Esmolol for the Control of Hypertension After Neurologic Surgery

We sought to evaluate the effectiveness of esmolol vs placebo in 40 patients emerging from general anesthesia for neurosurgery. Efficacy was defined as a decrease in systolic blood pressure to within 20% above average ward pressure. The need for additional antihypertensive agents to control blood pressure was also used to define efficacy. During the infusion period 20 of 21 (95%) of the esmolol-treated patients and two of 19 (11%) of the patients receiving placebo had return of systolic blood pressure to within 20% of average ward pressure (p < 0.001). One out of 21 (5%) esmolol-treated patients and 14 of 19 (74%) of the placebo group required intervention with additional antihypertensive medications (p < 0.001). Esmolol was found to be effective in controlling hypertension that develops on emergence from general anesthesia in patients undergoing neurosurgery. Zebulon Mennell. A pioneer of neurosurgical anaesthesia.

Dr. Zebulon Mennell, the first anaesthetist to devote himself largely to anaesthesia for neurosurgery, was born on 15 June 1876. He graduated at St. Thomas's Hospital in London, where he was an anaesthetist. He was also an anaesthetist at the Royal National Hospital for Nervous Diseases, Queen Square, London, a post which he held until 1945. His writings indicate that he appreciated the problems of neurosurgical anaesthesia and he was the first individual to work in this field as a specialist.

A formal audit as continuing medical education: anaesthesia for neurosurgery.

A brief description of the change from a normative evaluation to a formal audit of anaesthesia for neurosurgery is described. The criteria to be applied and their significance for clinical practise are listed. It is emphasized that these items are not presented as criteria for the standard of anaesthesia practised but as matters deserving debate among anaesthetists participating in a formal audit, particularly where the case load does not permit statistical analysis of patient outcome and only discussion of individual patients or small groups is possible. It is suggested, as it has been by others, that formal audit in a department of anaesthesia can be developed as the form of continuing medical education most closely related to the clinical work of the anaesthetists working within it.

Total intravenous anaesthesia for neurosurgery and neuroradiology.

A method of total intravenous anaesthesia for neurosurgery and neuroradiology is described using a continuous infusion of alphaxalone/alphadolone (Althesin) with analgesics and muscle relaxants. It has been used successfully on 272 occasions in 243 patients. The reasons for using such a technique and the advantages and disadvantages inherent in it are discussed.

Acupuncture anesthesia in neurosurgery.

This paper explores the method of acupuncture anesthesia in craniocerebral operations: selection of cases and preoperative management; selection of acupuncture points and adjustment of stimulus parameters; preoperative administration of drugs and choice of proper adjuvants during the operation; and problems existing in craniocerebral operations during the application of acupuncture anesthesia and their countermeasures. The "triple contrary effects" of acupuncture anesthesia are considered, namely: anti-pain or analgesia, anti-shock, and anti-infection. How acupuncture activates the analgesic effects of the organism is discussed: the spinal afferent pathway for the needling sensations; the role of the caudate nucleus in acupuncture analgesia; and the co-relationship between endorphin in the cerebrospinal fluid and acupuncture analgesia.
