Multiple Glomus Tumors of the Skin

Glomus tumors are divided into two types: (1) the classical, painful, solitary type; and (2) multiple painless hemangiomas. Forty-three cases of multiple glomus tumors have been reported; in three instances a familial pattern was recorded. This paper presents a family in which the occurrence of multiple glomus tumors is common.

A case of multiple glomus tumors has been studied with angiography, thermography, plethysmography, and light and electron microscopy. No vascular abnormalities were found. Light microscopic studies revealed typical histopathology of glomus tumors of the multiple type. Electron microscopic studies confirmed that the glomus cells have the features of smooth muscle cells.

A case of multiple painful glomus tumors is reported in a 20-year-old woman. Multiple glomus tumors, although rare, have usually been unassociated with pain. The painful character of the lesions in the case presented above is explained on the basis of entrapment of myelinated nerve fibers in the stroma of the tumor. The various views on the histogenesis of such tumors are presented. It is believed that the tumor is probably related to Zimmermann's pericytes, differing, however, from a hemangiopericytoma in having a definite organoid pattern and lacking a malignant potential.

Multiple glomus tumors are uncommon. Sluiter and Postma,4in 1960, in an extensive review of the literature indicated that there were less than 2 dozen reported cases in contrast to the more than 350 cases of single glomangiomas. In view of their excellent paper, it is not our purpose in this brief communication to review the total subject of multiple glomus tumor. We wish only to(a) indicate the familial nature of this tumor,and(b) point out the microscopic difference between this lesion and the ordinary glomangioma. The number of case reports of familial occurrence of the multiple glomus tumor are few. In a complete review of the literature we were able to find only three. SUMMARYTwo cases of hemangiopericytoma are described, one lodging in the mediastinum and the other in the subcutaneous layer. Another case of multiple glomus tumors, earlier recorded by Bergstrand (5), is quoted. Among the latter re‐examination disclosed a tumor presenting the picture of hemangiopericytoma.

REPORTS of glomus tumors have appeared in the literature with increasing frequency since this entity was described by Masson1in 1924. The vast majority of these cases reported have been concerned with small solitary tumors situated on an extremity which were characteristically extremely sensitive to slightest pressure. Even with increased awareness of this entity, it is still relatively rare. Love2in 1935 described the first glomus tumor recognized at the Mayo Clinic. He recently reported 13 more such tumors and stated that other tumors not included in his series had been diagnosed in the Clinic.3In the Section on Dermatology at the Clinic a histological and clinical diagnosis of glomus tumor has been made in nine additional cases, only one of which presented multiple glomus tumors. MULTIPLE PAINFUL AND PAINLESS GLOMUS TUMORS

The classification of the glomus tumor among the painful tumors was secure until Weidman and Wise 1 in 1937 described a woman having forty-eight painless glomus tumors. This report is prompted by their observations and presents the picture of a patient with twelve such painless tumors associated with two typically painful lesions. The finding of multiple glomus tumors is indeed rare, but the occurrence of multiple painful and painless glomus tumors in the same patient is unusual enough to warrant a report. Masson 2 in 1924 described a vascular tumor made up of a tangled mass of arterioles, the walls of which had epithelioid and smooth muscle cells peculiarly arranged about the lumen. He further noted a large number of unmyelinated nerves which were directly continuous with the cytoplasm of the epithelioid cells. To this structure he gave the name ``neuro-myo-arterial glomus.''
