A Case of Metastatic Non-small Cell Lung Cancer Presenting with Numb Chin Syndrome

Numb chin syndrome is a rare clinical manifestation, characterized by focal sensory loss and paresthesia of the chin. It is more often associated with cancer than with benign disorders, and can be the first manifestation of a cancer. A 60-year-old man presented with focal numbness of right chin and gingiva for 10 days. Chest computed tomograghy showed a 3 cm sized mass on the distal left main- stem bronchus. Squamous cell carcinoma was diagnosed on bronchoscopic biopsy. However, bony metastasis of mandible was not evident on reontgenogram, CT scan, bone scintigram and positron emission tomography. Despite the chemotherapy with three cycles of paclitaxel and cisplatinum, the cancer was progressed and pain on the right chin was developed 4 months later. Bone scintigram showed multiple bony metastasis including mandible. Here we report this case with a brief review of the appropriate literature.

Numb chin syndrome as the presenting symptom of metastatic prostate carcinoma

We report a case of the numb chin syndrome as the presenting symptom in a patient with metastatic prostate carcinoma. The numb chin syndrome is characterized by facial numbness along the distribution of the mental branch of the trigeminal nerve. Most cases of this syndrome that are not dental in origin have been associated with diffuse metastatic disease, particularly with underlying lymphoproliferative and breast cancer. Although axial and vertebral bone metastases are common in patients with carcinoma of the prostate, mental nerve involvement is rare. We present a case of the numb chin syndrome as the initial clinical manifestation in a patient with metastatic prostate adenocarcinoma.

Numb chin syndrome as an initial symptom of acute lymphocytic leukemia: Report of three cases

This article describes three cases of acute lymphocytic leukemia that presented with mental neuropathy, or so-called “numb chin syndrome,” as the initial symptom of the disease. This symptom heralded the initial progression of the disease in the first and second cases and the recurrence of the disease in the third case. In these cases tenderness in the mental foramen, percussion pain of the teeth, loosening and extrusion of the teeth, and radiographic abnormalities were also, if not always, observed in association with mental neuropathy. The radiographic abnormalities included a disappearance of the mandibular canals, an enlarged periodontal ligament space, a loss or thinning of the lamina dura, and a destruction of the alveolar crestal bone. This report indicates that oral manifestations can therefore occasionally play an extremely important role in the early recognition of acute lymphocytic leukemia. The unexplained oral abnormalities such as numbness of the chin and lower lip must thus be considered a potentially ominous indication of acute lymphocytic leukemia.

Metastatic large cell lung cancer presenting with numb chin syndrome

The numb chin syndrome is an uncommon occurrence with cancer and consists of numbness in the area of distribution of the mental nerve. Most cases of this syndrome ocur in the presence of underlying breast cancer or lymphoma with survival measured in months once this syndrome is manifest. Reported below is the case of a man with metastatic large cell lung cancer whose presenting complaint was chin and lip numbness.

F acial numbness localized to the distribution of the mental nervemental nerve neuritis or mental nerve neuropathy-has been labelled the numb chin syndrome. The inferior alveolar nerve may also be involved. This uncommon neuropathy is almost always unilateral, with anesthesia or hypoesthesia of the chin and lower lip found on physical examination. When not associated with iatrogenic local anesthesia, the condition is most cornmonly due to dental diseases. In the absence of these causes. the condition is clinically significant because of its frequent association with malignant diseases [1-4). In some cases the numb chin syndrome may even be the presenting symptom of an unsuspected malignancy: more frequently it heralds a relapse in a patient with a known neoplasm [1]. In such cases this finding, despite its seemingly clinically benign symptoms, is ominous because it usually implies a grave prognosis [2, 4]. Although painless mental nerve neuropathy may be the only symptom of distant malignant disease, mandibular metastases may be accompanied by paresthesia, pain, local swelling, and bone destruction.

Numb chin syndrome in cancer patients: etiology, response to treatment, and prognostic significance.

We retrospectively evaluated 42 consecutive cancer patients with numb chin syndrome (NCS). Breast cancer comprised 64% of the primary tumors, and lymphoproliferative neoplasms comprised 14%. A standard workup (including imaging of the brain, base of skull, and mandible, and CSF analysis) led to the diagnosis of a metastatic etiology in 89% of the patients. Fifty percent of the patients had mandibular metastases, 14% base-of-skull bone lesions, and 22% leptomeningeal seeding. NCS was a late manifestation of malignancy, associated with disease progression in 67% of the patients or heralding a relapse, which was often confined to the leptomeninges, in 31%. Although various therapeutic strategies led to resolution of NCS, median survival after its diagnosis was 5 months when due to bone metastases and 12 months if associated with leptomeningeal seeding.

We describe a case of Burkitt's cell acute lymphoblastic leukemia presenting with the bilateral numb chin syndrome as the initial symptom of the disease. Postmortem study of the trigeminal nerve showed heavy infiltrations of leukemic cells and destruction of axon and myelin by leukemic cells in the mandibular nerve.
