Homocyst(e)ine and arterial occlusive diseases.

Homocysteine is a thiol-containing amino acid resulting from demethylation of methionine. The free and protein-bound forms of the amino acid and derived disulfides are called homocyst(e)ine [H(e)]. Multiple studies have shown elevated H(e) levels in patients with coronary, cerebrovascular, or peripheral arterial diseases; this association is frequent and independent of most other risk factors for atherosclerosis. In the 1993 Frontiers in Medicine Symposium investigators discussed the genetic, physiological, nutritional, and pharmacological mechanisms involved in the regulation of plasma H(e), the association of H(e) with arterial occlusive diseases, and the relationships of H(e) with nitric oxide and haemostasis. High plasma H(e) levels usually can be reversed with vitamin supplements. Whether vitamin supplements will affect the evolution of arterial occlusive diseases needs to be established in prospective, placebo-controlled, randomized, clinical trials.

Antigen Selection. Dr. Chulay: I would like to start by asking Dr. Peter Reeve to comment on the proposed antigen selection strategy that Dr. Russell Howard described. Dr. Reeve: For several reasons, I regard the selection of antigens for development into experimental vaccines for human trials to be the most important step in vaccine development. First, it implies commitment of significant resources, including funds. In industry this decision is reviewed at the very highest levels. It also makes a very visible statement of intent: expectations are therefore likely to be high.

Society of General Internal Medicine Symposium: Community Responsive Medicine: Defining an Academic Discipline

Academic medicine is entering a period of introspection created by changing patterns of health and disease and changing patterns in reimbursement and health policy. To respond to existing health challenges, innovative strategies for health care delivery and education must be devised. The migration of medical education and health care delivery into centralized and frequently difficult-to-access campuses is being reviewed by payors, policy analysts, and purchasers of care. One proposed solution to this problem responds to the identified shortcomings in medical education and some of the failures of our health care delivery system. Community Oriented Primary Care (COPC) and the related discipline of Community Responsive Medicine define health-oriented strategies that blend traditional primary care and public health. The classroom in Community Responsive Medicine is the community. The curriculum is defined by the elements and skills necessary to improve the health status of the target community. The model allows the student and resident to appreciate and experience the entire spectrum of health and disease and to understand the sequence of events that may lead to illness. Tertiary care is a critical curriculum component, but in Community Responsive Medicine the curriculum is balanced between the ambulatory and tertiary care settings. Community Responsive Medicine defines a role for new leadership that can create responsive educational and health care delivery systems accountable to the communities they serve.

Society of General Internal Medicine Symposium Principles of Intercultural Medicine in an Internal Medicine Program

Internal medicine and medicine-pediatric residents (n = 76) completed a questionnaire that measured variables including sociodemographics, family dynamics, cross-cultural exposure, and exposure to intercultural medicine principles. Questions were answered regarding perceptions of their patients and level of comfort discussing specific cultural variables. Gender, training status, and geographic background did not influence responses, but the responses of European-Americans (71%) vs. ethnic minorities and foreign medical graduates (29%) were significantly different. European-Americans were more likely to be men, less likely to have an urban background (p = .02), and their self-described socioeconomic status was uppermiddle to upper class (p = .02). European-Americans vs. all others differed in their perceptions of patients' financial support (p = .001), and reasons for doctor-patient miscommunications (p = .05). The European-Americans had significantly less exposure to friends and classmates (p = .002), and instructors (p = .0001) of ethnic origins different than their own prior to residency training. Our data support the inclusion of intercultural medicine principles in the general internal medicine curriculum.

Society of General Internal Medicine Symposium Integrating Substance Abuse Education in the Medical Student Curriculum

Physicians miss the diagnosis of substance abuse in significant numbers of patients, partly because of a lack of education about identifying and treating those patients. This article describes an attempt to integrate substance abuse into the curriculum of a traditionally organized medical school. Faculty selection, determination of the skills and knowledge needed, and methods for enriching the curriculum to include substance abuse are described. Problems encountered during the project, benefits of implementing the curriculum changes, and recommendations for other medical schools choosing to implement such a program also are provided.

Society of General Internal Medicine Symposium The Duke Minority Medical Student Summer Fellowship: One Program's Attempt to Attract Minority Residents

Despite numerous Federal and private funding programs aimed at increasing the numbers of underrepresented minorities in medicine, the participation of minority groups in the medical profession remains low and shows little sign of improvement. With the goal of improving minority representation in our primary care residency program, the Minority Medical Student Summer Fellowship started in July 1990. The program's purpose is to give minority medical students positive exposure to primary care and attract them back for residency training. In this four-week elective, students participate in seminars with our primary care residents, see patients in a variety of clinics, and complete a project. Key implementation issues include support of the medicine chairman and dean, funding, and malpractice coverage for the students. Program evaluation from the first two students rated the clinic experiences higher than the seminar sessions and the independent project. Both students stated our fellowship made them more likely to train at this medical center should they enter a primary care program. Implications for other programs are listed.

Society of General Internal Medicine Symposium Teaching Occupational and Environmental Medicine in Primary Care Residency Training Programs: Experience Using Three Approaches During 1984–1991

There is a critical shortage of physicians trained to recognize and treat occupational and environmental health problems. We implemented several required teaching programs for internal medicine and family medicine residents that focus on providing primary care for these problems. Clinical experiences were developed using the university and medical center as an example of a workplace with chemical and physical hazards. On-site experiences were also provided at local industries, but when resident stipend support for this aspect was discontinued, that part of the program was suspended. Didactic programs were associated with a statistically significant improvement in house staff knowledge scores. These occupational and environmental health issues can be introduced during residency, resulting in increased expertise in this discipline.

This monograph is the proceedings of an American Association of Physicists in Medicine (AAPM) symposium in co-operation with the Bowman Gray School of Medicine held in April 1988, with contributions from a number of academic and commercial magnetic resonance imaging (MRI) scientists and engineers. Image quality in MRI is dependent upon a highly complex inter-relationship between a number of subsystems each requiring optimal adjustment. The book addresses the criteria for objective evaluation of image quality and system performance and is aimed at the clinical physicist with the responsibility for making such measurements. The first chapter provides a brief overview of the influence of particular MRI system components on image quality, whilst the next four chapters describe the main image parameters required for adequate monitoring of the sensitivity and geometric characteristics of a clinical MR imaging system.

Bioconjugate Chemistry Attracts Growing Interest

Bioconjugate chemistry, a field of research involving the covalent attachment of small molecules to biopolymers, an area of growing multidisciplinary interest among chemists, biochemists, and molecular biologists. Recent advances in bioconjugate chemistry and the advent of a new American Chemical Society Bioconjugate Chemistry , scheduled for startup next January, were dual focuses of a Division of Medicinal Chemistry symposium at the ACS national meeting last month in Dallas. symposium addressed bioconjugate chemistry applications in tumor therapeutics, DNA probes, diagnostic reagents, synthetic vaccines, and other areas. The central theme of the new journal, says editor Claude F. Meares, professor of chemistry at the University of California, Davis, is the joining of two different molecular functions by either cal or biological means, principally chemical. Anterolateral rotatory instability of the knee

In an attempt to correct anterolateral rotatory instability of the knee, 30 consecutive patients underwent a surgical procedure similar to that described by Ellison (Ellison AE: A modified procedure for the extra-articular replacement of the anterior cruciate ligament. American Orthopaedic Society for Sports Medicine Symposium, New Orleans, LA, July 15, 1975), called the iliotibial band transfer. Fashioning the passageway for the transfer closer to the attachment of the fibular collateral ligament on the femur and meticulous fascial closure over the transplant were not found to be crucial to success. Twenty-nine of the 30 patients were injured during athletic activities, and the remaining patient was injured in a motorcycle accident. They often complained of an unstable knee with symptoms of pain and giving way. Twenty-eight patients with a minimum followup of nine months (the average followup was 25 months) were evaluated subjectively by an interview and by objective clinical examination. Subjective results (including asymptomatic return to their previous level of athletic activity) and clinical improvement of anterolateral rotatory instability (based on the flexion-extension-valgus test or the Slocum anterolateral rotatory instability test) were encouraging. Areas of concern were that a small number of patients developed asymptomatic varus instability and a few had relative strength deficits which may or may not have long-term significance.
