Contrast-enhanced dynamic MR mammography can provide important additional diagnostic information when performed for certain indications. When suspicious lesions are identified on MR mammography and cannot be reproduced using other imaging modalities, a decision must be made as to its management, i. e. further diagnostic work-up. One possibility is the short-term follow-up of such findings, resulting in higher costs and a possible delay in the start of treatment of a malignant lesion. An alternative to a follow-up is an MR-guided intervention. MR-compatible equipment has been developed for this purpose. This includes equipment specialized for percutaneous biopsy and preoperative localization. The following is an overview of the diagnostic value of MR-guided biopsy and preoperative localization including the relevant literature.

Comparison of MRI features of different grades of DCIS and invasive carcinoma of the breast

The purpose of the study is to compare mammography and magnetic resonance (MR) mammography in detection and assessment of extent, of histologically proven ductal carcinoma in situ (DCIS) and to compare MR features of DCIS, with features of invasive carcinoma. Forty histopathologically proven and MR detected pure DCIS lesions were described and compared with 213 enhancing invasive carcinomas. Histopathological examination revealed 49 pure DCIS, MR detected 40 of them (81.6%). There was a good correlation between diameter measured on mammography, MR and histopathology. MR was able to detect additional foci. Ductal enhancement, a focal area or a mass were perceived in respectively 8 (20%), 8 (20%) and 24 (60%) DCIS and in 0, 6 (2.8%) and 207 (97.2%) invasive lesions. Maximal contrast enhancement after 3 minutes was seen in 60.9% of DCIS and before 3 minutes in 61% of invasive masses. Signal intensity increase of more than 100% was seen in 76.9% of DCIS and in 91.1% of invasive carcinomas. DCIS had a wash out in 53.8% and invasive carcinomas in 65.3%. MR was able to detect 81.6% of DCIS. Diameter prediction was good on mammography and MR mammography. The only MR feature exclusively seen in DCIS was ductal enhancement.

This is another excellent book on mammography by the same author who gave us Practical MR Mammography. In this teaching atlas, the author was joined by Drs. F. Baum, L. Fuezesi, and D. von Heyden. The book opens with a useful TNM classification of breast carcinoma and by a concise but informative schematic review of the most frequently detected images that can be observed in mammography, ultrasonography, and magnetic resonance imaging. This review is reproduced also on a 5′×12′ hard card that can be consulted at any time.

Evaluation of the three-time-point method for diagnosis of breast lesions in contrast-enhanced MR mammography

To evaluate the three-time-point (3TP) method for diagnosis of breast lesions detected on contrast-enhanced MR mammography. MR imaging was performed in 40 women with 120 suspected breast lesions in mammography and/or sonography. The contrast kinetics was converted by 3TP software on a pixel-by-pixel basis into color-coded images. Lesion diagnosis was made by analysis of color intensity and color hue. The 3TP results were compared with the results of the region-of-interest (ROI) method. In 16 patients, we were able to correlate the results with histopathological findings. The 3TP method could successfully be performed in all MR mammographies. Forty (33%) lesions had a diameter of less than 5 mm, 56 (47%) lesions between 5 and 10 mm, and 24 (20%) lesions were greater than 1 cm. Of all 120 lesions, 65 (54%) showed heterogeneous contrast enhancement. In 117 (97%) of all 120 lesions the results of ROI and the automated 3TP method were considered equivalent. However, in three lesions the manual ROI differed from the 3TP method. After a second, repeated manual ROI placement, we were able to confirm equivalent results with the 3TP images as well. The 3TP method automatically and reliably converts contrast kinetic information of the entire breast into a color-coded image. The 3TP method presents kinetic information of the entire dynamic series in an easy-to-interpret format and this automated method may allow to forego time-consuming and sometimes subjective manual ROI placements. This method displays the heterogeneity of the contrast enhancement pattern often observed in malignant lesions and makes it usable as diagnostic criterion.

Application and assessment of a robust elastic motion correction algorithm to dynamic MRI

The purpose of this study was to assess the performance of a new motion correction algorithm. Twenty-five dynamic MR mammography (MRM) data sets and 25 contrast-enhanced three-dimensional peripheral MR angiographic (MRA) data sets which were affected by patient motion of varying severeness were selected retrospectively from routine examinations. Anonymized data were registered by a new experimental elastic motion correction algorithm. The algorithm works by computing a similarity measure for the two volumes that takes into account expected signal changes due to the presence of a contrast agent while penalizing other signal changes caused by patient motion. A conjugate gradient method is used to find the best possible set of motion parameters that maximizes the similarity measures across the entire volume. Images before and after correction were visually evaluated and scored by experienced radiologists with respect to reduction of motion, improvement of image quality, disappearance of existing lesions or creation of artifactual lesions. It was found that the correction improves image quality (76% for MRM and 96% for MRA) and diagnosability (60% for MRM and 96% for MRA).

Overall Breast Density in MR Mammography

The objective of this study was to assess the efficacy of MR mammography (MRM) in evaluating breast cancer extent in women with fatty or dense breasts, and its contribution to the therapeutic approach. The authors reviewed 97 carcinomas detected in 93 women (both symptomatic and from screening) that were classified in two groups according to breast density pattern. Mammography, ultrasound (US), and MRM were performed to evaluate size, extension of the in situ component, presence of multifocal/multicentric disease, and contralateral involvement. Results obtained on mammography plus US were balanced against MRM, considering pathologic analysis as the gold standard. For fatty breasts (n=47), exact measurement was found on mammography plus US and on MRM alone in 70%, underestimation on mammography plus US 23.5% and on MRM 11% (P=0.005). For dense breasts (n=50), exact measurement was found on mammography plus US in 40% and on MRM alone 68%, underestimation on mammography plus US 52% and on MRM 10% (P=0.005). Overall, good correlation (R>0.71) was found between pathologic and clinical size with all imaging methods; nevertheless, when evaluating multifocal/multicentric disease, a poor correlation was observed between histologic assessment and mammography plus US (R=0.52), but it was excellent with regard to MRM (R=0.99). In fatty breasts, the combination of mammography and US allows for a precise assessment of tumoral extension. However, these results show that in dense breasts, MRM is superior to mammography plus US, suggesting that its systematic use in this group of patients is justifiable.

Multi-volume Fusion Imaging of MR Ductography and MR Mammography for Patients with Nipple Discharge

Because nipple discharge is caused by carcinoma as well as benign disease, identification of intraductal abnormalities with ductography is important. Ductography is an invasive mammographic examination in which contrast material is injected directly into the duct. Failure to cannulate or extravasation may occur. Ductography shows evidence of lesions, such as filling defects, duct obstruction, or wall irregularity, but it does not reveal the lesion itself. Furthermore, ductography produces a two-dimensional image, so it does not show the shape of the dilated duct or the precise location of the intraductal lesion in the breast. We applied three-dimensional (3D) heavily T2-weighted imaging with fat suppression of the breast to produce MR ductography. The dilated ducts are seen in 3D as tubular structures with high signal, and intraductal abnormalities are seen as signal defects. Furthermore, MR ductography can show an obstructed duct that cannot be seen on ductography. We also performed 3D breast MRI with the intravenous infusion of contrast material to show the lesion itself. Finally, we fused these 2 volume images into a single 3D fused image that not only shows the existence of intraductal abnormality, but reveals the shape, size, and extent of lesion, allowing us to understand easily the relationship between the ducts with dilation and any intraductal lesions in the breast. We herein introduce and describe this noninvasive method and discuss various factors related to its diagnostic use.

Pharmacokinetic analysis of ductal carcinoma in situ of the breast using dynamic MR mammography

The purpose of this study was to assess the relationship between functional parameters derived from dynamic MR imaging and the histological findings of breast ductal carcinoma in situ (DCIS) and DCIS with invasive foci, and to evaluate whether these parameters might predict DCIS patient outcome. Two parameters, amplitude A and k21, were determined from multicompartmental pharmacokinetic analyses of dynamic MR mammography in 39 patients with needle biopsy-proven primary DCIS. After surgery, the histological tumor characteristics, including microvessel density (MVD) (anti-CD34), vascular permeability (anti-VEGF antigen), and histological grade, were evaluated. Histology revealed 27 pure DCIS and 12 DCIS with invasive foci. In pure DCIS, positive correlations between MVD and amplitude A (r=.56, P<.0025) and between MVD and k21 (r=.43, P=.02) were found. As for histological grade, the differences in both functional parameters of grade 1 versus grade 2 and grade 1 versus grades 2 and 3 combined were significant (P<.05). No significance was found in the analysis of DCIS with invasive foci. Our results indicated that functional MRI-based parameters might possess the potential to predict the outcome of patients with DCIS. Further study will be needed with larger series over longer periods.

Two-view mammography is at present the only imaging method used for breast screening. But there are some deficits in differential diagnosis, especially in dense breast tissue and non-calcified tumors. Purpose: The purpose of this study was the demonstration that one-view mammography (EEMG) combined with ultrasound mammography (USMG) and clinical examination (TB) is equal to or more effective for breast screening than two-view mammography (ZEMG). Material and Methods: 3743 female patients examined from 1993-2003 in the Institute for mamma diagnostics (IMZE) in Esslingen/Germany underwent curative mammography with TB, ZEMG, USMG; after surgical excision a definitive histological diagnosis was made. To avoid interobserver variability, only patients examined by the same doctor were included. Applying these criteria 343 female patients with surgically and histologically proved 334 malignant lesions and 9 benign lesions were found. These patients were divided into five age-groups. Positive predictive value (PPV), negative predictive value (NPV), sensitivity, specificity, false negatives and positives were calculated in succession for EEMG, ZEMG, USMG, EEMG + USMG combined and EEMG + USMG + TB combined for the different age-groups. For EEMG only the oblique view of the two-view mammograms was evaluated. Additionally, extended diagnostic methods, like magnetic-resonance mammography (MRM) were analysed, to document a possible diagnostic optimization. Results: The sensitivities for a malignant diagnosis (high-risk lesion, invasive cancer) was 83% for EEMG (95% Cl 78.4-86.8%: p<0.0001), 84% for ZEMG (95% Cl 79.4-87.6%; p<0.0001), 91% for USMG (95% CI 87-93.6%; p<0.0001), 97% for EEMG + USMG (95% Cl 94.5-98.5%; p<0.0001) and 98.5% for TB + EEMG + USMG (95% Cl 96.1-99.3%: p=0.7); the false negatives were 17% (EEMG), 16% (ZEMG), 9% (USMG), 3% (EEMG + USMG) and 1.5% (TB + EEMG + USMG 5/343 interval cancers). The cranio-caudal view was useful only in one case in 10 years. The differences in sensitivity and false negatives were visible in all age-groups and comparable with international studies. Problematic diagnoses (sensitivity: lobular 97%, ductal 97%, ductolobular 100%, apocrine carcinoma 100%, adenocarcinoma 100%) were detected better with EEMG + USMG than with ZEMG (lobular 82%, ductal 84%, ductolobular 40%, apocrine carcinoma 0%, adenocarcinoma 0%). The combination of EEMG + USMG + TB increased sensitivity in women older than 50 years. Below 50 years there was no difference to EEMG + USMG. EEMG + USMG was associated with lower costs than ZEMG (saving 4.4%). Conclusion: In a curative collective EEMG combined with USMG is not only able to replace ZEMG, but increases detection rates in imaging procedures. The advantages of both methods in the detection of high risk lesions and invasive cancer were combined. Therefore EEMG + USMG represent a possible alternative breast screening method with high patient acceptance, because of reduced costs and a lower radiation dose.

Breast Lesion Detection and Characterization at Contrast-enhanced MR Mammography: Gadobenate Dimeglumine versus Gadopentetate Dimeglumine

To prospectively and intraindividually compare equivalent (0.1 mmol per kilogram of body weight) doses of gadobenate dimeglumine and gadopentetate dimeglumine for accuracy of detection and characterization of breast lesions at contrast material-enhanced magnetic resonance (MR) mammography. Ethics committee approval and informed consent were obtained. Twenty-six consecutive women (mean age, 47.8 years) suspected of having a breast tumor at mammography and sonography underwent two identical MR examinations at 1.5 T; examinations were separated by more than 48 hours but less than 72 hours. A T1-weighted three-dimensional gradient-echo sequence was used, and images were acquired before and at 0, 2, 4, 6, and 8 minutes after randomized injection of gadopentetate dimeglumine or gadobenate dimeglumine at an identical flow rate of 2 mL/sec. Separate and combined assessment of unenhanced, contrast-enhanced, and subtracted images was performed blindly by two readers in consensus. Accuracy for lesion detection was determined against a final diagnosis based on findings at conventional mammography, sonography, and surgery. Sensitivity, specificity, positive and negative predictive values, and overall accuracy for malignant lesion identification were determined against histologic results. Data were analyzed with the McNemar test, proportional odds models, and analysis of variance. MR mammography with gadobenate dimeglumine depicted significantly (P = .003) more lesions (45 of 46) than did that with gadopentetate dimeglumine (36 of 46), and detected lesions were significantly (P < .001) more conspicuous with gadobenate dimeglumine. Confidence for characterization was significantly (P = .031) greater with gadobenate dimeglumine. Comparison of the contrast agents for their ability to help identify malignant lesions revealed significant (P = .02) superiority for gadobenate dimeglumine: Sensitivity, specificity, positive predictive value, negative predictive value, and overall accuracy for malignant lesion identification were, respectively, 94.7%, 100%, 100%, 80.0%, and 95.6% with gadobenate dimeglumine and 76.3%, 100%, 100%, 47.1%, and 80.4% with gadopentetate dimeglumine. Quantitative evaluation of signal intensity-time curves revealed significantly (P < .001) greater lesion enhancement with gadobenate dimeglumine. Detection of breast lesions and accurate identification of malignant lesions at MR imaging are significantly superior with gadobenate dimeglumine in comparison with gadopentetate dimeglumine.

Supine MR mammography using VIBE with parallel acquisition technique for the planning of breast-conserving surgery: Clinical feasibility

Volumetric interpolated breath-hold examination (VIBE), which provides both high-spatial and high-temporal resolution images, is a useful sequence for determining the extent of breast cancer. The VIBE sequence in combination with the parallel acquisition technique allows for a single breath-hold evaluation of the breast. We evaluated the clinical feasibility of MR mammography in the supine oblique position (supine MRM) using this technique for planning breast conservation surgery. The accuracy of determination of the tumor extent with a deviation of less than 2 cm in length was 90% (36/40). Two cases showed a positive surgical margin (7.4%). Supine MRM is feasible for the planning of breast-conserving surgery.

Ultrasound-Guided, Percutaneous Cryotherapy of Small (≤15 mm) Breast Cancers

The purpose of this study was to investigate the feasibility, efficacy, and safety of ultrasound-guided percutaneous cryotherapy of stage T1 breast cancers. Thirty patients with biopsy-confirmed breast cancers with tumor diameters of 15 mm or smaller (range, 5-15 mm; median, 12 mm) underwent cryotherapy. After local anesthesia, a 3-mm cryo probe was placed into the tumor under ultrasound guidance. All tumors were subjected to 2 freeze cycles with an interposing thawing cycle. The size of the ice-balls, their distance to the skin, and the temperature at the tip of the probe were closely monitored during the procedure. The patients underwent surgery within 6 weeks and the specimens were evaluated histologically. The median minimum temperature reached -146 degrees C (range, -117 degrees C to -167 degrees C). In 5 of 29 patients, remnant ductal carcinoma in situ was detectable histologically after cryotherapy beyond the margin of the cryosite in the specimens after open surgery. In 24 patients, no viable tumor cells were found. No severe side effects occurred. In one patient, the cryo procedure was not performed completely because of technical problems. Percutaneous cryotherapy is a feasible and safe procedure in minimally invasive therapy for small breast cancers. Residual ductal carcinoma in situ may be attributable to the beginning of a learning curve or by false-negative detection in pre-interventional imaging. Magnetic resonance mammography might aid in treatment planning and for therapy monitoring to better define target tissue and to correlate the tumor margin with the ice-ball.

Classification of Signal-Time Curves Obtained by Dynamic Magnetic Resonance Mammography

This study compares the performance of quantitative methods for the characterization of signal-time curves acquired by dynamic contrast-enhanced magnetic resonance mammography from 253 females. Signal-time curves obtained from 105 parenchyma, 162 malignant, and 91 benign tissue regions were examined (243 lesions were histopathologically validated). A neural network, a nearest-neighbor, and a threshold classifier were applied to either the entire signal-time curve or pharmacokinetic and descriptive parameters calculated from the curves to differentiate between 2 (malignant or benign) or 3 tissue classes (malignant, benign, or parenchyma). The classifiers were tuned and evaluated according to their performance on 2 distinct subsets of the curves. The accuracy determined for the neural network and the nearest-neighbor classifiers was nearly identical (approximately 80% in case of 3 tissue classes, and approximately 76% in case of the 2 classes). In contrast, the accuracy of the threshold classifier applied to the discrimination of 3 classes was low (65%). Quantitative classifiers can support the radiologist in the diagnosis of breast lesions.

Further Signs in the Evaluation of Magnetic Resonance Mammography

To increase accuracy and reliability of magnetic resonance breast imaging, a new evaluation method might be helpful. The recently suggested evaluation method (Fischer U, et al) resulted in a relevant number of equivocal cases (3 or 4 points). Additional morphologic and dynamic signs as an extension of this score were evaluated. One hundred thirty-two histologically verified lesions were evaluated by 3 radiologists double-blinded using 2 evaluation methods: 1) method 1 (according to Fischer, et al): 2pt: initial signal increase >100%, washout, centripetal enhancement, 1pt: initial signal increase 50-100%, plateau phenomenon, centrifugal inhomogeneous enhancement, irregular borders, linear, stellar or dendritic structure; and 2) method 2 (according to Malich, et al): 3pt: hook sign (sign of pectoral invasion), 2pt: unifocal edema, blooming. 1pt: hypointensity in T2, lymph nodes >10 mm, skin thickening, adjacent vessels, a lesion's distorted inner architecture, disruption of the mamillary edge; -1pt: isointensity in T2, no edema, enhancing septations; -3pt: hyperintensity in T2, non enhancing septations. Method 1 judged a lesion to be malignant if 5 or more points were given and benign if 2 or less points were given, respectively. Method 2 (mean value of 3 radiologists) was tested in those cases in which a clear possible decision using method 1 was not sufficiently possible. Method 1 alone resulted in a negative predictive value of 96.8% and a positive predictive value of 90.8% (without carcinoma in situ), a sensitivity of 83.1%, a specificity of 58.8%, and revealed uncertain results (3 and 4 points) in 29 cases (out of 132; 22%). Adding the new scoring system in these 29 equivocal cases and an increase of 2 or more points by using method 2 is supposed to be a sign of malignancy; findings suggest a sensitivity of 90.9% and a specificity of 60% if an increase of maximum 1 is observed in benign lesions. In conclusion, our results show that Göttingen score alone has a sensitivity of 83.1%, a specificity of 58.8%; the second evaluation method reveals a sensitivity of 90.9% and a specificity of 60% in equivocal cases of Göttingen score. Göttingen score then reaches in all cases and second, adding the second evaluation method in equivocal cases, a sensitivity of 97% and a specificity of 76.5%. The application of a second evaluation method in those cases remaining unclear in Göttingen score can lead to a decrease of uncertainty and a higher sensitivity and specificity of diagnosis in MR mammography. In this study, Göttingen score reaches a sensitivity of 83.1% and a specificity of 58.8%, increasing to a sensitivity of 97% and a specificity of 76.5% when being extended by a second evaluation method in unclear cases.

Color-Coded Automated Signal Intensity Curves for Detection and Characterization of Breast Lesions

The objective of this study was to evaluate the value of a color-coded automated signal intensity curve software package for contrast-enhanced magnetic resonance mammography (CE-MRM) in patients with suspected breast cancer. Thirty-six women with suspected breast cancer based on mammographic and sonographic examinations were preoperatively evaluated on CE-MRM. CE-MRM was performed on a 1.5-T magnet using a 2D Flash dynamic T1-weighted sequence. A dosage of 0.1 mmol/kg of Gd-BOPTA was administered at a flow rate of 2 mL/s followed by 10 mL of saline. Images were analyzed with the new software package and separately with a standard display method. Statistical comparison was performed of the confidence for lesion detection and characterization with the 2 methods and of the diagnostic accuracy for characterization compared with histopathologic findings. At pathology, 54 malignant lesions and 14 benign lesions were evaluated. All 68 (100%) lesions were detected with both methods and good correlation with histopathologic specimens was obtained. Confidence for both detection and characterization was significantly (P < or = 0.025) better with the color-coded method, although no difference (P > 0.05) between the methods was noted in terms of the sensitivity, specificity, and overall accuracy for lesion characterization. Excellent agreement between the 2 methods was noted for both the determination of lesion size (kappa = 0.77) and determination of SI/T curves (kappa = 0.85). The novel color-coded signal intensity curve software allows lesions to be visualized as false color maps that correspond to conventional signal intensity time curves. Detection and characterization of breast lesions with this method is quick and easily interpretable.
