Prevalence of infertility and barriers to family building among male physicians.

Fertility declines with increasing age and physicians often delay childbearing beyond prime reproductive years for the sake of medical training. However, the risks of infertility in male physicians compared to the general population remain poorly studied. To characterize rates of infertility among male physicians and identify barriers in access to fertility care and family building. Between June 2022 and December 2022, male physicians were invited to complete a questionnaire regarding fertility and family building. Surveys were disseminated electronically via social media and professional medical societies using Qualtrics (Provo, UT). Two hundred thirty-five responses were included in the final analysis. The mean age of respondents was 36.3±7.4 years. Of 151 respondents with children or currently attempting to have children, 66 (43.7%) delayed family building due to their medical training or career. The most influential factors affecting timing of children were lack of flexibility in schedule, lack of time, stress, and financial strain. Forty-three (18.3.%) respondents had seen a doctor for fertility evaluation; an additional 12 (5.1%) said they considered doing so but did not, mostly due to being too busy. Sixty (25.5%) had undergone semen testing in the past. Thirty-one (13.2%) reported a diagnosis of fertility issues in either themselves or their partner. Twenty-seven (11.5%) endorsed either them or their partner having undergone assistive reproductive technologies or other procedures for infertility. A significant proportion of male physicians delayed building their family or seeking fertility evaluation due to their medical career. Around 23.4% of male physicians have either seen or considered seeing a physician for fertility evaluation, suggesting a high prevalence of infertility in this cohort. Our results indicate a need for interventions to support family building and fertility evaluation and treatment among male physicians.

MTHFRC 677T Gene Polymorphism and Homocysteine in North China Patients with Varicocele

To investigate the correlation between methylenetetrahydrofolate reductase (MTHFR) C677T gene polymorphism, homocysteine, and male infertility with varicocele in North China. One Hundred infertile males with varicocele (VC; grade II-III, VC group) and 100 healthy males with normal semen parameters and no varicocele (NC group) were recruited for PCR microarray, blood and semen testing. Compared with the CC genotype in the NC group, the TT genotype in the NC group and the CC genotype in the VC group showed no significant changes in sperm motility (P=0.191; P=0.130), sperm density (P=0.591, P=0.643), plasma homocysteine level (P=0.511; P=0.677), and seminal plasma MDA (P=0.752; P=0.451). In contrast, VC patients with the TT genotype had higher plasma homocysteine level and seminal plasma MDA levels (P <0.001), lower partial pressure of oxygen in seminal pulse (PO2; P <0.001) and poorer sperm quality (P <0.001), as compared with the CC genotype. This suggests that MTHFR C677C>T may not be a risk factor for male Varicocele in North China. However, this may affect the oxidative stress associated with homocysteine expression, which in turn affects semen parameters in VC patients. Larger studies are needed to validate our findings.

A preconception cohort study of sugar-sweetened beverage consumption and semen quality.

Dietary factors, including high sugar intake, may have adverse effects on male reproduction. Studies of the association between sugar-sweetened beverage (SSB) intake and semen quality have reported inconsistent results. We estimated the effects of SSB consumption on semen quality in a North American preconception cohort study. We analyzed baseline data from 690 males (n=1,247 samples) participating in Pregnancy Study Online (PRESTO) during 2015-2022. Participants aged ≥21 years completed a baseline questionnaire on which they reported information about intake of SSBs, including sodas, energy drinks, sports drinks, and fruit juices. After enrollment, we invited U.S. participants to a semen testing substudy, in which they collected and analyzed two samples using an at-home semen testing kit. We used linear regression models to estimate adjusted percent differences (%D) and 95% confidence intervals (CI) for associations of SSB intake with semen volume, sperm concentration, total sperm count (TSC), motility, and total motile sperm count (TMSC). We used modified Poisson regression models to estimate adjusted risk ratios (RRs) and 95% CIs for the association of SSB intake with World Health Organization semen parameter cut points. Relative to non-consumers of SSBs, those who consumed ≥7 SSBs/week had lower semen volume (%D=-6, 95% CI: -13, 0), sperm concentration (%D=-22, 95% CI: -38, 0), TSC (%D=-22, 95% CI: -38, -2), motility (%D=-4, 95% CI: -10, 2), and TMSC (%D=-25, 95% CI: -43, -2). High SSB consumers also had greater risks of low sperm concentration (≤16 million/mL; RR=1.89, 95% CI: 1.11, 3.21), low TSC (≤39 million; RR=1.75, 95% CI: 0.92, 3.33), low motility (≤42%; RR=1.23, 95% CI: 0.87, 1.75) and low TMSC (≤21 million; RR=1.95, 95% CI: 1.12, 3.38). Associations were stronger among participants with body mass index ≥ 25kg/m2 . Greater SSB consumption was associated with reduced semen quality in a North American preconception cohort.

Patient Portal Engagement Affects Postvasectomy Semen Analysis Completion

To identify factors that affect completion of post-vasectomy semen analysis (PVSA) in men receiving telehealth pre-vasectomy counseling. Telehealth visits have become increasingly common for pre-vasectomy consultations. Prior studies have shown that men prefer telehealth vasectomy consultations over in-person options. Post-vasectomy semen testing should be completed to confirm sterilization. Three-hundred and seventy-one men aged 19 and older who saw a single physician for a telehealth pre-vasectomy consultation and completed an in-office vasectomy were included in the study. Demographic information such as age, patient relationship status, and distance from the clinic were accessed via EMR. Patients were assessed based on their engagement with electronic pre-procedure instructions, and the primary outcome measured was completion of PVSA. 45.6% of men completed a PVSA. There was no significant difference in completion of the PVSA between those who opened their electronic instructions before their vasectomy and those who did not (46.1% vs 44.4%, p=0.77). Of those who messaged the clinic for any reason at least once after their consultation, 62% completed their PVSA; 41% who did not contact the clinic completed the PVSA (p=0.0009). While there was no difference in completion of PVSA in patients who opened their instructions vs. those who did not, patients with a higher level of engagement with the patient portals were more likely to complete their semen test. By understanding factors influencing patient compliance with post-vasectomy semen testing, healthcare professionals can develop targeted interventions to ensure safe and successful outcomes.

Determinants of puberty and age of sexual maturity in Nigerian indigenous breeds of male goats

This study was conducted to ascertain the pubertal and sexual maturity characteristics of Nigerian native goat breeds. The research was carried out on three goat breeds; Sahel (SG), Red Sokoto (RSG), and West African Dwarf (WAD) goats. Each breed was represented by eight (8) animals (n=24) and housed in separate pens based on breed type. All bucks were weighed at birth and bi-weekly using digital weighing scales until they reached puberty. Scrotal circumference measurements were taken every two weeks until the end of the study. Semen was collected immediately after preputial separation and all semen samples were labelled and placed in a thermo-flask immediately and subsequently transported to the laboratory for evaluation. The mass motility and the forward motilities of the spermatozoa were observed using a light microscope. The percentage of live, dead, and abnormal sperms were determined after staining with eosin and negrosin stains. The sperm cell concentration was determined by counting spermatozoa with a hemocytometer. Result obtained indicate that at the 14th and 16th weeks of age, the mean scrotal circumference of bucks ranged from 5.14±0.66 cm to 7.06±0.71 cm, 6.32±0.29 cm to 8.05±0.42 cm, and 5.50±0.74 cm to 7.06±0.71 cm in SG, RSG, and WAD bucks, respectively. Puberty in Red Sokoto bucks was achieved at an average age of 14 weeks, with a mean sperm concentration of (116.17±28.47 x 106), progressive sperm motility of (65.00±9.92%), and a mean body weight of 10.17±1.26 kg. Sahelian and WAD bucks reached puberty at the 16th weeks, with sperm concentrations of 106.29±25.18 x 106 and 107.60±19.99 x 106, mean sperm motility of 65.00±9.94% and 76.00±4.8%, and mean body weights of 11.29±1.59 kg and 7.78±0.70 kg, respectively. To summarise, this study revealed that breed, age, scrotal circumference and body weight at puberty played significant roles in determining sperm characteristics and sexual maturity.

Evaluation of sperm counting accuracy on computer-assisted sperm analysis with GoldCyto® slides and glass slides.

Worldwide, various counting chambers and computer-assisted sperm analysis (CASA) devices are in use. The semen's concentration can vary depending on the depth of the counting chamber and how it is loaded. The study's objectives were to analyze the effects of various counting chambers on semen concentration results using a GoldCyto® slide and a glass slide in the CASA system and to ascertain the precision of concentration measurements made using glass slides on CASA. The study's control group was composed of samples with known concentrations (72-80 million sperm/mL) as determined by a spectrophotometer. A total of 21 frozen straws from the same bull of the same date were thawed at 37°C for 30 s and loaded into two different sperm-counting chambers (GoldCyto® slide and glass slide). The sample semen placed in the sperm counting chambers was 5 μL and the same value was entered in the CASA software as 5 μL. Measurements were done and evaluated in 5 different areas. According to the data we obtained, using the glass slide were statistically lower than the spectrophotometer (p < 0.001). GoldCyto® slide results were consistent with spectrophotometer results. Consequently, measurements with GoldCyto® slides in the CASA had consistent results, while measurements with glass slides were inconsistent. It was concluded that GoldCyto® slides are more suitable than glass slides in the concentration examinations of semen. Therefore, more study is needed to optimize the use of glass slides.

Fertility and Obstetric Outcomes of Assisted Reproductive Technology (ART) in Women With Adenomyosis Following Gonadotropin-Releasing Hormone Agonist Therapy: A Single-Center Experience.

Adenomyosis is an ambiguous disorder causing a wide variety of implications from dysmenorrhea, heavy menstrual bleeding, and infertility to pregnancy complications. Adenomyosis is associated with altered endocrine and inflammatory milieu, resulting in impaired implantation and reduced fertility potential. It is also associated with increased incidence of obstetric complications such as miscarriage, antepartum hemorrhage, placental mal-position, hypertensive disorders, small for gestational age-intrauterine growth restriction (SGA-IUGR), cesarean section, preterm labor, preterm premature rupture of membranes (PPROM), and neonatal intensive care unit (NICU) admissions. The aim of our study was to investigate the fertility and obstetric outcomes in women with adenomyosis treated with GnRH agonists compared to controls with normal uteri undergoing in-vitro fertilization (IVF) at our center, thereby establishing the role of gonadotropin-releasing hormone (GnRH) agonists in managing sub-fertile women with adenomyosis. We carried out a retrospective cohort study at our hospital to analyze the effects of adenomyosis on IVF and pregnancy outcomes. This study (n=83) involves women with adenomyosis between the ages of 21 and 37 years who were followed up at our center between 2013 and 2022. The controls (n=83) were selected from women who underwent IVF-intracytoplasmic sperm injection (IVF-ICSI) for tubal or mild male factor infertility with normal appearing uterus within the same time frame. Women with adenomyosis were given GnRH agonist as long/ultralong agonist protocol before controlled ovarian stimulation or as down-regulated frozen embryo transfer (FET). The length of suppression was between one and six months based on the size of the uterus and response to treatment. Fertility and obstetric outcomes were analyzed. The implantation rates were found to be equivocal: 54.2% and 53% in the adenomyosis and control groups, respectively (p=0.208). The cumulative live birth rate was 50.6% and 48.2% in the study and control groups,respectively (p=0.341). The biochemical pregnancy rate and the first- and second-trimester miscarriage rates were not significantly different between the group with adenomyosis and the group with normal uterus. The incidence of preterm deliveries and antepartum hemorrhage was found to be significantly increased in the study group. Medical management in women with adenomyosis optimizes the live birth rates giving results at par with the control population.

Comparable bio-evaluation of curcumin and chitosan-encapsulated curcumin nanoparticles against the reprotoxic potential of fenpropathrin pyrethroid in rats: Genomic and morphometric prospectives.

This study delves into the intricate exploration of potential toxic effects resulting from subchronic exposure to fenpropathrin (FNP) on the reproductive system of male SD rats. Adding to the novelty, our study undertakes a pioneering comparison of the effects of curcumin (CUR) and curcumin-encapsulated chitosan nanoparticles (CS.CUR.NPs) on these toxic effects. The study involved a cohort of sixty male SD rats (six groups): vehicle control, CUR, Cs.CUR.NPs, FNP, and two combination groups (FNP with CUR or Cs.CUR.NPs). The synthesized Cs.CUR.NPs nanoparticles underwent meticulous characterization using Fourier Infrared spectroscopy (FT-IR) and transmission electron microscopy (TEM). The findings revealed that FNP caused oxidative stress, sperm abnormalities, reduced motility and sperm count FNP decreased serum LH, FSH, 17-β estradiol, and testosterone levels. FNP downregulated the mRNA expression of the spermatogenesis and steroidogenesis-related genes, While, downregulated hypothalamic KISS-1 and KISS-1r expression. Histopathological alterations were assessed and scored. Surprisingly, the treatment with CUR and Cs.CUR.NPs exhibited remarkable restorative effects on semen quality, sex hormone levels, antioxidant capacity, and mRNA expression of the targeted genes. Notably, Cs.CUR.NPs displayed superior properties when compared to CUR. Nevertheless, further research is imperative to evaluate their efficacy across various bodily tissues.

Mitigative potential of rhoifolin against cisplatin prompted testicular toxicity: biochemical, spermatogenic and histological based analysis.

Rhoifolin (ROF) is a naturally occurring flavonoid compound with diverse pharmacological and therapeutic benefits. The current investigation was designed to evaluate the curative potential of Rhoifolin (ROF) against Cisplatin (CP) induced testicular damage. Mature male albino rats (n=48) were randomly distributed into 4 equal groups: control, CP (10mg/kg), CP+ROF (10mg/kg+20mg/kg) and ROF (20mg/kg) supplemented group. Following 56days of the trial, biochemical, inflammatory markers, spermatogenic, steroidogenic, hormonal, apoptotic, anti-apoptotic, and histopathological parameters were evaluated. The exposure to CP markedly (p<0.05) lowered the activities of anti-oxidant enzymes, glutathione reductase (GSR), catalase (CAT), and glutathione peroxidase (GPx) as well as superoxide dismutase (SOD) in testicular tissues of male albino rats. Besides the levels of reactive oxygen species (ROS) and thiobarbituric acid reactive substances (TBARS) were considerably augmented in CP exposed rats. The administration of CP also increased the level of inflammatory cytokines i.e. IL-6, TNF-α, 1L-1β and NF-κβ as well as COX-2 activity. Additionally, a notable (p<0.05) upsurge was observed in dead sperms count, abnormality in the tail, midpiece as well as head of sperms along with a notable decline in sperm motility in CP treated rats. Moreover, the expressions of steroidogenic enzymes were also lowered in CP administered group. The levels of follicle stimulating hormone (FSH) and plasma testosterone as well as luteinizing hormone (LH) were decreased in CP treated group. Moreover, the expression of Bax as well as Caspase-3 (apoptotic markers) were increased. On the other hand, Bcl-2 expression (anti-apoptotic marker) was reduced. Furthermore, the histopathological analysis showed that CP considerably (p<0.05) damaged the testicular tissues. However, the administration of ROF significantly reduced the damaging effects of CP in testicular tissues. The results of our study suggested that ROF can potentially alleviate CP-induced testicular damages due to its androgenic, anti-oxidant and anti-inflammatory as well as anti-apoptotic nature.

The Racial and Socioeconomic Characteristics of Men Using Mail-in Semen Testing Kits in the United States

To describe the racial and socioeconomic characteristics of men using mail-in semen testing kits for their fertility care in the United States over 3 years. A cross-sectional analysis was conducted of men who used mail-in semen analyses from a single direct-to-consumer (DTC) fertility platform between 2020 and 2022. Age, race/ethnicity, place of residence, and education level of men using mail-in semen analysis kits was collected from user questionnaires. Zip code level socioeconomic status was estimated using the Area Deprivation Index (ADI). These characteristics were compared to the overall population of the United States. 4,342 men used mail-in semen analyses from a single DTC fertility company between 2020 and 2022 and completed the questionnaire. The median age of this cohort was 36 (IQR: 32,40) years. The group consisted of predominantly white men (72%), with 7.9% Asian and only 5.1% Black, 3.2% Hispanic, 0.4% Indian American, 0.1% pacific islander, 5.8% mixed race, and 2.8% other groups. 45.8% of the group had completed college, 22% had a master's degree, and 9% had a doctoral degree. The mean ADI was 83.47 (SD 21.44) compared to the average ADI of 100 for the general US population (p<0.005). DTC mail-in semen analyses have the potential to improve access to affordable fertility care. Racial and ethnic minorities and lower socioeconomic classes are underrepresented for men seeking DTC fertility testing as they are for men presenting for in-office fertility care. Additional studies are needed to determine the factors responsible for these discrepancies.

Male fertility: a summary overview of the publications October 2022 – March 2023

The article presents an overview of the most significant publications on the topic of male infertility. The main selection criteria were the practical significance of the article, as well as the impact factor of the journal in which it was published, according to the SCImago Journal Rank (SJR). As a result, a list of 10 works published from October 2022 to March 2023 was formed. The review includes articles on the following topics: genetic causes of male infertility, the role of oxidative stress in assisted reproductive technologies (ART) programmes, reference values when performing scrotum and prostate ultrasound, the impact of ART procedures on child development, the impact of DNA fragmentation of spermatozoa on the outcomes of ART, the role of circadian rhythms in spermatogenesis, automated semen testing devices and predictors of success / failure of testicular biopsy.

Comparison of clinical and preimplantation genetic testing for aneuploidy outcomes between in vitro fertilization and intracytoplasmic sperm injection in sibling mature oocytes from high-risk patients: A retrospective study.

To evaluate the influence of insemination methods on clinical outcomes by assessing preimplantation genetic testing for aneuploidy (PGT-A) outcomes in embryos obtained using in vitro fertilization (IVF) and intracytoplasmic sperm injection (ICSI) in sibling mature oocytes from high-risk patients. This retrospective study involved 108 couples with nonmale or mild male factor infertility who underwent split insemination cycles from January 2018 to December 2021. PGT-A was performed using trophectoderm biopsy, array comparative genome hybridization, or next-generation sequencing with 24-chromosome screening. Mature oocytes were divided into IVF (n = 660) and ICSI (n = 1028) groups. The normal fertilization incidence was similar between the groups (81.1% vs. 84.6%). The total number of blastocysts biopsied was significantly higher in the IVF group than in the ICSI group (59.3% vs. 52.6%; p = 0.018). However, euploidy (34.4% vs. 31.9%) and aneuploidy (63.4% vs. 66.2%) rates per biopsy and clinical pregnancy rates (60.0% vs. 58.8%) were similar between the groups. Implantation (45.6% vs. 50.8%) and live birth or ongoing pregnancy (52.0% vs 58.8%) rates were slightly higher in the ICSI group than in the IVF group and miscarriage rate per transfer was slightly higher in the IVF group than in the ICSI group (12.0% vs 5.9%); however no significant difference was observed. IVF and ICSI using sibling mature oocytes had similar clinical outcomes, and euploidy and aneuploidy rates in couples with nonmale and mild male factor infertility. These results suggest that IVF is a useful option, along with ICSI, as an insemination method in PGT-A cycles, especially in high-risk patients.

Human Sperm Head Vacuoles Are Related to Nuclear-Envelope Invaginations.

Nuclear vacuoles are specific structures present on the head of the human sperm of fertile and non-fertile men. Human sperm head vacuoles have been previously studied using motile sperm organelle morphology examination (MSOME) and their origin related to abnormal morphology, abnormal chromatin condensation and DNA fragmentation. However, other studies argued that human sperm vacuoles are physiological structures and consequently, to date, the nature and origin of the nuclear vacuoles remains to be elucidated. Here, we aim to define the incidence, position, morphology and molecular content of the human sperm vacuoles using transmission electron microscopy (TEM) and immunocytochemistry techniques. The results showed that ~50% of the analyzed human sperm cells (n = 1908; 17 normozoospermic human donors) contained vacuoles mainly located (80%) in the tip head region. A significant positive correlation was found between the sperm vacuole and nucleus areas. Furthermore, it was confirmed that nuclear vacuoles were invaginations of the nuclear envelope from the perinuclear theca and containing cytoskeletal proteins and cytoplasmic enzyme, discarding a nuclear or acrosomal origin. According to our findings, these human sperm head vacuoles are cellular structures originating from nuclear invaginations and contain perinuclear theca (PT) components, allowing us to define a new term of 'nuclear invaginations' rather than 'nuclear vacuoles'.

The Protective Effect of Grape Seed Oil on some Fertility Parameters in Male White Rats (Rattus norvegicus) Treated with Tamsulosin

This study was conducted to investigate the protective effect of Grape seeds oil on some fertility parameters in white rats (Rattus norvegicus). This study used (36) male rats age (2.5-3) month, weights ranged (210-270) g which divided into (6) groups (6) rats/group. The first group (Control group), the second group (Tamsulosin group), the third group (Grape Seeds Oil 0.2 ml/kg) body weight, the fourth group (Grape Seed Oil 0.4 ml/kg) B.W, fifth group (Grape Seeds Oil 0.2 ml/kg+ 0.4 mg/kg, Tamsulosin). The sixth group (Grape Seed Oil 0.4 ml/kg+ 0.4 mg/kg. Tamsulosin). All treatment were given doses every day for (7) weeks. Result showed a significant decreased (p> 0.05) in testosterone levels in Tamsulosin group after the fourth and seventh group weeks of treatment as a compare with control group. Where the testosterone levels showed a significant increase in groups of Grape Seeds Oil 0.2,0.4ml/kg B.W after the fourth and seventh weeks of treatment. The fifth and sixth groups showed a significant decreased (p> 0.05) in testosterone levels after the fourth week, where group-two showed a significant increase after the seventh week. The results revealed a significant decreased in the total sperms count and the density of sperms and the percentage of living sperm (p> 0.05) in Tamsulosin group, and increased the percentage of dead and abnormalities sperms of Tamsulosin groups as compare with control group.

A North American study of anthropometric factors and semen quality

To evaluate the association between anthropometric measures and semen parameters. Cross-sectional study. General population, United States. Male participants aged ≥21 years. We analyzed data from 659 males (1,185 samples) participating in a semen testing substudy of Pregnancy Online Study (PRESTO), a North American preconception cohort study. After enrollment, we invited males aged ≥21 years to perform at-home semen testing using the Trak™ system. Participants reported selected anthropometric variables (current weight, height, waist circumference, and weight at age 17 years) and covariate data via online baseline questionnaire. We used generalized estimating equations models to estimate the percent difference in mean log-transformed semen parameter values and 95% confidence intervals (CI) for associations between selected anthropometric variables and semen volume (mL), sperm concentration (million/mL), and total sperm count (million), adjusting for socio-demographics, lifestyle factors, and medical history. We also evaluated WHO-defined thresholds for low semen quality. Percentage differences in mean log-transformed semen volume, sperm concentration, and total sperm count (95% CI) comparing current BMI ≥35 vs. <25 kg/m2 were -6.3 (-15.8, 4.3), -6.4 (-24.6, 16.2) and -12.2 (-31.1, 11.8), respectively. Percentage differences (95% CIs) comparing waist circumferences of ≥42 vs. <31 inches were -4.2 (-15.0, 8.0), -6.4 (-27.6, 21.0) and -10.4 (-31.9, 17.9) for semen volume, sperm concentration, and total sperm count, respectively. Greater adult weight gain since age 17 years was associated with reduced semen volume (≥25 vs. <5 kg: percent difference -9.7, 95% CI: -18.4, 0.1), but not sperm concentration or total sperm count. The highest categories of each anthropometric variable were generally associated with WHO-defined low total sperm count (≤39 million). Selected anthropometric factors were modestly associated with poorer semen quality.

Rhamnetin alleviates polystyrene microplastics-induced testicular damage by restoring biochemical, steroidogenic, hormonal, apoptotic, inflammatory, spermatogenic and histological profile in male albino rats.

The current research was performed to evaluate the ameliorative effects of Rhamnetin (RHM) on polystyrene microplastics (PS-MPs)-instigated testicular dysfunction in male albino rats. 48 albino rats were distributed in four groups, i.e., control, PS-MPs treated, PS-MPs + RHM co-treated and RHM only supplemented group. PS-MPs exposure considerably reduced anti-oxidant enzymes i.e., catalase (CAT), glutathione peroxidase (GSR), superoxide dismutase (SOD) and glutathione reductase (GPx) activities. Whereas, reactive oxygen species (ROS) level along with malondialdehyde (MDA) was considerably escalated in PS-MPs treated rats as well as a potential decline was observed in sperm progressive motility. Additionally, a substantial upsurge was noticed in the count of dead sperms, deformity in the tail, mid-piece and head of sperms in PS-MPs treated rats. PS-MPs exposure also decreased steroidogenic enzymes, 17β-hydroxysteroid dehydrogenase (17β-HSD), steroidogenic acute regulatory protein (StAR) and 3β-hydroxysteroid dehydrogenase (3β-HSD) expressions. Moreover, the levels of inflammatory indices i.e., Interleukin-6 (IL-6), Nuclear factor kappa-B (NF-κB), Interleukin-1β (IL-1β), tumour necrosis factor-α (TNF-α) and cyclooxygenase-2 (COX-2) activity were also increased in PS-MPs administrated group. Besides it increased the expression of apoptotic markers (Bax and caspase-3) expression. Whereas, anti-apoptotic marker i.e., Bcl-2 expression was reduced. Moreover, luteinizing hormone (LH), follicle-stimulating hormone (FSH) as well as plasma testosterone levels were also decreased. PS-MPs exposure also led to a substantial histopathological damage in testicular tissues. However, RHM supplementation potentially reduced the damaging effects of PS-MPs in the reproductive tissues of male albino rats. Thus, the current study revealed, RHM possesses potential to prevent PS-MPs-induced testicular damage due to its anti-oxidant anti-apoptotic, anti-inflammatory as well as androgenic properties.

Viral dynamics in patients with monkeypox infection: a prospective cohort study in Spain.

Monkeypox DNA has been detected in skin lesions, saliva, oropharynx, urine, semen, and stool of patients infected during the 2022 clade IIb outbreak; however, the viral dynamics within these compartments remain unknown. We aimed to characterise the viral load kinetics over time in various parts of the body. This was an observational, prospective, multicentre study of outpatients diagnosed with monkeypox in two hospitals and two sexual health clinics in Spain between June 28, 2022, and Sept 22, 2022. Men and women aged over 18 years were eligible if they reported having symptom onset within the previous 10 days of presentation, and were ineligible if disease was severe enough to be admitted to hospital. Samples were collected from five body locations (skin lesions, oropharynx, rectum, semen or vagina, and a dried blood spot) at six time points up to 57 days after the screening visit. Samples were analysed by quantitative PCR and a subset by cell culture. The primary endpoint was time from symptom onset to viral DNA clearance. Overall, 1663 samples were collected from 77 study participants. 75 (97%) participants were men, the median age was 35·0 years (IQR 29·0-46·0), and 39 (51%) participants were living with HIV. The median time from symptom onset to viral clearance was 25 days (95% CI 23-28) in the skin lesions, 16 days (13-19) in the oropharynx, 16 days (13-23) in the rectum, 13 days in semen (9-18), and 1 day in blood (0-5). The time from symptom onset to viral clearance for 90% of cases was 41 days (95% CI 34-47) in skin lesions and 39 days (27-56) in semen. The median viral load in skin lesions was 7·3 log10 copies per mL (IQR 6·5-8·2) at baseline, compared with 4·6 log10 copies per mL (2·9-5·8) in oropharyngeal samples, 5·0 log10 copies per mL (2·9-7·5) in rectal samples, 3·5 log10 copies per mL (2·9-4·7) in semen samples, and 4·0 log10 copies per mL (4·0-4·0) in blood specimens. Replication-competent viruses were isolated in samples with high DNA levels (>6·5 log10 copies per mL). In immunocompetent patients with mild monkeypox disease, PCR data alone would suggest a contact isolation period of 3 to 6 weeks but, based on detection of replication-competent virus, this time could be reduced. Based on findings from this cohort of patients, semen testing and prolonged use of condoms after recovery from monkeypox might not be necessary. Extremely Low Frequency-Electromagnetic Fields (ELF-EMF) Can Decrease Spermatocyte Count and Motility and Change Testicular Tissue

Substantial evidence indicates that exposure to extremely low frequency-electromagnetic fields (ELF-EMFs) affects male reproductive system. The goal of this study was to evaluate the effects of long-term irradiation with ELF-EMF on sperm quality and quantity and testicular structure. In this case-control study, sixty male Sprague-Dawley rats were randomly divided into six groups. Experimental groups were exposed to ELF-EMF (50 Hz EMF, 100 µT) for either 1 h/day for 52 days (Group 1), 4 h/day for 52 days (Group 3), 1 h/day for 5 days (Group 5), 4 h/day for 52 days (Group 7). Groups 2, 4, 6 and 8 were only sham exposed at durations equal to Groups 1, 3, 5 and 7, respectively. Both count and motility of sperms were significantly decreased in animals exposed to ELF-EMF (1 h/day for 52 days, 4 h/day for 52 day, and 4 h/day for 5 days) compared to the sham-exposed groups (P<0.05). Serum testosterone levels showed a significant decrease in the animals exposed to ELF-EMF (4 h/day for 5 days) compared to the control groups (P<0.05). A significant decrease was observed in the volume of the seminiferous tubules, seminiferous tubules epithelium and interstitial tissue in the animals exposed to ELF-EMF for 4 h/day for 5 days. Tubules length was also reduced by 18% in animals exposed to ELF-EMF (4 h/day for 5 days). Our results show that ELF-EMF can reduce spermatocyte count and motility and is able to induce structural changes in testicular tissue.

Protective effect of Ginkgo biloba and Olive oil on the reproductive performance of male albino rats exposed to oxidative stress induced by Potassium dichromate

The present study is designed to demonstrate the effect of Ginkgo biloba and Olive oil on the reproductive performance of male rats exposed to oxidative stress induced by Potassium dichromate with respect to male sex hormones, semen parameters and testicular antioxidant system,. Forty Wister albino rats were distributed into four groups and each group have ten rats, control group, Potassium dichromate group (Potassium dichromate 24 mg/kg administered with gavage for 2 month), Potassium dichromate treated with Ginkgo biloba group (Potassium dichromate and Ginkgo biloba 40 mg/kg administered at half an hour interval for 2 months) and Potassium dichromate treated with olive oil group (Potassium dichromate and Olive oil 1 ml/kg administered at half an hour interval for 2 months). The statistical analysis of results showed a significant (P&lt;0.05) decrease in sperms count, the percentage of sperms viability, the level of sex hormones, ceruloplasmin, glutathione concentration, and superoxide dismutase activity, antioxidant activity (AOA), germinal epithelium height and diameter of seminiferous tubules and a significant increase of sperm percentage abnormalities, dead sperm, malondialdelyde (MDA), peroxynitrite (ONOO-) in Potassium dichromate group, compared with control group. The administration of Potassium dichromate with Ginkgo biloba and Olive oil separately result in a significant increase of sperms count, sperms viability, sex hormones levels, ceruloplasmin, glutathione concentration , superoxide dismutase and antioxidant activity, germinal epithelium height and diameter of seminiferous tubules and a significant decrease of sperm abnormalities, dead sperm percentage, MDA, ONOO-. In conclusion the levels of antioxidant parameters, sperm count and sperm viability were restored to normal values when Potassium dichromate administered in combination with Ginkgo biloba or with Olive oil therefor Ginkgo biloba and Olive oil may be beneficial for improving reproductive performance of male rats.

Effects of photobiomodulation therapy on human sperm function

IntroductionSperm motility is a crucial factor in male infertility and it depends on mitochondrial tail movements. Photobiomodulation light therapy allows the cells to produce their energy through activation of the mitochondria. The aim of the present study was to examine the impact of photobiomodulation on sperm motility in astenozoospermic individuals. Materials and methodsFollowing semen analyses of 20 astenozoospermic individuals, collected semen samples were centrifuged. Pellet was obtained and homogenized through mixing with culture media in 1:1 ratio. Each semen samples were divided into 3 groups. In the first group, control samples were not exposed to laser irradiation. The Group 2 and Group 3 were exposed to 650nm wavelength of photobiomodulation from 10cm distance in dark environment via a 36cm2 aperture sizer with 200mW output power for 30 and 60min duration, respectively. Sperm motilities were evaluated and chromatin condensation of sperms was determined. ResultsSperm motilities were significantly increased in photobiomodulation groups compared with the controls. Sperm motilities tended to be different between the 30 and 60min red light exposure groups; however, it was not statistically significant. When the motility grades were compared, no significant difference was observed in non-progressive motility sperms. While immotile sperms decreased significantly in the photobiomodulation groups compared to the control group, progressive sperms increased. ConclusionsThe results of the present study demonstrated that the photobiomodulation is an efficient method to increase the sperm motility of astenozoospermic individuals independent of the duration of exposure.
