How professional quality of life is influenced by perceptions of empowerment in midwives

IntroductionProfessional quality of life is affected by various factors, such as people's perception of professional empowerment. This study aimed to investigate the role of midwives' personal perception of empowerment in their professional quality of life.MethodsThis cross-sectional study was conducted with 380 midwives providing maternal and neonatal care services across five provinces of Iran. Data were collected using virtual platforms and analysed using descriptive and analytical tests including regression analysis.ResultsThe mean professional quality of life was high (59.7%) in most participants. Midwives' personal perception of empowerment alone predicted 17% of professional quality of life.ConclusionsGiven midwives' role in maintaining and improving maternal and neonatal outcomes, it is important to devise policies and plans to enhance midwives' perception of empowerment, especially with regards to autonomous practice and management.

The increase in an unnecessary caesarean section can endanger the mother, baby and also threaten the sustainability of the financing of the National Health Insurance (NHI) program. This study aims to identify critical issues surrounding managing consumer intentions to perform unnecessary caesarean sections and identify the role of stakeholders. Data collection was carried out through Focus Groups Discussion (FGD) with 14 experts and In-Depth Interviews (IDI) with 6 postpartum women. We transcribed interviews during the FGD and IDI into an interview text. The method of content analysis was carried out on the interview text. This study highlights the importance of managing the intention of the caesarean section in participants of the NHI and the importance of the three stakeholders, namely the role of the Ministry of Health, the role of BPJS Kesehatan, and the role of professional organizations. In particular, it is necessary to increase the role of doctors, midwives, and health workers through professional organizations together with the Ministry of Health and BPJS Health in managing the intention of caesarean section to educate pregnant women, especially primigravida women. In addition, the Ministry of Health needs to immediately establish guidelines for caesarean section services for medical indications, which are value for money guaranteed in the JKN program. The Effect of Organizational Development and Work Behavior Towards the Role of Midwives in Prevention Mother Actions to Child Transmission (PMTCT) and Independence Care of Pregnant Women in the Pandemic Time Covid-19

This research was motivated by pregnant women who endured HIV/AIDS during the Covid-19 pandemic. Their self-sufficiency was low or very low because they were incapable generate a readiness to prevent and overcome the disease problems they suffered. The theory of reasoned action (TRA) developed into the theory of planned behavior (TPB). This conceptual framework refers to combining the Kopelmen's with the theory of planned behavior (TPB) in influencing midwives in PMTCT actions on HIV AND AIDS care independence. Data analysis uses Structural Equation Model analysis - SEM. It combines several aspects contained in path analysis and confirmatory factor analysis to estimate several equations simultaneously. The study found that organizational characteristics (X1) effect on the midwives' role (Y) had a positive and significant effect. The work characteristics (X2) effect on midwives' role (Y) is positive and significant. The organizational characteristics (X1) effect on the pregnant women's independence (Z) is positive and not significant. The work characteristics (X2) on the independence of pregnant women (Z) have a positive and significant effect, and the role of midwives (Y) on the independence of pregnant women (Z) has a positive and significant effect. The result of the indirect effect shows that organizational characteristics (X1) through the role of the midwife (Y) have a positive and insignificant effect on the independence of pregnant women (Z), and work behavior (X2) through the role of the midwife (Y) has a positive and significant effect on the independence of pregnant women ( Z). Research recommends improving the reward system, training, and development as well as the leadership of each midwife. To carry out their duties and functions of providing health services to HIV and AIDS care in each Puskesmas as a form of organizational characteristics that affect the role of midwives in PMTCT actions in Makassar City. Maintaining the achievement of performance goals and the resulting feedback as work characteristics of midwives that will affect the role of midwives in PMTCT actions in Makassar City.

Secondary traumatic stress in iranian midwives: stimuli factors, outcomes and risk management

BackgroundThe present qualitative study was conducted to explain the experiences of secondary traumatic stress (STS) and its related factors in midwives working in maternity wards.MethodsData were collected using semi-structured interviews with 11 midwives working in the maternity wards of hospitals in Urmia, Iran, through in-depth interviews with open-ended questions. Data were analyzed using the conventional content analysis approach.ResultsThe results of data analysis led to the extraction of three themes, seven main categories, and 18 subcategories. The first theme was “STS stimuli,” with the two categories of “Discriminatory approach to midwifery” and “The nature of the midwifery profession”. The second theme was “Traumatic outcomes”, which included the subcategories of “Psychological-emotional trauma”, “Physical trauma” and “Social trauma”. The third theme was “Risk management”, which had the two subcategories of “Reactive approach” and “Proactive approach”.Conclusions The results showed that, in addition to the traumatic nature of events that midwives experience during work as the secondhand victims, factors such as governance-organizational structure, unbalanced distribution of power, and poor supportive laws undermine their professional role and provide conditions conducive to STS. Therefore, avoiding traumatic situations and scientific and skill self-empowerment were the most important strategies adopted by the midwives in this study to prevent risky situations and cope with the consequences of STS. The participation of midwifery stakeholders in policy-making and adopting supportive legislation in redefining the position and role of midwives can play a major role in reducing STS and sustaining their role and position in maternal care.

Role of Nurses in Midwifery and Obstetrical Nursing

The main role of midwives and nurses in the maternity care has been changed in the last 20-30 years and they are most helpful in taking care of a pregnant woman and her family. In addition, the main importance of the antenatal care is the physical care of a mother that is throughout the overall maternity cycle. The role of the midwife and nurse within the maternity care involves delivery, labor, pregnancy and they provide the best healthcare to every pregnant mother. Hence, the nurses and midwives try their best to feel secure for the pregnant mothers in bringing their child into this world and also help them in growing up in a healthy and mature environment.

The Image of Nurses and Midwives in Ghana: Patient and Family Perspectives

This study aims to explore the perceptions of patients and family caregivers on the image of nurses and midwives in Ghana. The study adopted a qualitative exploratory descriptive approach. A total of 25 participants were interviewed during data collection. Content and thematic analysis were applied in the data analysis to develop themes. The findings are captured under two major themes that describe the primary influences on participant images of nurses: Thus, (1) nurses’ and midwives’ attributes with four subthemes; staff appearance, communication strategies and behaviors, work attitudes, and professional competence and (2) patients’ status and subthemes were; uneducated poor and educated rich. We conclude that patients and families in Ghana recognize the professional attributes of the nurse and midwife, which reflect in their personality, grooming, communication, competencies, and attitudes. However, low publicity of the professional roles of nurses and midwives may have negative repercussions for their professional image. A policy to perform a regular public audit on the image of the Nurse/Midwife is important for professional advancement.

Background: Cases related to adolescent health have increased during the COVID-19 pandemic. Midwives have a very important role in conducting counseling related to adolescent health. This is a form of commitment in fulfilling the right to health of adolescents. The role of the midwife is stated in terms of reproductive health in accordance with the Regulation of the Minister of Health of the Republic of Indonesia Number 28 of 2017 concerning Permits and Implementation of Midwifery Practices article 21a. The purpose of this research is to know the role of midwives in the implementation of adolescent counseling as the fulfillment of adolescent reproductive health rights.Methods: Qualitative research methods, with a sociological juridical approach. The study used in-depth interviews with midwife and adolescent informants. The research was conducted from May to June 2021. Data analysis was carried out by qualitative data analysisResults: The results of the study show that BIdans have carried out their authority and obligations in the role of counselors based on the Midwifery Law Number 4 of 2019 and the Regulation of the Minister of Health of the Republic of Indonesia Number 28 of 2017 concerning Permits and Implementation of Midwifery Practices article 21a. One of the efforts to treat vitamin A deficiency in postpartum women is by giving vitamin A capsules to postpartum women twice. Postpartum mothers who get enough vitamin A will increase the content of vitamin A in breast milk (ASI), so that breastfed babies are more immune to disease. In addition, the mother recovers her health faster. The purpose of this study was to determine the relationship between knowledge, education, attitudes of postpartum mothers and the role of midwives simultaneously with adherence to vitamin A consumption in the Work Area of ??Talang Pangeran Health Center, Ogan Ilir Regency in 2021. This type of research uses an analytical survey with a cross sectional approach. The sample size in this study was 59 respondents' mothers taken by accidental sampling. Data analysis using univariate analysis and bivariate analysis using chi square test. The results of the study showed that there was a relationship between knowledge (p value = 0.022), education (p value = 0.007), postpartum mother's attitude (p value = 0.013) and the role of midwives (p value = 0.038) with adherence to vitamin A consumption in the Talang Pangeran District Health Center Work Area. To Assess the Effectiveness of Planned Teaching on Knowledge Regarding Mid-Wife Led Continuity of Care Model (MLCC) on Final Year Nursing Students

Background: In many parts of the world, midwives are the primary providers of care for childbearing women. There are considerable variations in the organisation of midwifery services and in the education and role of midwives. Childbearing women are often faced with different opinions as to which option might be best for them. The midwife‐led continuity model of care is based on the premise that pregnancy and birth are normal life events. Pregnancy and birth are significant life events for women and their families and midwife supports a woman throughout pregnancy, birth, and the postnatal period. So, the demand for services that are family friendly, women focused, safe and accessible is increasing.
 Objectives: 1) To assess the existing knowledge regarding mid-wife led continuity of care model on final year nursing students. 2) To evaluate the effectiveness of planned teaching on knowledge regarding mid-wife led continuity of care model (MLCC) on final year nursing students. 3) To find out the association between knowledge regarding mid-wife led continuity of care model (MLCC) with selected demographic variables.
 Materials and Methods: In this study, evaluatory research approach and one group pre-test post-test research design was used. The study was conducted at a Selected Nursing College. The sample was collected from final year nursing students (120) by using non-probability convenient sampling technique.
 Results: In this study the study of the findings shows that 10.83% had good level of knowledge score in pre-test. While in post-test, out of 120 subjects the majority of the subjects and 92.50% had good level of knowledge score. This statistically shows that there is profound level of improvement on knowledge.
 Conclusion: Hence, this implies that the planned teaching was effective. Also, the study shows that the knowledge scores of final year nursing students is associated with the age in year and course of study. Hence, this means that greater the number of age in years and the course, greater level of knowledge scores. However, gender was not found to be associated.

Background: Postpartum birth control is one of the family planning programs that still have low coverage. Midwives have a big role in increasing postpartum family planning coverage. Postpartum mothers will often contact the midwife so that the midwife can advise the postpartum mother to use postpartum birth control. The aim of this study is to evaluate the role of midwives and postpartum mothers in the use of postpartum family planning in the Asahan regency
 Methods: This study used a cross-sectional design, the location study was Asahan regency, and the population was 81 postpartum mothers in Asahan Regency. The researcher used purposive sampling with the criteria of giving birth in the last one month and having more than two children. The instrument used was a questionnaire. This research will conduct cross-tabulation distribution.
 Results: The results of the cross-tabulation showed that 24 birth control users at the midwife's clinic gave birth at a midwife clinic, 18 people gave birth at a midwife clinic (22.2%), and 21 people had postnatal health checks (25, 9%), received advice on using postpartum family planning as many as 15 people (18.5%) and received family planning counselling after delivery as many as 14 people (17.3%). This study indicated that postpartum family planning was carried out in the midwife clinic as many as 18 people (22.2%) and in the hospital or public health centres as many as nine people (11.1%).
 Conclusion: Postpartum mothers who do postpartum health checks to midwives are more likely to use postpartum family planning than postpartum mothers who do not undergo postpartum health checks. Postpartum mothers who advised to use postpartum family planning from a midwife will have a greater risk of using postpartum FP than postpartum mothers who do not receive postpartum FP.

PEMBERIAN ASI EKSLUSIF TERHADAP PERKEMBANGAN ANAK BERDASARKAN KPSP DI PUSKESMAS MANGASA MAKASSAR

Breast milk is the best and most nutritious food for babies. Exclusive breastfeeding for the first 6 months, without additional food or drink is very important in the process of growth and development of babies such as increasing weight & weight. This basic growth will influence and determine the next child's development. The purpose of this study was to determine the effectiveness of exclusive breastfeeding for infants in monitoring the development of children aged 6-24 months in the Mangasa Health Center area of ​​Makassar City. This type of observational analytic research with a cross sectional approach, the population of mothers under five as many as 397 people, the sampling technique was purposive sampling as many as 30 people. The research data was tested using chi square. The results showed that there was an effect of the effectiveness of exclusive breastfeeding on child development using KPSP in the Mangasa Health Center Makassar City with a p value of 0.06. It is expected that the role of midwives is related to exclusive breastfeeding for child development by using KPSP in the work area of ​​the Mangasa Health Center Makassar City in 2021

The ASPIRE Study – 3. Reflections on Midwifery-led Research by Research Midwives: Qualitative Research

This is the third article in a series reflecting on the role of research midwives in the ASPIRE (Achieving Safe and Personalised maternity care In Response to Epidemics) study. Within our role as research midwives working on NIHR portfolio studies, there is limited opportunity to engage with qualitative research. ASPIRE has given us the chance to refresh our knowledge and learn new skills, and encouraged us to seek more midwifery-led, qualitative projects in the future. ASPIRE presented some challenges but also many opportunities, including gaining greater insight into the working practices of our midwifery colleagues. We encourage research midwives to embrace working on qualitative projects where possible and further explore the use of digital tools such as MS Teams.

Maternal health and Indigenous traditional midwives in southern Mexico: contextualisation of a scoping review.

ObjectivesCollate published evidence of factors that affect maternal health in Indigenous communities and contextualise the findings with stakeholder perspectives in the Mexican State of Guerrero.DesignScoping review and stakeholder fuzzy cognitive mapping.Inclusion and exclusionThe scoping review included empirical studies (quantitative, qualitative or mixed methods) that addressed maternal health issues among Indigenous communities in the Americas and reported on the role or influence of traditional midwives before June 2020. The contextualisation drew on two previous studies of traditional midwife and researcher perspectives in southern Mexico.ResultsThe initial search identified 4461 references. Of 87 selected studies, 63 came from Guatemala and Mexico. Three small randomised trials involved traditional midwives. One addressed the practice of traditional midwifery. With diverse approaches to cultural differences, the studies used contrasting definitions of traditional midwives. A fuzzy cognitive map graphically summarised the influences identified in the scoping review. When we compared the literature’s map with those from 29 traditional midwives in Guerrero and eight international researchers, the three sources coincided in the importance of self-care practices, rituals and traditional midwifery. The primary concern reflected in the scoping review was access to Western healthcare, followed by maternal health outcomes. For traditional midwives, the availability of hospital or health centre in the community was less relevant and had negative effects on other protective influences, while researchers conditioned its importance to its levels of cultural safety. Traditional midwives highlighted the role of violence against women, male involvement and traditional diseases.ConclusionsThe literature and stakeholder maps showed maternal health resulting from complex interacting factors in which promotion of cultural practices was compatible with a protective effect on Indigenous maternal health. Future research challenges include traditional concepts of diseases and the impact on maternal health of gender norms, self-care practices and authentic traditional midwifery.

The ASPIRE Study: A Midwifery-led Research Response to COVID-19 and Beyond

This is the first article in a series reflecting on the role of research midwives in the ASPIRE (Achieving Safe and Personalised maternity care In Response to Epidemics) study. In this article we introduce ASPIRE and provide an overview of the study. We also reflect on what makes ASPIRE uniquely midwifery-led within the National Institute for Health Research (NIHR) Portfolio and the role of research midwives within recruitment sites. We hope this article, and those that follow in the series, written by ASPIRE research midwives, will serve as a roadmap to inspire the next chapter of midwifery research in England.

: Understanding of pregnant women abouexclusive breastfeeding as evidenced by Exclusive breastfeeding is breastfeeding from birth until the child is 6 months old. During this period children are not expected to get additional fluids, such as formula milk, lemon juice, tea water, honey, and plain water. In exclusive breastfeeding, children are also not given additional food such as bananas, biscuits, rice porridge, teams, and so on. Exclusive breastfeeding for up to 6 months can meet the needs of children, and requires additional food at the age of over 6 months (Maryunani, 2012). The role of midwives in promoting exclusive breastfeeding can be started from the time the mother is pregnant. When pregnant women check their pregnancies, pregnant women will get information about the benefits of breastfeeding and how to successfully breastfeed when their babies are born (Atik, 2016). This activity was carried out in juni 2021, the Covid-19 pandemic then the participant who follows there were 20 mother pregnant and this activity went smoothly.

The Critical Role of Midwives in Safe Self-Managed Abortion.

As access to legal abortion in the United States becomes more complex, there is increasing interest in self-managed abortion. Choosing to seek abortion care outside the clinical setting can also help people marginalized or harmed by existing health care systems to access needed care in a way that feels safe and empowering. However, patients and midwives alike often have a lack of information about expected outcomes and potential complications that may arise, as well as how to manage these in a health care system that may make appropriate follow-up difficult to access if needed. This article discusses patient education as a harm-reduction approach, and reviews ways that midwives may strategically and ethically participate in this patient education need. As trusted health care providers who are expert in pregnancy and reproductive health, midwives are ideally positioned to meet patient knowledge needs around self-managed abortion.

Factors affecting the mother’s choice of infant feeding method in Poland: a cross-sectional preliminary study in Poland

BackgroundThe World Health Organization identifies exclusive breastfeeding as the gold standard for child nutrition. Breastfeeding provides many benefits to both the baby and the mother. First days after birth are crucial for breastfeeding and determining its further path. Unfortunately, very often it is also the time of facing the first breastfeeding problems. The aim of this study was to analyse selected factors influencing a mother’s decision to breastfeed.MethodsThis is a cross-sectional study conducted between March 2019 and November 2019 in selected Polish maternity wards. Data were collected through completion of author’s questionnaire and a standardised questionnaire and the Multidimensional Perceived Social Support Scale and also through informal interviews with mothers. During statistical analysis, the chi-square test of independence and the Kolmogorov–Smirnov test as well as the non-parametric Mann–Whitney U test in addition to Fisher’s exact test were performed.ResultsThe results showed that there is no correlation between the mode of delivery and the mother’s choice of infant feeding method. Knowledge of the benefits of breastfeeding and received support affects the mother’s decision on infant feeding method. The study also showed that the 83% of newborns were put to the breast within the first 2 h after birth. However, only 58% continued to be breastfed in the subsequent days. As many as 42% of the newborns were supplemented with formula despite early initiation of breastfeeding. Analyses showed that exclusive breastfeeding was more often chosen by mothers with higher education. And the most common breastfeeding problem reported by 62% of the respondents was perceived insufficient milk supply and also breastfeeding pain, reported in 48.5% mothers.ConclusionsIt is extremely important to continually promote breastfeeding among women. The role of midwives is crucial in this regard, as they should encourage the initiation of breastfeeding, support mothers during the perinatal and postnatal periods, and increase their sense of competence and confidence in their ability to breastfeed. Accessible, professional, and empathetic support can help reduce the number of women supplementing with modified milk. Undoubtedly, the postpartum period is important for breastfeeding outcomes, but it is significant that breastfeeding education and promotion begin during pregnancy.
