The Effect of Premolar Extraction on Tooth Size Discrepancies of the Patients Treated in Orthodontic Department at BSMMU

Aims: The present cross sectional study was carried out to investigate the effect of first premolar extraction on tooth size discrepancies (TSD) in an orthodontic population seeking treatment at orthodontic department, BSMMU.Material and methods: Mesiodistal tooth widths were measured on 50 pair of pretreatment and post treatment dental casts of subjects with Class I variation – Class I with bimaxillary proclination, and Class I crowding. The overall ratios and tooth-size discrepancies were determined before and after 1st premolar extractions. Before and after extractions, the subjects were divided into Bolton small (BS), Bolton normal (BN) and Bolton large (BL) overall ratio groups categorized by the Bolton standard deviation definition.Results: The Bolton overall ratio after 1st premolar extraction was smaller than those before extraction in each group. Some of the patients in the BN group and BB group were moved into the BS group and BN group respectively after 1st premolar extraction. A tooth size discrepancy occurred in some patients with normal overall ratios after premolar extractions, but a tooth size discrepancy might be corrected in some patients with big overall ratios after premolar extraction.Conclusion: Orthodontists should take into account the following when devising a treatment plan for premolar extractions, overall ratios might decrease and remain normal, and clinically significant tooth-size discrepancies could change mutually after extractions.Ban J Orthod & Dentofac Orthop, April 2015; Vol-5 (1-2), P.13-16

Tooth width Ratio in Crowding and Noncrowding dentition among patient Attending BSMMU

Crowding is a common presentation among thepatients seeking orthodontic treatment. The cause of crowding could be various such as mesiodistal tooth width discrepancy, boney discrepancy among the jaw, earlyloss of deciduous molar, different patterns of malocclusion, ethnic group, nutritional and developmental factor.Materials and Methods: One hundred cases were selected among which 31 were male and 69 were female, 70 cases had crowded dentition and 30 cases had non crowded dentition. Bolton’s Overall ratio and Anterior ratio were calculated for every case and comparison was done among crowded and non crowded dentitions.Result: Mean Overall ratio and mean anterior ratio were found to be 91.06% and 79.66% Neither Overall ratio nor Anterior ratio was significant for crowding and non crowding cases. Odontometry and skull anthropometry: A possible tool for stature estimation using minimum armamentarium

Introduction: In the Asia Pacific region, because of variation in topography and climatic conditions, India is a disaster-prone country. The identification of an individual is one of the most important aspects of forensic medicine. There are different methods for identification of a person's age and gender. However, there are relatively fewer methods to estimate the stature of an individual. Stature correlation to skull and jaw dimensions is less frequently reported. The aim of the study is to investigate the relationship between height of the person and diameter of the head (DH) and circumference of the head with the combined mesiodistal (MD) width of maxillary anterior teeth using minimum and easily available armamentarium. Materials and Methods: Fifty model casts of students were considered to measure the MD width of the anterior teeth along with the circumference of the head and DH (distance from glabella to inion) with the help of a nonstretchable measuring tape. Heights of the students were recorded in inches. All the findings were tabulated and statistically analyzed. Observations and Results: When combined MD width of the maxillary anterior teeth was plotted against height, a statically significant correlation was seen. When head circumference was plotted against height, a higher correlation was seen. When two measurements were added and plotted against height, improved elevated correlation was seen. Similarly, when the MD width was added to circumference of the head and DH, a greater correlation was observed. Conclusion: It can be safely concluded that with more data to prove the study, a formula can be drawn to estimate the height of the person using MD width of the anterior teeth, DH, and circumference of the head.

Correlation between Interalar Distance and Mesiodistal Width of Maxillary Anterior Teeth in Thrissur, Kerala, Indian Population.

Aims and Objectives:The aim of this study was to assess the validity of interalar distance as an aid in maxillary anterior teeth selection in Thrissur, Kerala, Indian population. The study also evaluated whether interalar distance and maxillary anterior teeth undergo any changes as age increases in this population and whether there is a gender difference in these parameters in this population.Materials and Methods:A study was planned on 1200 participants in Thrissur Municipal Corporation area, Kerala, India, with the aim of checking the validity of interalar distance as a successful aid in maxillary anterior teeth selection. The collected data were analyzed using SPSS version 21.0. Mean standard deviation and 95% confidence intervals were estimated. Student's t-test was used for comparison between gender and two age groups such as 18–25 and 40–50 years. One-way ANOVA analysis was done for combined effects.Results:The study showed that there is a high statistical significance between the interalar distance and the mesiodistal width of six maxillary anterior teeth in females (P < 0.01) and no significance in males.Conclusion:The study concluded that, within the population evaluated, there was a high significant correlation between interalar distance and the mesiodistal width of six maxillary anterior teeth in both males and females. Both interalar distance and mesiodistal width of maxillary anterior teeth showed a significant increase in size as age increases. There was a significant increase in size difference for males compared to females in relation to interalar distance and not for mesiodistal width of maxillary anterior teeth.

Validity and Reproducibility of Digital Models Obtained by Cone Beam Computed Tomography Imaging

Introduction: Impression making and fabrication of plaster models serve as a basis for documentation, diagnosis and treatment planning. Model analysis is used routinely in orthodontics and is a key factor for treatment planning and review of orthodontic progress. Recently newer technologies in imaging and digitalisation have made it possible to obtain 3-Dimensional (3D) virtual models. Virtual models score over plaster models with respect to accessibility, storage and long life. However, there are concerns over its accuracy and reproducibility. Aim: To evaluate the accuracy of three dimensional digital models obtained from Cone Beam Computed Tomography (CBCT). Materials and Methods: This was an ex-vivo study. Thirty dental models of patients were selected and scanning of these models were done using CBCT. Means and standard deviations of the space analysis measurements were calculated by using both plaster and digital models. Statistical analyses were used to investigate the accuracy and repeatability of the methods studied using Statistical Package of Social Sciences (SPSS) 20.0. Paired t-tests for both mean and standard errors were calculated. R esults: The results for all 30 maxillary and mandibular models were similar. There was a statistically significant difference in calculating sum of ten teeth for both maxillary and mandibular teeth between the plaster and dental digital models and in mesio distal measurements between digital and plaster dental models. C onclusion: Digital models provide a more accurate measurement of mesio distal width of tooth with a statistically significant difference of (p-value=0.002).

Prevalence and Distribution of Carabelli Cusp in Maxillary Molars in Deciduous and Permanent Dentition and Its Relation to Tooth Size in a Group of Iranian Adult and Pediatric Dental Patients

Background: Carabelli cusp is a dental morphologic anomaly arising on the palatal side of the mesiopalatal cusp of maxillary first or second molars. It is believed that this cusp is seen in people with larger teeth. Since it has different prevalence among populations, it can be used in forensic dentistry. As well, dentists should be aware of common dental anomalies that might impact dental treatments. In this study, the prevalence of Carabelli cusp and its relation to tooth size in permanent and deciduous dentitions in Iranian population was assessed. Methods: This analytic-descriptive study was performed on 129 (43 deciduous, 86 permanent) patients and their dental casts. First and second molars were observed for the presence of Carabelli cusp. Mesiodistal width of teeth was recorded by using a vernier calliper. The data were analyzed by the Independent t test, Chi-square and Mann Whitney statistical tests using SPSS 18. Results: Frequency of Carabelli trait were respectively 72% and 62% in deciduous and permanent dentitions. No significant difference was seen between males and females in this study in both studied dentitions (P>0.05). Dentitions with Carabelli cusp had larger mesiodistal width compared to other groups in both dentitions (P<0.001). Conclusion: Carabelli cusp in the studied Iranian population was higher compared to other Asian populations. In addition, its bilateral occurrence like a shallow groove in first maxillary molars was related to larger tooth size in both dentitions.

A study to correlate various facial landmarks with intercanine distance.

Ideal interaction of dental and facial beauty proportions highly influences a harmonious facial appearance. Racial anatomical variations have been significantly noted and using these norms during prosthodontic rehabilitation will be greatly helpful in successful treatment outcome. The present study aims to correlate various facial landmarks (interpupillary distance [IPD], intercanthal width, bizygomatic width [BZW], and interalar width) with intercanine distance (ICaD) in Indian young adults to determine the mesiodistal width of the maxillary anterior teeth in edentulous patients. This is an observational cross-sectional study. This study was conducted in 500 individuals ranging in age from 18 to 28 years. IPD, intercanthal width, BZW, and interalar width were measured with a digital caliper (accuracy - 0.01 mm). A "T-" shaped flat metal plate called "canine tip marker" was made to mark the tips of the maxillary canines, which were further measured with the digital caliper. The data were summarized in table form and were statistically analyzed. Student's t-test and ANOVA test were used. A significant correlation (P < 0.001) was found between these landmarks with ICaD, in which interalar width shows highest degree of correlation (r = 0.639). ANOVA test showed that maximum number of individuals had < 5% variability range between actual values and calculated values of ICaD for all extraoral facial landmarks except intercanthal width. Combination of various facial and oral factors should be considered while selecting artificial teeth. Regression equations obtained can be well used during teeth selection to achieve a significant result.

A New Regression Equation Proposed For Mixed Dentition Analysis in Chhattisgarh, Central India Population

Objective: To develop and propose a new regression for mixed dentition analysis in Chhattisgarh, Central India population. Material and Methods: The permanent dentition dental casts of 800 (400 males and 400 females) Chhattisgarh subjects were selected. Digital caliper was used to measure the mesiodistal crown widths of teeth. The linear regression equations and correlation between four mandibular incisors and the canine-premolars segments of maxillary and mandibular arches were developed (modified Tanaka-Johnston equation) and proposed for Chhattisgarh population. Results: New standardized regression equations were formulated to predict the mesiodistal widths of unerupted canines and premolars especially for Chhattisgarh, Central India population. The equation in males for maxillary arch was Y = 11.90 + 0.39 (X) and for mandibular arch was Y = 12.23 + 0.36 (X). Similarly, the equation in females for maxillary arch was Y = 14.40 + 0.26 (X) and for mandibular arch was Y= 10.26 + 0.43 (X). A significant sexual dimorphism in teeth sizes was seen with higher mesio distal dimension in males in Chhattisgarh population. Conclusion: Sum of the mesiodistal diameter of permanent mandibular incisors can be used reliably to predict/estimate the sum of mesiodistal diameters of unerupted canines and premolars with the new regression equations.

THE RELIABILITY OF THE BOLTON RATIO WHEN APPLIED TO THE INDONESIAN POPULATION

Objective: The objective of this study is to determine whether Bolton ratio can be applied clinically to the Indonesian population and to determine a more suitable Bolton ratio for the Indonesian population.Methods: This study was conducted on 120 readily available study models of treated cases comprising 37 males and 83 females. Two investigators separately measured the mesiodistal crown width of maxillary and mandibular tooth on each study model using sliding calipers. According to Bolton’s method, the overall and anterior ratios from each sample were calculated and the mean was generated. Using Student’s t-test with a 95% confidence interval, the investigators compared whether there is a significant difference between the ratio from Bolton’s samples and the ratio from the Indonesian samples.Results: The result of this study showed that, for Indonesian samples, the overall ratio is 89.7±2.05, while the anterior ratio is 76.4±2.76. Student’s t-test showed that there is a statistically significant difference (p&lt;0.05) between the results of this study and that of Bolton’s study for both the anterior and overall ratios.Conclusion: It can be concluded that there is a difference between Indonesian population and Caucasian population in tooth size and Bolton ratio value. Therefore, original Bolton ratio value cannot be used as an accurate diagnostic tool for Indonesian population.

Comparison of Bolton analysis and Little’s irregularity index on laser scanned three-dimensional digital study models with conventional study models

Three-dimensional digital study models were introduced following advances in digital technology. This study was carried out to assess the reliability of digital study models scanned by a laser scanning device newly assembled. The aim of this study was to compare the digital study models and conventional models. Twelve sets of dental impressions were taken from patients with mild-to-moderate crowding. The impressions were taken twice, one with alginate and the other with polyvinylsiloxane. The alginate impressions were made into conventional models, and the polyvinylsiloxane impressions were scanned to produce digital models. The mesiodistal tooth width and Little’s irregularity index (LII) were measured manually with digital calipers on the conventional models and digitally on the digital study models. Bolton analysis was performed on each study models. Each method was carried out twice to check for intra-observer variability. The reproducibility (comparison of the methods) was assessed using independent-sample t-tests. The mesiodistal tooth width between conventional and digital models did not significantly differ (p > 0.05). Independent-sample t-tests did not identify statistically significant differences for Bolton analysis and LII (p = 0.603 for Bolton and p = 0894 for LII). The measurements of the digital study models are as accurate as those of the conventional models.

Correlation between iris diameter and the width of the maxillary central incisor with digital image analysis

Statement of problemVarious methods have been suggested for determining the width of maxillary anterior teeth for complete dentures. A straightforward and reliable technique for anterior tooth selection in complete denture is still needed. PurposeThe purpose of this clinical study was to determine the correlation between the horizontal visible iris diameter (HVID) of the eye and the mesiodistal width of the maxillary central incisor tooth in a dentate Indian population. Material and methodsOne hundred Indian dental students participated in this study. A standardized image of the face revealing the eyes and teeth on smile was captured. The measurements of the HVID and the mesiodistal width of the maxillary central incisor at the cervical third, middle third, and incisal edge were evaluated with photoimaging software. The data were subjected to statistical analysis by paired-sample t test to compare the right and left sides, independent-sample t tests to compare between sexes, and the Pearson correlation coefficient (r) to investigate the association between variables (α=.05) ResultsA positive correlation between the HVID and the mesiodistal width of the maxillary central incisor was found in men, with r values of .62, .78, and .79 in the cervical, middle, and incisal third regions, respectively. In women, respective r values of .59, .73, and .75 were found. Minimal differences were observed between right- and left-side measurements. ConclusionsThe HVID of the eyes can be used to estimate the mesiodistal width of the maxillary central incisor tooth.

An In Vivo Study to Compare and Evaluate the Correlation of the Facial Measurements with the Combined Mesiodistal Width of the Maxillary Anterior Teeth between Males and Females.

Aim:The aim of this study was to compare and evaluate the correlation of the facial measurements (interalar, intercommissural, intercanthal, and interpupillary distance) with the combined mesiodistal width of the maxillary anterior teeth between males and females.Objectives:The objectives of this study were: (a) To evaluate the correlation of the facial measurements (interalar, intercommissural, intercanthal, and interpupillary distance) and combined mesiodistal width of maxillary anterior teeth. (b) To compare the correlation of the facial measurements (interalar, intercommissural, intercanthal, and interpupillary distance) with the combined mesiodistal width of maxillary anterior teeth between males and females.Materials and Methods:This study was carried out on 120 dentate Subjects selected from Yenepoya Dental College, Mangalore. The vernier caliper was used to measure the interalar, intercommissural, intercanthal, and interpupillary distance. The mesiodistal width of maxillary anterior teeth was measured on the cast. For accuracy, every distance was measured three times and the mean taken.Results:Comparison of parameters between males and females was calculated using independent sample t-test. The values obtained for each variables are interalar distance: for males 38.89 ± 2.19 and for females 35.58 ± 2.33. Intercommissural distance: for males 50.96 ± 2.99 and for females 47.15 ± 2.97. Intercanthal distance: for males 31.92 ± 1.46 and for females 30.46 ± 2.07. Interpupillary distance: for males 65.15 ± 2.82 and for females 61.78 ± 3.18. Mesiodistal width of maxillary anterior teeth: for males 52.55 ± 1.82 and for females 51.33 ± 2.50.Conclusion:Within the limitations of this study, the study concluded there is a significant correlation between interalar, intercommissural, intercanthal, interpupillary distance, and mesiodistal width of maxillary anterior teeth in total subjects. In females, there is a significant correlation between interalar, intercanthal, interpupillary distance, and mesiodistal width of maxillary anterior teeth.

Relationship and correlation between lower anterior crowding and collective mesiodistal width of mandibular central incisors-An In vitro study

Objective: to determine whether the mesiodistal width of the mandibular incisors is factor contributing to lower anterior crowding. Materials and methods: Mandibular casts of 100 patients from the Orthodontics department , Saveetha dental college are to be taken and the mean collective mesiodistal width of the mandibular central incisors are calculated and compared with the littles irregular index value for that cast. Results: No correlation was found between the collective mesiodistal width and crowding of lower anterior teeth. Conclusion: The mesiodistal width of lower anterior teeth does not contribute to crowding.

Odontometric and Skull Anthropometric Parameters as a Forensic Tool in Stature Estimation: A Cross-sectional Analysis

ABSTRACTBackgroundThe aim of the study was to evaluate the relation between height of an individual with the circumference of skull and combined mesiodistal width (CMDW) of the permanent maxillary anterior teeth in forensic and medicolegal investigations.Materials and methodsIn this study, 100 persons were evaluated. Mesiodistal widths of the permanent maxillary anterior teeth and height and circumference of the skull were measured. Linear regression analysis was used for the known heights of the combined data and data for males and females against the odontometric and anthropometric variables.ResultsHighly significant correlation was observed between height and other parameters for combined data and data for males. There was statistically significant difference between equation relating height to the CMDW of the maxillary anterior teeth and equation relating height to head circumference.ConclusionIt can be stated that the skull with teeth may provide accurate clues to stature from an individual's fragmentary remains and thus provide a reliable method of estimation of height from skeletal remains during forensic investigations.Clinical significanceOdontometric parameters along with regression equations may be used as a supplementary approach for stature estimation, especially in situations of unavailability of extremities. Tooth width measurement using the Lythos digital scanner

Abstract Introduction Digital models have become more widely accepted for orthodontic diagnostic purposes. Intraoral scanners have the advantage of eliminating the need for conventional impressions. The aim of the present study was to assess the reliability and reproducibility of the Lythos intraoral scanner and to determine if a significant advantage is delivered over stone model and caliper measurements in tooth width and Bolton ratio accuracy. Methods The study comprised 30 typodont models for which conventional alginate impressions and digital scans were obtained to generate stone and digital models, respectively. Mesiodistal tooth width measurements and Bolton ratios were obtained with either calipers and stone models or with Digicast (Ormco Lythos digital model software, Ormco, CA, USA) software using digital models. Pearson’s correlation coefficients tested intra-examiner reliability. Interclass correlation coefficients were used to assess agreement between examiners (reproducibility). The differences in the mean tooth width measurements and Bolton ratios from the typodont values and either the digital or conventional method were used to assess validity by applying two tailed t-tests. Results The measurements obtained from the Lythos and stone models had near perfect intra-examiner agreement (Pearson ≥ 0.98). The inter-examiner reproducibility for tooth widths, anterior Bolton and overall Bolton ratio was high and similar for both methods (Lythos scanner Interclass correlation coefficient (ICC) above 0.89, stone models ICC above 0.92). Stone model measurements were statistically twice as accurate as those derived from the digital system (0.032 mm versus 0.074 mm). There were no significant differences in accuracy between the methods for Bolton calculations. Clinically, there was no difference between the methods for tooth width measurements and Bolton calculations. Conclusions The Lythos system is as reliable and reproducible as conventional calipers and stone models in tooth width measurements and Bolton calculations. The caliper method presents a statistically more valid tooth width measurement technique but the clinical significance of this is questionable.

Bolton tooth size ratio among qatari population sample: An odontometric study.

Objectives:To establish the overall and anterior Bolton ratio among a sample of Qatari population and to investigate whether there is a difference between males and females, as well as to compare the result obtained by Bolton.Materials and Methods:The current study consisted of 100 orthodontic study participants (50 males and 50 females) with different malocclusions and age ranging between 15 and 20 years. An electronic digital caliper was used to measure the mesiodistal tooth width of all maxillary and mandibular permanent teeth except second and third molars. The Student's t-test was used to compare tooth-size ratios between males and females and between the results of the present study and Bolton's result.Results:The anterior and overall ratio in Qatari individuals were 78.6 ± 3.4 and 91.8 ± 3.1, respectively. The tooth size ratios were slightly greater in males than that in females, however, the differences were not statistically significant (P > 0.05). There were no significant differences in the overall ratio between Qatari individuals and Bolton's results (P > 0.05), whereas statistical significant differences were observed in anterior ratio (P = 0.007).Conclusions:Within the limitation of the limitations of the present study, definite conclusion was difficult to establish. Thus, a further study with a large sample in each malocclusion group is required.

Bolton discrepancy among patients with anterior open bite malocclusion

IntroductionSeveral investigators studied Bolton's discrepancy in relation to different types of malocclusions, but no studies were found investigating Bolton's discrepancy in open bite cases. The aim of the present study was mainly to determine tooth size discrepancy in a sample of orthodontic patients with open bite malocclusion and to test any correlation between open bite and anterior, posterior, and overall ratios. MethodEighty-four study models of patients with open bite (42 females and 42 males) and 33 study models of ideal occlusion as control (18 females and 15 males) were selected. The mesiodistal width of upper and lower teeth, excluding second and third molars, was measured using a digital calliper, and the anterior, posterior, and overall ratios were calculated. ResultsThe anterior, posterior, and overall ratios for the open bite Saudi sample was 77.14 (SD 3.27), 104.5 (SD 4.80), and 90.46 (SD 3.90), respectively. The Student's t test showed no significant difference between the open bite and control groups in all ratios. The Pearson correlation coefficient showed no significant correlations between open bite and anterior ratio, total ratio, and posterior ratio. ConclusionEven though the anterior, posterior, and overall ratios are not significantly different from control or Bolton's ratios, open bite cases require a close evaluation of tooth size discrepancy in all anterior, posterior, and total ratios to achieve ideal occlusion.

Effect of premolar extraction on Bolton overall ratio in north Indian population

Introduction: Bolton ratio is considered as seventh key of normal occlusion. Many orthodontic patients have to undergo premolar extractions as a part of treatment. Aims and objectives: The present study was taken up to investigate the effect of premolar extractions on Bolton’s overall ratios in a sample of north Indian population. Materials and methods: Mesiodistal tooth widths were measured on pretreatment dental casts of 200 subjects (100 males and 100 females). The Bolton’s overall ratios were determined before and after hypothetical premolar extractions and the subjects were divided into small, normal, and large overall ratio groups categorized by the Bolton standard deviation definition. Extractions were performed in the following combinations: (1) all first premolars, (2) all second premolars, (3) maxillary first and mandibular second premolars, and (4) maxillary second and mandibular first premolars. Results: The overall ratios decreased in every group after extraction of any combination of premolars. Maximum discrepancy was observed in the extraction of all first premolars and least in second premolars. Statistically significant differences were seen in the extraction pattern amongst males and females. Conclusions: In cases which require premolar extractions, it is mandatory to check if any discrepancy exists between different extraction patterns since the ratios may shift from normal to abnormal or vice versa as it may affect the interdigitation of teeth.
