Improving the quality of computed tomography brain images in the presence of cochlear implant induced metal artefacts through the additional use of tissue mimicking materials alongside metal artefact reduction software

IntroductionMetal artefact reduction software (MAR) can be used to improve Computed Tomography (CT) image quality in the presence of implanted metalwork; however, this software is not effective for superficial metallic structures such as cochlear implants (CI).This study aimed to investigate whether the effectiveness of MAR software could be improved for brain scans with CI present through the use of tissue mimicking materials (TMM) placed exteriorly to the implant. MethodsIn this two-part study, a CI was positioned on the surface of water and anthropomorphic phantoms and imaged using a helical CT brain protocol. Three TMM, Superflab, Sure Thermal heat packs, and Bart's Bolus, were utilised and images were acquired to assess the resulting artefact reduction in terms of CT numbers, noise and artefact index (Aind). Changes in CTDIvol were assessed for the anthropomorphic phantom scans. ResultsIn the water phantom, statistically significant reductions in CT number (p = 0.038) and noise (p = 0.033) were observed for Superflab, whilst the heat packs produced similar significant reductions in CT number (p < 0.001) and noise (p = 0.001) for the anthropomorphic phantom images. Aind values were significantly reduced through the use of Superflab (p = 0.009) and the heat packs (p < 0.001). No significant effects were observed for Bart's Bolus. CTDIvol increases of generally less than 5% were observed for scans with TMM in place. ConclusionThe additional use of TMM alongside MAR software yielded statistically significant reductions in CI induced metal artefacts on both water and anthropomorphic phantom scans with minimal dose increases. Implications for practiceThe extent of metal artefacts in clinical head scans with CI in place could be significantly reduced through combined use of TMM and MAR software, consequently providing greater diagnostic confidence in the images.

Metal Artifact Reduction by Using MARS System of a CBCT Unit with Different Components of Fixed Orthodontic Appliances: An In vitro CBCT Analysis

Introduction: During Cone beam computed tomography (CBCT) imaging in the head and neck region of patients who are undergoing fixed orthodontic treatment, the different components of fixed orthodontic appliances can produce various metal artifacts which may degrade the diagnostic image quality in the region of interest. Therefore, the metal artifact reduction software (MARS) system can be used in most CBCT units to reduce the amount of artifacts generated from various fixed orthodontic appliance components including skeletal anchorage devices. Aims and objectives: The main purpose of this study was to quantitatively evaluate the effectiveness of MARS system of a CBCT unit in reducing the amount of artifacts generated by various fixed orthodontic appliance components during CBCT imaging. Materials and methods: Four dental arch shaped phantoms that were fabricated along with different fixed orthodontic appliance components (metal bracket, molar band, transpalatal arch, mini-implant) underwent CBCT scanning without and with the MARS setting option. After using the MARS system of a CBCT unit, the reduction in the amount of artifacts that were generated by various fixed orthodontic appliance component was calculated by using Google Colab software that utilized Python code and OpenCV system to generate a binary mask image. The resulting mask image highlights the areas where artifacts are detected, allowing for further analysis or visual inspection. The contours found within the mask image represent the boundaries of the artifact regions. Results: After calculating the percentage decrease in artifact area for each component of fixed orthodontic appliance, one-way analysis of variance (ANOVA) test was performed to compare the percentage decrease in artifact area for each component of fixed orthodontic appliances. *P-values are calculated using one-way analysis of variance ( p &lt; 0.05). The percentage decrease in artifact area for different components of fixed orthodontic appliance ranged from 39.67% to 48.58%. The stainless steel bracket showed the significant artifact area reduction with a mean percentage decrease of 48.58% as compared to other components. Conclusion: The effect of MARS system in reducing the amount of metal artifacts generated varied according to the component type of fixed orthodontic appliance and, MARS system was more efficient for stainless steel brackets as compared to other components. CBCT imaging of head and neck region can be carried out without taking off the orthodontic appliances by applying MARS system which can significantly decrease the artifact area.

Quantitative Assessment of Lipiodol-Related Artifact Reduction for Dual-Energy Computed Tomography After Transcatheter Arterial Chemoembolization: A Phantom Study Evaluating the Use of Metal Artifact Reduction Algorithms.

This study used metal artifact reduction (MAR) software to examine the computed tomography (CT) number of dual-energy CT (DECT) of hepatocellular carcinoma after transcatheter arterial chemoembolization. Hollow columnar acrylic phantoms were filled with lipiodol and inserts of 2 sizes (large and small) were used to simulate liver tumors on a Revolution GSI CT scanner. The CT numbers of a single test object were collected twice: once with and once without the MAR algorithm. Lipiodol beam-hardening artifacts were quantified by measuring CT numbers in a region of interest around the tumor-simulating insert. The virtual monochromatic CT numbers of large and small tumors were closely related to energy. For small tumors, CT numbers increased with energy. For large tumors, CT numbers increased with energy at 1 cm from the margin but decreased with an increase in energy at 5 cm. Regardless of the size, distance, or location of the tumor, the CT numbers fluctuated more at low energy levels. At 1 cm from the margin, the CT numbers with MAR were significantly different from those without MAR. Low-energy CT numbers with MAR were near reference values. Metal artifact reduction exhibited superior performance for small tumors. Tumor margin images are affected by artifacts caused by Lipiodol. However, with MAR, CT numbers can be effectively calibrated, thus enabling clinicians to more accurately evaluate hepatocellular carcinoma development and identify residual tumors and recurrent or metastatic lesions.

Effect of Dental Distance from Implant and MAR Algorithm on the Detection of Vertical Root Fracture in CBCT Images

Three-dimensional cone-beam computed tomography (CBCT) has an important role in the detection of vertical root fractures (VRFs). The effect of artifact generation by high-density objects like dental implants on image quality was well documented. This study aimed to assess the effect of tooth-implant distance and the application of metal artifact reduction (MAR) algorithm on the detection of VRFs on CBCT scans. This study was conducted on 20 endodontically treated single-rooted teeth. VRFs were induced in 10 teeth, while the other 10 remained intact. The implant was inserted in the right second premolar socket area, and two teeth were inserted in right canine and right first premolar sockets area randomly and underwent CBCT with and without the application of MAR algorithm. SPSS 21 was used to analyze the results (alpha=0.05). According to the findings of this study, all four variables of sensitivity, specificity, accuracy, and positive predictive values in diagnosis were higher in cases without MAR software at both close(roots in first premolar sockets) and far distances (roots in canine sockets) from the implant. However, the highest rate of diagnosis accuracy of the first and second radiologists was in the far distance group from the implant without MAR, and the lowest rate of diagnosis accuracy in the first and second radiologists was in the close distance to the implant. Applying MAR algorithm had no positive effect on detection of VRFs on CBCT scans in both close and distant scenarios.

Dual-Energy Computed Tomography Applications to Reduce Metal Artifacts in Hip Prostheses: A Phantom Study

Metal components of hip prostheses cause severe artifacts in CT images, influencing diagnostic accuracy. Metal artifact reduction (MAR) software and virtual monoenergetic reconstructions on dual-energy CT (DECT) systems are possible solutions that should be considered. In this study, we created a customized adjustable phantom to quantify the severity of artifacts on periprosthetic tissues (cortical and spongious bone, soft tissues) for hip prostheses. The severity of artifacts was classified by different thresholds of deviation from the CT numbers for reference objects not affected by artifacts. The in vitro setup was applied on four unilateral and three bilateral configurations of hip prostheses (made of titanium, cobalt, and stainless steel alloys) with a DECT system, changing the energy of virtual monoenergetic reconstructions, with and without MAR. The impact of these tools on the severity of artifacts was scored, looking for the best scan conditions for the different configurations. For titanium prostheses, the reconstruction at 110 keV, without MAR, always minimized the artifacts. For cobalt and stainless-steel prostheses, MAR should always be applied, while monoenergetic reconstruction alone did not show clear advantages. The available tools for reducing metal artifacts must therefore be applied depending on the examined prosthetic configuration.

Metal artifact reduction in kV CT images throughout two-step sequential deep convolutional neural networks by combining multi-modal imaging (MARTIAN)

This work attempted to construct a new metal artifact reduction (MAR) framework in kilo-voltage (kV) computed tomography (CT) images by combining (1) deep learning and (2) multi-modal imaging, defined as MARTIAN (Metal Artifact Reduction throughout Two-step sequentIAl deep convolutional neural Networks). Most CNNs under supervised learning require artifact-free images to artifact-contaminated images for artifact correction. Mega-voltage (MV) CT is insensitive to metal artifacts, unlike kV CT due to different physical characteristics, which can facilitate the generation of artifact-free synthetic kV CT images throughout the first network (Network 1). The pairs of true kV CT and artifact-free kV CT images after post-processing constructed a subsequent network (Network 2) to conduct the actual MAR process. The proposed framework was implemented by GAN from 90 scans for head-and-neck and brain radiotherapy and validated with 10 independent cases against commercial MAR software. The artifact-free kV CT images following Network 1 and post-processing led to structural similarity (SSIM) of 0.997, and mean-absolute-error (MAE) of 10.2 HU, relative to true kV CT. Network 2 in charge of actual MAR successfully suppressed metal artifacts, relative to commercial MAR, while retaining the detailed imaging information, yielding the SSIM of 0.995 against 0.997 from the commercial MAR.

The impact of a metal artefact reduction algorithm on treatment planning for patients undergoing radiotherapy of the pelvis.

Metallic implants cause artefacts in computed tomography (CT) images and can introduce significant errors to structure visualisation and dosimetric calculation within the radiotherapy planning process. This study evaluated an orthopaedic metal artefact reduction algorithm and its effect on the CT number, image noise, structure delineation, and treatment dose. Raw CT data were reconstructed using standard filtered back projection and an artefact reduction algorithm to create 'standard' and 'corrected' images. A phantom containing tissue-mimicking inserts and two titanium plugs was imaged. The average CT number was compared to baseline data acquired without metal inserts. Data from 11 pelvic external beam radiotherapy (EBRT) patients with bi- or uni-lateral hip implants were retrospectively analysed. The clinically used treatment plans were re-computed on the corrected images. A prostate-mimicking phantom containing metal 'implants' was imaged, and 11 observers contoured both reconstructions. The artefact reduction algorithm improved the CT number in those areas most affected by metal artefacts and decreased noise by 19% (P=.04) Changes in dose distributions on corrected images compared to those calculated using the current clinical protocol were clinically insignificant. Volumes contoured on the corrected phantom images had larger Dice coefficients than those contoured on the standard images (P=.001), as well as a 36% lower standard deviation in volumes. This study demonstrates that the metal artefact reduction software reduces the error in CT numbers, can improve delineation accuracy, and can reduce inter-observer variability. It has the potential to streamline the planning pathway and improve treatment planning accuracy.

Application of metal artifact reduction software in gemstone spectral computed tomography for patients after total knee arthroplasty

BackgroundTo explore the feasibility and effectiveness of the metal artifact reduction software (MARs) reconstruction algorithm in reducing metal artifacts of knee prostheses and to explore the optimal monochromatic level of virtual monochromatic spectral (VMS) images for artifact reduction to provide high-quality images and reliable diagnosis in patients after total knee arthroplasty (TKA).MethodsA total of 31 patients underwent gemstone spectral computed tomography. VMS images with MARs and without MARs were obtained at different energy levels (80, 100, 120, and 140 keV). Two observers scored each group of images, and interobserver agreement was evaluated. Artificial indices (AIs), percentage500HU and structural similarity index measure (SSIM) values were calculated in the objective analysis to evaluate the image quality and impact of metal artifacts.ResultsThe consistency of the scores of the 2 observers was good (kappa value =0.78), and the score of the VMS images with MARs was higher than that of VMS images without MARs. AI values and percentage500HU of the MARs group were significantly lower than those of the without MARs group, while SSIM values were significantly higher. In the comparison of different keV images, the AI value decreased with the increase in keV in the range of 80–120 keV, but there was no significant difference between the 120 keV images and 140 keV images. In the group with MARs, the percentage500HU of 100–140 keV images was significantly lower than that of the 80 keV images, but there was no significant difference between 100, 120, and 140 keV images. In the group without MARs, the percentage500HU was significantly different among all keV groups.ConclusionsVMS images combined with the MARs algorithm can significantly reduce the metal artifacts of knee prostheses and improve image quality. At an energy level of 100–120 keV, a good metal artifact removal effect and soft tissue contrast can be achieved, and the best metal artifact removal effect can be achieved at 140 keV.

Reduction of Metal Artifacts Caused by Titanium Peduncular Screws in the Spine by Means of Monoenergetic Images and the Metal Artifact Reduction Software in Dual-Energy Computed Tomography.

Objectives:To evaluate the reduction of metal artifacts in patients with titanium peduncular screws in the spine using (1) conventional images (CI), (2) virtual monoenergetic reconstructions (VMRs), and (3) VMR + Metal Artifact Reduction Software (VMR + MARS), with dual-energy computed tomography (DECT).Materials and Methods:Twenty-four patients with titanium peduncular screws in the spine were studied using a 64-channel DECT. During the postprocessing phase, the CI, the VMRs from 100 to 140 keV, and the VMR at 140 keV + MARS were synthesized. All the images were considered, and a quantitative evaluation was performed measuring the attenuation values (in terms of Hounsfield Units) with region of interest, in correspondence with the most hyperdense and hypodense artifacts. All the values were then compared. A qualitative evaluation, in terms of image quality and extent of artifacts, was also performed by two radiologists.Results:In quantitative terms, the 140 keV + MARS reconstruction was able to significantly reduce both bright and dark metal artifacts, compared to CI and to VMRs. The VMR was capable of significantly reducing both dark and bright artifacts, compared to CI. In qualitative terms, the VMR at 140 keV proved to be the best, compared to CI and VMR + MARS images.Conclusions:The VMR + MARS image reduces metal artifacts from titanium peduncular screws more than VMRs alone and CI. Furthermore, the VMR can decrease metal artifacts from a quantitative and a qualitative point of view. Combining information from VMRs and VMR + MARS images could be the best way to solve the issue of metal artifacts on computed tomography images.

Iterative Metal Artifact Reduction (iMAR) of the Non-adhesive Liquid Embolic Agent Onyx in Computed Tomography

BackgroundA drawback of Onyx, one of the most used embolic agents for endovascular embolization of intracranial arteriovenous malformations (AVM), is the generation of imaging artifacts (IA) in computed tomography (CT). Since these artifacts can represent an obstacle for the detection of periprocedural bleeding, this study investigated the effect of artifact reduction by an iterative metal artifact reduction (iMAR) software in CT in a brain phantom.MethodsTwo different in vitro models with two-dimensional tube and three-dimensional AVM-like configuration were filled with Onyx 18. The models were inserted into a brain imaging phantom and images with (n = 5) and without (n = 10) an experimental hemorrhage adjacent were acquired. Afterwards, the iMAR algorithm was applied for artifact reduction. The IAs of the original and the post-processed images were graded quantitatively and qualitatively. Moreover, qualitative definition of the experimental hemorrhage was investigated.ResultsComparing the IAs of the original and the post-processed CT images, quantitative and qualitative analysis showed a lower degree of IAs in the post-processed images, i.e. quantitative analysis: 2D tube model: 23.92 ± 8.02 Hounsfield units (HU; no iMAR; mean ± standard deviation) vs. 5.93 ± 0.43 HU (with iMAR; p < 0.001); qualitative analysis: 3D AVM model: 4.93 ± 0.18 vs. 3.40 ± 0.48 (p < 0.001). Furthermore, definition of the experimental hemorrhage was better in the post-processed images of both in vitro models (2D tube model: p = 0.004; 3D AVM model: p = 0.002).ConclusionThe iMAR algorithm can significantly reduce the IAs evoked by Onyx 18 in CT. Applying iMAR could thus improve the accuracy of postprocedural CT imaging after embolization with Onyx in clinical practice.

Metal Artifact Reduction With Tin Prefiltration in Computed Tomography: A Cadaver Study for Comparison With Other Novel Techniques.

With the aging population and thus rising numbers of orthopedic implants (OIs), metal artifacts (MAs) increasingly pose a problem for computed tomography (CT) examinations. In the study presented here, different MA reduction techniques (iterative metal artifact reduction software [iMAR], tin prefilter technique, and dual-energy CT [DECT]) were compared. Four human cadaver pelvises with OIs were scanned on a third-generation DECT scanner using tin prefilter (Sn), dual-energy (DE), and conventional protocols. Virtual monoenergetic CT images were generated from DE data sets. Postprocessing of CT images was performed using iMAR. Qualitative (bony structures, MA, image noise) image analysis using a 6-point Likert scale and quantitative image analysis (contrast-to-noise ratio, standard deviation of background noise) were performed by 2 observers. Statistical testing was performed using Friedman test with Nemenyi test as a post hoc test. The iMAR Sn 150 kV protocol provided the best overall assessability of bony structures and the lowest subjective image noise. The iMAR DE protocol and virtual monochromatic image (VMI) ± iMAR achieved the most effective metal artifact reduction (MAR) (P < 0.05 compared with conventional protocols). Bony structures were rated worse in VMI ± iMAR (P < 0.05) than in tin prefilter protocols ± iMAR. The DE protocol ± iMAR had the lowest contrast-to-noise ratio (P < 0.05 compared with iMAR standard) and the highest image noise (P < 0.05 compared with iMAR VMI). The iMAR reduced MA very efficiently. When considering MAR and image quality, the iMAR Sn 150 kV protocol performed best overall in CT images with OI. The iMAR generated new artifacts that impaired image quality. The DECT/VMI reduced MA best, but experienced from a lack of resolution of bony fine structures.

Dose perturbation caused by metallic port in breast tissue expander in proton beam therapy

Proton beam treatment is being looked favourably now in breast treatment. Tissue expanders are often placed after mastectomy that contains metallic port for saline injection which produces dose perturbations in proton beam therapy with uncertain dosimetry. Dose perturbation for a stainless-steel injection port from a breast implant is investigated in this study. Measurements, Monte-Carlo simulation, and calculated dose distribution of plans based on kVCT and MVCT images are compared. Treatment plans are performed on kVCT and MVCT images to observe the effect of metal artifact from the breast implant. The kVCT based plan underestimates the beam range due to the overestimated water equivalent thickness of the metal ports as a result of image degradation. Compared to the measurement with metal port in the proton beam, the MVCT-based treatment planning provides more accurate dose calculation than the kVCT-based results. The dose perturbation factor calculated from MVCT planning is within 10% of the measurement results while HU corrected kVCT plan still shows dose difference as large as 100% due to the incorrect range pull back calculation caused by the misrepresentation of the volume between the plastic cap and the stainless-steel base. The dose enhancement observed at the metal and solid water interface is as large as 15%, which needs to be accounted for in the planning process if there is a clinical concern. Dose reduction as large as 16% is observed with depth from 1 cm to 4 cm underneath the thickest part of the metallic port whereas lateral dose perturbation is also seen up to 7 mm. The measurement data are supported by the Monte-Carlo simulated results with a maximum dose difference of 6%. It is concluded that if proton beam is used with metallic port, MVCT imaging data is recommended. In lieu of MVCT, DECT, CT scanner with metal artifact reduction software or in the very least, extended HU range should be used to reduce the streaking artifact as well as to produce a more accurate image of the metallic port.

Evaluation of Metal Artifact Reduction software in Computed Tomography

Introduction: The image quality of computed tomography (CT) can be seriously lowered by metal implants of patients. These implants are known to exert a significant impact on diagnostic accuracy due to artifacts. The current study aimed to assess the usefulness of Metal Artifact Reduction (MAR) software in the reduction of metal artifacts, in comparison to iterative reconstruction algorithm (IDREAM). Material and Methods: Water phantom with raw chicken leg underwent CT scan (Sinovision, Insitum 16) before (reference group (Gp ref) and after metal implantation: ((GPA (IDREAM without MAR) and GPB(IDREAM with MAR)). A total number of 30 patients [GP1 (instrumented spine (n=15)), GP2 (Brain clips (n=15))] underwent CT scan (Sinovision ,Insitum 16). GP1 and GP2 were reconstructed using two procedures including IDREAM without MAR vs. 2: IDREAM with MAR. All images were evaluated using subjective and quantitative assessment. Results: In subjective image quality assessment, the scores of MAR images were higher than IDREAM images (p <0.05) as indicated by four radiologists. The absolute CT difference (ΔCT) and Artifact index (AI) demonstrated that MAR appeared to be superior for the reduction of metal artifacts (p <0.05). Conclusion: As evidenced by the obtained results, MAR software can be efficiently used for metal artifact reduction in computed tomography (instrumental spine and brain clips).

Metal artifact reduction using mono-energy images combined with metal artifact reduction software in spectral computed tomography: a study on phantoms.

This study aimed to evaluate the effectiveness of spectral computed tomography (CT) mono-energy imaging combined with metal artifact reduction software (MARs) for metal implant artifact reduction using a phantom. A quantitative standard phantom with 9 cylinders was used to simulate the attenuation of the different tissues of the human body around the metal implant. Groups A and B were divided according to conventional CT scan mode and spectral CT scan mode. Three sets of reconstructed images, including 120 kVp-like + MARs images, mono-energy images (MonoE), and MonoE + MARs images, were generated after spectral CT scanning. High-attenuation artifacts and low-attenuation artifacts were observed around the coil in the images of groups A and B. The CT values (Hounsfield unit) and standard deviation (SD) values of the artifacts were measured, and the artifact index and hardening artifact removal rate were calculated. Compared to conventional poly-energy CT images, for high-attenuation and low-attenuation artifacts, the artifact indices of 120 kVp-like + MARs, MonoE, and MonoE + MARs images were all reduced significantly. The hardening artifact removal rates of the high-attenuation and low-attenuation artifacts of 120 kVp-like + MARs images were 82% and 92%, respectively. The hardening artifact removal rate of the high-attenuation and low-attenuation artifacts of MonoE and MonoE + MARs images increased with the mono-energy level. Spectral CT using the 120 kVp-like + MARs, 110-140 keV MonoE, and MonoE + MARs reconstruction methods can reduce metal implant artifacts in varying degrees. MonoE + MARs reconstruction was the best method for reducing metal artifacts.

Comparison of quantitative measurements of four manufacturer's metal artifact reduction techniques for CT imaging with a self-made acrylic phantom.

BACKGROUND: Metal artifact reduction (MAR) techniques can improve metal artifacts of computed tomography (CT) images.OBJECTIVE: This work focused on conducting a quantitative analysis to compare the effectiveness of four commercial MAR techniques on three types of metal implants (hip implant, spinal implant, and dental filling) with a self-made acrylic phantom.METHODS: A cylindrical phantom was made from acrylic with a groove in the middle, and then three types of metal implants were placed in the groove. The phantom was scanned by four CT scanners and four commercialized MAR techniques were used to analyze the images. The techniques used were single-energy metal artifact reduction (SEMAR, Canon), smart metal artifact reduction software (Smart-MAR, GE), iterative metal artifact reduction (IMAR, Siemens), and metal artifact reduction for orthopedic implants (OMAR, Philips). Quantitative analysis methods included objective and subjective analysis.RESULTS: The expected value of SEMAR, Smart-MAR, IMAR, and OMAR were 36.6, 37.8, 5.0, and 2.3, respectively. SEMAR and Smart-MAR achieved optimal results.CONCLUSION: This study successfully evaluated the effects of four commercial MAR techniques on three types of metal implants in a phantom. All MAR techniques effectively reduced metal artifacts, but the effect was not significant with dental fillings due to high-density material.

This study aimed to evaluate the image quality and degree of metal artifact reduction using the new-generation gemstone spectral imaging (GSI) and metal artifact reduction software (MARs) and to demonstrate the optimal monochromatic energy level for dual-energy cerebral computed tomography angiography (CTA) in patients with intracranial aneurysm after endovascular treatment. A total of 20 patients with cerebral aneurysms treated with coils or clips underwent CTA using gemstone spectral computed tomography. Artifact index was calculated at each energy level with and without MARs. Signal-to-noise (SNR) and contrast-to-noise (CNR) ratios were calculated on all axial images with MARs; subjective evaluation was done by using a 4-point scale and a 3-point scale for assessing noise and vessel contrast, respectively, and compared between the monochromatic energy levels. The artifact index value of group GSI-MARs was significantly lower than that of group GSI at each monochromatic energy level (all, P < 0.01). Contrast-to-noise ratio and SNR of the parent arteries decreased as the energy increased from 40 to 140 keV in group GSI-MARs (all, P < 0.01). Signal-to-noise ratio and CNR between each 2 adjacent monochromatic energy level showed significant difference (all, P < 0.01). Subjective evaluation showed that a monochromatic energy level between 40 and 70 keV provided the optimal image quality. Gemstone spectral imaging with MARs could reduce metal artifacts and improve the image quality of cerebral CTA after coil or clip treatment. The new generation of GSI could provide better CNR and SNR at lower energy level, and the best image quality was obtained at energy level 40 to 70 keV for GSI-MARs.

Quantitative assessment of three vendor’s metal artifact reduction techniques for CT imaging using a customized phantom

A metal implant was placed in an acrylic phantom to enable quantitative analysis of the metal artifact reduction techniques used in computed tomography (CT) scanners from three manufacturers. Two titanium rods were placed in a groove in a cylindrical phantom made by acrylic, after which the groove was filled with water. The phantom was scanned using three CT scanners (Toshiba, GE, Siemens) under the abdomen CT setting. CT number accuracy, contrast-to-noise ratio, area of the metal rods in the images, and fraction of affected pixel area of water were measured using ImageJ. Different iterative reconstruction, dual energy, and metal artifact reduction techniques were compared within three vendors. The highest contrast-to-noise ratio of three scanners were 85.7 ± 8.4 (Toshiba), 85.9 ± 11.7 (GE), and 55.0 ± 14.8 (Siemens); and the most correct results of metal area were 157.1 ± 1.4 mm2 (Toshiba), 155.0 ± 1.0 (GE), and 170.6 ± 5.3 (Siemens). The fraction of affected pixel area obtained using single-energy metal artifact reduction of Toshiba scanner was 2.2% ± 0.7%, which is more favorable than 4.1% ± 0.7% obtained using metal artifact reduction software of GE scanner (p = 0.002). Among all quantitative results, the estimations with fraction of affected pixel areas matched the effect of metal artifact reduction in the actual images. Therefore, the single-energy metal artifact reduction technique of Toshiba scanner had a desirable effect. The metal artifact reduction software of GE scanner effectively reduced the effect of metal artifacts; however, it underestimated the size of the metal rods. The monoenergetic and dual energy composition techniques of Siemens scanner could not effectively reduce metal artifacts.

Establishing Guidelines for Quality Assurance and Clinical Application of Metal Artifact Reduction (O-MAR) Software in Radiation Therapy

During CT simulation, metal and other high-density implants in the field of view can cause severe artifacts that obscure targets and affect radiation therapy planning. Metal Artifact Reduction for Orthopedic Implants (O-MAR, Philips Healthcare) has been shown to improve image quality by recovering structure delineation, correcting CT number accuracy and decreasing image noise. However, application of O-MAR does not perform successfully in some clinical situations and it is contraindicated by the vendor.
