Research Progress on the Basic Pharmacodynamic Substance and Mechanism of <i>Citri Sarcodactylis Fructus</i> in Relieving Anxiety

Anxiety disorder resulting from the interaction of psychological factors-caused anxiety and mood disorder symptoms is one of the clinical mental diseases. Excessive worry, nervousness and fear often bring about several negative outcomes, and seriously impact the physical, mental, and social functioning of patients. What’s worse, with poor treatment, anxiety disorder may develop into mental illnesses such as depression and schizophrenia. The treatment of anxiety has mainly recourse to drug therapy and psychotherapy; however, Western drug therapy represented by 5-hydroxytryptamine reuptake inhibitor often causes evident adverse effects, including nausea and constipation, as well as poor long-term medication compliance of patients. Citri Sarcodactylis Fructus can sooth the liver and regulate qi, which can be applied to alleviate anxiety disorder as an ancillary drug. Modern pharmacological research revealed that bergamot volatile oil, also known as bergamot essential oil (BEO), is the main active chemical component of Citri Sarcodactylis Fructus for anti-anxiety. BEO has been confirmed to exert an anti-anxiety effect through modulating neuronendocrine-immune network, including immunoregulation, suppression of hyperactive hypothalamic-pituitary-adrenal (HPA) axis, enhancement of function of hypothalamic-pituitary-thyroid (HPT) axis, modulation of neurotransmitter, and promotion of neurotrophic effect. The study discusses the current research status regarding the application of Citri Sarcodactylis Fructus for anti-anxiety in recent years, and deeply probes into the mechanism of Citri Sarcodactylis Fructus playing an anti-anxiety role via neuronendocrine-immune network, in an effort to provide references to clinical treatment of anxiety disorder and new ideas for developing drugs targeting anxiety disorders.

Medication compliance and related factors among readmission people with type 2 diabetes at 108 Military central Hospital

Objectives: Survey on medication compliance rate and related factors in people with type 2 diabetes readmitted for inpatient treatment at 108 Central Military Hospital. Participants and methods: A cross-sectional descriptive study was conducted on 168 patients with Type 2 diabetes readmission from January 2021 to December 2022. The study assessed medication adherence using the MARS–5 questionnaire (Medication Adherence Report Scale 5). Results: The rate of medication compliance in patients was 21.8%, non-compliance accounted for 78.2%. Patients who occasionally forget to take medication account for the highest rate of 51.8%, arbitrarily reducing the dose to less than the prescribed dose accounted for 32.1%, rarely skipping doses, and stopping medicine for a period of time accounted for 35.7% and 50.2%, respectively. There was a relationship between age and current medication and medication compliance. The difference was statistically significant(p < 0.05). Conclusion: The medication compliance rate in diabetic patients readmitted to the hospital in the study was low, only 21.8%, of which patients forgot to take medication and arbitrarily reduced the dose and stopped using medication for a period of time accounting for the highest proportion. high rate. Strengthening health education on medication adherence during follow-up examinations for patients and caregivers is extremely necessary to reduce the risk of hospital readmission, maintain blood sugar at normal levels, and minimize diabetes complications.

The Relationship of Family Support With Medication Compliance in Acute Psychotic Patients

The aim of this study was to determine the relationship between family support and medication adherence in acutely psychotic patients. This research is a quantitative research. The results showed that the lower the emotional support from the family, the lower the adherence to taking medication in acute psychotic patients. The lower the family support in the esteem support aspect, the lower the medication adherence in acute psychotic patients. The lower the family support in the instrumental support aspect, the lower the medication adherence in acute psychotic patients. The lower the family support in the informational support aspect, the lower the medication adherence in acute psychotic patients. Of the four aspects of family support, the emotional support aspect has the highest correlation with adherence to taking medication in acute psychotic patients while the one with the lowest correlation is the instrumental support aspect in acute psychotic patients at the Bandung Mental Hospital.

Description of Factors that Influence Medication Compliance in Hypertension Patients at the Separi 3 Kutai Kartanegara Health Center

Hypertension is a lifelong disease that cannot be permanently cured but can only be controlled by controlling blood pressure by managing a healthy lifestyle and taking recommended antihypertensive drugs. Purpose: this study is to describe the factors of knowledge, family support, and access to health facilities that influence medication adherence in hypertensive patients at the Separi Health Center. Methods: this study used a descriptive research design with a cross sectional approach. The sampling technique used is probability sampling technique with proportional stratified random sampling method with a total sample of 84 respondents. Results: the study showed that 82.1% of patients did not adhere to taking medication, and 17.9% of patients adhered to taking medication. Patients with less knowledge 31.0% and patients with good knowledge 69.0%. Patients with poor family support 56.0% and patients with good family support 44.0%. Patients with poor access to health services 46.4% and patients with good access to health services 53.6%. Conclusion: the results of the study show that despite having good knowledge, family support, and access to health services, most patients do not adhere to taking medication. Suggestions for existing health services are expected to improve service quality and provide information and education to people with hypertension.

Remedial Dosing Recommendations for Sirolimus Delayed or Missed Dosages Caused by Poor Medication Compliance in Pediatric Tuberous Sclerosis Complex Patients.

Delayed or missed dosages caused by poor medication compliance significantly affected the treatment of diseases in children. The present study aimed to investigate the influence of delayed or missed dosages on sirolimus pharmacokinetics (PK) in pediatric tuberous sclerosis complex (TSC) patients and to recommend remedial dosages for nonadherent patients. A published sirolimus population PK model in pediatric TSC patients was used to assess the influence of different nonadherence scenarios and recommend optimally remedial dosages based on Monte Carlo simulation. Thirteen nonadherent scenarios were simulated in this study, including delayed 2h, 4 h, 6 h, 8 h, 10 h, 12 h, 14 h, 16 h, 18 h, 20 h, 22 h, 23.5 h, and missed one dosage. Remedial dosing strategies contained 10-200% of scheduled dosages. The optimal remedial dosage was that with the maximum probability of returning the individual therapeutic range. For delayed or missed sirolimus dosages in pediatric TSC patients, when the delayed time was 0-8 h, 8-10 h, 10-18 h, 18-22.7 h, 22.7-24 h, 70%, 60%, 40%, 30%, 20% scheduled dosages were recommended to take immediately. When one dosage was missed, 120% of scheduled dosages were recommended at the next dose. It was the first time to recommend remedial dosages for delayed or missed sirolimus therapy caused by poor medication compliance in pediatric TSC patients based on Monte Carlo simulation. Meanwhile, the present study provided a potential solution for delayed or missed dosages in clinical practice.

Relationship between Knowledge Level about Drug-Resistant TB (TB-RO) and Medication Compliance in Pulmonary TB Patients

Introduction: Drug-resistant tuberculosis (TB RO) is still a threat in TB control and is one of the main public health problems in many countries in the world. Globally in 2019, it is estimated that 3.3% of new TB patients and 17.7% of previously treated TB patients will be drug-resistant TB patients.
 Objective: To determine the relationship between the level of knowledge about Drug-Resistant TB (TB-RO) and the level of medication adherence in pulmonary TB patients.
 Method: This type of research is quantitative research with analytical observational methods using a cross sectional approach. The population in this study was TB sufferers who were undergoing treatment at the Limboto Community Health Center, totaling 70 respondents. The research technique used accidental sampling to obtain a sample of 35 respondents. Data analysis used the Fishers exact test with the knowledge level questionnaire instrument and MMS-8.
 Results: The results showed that 29 respondents had good knowledge and were compliant with taking medication, 4 respondents had poor knowledge and were compliant with taking medication, and 2 respondents had poor knowledge and were not compliant with taking medication. The statistical test results showed that p-Value = 0.025, which means less than (α = 0.05), indicating that there is a relationship between the level of knowledge about TB-RO and adherence to taking medication.
 Conclusion: The patient's lack of knowledge about the disease and the use of drugs for therapy results in the patient's lack of understanding of the therapy being undertaken, which causes patient non-compliance in using the medication.

Ecological spectroscopic methodologies for quantifying co-administered drugs in human plasma by photochemical quantum mechanical simulation

Urinary tract infections (UTIs) constitute the second most prevalent bacterial infections in the elderly demographic. The treatment landscape involves various antibiotics targeting the causative organisms; nevertheless, the emergence of resistance significantly impacts therapeutic effectiveness. Presently, a fixed-dose pharmaceutical combination is advocated to optimize patient outcomes by mitigating the risks of bacterial resistance and associated side effects. Ofloxacin (OFL) and cefpodoxime proxetil (CPD) combinations, co-administered with flavoxate hydrochloride (FLV), have demonstrated efficacy in UTI cases, offering relief from concomitant symptoms. In the pharmaceutical market, fixed-dose combinations have gained prominence, driven by advantages such as enhanced patient medication adherence and compliance. In the realm of analytical chemistry, the integration of green practices in the initial phases of method development is exemplified by the Greenness by Design (GbD) strategy. While univariate spectroscopic methods are conventionally considered suboptimal compared to chemometric techniques for resolving intricate mixtures, GbD approach, when applied to UV spectroscopy, enable univariate methods to attain comparable or superior outcomes. GbD adopts a systematic approach to optimize experimental conditions, minimizing environmental impact and maximizing analytical performance. Critical to GbD applications in UV spectroscopy is solvent selection, influencing spectral resolution and measurement sensitivity. GbD employs a combination of in-vitro and in-silico experiments to evaluate solute-solvent interactions with underlying photochemical quantum phenomena affecting the resulting spectral morphology, identifying an optimal compromise solvent with high resolution and minimal ecological impact. Consequently, it facilitates the efficient resolution of spectral overlapping and determination of complex mixtures in UV spectroscopy using univariate methods. Comparative analysis with chemometric techniques, acknowledged as potent spectral resolving methods, demonstrated that GbD-based univariate methods performed equivalently. The methodology was validated according to ICH recommendations, establishing a linear quantitation range (2–30 μg/mL) and a limit of detection (0.355–0.414 μg/mL) for the three drugs in human plasma. The greenness of the developed methodology was affirmed through the AGREE assessment protocol, confirming its environmentally conscious attributes.

Home Care Nurses' Knowledge and Attitudes Regarding Telehealth to Promote Medication Compliance among Older Adults.

Medication noncompliance among older adults is a persistent problem resulting in morbidity, hospital readmissions, and decreased quality of life. Home care nurses are in a key position to assist older adults to employ medication compliance strategies that are tailor-made to fit their individual needs and abilities. The purpose of this study was to understand the knowledge and attitudes of home care nurses regarding telehealth practices to promote medication compliance of older adult patients and their perceived readiness to do so. We employed a qualitative research approach using semi-structured interviews. Individual interviews were conducted via Teams platform for a maximum of 60 minutes. Participants were home care nurses who use telehealth technology to advance medication compliance strategies. Ten interviews were conducted. Several themes emerged from the data which suggested home care nurses accept telehealth technology, are supportive of its continued use, and are willing to be educated on future advances in this technology to assist older adults with medication compliance strategies. Universities and home care agencies must provide opportunities to prepare nurses to utilize telehealth technologies to promote medication compliance among older adult home care patients.

Association Between Medication Compliance and Various Risky Behaviors in Patients With Schizophrenia

To investigate the status of medication adherence and various types of risky behaviors of schizophrenia patients in a certain area of western China and to explore accordingly the correlation between the two. A total of 292 667 patients with schizophrenia were enrolled in a follow-up survey between 2006 and 2018. In addition, based on the outcome-wide analysis strategy, a multivariate Cox proportional risk regression model was used to estimate and compare the impact of medication adherence on different types of risky behaviors in schizophrenia patients. In this 13-year prospective cohort, 65 175 patients (31.4%) showed good medication adherence, while 142 394 patients (68.6%) showed poor medication adherence. The incidence rates of various risky behaviors during the follow-up period were as follows, minor nuisances, 12.25%, violation of the Law of the People's Republic of China on Penalties for Administration of Public Security (APS law), 3.82%, violation of criminal law, 0.94%, suicide completed, 0.28%, self-harm, 1.42%, and attempted suicide, 0.82%. Schizophrenia patients who had poor medication adherence had higher risks of committing violence against others and self-inflicted injury compared to patients with good medication adherence did, with the associated effects being minor nuisances (hazard ratio [HR]=1.31, 95% confidence interval [CI]: 1.27-1.35), violation of APS law (HR=1.47, 95% CI: 1.38-1.56), violation of criminal law (HR=1.17, 95% CI: 1.05-1.31), and self-harm (HR=1.43, 95% CI: 1.32-1.56), respectively, while the risk of suicide completed is lower in schizophrenia patients with poor medication adherence than that in patients with good medication adherence (HR=0.56, 95% CI: 0.47-0.66). There was no statistically significant association between attempted suicide and medication adherence. There are variations in the direction and strength of the association between medication adherence and different types of risky behaviors and further research is needed to elucidate the mechanisms of the association.

The Role of The Pharmacist in The Success of Tuberculosis (Tb) Treatment in Jeneponto Regency

Tuberculosis (TB) is an infectious disease caused by the bacterium called Mycobacterium tuberculosis leading to the inflammation of the lung parenchyma. This study aims to determine the effect of counseling on drugs and disease on TB patients and Medication Supervisors (PMO) as well as to determine the effect of PMO in increasing TB patient compliance with monitoring of each respondent. This study used two instruments, the first instrument was the TB-P10 instrument to measure the knowledge of respondents and PMO with pre and post-monitoring. The second instrument was MARS-5, given to the respondent and PMO to see how the respondent's compliance after therapy. Based on the results of the analysis of the paired t-test, it is known that the significance so it can be concluded that there is an average difference between the pre-test and post-test knowledge levels of PMO, which means that there is an effect of pharmacist counseling interventions to respondents and PMO in improving their knowledge. Meanwhile, the performance of PMO in increasing patient medication compliance showed a good performance with 48 patients (96%) who adhered to taking medication, while the poor performance was 2 patients who did not adhere to taking medication (4%). Based on the results of the analysis of the Wilcoxon test, it is known that the significanc, so it can be concluded that there is a relationship between PMO performance on patient medication compliance with clinical outcomes in the presence of patient weight gain and negative final of AFB results.

KNOWLEDGE INFLUENCE IN THE COMPLIANCE RATE OF TAKING DRUGS TUBERCULOSIS

Compliance with medication for tuberculosis patients is one of the determinants of the success of therapy in addition to the effectiveness of antibiotics. The aim of this study is to identify the factors that influence the success of a therapy, in the long-term use of antibiotics for tuberculosis. We use questionnaires that we have created and validated in interviews and can be used to directly measure the level of compliance with the therapy experienced by the patient. This research method is an observational study with cross-sectional design and retrospective data collection. Respondents were patients with tuberculosis, both new and recurrent. The study sample consisted of 30 respondents with sequential sampling selection and the basis for patient interviews. The analysis used is a Chi-square binary between a variable in the history of tuberculosis with a long diagnosis during therapy that shows a relationship between education and family involvement in helping treatment especially in male patients with a P value <0,05. Multivariate analysis showed that men who never completed education even though they were still in a productive age had a low level of compliance. The results of the interviews showed that patients who obeyed the drug only reached 30% with scores reaching a score of 8. This is due to the large amount of medication taken, so the risk of success of therapy is very large. This research shows that the level of education, information, and family environment can support patient medication compliance. In addition, the more the amount of medication taken, the lower the patient's compliance.

Evidence from the first Shared Medical Appointments (SMAs) randomised controlled trial in India: SMAs increase the satisfaction, knowledge, and medication compliance of patients with glaucoma.

In Shared Medical Appointments (SMAs), patients with similar conditions meet the physician together and each receives one-on-one attention. SMAs can improve outcomes and physician productivity. Yet privacy concerns have stymied adoption. In physician-deprived nations, patients' utility from improved access may outweigh their disutility from loss of privacy. Ours is to our knowledge the first SMA trial for any disease, in India, where doctors are scarce. In a 1,000-patient, single-site, randomized controlled trial at Aravind Eye Hospital, Pondicherry, we compared SMAs and one-on-one appointments, over four successive visits, for patients with glaucoma. We examined patients' satisfaction, knowledge, intention-to-follow-up, follow-up rates, and medication compliance rates (primary outcomes) using intention-to-treat analysis. Of 1,034 patients invited between July 12, 2016 -July 19, 2018, 1,000 (96.7%) consented to participate, and were randomly assigned to either SMAs (NSMA = 500) or one-on-one appointments (N1-1 = 500). Patients who received SMAs showed higher satisfaction (MeanSMA = 4.955 (SD 0.241), Mean1-1 = 4.920 (SD 0.326); difference in means 0.035; 95% CI, 0.017-0.054, p = 0.0002) and knowledge (MeanSMA = 3.416 (SD 1.340), Mean1-1 = 3.267 (SD 1.492); difference in means 0.149; 95% CI, 0.057-0.241, p = 0.002) than patients who received one-on-one appointments. Across conditions, there was no difference in patients' intention-to-follow-up (MeanSMA = 4.989 (SD 0.118), Mean1-1 = 4.986 (SD 0.149); difference in means 0.003; 95% CI, -0.006-0.012, p = 0.481) and actual follow-up rates (MeanSMA = 87.5% (SD 0.372), Mean1-1 = 88.7% (SD 0.338); difference in means -0.012; 95% CI, -0.039-0.015, p = 0.377). Patients who received SMAs exhibited higher medication compliance rates (MeanSMA = 97.0% (SD 0.180), Mean1-1 = 94.9% (SD 0.238); difference in means 0.020; 95% CI, 0.004-0.036, p = 0.013). SMAs improved satisfaction, learning, and medication compliance, without compromising follow-up rates or measured clinical outcomes. Peer interruptions were negatively correlated with patient satisfaction in early-trial SMAs and positively correlated with patient satisfaction in later-trial SMAs. Trial registration: The trial was registered with Clinical Trials Registry of India (https://ctri.nic.in/) with reference no. REF/2016/11/012659 and registration no. CTRI/2018/02/011998.

Comparative Different Interventions to Improve Medication Adherence in Patients with Hypertension: A Network Meta-analysis.

Hypertension has become a major public problem. One of every 4 adults has hypertension. Medications are critical in controlling blood pressure, but patient medication adherence is low. Therefore, it is very crucial to promote medication adherence. However, the complexity and variety of interventions cause clinical decision-making difficulties for health managers and patients. The aim of this study was to compare the effectiveness of different interventions to improve medication adherence in patients with hypertension. We searched PubMed, Cochrane Library, Web of Science, EMBASE, Wan Fang, China National Knowledge Infrastructure, China Science and Technology Journal Database, and China Biology Medicine disc databases for eligible studies. Medication adherence rate and medication adherence difference were assessed as outcomes. Sensitivity analysis and inconsistency detection were performed to evaluate whether the exclusion of high-risk studies affected the validity. The risk of bias was assessed using the risk of bias table in Review Manager 5.4. The surface under the cumulative ranking curve was used to estimate the rankings among different interventions. Twenty-seven randomized controlled trials were included, and the interventions involved were categorized into 8 different categories. The network meta-analysis showed that the health intervention was the best to promote medication compliance in patients with hypertension. Health intervention is recommended to improve medication adherence in patients with hypertension. Health managers are recommended to provide health intervention to patients with hypertension to improve their medication adherence. This approach reduces morbidity, mortality, and healthcare costs for patients with cardiovascular disease.

Study on the effect of internet plus continuous nursing on functional recovery and medication compliance of patients with knee joint replacement

ObjectiveTo evaluate the effect of "Internet + " continuity of care on postoperative functional recovery and medication compliance in patients with knee arthroplasty.MethodsIn this retrospective study, 100 patients who underwent knee replacement in our hospital between January 2021 and December 2022 were recruited and assigned to receive routine care (routine group) or "Internet + " continuity of care (continuity group), with 50 patients in each group. Outcome measures included knee function, sleep quality, emotional state, medication compliance, and self-care ability.ResultsPatients in the continuity group showed better knee function after discharge and during follow-up versus those in the routine group (P < 0.05). Continuity care resulted in significantly lower Pittsburgh Sleep Quality Index (PSQI), Self-Rating Anxiety Scale (SAS), and Self-Rating Depression Scale (SDS) scores versus routine care (P < 0.05). Patients in the continuity group showed higher treatment compliance, ability of daily living (ADL) scores, and nursing satisfaction than those in the routine group (P < 0.05).ConclusionThe "Internet + " continuity of care is highly feasible and can effectively promote the postoperative functional recovery of knee replacement patients, improve patients' medication compliance, sleep quality, and self-care ability, mitigate negative emotions, and provide enhanced home care.

Medication Compliance of Patients Attending a Pain Clinic at a Tertiary Hospital in Malaysia

Pain is a public health problem with profound physical, emotional, and societal costs. Conventional oral analgaesics are usually the first treatment, which is cost-effective and relatively safe. However, medication noncompliance is a serious healthcare concern. Medication noncompliance has remained a significant challenge despite considerable efforts to improve patient compliance. Therefore, a study was done to assess medication compliance at a Pain Clinic in a tertiary hospital in Malaysia. The study period was from December 2019 to January 2020. A total of 180 patients participated in the study. The study showed that pain clinic patients’ medication compliance was 65%. With the improvement of medication compliance, the quality of life of patients with pain can be improved.
