ADAPTATION COMPONENT AS A CONSTITUENT OF FOREIGN STUDENTS SUCCESSFUL TRAINING IN MEDICAL UNIVERSITIES OF UKRAINE

Abstract. The article highlights the main tasks of higher medical education in Ukraine. It is noted that one of the global tasks of higher medical education development in Ukraine is the qualitative training of foreign students. The successful process of their education is a kind of indicator of high level of education in Ukraine, a confirmation of its international scientific, educational and cultural authority. It is indicated that attraction of foreign students to Ukrainian medical institutions of higher education performs not only economic functions, replenishing the country's budget at the expense of students studying on a contract form of education, but also stimulates cultural exchange between countries, affects the formation of future medical elite. Foreign students who remain in our country after graduating from Ukrainian higher education institutions have particular interest for the research. During their studies, they not only receive education, but also master the Ukrainian language, adapt to economic, cultural and other living conditions. Such graduates are very valuable for the future of our country. In connection with this, the question of adaptation of foreign students to living and studying in Ukraine is being brought up to date as the quality of education and professional socialization of foreigners depends on its success.
 The adaptation component plays an important role in studying process of foreign students. Students encounter adaptation difficulties from the first days of their studies. Foreigners find themselves in an unfamiliar cultural, linguistic, household and social environment, which they need to master, so the educational process is accompanied by adaptation to new living conditions. An important role in the process of foreign students’ adaptation in the universities of Ukraine has the organization of the educational process itself and the direct participation of the teacher in it. However, the successful implementation of the adaptation component is facilitated not only by classroom work, but also by extracurricular work, as it is an integral part of educational and educational processes.

Beri-beri and Japanese doctors in Australia

There has been a range of lengthy examinations of the history of the discovery of the cause of beri- beri and particularly its relationship with western or ‘scientific medicine’ in Japan, and the failure of that country’s medical elite to adopt clearly prophylactic measures. However, there has been negligible consideration of the history of beri-beri in Australia, despite it having been a regulated ‘contagious disease’ over many decades. There has also been very little consideration given to the work of the small number of Japanese doctors who came to Australia more than a century ago when beri-beri was a major affliction in Asia and the Indian sub-continent. The basic contention of this paper is that, even though, or perhaps because, it was largely confined to persons of Asian background, beri-beri was, and remained for a long period, a far more significant affliction in Australia than is generally acknowledged. Moreover, its local history is inevitably interwoven with what occurred in Japan, and the way it was dealt with in Australia reflects a similar intransigence among those who had or perceived a vested interest, or who simply could not escape the attitudes of their own upbringing and education.

After 1919, the medical elite and numerous intellectuals began to show interest to matters such as: the improvement of the human stock, the topic of health and the biological future of the Romanian nation. This essay aims to present and analyze the importance of the biological improvement of the nation around the year 1935. The year itself is significant due to the fact that it coincides with the foundation of the Department of Demography, Anthropology and Eugenics of the Romanian Social Institute and also in the same year the Royal Romanian Society for Eugenics and the Study of Heredity.

Caesar Alphonse RU - Professor, founder of reconstructive surgery of the gastrointestinal tract. To the 165th of birthday

Cesar Alfonce Roux was born on March 23, 1857 in the Swiss Republic, in the town of Mont-La-Ville, canton of Vaud. He did not belong to the medical elite of that time, as he grew up in a poor family, having been accustomed to work since childhood and endowed with a sense of responsibility. His father worked as a school inspector, and Caesar himself was the oldest among his eleven brothers and sisters. After graduating from a local school and college, on the instructions of a family friend a rural doctor - he decides to enter the medical faculty. Due to the fact that Caesar Roux's family was from among the French Huguenots, which excluded the possibility of studying at the universities of Lausanne or Geneva, he chose the faculty in Bern. In it, Caesar, in addition to studying, also combined work as a laboratory assistant at the departments of anatomy, physiology and, finally, pathological anatomy. Being an extraordinary person and a great workaholic, by the end of his studies he was noticed by one of the "masters" of surgery at that time Theodor Kocher, who offered him an internship at the clinic "Hpital de l'Isle". At this time, Caesar also managed to visit Vienna and get acquainted with the works of advanced surgeons of his time. In 1884, he began working as a doctor in the city of Lausanne. Due to his professional qualities, great humanism and mercy, Caesar is gaining popularity relatively quickly and already in 1887 he headed the surgical department. After 3 years, the Faculty of Medicine opens in Lausanne, where Caesar Roux receives a professorship. Over time, he brought his department to an unattainable level and Lausanne became one of the centers of European surgery.It is necessary to note the huge range of surgical areas in which there are author's innovations of Caesar Ru, but he considered abdominal surgery and surgical gastroenterology to be his most favorite sections of medicine. It was the achievements in the latter field that immortalized his name. The technique of turning off the loop of the small intestine has firmly taken its place in surgery and is widely used in our time. The famous Y-shaped anastomosis performed during gastric resection according to Billrot-II in the Ru modification is still showing good long-term results. Among the achievements of the great surgeon are operations for prethoracic plastic surgery of the esophagus with a small intestine. In 1883, Caesar Roux first applied the method of surgical treatment of acute appendicitis appendectomy, and already in 1888 the operation became officially recognized. He proposed a huge number of author's surgical interventions not only in abdominal, but also in thoracic surgery, urology, and even in operative endocrinology, because Caesar Ru continued the work of Emil Theodor Kocher, developing surgical treatment of goiter. Under the leadership of Caesar Ru, 126 scientific studies were defended (almost all of them doctoral dissertations) and a large number of students were trained, who later became famous surgeons. Among them is Peter Alexandrovich Herzen, the founder of oncology in Russia, one of the world's first female surgeons in the world, Vera Ignatievna Giedroyts, who defended her doctoral dissertation, proving the high efficiency of the method of inguinal hernia repair using the Ru method.Caesar Roux worked as a professor in Lausanne until 1926, when he had to retire due to deteriorating health, but even after that he still continued to operate, work in the clinic. On December 21, 1934, during a patient's consultation, the surgeon's heart, weakened by long-standing angina, could not stand it.Caesar Ru has left us a magnificent legacy among the author's operations, talented students, and in the memory of his contemporaries he is imprinted as a great professional in his field, a man with a big soul, mercy and simplicity.

A Qualitative Study Exploring Perceptions to the Human T Cell Leukaemia Virus Type 1 in Central Australia: Barriers to Preventing Transmission in a Remote Aboriginal Population.

BackgroundCentral Australia has the highest recorded prevalence of infection with the human T cell leukaemia virus type 1 (HTLV-1) worldwide. Each of the clinical diseases associated with HTLV-1 have been reported in this region, including deaths due to adult T cell leukaemia, which is causally linked to HTLV-1. Nevertheless, no public health response has been implemented to reduce HTLV-1 transmission among the affected Aboriginal population. In the first study to explore the perceptions of healthcare professionals along with those of Aboriginal people whose communities are actually impacted by HTLV-1, we sought to understand the barriers to preventing HTLV-1 transmission in this remote area.Methodology/Principal FindingsSemi and un-structured interviews were conducted with 30 Australian Aboriginal people, 26 non-Aboriginal healthcare professionals and 3 non-Aboriginal community workers. The purpose of the interviews was to explore perceptions towards HTLV-1 in a health context with a focus on sexual and reproductive rights. Deductive and inductive analyses were applied to the data and a decolonizing lens brought peripheral stories to the fore. A major finding was the contrast between views expressed by Aboriginal participants and healthcare professionals regarding the provision of knowledge to those affected. Aboriginal participants consistently articulated that they and their communities should be informed of, and can hold, knowledges pertaining to HTLV-1. This finding controverted the perceptions of healthcare professionals that the complexities of the virus would not be well-understood by their Aboriginal patients and that sharing HTLV-1 knowledges might overwhelm Aboriginal people. Further analyses revealed a spectrum of understanding and clinical practice, while also delineating signs of an imagined public health response.Conclusions/SignificanceHTLV-1 remains a neglected infection in Australia. Knowledge of HTLV-1 is held by a privileged medical elite and does not flow to marginalised Aboriginal people living in affected communities. We demonstrate that differences in the perspectives of stakeholders presents a significant barrier to the development of cohesive, culturally safe prevention programs that foster a shared knowledge of HTLV-1. The interview data suggests that a successful public health program is likely to require a dual approach that includes clinical care and community-driven health promotion. Aspects of this approach, which would raise awareness and potentially reduce transmission and lower HTLV-1 prevalence in Central Australia, may be applicable to other endemic settings with similar conditions of social disadvantage, geographic remoteness, resource limitations and cross-cultural challenges.

‘A thankless enterprise’: Lady Mary Wortley Montagu's campaign to establish medical unorthodoxy amongst her female network

Scholarship on Lady Mary Wortley Montagu and her involvement in the introduction of smallpox inoculation into 1720s English society generally concurs that, were it not for Montagu, inoculation might never, or would have taken significantly longer to, come about. This article argues that whilst Montagu can take the credit for popularizing the notion of inoculating against smallpox, it was not the method she personally introduced that ended up becoming general practice. Her ‘Turkish’ technique was shunned, not just by the male medical elite, but by those in her female network. For whilst they were evidently convinced by the idea of inoculation, they were not prepared to apply her foreign method, which strayed so considerably outside the bounds of what made sense to them intellectually and morally. Sanctioning a version of inoculation that adhered to the tenets of humoral medicine, they effectively advanced an ‘English’ version of it. These findings not only nuance the true nature of Montagu's contribution to the introduction of smallpox inoculation, but also reveal the extent to which its success depended upon the knowledge, experience and say-so of the female circle who first adopted it.

A Noise Based Medical Elites Silence Model and Public Health Opinion Distortion in Social Networks.

Under the impact of internet populism, internet violence, and other noises on the internet, medical elites, who have a professional background, did not intend to share their opinions on the internet. Thus, misinformation about health is increasingly prevalent. We roughly divided the users in social networks into ordinary users, medical elites, and super-influencers. In this paper, we propose a communication model of health information based on the improved Hegselmann-Krause (H-K) model. By conducting MATLAB-based simulation, the experimental results showed that network noise was an important factor that interfered with opinion propagation regarding health. The louder the noise is, the harder it is for health opinions within a group to reach a consensus. But even in a noisy environment, super-influencers could influence the overall cognition on public health in the social network fundamentally. When the super-influencers held positive opinions in public health, the medical elite keeping silent had a noise-tolerant effect on opinion communication in public health, and vice versa. Thus, three factors concerning noise control, the free information release of medical elites, and the positive position of super-influence are very important to form a virtuous information environment for public health.

Pragmatic Utterance Effect of the Medical Discourse Subject in Public Education Challenge

The paper actualizes the problem of discourse pragmatics. The authors provide the detailed description of the communicative techniques for enhancing the pragmatic utterance effect of the medical discourse subject in the open communication situation. The study is based on the texts of conference reports and interviews of representatives of the medical and scientific elite who have addressed the topic of a new coronavirus infection pandemic in 2020-2021 and those are solving the task of the public education in the area. Based on this analysis the authors provide a system of speech tactics that ensure the solution of this problem, and linguistic techniques for their implementation, which together form the pragmatic effect of the statements of the subject of medical discourse.

Interpreting the Remnants of Women’s Reproductive Crises: Physicians and the Shifting Legal Terrain of Infanticide in Nineteenth-Century Cuba

In the mid-nineteenth century, Cuban authorities produced an unprecedented volume of documentation concerning the conspicuous appearance of fetal remains, infant cadavers, and abandoned infants in public places across the island. Although the physical presence of these bodies and tissue was not new to the mid-nineteenth century, the evolution of the law and a rapidly shifting urban landscape made the material evidence of women’s responses to unwanted pregnancy more legible and more meaningful to Cuba’s colonial state. Yet, for police and judges, the domains of pregnancy and childbirth remained far beyond their training and expertise. They relied on an emerging professional class of physicians to produce and interpret evidence confirming criminality. However, the ongoing professionalisation of Cuban medicine, and in particular the emergence of a medical elite of foreign-educated obstetricians, created frictions within the medical community, and between physicians and judges. Each group of men articulated different – often competing – interpretations of the evidence, which served their broader professional goals and vision of Cuba’s future. Emerging out of these confrontations over the meaning of the material remains of reproductive crises were key contradictions in the prevailing understandings of the law, ones that hinged on race, class, and gender in perceptions of criminality.

Single-Cell RNA-Seq Reveals LGALS1 and LAG3 as Novel Drivers of Ibrutinib Resistance in Chronic Lymphocytic Leukemia

Background: Chronic lymphocytic leukemia (CLL) is a highly heterogeneous malignant lymphoproliferative B-cell disorder that can be treated using ibrutinib, a Bruton’s tyrosine kinase inhibitor. However, the ibrutinib resistance of CLL patients has caused widespread concerns, necessitating the development of novel treatment strategies. Methods: Here, we identified lectin galactoside-binding soluble 1 (LGALS1) and lymphocyte-activating gene 3 (LAG3) as potential markers for ibrutinib-resistant CLL using single-cell RNA sequencing (scRNA-seq), and the results were validated in an ibrutinib-resistant CLL cell line (MEC1-IR) and primary cells from CLL patients. Marker-gene expression was detected while functional analyses were conducted with or without OTX008, a selective Galectin-1 inhibitor. ScRNA-seq revealed that the biological features, gene expression profiles, and clonal signatures of peripheral blood mononuclear cells (PBMCs) from patients with ibrutinib-resistant CLL were distinct from those displayed by PBMCs from ibrutinib-sensitive patients. Findings: A close correlation between LGALS1 and LAG3 expression was observed and these factors were found to be highly expressed in ibrutinib-resistant CLL, with diagnostic and prognostic stratification, indicating that they may serve as drivers of ibrutinib-resistant CLL. Concordantly, LGALS1 and LAG3 expression was higher in ibrutinib-resistant CLL cells and primary cells, and OTX008 suppressed proliferation and induced apoptosis in both cells. Interpretation: LGALS1 and LAG3 gene panel is promising indicator of ibrutinib-sensitivity and prognosis marker of CLL. LGALS1 inhibitor OTX008 is effective in CLL patients, both those naive to and those resistant to ibrutinib. This work highlights the pathogenic mechanism and represents a novel therapeutic target for CLL. The work aimed to introduce new scientific data and use well-known data about the professor, doctor of medicine, a well-known therapist, director of the Kazan Clinical Institute (Kazan Institute for Advanced Medical Studies) in 19201930 Roman Albertovich Luria. The multifaceted activity of R.A. Luria as a health care organizer, teacher, scientist, practitioner, author of unique monographs on internal medicine is shown. In 19201941, R.A. Luria was a member of the Soviet scientific and medical elite of healthcare organizers and therapists. In the history of domestic medicine R.A. Luria is the organizer of the Soviet system of postgraduate education of doctors, a prominent scientist who made a significant contribution to the development of the national clinic of internal diseases in the first half of the twentieth century, and the pride of Kazan medicine, who remained in the memory of Kazan citizens as a doctor capable of creating a miracle.

A Ghost Textbook on the History of Medicine: A Case Study of the Legacy of a Stalinist Scholarly Canon

Utilizing the minutes of preparations of a manuscript textbook on the history of medicine (1948-1953), the authors reconstruct how it was decided to depict the history of world and Russian medicine; in so doing sacralizing the Soviet state and wildly overstating its care for the health of Soviet people. The archival documents allowed the authors of the article to show how the aspirations and interests of the medical elite in the sacralization of their own role encouraged historians of medicine to develop not a scientific, but an epic version of the past and to repress other versions through political accusations and condemnation of colleagues. The textbook, which had been created and discussed for a long time in the 1940s, was never published. Nevertheless, the authors' reconstruction of its aborted conception made it possible to reveal its enduring formulations in later Soviet and even present-day textbooks, and enduring capacity to shape a Soviet style historical imagination in doctors.

An independent Department of Science and Higher education at the KC PZPR was created in 1949 to take responsibility for all matters related to the development of science and higher education in the post-war Stalinist Poland. The department was responsible not only for the development of new solutions in the domain of science and higher education, but it also created reports on the entire process of education itself, the level of the ideological education of the students, the number of graduates etc. College faculty was another major area of interest. The surviving documentation consists of i.a. copies of various international trip request forms, reports from scientifi c conferences, memos describing the scientifi c community of a given college, as well as various types of complaints and denunciations. The article focuses mainly on the professors from the various medical colleges and aims at presenting a snapshot of the then-contemporary medical elite in the scientifi c domain as seen by Party offi cials. To what extent was the presented image distorted and what was the reason for its creation? – those are only some of the questions that the article tries to answer.

“One aspect of the coaching business.” Function and role of sports physiotherapists from the perspective of coaches in German elite athletics

ABSTRACT According to social differentiation theory, professional activity of elite sports physiotherapists is characterised by potential conflict between the social logics of the medical sector and elite sports. Sports physiotherapists in Germany are part of the medical sector. By their work, they prevent illness and promote health. However, within the context of elite sports they run the risk of being instrumentalised for achieving victories in athletic comparisons. Using a reconstructive hermeneutic approach, the present study explored the function and role expectations of sports physiotherapists from the perspective of professional coaches engaged in elite athletics. Four semi-structured interviews were conducted. Two main themes were identified: 1) Attributed tasks and expected physiotherapeutic effects, 2) expectations on the physiotherapeutic time arrangement. Findings suggested that the narratives of the coaches were mostly bound to the agonal logic of elite sports and mainly designated sports physiotherapists as performance enhancers over health.

The Impacts of Zanubrutinib on Immune Cells in Patients with Chronic Lymphocytic Leukemia/Small Lymphocytic Lymphoma

Ibrutinib, a first-generation Bruton's tyrosine kinase (BTK) inhibitor, could improve immunity of relapsed or refractory (R/R) chronic lymphocytic leukemia/small lymphocytic lymphoma (CLL/SLL) patients. Whether zanubrutinib, a second-generation BTK inhibitor, has similar effects as ibrutinib remains to be determined. Dynamics of number and immunophenotype of immune cells during zanubrutinib treatment in 25 R/R CLL/SLL patients were examined by flow cytometry and blood routine tests. The expression intensity of programmed cell death protein 1 (PD-1) on total CD4+, total CD8+ and T helper cells, and cytotoxic T lymphocyte-associated antigen-4 on total CD4+ (P=0.010) and regulatory T cells (P<0.05) reduced after treatment (P<0.05). There were significant differences in expression intensity of CD19 (P<0.01), C-X-C chemokine receptor type 5 (CXCR5) (P<0.01) and CD49d (P<0.05) on B cells before and after treatment. Down-regulation of PD-1 on T cells and CXCR5 and CD19 on B cells were observed in nearly all patients after zanubrutinib treatment. The number of B cells and programmed death-ligand 1 expression down-regulated, especially in the young, CLL, normal spleen, normal I²2-macroglobulin (I²2-MG), abnormal lactate dehydrogenase (LDH) and normal tumor protein 53 subgroups, while CD49d expression tended to decrease in the male, old, CLL, splenomegaly, abnormal I²2-MG and abnormal LDH subgroups after zanubrutinib treatment. These findings suggest that zanubrutinib can regulate immunity primarily by improving T cell exhaustion, inhibiting suppressor cells and disrupting CLL cells migration through down-regulation of adhesion/homing receptors. Furthermore, favorable changes in cell number and immunophenotype were preferably observed in patients without adverse prognostic factors. Sobre él, las realidades locales y regionales evidencian características distinguibles, aunque se destaca como rasgo común que la consolidación de la élite médica como corporación se dio a partir de la lucha con otros actores sociales que también se ocupaban de la salud de la población. El presente artículo analiza los argumentos esgrimidos por el sanador espiritista Juan Quinteros, frente a la persecución que sufrió a manos del Consejo de Higiene, con el fin de identificar qué características del espiritismo fueron las que Quinteros hizo visibles en la defensa de su subjetividad y de su práctica sanadora.

Gemcitabine-oxaliplatin plus rituximab (R-GemOx) as first-line treatment in elderly patients with diffuse large B-cell lymphoma: a single-arm, open-label, phase 2 trial

The combination of rituximab, gemcitabine, and oxaliplatin (R-GemOx) has shown high efficacy with a low toxicity profile in elderly patients with relapsed and refractory diffuse large B-cell lymphoma. We aimed to evaluate the efficacy, safety, and feasibility of the R-GemOx regimen as a first-line treatment in elderly patients with diffuse large B-cell lymphoma. In this single-arm, open-label, phase 2 clinical trial, we enrolled patients with previously untreated, histologically confirmed, CD20-positive diffuse large B-cell lymphoma, aged 70 years or older, or aged 60-69 years with an Eastern Cooperative Oncology Group (ECOG) performance status score of 2 or greater. Patients were recruited from Jiangsu Province Hospital (Jiangsu Sheng, China). The R-GemOx regimen was administered intravenously: rituximab 375 mg/m2 on day 0; gemcitabine 1 g/m2 on day 1; and oxaliplatin 100 mg/m2 on day 1. The cycle was repeated every 14 days. Six cycles were planned if the patient achieved at least partial remission after the interim assessment. The primary endpoint was the proportion of patients who achieved an overall response at the end of treatment (defined as complete response plus partial response). Analyses were done by intention to treat. The trial is ongoing but no longer recruiting patients. This study is registered with ClinicalTrials.gov, number NCT01670370. Between Aug 22, 2012, and Dec 31, 2015, 60 patients were enrolled and included in the study. The median age of the patients was 75 years (IQR 70-80) and 27 (45%) patients had a poor performance status with an ECOG score of 2 or greater. 45 (75%) patients achieved an overall response at the end of the treatment, with 28 (47%) achieving a complete response. Common grade 3-4 adverse events were haematological toxicities (thrombocytopenia in five [8%] patients, anaemia in four [7%], and neutropenia in nine [15%]) and gastrointestinal complications (nausea in five [8%] patients, vomiting in three [5%], and diarrhoea in one [2%]). No treatment-related deaths were reported. The R-GemOx regimen shows high efficacy and safety as a front-line treatment in an elderly patient subpopulation and might be a therapeutic option for management of diffuse large B-cell lymphoma in elderly patients.
