Science mapping the literature in Applied Linguistics secondary research: navigating knowledge evolution from an epistemic perspective (1970–2022)

Applied Linguistics (AL) research has undergone significant transformation in recent decades, accompanied by a surge in secondary research within the field. Using WoS data, we conducted an extensive bibliometric analysis of 881 secondary research articles and 44,752 references spanning 1970 to 2022. Utilizing CiteSpace's “author co-citation network,” “keyword co-occurrence network,” and “burst detection” analyses, we identified core research clusters, their key topics, connections, and significant bursts. Then we synthesize all the main clusters' content through in-depth analysis. Our findings reveal the growing popularity of secondary research in AL, with substantial contributions from Asian countries despite Anglo-American dominance. Key disciplines influencing the AL's intellectual structure include Psychology, Neurology, Sociology, Education, and Computer Science. Four thematic strands anchor AL's conceptual structure: Language Education & Computer Science, Psycholinguistics, Neurolinguistics, and Sociolinguistics. The top-ranked co-cited authors by burst strength are Moher, Chomsky, and Botting. Using a data-driven theory of scientific revolution, we identify emerging research trends and track dynamic changes from an epistemic perspective. This dynamic perspective contributes to a more nuanced understanding of the evolution of the AL domain, signaling a new era of Critical Applied Linguistics. This study, an empirically-based overview of AL secondary research, offers important implications for future research.

Pharmacogenomics predictors of aromatic antiepileptic drugs-induced SCARs in the Iraqi patients.

Severe cutaneous adverse reactions (SCARs) are life-threatening and often linked to antiepileptic drugs (AEDs). Common types of SCARs include Stevens-Johnson syndrome (SJS), toxic epidermal necrolysis (TEN), and drug reaction with eosinophilia and systemic symptoms (DRESS). Immune-mediated mechanisms involving human leukocyte antigen (HLA) alleles have been implicated in the pathogenesis of this reaction. This study examines the association between specific HLA alleles (HLA-A, -B, and -DRB1) and AED-induced SCARs in the Iraqi population. A total of 50 patients diagnosed with SCARs and 90 tolerant controls were recruited from Dr. Saad Al-Wattari Hospital for Neurological Sciences and Baghdad Hospital - Medical City. HLA genotyping was performed using PCR-SSO method from peripheral blood samples. Statistical comparisons were made using the t-test or chi-square test, while univariate logistic regression with Bonferroni's correction (p<0.05) were used to assess associations between HLA alleles and SCARs. Among the patients, SJS was the most prevalent type of SCARs observed. Analysis of HLA allele frequencies revealed significant associations between specific alleles. HLA-A∗02:01 was found to be significantly associated with a lower risk of AED-induced SJS (OR=0.36; 95% CI: 0.13-0.97), while HLA-A∗24:02 and HLA-B∗15:02 were associated with an increased risk of AED-induced SJS (OR=3.60; 95% CI: 1.21-10.72 and OR=4.41; 95% CI: 1.18-16.47, respectively). For AED-induced TEN, HLA-A∗01:02, HLA-B∗15:02, and HLA-B∗52:01 showed significant associations (OR=6.92; 95% CI: 1.39-34.37 and OR=6.55; 95% CI: 1.62-26.52, respectively), with HLA-DRB1∗03:01 being highly significant (OR=5.09; 95% CI: 1.72-15.00). Additionally, HLA-B∗40:02 was strongly associated with AED-induced DRESS (OR=29.33; 95% CI: 3.50-245.32). This study identifies key HLA alleles associated with AED-induced SCARs in the Iraqi population. These findings could facilitate personalized medicine approaches, aiding in better prediction and prevention of SCARs in AED therapy.

Management of Velopharyngeal Impairment in Adults With Dysarthria: A Systematic Review.

Velopharyngeal impairment (VPI) is a common manifestation of dysarthria, yet there is uncertainty regarding the evidence for treatment options. The aim of this systematic review was to identify and evaluate published intervention studies of VPI and to update the original Practice Guidelines for VPI, as part of the Dysarthria Writing Group of the Evidence-Based Clinical Research Committee of the Academy of Neurologic Communication Disorders and Sciences. Four electronic databases were searched for articles published through September 2022 in accordance with Preferred Reporting Items for Systematic Reviews and Meta-Analyses standards. Multiple indices of methodological quality and risk of bias were used to evaluate studies. The search yielded 1,347 articles, of which 31 met inclusion criteria and were reviewed by two blinded co-investigators and a third consensus reviewer. Therapeutic approaches centered on prosthetic treatment (n = 13), prosthetic treatment combined with another form of therapy (n = 7), and behavioral exercise (n = 7), with four additional studies reporting unconventional approaches (e.g., use of medication). No single treatment approach garnered strong evidence. However, converging moderate evidence suggests positive treatment outcomes for prosthetic approaches for moderate-severe speakers with flaccid/spastic dysarthria and behavioral treatment approaches for speakers spanning a range of nonprogressive dysarthrias. There is a critical need for well-controlled treatment studies of VPI that incorporate comprehensive and systematic description of baseline speech function, treatment protocols, and operationally defined outcome measures, particularly related to social validity and participation goals. Bioelectronic medicine is a multidisciplinary field that combines molecular medicine, neurology, engineering, and computer science to design devices for diagnosing and treating diseases. The advancements in bioelectronic medicine can improve the precision and personalization of illness treatment. Bioelectronic medicine can produce, suppress, and measure electrical activity in excitable tissue. Bioelectronic devices modify specific neural circuits using electrons rather than pharmaceuticals and uses of bioelectronic processes to regulate the biological processes underlining various diseases. This promotes the potential to address the underlying causes of illnesses, reduce adverse effects, and lower costs compared to conventional medication. The current review presents different important aspects of bioelectronic medicines with recent advancements. The area of bioelectronic medicine has a lot of potential for treating diseases, enabling non-invasive therapeutic intervention by regulating brain impulses. Bioelectronic medicine uses electricity to control biological processes, treat illnesses, or regain lost capability. These new classes of medicines are designed by the technological developments in the detection and regulation of electrical signaling methods in the nervous system. Peripheral nervous system regulates a wide range of processes in chronic diseases; it involves implanting small devices onto specific peripheral nerves, which read and regulate the brain signaling patterns to achieve therapeutic effects specific to the signal capacity of a particular organ. The potential for bioelectronic medicine field is vast, as it investigates for treatment of various diseases, including rheumatoid arthritis, diabetes, hypertension, paralysis, chronic illnesses, blindness, etc.

The National Association of Comorbid Neurology Experts plays a key role in advancing neurological science and practice in comorbid conditions. The Association actively initiates and supports numerous research efforts to identify and study the interrelationships and interactions of various diseases affecting the nervous system. Through their efforts, the Association has become a leading platform for sharing best practices and knowledge in this field. International co-operation is one of the central strategies for the development of the Association. It allows integrating the achievements of world science and adapting them to solve the urgent problems of domestic medicine. Within the framework of the First International Council of Experts, the concept of development of the National Association of Experts in Comorbid Neurology was presented, as well as the expediency of concluding memoranda on international cooperation. International experts consider it advisable to recommend the widespread use of effective and safe drugs, including choline alphoscerate and chondroitin sulphate, by primary care specialists (district therapists, general practitioners) in general practice, starting from the early stages and in the progression of cognitive impairment and/or diseases of the musculoskeletal system.

Contemporary issues of the birth trauma

Background. Birth trauma is a group of diseases caused by mechanical factors during childbirth. Birth trauma is an interdisciplinary problem, affecting various fields of medicine, such as obstetrics, pediatrics, child neurology and other sciences. Birth trauma occupies an important place in the structure of perinatal morbidity and mortality. Birth injuries of newborns during vaginal birth occur in 3.6% of cases, during cesarean section — in 1.2%. However, at present, despite a comprehensive study of the problem, the incidence of birth trauma in newborns remains high. Along with obstetric birth trauma, spontaneous birth trauma also occurs — not associated with any obstetric care.Purpose of review. To summarize and analyze the available data on the causes of birth trauma in newborns depending on the type of birth injury.Materials and methods. The review includes published data over the past 10 years regarding the study of the causes of the main types of birth injuries in newborns. The literature search was conducted in Web of Science, Google Scholar, PubMed, and ELibrary databases.Results. The problem of fetal-pelvic disproportion in the genesis of the development of risk factors for maternal-fetal trauma remains relevant. Fetal macrosomia is one of the most significant factors in birth trauma. The most common skull injury in newborns is cephalohematoma. According to the mechanism of occurrence, birth trauma is divided into spontaneous — occurring during physiological childbirth — and obstetric — associated with any obstetric benefits. The degree of configuration of the fetal head plays a significant role in the pathogenesis of birth trauma. Birth trauma is considered as a systemic reaction on the part of the newborn’s body, which leads to a breakdown of compensatory and adaptive mechanisms and the development of critical damage to the central nervous system and is differentiated from birth damage, which includes only local changes.Conclusion. To search for preventive measures aimed at preventing birth injuries in newborns, it is necessary to systematize the groups of causes that influence the increased risk of birth injury. Timely assessment of the pathological configuration of the fetal head is extremely important when deciding on the advisability of using obstetric aids during vaginal operative delivery, since it is an early predictor of the formation of fetal-pelvic disproportion — a significant factor in birth trauma in the newborn. In recent years, taking into account the active study of molecular genetic mechanisms in the genesis of the formation of various pathological conditions, the search for various genetic and epigenetic factors that influence the risk of developing birth traumatic injuries in a newborn against the background of individual susceptibility to the effects of any physical factors during childbirth.

Impact of Stroke on the Quality of Life of its Survivors

Introduction: Ischemic Stroke is due to sudden loss of blood circulation to an area of the brain, resulting in loss of neurologic function. The incidence in India ranges between 105-152/100,000 people/year. Despite all the advancements in care, stroke continues to be the 2nd leading cause of mortality and the 1st cause of long-term disability. The activities of daily living of patients with chronic stroke affect their Quality of Life (QoL). The majority of stroke survivors continue to live with disabilities, and this can have a profound impact on their QoL. Aims &amp; Objectives: To assess the impact of stroke on the various domains of the quality of life of its survivors, who are treated by the Department of Neurological Sciences, Christian Medical College, Vellore. Methods: A descriptive study design employing a non-probability purposive sampling technique was used to recruit 90 stroke survivors. Written consent was obtained, demographic and clinical variables were collected using interviews, and the impact of stroke on its survivors was assessed using the Stroke Specific Quality of Life (SS-QoL) scale. Results: The participant's mean age group was 48.92±12.97 ranging between 26 -78 years respectively; 67.8% were males, 34.4% of them were unskilled workers, 66.7% had a thrombotic type of ischemic stroke, 56.7% had a moderate National Institutes of Health Stroke Scale grade at admission, 26.7% and 25.6% had a modified Rankin Score of 2 and 1, respectively. Among the participants, 47.8% of them had a stroke at the ages of 31-50 years, and the majority (91.1%) of them had comorbidities. The majority (44.77%) of them had a moderate QoL. Among the SS-QoL domains, the mean scores were the highest in the vision domain (4±1.26) and the lowest in the energy domain (2.47±1.25). There was a significant association between the QoL of stroke survivors and their socioeconomic status (p=0.033), type of ischemic stroke (p=0.047), and limb involvement (p=0.034). A positive correlation (p=0.00) was observed among various QoL domains. Conclusion: These findings provided not just insight into the impact of stroke on the QoL of its survivors but also emphasized the need to sensitize the healthcare care professionals to provide care during both the acute period and also continue to ensure all the domains of a good QoL is addressed during the follow-up and rehabilitation. Patisiran for the treatment of patients with p.Ile88Leu hereditary transthyretin amyloidosis: an Italian real-life experience.

Evidence on the activity of patisiran therapy in specific subgroups of patients with hereditary transthyretin amyloidosis variant (ATTRv) is still scarce. This prospective real-world study was designed to provide the first in-depth clinical data on the effectiveness of patisiran in patients with ATTRv reporting the p.Ile88Leu variant, the most widespread variant in the Emilia-Romagna regional area, which has been less represented in previous clinical trials. This prospective study evaluated all the patients with genetically proven ATTRv (p.Ile88Leu) and polyneuropathy treated with patisiran in the Emilia-Romagna referral centers for ATTRv (Institute of Neurological Sciences in Bologna and Division of Neurology in Rimini) from March 2021 to April 2023. All subjects underwent clinical and neurological evaluations at baseline and after 9-12 months of treatment. A total of 22 patients were included in the study; the median age was 73 years (IQR: 9), the age at diagnosis was 72 years (IQR: 10), and the disease duration was 1.6 years (IQR: 2.3). We observed stability of all considered neurological and cardiological parameters at 9-12 months after the beginning of patisiran treatment. Our findings support the clinical data regarding the effectiveness of patisiran in stabilizing the disease course and extend this activity to the subset of patients with the p.Ile88Leu variant.

The interplay between gut microbiota and the brain-gut axis in Parkinson's disease treatment.

This study delves into the pivotal role of the gut microbiota and the brain-gut axis in Parkinson's Disease (PD), a neurodegenerative disorder with significant motor and non-motor implications. It posits that disruptions in gut microbiota-dysbiosis-and alterations in the brain-gut axis contribute to PD's pathogenesis. Our findings highlight the potential of the gastrointestinal system's early involvement in PD, suggested by the precedence of gastrointestinal symptoms before motor symptoms emerge. This observation implies a possible gut-originated disease pathway. The analysis demonstrates that dysbiosis in PD patients leads to increased intestinal permeability and systemic inflammation, which in turn exacerbates neuroinflammation and neurodegeneration. Such insights into the interaction between gut microbiota and the brain-gut axis not only elucidate PD's underlying mechanisms but also pave the way for novel therapeutic interventions. We propose targeted treatment strategies, including dietary modifications and fecal microbiota transplantation, aimed at modulating the gut microbiota. These approaches hold promise for augmenting current PD treatment modalities by alleviating both motor and non-motor symptoms, thereby potentially improving patient quality of life. This research underscores the significance of the gut microbiota in the progression and treatment of PD, advocating for an integrated, multidisciplinary approach to develop personalized, efficacious management strategies for PD patients, combining insights from neurology, microbiology, and nutritional science.

This early description of torticollis in the book Pantagruel was an inspiration for the understanding of cervical dystonia. The art expressed in Rabelais' literature ‒ which was immortalized by the drawings of Gustave Doré ‒ influenced poetry, art, and photography, and led to the adoption of the term torticollis in the neurological sciences.

Exploring the Ethical and Technical Data of Machine Consciousness: Hazards, Implications, and Future Directions

The study of machine consciousness has a wide range of potential and problems as it sits at the intersection of ethics, technology, and philosophy. This work explores the deep issues related to the effort to comprehend and maybe induce awareness in machines. Technically, developments in artificial intelligence, neurology, and cognitive science are required to bring about machine awareness. True awareness is still a difficult to achieve objective, despite significant progress being made in creating AI systems that are capable of learning and solving problems. The implications of machine awareness are profound in terms of ethics. Determining a machine's moral standing and rights would be crucial if it were to become sentient. It is necessary to give careful attention to the ethical issues raised by the development of sentient beings, the abuse of sentient machines, and the moral ramifications of turning off sentient technologies. Philosophically, the presence of machine consciousness may cast doubt on our conceptions of identity, consciousness, and the essence of life. It could cause us to reevaluate how we view mankind and our role in the cosmos. It is imperative that machine awareness grow responsibly in light of these challenges. The purpose of this study is to provide light on the present status of research, draw attention to possible hazards and ethical issues, and offer recommendations for safely navigating this emerging subject. We want to steer the evolution of machine consciousness in a way that is both morally just and technologically inventive by promoting an educated and transparent discourse.

107 A pragmatic approach to portable neuroimaging utilized in clinical research

OBJECTIVES/GOALS: The aim of thispaper is to raise awareness of the limitations of the current pMRI training paradigm and to recommend a standardization of skills to expand diversity among field-based neuroimaging technicians. METHODS/STUDY POPULATION: Currently, there are seven international brain research initiatives. The goal is to establish and understand the cultural values a society holds and how the outcomes of research may be adopted into societal practice. We must also consider the benefits of early detection amongst minoritized communities in neuroscience research. Neuroimaging in remote settings can allow patients to advocate more accurately for timely medical care which can lead to better health outcomes. According to the Journal of Neurological Sciences, neuroscience accounts for 9% of total medical publications. RESULTS/ANTICIPATED RESULTS: Neuroimaging research continues to evolve in terms of resolution and portability. T By harnessing diverse data, we are able to utilize advanced neuroimaging techniques and software technology to recognize trends amongst subgroups of individuals who were previously considered geographically inaccessible. DISCUSSION/SIGNIFICANCE: Despite the excitement and promise of portable neuroimaging devices, there is a fundamental need to establish standardized training procedures that can be accessed by individuals of all socioeconomic backgrounds.

Advances in Cerebral Small Vessel Disease: Sandra E. Black Lecture to the Canadian Neurological Sciences Federation.

Cerebral small vessel diseases (CSVDs) are among the most common age-related pathologies of the brain. Arteriolosclerosis and cerebral amyloid angiopathy (CAA) are the most common CSVDs. In addition to causing stroke and dementia, CSVDs can have diverse covert radiological manifestations on computed tomography and magnetic resonance imaging including lacunes, T2-weighted white matter hyperintensities, increased density of visible perivascular spaces, microbleeds and cortical superficial siderosis. Because they cannot be visualized directly, research on the pathophysiology of CSVD has been difficult. However, advances in quantitative imaging methods, including physiological imaging such as measurement of cerebrovascular reactivity and increased vascular permeability, are beginning to allow investigation of the early effects of CSVD in living people. Furthermore, genomics, metabolomics and proteomics have the potential to illuminate previously unrecognized pathways to CSVD that could be important targets for new clinical trials.

The use of elements of neuropedagogy in the creation of virtual simulators for in-depth study of chemistry in higher education

This study investigates the efficacy of incorporating neuropedagogical elements into virtual simulators for advanced chemistry education in universities. Utilizing a mixed-method approach, it uses questionnaires completed by 50 organic chemistry students. These questionnaires included both general and Likert scale questions, focusing on students' experiences with the PhET Interactive Simulation. The results highlight the positive impact of neuropedagogy on students' learning attitudes, cognitive abilities, and skill development, particularly in areas like critical thinking, analytical skills, and social competencies. A significant portion of students reported enhanced educational outcomes: 76% observed improved training effectiveness, 88% better understood the subject, and 96% found the virtual simulator-based learning more engaging. Furthermore, the study notes improvements in students' problem-solving abilities (68%), logical reasoning (92%), and comprehension of chemical processes (94%). These findings emphasize the value of integrating neuroscience principles in chemical education, potentially benefiting both tertiary and secondary education sectors. They indicate the necessity for educational adaptation in line with interdisciplinary research in cognitive and neurological sciences. This research not only serves as a valuable resource for chemistry teachers but also sets the stage for future empirical studies exploring neuroscience's role in teacher education. The study underscores the importance of further investigation into how teachers implement neuropedagogical techniques and the effectiveness of such applications, advocating for continuous development in educational methodologies.

Infant massage is one of the tactile stimulation and has become a hereditary tradition and has been studied in research in the fields of neonatal science, neurology, psychology, and several other health sciences. The incidence of hyperbilirubinemia in general is a physiological problem that occurs in almost 80% of newborns with prematurity and up to 60% of aterm newborns and occurs in the first week of life. This study aims to determine the implementation of massage therapy in neonates to reduce bilirubin levels. The research method used literature review by searching articles obtained through databases including Science Direct, Elsevier and Google Scholar. The results showed five articles involving neonates in five countries with a randomized control trial (RCT) design, stating that infant massage therapy is the most influential in reducing the degree of neonatal jaundice in the group given massage therapy because it can stimulate defecation. In addition, the provision of physiological massage therapy can improve the circulation of blood flow, lymph and tissue fluids thereby increasing bilirubin output.

Morphometric Assessment of Sellar Turcica: Tertiary Neurocenter study from Nepal.

Introduction: The sella turcica (ST) is a vital structure located in the median position within the middle cranial fossa and is frequently utilized in cephalometric for assessing craniofacial features. However, there is limited literature available regarding cephalometry of the ST in Nepal. Consequently, this study aimed to establish the normal dimensions of the ST in the Nepalese population and explore the potential difference in radio-morphometric measurement among sexes. Materials &amp; Methods: This study encompassed a descriptive, cross-sectional chart review conducted at the Upendra Devkota Memorial National Institute of Neurological and Allied Sciences over a three-month period. Morphometric assessment of the ST was performed using computed tomography scans. A single radiologist employed an EV Insite DICOM viewer to measure and record the length, depth, and antero-posterior diameter of the ST. Independent-Samples t-test was employed to compare the means between sexes, and a significance level of p &lt; 0.05 was considered statistically significant. Results: Among the 385 cases analyzed, 199 were male (51.8%) and 185 were female (48.2%), resulting in a male-to-female ratio of 1.07:1. The age range of the patients was 20 to 87 years, with a mean age of 44.5 ± 18.5 years. The mean dimensions of the sella turcica were determined as 9.32 mm for length, 7.07 mm for depth, and 10.29 mm for the antero-posterior diameter. Conclusions: The findings of this study indicate that the ST dimensions in the Nepalese population are relatively smaller compared to those reported in various other countries. Additionally, no statistically significant differences were observed in ST dimensions between males and females within the Nepalese population. These results emphasize the importance of considering populationspecific variations when utilizing cephalometric measurements and provide valuable reference standards for future studies and accurate assessment of craniofacial morphology in the Nepalese population.

Utilization of neurological dominance science (NDS) to enhance students’ creativity in communication as implementation outcome based education (OBE)

This study aims to investigate the contribution of the Neuro Dominance Science (NDS) program in improving students' creative communication skills and the completion of six tasks as a result of the Indonesian National Qualifications Framework called KKNI, with six (6) tasks of KKNI. This study uses qualitative and quantitative method in analyzing the data. The participants in this study were 50 students of English language and Literature Department, Universitas Negeri Medan that completed NDS workshop as one of the programmed of Creative Innovative Communication (CIC) subject at the fourth Semester. Data was collected through a questionnaire made in the form of a Google form to measure students' creative communication abilities and completion of KKNI assignments. The results of this study indicate that the NDS program has a significant contribution to improving students' creative communication skills. Quantitative data analysis shows an increase in scores on creative communication assessment after students join the NDS program. In addition, the NDS program also assists students in completing the six tasks required by the KKNI, such as problem-solving, critical thinking, and collaboration. This research provides valuable insight into the contribution of the NDS program in improving students' creative communication skills and completing KKNI assignments. The results of this study can be used by policymakers and educators to consider implementing the NDS program in schools as an effort to improve students' skills in creative communication and achievement of KKNI tasks.

Chemical Characteristics and Biotechnological Potentials of Mumio

Mumio is a valuable substance that ranges in color from pale brown to blackish-brown and is obtained from rock layers in various mountain ranges worldwide, particularly in the Tien Shan ranges of Kyrgyzstan. It is also a natural chemical found largely in the Himalayas, created over ages by the progressive degradation of particular plants by bacteria. Mumio is essentially a nutraceutical product with considerable human health advantages. To fully appreciate the predicted impact of mumio use in the medical area, particularly in neurological sciences, more research at both the fundamental biology level and through well-designed clinical trials is required.
