Na + in cystic fibrosis: is it important?

Research into cystic fibrosis has progressed rapidly over the past few years with, in 1989, the isolation of the cystic fibrosis gene1 and, two years later, the identification of the gene product, cystic fibrosis transmembrane conductance regulator (CFTR), as a Cl- channel?. It is a defect in this channel which is thought to lead to the respiratory morbidity in cystic fibrosis and attempts are presently underway to correct this defect by transfecting cystic fibrosis airway epithelial cells in viva with the normal CFTR gene. Although this approach may yet prove to be successful there are several questions about ionic movements across cystic fibrosis airway epithelium which remain unanswered. In addition to the reduced apical epithelial Cl- flux in cystic fibrosis3 there is a two to threefold increase in the rate of Na+ absorption across airway epithelial cell+ (Fig. 1). It is this increase in Na+ reabsorption which results in an increase in the transepithelial nasal potential difference found in cystic fibrosis6 and, in conjunction with the movement of water, may be responsible for the dehydration of airway surface liquid, predisposing the lungs to chronic pulmonary infections and ensuing bronchiectasis7,s. Antigen reduces nasal transepithelial electric potential differences and alters ion transport in allergic rhinitis in vivo.

The change of ion transport in acute allergic reactions in vivo was studied by measuring the nasal transepithelial potential difference (PD) in patients with nasal allergy to Japanese cedar pollens. Comparison of nasal PD in the pollen season revealed a lower PD in the allergic patients than in the normal control subjects. We challenged the patients with allergen in the non-pollen season and measured the time course change of nasal PD and rate of inhibition of PD by amiloride and indomethacin. Nasal PD reached the lowest value 15 min after nasal allergen challenge. Percent inhibition of PD by amiloride was greater without the allergen challenge than it was in those patients after allergen challenge (51.7% versus 29.4%, p less than 0.01). Indomethacin did not change PD without allergen challenge, whereas it depressed nasal PD by 25.1% after allergen challenge. These results suggest that decreased sodium absorption and increased chloride secretion occur in local allergic reactions. Both changes may contribute to the increase in fluid transport towards the lumen, and this may lead to abnormalities of nasal secretion during acute allergic reactions.

Increased nasal potential difference and amiloride sensitivity in neonates with cystic fibrosis

Patients with cystic fibrosis (CF) have an increased nasal transepithelial potential difference (PD) which reflects increased sodium absorption across epithelium relatively impermeable to chloride. To evaluate nasal epithelial function in neonates with CF, the PD was recorded and the voltage response to superfusion of 10(-5M) amiloride, an inhibitor of sodium transport, measured between a Ringer perfused bridge on the nasal mucosa and a reference electrode in the subcutaneous space. We studied three neonates with CF with meconium ileus and compared the results with those in 24 term healthy neonates, including one obligate heterozygote for CF, and 27 control neonates with disease. All three CF neonates had raised sweat chloride values (mean 100 mEq/L) at 2 months. The CF neonates had higher PDs (-64.0 +/- 8.4 mV) than those in normal (-24.4 +/- 2.0 mV) or control (-25.8 +/- 2.0 mV) neonates. Superfusion with amiloride induced a 72% reduction in PD in the CF neonates as compared with healthy (37.5 +/- 1.0%) and diseased (36.0 +/- 1.3%) neonates. The PD and amiloride response in CF neonates are similar to those in CF infants (2.24 months), older CF children (greater than 6 years), and CF adults (-64.9 +/- 9.3 mV; 77.7 +/- 1.8%, n = 51). These results suggest that (1) nasal epithelial dysfunction is present in patients with CF shortly after birth, and (2) the nasal PD may be a diagnostic adjunct to the sweat test in the early diagnosis of CF.

1767 INCREASED NASAL POTENTIAL DIFFERENCE AND AMILORIDE SENSITIVITY IN NEONATES WITH CYSTIC FIBROSIS

Cystic fibrosis (CF) patients have an increased nasal transepithelial potential difference (PD) which reflects an increased Na+ absorption across a relatively Cl− impermeable membrane. To evaluate respiratory epithelial function in CF neonates, the maximal PD and the voltage response to superfusion of 10−5M amiloride (Am), an inhibitor of Na+ transport, were recorded between a Ringer perfused bridge on the nasal mucosa and a reference electrode in the subcutaneous space. We studied two term CF neonates presenting with meconium ileus (MI) and compared the results to 15 term healthy neonates including 1 obligate heterozygote (OH) for CF, and 12 term disease control (DC) neonates including 2 with ileal atresia, 2 with meconium aspiration, 1 with pneumonia, and 7 with Transient Tachypnea of the Newborn. Both neonates with MI had raised sweat Cl− values (x = 110meq/l) at 2 mo. The CF neonates had higher PDs than normal or disease controls (fig). Superfusion with Am induced a 72% reduction in PD in the CF neonates as compared to healthy (40.0 ± 3.7%) and disease (41.0 ± 2.6%) neonates. Also the neonatal PDs are comparable to values in older infants (2-24mo) where a similar PD pattern is present (fig). Moreover, the PD and Am response in CF neonates are similar to older (>6yr) CF children and adults (-64.9.+9.3mV; 77.7±1.8%, n=51). These results suggest that: (1) nasal epithelial dysfunction is present in CF patients shortly after birth, and (2) the nasal PD may be a diagnostic adjunct to the sweat test in the early diagnosis of CF.

Measurements of nasal transepithelial electric potential differences in normal human subjects in vivo.

Nasal transepithelial electrical potential difference (PD) was estimated in vivo in normal human subjects. The PD between a bridge perfused with Ringer's solution placed on the nasal mucosal surface and a reference electrode in a subcutaneous space was recorded. The magnitude of the PD depended upon the region of the nasal cavity touched by the exploring bridge. The PD values ranged from -4 mV (lumen negative) on the anterior tip of the inferior turbinate to approximately -30 mV on the inferior surface of the turbinate. Biopsies of the anterior turbinate were characterized by squamous epithelial cells, whereas ciliated cells dominated the epithelium from regions of higher PD. Epithelial abrasion abolished PD. No differences in PD were noted between the left and right nasal cavities, and repetitive measurements of PD at the same site yielded comparable values. No differences in nasal PD were noted between young male or female subjects but PD declined as a function of age. Amiloride (10(-4)M) and lidocaine (10(-1)M) reduced PD (-59% and -30%, respectively), whereas amphotericin B (5 X 10(-5)M) raised PD (+29%). We concluded that nasal PD can be accurately measured in vivo in humans, and that measurements of nasal bioelectric properties may serve as a relatively noninvasive index of respiratory epithelial function.
