Non-invasive electrocardiographic assessment of arrhythmic risk in ischaemic cardiomyopathy patients undergoing revasularisation

Abstract Background Ventricular arrhythmias are a leading cause of death in patients with ischaemic cardiomyopathy (ICM). Ischaemia is thought to increase arrhythmic risk and coronary revascularisation is frequently undertaken to reduce this risk, although the recent REVIVED-BCIS2 trial did not show a difference in the incidence of death, device therapies or sustained arrhythmias in patients assigned to have percutaneous coronary intervention (PCI) compared to optimal medical therapy alone(1). Purpose We assessed the mechanistic effects of revascularisation on arrhythmic risk in patients with ICM using non-invasive electrocardiographic imaging (ECGi). Methods Patients were eligible if they had a left ventricular ejection fraction (LVEF) <40%, extensive coronary disease (BCIS Jeopardy Score≥6) and were scheduled to undergo revascularisation with coronary artery bypass grafting or PCI. ECGi assessment was performed using a 252-electrode vest with anatomical co-registration using a CT thorax. Patients underwent stress perfusion cardiac magnetic resonance (CMR) imaging; both ECGi and CMR were conducted at baseline and 3 months post revascularisation(Figure 1). The primary outcome measure was dispersion of repolarisation measured using global activation recovery interval (ARI) dispersion. Secondary outcome measures included repolarisation time (RT) dispersion, mean ARI and LV ARI dispersion. We also assessed the relationship between ARI dispersion, revascularisation and left ventricular scar burden. Data are presented as mean ± SD or median [IQR]. Results Thirty patients were recruited between 2020-2023; age 67±11 years, 90% male, BMI 28±5 and baseline LVEF was 29±8%. Twelve patients underwent CABG, 18 (60%) had PCI. The BCIS jeopardy score was 10 [9-11] at enrolment and decreased to 2 [0-4] after revascularisation. The LV scar burden at baseline was 15.1±6.5% and the change at 3 months was -0.5±5% (p=0.70). ARI dispersion and LV ARI dispersion were correlated with LV scar burden (r=0.62,p=0.01 and 0.71,p=0.002 respectively). No differences were seen in ARI dispersion (44.5 vs 44.1, p=0.86) or secondary outcome measures before and after revascularisation (RT dispersion: 45.5 vs 44.7, p=0.49; Mean ARI: 281.3 vs 282.6, p=0.74; ARI dispersion: 44.5 vs 44.1, p=0.86; LV ARI dispersion: 38.4 vs 36.7, p=0.39)(Figure 2). The LVEF change on CMR at 3 months was +6.9±10%; no relationship was seen between change in LVEF and ARI dispersion. Conclusion Arrhythmic risk assessed by non-invasive ECGi was not altered by coronary revascularisation. LV ARI dispersion was highly correlated with scar burden, which is not altered by revascularisation. Future work includes analysis of perfusion data. Our results support the findings of the REVIVED arrhythmia study that revascularisation does not reduce arrhythmic risk in patients with ICM(1). ECGi derived LV ARI dispersion correlates with scar and may represent a non-invasive arrhythmic risk assessment tool.Figure 1Figure 2

High-resolution repolarization changes preceding short-coupled ectopy in idiopathic ventricular fibrillation, and effects of quinidine

Abstract Background Idiopathic ventricular fibrillation (VF) is linked to early repolarization, increased repolarization heterogeneity, and (short-coupled) premature ventricular complexes (PVCs). Quinidine, an antiarrhythmic drug with class-IA and -III effects, reduces the occurrence of PVCs and VF, but its effects on repolarization have not yet been studied at the whole-heart level in idiopathic-VF patients. Purpose To study dynamic changes in repolarization preceding short-coupled PVCs using noninvasive electrocardiographic imaging (ECGI) in a patient with idiopathic VF, and the antiarrhythmic effects of quinidine. Methods ECGI was performed during sinus rhythm in a 61-year-old male with an ICD (secondary prevention; 2.5 mg nebivolol daily) two days after recurrent idiopathic VF (Fig. A). This was done 1) at baseline, 2 minutes before short-coupled ectopy ("baseline"); 2) at baseline, 10 seconds before the short-coupled PVC ("pre PVC"); and 3) at 2 hours after start of quinidine 200 mg ("on-quinidine"). The ECGI procedure consisted of a 224-electrode body surface potential map and a contrast-enhanced CT scan to obtain torso and heart geometries. After reconstruction of epicardial unipolar electrograms (UEGs), local activation and repolarization times (ATs and RTs) were derived from UEGs by assessing the steepest downslope of the QRS complex and steepest upslope of the T waves, respectively, of 10-second signal-averaged beats. AT and RT duration (first to last AT or RT) were assessed. Results At baseline, QRS duration and QT interval measured 107 ms and 343 ms, respectively, on the standard 12-lead ECG. ECGI-derived epicardial AT duration was 72 ms, and RT histogram span 92 ms. The coupling interval of the PVC that triggered VF in the ICD readout was 340 ms (Fig. A). During 44 minutes of ECGI, 157 short-coupled PVCs from the moderator-band region were recorded (Fig. B; no tachycardia or VF). Just prior to a PVC, epicardial early repolarization at the PVC origin decreased by 26 ms (star in Figs. C-E), and the RT histogram broadened to 109 ms (Fig. E). While on-quinidine, (25 minutes; QRS duration 107 ms, QT interval 419 ms), epicardial repolarization was prolonged (Fig. C/D), also at the site of abnormal impulse formation. Quinidine had increased the epicardial AT duration to 79 ms, and RT histogram span to 133 ms. PVCs were completely suppressed. The patient remained free of sustained arrhythmias while on-quinidine. Conclusion(s) In this case of short-coupled idiopathic VF, epicardial early repolarization shortened momentarily prior to abnormal impulse formation. We found dynamic substrate-trigger characteristics that likely facilitate reentrant excitation. Quinidine prolonged the duration of epicardial activation and repolarization, suppressing the ectopy and VF recurrence.

Non-invasive assessment of the ventricular tachycardia isthmus during sinus rhythm

Abstract Background The ablation of ventricular tachycardias (VT) presents significant challenges due to the complex and time-consuming approaches involved in identifying arrhythmogenic regions. Combining non-invasive electrocardiographic imaging (ECGI) with endocardial mapping could potentially enhance the efficacy of interventions. Objective To evaluate the capability of ECGI in guiding operators towards areas of VT isthmuses during sinus rhythm. Methods Patients with scar-related VT, confirmed by late gadolinium enhancement cardiac magnetic resonance, were prospectively enrolled. We compared the areas of slowed conduction identified by ECGI with the VT sustaining regions identified endocardially. Simultaneous Electroanatomical Mapping (EAM) and ECGI mapping were performed during VT ablation procedures. In patients in whom a hemodynamically stable VT was maintained, the reentry circuit was mapped endocardially, and the ablation target was identified. The left ventricle was segmented into 9 regions, categorized as either VT-isthmus or non-VT isthmus based on EAM findings. Prior to ablation, local activation times (LAT) and conduction velocity (CV) ECGI sinus maps were computed. Slow conduction was quantified non-invasively by computing different ECGI metrics per region, including the mean and the standard deviation (SD) of LAT and CV, the regional Total Activation Time (rTAT) – time span between the earliest and latest activated nodes–, and the Slow Conduction Velocity (SCV) – the percentage of nodes where velocity falls below 60 cm/s -. Results The study included 20 consecutive patients with ischemic cardiomyopathy who were referred for VT ablation. A total of 21 regions linked to the VT isthmus were identified endocardially in 17 patients (age: 66.1±8.8 years, 94.1% male, left ventricular ejection fraction: 26.4±7.3%). Figure 1 illustrates the correlation between the VT circuit and the ECGI sinus map in two patients with extensive left ventricular fibrosis. ECGI metrics related to VT isthmuses exhibited a higher standard deviation in both LAT (18.1±2.0 vs 9.8±1.2 ms; p=0.001), and CV (50.4±4.7 vs 37.1±2.8 cm/s; p=0.011), and a greater percentage of nodes with slowed conduction (13.7±2.7 vs 7.4±1.6 %; p=0.029), compared to non-VT isthmus regions, Figure 2. Multivariate logistic regression analysis revealed that SD-LAT was the strongest predictor of VT isthmus localization. Conclusion ECGI has demonstrated the capability to pinpoint slowed conduction areas responsible for VT in sinus rhythm, showing promise in planning and guiding VT ablation strategies. This advancement paves the way for further research into using this non-invasive tool to improve risk stratification methods for ventricular arrhythmias.

Imageless electrocardiographic imaging for atrial electrophysiological characterization: a validation study

Abstract Background The mechanisms responsible for atrial fibrillation (AF) are not yet fully understood and treatment strategies remain suboptimal. Electrocardiographic imaging (ECGi) could provide new insights into the arrhythmogenic substrate’s characterization. Objective To validate a novel imageless ECGi technique against electroanatomical mapping (EAM) systems to evaluate its use in guiding therapeutic decisions. Methods Periprocedural ECGi was implemented on 12 patients submitted for pulmonary vein isolation with EAM arriving in sinus rhythm. As opposed to current methods requiring patient-specific torso-heart geometries from CT/MRI scans, the employed ECGi system computes a cardiac anatomical model from a 3D-camera torso reconstruction. Both ECGi signals and intracavitary recordings were acquired simultaneously during catheter stimulation from a single atrial site at two different pacing cycles and local activation time (LAT) maps were derived. As a result, an ECGi and an EAM map were obtained for both 300 and 600 ms distal coronary sinus pacing for each patient. These were then compared by splitting the left atrial geometries from both mapping methodologies into 7 predefined, clinically relevant regions and identifying the earliest and latest activation sites in each. Results For all 24 LAT maps under consideration, the pacing sites identified by non-invasive ECGi maps were consistent with those determined through EAM as 100% accuracy was obtained in terms of detecting the first activated region. As well as the correct stimulation area, according to the corresponding EAM instance, the earliest ECGi activations were also found in one of its immediate neighbours in 38% of the maps, indicating a broader region of interest. Regarding the latest activation sites, ECGi achieved an accurate localisation in 92% of the activation sequences and, similarly, other neighbouring regions showed the maximum LAT in one third of the studied maps. Conclusions Considering the spatial resolution constraints inherent to its non-invasive nature, imageless ECGi activation patterns are congruent with the gold-standard technique for substrate mapping without the need for prior medical imaging.

The Dutch Idiopathic Ventricular Fibrillation Registry: progress report on the quest to identify the unidentifiable.

Idiopathic ventricular fibrillation (iVF) is arare cause of sudden cardiac arrest and, by definition, adiagnosis of exclusion. Due to the rarity of the disease, previous and current studies are limited by their retrospective design and small patient numbers. Even though the incidence of iVF has declined owing to the identification of new disease entities, an important subgroup of patients remains. To expand the existing Dutch iVF Registry into alarge nationwide cohort of patients initially diagnosed with iVF, to reveal the underlying cause of iVF in these patients, and to improve arrhythmia management. The Dutch iVF Registry includes sudden cardiac arrest survivors with an initial diagnosis of iVF. Clinical data and outcomes are collected. Outcomes include subsequent detection of adiagnosis other than 'idiopathic', arrhythmia recurrence and death. Non-invasive electrocardiographic imaging is used to investigate electropathological substrates and triggers of VF. To date, 432 patients have been included in the registry (median age at event 40years (interquartile range 28-52)), 61% male. During amedian follow-up of 6(2-12) years, 38patients (9%) received adiagnosis other than 'idiopathic'. Eleven iVF patients were characterised with electrocardiographic imaging. The Dutch iVF Registry is currently the largest of its kind worldwide. In this heterogeneous population of index patients, we aim to identify common functional denominators associated with iVF. With the implementation of non-invasive electrocardiographic imaging and other diagnostic modalities (e.g. echocardiographic deformation, cardiac magnetic resonance), we advance the possibilities to reveal pro-fibrillatory substrates.

Abstract 17749: Regionalization of Non-Pulmonary Vein Triggers for Atrial Fibrillation When the P Wave is Obscured Using Non-Invasive Electrocardiographic Imaging QRS-T Subtraction Algorithm

Introduction: Ablation of non-pulmonary vein triggers (NPVTs) after pulmonary vein isolation (PVI) may reduce atrial fibrillation (AF) recurrence. Localizing a trigger which initiates AF after a single atrial ectopic beat is challenging, especially in the scenario when the trigger P wave is obscured by preceding QRS-T complex. Aim: To evaluate the potential for noninvasive electrocardiographic imaging (ECGi) to localize NPVTs when the P wave morphology is superimposed on the T wave of the previous beat. Methods: We developed an algorithm which overlays and subtracts the preceding QRS-T wave complex from the QRS-T wave complex which is obscuring the P wave of interest, thus revealing the P wave morphology (Figure 1A). Resultant unobscured P waves (from multiple body surface electrodes) are then used for ECGi computation. In five patients undergoing AF ablation, after PVI, we paced from 15 atrial sites where NPVTs commonly arise, and evaluated the epicardial activation maps generated by ECGi both when pacing was timed to ensure an unobscured P wave and when timed to coincide with preceding QRS-T complex. Co-registration of CT-based ECGi activation maps with invasive electroanatomic map (EAM) allowed comparison of the earliest activation site on ECGi map from both the obscured P waves after QRS-T subtraction and unobscured P waves with true pacing locations on EAM (Figure 1B). Results: From 146 pacing sites in our patient cohort, for the unobscured P waves, median distance between earliest site on ECGi map and EAM pacing location was 16 mm (10-21 mm), and for obscured P waves after QRS-T subtraction, median distance was also 16 mm (12-23 mm) (Figure 1C). Conclusion: Using a QRS-T subtraction algorithm, ECGi can approximate origin of paced P waves whether P wave is unobscured or obscured by the preceding QRS-T complex. Spontaneous NPVT P waves are commonly obscured by the QRS-T wave and the ability to rapidly localize an early coupled P wave may facilitate NPVT mapping and ablation.

Noninvasive ECG imaging of the intrinsic atrial pacemaker and atrial activation in surgically repaired or palliated congenital heart disease.

Sinus node location, function, and atrial activation are often abnormal in patients with congenital heart disease (CHD),due to anatomical, surgical, and acquired factors. We aimed to perform noninvasive electrocardiographic imaging (ECGI)of the intrinsic atrial pacemaker and atrial activation in patients with surgically repaired or palliated CHD, compared with control patients with structurally normal hearts. Atrial ECGI was performed in eightCHD patients with prespecified diagnoses (Fontan circulation, dextro transposition of the great arteries post Mustard/Senning, tetralogy of Fallot), and three controls. Activation and propagation maps were constructed in presenting rhythm. Wavefront propagation was analyzed to identify (1) intrinsic atrial pacemaker breakout site, (2) morphological right atrial (RA) activation pattern, (3) morphological left atrial (LA) breakout sites (i.e., interatrial connections), (4) LA activation pattern, and (5) putative lines of block. Physiologically appropriate atrial activation and propagation maps were able to be constructed. In the majority of patients, atrial breakouts were in keeping with the sinus node, observed in a crescent-shaped distribution from the anterior superior vena cava to the posterior RA. Ectopic atrial pacemaker sites were demonstrated in the atriopulmonary (AP) Fontan patient (very diffuse posterolateral RA) and Mustard patient (very posterior RA competing with a low RA focus). RA propagation was laminar in controls, but suggested either a line of block or conduction slowing consistent with an atriotomy scar in the tetralogy of Fallot (TOF) patients. Putative lines of block were more complex and RA propagation more abnormal in the atrial switch and AP Fontan patients, compared with the TOF patients. RA activation in the extracardiac Fontan patients was relatively laminar. Earliest LA breakout was most commonly observed in the region of Bachmann's Bundle in both controls and CHD patients, except for posterior LA breakouts in two patients. LA activation was typically more homogeneous than RA activation in CHD patients. ECGI can be utilized to create a noninvasive mapping model of atrial activation in postsurgical CHD, demonstrating atrial pacemaker location, putative lines of block and interatrial connections. Once validated invasively, this may have clinical implications in predicting risk of sinus node dysfunction and atrial arrhythmias, or in guiding catheter ablation.

Comparison of dipole-based and potential-based ECGI methods for premature ventricular contraction beat localization with clinical data.

Introduction: Localization of premature ventricular contraction (PVC) origin to guide the radiofrequency ablation (RFA) procedure is one of the prominent clinical goals of non-invasive electrocardiographic imaging. However, the results reported in the literature vary significantly depending on the source model and the level of complexity in the forward model. This study aims to compare the paced and spontaneous PVC localization performances of dipole-based and potential-based source models and corresponding inverse methods using the same clinical data and to evaluate the effects of torso inhomogeneities on these performances. Methods: The publicly available EP solution data from the EDGAR data repository (BSPs from a maximum of 240 electrodes) with known pacing locations and the Bratislava data (BSPs in 128 leads) with spontaneous PVCs from patients who underwent successful RFA procedures were used. Homogeneous and inhomogeneous torso models and corresponding forward problem solutions were used to relate sources on the closed epicardial and epicardial-endocardial surfaces. The localization error (LE) between the true and estimated pacing site/PVC origin was evaluated. Results: For paced data, the median LE values were 25.2 and 13.9mm for the dipole-based and potential-based models, respectively. These median LE values were higher for the spontaneous PVC data: 30.2-33.0mm for the dipole-based model and 28.9-39.2mm for the potential-based model. The assumption of inhomogeneities in the torso model did not change the dipole-based solutions much, but using an inhomogeneous model improved the potential-based solutions on the epicardial-endocardial ventricular surface. Conclusion: For the specific task of localization of pacing site/PVC origin, the dipole-based source model is more stable and robust than the potential-based source model. The torso inhomogeneities affect the performances of PVC origin localization in each source model differently. Hence, care must be taken in generating patient-specific geometric and forward models depending on the source model representation used in electrocardiographic imaging (ECGI).

Non-invasive electrocardiographic imaging for the characterization of complex atrial tachyarrhythmias

Background – The non-invasive characterization of complex atrial tachyarrhythmias, including atrial flutter (AFL) and focal atrial tachycardia (AT) is challenging. Conventional 12-lead electrocardiogram (ECG) faces several limitations. Planning a precise and preprocedural strategy for complex tachyarrhythmias catheter ablation (CA) is not easy. Purpose To evaluate the diagnostic capacity of a novel electrocardiographic imaging (ECGI) system that does not require previous CT/MRI thoracic imaging. Methods – 42 patients (27 males, CHA2DS2-VASc score 2±1, LVEF 54.5 ± 11.0 % and LA area 31.3 ± 7.0 cm2) undergoing CA for focal AT or AFL were prospectively included in the study. A preprocedural ECGI was managed based on a 64-electrode vest, a torso reconstruction using a 3D real-time acquisition camera, and an artificial intelligence-based method to estimate the patient atrial geometry. The differential diagnostic capacities of non-invasive 12-lead ECG and ECGI were assessed compared with endocavity electroanatomical mapping (EAM) regarding three endpoints: 1) the identification of the involved atrial cavity, 2) the mechanism (focal/micro-reentry or macro-reentry), and (3) the ablation target site defined as the area where the earliest activation was located for focal/micro re-entrant arrhythmias, and the precise anatomical pathway for macro re-entrant tachyarrhythmias. Regarding the ECG, the three endpoints were assessed based on validated algorithms by 2 different observers blinded from the diagnosis. Results – 48 atrial tachyarrhythmias of which 59.5% occurred in the context of a history of CA were evaluated. The non-invasive characterization of complex atrial arrhythmias using this novel ECGI system was possible in 94% of the cases (Figure A): directly for 36 arrhythmias and after decreasing ventricular response using vagal maneuvers for 9. ECGI obtained global accuracy to identify the involved atrial cavity of 91.7%, the mechanism of 89.6%, and the ablation target of 83.3%. In challenging cases, such as in patients with history of CA for AF (n = 27) and in patients without a final diagnosis of counterclockwise typical AFL (n=38) the diagnosis capacity of ECGI is consistent compared to the whole study population. The ECGI diagnostic capacity significantly outclassed the ECG one regarding the identification of the involved cavity (p<0.001), the mechanism (p<0.05), and the ablation target (p<0.001). ECGI was able to prevent the totality of unnecessary transseptal punctures (n=7), representing 28% of the overall punctures. Conclusions (Figure B) – This novel non-invasive ECGI system that does not require CT/MRI thoracic imaging accurately characterizes complex atrial tachyarrhythmia and outclasses ECG, the current noninvasive diagnostic tool of reference.

Delayed ventricular conduction alters ventricular repolarization in patients with heart failure: an electrocardiographic imaging study

Background In the normal human heart, the polarity of the T-wave is concordant with the QRS complex. This phenomenon is often explained by longer repolarization of earlier activated regions. Patients with delayed ventricular conduction often have discordant T-waves, but the link to regional repolarization is unknown. Aim To compare epicardial repolarization-activation (RT-AT) relations in patients with heart failure and reduced ejection fraction (HFrEF) with those having a structural normal heart. Methods 14 patients with HFrEF and delayed ventricular conduction and 11 patients no history of structural heart disease and a normal surface ECG (nQRS) were analyzed using electrocardiographic imaging (ECGI). HFrEF patients were subcategorized as left bundle branch block (LBBB, n=6) or non-specified intraventricular conduction disturbance (IVCD, n=8). Noninvasive ECGI was used to assess epicardial activation (ATs) and repolarization times (RTs). RT-AT relationships were evaluated for 24 epicardial segments using linear regression analysis. Interventricular repolarization dispersion (inter-RTD) was defined as the difference between average left-ventricular (LV) and right-ventricular RT, while total epicardial repolarization dispersion (total-RTD) was defined as the standard deviation of all epicardial RTs. The maximum of the local RT gradients (RTG) was defined as their 95th percentile. RT parameters were described as median±IQR and Kruskal Wallis tests were used to evaluate the differences between the groups. Results An HFrEF patient’s representative 24-segment AT and RT bullseyes with a positive RT-AT relation (~depolarize last, repolarize last) is provided in Figure1A. A positive RT-AT slope was present in the majority of HFrEF patients (1.14±0.17 for LBBB and 1.16±0.55 for IVCD), whereas the nQRS group (-0.25±2.16) demonstrated an overall slightly negative slope with high variability (Figure 1B and 1C). Individual (thin lines) and group-averaged (thick lines) RT-AT slopes normalized for the earliest AT and earliest RT (in case of a positive RT-AT slope) or latest RT (in case of a negative slope) are visualized in Figure 1C. Both inter-RTD (41±25 for LBBB, 32±13 ms for IVCD, and 8±20 ms for nQRS) as well as total-RTD (34±9for LBBB, 34±12 ms for IVCD, and 24±8 ms for nQRS) was significantly higher in the LBBB and IVCD group compared to the nQRS group, whereas RTG did not differ between the groups (Figure 1B). RT-AT slopes, inter-RTD, total-RTD, and RTG did not significant differ between the LBBB and IVCD subgroup. Conclusion This ECGI study shows that in patients with HFrEF, later activated regions also have a later completion of repolarization, regardless of conduction delay, whereas a variable RT-AT relation is present in subjects with structural normal hearts. HFrEF additionally initiates a higher degree of interventricular and total repolarization dispersion.

Local conduction velocities determined by non-invasive electrocardiographic imaging predict arrhythmia-free survival after pulmonary vein isolation

As catheter ablation is primarily targeting pulmonary vein (PV) ectopic activity, it is not surprising that extra-PV arrythmogenic substrate is a key determinant of arrhythmia recurrence. Against this background, several studies have proposed assessment of extra-PV substrate in terms of atrial fibrosis or locally reduced conduction velocities (CV) to guide treatment. However, to date no non-invasive method directly assessing electrical arrhythmogenic substrate has been established in clinical practice, and treatment decisions are commonly based on crude surrogates like AF type or left atrial size. Here we establish and validate a novel non-invasive method based on electrocardiographic imaging (ECGi) to determine atrial arrhythmogenic substrate in terms of reduced local CVs and its predictive value regarding arrhythmia recurrence after PVI. Methods and results 52 consecutive patients scheduled for AF ablation (PVI-only) and 19 healthy controls were prospectively included and received ECGi to assess left and right atrial arrhythmogenic substrate. This ECGi-based method uses 64 electrodes placed on the torso. Subsequently, a 3D model of the torso is acquired as an anatomical reference using a 3D reconstruction camera. A personalised 3D atrial geometry is then derived from a database of human atria using an artificial intelligence-based algorithm. Finally, unipolar surface electrograms are projected onto the cardiac geometry and local CVs are estimated. Mean ECGi-determined atrial CVs were significantly lower in AF patients than in healthy controls, both in a global analysis (1.45±0.15 m/s vs. 1.64±0.15 m/s; p<0.0001) and a regional analysis of 19 predefined left and right atrial segments (Fig. 1). Considering only the segments with the lowest average CV in each patient, differences in CVs were more pronounced (0.80±0.22 vs. 1.08±0.26 m/s; p<0.0001). Multivariate logistic regression analyses combined with c-statistics including other previously proposed predictors found this mean CV of the "slowest" segment to be the strongest predictor of arrhythmia recurrence. A ROC analysis revealed that a cut-off for this variable of 0.72 m/s best discriminates PVI responders from non-responders: patients with a mean CV >0.72 m/s in all atrial regions showed a 6-months arrhythmia-free survival of 90.9%, whereas patients with one or more atrial regions with a mean CV <0.72 m/s had a poor outcome with an arrhythmia-free survival rate of only 57.9% (Fig. 2). Conclusion This was the first study to investigate local atrial CV non-invasively and to validate their predictive value regarding outcome after PVI. The absence of ECGi-determined areas of slow conduction well discriminated PVI responders from non-responders. Such non-invasive assessment of electrical arrhythmogenic substrate may guide treatment strategies and be an important step towards personalised AF therapy.

Generation of cardiac digital twins based on noninvasive cardiac mapping

However, these personalization frameworks normally rely on invasive recordings which limits their clinical applicability. Electrocardiographic Imaging (ECGI) is a mapping technique that allows the reconstruction of global cardiac activity non-invasively. Purpose Evaluate the applicability of ECGI as a non-invasive tool for the planification of ablation strategies based on personalized digital twins. Methods Cardiac digital twins generated from endocardial or ECGI mapping were compared in a patient with Atrial Flutter prescribed for cardiac ablation. Endocardial mapping was obtained in the right atrium by using electroanatomical mappings. ECGI was recorded during the ablation procedure with a mapping system of 128 body surface recordings. For the cardiac digital twin based on endocardial mapping, the reentrant circuit around the tricuspid valve was reproduced based on the activation time maps obtained with the electroanatomical mapping system. Total activation time of both atria was personalized based on the duration of the F-waves on the ECG since the left atrium was not endocardialy mapped. For the cardiac digital twin based on the ECGI, activation times obtained in both left and right atrium were used to set-up the personalized model. From both approaches, computed torso potentials were calculated. The correlation with real body surface recordings was compared. To further evaluate the fidelity of the models, the efficacy of the ablation strategy performed on the real patient was evaluated over the digital twins. Results Both digital twins, the one based on endocardial mapping, and the one based on ECGI were compared. Regarding dynamics of the model, both models reproduced the reentrant circuit around the tricuspid valve. Interestingly, only the ECGI digital twin reproduced a reentrant wave around the right pulmonary veins. When endocardial map was used for the personalization, a median conduction velocity (CV) of 90.5 cm/s was obtained for the right atrium and 174.7 cm/s for the left; whereas when ECGI was used, the median CV were 104.2 cm/s at the right and 129.1 cm/s at the left atrium. The ability to reproduce body surface ECGs of both digital twins demonstrated that the one based on endocardial mapping obtained a low correlation of 0.13, whereas the ECGI digital twin increase the correlation up to 0.66. Regarding the ablation outcome, ablation terminated the arrhythmia only in the cardiac digital twin based on ECGI. Conclusion The possibility to generate cardiac digital twins based on non-invasive ECGI mapping has been demonstrated. This opens the door to obtaining personalized cardiac digital twins before endocardial interventions which can be used to plan ablation procedures.

PO-02-226 NON-INVASIVE ELECTROCARDIOGRAPHIC IMAGING FOR THE CHARACTERIZATION OF COMPLEX ATRIAL TACHYARRHYTHMIAS

Non-invasive characterization of complex atrial tachyarrhythmias, regarding atrial flutter (AFL) and focal atrial tachycardia (AT) is challenging. Conventional electrocardiogram (ECG) faces several limitations. Planning a preprocedural strategy before AFL-AT catheter ablation (CA) is needed. To evaluate the diagnostic capacity of a novel non-invasive electrocardiographic imaging (ECGI) system that does not require previous CT or MRI cardiac imaging. 42 patients (27 males, CHA2DS2-VASc score 2±1, LVEF 54.5 ± 11.0 %, and LA area 31.3 ± 7.0 cm2) undergoing CA for AT-AFL were prospectively included in the study. Before the procedure, the ECGI system estimated the patient atria geometry using a 3D torso reconstruction and an artificial intelligence-based algorithm. Multiple electrocardiograms were recorded using a 64-electrodes vest during the CA procedure. The different diagnostic capacities of the periprocedural ECG and ECGI were assessed compared with endocavitary electroanatomical mapping (EAM) regarding three endpoints: (1) the involved atrial cavity, (2) the mechanism (focal or macro-reentry), and (3) the ablation target site defined as the area where the earliest activation was located for focal arrhythmias, and the precise anatomical pathway for macro re-entrant tachyarrhythmias. Regarding the ECG, endpoints were assessed based on validated algorithms by 2 different observers blinded from the diagnosis. 48 atrial tachyarrhythmias were evaluated and its characterization using this novel ECGI system was possible in 94% of the cases: directly for 36 arrhythmias and after decreasing ventricular response using vagal maneuvers for 9. ECGI obtained global accuracy to identify the involved atrial cavity of 92%, the mechanism of 90%, and the ablation target of 83% (Table). In challenging cases, such as in patients with a history of CA for atrial fibrillation and in patients without a final diagnosis of typical counterclockwise AFL, the accuracy of ECGI was consistent compared to the whole study population (Table). The ECGI diagnostic capacity significantly outclassed the ECG regarding the identification of the involved cavity (p<0.001), the mechanism (p<0.05), and the ablation target (p<0.001). ECGI could have prevented all unnecessary transseptal punctures (n=7), representing 28% of the overall punctures. This novel non-invasive ECGI system accurately characterizes complex atrial tachyarrhythmias (Figure) without needing previous CT or MRI cardiac imaging.

Novel non-invasive ECG imaging method based on the 12-lead ECG for reconstruction of ventricular activation: A proof-of-concept study.

Current non-invasive electrocardiographic imaging (ECGi) methods are often based on complex body surface potential mapping, limiting the clinical applicability. The aim of this pilot study was to evaluate the ability of a novel non-invasive ECGi method, based on the standard 12-lead ECG, to localize initial site of ventricular activation in right ventricular (RV) paced patients. Validation of the method was performed by comparing the ECGi reconstructed earliest site of activation against the true RV pacing site determined from cardiac computed tomography (CT). This was a retrospective study using data from 34 patients, previously implanted with a dual chamber pacemaker due to advanced atrioventricular block. True RV lead position was determined from analysis of a post-implant cardiac CT scan. The ECGi method was based on an inverse-ECG algorithm applying electrophysiological rules. The algorithm integrated information from an RV paced 12-lead ECG together with a CT-derived patient-specific heart-thorax geometric model to reconstruct a 3D electrical ventricular activation map. The mean geodesic localization error (LE) between the ECGi reconstructed initial site of activation and the RV lead insertion site determined from CT was 13.9 ± 5.6 mm. The mean RV endocardial surface area was 146.0 ± 30.0 cm2 and the mean circular LE area was 7.0 ± 5.2 cm2 resulting in a relative LE of 5.0 ± 4.0%. We demonstrated a novel non-invasive ECGi method, based on the 12-lead ECG, that accurately localized the RV pacing site in relation to the ventricular anatomy.

A Patchwork Method to Improve the Performance of Current Methods for Solving the Inverse Problem of Electrocardiography.

Noninvasive electrocardiographic imaging (ECGI) reconstructs cardiac electrical activity from body surface potential measurements. However, current methods have demonstrated inaccuracies in reconstructing sinus rhythm, and in particular breakthrough sites. This study aims to combine existing inverse algorithms, making the most of their advantages while minimizing their limitations. The "patchwork method" (PM) combines two classical numerical methods for ECGI: the method of fundamental solutions (MFS) and the finite-element method (FEM). We assume that the method with the smallest residual in the predicted torso potentials, computed using the boundary element method (BEM), provides the most accurate solution. The PM selects for each heart node and time step the method whose estimated reconstruction error is smallest. The performance of the PM was evaluated using simulated ectopic and normal ventricular beats. Cardiac potentials and activation maps obtained with the PM (CC = 0.63 ± 0.01 and 0.61 ± 0.05 respectively) were more accurate than MFS (CC = 0.61 ± 0.01 and 0.48 ± 0.05 respectively), FEM (CC = 0.58 ± 0.01 and 0.51 ± 0.02 respectively) or BEM (CC = 0.57 ± 0.02 and 0.49 ± 0.02 respectively). The PM also located all epicardial breakthrough sites, whereas the traditional numerical methods usually missed one. Furthermore, the PM showed its robustness and stability in the presence of Gaussian noise added to the torso potentials. The PM overcomes some of the limitations of classical numerical methods, improving the accuracy of mapping important features of activation during sinus rhythm and paced beats. This novel method for optimizing ECGI solutions opens a new avenue for improving not only ECGI but also other inverse problems.

Mechanisms of persistent atrial fibrillation and recurrences within 12 months post-ablation: Non-invasive mapping with electrocardiographic imaging.

Catheter ablation of persistent AF has not been consistently successful in terminating AF or preventing arrhythmia recurrences. Non-invasive Electrocardiographic Imaging (ECGI) can help to understand recurrences by mapping the mechanisms of pre-ablation AF and comparing them with the patterns of recurrent arrhythmias in the same patient. Seventeen persistent AF patients underwent ECGI before their first catheter ablation. Time-domain activation maps and phase progression maps were obtained on the bi-atrial epicardium. Location of arrhythmogenic drivers were annotated on the bi-atrial anatomy. Activation and phase movies were examined to understand the wavefront dynamics during AF. Eight patients recurred within 12 months of ablation and underwent a follow-up ECGI. Driver locations and movies were compared for pre- and post-ablation AF. A total of 243 focal drivers were mapped during pre-ablation AF. 62% of the drivers were mapped in the left atrium (LA). The pulmonary vein region harbored most of the drivers (43%). 35% of the drivers were mapped in the right atrium (RA). 59% (10/17) and 53% (9/17) of patients had repetitive sources in the left pulmonary veins (LPV) and left atrial appendage (LAA), and the lower half of RA, respectively. All patients had focal drivers. 29% (5/17) of patients had macro-reentry waves. 24% (4/17) of patients had rotors. Activation patterns during persistent AF varied from single macro-reentry to complex activity with multiple simultaneous wavefronts in both atria, resulting in frequent wave collisions. A total of 76 focal driver activities were mapped in 7/8 patients during recurrence. 59% of the post-ablation AF drivers were mapped in the LA. The pulmonary vein region harbored 50% of total drivers. 39% of sources were mapped in the RA. AF complexity remained similar post-ablation. 58% (44/76) of pre-ablation sources persisted during recurrence. 38% (3/8) of patients had macro-reentry and one patient had rotors. ECGI provides patient-specific information on mechanisms of persistent AF and recurrent arrhythmia. More than half pre-ablation sources repeated during post-ablation recurrence. This study provides direct evidence for drivers that persist days and months after the ablation procedure. Patient-tailored bi-atrial ablation is needed to successfully target persistent AF and prevent recurrence. ECGI can potentially predict recurrence and assist in choice of therapy.

Abstract 14320: Regionalization of Non-Pulmonary Vein Triggers for Atrial Fibrillation Using Non-Invasive Electrocardiographic Imaging

Introduction Ablation of non-pulmonary vein triggers (NPVTs) after pulmonary vein isolation (PVI) reduces atrial fibrillation (AF) recurrence, but localizing NPVTs can be challenging. We evaluated the potential for noninvasive electrocardiographic imaging (ECGi) to regionalize NPVTs and expedite more focused catheter-based mapping. ECGi combines measured body surface potentials with heart-torso geometry acquired from computed tomography (CT), to generate an activation map from a single recorded beat. Methods In 12 patients with AF undergoing first time AF ablation, the ECGi vest was fitted for preprocedural CT scan and then worn during procedure. Following completion of PVI, we paced from 15 atrial sites where NPVTs commonly arise (Figure 1), and evaluated the epicardial activation map generated by ECGi during steady state pacing at cycle length of 700-800 ms allowing an unobscured P wave. Merging of invasive anatomic map with CT-based ECGi map allowed comparison of ECGi activation map with true pacing location (Figure 2). Results From 180 pacing sites in our patient cohort (67% male, 58% persistent AF, 67% left atrial dilation), median distance between earliest activation site on ECGi map and pacing location was 16 mm (interquartile range, 11-22 mm), with 95% of sites within 30 mm (Figure 2). Septal pacing sites were measured to the epicardial aspect of the interatrial septum. Conclusions ECGi is able to consistently approximate the origin of paced beats from common NPVT sites and suggests the potential for rapid regionalization of NPVTs. The identified region can then be the focus of more detailed catheter-based mapping techniques to facilitate successful ablation of NPVTs.

State-of-the-Art Multimodality Imaging in Sudden Cardiac Arrest with Focus on Idiopathic Ventricular Fibrillation: A Review.

Idiopathic ventricular fibrillation is a rare cause of sudden cardiac arrest and a diagnosis by exclusion. Unraveling the mechanism of ventricular fibrillation is important for targeted management, and potentially for initiating family screening. Sudden cardiac arrest survivors undergo extensive clinical testing, with a growing role for multimodality imaging, before diagnosing “idiopathic” ventricular fibrillation. Multimodality imaging, considered as using multiple imaging modalities as diagnostics, is important for revealing structural myocardial abnormalities in patients with cardiac arrest. This review focuses on combining imaging modalities (echocardiography, cardiac magnetic resonance and computed tomography) and the electrocardiographic characterization of sudden cardiac arrest survivors and discusses the surplus value of multimodality imaging in the diagnostic routing of these patients. We focus on novel insights obtained through electrostructural and/or electromechanical imaging in apparently idiopathic ventricular fibrillation patients, with special attention to non-invasive electrocardiographic imaging.

Accuracy of noninvasive electrocardiographic imaging using isopotential versus isochronal map for identifying the site of origin of ventricular arrhythmias

Main funding source(s): Learning Health Background and aim Previous studies reporting the results of noninvasive electrocardiographic imaging (ECGI) used both isopotential map and isochronal map to assess the site of origin (SOO) of premature ventricular contractions (PVCs). The aim of this study was to evaluate the accuracy of both methods globally, and according to the location of the arrhythmia: from the right ventricular outflow tract (RVOT) or other locations (non-RVOT). Methods We studied with ECGI 35 consecutive patients with frequent (&amp;gt;10.000/24 h) PVCs. Patients were excluded if the ablation was not performed (7 patients) or was unsuccessful (7 patients). The study group consisted of 21 patients, 11 male, median age of 56 (44-71) years. The ECGI was performed with the epi-endocardial system Amycard. Two noninvasive maps were obtained: isopotential map based on the analysis of the shape and amplitude of the unipolar electrogram (FND method) and an isochronal map obtained using the activation direction method (ADM) (Figure). The invasive activation map was obtained with the Carto or the Ensite system and radiofrequency was applied at the earliest activation site with QS morphology on the unipolar electrogram and a pace match of at least 11/12. The SOO of the PVCs was considered the site where the PVCs were abolished. We assessed the accuracy of the ECGI to identify the SOO of the PVCs using both methods. A perfect match was defined as a predicted location by the ECGI within the same anatomic segment of the actual SOO of the PVCs, whereas a near match as a predicted location within the same segment or a contiguous one. Values are presented as median (Q1-Q3) Results PVCs originated in the RVOT in 11 (52%) patients. The percentage of near matches was not significantly different between FND and ADM methods (95% vs 86%, p=0.50), however the percentage of perfect matches was significantly higher with the FND than with the ADM technique (95% vs 67 %, p=0.031). We found no significant differences in accuracy according to the location of the PVCs, in the RVOT or outside (Table). Conclusions We found a good agreement between ECGI and invasive maps, however the FND technique showed a better accuracy regardless the site of the PVCs.
