investigate the impact of preoperative forced expiratory volume % in 1s (FEV1.0%) on intraoperative blood loss in laparoscopic hepatectomy. All patients who underwent pure laparoscopic or open hepatectomy from April 2011 to July 2020 were classified into two groups by preoperative spirometry: those with obstructive ventilatory impairment (obstructive group; FEV1.0% < 70%) and those with normal respiratory function (ormal group; FEV1.0% ≥ 70%). Massive blood loss was defined as 400mL for laparoscopic hepatectomy. In total, 247 and 445 patients underwent pure laparoscopic and open hepatectomy, respectively. Regarding laparoscopic hepatectomy group, blood loss was significantly greater in the obstructive group (122 vs. 100mL, P = 0.042). Multivariate analysis revealed that high IWATE criteria which classify the surgical difficulty of laparoscopic hepatectomy (≥ 7, odds ratio (OR): 4.50, P = 0.004) and low preoperative FEV1.0% (< 70%, OR: 2.28, P = 0.043) were independent risk factors for blood loss during laparoscopic hepatectomy. In contrast, FEV1.0% did not affect the blood loss (522 vs. 605mL, P = 0.113) during open hepatectomy. Obstructive ventilatory impairment (low FEV1.0%) may affect the amount of bleeding during laparoscopic hepatectomy.

Sleep-Disordered Breathing and Chronic Respiratory Infections: A Narrative Review in Adult and Pediatric Population

Sleep-disordered breathing (SDB) comprises different diseases characterized by abnormal respiratory patterns during sleep including obstructive sleep apnea. SDB prevalence and impact in patients with chronic respiratory infections have been only marginally studied. The purpose of this narrative review is to report the prevalence and impact of SDB in chronic respiratory infections, including cystic fibrosis (CF), bronchiectasis and mycobacterial infections, and explore the possible pathophysiological mechanisms. Common pathophysiological mechanisms, underlying SDB onset in all chronic respiratory infections, include inflammation, which plays a central role, chronic nocturnal cough and pain, excessive production of mucous plugs, presence of obstructive and/or restrictive ventilatory impairment, upper airways involvement, and comorbidities, such as alteration of nutritional status. SDB may affect about 50% of patients with bronchiectasis. The severity of the disease, e.g., patients colonized with P. aeruginosa and frequent exacerbators, as well as comorbidities, such as chronic obstructive pulmonary disease and primary ciliary dyskinesia, may impact SDB onset. SDB may also frequently complicate the clinical course of both children and adults with CF, impacting the quality of life and disease prognosis, suggesting that their routine assessment should be incorporated into the clinical evaluation of patients from the first stages of the disease regardless of suggestive symptoms, in order to avoid late diagnosis. Finally, although the prevalence of SDB in patients with mycobacterial infections is uncertain, extrapulmonary manifestations, particularly nasopharyngeal locations, and concomitant symptoms, such as body pain and depression, may act as atypical predisposing factors for their development.

Significance of Preoperative Pulmonary Function on Short- and Long-Term Outcomes Following Gastrectomy for Gastric Cancer

BackgroundPreoperative pulmonary function assessment is useful for selecting surgical candidates and operative methods and assessing the risk of postoperative pulmonary complications. However, few studies have investigated the relationship between preoperative pulmonary function and short- and long-term outcomes in patients who underwent gastrectomy for gastric cancer. MethodsOf the 1040 patients with gastric cancer (stages I–III) who had undergone R0 gastrectomy between 2009 and 2020, 750 who underwent preoperative spirometry were included. Restrictive ventilatory impairment was defined as a vital capacity of the predicted value (%VC) < 80%, while obstructive ventilatory impairment was defined as forced expiratory volume in one second (FEV1%) < 70%. Postoperative complications were assessed using the Clavien–Dindo (CD) classification. The relationship between clinical factors, including %VC, FEV1%, severe postoperative complications (CD ≥ 3b), overall survival (OS), and relapse-free survival, were assessed. ResultsThe mean age of the 750 patients was 68 ± 10.5 years. Severe postoperative complications were observed in 25 (3.3%) patients and were significantly associated with FEV1% < 70% in the univariate analysis. The 5-year OS was 72.5%. Multivariate analysis showed that the cancer stage, age > 75 years, preoperative comorbidities, %VC < 80%, total gastrectomy, severe postoperative complications, and postoperative adjuvant chemotherapy were the significant independent factors affecting OS. Pneumonia was significantly associated with %VC < 80%. ConclusionsFEV1% < 70%was associated with the development of severe postoperative complications, while %VC < 80% was associated with poor OS independent of the cancer stage because of death from pneumonia. Spirometry helps surgeons and patients discuss the risks and benefits of surgery.

Spirometry: how to evaluate the results?

Introduction. The European Respiratory Society, the American Thoracic Society, and the Russian Respiratory Society are currently working on updating the technical standards and interpretive strategies for routine lung function tests. There is a search for recommendations for the best choice of reference values, the optimal limits of normal and severity grading of detected impairments. An important step in this work is the standartization of pulmonary function tests interpretation, including spirometry. Aim. Review of existing concepts for the spirometry interpretation, according to new approaches to their quantitative and qualitative assessment. Materials and methods. The scientific publications on the PubMed and eLIBRARY.RU platforms were analyzed. The materials posted on the official websites of the European Respiratory Society, the Russian Respiratory Society, the American Thoracic Society and the Global Lung Function Initiativewere also used. Results. A brief overview of the main reference values for spirometry is presented: the European Coal and Steel Community (1993), Global Lung Function Initiative (2012, 2021), R.F.Klement et al. (1986, 1991). The issues of defining the lower limit of the normal, diagnosing ventilatory impairments and assessing the severity of lung function reduction using the z-score and a percentage of the predicted value when analyzing the results of spirometry are considered. Conclusion. The type of ventilatory impairment and severity should be presented in the spirometry interpretation just like the vital capacity assessment. The obstructive ventilatory impairment is generally diagnosed by spirometry, if the signs of restrictive or mixed patterns are present, it is recommended to determine the total lung capacity by body plethysmographymethod. Assessment of the limits of normal and the severity levels for any spirometry indices should be carried out using the z-score values.

Clinical implications of opioid-induced ventilatory impairment.

Opioid-induced ventilatory impairment is the primary mechanism of harm from opioid use. Opioids suppress the activity of the central respiratory centres and are sedating, leading to impairment of alveolar ventilation.Respiratory physiological changes induced with acute opioid use include depression of the hypercapnic ventilatory response and hypoxic ventilatory response. In chronic opioid use a compensatory increase in hypoxic ventilatory response maintains ventilation and contributes to the onset of sleep-disordered breathing patterns of central sleep apnoea and ataxic breathing. Supplemental oxygen use in those at risk of opioid-induced ventilatory impairment requires careful consideration by the clinician to prevent failure to detect hypoventilation, if oximetry is being relied on, and the overriding of hypoxic ventilatory drive. Obstructive sleep apnoea and opioid-induced ventilatory impairment are frequently associated, with this interrelationship being complex and often unpredictable. Monitoring the patient for opioid-induced ventilatory impairment poses challenges in the areas of reliability, avoidance of alarm fatigue, cost, and personnel demands. Many situations remain in which patients cannot be provided effective analgesia without opioids, and for these the clinician requires a comprehensive knowledge of opioid-induced ventilatory impairment.

Vascular involvement in chronic thromboembolic pulmonary hypertension is associated with spirometry obstructive impairment

BackgroundChronic thromboembolic pulmonary hypertension (CTEPH) is a type of pulmonary hypertension caused by persistent thromboembolism of the pulmonary arteries. In clinical practice, CTEPH patients often show obstructive ventilatory impairment, even in the absence of a smoking history. Recent reports imply a tendency for CTEPH patients to have a lower FEV1.0; however, the mechanism underlying obstructive impairment remains unknown.MethodsWe retrospectively analyzed CTEPH patients who underwent a pulmonary function test and respiratory impedance test to evaluate their exertional dyspnea during admission for right heart catheterization from January 2000 to December 2019. We excluded patients with a smoking history to rule out the effect of smoking on obstructive impairment.ResultsA total of 135 CTEPH patients were analyzed. The median FEV1.0/FVC was 76.0%, %FEV 1.0 had a negative correlation with the mean pulmonary artery pressure and pulmonary vascular resistance and the CT Angiogram (CTA) obstruction score. A multivariate regression analysis revealed that the CTA obstruction score was an independent factor of a lower %FEV1.0. In the 54 patients who underwent pulmonary endarterectomy, %FEV1.0 was improved in some cases and was not in some. Mean PAP largely decreased after PEA in the better %FEV1.0 improved cases, suggesting that vascular involvement in CTEPH could be associated with spirometry obstructive impairment.Conclusion%FEV1.0 had a significant correlation with the CTA obstruction score. Obstructive impairment might have an etiological relationship with vascular involvement. Further investigations could shed new light on the etiology of CTEPH.

Lung function assessment and its association with blood chromium in a chromate exposed population

Hexavalent chromium [Cr(VI)] and its compounds have been associated with various respiratory diseases, while few studies have attempted to determine its adverse effect on lung function. To explore the potential early indicators of health surveillance for respiratory diseases induced by chromate exposure, a longitudinal cohort study including 515 workers with 918 measurements across 2010–2017 was conducted to investigate the impact of individual internal exposure on lung function. Inductively coupled plasma mass spectrometry (ICP-MS) and spirometry were used to measure whole blood chromium (blood Cr) and lung function respectively. In the linear mixed-effects analysis, each 1- unit increase in Ln- transformed blood Cr was significantly associated with estimated effect percentage decreases of 1.80 (0.35, 3.15) % in FEV1, 0.77 (0.10, 1.43) % in FEV1/FVC, 2.78 (0.55, 4.98) % in PEF, and 2.73 (0.59, 4.71) % in FEF25–75% after adjusting for related covariates. Exposure- response curve depicted the reduction of lung function with blood Cr increase, and the reference value of blood Cr was proposed as 6 μg/L considering the lung function as health outcome. Based on the repeated-measure analysis, compared with the low frequency group, subjects with high frequency of high exposure across 2010–2017 had an additional reduction of 5.65 (0, 11.3) % in FVC. Subjects with medium frequency showed more obvious declines of 9.48 (4.16, 14.87) % in FVC, 8.63 (3.49, 13.97) % in FEV1, 12.94 (3.34, 22.53) % in PEF and 10.97 (3.63, 18.30) % in MVV. These findings suggested that short- term high exposure to Cr associated with obstructive ventilatory impairment, and long- term exposure further led to restrictive ventilatory impairment.

Lung Function Impairment in Construction Workers – Influence of Smoking and Exposure Duration

AIM: The objective of the study was to assess the influence of exposure duration and smoking on ventilatory impairment among construction workers. METHODS: A cross-sectional study was performed, including 83 construction workers aged 18–64 years, compared to equivalent number of office controls matched by age, workplace exposure duration, and smoking status. Data on chronic respiratory symptoms, work history, and smoking status were collected by standardized questionnaire, while lung functional testing of the examined subjects was performed by spirometry. RESULTS: Mean values of spirometric parameters were lower in construction workers compared to controls with statistical significance registered for maximal expiratory flow (MEF25), MEF50, and MEF75. Lung functions of construction workers have been found to decrease in relation to exposure duration but reached significance only for small airways changes. There was a significant difference in detected ventilatory impairment between exposed workers and controls for any type of ventilatory impairment, as well as obstructive and combined ventilatory pattern and obstructive ventilatory pattern in small airways. Obstructive ventilatory impairment was significantly associated with life-time smoking in construction workers, while obstructive ventilatory pattern in small airways was significantly associated with life-time smoking. The combined effect of daily smoking, life-time smoking, and number of cigarettes smoked daily was shown to have a significant influence in their development. The risk for obstructive ventilatory pattern in small airways among exposed subjects was about 4 fold higher in those exposed more than 20 years (odds ratio [OR] = 3.68 [1.01–14.59] confidence interval [CI] 95%), and about 2.5 fold higher in smokers (OR = 2.57 [0.92-7.25] CI 95%). Exposure duration, smoking and age had independent effect only on small airways changes and force expiratory volume in the 1st s/force vital capacity %. CONCLUSION: Our data suggest the importance of the joint effect of job exposure in construction and daily smoking on the development of lung function impairment and airflow limitation, being dominant, especially on small airways.

Efficacy of preoperative inhaled LAMA/LABA combinations in patients with lung cancer and untreated COPD.

Chronic obstructive pulmonary disease (COPD) is associated with a high incidence of postoperative pulmonary complications (PPCs). When untreated COPD is found before lung cancer surgery, we have been actively intervening therapeutically with inhaled long-acting muscarinic antagonist (LAMA)/long-acting β2-agonist (LABA) combinations. We investigated the efficacy of preoperative LAMA/LABA treatment. We reviewed data from 261 patients who underwent pulmonary resection for primary lung cancer. Of these, 59 patients showed unrecognized obstructive ventilatory impairment on respiratory function testing. We administered inhaled drugs for 38 patients, of whom 22 patients treated with LAMA/LABA combinations and diagnosed with COPD were retrospectively analyzed regarding improvement of respiratory function and postoperative course. Median duration of LAMA/LABA treatment was 19.5 days (interquartile range (IQR), 10.5-28.3 days). Percentage predicted vital capacity (%VC) (pretreatment: 95.6%, IQR 91.9-111.7 vs. posttreatment 102.8%, IQR 92.3-113.0), forced expiratory volume in 1 s (FEV1) (1.76 L, 1.43-2.12 vs. 2.00 L, 1.78-2.40), forced VC (FVC) (2.96 L, 2.64-3.47 vs. 3.22 L, 2.95-3.74) and percentage predicted FEV1 (80.1%, 68.4-97.0 vs. 91.6%, 80.3-101.9) were all significantly improved (P<0.05 each). FEV1/FVC tended to be improved, but not significantly. No significant difference in improvement of respiratory function was seen between short-term (≤2 weeks) and normal-term (>2 weeks) treatment. PPCs occurred in 4 of 22 patients (18.2%), showing no significant difference compared to patients with COPD previously treated with inhaled drugs (2/20; 10.0%). Respiratory function is improved by preoperative LAMA/LABA treatment even in the short term. Starting treatment allows even COPD patients diagnosed on preoperative screening to experience the same frequency of PPCs as previously treated patients.

Evaluation of intrathoracic tracheal narrowing in patients with obstructive ventilatory impairment using dynamic chest radiography: A preliminary study

PurposeDynamic chest radiography (DCR) can observe the dynamic structure of the chest using continuous pulse fluoroscopy irradiation. However, its usefulness remains largely undetermined. The purpose of this study was to examine the relationship between changes in tracheal diameter during deep breathing and obstructive ventilation disorders using DCR. MethodTwelve participants with obstructive ventilatory impairment and 28 with normal pulmonary function underwent DCR during one cycle of deep inspiration and expiration. Three evaluators blinded to pulmonary function test results independently measured lateral diameters of the trachea in DCR images to determine whether there was a difference in the amount of change in tracheal diameter depending on the presence or absence of pulmonary dysfunction. Tracheal narrowing was defined as a decrease in the lateral tracheal diameter of more than 30 %. Participants were divided into a narrowing group and a non-narrowing group, and it was examined whether each group correlated with values of pulmonary function tests. ResultsTracheal diameter was significantly narrowed in subjects with obstructive ventilatory impairment compared to normal subjects (P < 0.01). When subjects were divided into narrowing (tracheal narrowing rate [TNr] = 41.5 ± 7.7 %, n = 9) and non-narrowing groups (TNr = 9.1 ± 7.0 %, n = 31, p < 0.01), FEV1%-G, and %V25 were significantly smaller in the narrowing group than in the non-narrowing group (p < 0.01). ConclusionsChanges in tracheal diameter during deep breathing were easily evaluated using DCR. DCR may, therefore, be useful for evaluating obstructive ventilation disorders.

Identification of a Mutation in the Novel Compound Heterozygous CFTR in a Chinese Family with Cystic Fibrosis.

Cystic fibrosis (CF) is one of the most common autosomal recessive disorders among Caucasians of Northern European descent but is uncommon in the Chinese population. Objectives. To elucidate the mutation in the novel compound heterozygous CFTR causing CF in Chinese family. Materials and Methods. Clinical samples were obtained from a Chinese family, the brother and sister with recurrent airway infections, hypoxemia and obstructive ventilatory impairment, sinusitis, clubbed fingers, salty sweat, and nasal polyposis. We performed whole-exome sequencing on the family and validated all potential variants by Sanger sequencing. Results. Next-generation sequencing showed a novel compound heterozygous CFTR mutation (c.400 A > G p.Arg134Gly and c.3484 C > T p.Arg1162∗) which resulted in CF in the family. Conclusions. As this mutation is consistent with the observed clinical manifestations of CF and no other mutations were detected after scanning the gene sequence, we suggest that their CF phenotypes are caused by the compound heterozygous mutation, c.400 A > G p.Arg134Gly and c.3484 C > T p.Arg1162∗. As c.400 A > G is not currently listed in the Cystic Fibrosis Mutation Database, this information, regarding the CF-causing mutations in two Chinese patients, is of interest.

Smoking in People Living with HIV (PLHA) and followed up in the Outpatient Department of the Hospital Fann of Dakar

IntroductionLe tabagisme doublerait la mortalité chez les PvVIH un risque accru de survenue de maladies non classant Sida. La prévalence de la consommation de tabac est plus élevée chez les PvVIH que dans la population générale. Nous nous sommes fixés comme objectifs d'évaluer la prévalence du tabagisme chez les PvVIH, de décrire les caractéristiques cliniques et spirométriques des fumeurs et ex-fumeurs et d'évaluer leurs connaissances et attitudes face au tabagisme.MéthodesNous avons donc mené une étude transversale, descriptive et analytique du 15 Juillet au 15 Décembre 2015, chez les PvVIH suivis au Centre de Traitement Ambulatoire du Centre Hospitalier Universitaire National de Fann.RésultatsLa population d'étude concernait trois cents (300) PvVIH. Le sex-ratio était de 0,8. Nous avons retrouvé 15% de fumeurs et 23,7% d'ex-fumeurs. L'âge moyen des fumeurs était de 44,38 ± 9,55 ans. La quasi-totalité des fumeurs 91,1% avait déjà commencé à fumer avant la connaissance de leur statut sérologique et 35,6% d'entre eux avaient majoré leur consommation de tabac après. Les signes respiratoires étaient dominés par la gêne respiratoire dans 64,4% des cas chez nos fumeurs. Parmi les fumeurs qui ont bénéficié de la spirométrie, 67% d'entre eux avait un trouble ventilatoire obstructif non amélioré par les béta2-mimétiques et 28,1% un syndrome restrictif. Parmi les ex-fumeurs, 40,8% affirmait que le statut sérologique était le motif de sevrage.ConclusionLe tabagisme peut être initié ou majoré après connaissance du statut sérologique. Il peut être à l'origine de nombreuses complications chez le PvVIH.

SERUM CLARA CELL PROTEIN (CC16) IN RELATION TO PULMONARY FUNCTIONS AMONG WORKERS EXPOSED TO NAPHTHALENE IN PVC MANUFACTURE

AbstractIntroduction: Naphthalene is a harmful environmental tox reduced in serum and/or airway lining fluid of patients with COPD, asthma, bronchopulmonary dysplasia, silicosis, and post-transplant rejection. Aim of work: This study was done to evaluate the use of serum CC16 level as a biological marker for occupational respiratory diseases in relation to pulmonary function tests among workers exposed to naphthalene in polyvinyl chloride (PVC) manufacture. Material and methods: The study included 33 exposed workers and 33 control subjects. The study groups were subjected to full history taking, clinical examination and pulmonary function testing. Serum levels of CC16 and naphthalene levels measured for both groups. Results: Pulmonary functions parameters (FVC, FEV1, FEV1/FVC, FEF 25%, FEF 50%, FEF 75%), were significantly decreased in the exposed group compared to the control and indicated obstructive ventilatory impairment among the exposed workers. Serum naphthalene was significantly higher, while serum CC16 was significantly lower among the exposed group compared to the control. Exposed smokers exhibited the highest serum naphthalene and the lowest serum CC16 among all study groups. The duration of naphthalene exposure positively correlated with the serum level of naphthalene and negatively correlated with pulmonary functions and the serum level of CC16. Also serum level of CC16 correlated negatively with the serum naphthalene level and smoking index.Conclusion: Occupational exposure to naphthalene in PVC manufacture is associated with obstructive ventilatory impairment, with corresponding decrease in the serumlevel of Clara cell protein (CC16), which can be used as a biomarker for respiratory system affection among workers exposed to naphthalene.

The utility of hand-held mobile spirometer technology in a resource-constrained setting.

Mobile phone-linked spirometry technology has been designed specifically for evaluating lung function at primary care level. The Air-Smart Spirometer is the first mobile spirometer accepted in Europe for the screening of patients with chronic respiratory diseases. To prospectively assess the accuracy of the device in measuring forced expiratory volume in the first second (FEV1) and forced vital capacity (FVC) in a South African population, and to investigate the ability of the device to detect obstructive ventilatory impairment. A total of 200 participants were randomly assigned to perform spirometry with either the mobile spirometer connected to a smartphone or the desktop spirometer first, followed by the other. The FEV1/FVC ratio as well as the absolute FEV1 and FVC measurements were compared, using each participant as their own control. A Pearson correlation and Bland-Altman analysis were performed to measure the agreement between the two devices. We defined obstructive ventilatory impairment as FEV1/FVC &lt;0.7 measured by desktop spirometry in order to calculate the sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) of the Air-Smart Spirometer. There was a strong correlation between the absolute FEV1 and FVC values and FEV1/FVC ratio measured with the mobile Air-Smart Spirometer and more conventional pulmonary function testing, with r=0.951, r=0.955 and r=0.898, respectively. The Air-Smart Spirometer had a sensitivity of 97.6%, specificity of 74.4%, PPV of 73.0% and NPV of 97.8% for obstructive ventilatory impairment. The mobile Air-Smart Spirometer compared well with conventional spirometry, making it an attractive and potentially affordable tool for screening purposes in a primary care setting. Moreover, it had a high sensitivity and NPV for obstructive ventilatory impairment.
