Benzodiazepine dependent patients and their psychological treatment

1. 1. 91 patients were referred to a tranquillizer withdrawal clinic. 44 of these entered a withdrawal programme. The characteristics of the patients are described. 2. 2. 72% of patients accepted for benzodiazepine withdrawal had a history of previous psychiatric contact. They also had significantly higher scores on S.T.A.I. than control groups of non-psychotic psychiatric out-patients indicating a considerable psychiatric morbidity prior to withdrawal. 3. 3. 12 patients were treated with psychological group therapy using anxiety management techniques. The outcome of this pilot study showed that 50% of subjects were able to discontinue their benzodiazepines despite previous failures. 4. 4. Patients found learning to cope with symptoms, sharing problems with others and learning to change thoughts the most useful components of the anxiety management package during withdrawal.

Comparative Outcome in Time-Limited Individual and Group Psychotherapy

ABSTRACTIn this randomized clinical trial, we compared time-limited group psychotherapy and time-limited individual psychotherapy. Ninety-eight nonpsychotic psychiatric outpatients participated. There were five therapists, all highly experienced with brief therapies. Significant improvement and maintenance of improvement occurred in both treatments. Although both treatments were quite beneficial on a number of our subjective measures, there was a clear preference by patients for the individual therapy. Recommendations are offered for improving the acceptability of group treatment approaches.

Hypochondriacal beliefs and attitudes in family practice and psychiatric patients.

Beliefs and attitudes which can be responsible for hypochondriacal behavior were explored by administering the Illness Attitude Scales and two distress scales to patients attending a family practice clinic, nonpsychotic psychiatric outpatients and a random group of employees. Family practice patients were more distressed, had more hypochondriacal concerns and had more bodily preoccupations than employees and took more precautions about their health. Psychiatric patients were more distressed and had more fears about illness and death than family practice patients, yet took fewer precautions about their health. The findings appear to have implications for treatment.

Social networks, attachments and support in minor affective disorders: a replication.

Significant deficiencies in personal social networks and social support in non-psychotic psychiatric out-patients have been reported by Henderson ad his colleagues. In a replication study, 50 non-psychotic psychiatric out-patients were compared with 50 matched controls. Patients reported spending less time in social interaction, but more time in unpleasant interaction during the previous week; they also had fewer attachment figures, close relatives and good friends and fewer social contacts than controls. However, the separate analysis of Neurotic and Retarded depressives (using the Present State Examination) failed to replicate all these findings in the Retarded depressives. It is concluded that these associations are confirmed in neurotic patients, but that the more severe depressive disorders require further investigation.
