Characteristics of patients leaving the emergency department without being seen by a doctor: The first report from Türkiye.

The presence of patients who leave without being seen (LWBS) by a physician in the emergency department (ED) is an essential indicator of the accessibility and quality of healthcare delivery. A high LWBS rate implies low patient satisfaction. This study aims to analyze the prevalence and characteristics of LWBS patients in Türkiye and their ED readmission rate within 3 days. This retrospective, single-center, observational study was conducted in the ED of a Turkish tertiary university hospital. It looked at patients who left without being evaluated by a physician admitted to the ED between June 01, 2021, and June 01, 2023. Data on age, gender, residential area, health insurance status, ED readmission within 3 days, complaints, and hospitalization were obtained through the hospital's electronic record system. The prevalence of LWBS patients was 0.43%. Only 0.6% (n = 5) of LWBS patients had no health insurance. About 12.8% (n = 99) of them were 65 years and older. About 10.2% (n = 79) of LWBS patients were readmitted to the ED within 72 hours. The most common reason for readmission was musculoskeletal system complaints. Statistically, ED readmission was significantly more common in older adults (P = .05). Older LWBS patients are more likely hospitalized upon return visits to the ED (P = .014). LWBS patients are both a result and a consequence of ED crowding. A zero LWBS rate is crucial for older adults' health as they may fall victim to an invisible accident while avoiding ED traffic. Prioritizing older adults and reorganizing waiting areas can reduce ED crowding and cost burden due to delayed treatments.

<i>Lampit</i> Seating Design Reduce Musculoskeletal Complaints in Semaja Tabanan Bali Indonesia

<i>Lampit </i>is one of the customary apparatuses for handling rice fields to even out the land for establishing rice. The activity caused different objections in the outer muscle framework, particularly in the rump and back by reason of the little size and hard stick of<i> the lampit</i>. Improvements were made to overcome this situation by designing a flat seat with foam pads and adjusting the press lever found on the attached rod. This experimental research study used the same subject design, chose 30 examination subjects from ranchers in Dusun Semaja Bengkelsari, Tabanan Bali. The musculoskeletal system's complaints were obtained by using the Nordic Body Map which was measured before and after repairing the <i>lampit </i>design. The data were contrasted and the mean qualities when improvement, analyzed using the paired t-test (α = 0.05). The result showed a decrease in the musculoskeletal system by 51.52% (p <0.05). It could be concluded that by repairing the design of the <i>lampit</i> reduced complaints of the musculoskeletal system, it is recommended to continue to use and improve the <i>lampit</i> that has been repaired for farmers.

The Investigation of musculoskeletal disorders, sleep quality, and fatigue of frontline healthcare workers during the COVID-19

The purpose of the study is to compare musculoskeletal system complaints, fatique, and sleep quality in front-line and second-line healthcare worker trating COVİD-19 patients. 152 healthcare worker between the ages of 18 and 65 who worked in outpatient clinics, services, COVİD-19 units, or administrative management were included in this study. Healthcare worker who had mission in the diagnosis, treatment, and isolation of patients diagnosed with COVİD-19 were determined as front-line workers (26F, 45M) and healthcare worker who didn’t have this kind of mission were defined as second-line workers (40F, 41M). After the collection of the participants “demographic data” the Nordic Musculoskeletal Questionnaire (NMQ), the Fatique Severity Scale (FSS), and the Jenkins Sleep Scale (JSS) were used to assess the participant musculoskeletal pain, fatique, and sleep quality, respectively. A significant difference was found in the training time, job experience, and number of weekly shifts of healthcare workers working on the front-line healthcare workers compared to operating the second-line healthcare workers (p≤0.05). Front-line healthcare workers reported more neck and back pain, while second-line healthcare workers reported more lower back pain. It was found that front-line healthcare workers had higher levels of fatique severity (t(150): 4.264, p≤0.001, Cohen’s d:0.69), lower levels of sleep quality (t(150): 4.479, p≤0.001, Cohen’s d:0.72). Shift programs should be arranged to reduce fatique and sleep problems of front-line healthcare workers, and protective neck and back muscles exercise programs should be given for front and second-line healthcare workers.

Anti-nuclear antibody testing in children: How much is really necessary?

Anti-nuclear antibody (ANA) testing is most commonly ordered by general pediatricians to evaluate children with musculoskeletal system complaints. Given the limited utility of the test, we aimed to estimate the effectiveness of ordering ANA testing in childhood. Children referred to our department to be examined due to positive ANA findings between 2008 and 2020 were included in the study. Those with less than one-year follow-up period, those with previously known rheumatic or autoimmune disease, and those diagnosed as an autoimmune and/or rheumatic disease at the first visit were excluded. Data were obtained from their medical records, retrospectively. The parents of all of the patients were called via phone, data were verified, and missing information was collected. Three hundred and fifty-eight patients (230 females) were eligible for the study. The median age of positive ANA findings was 9.31 (1.3-17.86) years and the median follow-up duration was 4.85 (1-11.91) years. Most of the patients had no underlying disease (n=337, 94.1%). The most common reason for ordering ANA testing was to evaluate musculoskeletal system symptoms (n=225, 62.8%). None of our patients referred to us due to positive ANA findings developed any autoimmune conditions or ANA associated rheumatic disease. Hypermobility syndrome is the most common final diagnosis among our ANA-positive patients. We suggest that instead of using it as a screening tool, ANA testing should be performed only if there is a strong suspicion of autoimmune diseases or certain rheumatic conditions, such as systemic lupus erythematosus.

A relationship between musculoskeletal pain and prognosis in hospitalized COVID-19 patients

Introduction: Musculoskeletal system complaints are often encountered in patients with Covid-19. The aim of this study was to evaluate the frequency of symptoms such as arthralgia, myalgia, and arthritis in hospitalized patients and their relationship with the final prognosis. &#x0D; &#x0D; Methods: Complaints related to myalgia, arthralgia, arthritis-like symptoms, laboratory parameters, VAS scores and localized painful areas of 154 hospitalized patients who were treated with a COVID-19 diagnosis were recorded on admission and during their hospitalization period. The relationship between these clinical and laboratory data and the duration of hospital stays, need for intensive care and death-recovery states was evaluated. &#x0D; &#x0D; Results: Of 154 cases, 45.5% (n=70) were female, 71.4% (n=110) had myalgia while 55.8% (n=86) had arthralgia. Mean VAS value was 6.39±2.04. The most commonly reported painful locations were dorsum in 68.2% (n=75) and chest in 63.6% (n=70) of the patients. The death rate was significantly higher in patients with dorsum pain. 25-0H-Vitamin D levels did not have a significant effect on the prognosis and in terms of needing intensive care. &#x0D; &#x0D; Conclusion: Myalgia and arthralgia are present in a significant part of patients with a diagnosis of Covid-19. Pain localized in the chest and dorsum area is associated with bad prognosis.

Subjective pain relief experienced in Musculoskeletal System (MSS) Disorders by elderly patients on homoeopathic medicines: An observational study

Objectives: To investigate the role of individualized homoeopathic medicines in treatment of the Musclo Skeletal Disorders (MSS) of the elderly.Materials and Methods: An Observational cohort study was done by the secondary data analysis of 278 patients (age group of 60-65 years) visiting the Homoeopathic hospital from a period of 10 years (2000- 2010) to observe the subjective relief with homoeopathic medicines in the Musclo Skeletal System Disorders (MSS).Result: In the study after using homoeopathic medicines, 7.32% of patients reported 25% relief, 35% of patients had 50% relief, 17% patients were better by 75%, 14% patients were free of pain and ease in mobility, while 16% patients reported no relief. Homoeopathic medicines like Kali Carb (27.3%), Silicea (21.2%), and Calcarea Flour (13.7%) were the most indicated remedies in the elderly patients with MSS complaints and centesimal scale (30C - 38% of patients & 200C - 59% of the patients) was the most common potency scale used.Conclusion: Homoeopathy is effective in offering subjective sense of relief in elderly patients from their MSS complaints.

The effect of the COVID-19 Pandemic on anxiety, depression, and musculoskeletal system complaints in healthcare workers.

COVID-19 increased psychological and physical complaints, including anxiety, depression, and pain. We aimed to examine anxiety, depression, and musculoskeletal complaints that may occur in healthcare workers during the COVID-19 pandemic. Data were cross-sectional, collected by the online questionnaire method consisted of demographic questions, Generalized Anxiety Disorder (GAD-7), Patient Health Questionnaire (PHQ-9), Nordic Musculoskeletal System Query (NMQ), and Visual Analogue Scale (VAS). Among the groups, the number of doctors in the frontline group, the fear of being infected, the number of tests performed, and personal protective equipment were higher than the second-line group (p < 0.05). It was found that 73.4% of the participants had anxiety, 83.5% had depression, and 94.9% had musculoskeletal pain. It was observed that there was no statistically significant difference between the applied scale scores between the groups (p > 0.05). About musculoskeletal system complaints of the participants, neck pain presence was determined as 73.4% in the last 12 months, and upper back pain was the highest with 61.4% in the last week. Neck (p < 0.005) and upper back pain (p = 0.04) parameters in the last seven days of the NMQ scale applied to the participants were found to be statistically significantly higher in the contact group. Anxiety, depression, and musculoskeletal pain of healthcare workers were found to be high. While neck and back pain was the most common in musculoskeletal complaints, it was found that these pains were higher in the frontline group. In this regard, healthcare professionals can be given psychological support and exercise training.

The evaluation of low back pain and perceived stress among prehospital emergency care workers

Abstract Background Low back pain (LBP) is one of the most common musculoskeletal system complaints in Prehospital Emergency Care workers consisting of paramedics, emergency medical technicians and doctors. Since their high stress job also includes physical work, such as lifting and bending, it's important to address the LBP. The aim of the study was to evaluate the low back pain and perceived stress in prehospital emergency care workers. Methods This is a cross-sectional study conducted on Eskişehir prehospital emergency care workers in 2020. The study group consisted of 372 (77.5%) personnels out of 480. The questionnaire form included sociodemographics, factors related to low back pain, Back Pain Functional Scale (BPFS) and Perceived Stress Scale (PSS). Scoring high on BPFS implies function loss due to LBP while it means high stress for PSS. Mann Whitney U, Kruskal Wallis and Spearman correlation analyses were used to determine the relationship between the variables. Results Females made up 55.1% of the 372 participants. The mean age was 32.15±7.5 and it ranged from 18 to 60. In the study group, 280 (75.3%) reported to have low back pain. Among those with LBP, 133 (47.5%) stated that they have had LBP for less than 5 years, 194 (69.2%) workers reported lifting is the most common reason for their LBP and 158 (56.4%) expressed that they've seen a doctor to deal with the LBP. The prehospital emergency care workers scored between 5 and 60 from BPFS, the median score was 49.0 while the mean was 45.74±12.09. There was a weak negative correlation between perceived stress and BPFS (R=-0.187, p &amp;lt; 0.001). Conclusions LBP was a common complaint among prehospital emergency care workers. Those who are older than 30, have chronical disease(s), had a family history of LBP, had an injury/trauma, had LBP for more than 5 years and those who worked 24 hour shifts scored less on the FBPS implying a funcionality loss due to LBP. Key messages Prehospital emergency care workers are overworked and it takes a toll on both mental and body health. Educating the workers to create a better understanding on lifting and bending may help them with low back pain.

Effect Of Dentist Working Conditions On Occupational Musculoskeletal System Diseases

The profession of dentistry that has the obligation to work in the same position for is at risk of some occupational diseases which develops due to the use of musculoskeletal system in inappropriate forms. Some health problems such as mechanical pain, allergy, infection, dermatitis are frequently seen in these occupational diseases.In present study the musculoskeletal system problems of dentists working in Oral and Dental Health Centers in Sakarya, Bilecik ,Bozüyük and factors affecting these complaints were investigated. This was a descriptive type of study. The data was collected of 83 dentist, by the investigator between April 2017-May 2017 with Extended Nordic Musculoskeletal Questionnaire. Of 83 dentist. Involved in study 50.6% were male. Mean age of participants was 37,81±8,23 years. Musculoskeletal system complaints among the dentists participated in the study were %79,5% in last 12 months, %69,9% in last month and %48,2% in the day of questionnare. In the last 12 months the prevelance of musculoskeletal system complaints were most frequently seen at neck region (78.3%), low back region (56.6%) and upper back region (55,4%) respectively. The frequency of musculoskeletal system problems in the last month was higher among females (p=0.001),working time is over 15 years (p=0,019), working in bending position constantly (p=0,013).Those who evaluate the working environment as risky complain about painful areas twice higher than those who do not evaluate the working environment as risky. It is thought that arrangement of in service training to establish occupational risk and behavioural changes against job work, working position and life style of dentists will be useful.

The musculoskeletal system complaints of office workers at a vehicle production factory

This study aimed to determine the prevalence of and factors contributing to musculoskeletal system complaints (MSC) in office workers of a vehicle production factory and absenteeism due to these complaints. A total of 86.2% (n = 333) of the workers completed the survey. Of the workers, 73 (21.9%) were women, and 260 (78.1%) were men, and the mean age was 36.9 ± 8.75 years. The majority of the personnel had more than one symptom with a prevalence of 80.8%. The most frequently observed discomfort was tension and pain in the back muscles (58.6%) and in the neck muscles (58.3%), followed by low back pain (41%) and shoulder pain (30.3%). Pain in the shoulders, neck, and the back and pain and numbness in the fingers were statistically significantly more prevalent in employees who spent more than 5 hours per day performing desk work. Furthermore, all of the complaints, excluding pain in the elbows and knees, were statistically significantly more common in the women. When each complaint and the effects of contributing risk factors were analyzed by logistic regression analysis, pain in the shoulders, neck, and back; pain that spread to the arms; and pain in the fingers were correlated with longer work hours and the female gender (p < 0.05). The number of employees who had used sick leave within the past year due to MSCs was 11.7%. In conclusion, duration of work hours, frequency of breaks, and gender were determined as the most important risk factors in computer users regarding MSCs. © 2011 Wiley Periodicals, Inc. © 2011 Wiley Periodicals, Inc.

Prevalence of work-related musculoskeletal disorders in Brazilian hairdressers

There are occupational risks inherent to the activities of professional hairdressers, which are not frequently studied, and therefore not considered in the formulation of health policies for this group. To verify the prevalence of work-related musculoskeletal disorders (WRMDs) in hairdressers through symptom reports, to characterize the most frequently affected anatomical parts and to identify and analyse risk factors of WRMDs in hairdressing. A cross-sectional epidemiological study of 220 hairdressers from beauty parlours in São Paulo (Brazil) was carried out. Each hairdresser completed a self-administered questionnaire which included information on socio-demographic characteristics, working conditions and health-related musculoskeletal system complaints. Ergonomic analyses were also performed in six parlours. The prevalence of WRMDs was 71%. Risk factors were associated with psychosocial factors and factors related to discomfort and work fatigue such as lack of acknowledgement of work and uncomfortable posture at work [odds ratio (OR) = 3.54; 95% confidence interval (CI) 1.51-8.30], not feeling comfortable with body/neck/shoulders while working (OR = 2.78; 95% CI 1.40-5.54) and having >15 years of professional activity (OR = 3.04; 95% CI 1.17-7.91). Occupational risk factors associated with the development of WRMDs in hairdressers are related to biomechanical, organizational and psychosocial work factors. The high prevalence of WRMDs found highlights the importance of disseminating recommendations for prevention of symptoms with regards to the provision of suitable furniture, equipment and work tools, environmental conditions, size of workplace, work organization and psychosocial work factors.
