The article presents the recommendations for managing patients with rhinitis in Slovenia. It covers the treatment of allergic and non-allergic rhinitis, as well as rhinosinusitis with or without nasal polyps. It also addresses allergy management, the use of allergen immunotherapy, and its effect on symptom reduction. The national recommendations are based on international guidelines and offer a comprehensive overview of pharmacological and non-pharmacological treatment options for these conditions. The article emphasizes the importance of clinical decision-making, patient and healthcare provider collaboration at all levels, and regular monitoring. The recommendations aim to support professionals at all levels in providing evidence-based care for patients with rhinitis and related conditions.

A Specialised Antenatal Music Therapy (SAMT) programme for pregnant women with mental health conditions: A mixed-methods proof-of-concept study in Ireland

ABSTRACT Introduction Perinatal mental health has a long-term impact on maternal and infant well-being. Non-pharmacological treatment options are needed for pregnant women and emerging evidence has shown the potential of music therapy to improve perinatal mental health and antenatal attachment. However, research is lacking on individual music therapy for pregnant women with mental health conditions and formal music therapy services are currently absent within perinatal mental health care in Ireland. This mixed-methods, proof-of-concept study aimed to assess the feasibility and preliminary evidence on the effectiveness of a specialised antenatal music therapy (SAMT) programme for pregnant women attending the Specialist Perinatal Mental Health Service (SPMHS). Method Pregnant women between 25 and 33 weeks gestation with various mental health conditions were recruited through the SPMHS to receive six in-person or online sessions of SAMT. Feasibility outcomes included retention, adherence, and fidelity.\\ Assessment of participants’ wellbeing, anxiety, and antenatal attachment were conducted before and after SAMT and each session. Semi-structured interviews were conducted and analysed using thematic analysis. Results Twelve pregnant women were included and nine completed SAMT and assessments, demonstrating a 75% retention rate. High levels of adherence and fidelity were reported. Significant improvements were observed in participants’ wellbeing, anxiety levels, and antenatal attachment. Qualitative findings highlighted the therapeutic benefits, the accessibility and challenges of the online setting, distinctive therapeutic experiences, and participants’ recommendations. Discussion The findings suggest implementing SAMT within the Irish SPMHS is feasible and beneficial. The study offers insights into the therapeutic mechanisms of SAMT and supports the progression to a larger trial.

Nonpharmacologic Treatment Options in Pediatric Tension-Type Headache Patients: A Systematic Review.

Objective: This systematic review aims to examine effects of nonpharmacologic treatment options for pediatric tension-type headache patients as well as evaluate current results from these treatment options. Background: Headache is a worldwide problem among both adults and children, with tension-type headache among the most common. Methods: Eighteen relevant studies were found using PubMed and evaluated in this review. These include therapy programs, biofeedback, alternative medicine, strength exercise, physical therapy, mindfulness therapy, and psychotherapy. Results: Several nonpharmacologic treatment methods showed improvement in pediatric headache patients, with few reported adverse effects. This suggests that nonpharmacologic treatment is safe for use and might have an effect in most cases. Conclusion: No single intervention has been proven superior to another, and some studies would need replication with a control group to confirm the findings. This study illustrates the variety of nonpharmacologic treatment options and the importance of future research on this topic.

Biofeedback therapy using Cerebri for the prevention of migraine attacks in adults with episodic migraine (BioCer): a randomized, wait-list controlled trial – the study protocol

Introduction Biofeedback is a non-pharmacological treatment option valued for its minimal risk of adverse events and offers a safe alternative for individuals seeking preventive care for migraine. Despite level A evidence for migraine prevention, biofeedback treatment is still unavailable to most patients. We developed a novel medical device (Cerebri) for multimodal biofeedback treatment that omits the need for healthcare personnel involvement. Cerebri consists of a smartphone application (app) and two wireless sensors. Unique in its approach, the Cerebri app seamlessly integrates three biofeedback modalities – heart rate variability, temperature, and electromyography – making it a comprehensive, therapist-independent solution for non-pharmacological migraine management. Methods Biofeedback therapy using Cerebri for the prevention of migraine attacks in adults with episodic migraine (The BioCer study) was an open-label, randomized, waitlist-controlled, multicenter trial. This study investigated the safety and efficacy of daily home-based biofeedback sessions using the Cerebri device. A total of 286 participants will be randomized to either a 12-week intervention arm or waitlist control arm. The primary outcome was the change in the mean number of migraine days from baseline to the last 28-day period during the treatment phase in the treatment group compared with the control group. The primary outcome variable was prospectively collected through daily eDiary entries. Ethics and Dissemination Approval from the ethics committee and competent authorities was obtained prior to study initiation. Participation was voluntary and informed and written consent was obtained prior to inclusion. The results of this trial will be published in peer-reviewed international medical journals and communicated to patients and healthcare personnel through the relevant channels. Conventional and Novel Treatment Strategies for Porokeratoses: A Narrative Review.

Porokeratoses are a heterogenous group of autoinflammatory keratinization disorders all characterized by the presence of a cornoid lamella. In addition to gene mutations affecting the mevalonate pathway, environmental factors such as UV radiation, immunosuppression, trauma, and infection are also thought to contribute to porokeratoses. To date, there are no management guidelines or levels of evidence for commonly used pharmacologic and non-pharmacologic treatment options for porokeratoses. Conventional treatment strategies encompass topical and systemic drugs (e.g., salicylic acid, topical glucocorticoids, and retinoids), phototherapy, laser, and surgical interventions. Better insights into the pathogenesis of porokeratoses have paved the way for the development of novel therapeutic approaches, such as topical statins or the use of monoclonal antibodies. This narrative review aims to summarize both conventional and novel treatment options, including their level of evidence, advantages, and disadvantages.

Nonpharmacological Approaches to Managing Heart Failure With Preserved Ejection Fraction.

Heart failure with preserved ejection fraction (HFpEF) is a common subtype of heart failure marked by impaired left ventricular diastolic function and decreased myocardial compliance. Given the limited availability of evidence-based pharmacological treatments for HFpEF, there is a growing interest in nonpharmacological interventions as viable therapeutic alternatives. This review aims to explore the pathophysiology of HFpEF and present recent advancements in nonpharmacological management approaches, encompassing noninvasive therapies, invasive procedures and targeted treatments for comorbidities. An extensive literature review was undertaken to identify and synthesize emerging nonpharmacological treatment options for HFpEF, assessing their potential to enhance patient outcomes. Nonpharmacological strategies, such as vagus nerve stimulation, percutaneous pulmonary artery denervation, renal denervation, transcatheter insertion of atrial shunts and pericardial resection, demonstrate promising potential for alleviating HFpEF symptoms and improving patient prognosis. Moreover, addressing comorbidities, such as hypertension and diabetes, may offer additional therapeutic benefits. These cutting-edge techniques, in conjunction with well-established exercise therapies, pave the way for future research and clinical applications in the field. Nonpharmacological interventions hold promise for advancing HFpEF patient care and fostering a deeper understanding of these treatment approaches, which will facilitate new clinical applications and contribute to the development of more targeted therapies.

What improvements do general exercise training and traditional Chinese exercises have on knee osteoarthritis? A narrative review based on biological mechanisms and clinical efficacy.

Knee osteoarthritis (KOA) is a disease that significantly affects the quality of life of patients, with a complex pathophysiology that includes degeneration of cartilage and subchondral bone, synovitis, and associations with mechanical load, inflammation, metabolic factors, hormonal changes, and aging. This article aims to comprehensively review the biological mechanisms and clinical effects of general exercise training and traditional Chinese exercises (such as Tai Chi and Qigong) on the treatment of KOA, providing references for the development of clinical exercise prescriptions. A systematic search of databases including PubMed, Web of Science, Google Scholar, and China National Knowledge Infrastructure (CNKI) was conducted, reviewing studies including randomized controlled trials (RCTs), observational studies, systematic reviews, and meta-analyses. Keywords included "knee osteoarthritis," "exercise therapy," "physical activity," and "traditional Chinese exercise." General exercise training positively affects KOA by mechanisms such as promoting blood circulation, improving the metabolism of inflammatory factors, enhancing the expression of anti-inflammatory cytokines, and reducing cartilage cell aging. Traditional Chinese exercises, like Tai Chi and Qigong, benefit the improvement of KOA symptoms and tissue repair by regulating immune function and alleviating joint inflammation. Clinical studies have shown that both types of exercise can improve physical function, quality of life, and pain relief in patients with KOA. Both general exercise training and traditional Chinese exercises are non-pharmacological treatment options for KOA that can effectively improve patients' physiological function and quality of life. Future research should further explore the long-term effects and biological mechanisms of these exercise interventions and develop personalized exercise programs based on the specific needs of patients.

TaKeTiNa Music Therapy for Outpatient Treatment of Depression: Study Protocol for a Randomized Clinical Trial.

Depression is a prevalent and debilitating illness that significantly affects psychological and physical well-being. Apart from conventional therapies such as psychotherapy and medication, individuals with depression often lack opportunities for activities that are generally perceived as enjoyable, such as music, meditation, and arts, which have demonstrated therapeutic effectiveness. TaKeTiNa music therapy has been employed as a therapeutic intervention for more than two decades. However, there is a notable absence of well-designed clinical trials investigating its antidepressant effects, a gap we aim to address in our current study. Furthermore, shifts in the progression of depression may manifest both psychologically, by influencing emotional states, and physiologically, by leading to alterations in lipid and sphingolipid metabolism, cortisol levels, and immune system function. Our study seeks to analyze the impact of TaKeTiNa music therapy on both levels. This is a prospective monocentric randomized waitlist-controlled clinical trial. It investigates the influence of TaKeTiNa music therapy on patients with major depression in an outpatient setting. Therefore, interested persons are randomly assigned to two groups, an intervention group or a control group, after completing a screening procedure. The intervention group starts with an eight-week TaKeTiNa music therapy intervention. The waiting group receives the same therapy program after completing the follow-up period. Blood and saliva sampling as well as responses to questionnaires are obtained at specific time points. Our study investigates the effects of TaKeTiNa music therapy, a non-pharmacological antidepressant treatment option, on depressive symptoms. We also address functional and causal immunological changes; hormonal changes, such as changes in cortisol levels; and metabolic changes, such as changes in serum lipids and sphingolipids, during the course of depression. We expect that this study will provide evidence to expand the range of treatment options available for depression.

The Effect of Administration of Solanum Lycopersium Var Cerasiforme on Changes in Blood Pressure in Hypertension Patients

Background : Hypertension is still a problem for some people because treatment must be carried out regularly and requires regular visits to health facilities. This is a problem for some groups of society, along with the continued use of drugs that have side effects that they must consume with long-term use. Hypertension occurs when systolic blood pressure > 140 mmHg and diastolic blood pressure > 90 mmHg. Apart from medicines, we can also treat high blood pressure by using natural ingredients such as tomato juice. Tomato juice contains potassium and lycopene, which can lower blood pressure. Objectives : To determine the effect of giving tomato juice on changes in blood pressure in hypertensive patients. Methods : Quantitative research with a quasi-experimental design using a non-randomized control group pretest-posttest approach and a sampling technique using purposive sampling. The total sample was 62 respondents. Wilcoxon test and Mann-Whitney test were used for statistical tests. Results : Wilcoxon test results for systolic and diastolic blood pressure in the intervention group were recorded for each p-value <0.05 before and after the intervention. The results of the Mann-Whitney test for systolic and diastolic blood pressure after intervention showed a p-value <0.05. Conclusion : Giving tomato juice (Solanum lycopersicum) affects changes in blood pressure in hypertensive patients, so it is recommended as an alternative non-pharmacological treatment option.

Motor behavior improvement in ischemic gerbils by cholinergic receptor activation and treadmill training

Introduction: Treadmill exercise training is one of the most investigated non-pharmacological treatment options for experimental brain ischemia. However, the cholinergic system is essential for improving motor behavior responses. Objective: To analyze the effects of a nicotinic acetylcholine receptor (nAChR) agonist (1, 2, and 4 mg/kg) on the motor behavior of ischemic gerbils subjected to forced treadmill training.Methods: In this experimental study, 72 gerbils, weighing 65–80 g, were divided into eight groups: Sal, Ni1, Ni2, Ni4, I, INi1, INi2, and INi4. Behavioral assessment was initiated 24 hours after the last motor stimulation on the treadmill. Rotarod test (RR) was employed to analyze animal behavior. The data were analyzed using one-way analysis of variance (ANOVA), and the Newman-Keuls post hoc test evidenced differences detected between groups. Results: Data regarding the RR test revealed decreased time spent on the RR apparatus for the Ni1, Ni4, and I groups compared to the Sal and Ni2 groups. However, the INi1 and INi2 groups showed increased time spent compared with the ischemia and INi4 groups (F7,64=4.63; p<0.05).Conclusions: The present study indicates that treadmill training with a concomitant 1 and 2 mg/kg of nAChR agonist effectively improves the behavior of ischemic gerbils.

Therapeutic role of yoga in hypertension.

Systemic hypertension is an established risk factor for coronary artery disease and cerebrovascular accident and control of blood pressure reduces the risk of a major cardiovascular event. Both non-pharmacological and pharmacological treatment options are available to treat hypertension. Yoga, recently received more attention as a treatment modality for various lifestyle disorders, even though practiced in India since ancient times. In this review, we are analyzing the role of yoga in the treatment of systemic hypertension.

Abstract P164: Patient-Reported Awareness and Understanding of the Diagnosis of Heart Failure With Preserved Ejection Fraction

Background: Heart failure with preserved ejection fraction (HFpEF) accounts for half of all HF cases, is difficult to diagnose and manage, and represents the fastest growing subtype of HF in the US. Lack of awareness about the HFpEF diagnosis among patients contributes to delays in seeking care, reduced participation in self-management, and worse health outcomes. Methods: This study explored HFpEF-related knowledge among patients using semi-structured interviews guided by the “Integrated Health Literacy Model”. Patients were asked to describe 1) their heart condition, as they understand it; 2) where they currently learn about their heart condition; 3) what they want to learn more about; and 4) who can help them learn more. Results: In this study, 10 patients with HFpEF were recruited from a local health care system ( Table ). All patients were unaware of having HFpEF; 8 out of 10 thought they had "congestive HF." Patients learned about their condition from cardiologists, primary care physicians and clinic staff. All reported using the Internet to seek information, though in some cases information was confusing or contradictory to what they were told before. Patients noted that they had symptoms for a long time before learning the symptoms were HF-related. Patients wish to know more about the prognosis, factors that can help or make their condition worse, signs and symptoms to monitor, use of wearable devices to monitor, how to live longer and non-pharmacological treatment options. Most patients wanted to learn more about their condition from cardiologists, however, some faced challenges in their current cardiology care, citing lack of time during the clinic visit, difficulty with medical terminology and inability to ask follow-up questions. Patients emphasized a need for more emotional support at the time of diagnosis and help addressing mental health. Conclusion: Patients were not aware they had HFpEF, expressed multiple needs for education and support, and identified cardiologists as the most credible source of education.

Feasibility of transcranial direct current stimulation (tDCS) combined with pelvic floor muscle training (PFMT) in female urinary incontinence: randomized controlled trial

INTRODUCTION: Pelvic floor muscle training (PFMT) is widely regarded as the most conservative and effective non-pharmacological treatment option for women suffering from urinary incontinence (UI). Transcranial direct current stimulation (tDCS) is a non-invasive, non-pharmacological neuromodulation technique that has demonstrated promising results in a variety of medical conditions. However, there has been little research into the feasibility of tDCS as an adjunct therapy to PFMT in improving symptoms in women with UI. OBJECTIVE: To explore the feasibility (recruitment and retention) of tDCS as an adjunct therapy to PFMT to relieve symptoms of female UI. METHODS AND MATERIALS: Eleven female patients were randomly assigned to receive 12 non-consecutive sessions of PFMT combined with 20 minutes (2mA) of anodal (Cz) or sham tDCS for 4 weeks. Feasibility (primary outcome) was assessed through recruitment and retention rates. Secondary outcomes included four domains: (1) urinary leakage, (2) severity of incontinence, (3) impact on quality of life, and (4) symptoms and adverse events. RESULTS: Eleven patients were evaluated, and nine women completed the treatment protocol. The recruitment rate was 100%, and retention was 81.8%. Clinical results showed that anodal tDCS is viable as adjunctive therapy to PFMT protocol and may result in minimal clinically important differences (MCID) in UI symptoms. CONCLUSION: The high rate of recruitment and retention indicates that tDCS in combination with PFMT is a feasible adjunct therapy for female UI treatment. This research supports the recommendation for a full RCT, with prioritization of outcomes required for hypothesis testing.

Quantifying preferences for urea cycle disorder treatments using a discrete choice experiment

Aims: Urea cycle disorders (UCDs) can cause ammonia accumulation and central nervous system toxicity. Nitrogen-binding medications can be efficacious but certain attributes may negatively impact adherence. This study sought to quantify the administration-related attributes influencing overall prescription selection and patient adherence. Methods: A web-based, quantitative survey including discrete choice experiment (DCE) methodology captured responses from health care providers for patients with UCDs. A series of hypothetical treatment profile sets with attributes such as route of administration, taste/odor, preparation instructions, packaging, dose measurement, and weight use restrictions were presented. From 16 sets of 3 hypothetical product profiles, respondents evaluated attributes most preferred for prescription selection or patient adherence. Attributes assumed a higher overall preference if relative importance (RI) scores were >16.67% (the value if all attributes were of equal importance). Preference weight scores were assessed. A nine-point Likert scale assessed respondent attitudes, such as satisfaction. Results: A total of 51 respondents completed the survey. Respondents reported dissatisfaction with current treatments (mean [SD] = 5.4 [1.7]). For prescription selection, four attributes achieved RI >16.67%: taste/odor (24%), weight restrictions (21%), preparation instructions (18%), and route of administration (17%). For adherence, three attributes related to administration achieved RI >16.67%: taste/odor (28%), preparation instructions (21%), and route of administration (17%). Preference weights for “taste/odor masked” were higher than “not taste/odor masked” for prescription selection (mean [SD]; 1.52 [1.10] vs -1.52 [1.10]) and treatment adherence (73.8 [55.2] vs -73.8 [55.2]). Limitations: This study contained a relatively small sample size. Survey respondent selection, the use of hypothetical product profiles, and exclusion of non-pharmacologic treatment options could have contributed to potential biases. Conclusions: Among attributes tested, taste/odor was the most important attribute influencing overall preference for both prescribing and patient adherence, with taste/odor masking preferred. Optimizing nitrogen-binding medications through masking taste/odor may support improved patient adherence and outcomes in UCDs.

Effectiveness of Live-Streaming Tele-Exercise Intervention in Patients With Parkinson's Disease: A Pilot Study.

Exercise can improve both motor and nonmotor symptoms in people with Parkinson's disease (PwP), but there is an unmet need for accessible and sustainable exercise options. This study aimed to evaluate the effect, feasibility, and safety of a regularly performed live-streaming tele-exercise intervention for PwP. A live-streaming exercise intervention for PwP was implemented twice a week for 12 weeks. We measured the motor and nonmotor symptom scores of the included patients before and after the intervention. Changes in clinical scores from baseline to postintervention were analyzed using paired t-tests. Factors associated with improvements in clinical scores and compliance were analyzed using Pearson's correlation analysis. Fifty-six participants were enrolled in the study. There were significant improvements in Hospital Anxiety and Depression Scale (HADS)-anxiety (p = 0.007), HADS-depression (p < 0.001), Unified Parkinson's Disease Rating Scale (UPDRS) part III (p < 0.001), UPDRS total (p = 0.015), Hoehn and Yahr stage (p = 0.027), and Parkinson's Disease Fatigue Scale-16 (p = 0.026) scores after the intervention. Improvements in motor symptoms were associated with improvements in mood symptoms and fatigue. Higher motor impairment at baseline was associated with a greater compliance rate and better postintervention composite motor and nonmotor outcomes (ΔUPDRS total score). Overall, the 12-week tele-exercise program was feasible and safe for PwP. No adverse events were reported. The overall adherence rate was 60.0% in our cohort, and 83.4% of the participants were able to participate in more than half of the exercise routines. The live-streaming tele-exercise intervention is a safe, feasible, and effective nonpharmacological treatment option that can alleviate fatigue and improve mood and motor symptoms in PwP.

Limited use of virtual reality in primary care physiotherapy for patients with chronic pain

BackgroundChronic pain is a disabling condition which is prevalent in about 20% of the adult population. Physiotherapy is the most common non-pharmacological treatment option for chronic pain, but often demonstrates unsatisfactory outcomes. Virtual Reality (VR) may offer the opportunity to complement physiotherapy treatment. As VR has only recently been introduced in physiotherapy care, it is unknown to what extent VR is used and how it is valued by physiotherapists. The aim of this study was to analyse physiotherapists’ current usage of, experiences with and physiotherapist characteristics associated with applying therapeutic VR for chronic pain rehabilitation in Dutch primary care physiotherapy.MethodsThis online survey applied two rounds of recruitment: a random sampling round (873 physiotherapists invited, of which 245 (28%) were included) and a purposive sampling round (20 physiotherapists using VR included). Survey results were reported descriptively and physiotherapist characteristics associated with VR use were examined using multivariable logistic regression analysis.ResultsIn total, 265 physiotherapists participated in this survey study. Approximately 7% of physiotherapists reported using therapeutic VR for patients with chronic pain. On average, physiotherapists rated their overall experience with therapeutic VR at 7.0 and “whether they would recommend it” at 7.2, both on a 0–10 scale. Most physiotherapists (71%) who use therapeutic VR started using it less than two years ago and use it for a small proportion of their patients with chronic pain. Physiotherapists use therapeutic VR for a variety of conditions, including generalized (55%), neck (45%) and lumbar (37%) chronic pain. Physiotherapists use therapeutic VR mostly to reduce pain (68%), improve coordination (50%) and increase physical mobility (45%). Use of therapeutic VR was associated with a larger physiotherapy practice (OR = 2.38, 95% CI [1.14–4.98]). Unfamiliarity with VR seemed to be the primary reason for not using VR.DiscussionTherapeutic VR for patients with chronic pain is in its infancy in Dutch primary care physiotherapy practice as only a small minority uses VR. Physiotherapists that use therapeutic VR are modestly positive about the technology, with large heterogeneity between treatment goals, methods of administering VR, proposed working mechanisms and chronic pain conditions to treat.

Treatment Modalities for Insomnia in Adults Aged 55 and Older: A Systematic Review of Literature from 2018 to 2023.

Insomnia is the most common sleep disorder experienced by older adults. There is a wide range of pharmacological and non-pharmacological treatment options in existing literature. The purpose of this systematic review was to synthesize randomized controlled trials of insomnia treatment modalities for adults aged 55 and older over the last 5 years. We searched four databases, and after screening, there were 34 full-text manuscripts that met the inclusion/exclusion criteria. We found non-pharmacological interventions, including exercise and behavioral/psychoeducational therapies, remain effective and favorable. Complementary and alternative therapies ranged across studies and warrant further testing in larger, more diverse samples. Dual orexin receptor antagonist medications were tested in a few studies with positive benefits for sleep and minimal side effects. Finally, measures of insomnia/sleep disturbance outcomes varied among the studies, with the Pittsburgh Sleep Quality Index being used most frequently. Non-pharmacological interventions for insomnia in older adults are effective, and some newer medications may be safer, with less side effects, at managing insomnia in this population.

Pregnant women try to overcome this problem by using medicines or complementary therapies such as acupressure. This research aims to evaluate the effectiveness of complementary acupressure therapy in reducing the symptoms of emesis gravidarum in pregnant women. A study involving 40 pregnant women, of which 20 experienced significant symptoms, used acupressure techniques at pericardium point 6 to relieve symptoms of nausea and vomiting during the early trimester of pregnancy. This research used quasi-experimental methods (pre-test and post-test) and collected data through questionnaires RINVR and observations at Posyandu Mattirotappareng 1, Tempe District, Wajo Regency, South Sulawesi Province. The results showed a significant difference in the frequency of nausea and vomiting before and after acupressure therapy. After undergoing therapy, there was a significant reduction in the average number of nausea and vomiting events per day, decreasing from 9.0 to 5.2 times. The p-value &lt; 0.05 indicates that acupressure therapy has a significant impact in reducing the frequency of nausea and vomiting in pregnant women. Therefore, the use of acupressure therapy at pericardium point 6 as a non-pharmacological treatment option is effective in reducing symptoms in pregnant women during the early trimester of pregnancy.
