Factors Affecting Treatment Responses to Interferon- in Chronic Hepatitis C

Parameters have been studied to predict responses to interferon (IFN) therapy for chronic hepatitis C, but the definition of a response, the times at which responses were assessed, and the pretreatment parameters considered differ markedly from study to study. Thus, 113 patients with chronic hepatitis C were treated 3-6 months with 3 MU of IFN-alpha 2a three times a week and assessed for pretreatment parameters predictive of responses to IFN. In a multivariate analysis, a biochemical response (normal aminotransferase activity) at the end of treatment was significantly associated with low body weight, normal gamma-glutamyl transpeptidase activity, and a pretreatment hepatitis C virus (HCV) genotype other than 1. Six months after the end of treatment, a low virus burden and a lack of anti-HCV IgM core antibodies were independently associated with sustained virologic response (i.e., normal aminotransferase activity and HCV RNA negativity). Therefore, these pretreatment parameters should be taken into account when individual treatment protocols are designed.

Delta 4-3-oxosteroid 5 beta-reductase deficiency: failure of ursodeoxycholic acid treatment and response to chenodeoxycholic acid plus cholic acid.

In some infants with liver disease, 3-oxo-delta 4 bile acids are the major bile acids in urine, a phenomenon attributed to reduced activity of the delta 4-3-oxosteroid 5 beta-reductase required for synthesis of chenodeoxycholic acid and cholic acid. These patients form a heterogeneous group. Many have a known cause of hepatic dysfunction and plasma concentrations of chenodeoxycholic acid and cholic acid that are actually greater than those of the 3-oxo-delta 4 bile acids. It is unlikely that these patients have a primary genetic deficiency of the 5 beta-reductase enzyme. To document the bile acid profile, clinical phenotype, and response to treatment of an infant with cholestasis, increased plasma concentrations of 3-oxo-delta 4 bile acids, low plasma concentrations of chenodeoxycholic acid and cholic acid, and no other identifiable cause of liver disease. This infant was compared with normal infants and infants with cholestasis of known cause. Analysis of bile acids by liquid secondary ionisation mass spectrometry and gas chromatography-mass spectrometry. The plasma bile acid profile of the patient was unique. She had chronic cholestatic liver disease associated with malabsorption of vitamins D and E and a normal gamma-glutamyltranspeptidase when the transaminases were increased. The liver disease failed to improve with ursodeoxycholic acid but responded to a combination of chenodeoxycholic acid and cholic acid. Treatment of primary 5 beta-reductase deficiency requires the use of bile acids that inhibit cholesterol 7 alpha-hydroxylase.

Serum gamma-glutamyl transpeptidase activity in patients receiving chronic phenytoin therapy.

To better define the incidence and range of elevated gamma-glutamyl transpeptidase (GGT) activity in patients taking a drug known to induce hepatic microsomal enzymes, GGT was measured in the sera of 58 patients before and after six months of phenytoin therapy. Enzyme activity at six months was greater than baseline in 52 of 58 patients (90%). GGT activity (normal, 0-50 units/liter) was 45.2 +/- 9.9 (mean +/- SEM) at baseline and 135.8 +/- 18.1 after six months of therapy, a mean threefold increase (P less than 0.001). Of the 45 patients with normal baseline GGT activity, some had marked elevation of serum GGT activity with values rising over 200 units/liter in eight patients and over 300 units/liter in four patients. Mean serum GGT activity remained significantly elevated at 12 and 24 months. This elevation in GGT activity was not influenced by age, sex, or additional anticonvulsant drug therapy. Both baseline and six-month GGT activity was lowest in patients who drank no alcohol, higher in patients who drank 0-1 pint/week, and greatest in patients who drank greater than 1 pint/week. All 13 patients with elevated baseline GGT activity were regular users of alcohol and/or taking other enzyme-inducing drugs. In conclusion, increase in serum GGT activity occurs in 90% of patients on long-term phenytoin therapy, most often to moderate but occasionally to high levels, and this rise in GGT activity is accentuated by regular consumption of alcohol.

Ability of Calcium bis-Acetyl Homotaurine, a GABA Agonist, to Prevent Relapse in Weaned Alcoholics

After they had been weaned off alcohol in hospital 85 severe alcoholics (above 200 g alcohol/day) were included in a double-blind study of calcium bis acetyl homotaurine (Ca AOTA, 25 mg/kg/day), a new gamma-aminobutyric acid agonist, versus placebo. Patients were treated as outpatients during the 3-month study. The only other treatment that patients received was meprobamate, 800 to 1200 mg/day, in the first month. The criterion for success was abstinence at 3 months (with normal gamma-glutamyl transpeptidase being one of the criteria). Of the 70 patients who completed the study, 33 received Ca AOTA and 37 placebo. 20 patients on Ca AOTA did not relapse, compared with 12 on placebo (p less than 0.02 by X2 test). Side-effects were noted by 7 patients on Ca AOTA and 2 on placebo. The results suggest that Ca AOTA may be useful in helping severe alcoholics who have been weaned off alcohol not to relapse.

Microvillar enzymes in amniotic fluid: Considerations for the prenatal diagnosis of cystic fibrosis

The activity of gamma-glutamyl transpeptidase (GGT) and the proportion of alkaline phosphatase (ALP) activity that remains in cell-free amniotic fluid (AF) after inhibition by amino acids ("residual ALP activity") have been evaluated as possible prenatal diagnostic tests for cystic fibrosis. A total of 1511 AFs were examined. In 1435 "reference" AFs (excluding those from pregnancies with a known fetal abnormality and those with a known one in four risk of cystic fibrosis) at 14-24 weeks' gestation, the mean residual ALP activity in the presence of 2.5 mmol/L L-phenylalanine was 32% (median, 28%) and in 10.0 mmol/L L-homoarginine it was 70% (median, 72%). AFs were arbitrarily considered to be "abnormal" if the residual activity was greater than 50% in L-phenylalanine and less than 50% in L-homoarginine. An abnormal residual ALP activity pattern was found in nine of 27 pregnancies which resulted in a neural tube (or other developmental) defect and in five of the 10 pregnancies with a chromosomal abnormality; a further 23 (1.6%) abnormal patterns occurred in the 1435 reference AFs from pregnancies that were believed to have had a normal outcome. However, of an additional 23 AFs that were sampled at the 25th week of pregnancy or later, 13 had an abnormal residual activity pattern; the outcome at term in each case was normal. The residual activity of ALP in the presence of either inhibitor did not change with increasing gestational age. However, the absolute amount of ALP that was inhibited by L-phenylalanine (the so-called "phe-inhibitable activity") was greatest at 18 weeks. GGT activity decreased with increasing length of gestation. Of the AF samples from 16 pregnancies at one in four risk for cystic fibrosis that were obtained at 18 weeks' gestation, 11 AFs had a normal residual ALP activity pattern, normal GGT and normal phe-inhibitable ALP activity. Of these 11, five have come to term and the infants are not affected by cystic fibrosis. Three of the 16 AFs had an abnormal residual ALP activity pattern, low GGT and low phe-inhibitable ALP activity; these pregnancies were assessed to be affected by cystic fibrosis and termination was chosen in each case. The two remaining AFs had borderline, paradoxical GGT and residual ALP activity, and initially could not be classified clearly into either of the two groups; however, phe-inhibitable ALP activity was low in each. These pregnancies are continuing.
