Sinonasal Organized Hematoma: CT and MR Imaging Findings

Sinonasal organized hematoma (OH) is an uncommon, nonneoplastic benign condition that can be locally aggressive. The purpose of this work was to characterize the CT and MR imaging findings of sinonasal OH. CT (n = 11) and MR (n = 10) images of 12 patients (9 men and 3 women; mean age, 41 years; range, 12-76 years) with pathologically proved sinonasal OH were retrospectively reviewed. Particular attention was put on the location, shape, size, extent, internal architecture, and enhancement pattern of the lesion and associated sinus wall change. The lesions were seen as an expansile (n = 9) or nonexpansile (n = 3) mass, ranging in size from 2.2 to 6.0 cm (mean, 4.2 cm), primarily involving the maxillary sinus (n = 11) or nasal cavity (n = 1) unilaterally. The ipsilateral nasal cavity was also involved in 9 of 11 maxillary sinus lesions. Smooth sinus wall erosion other than the medial maxillary sinus wall was noted in 8 lesions. The internal architecture was best displayed on T2-weighted MR images on which all of the lesions were seen as a mixture of marked heterogeneous hypointensity and isointensity, surrounded by a hypointense peripheral rim, reflecting histologic heterogeneity of the lesion composed of hemorrhage, fibrosis, and neovascularization. Marked irregular nodular, papillary, or frondlike enhancement at the areas of neovascularization was also a typical finding seen in all of the lesions. An expansile soft tissue mass, smooth sinus wall erosion, marked heterogeneous signal intensity with a hypointense peripheral rim on T2-weighted MR images, and marked irregular nodular, papillary, or frondlike enhancement are characteristic CT and MR imaging findings of sinonasal OH.

Adenoid cystic carcinoma of nasal cavity - a case report

A 38 year old male patient presented with pain and swelling on left side of nose with mass in left nostril. The other symptoms were nasal obstruction, epistaxisc from left nostril and epiphora from left eye. CT scan showed a mass arising from lateral wall of nose on left side eroding anterior part of medial wall of maxillary sinus. The tumor was excised with wide margin through a lateral rhinotomy approach under general anaesthesia. In histopathologieal examination it was found to be cribriform type of Adenoid Cystic Carcinoma. This case is reported here for its rarit.

Maxillary sinus mucoceles: clinical presentation and long-term results of endoscopic surgical treatment.

To describe the clinical presentation of maxillary sinus mucoceles, understand their pathogenesis, and determine the long-term efficacy of the endoscopic surgical treatment. Retrospective review. Thirteen consecutive patients who presented with maxillary sinus muco(pyo) celes were studied. Subjects with history of preceding sinus/nasal surgery or facial trauma were excluded. The presenting signs and symptoms, radiological findings, and surgical management were reviewed. There were six women and seven men with an age range of 31 to 71 years. Two patients had environmental allergies. Nine patients complained of cheek pressure or pain, six of nasal obstruction, and eight of nasal drainage. On endoscopic nasal examination, the medial wall of maxillary sinus was bulging with prolapsed middle meatal mucosa in 10; drainage was seen in 7, but none had polyps. The sinus involvement was limited to the maxillary sinus and the ipsilateral ethmoid on computed tomographic studies in 10 cases. Patients were treated with endoscopic ethmoidectomy, middle meatal antrostomy, and marsupialization of the mucocele. Intraoperative cultures grew organisms in five patients. Postoperative follow-up ranged between 10 and 66 months. Two patients required lysis of adhesions in the middle meatus, and one, revision antrostomy. All patients had a patent middle meatal antrostomy and healthy maxillary sinus mucosa at latest follow-up. The presenting symptoms resolved or improved in 12 cases. The etiology of maxillary sinus mucoceles is not well understood. Mechanical obstruction or allergy or both do not seem to play an important role. An infectious origin is also not supported by the above data. Endoscopic sinus surgery is a reliable therapeutic measure with a favorable long-term outcome.

The Atelectatic Maxillary Sinus

The atelectatic maxillary sinus is a distinct anatomic variant. It may be identified in a symptomatic patient complaining of recurrent acute or chronic sinusitis or may be found incidentally, presenting as an asymptomatic opacified sinus. Recognition of the disorder is important. Lateral displacement of the posterior fontanelle may lead to the erroneous radiographic diagnosis of an air fluid level. At surgery, the infundibular atelectasis associated with the disorder may give rise to inadvertent orbital entry as the uncinate process is incised. Surgery may be further complicated by lateral displacement of the medial maxillary sinus wall, reducing access to the sinus by the transnasal approach. Although the atelectatic sinus may be identified at any age, its occurrence in childhood and the typical concurrence of sinus hypoplasia suggests sinus hypoventilation from an early age. The characteristic findings of the atelectatic maxillary sinus are presented and its surgery discussed.
