Frequency of the variant allele CYP2D6(C) among North American Caucasian lung cancer patients and controls

Previous reports of the association between the debrisoquine polymorphism and lung cancer risk are conflicting. Following the report of an association between lung cancer risk and the variant allele CYP2D6(C), we examined the presence of this allele in 98 incident Caucasian lung cancer patients and 110 age, race, and sex matched hospital controls from a case-control study conducted at the National Naval Medical Center in Bethesda, MD. Debrisoquine metabolic phenotype was determined by debrisoquine administration and analysis of debrisoquine and 4-hydroxydebrisoquine in the subsequent 8 h urine collected. Genomic DNA was genotyped by a specific polymerase chain reaction amplification and subsequent restrictionenzyme digestion, and Southern analysis. Twenty subjects were heterozygous for the CYP2D6(C) allele but none were homozygous for this allele. There was no significant difference in frequency of CYP2D6(C) between lung cancer patients and controls (5.61% and 4.09%, respectively), and there was no significant heterogeneity among cases by histologic type of lung cancer ( P = 0.08). However, 7 of 11 cases (64%) with the CYP2D6(C) allele had small cell lung cancer, and none had squamous cell carcinoma. Carrying the CYP2D6(C) allele did not impair debrisoquine metabolism to the same degree as the known inactivating mutations, CYP2D6(A) and CYP2D6(B), or deletion of CYP2D6. Thus, the CYP2D6(C) allele does not encode a completely inactivating mutation, and the suggestion of a role for this variant allele in the risk for specific histologic types of lung cancer justifies further investigation.

Epidemiology, Preventive Services, and Illnesses of International Travelers

Medical preparation of travelers to overseas locations is an important part of military medical care. We reviewed pre-travel records of patients attending the travel clinic at the National Naval Medical Center (NNMC) and used a post-travel questionnaire to determine the most frequent medical problems associated with international travel. Among 1,416 individuals who received pre-travel care at NNMC, there were 760 (54%) males and 656 (46%) females, the median patient age was 48 years, the most common reason for travel was pleasure, and the median duration of travel was 21 days. The most common destinations were Asia (27%), Africa (15%), Europe (13%), Central America/Caribbean (12%), and South America (11%). The median number of immunizations prescribed was three. Malaria chemoprophylaxis was prescribed to 45%. The average cost of vaccines and medications to medically prepare a traveler was $67. Among 271 (82%) who returned the post-travel questionnaire, the most common illnesses reported were diarrhea (23%) and upper respiratory infections (19%); medical treatment was sought by 9%. Properly informed, military physicians can provide a valuable service at a reasonable cost to reduce the risk of travel-acquired medical problems and illnesses.

Germ cell testis cancer: 15-year review.

Testis cancer affects 2-3 men per every 100,000 in the United States, making it the most common solid tumor of men in the 20-35-year-old age range. Since the average age of active duty military personnel is included in the age range of those at greatest risk for germ cell testis cancer, it is of pertinent clinical importance to physicians who treat these young patients. The National Naval Medical Center has been using cisplatin-based protocols since the time of their introduction. This study reviews the success of treating these patients and examines the treatment failures.

Delayed Diagnosis of Large Cerebellopontine Angle Tumors, Despite Hearing Conservation Training

Recently, at the National Naval Medical Center in Bethesda, Maryland, we have encountered several patients with large cerebello-pontine angle tumors. All were active duty and had evaded the military's stringent hearing screening program until the tumors were of life-threatening size. Early detection of these tumors can reduce overall morbidity and improve the patient's chances of returning to a normal lifestyle. We have analyzed each of these cases and discovered weaknesses in the program. The primary difficulty was in accurately following the algorithms set forth by military regulations. To that end we recommend: (1) establishing a cutoff for identification of those who should be evaluated after they obtain their enlistment/annual physical examinations; (2) specific training for hearing conservation program managers in the algorithms for hearing screening; (3) establishment of specific guidelines for primary care physicians for further evaluation of abnormal audiograms; and (4) reevaluation of form DD 2216.

Measures of Effectiveness: A Methodology of Integrating Planning, Measurement, and Continuous Improvement

In this article we discuss a management construct, called Measures of Effectiveness (MOEs), used by executive leadership at the National Naval Medical Center in its performance of its oversight functions. Three case studies are presented illustrating the usefulness of MOEs for planning, for performance measurement, and for making determinations regarding initiation of formal quality improvement efforts.

Current Management of Carotid Atherosclerosis

Carotid atherosclerosis affects thousands of individuals each year. A history of transient ischemic attacks should alert the clinician to the possibility of carotid artery disease. Carotid endarterectomy is of proven benefit in both symptomatic and asymptomatic individuals, with angiographically confirmed carotid stenosis of from 70 to 99%, respectively. Guidelines for the diagnostic and preoperative screening procedures as well as the surgical technique used by the Department of Neurosurgery at the National Naval Medical Center are reviewed.

Memories Remain of Rwandan Medical Mission

RWANDA IS gone from the news for the moment. But the region remains in the memories of physicians who helped size up the situation for US military medicine early in the crisis. Among these physicians are Ann P. Fallon, MD, MPH, a US Navy lieutenant commander stationed in Naples, Italy, pending assignment to the National Naval Medical Center, Bethesda, Md, and Michael E. Neuland, MD, a US Air Force colonel who is chief of clinical medicine, Ramstein Air Base, Germany. If their memories of the area were not vivid enough, the medical, refugee, and political problems that continue to simmer in Rwanda and in neighboring Burundi and Zaire doubtless would be constant reminders. Viral Imitations of Host Defense Proteins

CASE PRESENTATION A 40-year-old Indian woman with Takayasu's arteritis presented to the National Naval Medical Center with chest pain of 1 day's duration. One year previously, she had presented with malaise, diminished left upper extremity pulses, and an elevated erythrocyte sedimentation rate. A diagnosis of Takayasu's arteritis had been made by angiographic demonstration of stenosis of the left subclavian artery. She had been treated initially with 1 mg/kg per day of prednisone with angiographically evident improvement, and the drug dosage had been tapered over the past year to 10 mg of prednisone every other day. On the day of admission she noted constant, sharp left chest pain without radiation and without dyspnea. Her past medical history was otherwise unremarkable. The physical examination revealed a woman who appeared well nourished. Her temperature was 38.5°C orally. Her heart rate was 92 beats per minute, and her blood pressure was 126/78 mm Hg.
